Objective: To determine the feasibility, safety and effectiveness of a structured clinical pathway for stratification and management of patients presenting with chest pain and classified as having intermediate risk of adverse cardiac outcomes in the subsequent six months. Design: Prospective clinical audit. Participants and setting: 630 consecutive patients who presented to the emergency department of a metropolitan tertiary care hospital between January 2000 and June 2001 with chest pain and intermediate-risk features. Intervention: Use of the Accelerated Chest Pain Assessment Protocol (ACPAP), as advocated by the Management of unstable angina guidelines - 2000 from the National Heart Foundation and the Cardiac Society of Australia and New Zealand. Main outcome measure: Adverse cardiac events during six-month follow-up. Results: 409 patients (65%) were reclassified as low risk and discharged at a mean of 14 hours after assessment in the chest pain unit. None had missed myocardial infarctions, while three (1%) had cardiac events at six months (all elective revascularisation procedures, with no readmissions with acute coronary syndromes). Another 110 patients (17%) were reclassified as high risk, and 21 (19%) of these had cardiac events (mainly revascularisations) by six months. Patients who were unable to exercise or had non-diagnostic exercise stress test results (equivocal risk) had an intermediate cardiac event rate (8%). Conclusions: This study validates use of ACPAP. The protocol eliminated missed myocardial infarction; allowed early, safe discharge of low-risk patients; and led to early identification and management of high-risk patients.

Caveolins have been identified as key components of caveolae, specialized cholesterol-enriched raft domains visible as small flask-shaped invaginations of the plasma membrane. In polarized MDCK cells caveolin-1 and -2 are found together on basolateral caveolae whereas the apical membrane, where only caveolin-1 is present, lacks caveolae. Expression of a caveolin mutant prevented the formation of the large caveolin-1/-2 hetero-oligomeric complexes, and led to intracellular retention of caveolin-2 and disappearance of caveolae from the basolateral membrane. Correspondingly, in MDCK cells over-expressing caveolin-2 the basolateral membrane exhibited an increased number of caveolae. Objective: To develop a 'quality use of medicines' coding system for the assessment of pharmacists' medication reviews and to apply it to an appropriate cohort. Method: A 'quality use of medicines' coding system was developed based on findings in the literature. These codes were then applied to 216 (111 intervention, 105 control) veterans' medication profiles by an independent clinical pharmacist who was supported by a clinical pharmacologist with the aim to assess the appropriateness of pharmacy interventions. The profiles were provided for veterans participating in a randomised, controlled trial in private hospitals evaluating the effect of medication review and discharge counselling. The reliability of the coding was tested by two independent clinical pharmacists in a random sample of 23 veterans from the study population. Main outcome measure: Interrater reliability was assessed by applying Cohen's kappa score on aggregated codes. Results: The coding system based on the literature consisted of 19 codes. The results from the three clinical pharmacists suggested that the original coding system had two major problems: (a) a lack of discrimination for certain recommendations e. g. adverse drug reactions, toxicity and mortality may be seen as variations in degree of a single effect and (b) certain codes e. g. essential therapy were in low prevalence. The interrater reliability for an aggregation of all codes into positive, negative and clinically non-significant codes ranged from 0.49-0.58 (good to fair). The interrater reliability increased to 0.72-0.79 (excellent) when all negative codes were excluded. Analysis of the sample of 216 profiles showed that the most prevalent recommendations from the clinical pharmacists were a positive impact in reducing adverse responses (31.9%), an improvement in good clinical pharmacy practice (25.5%) and a positive impact in reducing drug toxicity (11.1%). Most medications were assigned the clinically non-significant code (96.6%). In fact, the interventions led to a statistically significant difference in pharmacist recommendations in the categories; adverse response, toxicity and good clinical pharmacy practice measured by the quality use of medicine coding system. Conclusion: It was possible to use the quality use of medicine coding system to rate the quality and potential health impact of pharmacists' medication reviews, and the system did pick up differences between intervention and control patients. The interrater reliability for the summarised coding system was fair, but a larger sample of medication regimens is needed to assess the non-summarised quality use of medicines coding system.

The epidemic that is osteoporosis has led to an increasing interest in bone mineral, and the factors that influence the levels of bone mineral, in recent years. While it is unrealistic to try and turn back the clock, a return to an increased level of physical activity may be an important consideration in terms of skeletal health. Peak bone mass is largely determined by heredity, but lifestyle and dietary patterns also influence the level of bone mineral accrued during the growing years. In this review, we summarize the evidence that vigorous weight-bearing physical activity and adequate calcium intake represent the best possibility for enhancing the attainment of an optimal level of bone mineral, within genetic limits.

The authors investigated how the intention to passively perform a behavior and the intention to persist with a behavior impact upon the spatial and temporal properties of bimanual coordination. Participants (N = 30) were asked to perform a bimanual coordination task that demanded the continuous rhythmic extension-flexion of the wrists. The frequency of movement was scaled by an auditory metronome beat from 1.5 Hz, increasing to 3.25 Hz in .25-Hz increments. The task was further defined by the requirement that the movements be performed initially in a prescribed pattern of coordination (in-phase or antiphase) while the participants assumed one of two different intentional states: stay with the prescribed pattern should it become unstable or do not intervene should the pattern begin to change. Transitions away from the initially prescribed pattern were observed only in trials conducted in the antiphase mode of coordination. The time at which the antiphase pattern of coordination became unstable was not found to be influenced by the intentional state. In addition, the do-not-intervene set led to a switch to an in-phase pattern of coordination whereas the stay set led to phase wandering. Those findings are discussed within the framework of a dynamic account of bimanual coordination.

The health benefits provided by probiotic bacteria have led to their increasing use in fermented and other dairy products. However, their viability in these products is low. Encapsulation has been investigated to protect the bacteria in the product's environment and improve their survival. There are two common encapsulation techniques, namely extrusion and emulsion, to encapsulate the probiotics for their use in the fermented and other dairy products. This review evaluates the merits and limitations of these two techniques, and also discusses the supporting materials and special treatments used in encapsulation processes. Aluminium (At) tolerance in plants may be conferred by reduced binding of Al in the cell wall through low root cation exchange capacity (CEC) or by organic acid exudation. Root CEC is related to the degree of esterification (DE) of pectin in the cell wall, and pectin hydrolysis plays a role in cell expansion. Therefore, it was hypothesised that Al-tolerant plants with a low root CEC maintain pectin hydrolysis in the presence of Al, allowing cell expansion to continue. Irrespective of the DE, binding of Al to pectin reduced the enzymatic hydrolysis of Al-pectin gels by polygalacturonase (E.C. 3.2.1.15). Pectin gels with calcium (Ca) were slightly hydrolysed by polygalacturonase. It was concluded, therefore, that Al tolerance conferred by low root CEC is not mediated by the ability to maintain pectin hydrolysis. Citrate and malate, but not acetate, effectively dissolved Al-pectate gel and led to hydrolysis of the dissolved pectin by polygalacturonase. The organic acids did not dissolve Ca-pectate, nor did they increase pectin hydrolysis by polygalacturonase. It was concluded that exudation of some organic acids can remove Al bound to pectin and this could alleviate toxicity, constituting a tolerance mechanism. Arguably the most complex conical functions are seated in human cognition, the how and why of which have been debated for centuries by theologians, philosophers and scientists alike. In his best-selling book, An Astonishing Hypothesis: A Scientific Search for the Soul, Francis Crick refined the view that these qualities are determined solely by cortical cells and circuitry. Put simply, cognition is nothing more, or less, than a biological function. Accepting this to be the case, it should be possible to identify the mechanisms that subserve cognitive processing. Since the pioneering studies of Lorent de No and Hebb, and the more recent studies of Fuster, Miller and Goldman-Rakic, to mention but a few, much attention has been focused on the role of persistent neural activity in cognitive processes. Application of modern technologies and modelling techniques has led to new hypotheses about the mechanisms of persistent activity. Here I focus on how regional variations in the pyramidal cell phenotype may determine the complexity of cortical circuitry and, in turn, influence neural activity. Data obtained from thousands of individually injected pyramidal cells in sensory, motor, association and executive cortex reveal marked differences in the numbers of putative excitatory inputs received by these cells. Pyramidal cells in prefrontal cortex have, on average, up to 23 times more dendritic spines than those in the primary visual area. I propose that without these specializations in the structure of pyramidal cells, and the circuits they form, human cognitive processing would not have evolved to its present state. I also present data from both New World and Old World monkeys that show varying degrees of complexity in the pyramidal cell phenotype in their prefrontal cortices, suggesting that cortical circuitry and, thus, cognitive styles are evolving independently in different species.

Como reflexo de uma tend??ncia que tem ganhado import??ncia crescente nas organiza????es p??blicas, privadas e n??o governamentais do Brasil e do mundo, a tem??tica gest??o do conhecimento motivou a realiza????o desta pesquisa, cujo foco imediato ?? fornecer subs??dios para auxiliar organiza????es p??blicas como escolas de governo em seus processos de desenvolvimento de conhecimento organizacional. Este conhecimento ?? criado mediante a intera????o dos conhecimentos t??cito e expl??cito. A intera????o entre eles pode ser verificada por meio de quatro convers??es, as quais formam um ciclo do conhecimento organizacional: a socializa????o, a externaliza????o, a combina????o e a internaliza????o. O estudo em quest??o visa apresentar um conjunto de crit??rios representantes do ciclo de desenvolvimento do conhecimento organizacional e, mediante an??lise comparada entre sete escolas de governo internacionais e uma nacional, a Funda????o Escola Nacional de Administra????o P??blica (Enap), elaborar o desenho de uma estrutura organizacional que maximize o desenvolvimento do conhecimento organizacional em escolas de governo.

A fines de los ???80, el Estado venezolano inici?? una descentralizaci??n pol??tico-territorial produciendo cambios en las gobernaciones, instancias receptoras de competencias en este proceso, las cuales realizan reformas a trav??s de la estrategia selectiva, incorporando algunas de la propuestas de la nueva gesti??n p??blica. Este trabajo explora estas reformas. Se observan: diferencias entre los servicios sociales y los relacionados directamente con la econom??a; privatizaci??n de ??stos; traspaso a la sociedad civil de algunos servicios sociales, creaci??n de entes descentralizados; automatizaci??n de procesos; flexibilizaci??n laboral; iniciativas con lento avance en rendici??n de cuentas por resultados y capacitaci??n para una nueva cultura administrativa. En conclusi??n: la tendencia es a la conformaci??n de una gesti??n p??blica eficiente sobre la base de la reducci??n del aparato estatal, de costos sociales, as?? como de costos y conflictos laborales y escasa incorporaci??n de las propuestas de participaci??n que permiten profundizar la democracia y promover el bienestar social.

Se nos estados federais os mecanismos de coopera????o e coordena????o intergovernamental j?? s??o relevantes, no caso brasileiro eles ganham centralidade ainda maior. A Constitui????o Federal de 1988 agregou complexidade ao desenho federativo brasileiro, reconhecendo o munic??pio como ente federado. Esse processo foi acompanhado por intensa descentraliza????o de pol??ticas p??blicas, pelo fortalecimento do poder local e por mecanismos pouco coordenados de rela????o vertical e horizontal entre os entes federativos. Ao mesmo tempo, a aus??ncia de pol??ticas de desenvolvimento regional acentuou as desigualdades locais e regionais observadas historicamente no pa??s. Diferentes experi??ncias de consorciamento foram levadas a cabo por munic??pios no pa??s e ainda hoje esse ?? um instrumento de larga utiliza????o. De forma diversa, as iniciativas de coopera????o entre estados s??o ainda incipientes. As limita????es institucionais e jur??dicas dos desenhos utilizados pelos munic??pios, no entanto, levaram ?? aprova????o da Emenda Constitucional n?? 19/98, que instituiu os cons??rcios p??blicos e a gest??o associada de servi??os p??blicos.

O artigo analisa o processo de reforma administrativa empreendida na Austr??lia ao longo das d??cadas de 70-80, destacando duas de suas principais tend??ncias: o fortalecimento da ???Fun????o Executiva Superior???e a valoriza????o da cidadania. Nele s??o descritos os princ??pios b??sicos norteadores da reforma ao longo dos anos 70, bem como, os objetivos visados com a segunda fase das reformas executadas a partir da d??cada de 80, implementadas por governos trabalhistas. Destaca-se a organiza????o e o funcionamento do sistema das Ag??ncias de Ombudsmen e dos Comiss??rios de Contas respons??veis pela fiscaliza????o e gest??o or??ament??ria. A seguir, s??o descritas algumas das a????es encaminhadas pela Public Service Commission (PSC) no sentido de aperfei??oar o gerenciamento e o melhor desempenho dos recursos humanos, destacando a introdu????o do importante conceito de Novo Profissionalismo, assentado em valores como responsabilidade para com o governo, foco nos resultados, m??rito como valor b??sico, altos n??veis de integridade e probidade na conduta, desempenho com accountability e melhoramento cont??nuo do desempenho das equipes e indiv??duos. Outro importante instrumento analisado ?? o Human Resource Management, atrav??s do qual as autoridades australianas introduziram novas pr??ticas como negocia????o no local de trabalho, plano de oportunidades igualit??rias no local de trabalho e o plano de treinamento e especializa????o. Para finalizar, o artigo enfoca a pol??tica de contratualiza????o de servi??os p??blicos adotada na Austr??lia ??? os Competitive Tendering and Contracting, evidenciando os fatores indutores da ado????o desta nova pr??tica de gest??o.

O presente texto tem por escopo a avalia????o da gest??o da despesa p??blica, num contexto de mudan??as inseridas por novas t??cnicas gerenciais, provocando uma amplia????o do conceito de gest??o da despesa, que, al??m de se preocupar com sistemas e t??cnicas operacionais, teria tamb??m sua aten????o voltada para os resultados e a satisfa????o do cliente. A transforma????o gerada pelo pacote emplementador da reforma ?? avaliada, partindo de uma simula????o em que as reformas seriam consideradas completamente instaladas, com o objetivo de ressaltar suas totais implica????es num contexto futuro. Elege-se o ano de 2020, como um marco, para levantar quest??es cruciais do referido pacote, abordando elementos que poderiam favorecer melhores resultados.

Os Conselhos Gestores s??o f??runs h??bridos onde se viabiliza a participa????o popular no desenho das pol??ticas p??blicas, nas negocia????es de interesses da coletividade, na elabora????o de programas e projetos sociais e na fiscaliza????o das a????es governamentais. O significado e a dimens??o dos Conselhos para a democracia participativa s??o enormes, justificando a import??ncia da realiza????o deste estudo, que se baseou na an??lise explorat??ria de 12 Conselhos locais do munic??pio mineiro de Vi??osa, com o objetivo de avaliar o desempenho dos mesmos. A avalia????o do desempenho levou em considera????o os mesmos par??metros dimensionais, tanto para a coleta de dados prim??rios quanto para a coleta secund??ria. A coleta de dados prim??rios foi realizada a partir da aplica????o de question??rios semi-estruturados aos presidentes dos Conselhos em an??lise. Utilizou-se o programa SPSS, vers??o 15.0, para an??lise estat??stica das quest??es objetivas dos question??rios e a t??cnica de an??lise de conte??do para exame das quest??es abertas. Ap??s essa etapa, realizou-se um workshop com os presidentes dos Conselhos, visando ?? obten????o de novos dados e informa????es. Os Conselhos municipais analisados apresentaram resultados pouco discrepantes entre a teoria aplicada ao tema e a sua efetiva pr??tica no campo p??blico. Contudo, a pesquisa revelou que o grau de desenvolvimento institucional dos Conselhos n??o ?? homog??neo, ou seja, h?? alguns melhores gestores que outros. A escolha independente e democr??tica dos membros n??o-governamentais dos Conselhos em Vi??osa revelou o seu grau de independ??ncia do Poder Executivo. Por??m, a capacita????o de conselheiros precisa ser implementada para evitar uma desfigura????o dos Conselhos. No que tange ?? governan??a, destacouse em Vi??osa o seu grau e formas de aplicabilidade observadas na gest??o de v??rias pol??ticas. Da mesma forma, foi observado um n??vel de governabilidade que confere legitimidade ??s pol??ticas implementadas. Por fim, verificou-se que a efetividade da accountability societal tem sido instrumentalizada pelos Conselhos no ??mbito das diversas pol??ticas setoriais. J?? os dados secund??rios da an??lise foram obtidos por meio das s??nteses dos relat??rios de fiscaliza????o de munic??pios do Estado de Minas Gerais, realizadas pela Controladoria Geral da Uni??o. Foram identificados todos os registros pertinentes aos Conselhos Gestores Municipais do Estado de Minas Gerais, no per??odo de 2003 a 2006. Verificou-se que os maiores problemas se referem aos Conselhos Municipais de Sa??de e Assist??ncia Social. Entre as in??meras falhas apontadas, destacam-se problemas graves na constitui????o e operacionaliza????o dos Conselhos, resultando na inoper??ncia desses mecanismos nos munic??pios fiscalizados. A avalia????o do desempenho dos Conselhos locais de Vi??osa possibilitou concluir que esses mecanismos apresentaram um desempenho bem satisfat??rio na gest??o e controle das pol??ticas p??blicas no munic??pio.

O trabalho analisa a potencialidade do desenvolvimento de alian??as entre o p??blico e o privado na gest??o p??blica municipal brasileira da sa??de. A relev??ncia da quest??o pauta-se na transfer??ncia de responsabilidade da presta????o de servi??os de sa??de para os munic??pios, posterior ?? promulga????o da Constitui????o Federal, aliada ?? limita????o da capacidade de gest??o dos mesmos. As an??lises aqui tratadas referem-se ??s alian??as previstas no arcabou??o legal brasileiro, estabelecidas entre o ente p??blico e o terceiro setor. Essas alian??as s??o introduzidas pela reforma do aparelho do Estado, em 1995, no ??mbito da qual se utilizou a estrat??gia de publiciza????o que tratou do fortalecimento dessa alian??a entre o Estado e o Terceiro Setor. A partir dos modelos poss??veis de parcerias com o terceiro setor, este estudo apresenta uma an??lise do modelo das organiza????es sociais (OS), trazendo ?? luz estrat??gias e desafios para sua implementa????o.
