The objectives of this study were to evaluate the outcomes of our patients admitted with hip fractures, and to benchmark these results with other hospitals, initially in Europe and subsequently in Australia. The Standardised Audit of Hip Fractures in Europe (SAHFE) questionnaires was used as the data gathering instrument. The participants were all patients admitted to Redcliffe Hospital with a fractured neck of femur prior to surgery. This paper reports the results of the first 70 consecutive patients admitted to Redcliffe Hospital with a fractured neck of femur from November 1st 2000. The main outcome measures were mobility, independence, residence prior to fracture; type of fracture and surgical repair; and time to surgery, survival rates and discharge destination. Results: 43 patients were admitted from home, but only 13 returned home directly from the orthopaedic ward. It is hoped that most of the 26 transferred to the rehabilitation ward will ultimately return home. 7 patients died, these were aged 82 to 102, and all had premorbid disease. Delays in surgery were apparent for 13 patients, mainly due to administrative problems. Conclusions: We support the recommendation in the Fifteenth Scottish Intercollegiate Guidelines Network Publication on the management of hip fractures, that all units treating this condition should enter an audit to evaluate their management. Purpose. Health promotion policy frameworks, recent theorizing, and research all emphasize understanding and mobilizing environmental influences to change particular health-related behaviors in specific settings. The workplace is a key environmental setting. The Checklist of Health Promotion Environments at Worksites (CHEW) was designed as a direct observation instrument to assess characteristics of worksite environments that are known to influence health-related behaviors. Methods. The CHEW is a 112-item checklist of workplace environment features hypothesized to be associated, both positively and negatively, with physical activity, healthy eating, alcohol consumption, and smoking. The three environmental domains assessed are (1) physical characteristics of the worksite, (2) features of the information environment, and (3) characteristics of the immediate neighborhood around the workplace. The conceptual rationale and development studies for the CHEW are described, and data from observational studies of 20 worksites are reported. Results. The data on CHEW-derived environmental attributes showed generally good reliability and identified meaningful sets of variables that plausibly may influence health-related behaviors. With the exception of one information environment attribute, intraclass correlation coefficients ranged from 0.80 to 1.00. Descriptive statistics on selected physical and information environment characteristics indicated that vending machines, showers, bulletin boards, and signs prohibiting smoking were common across worksites. Bicycle racks, visible stairways, and signs related to alcohol consumption, nutrition, and health. promotion were relatively uncommon. Conclusions. These findings illustrate the types of data on environmental attributes that can be derived, their relevance for program planning, and how they can characterize variability across worksites. The CHEW is a promising observational measure that has the potential to assess environmental influences on health behaviors and to evaluate workplace health promotion programs.

The validity of a brief self-report screening measure for elder abuse was examined on a nationally representative sample of more than 12,000 older women it? the baseline survey of the Australian Longitudinal Study on Women's Health. The screening instrument was a modification Of the Hwalek-Sengstock Elder Abuse Screening Test. Construct validity was examined using factor analysis and correlation with a wide range of sociodemographic, psychological. and health-related variables. Four factors, each of three items, were identified representing the following domains: Vulnerability, Dependence, Dejection, and Coercion. The Vulnerability and Coercion factors had the highest face validity for abuse and demonstrated moderate to good construct validity. The six items comprising these factors may provide a simple screening tool for elder abuse The identified correlates of abuse indicators have the potential to enhance policy development, screening, intervention, and carer support programs.

Based on a cognitive-social learning model of alcohol use, it was hypothesised that women with both alcohol and relationship problems would endorse more positive expectations of the effects of alcohol consumption on their relationship and would report lower relational efficacy than women without relationship or alcohol problems. Measures of relationship-referent alcohol expectancies and relational efficacy were completed by 174 married women with both alcohol and relationship problems (n = 20), alcohol problems alone (it = 26), relationship problems alone (n = 30), or neither problem (n = 98). Women without either alcohol or relationship problems strongly rejected expectations of enhanced relationship functioning (e.g., enhanced intimacy, increased emotional expression) following alcohol consumption, whereas women with both alcohol and relationship problems were ambivalent about these positive expectations. Women with both problems also reported lower relational efficacy than the other groups of women. Negative expectations about the effect of alcohol consumption on relationships in women with low relational efficacy may inhibit harmful drinking. (C) 2002 Elsevier Science Ltd. All rights reserved.

As end-user computing becomes more pervasive, an organization's success increasingly depends on the ability of end-users, usually in managerial positions, to extract appropriate data from both internal and external sources. Many of these data sources include or are derived from the organization's accounting information systems. Managerial end-users with different personal characteristics and approaches are likely to compose queries of differing levels of accuracy when searching the data contained within these accounting information systems. This research investigates how cognitive style elements of personality influence managerial end-user performance in database querying tasks. A laboratory experiment was conducted in which participants generated queries to retrieve information from an accounting information system to satisfy typical information requirements. The experiment investigated the influence of personality on the accuracy of queries of varying degrees of complexity. Relying on the Myers–Briggs personality instrument, results show that perceiving individuals (as opposed to judging individuals) who rely on intuition (as opposed to sensing) composed queries more accurately. As expected, query complexity and academic performance also explain the success of data extraction tasks.

Background: There is ample evidence of important symptomatic efficacy of tumour necrosis factor alpha (TNFalpha) inhibition in ankylosing spondylitis (AS). Moreover, studies suggest that anti-TNF could be considered as the first disease controlling antirheumatic treatment (DC-ART) for AS. Objective: To determine precisely which patients with AS are most likely to benefit from anti-TNFalpha treatment because of the cost and possible long term side effects of such treatment. Methods: Assessment in Ankylosing Spondylitis (ASAS) members were asked to use a Delphi technique to name the characteristics of patients with AS for whom they would start DC-ART, in three different clinical presentations (isolated axial involvement, peripheral arthritis, enthesitis). Results: Among the 62 invited ASAS members, more than 50% actively participated in the four phases of definition according to the Delphi technique. For each of the three clinical presentations, a combination of five to six domains was proposed, with an evaluation instrument and a cut off point defining a minimum level of activity for each domain. Conclusion: This study provides a profile for a patient with AS for considering initiation of biological agents that reflects the opinion of the ASAS members, using a Delphi exercise. Further studies are required to assess their relevance and their consistency with clinical practice.

Under certain conditions, cross-sectional analysis of cross-twin intertrait correlations can provide important information about the direction of causation (DOC) between two variables. A community-based sample of Australian female twins aged 18 to 45 years was mailed an extensive Health and Lifestyle Questionnaire (HLQ) that covered a wide range of personality and behavioral measures. Included were self-report measures of recent psychological distress and perceived childhood environment (PBI). Factor analysis of the PBI yielded three interpretable dimensions: Coldness, Overprotection, and Autonomy. Univariate analysis revealed that parental Overprotection and Autonomy were best explained by additive genetic, shared, and nonshared environmental effects (ACE), whereas the best-fitting model for PBI Coldness and the three measures of psychological distress (Depression, Phobic Anxiety, and Somatic Distress) included only additive genetic and nonshared environmental effects (AE). A common pathway model best explained the covariation between (1) the three PBI dimensions and (2) the three measures of psychological distress. DOC modeling between latent constructs of parenting and psychological distress revealed that a model which specified recollected parental behavior as the cause of psychological distress provided a better fit than a model which specified psychological distress as the cause of recollected parental behavior. Power analyses and limitations of the findings are discussed.

A hyphenated instrumental approach has been used to obtain reliable values for the propagation rate coefficients as a function of conversion for polymerizations of methyl methacrylate (MMA) and a mixture of MMA and ethyleneglycol dimethacrylate (EGDMA) with a 1:1 concentration of double bonds, from near the onset of the Trommsdorf region into the glass region. ESR spectroscopy was used to measure the radical concentration while FT-NIR fibre-optic spectroscopy was employed to measure instantaneously the double-bond concentration within the temperature-controlled cavity of the ESR instrument during polymerization. The advantage of this approach to the measurement of the rate coefficient is that it is equally applicable to branching and linear polymerizations. For the polymerization of methyl methacrylate, the values of the rate coefficient at the lowest conversions at which reliable values could be obtained were in agreement with recently reported values obtained by the PLP-SEC method. For the lowest conversions, the values obtained were 403 1 mol(-1) s(-1) at 306 K for MMA and 5201 mol(-1) s(-1) at 310 K for a 1:1 mixture of MMA and EGDMA. (C) 2003 Society of Chemical Industry.

Este trabalho pressup??e um Estado formulador de pol??ticas p??blicas e detentor de instrumentos de planejamento, o que nos remete a um vi??s ideol??gico: de que, no capitalismo contempor??neo, Estado e mercado coordenem os sistemas econ??micos, conforme explicitado no Plano Diretor da Reforma do Aparelho de Estado (Governo Federal, Brasil, 1995). S?? a aceita????o dessa hip??tese pode supor a exist??ncia e a necessidade de pol??ticas p??blicas, remetendo-nos o campo da ideologia e contrapondo-nos ao liberalismo.

Se nos estados federais os mecanismos de coopera????o e coordena????o intergovernamental j?? s??o relevantes, no caso brasileiro eles ganham centralidade ainda maior. A Constitui????o Federal de 1988 agregou complexidade ao desenho federativo brasileiro, reconhecendo o munic??pio como ente federado. Esse processo foi acompanhado por intensa descentraliza????o de pol??ticas p??blicas, pelo fortalecimento do poder local e por mecanismos pouco coordenados de rela????o vertical e horizontal entre os entes federativos. Ao mesmo tempo, a aus??ncia de pol??ticas de desenvolvimento regional acentuou as desigualdades locais e regionais observadas historicamente no pa??s. Diferentes experi??ncias de consorciamento foram levadas a cabo por munic??pios no pa??s e ainda hoje esse ?? um instrumento de larga utiliza????o. De forma diversa, as iniciativas de coopera????o entre estados s??o ainda incipientes. As limita????es institucionais e jur??dicas dos desenhos utilizados pelos munic??pios, no entanto, levaram ?? aprova????o da Emenda Constitucional n?? 19/98, que instituiu os cons??rcios p??blicos e a gest??o associada de servi??os p??blicos.

Este artigo tem como objetivo analisar o papel do Estado na formula????o do planejamento no Brasil a partir dos anos 30, per??odo que deu origem ??s primeiras iniciativas de planejamento, at?? os anos 80, momento em que se inicia a decad??ncia do planejamento governamental no pa??s. Optou-se por uma retrospectiva hist??rica que levantasse os principais aspectos inerentes ao planejamento como instrumento do desenvolvimento econ??mico deste per??odo, verificando as principais mudan??as ocorridas na sociedade. O artigo n??o pretende dar conta de todas as dimens??es e complexidades do tema nem esgotar o assunto, que ?? pol??mico e envolve muitas articula????es te??ricas com outras ??reas, como administra????o p??blica, economia e ci??ncia pol??tica. Desse modo, o artigo aponta na dire????o de que este balan??o de an??lises das experi??ncias de planejamento no Brasil demonstrou grandes avan??os econ??mico-financeiros e alguns fracassos de coordena????o e articula????o com outras esferas, como a executiva e a financeira. Assim, desde os anos 80, em detrimento da crise do Estado, o planejamento entra em decl??nio nas agendas governamentais, impossibilitado, por quest??es de ordem financeira, de realizar suas fun????es de racionaliza????o e efici??ncia econ??mica. Dessa forma, o Estado, nesse contexto, sempre representou, de forma geral, as diversas articula????es de interesses capitaneados pelo planejamento como instrumento de interven????o e controle social no Brasil.

A necessidade de viabilizar investimentos p??blicos em um contexto de restri????o fiscal impulsionou governos de diversos pa??ses a buscar na parceria com o setor privado a alternativa para a realiza????o de obras e servi??os p??blicos. A experi??ncia paradigm??tica do Reino Unido em programas de parceria p??blico-privada (PPP) norteou a implanta????o de iniciativas semelhantes mundo afora. No Brasil, a aprova????o, em dezembro de 2004, da lei que disciplina as normas para contrata????o de PPP deu in??cio a um processo que passar?? pela defini????o dos projetos priorit??rios e que culminar?? na consolida????o das parcerias como mais um instrumento para a viabiliza????o de obras e servi??os p??blicos de que o pa??s necessita. Esse artigo tem por objetivo elucidar o modelo brasileiro de parcerias p??blico-privadas. Ap??s uma contextualiza????o inicial, s??o discutidos os fundamentos que balizam a implanta????o de um programa de PPP e apresentadas algumas caracter??sticas gerais dos contratos. Por fim, s??o expostos os pontos centrais da lei brasileira.

O mercado de trabalho sofreu profundas transforma????es desde o come??o do capitalismo at?? o momento atual, sendo poss??vel compar??-lo a uma loteria, na qual os trabalhadores passaram a competir entre si por ascens??o profissional. Os bilhetes dessa loteria s??o os investimentos em capital humano. Nela haver??, necessariamente, vencedores e perdedores, com os primeiros apropriando-se do trabalho dos ??ltimos. Dessa realidade, surge uma nova forma de explora????o do trabalho, cujo instrumento de legitima????o ?? o capital humano, que refor??a a tend??ncia inerente do capitalismo ?? gera????o de desigualdade e coloca limita????es ?? vis??o tradicional de combate ?? desigualdade baseada na democratiza????o do capital humano. O crescimento tamb??m tem limita????es na redu????o da desigualdade, tendo em vista que a motiva????o microecon??mica que gera o crescimento macroecon??mico ?? justamente a busca pela desigualdade.

Este paper defende duas id??ias. A primeira, ?? apresentar os diferentes processos da reforma do Estado mexicano, analisados sob as prioridades diversas no decorrer do tempo. As reformas econ??mica, pol??tica, social e administrativa tiveram prioridades e objetivos divergentes. A reforma econ??mica foi sistem??tica e congruente, a pol??tica foi defensiva e pouco abrangente, e a social foi problem??tica e confusa. A reforma administrativa foi, comparativamente, limitada. Uma proposta para desenvolver um sistema administrativo respons??vel pelo servi??o p??blico foi postergada nos ??ltimos 12 anos at?? que, em maio de 1997, finalmente a atual administra????o prop??s um programa de moderniza????o. A segunda id??ia ?? que, mesmo que a atual administra????o tenha a inten????o de desenvolver um novo sistema de accountability e um plano de carreira para o servi??o p??blico, parece plaus??vel que a tradicional pol??tica de controle, atrav??s da estrutura administrativa, possa ser prejudicada se essas propostas realmente reduzirem o poder discricion??rio dos altos cargos p??blicos e submeterem reservas p??blicas a um esquema de controle social mais aberto e formal. Logo, ?? poss??vel dizer que a alternativa escolhida pelos reformistas foi usar uma abordagem gerenciadora, onde a melhoria dos procedimentos e a implementa????o de algumas t??cnicas administrativas avan??adas substituem, pelo menos at?? agora, uma transforma????o mais profunda da estrutura administrativa. Uma estrat??gia de gerenciamento permite, pelo menos at?? o momento, a melhoria dos servi??os atrav??s do aumento da participa????o de servidores p??blicos, sem prejudicar a atual administra????o, como um instrumento para implementar diferentes agendas pol??ticas usando recursos p??blicos. Nosso argumento ?? que, como o programa de moderniza????o est?? sendo implantado, ?? importante estudar minuciosamente as possibilidades de se implementar a reforma do servi??o p??blico e melhorar a responsabilidade do sistema, dada a particular rela????o que existe entre a administra????o p??blica e o sistema pol??tico no M??xico.

O artigo discute a participa????o dos usu??rios na gest??o das Organiza????es Sociais, prevista num projeto da reforma do aparelho de Estado atualmente em curso. A participa????o dos usu??rios na gest??o das institui????es p??blicas ?? um instrumento que pode resolver problemas da rela????o principal-agente, garantindo a implementa????o eficiente das pol??ticas. No projeto das Organiza????es Sociais, essa participa????o ?? viabilizada, entre outros meios, pela presen??a de entidades representativas da sociedade civil no Conselho de Administra????o da institui????o, pressupondo, portanto, a organiza????o dos usu??rios. Por essa raz??o, no artigo s??o analisadas as possibilidades de organiza????o dos usu??rios com base na abordagem da ???l??gica da a????o coletiva???.
