Objetivos: Evaluar parámetros clínicos, bacteriológicos y morbimortalidad en pacientes mayores de 65 años con bacteriemia (Grupo A) y compararlos con aquellas ocurridas en menores de 65 años (grupo B) hospitalizados en un servicio de clínica médica. Material y métodos: Estudio protocolizado, descriptivo y observacional, desde 1989-2006. Criterios de inclusión: dos o más hemocultivos positivos. Análisis estadístico con Epi Info 6.4 . Resultados: Se identificaron 668 bacteriemias: 258 (38,6%) en A y 410 (61,4%) en B. La edad media fue de 74,2 años (DS±7.01) y de 47,2 años (DS±13.7) respectivamente. No hubo diferencias en la permanencia media: 19,1 días (DS±17.07) en A y 19,9 (DS±18,1) en B, ni en el origen nosocomial (40,7 vs 44%). Los focos pulmonar (31 vs 21,2%)(p<0,008) y urinario (27 vs 18,8%)(p=0.017) fue más frecuentes en A. La fiebre >de 38 ºC fue menos frecuente en A (83,8 vs 90,9%)(p=0.0068), mientras que la hipotensión arterial (40,8 vs 32,2%)(p=0.03), oliguria (41,7 vs 30,9%)(p=0.005) y encefalopatía (54,5 vs 39,4%)(p=0.00014) fueron mas comunes en A. Los bacilos gram negativos predominaron en A (46 vs 37%)(p<0.01) con diferencias entre A.baumani (9,16 vs 4,6%) y E.coli (54,1 vs 42,7%)(p<0.05) y en B fueron más frecuentes por S.aureus MS (34.39 vs 24,63)(p=0.01). El shock séptico (33,9 vs 22,4%) y nuevas insuficiencias de órganos (31,1 vs 20,1%) prevalecieron en A (p<0,001). La mortalidad fue de 34,49% para A y de 23,41% para B (p=0,018) Conclusiones: Las bacteriemias en pacientes internados mayores de 65 años comparadas con las ocurridas en pacientes. de menor edad, se caracterizaron significativamente por tener predominio del foco pulmonar y urinario, de hipotensión arterial, oliguria y encefalopatía, de bacteriemia por BGN, principalmente A. baumanii y E. coli; mayor desarrollo de shock séptico, menor frecuencia de fiebre >38°C y de bacteriemia por MSS A, y significativa mayor mortalidad.

Prevalence of vitamin B12 deficiency is very common in elderly people and can reach values as high as 40.5% of the population. It can be the result of the interaction among several factors. Vitamin B12 deficiencies have been associated with neurological, cognitive deterioration, haematological abnormalities and cardiovascular diseases that have an important influence on the health of the elderly and their quality of life. It is necessary to approach the problems arisen from the lack of data relative to them. The main objective of this thesis was to analyse the evolution of vitamin B12 status and related parameters, lipid and haematological profiles and their relationship to health risk factors, and to functional and cognitive status over one year and to determine the effect of an oral supplementation of 500 μg of cyanocobalamin for a short period of 28 days. An additional objective was to analyze the possible effects of medicine intakes on vitamin B status. Three studies were performed: a) a one year longitudinal follow-up with four measure points; b) an intervention study providing an oral liquid supplement of 500 μg of cyanocobalamin for a 28 days period; and c) analysis of the possible effect of medication intake on vitamin B status using the ATC classification of medicines. The participants for these studies were recruited from nursing homes for the elderly in the Region of Madrid. Sixty elders (mean age 84 _ 7y, 19 men and 41 women) were recruited for Study I and 64 elders (mean age 82 _ 7y, 24 men and 40 women) for Study II. For Study III, baseline data from the initially recruited participants of the first two studies were used. An informed consent was obtained from all participants or their mentors. The studies were approved by the Ethical Committee of the University of Granada. Blood samples were obtained at each examination date and were analyzed for serum cobalamin, holoTC, serum and RBC folate and total homocysteine according to laboratory standard procedures. The haematological parameters analyzed were haematocrit, haemoglobin and MCV. For the lipid profile TG, total cholesterol, LDL- and HDLcholesterol were analyzed. Anthropometric measures (BMI, skinfolds [triceps and subscapular], waist girth and waist to hip ratio), functional tests (hand grip, arm and leg strength tests, static balance) and MMSE were obtained or administered by trained personal. The vitamin B12 supplement of Study II was administered with breakfast and the medication intake was taken from the residents’ anamnesis. Data were analyzed by parametric and non-parametric statistics depending on the obtained data. Comparisons were done using the appropriate ANOVAs or non-parametric tests. Pearsons’ partial correlations with the variable “time” as control were used to define the association of the analyzed parameters. XIII The results showed that: A) Over one year, in relationship to vitamin B status, serum cobalamin decreased, serum folate and mean corpuscular volumen increased significantly and total homocysteine concentrations were stable. Regarding blood lipid profile, triglycerides increased and HDL-cholesterol decreased significantly. Regarding selected anthropometric measurements, waist circumference increased significantly. No significant changes were observed for the rest of parameters. B) Prevalence of hyperhomocysteinemia was high in the elderly studied, ranging from 60% to 90 % over the year depending on the cut-off used for the classification. LDL-cholesterol values were high, especially among women, and showed a tendency to increase over the year. Results of the balance test showed a deficiency and a tendency to decrease; this indicates that the population studied is at high risk for falls. Lower extremity muscular function was deficient and showed a tendency to decrease. A highly significant relationship was observed between the skinfold of the triceps and blood lipid profile. C) Low cobalamin concentrations correlated significantly with low MMSE scores in the elderly studied. No correlations were observed between vitamin B12 status and functional parameters. D) Regarding vitamin B12 status, holo-transcobalamin seems to be more sensitive for diagnosis; 5-10% of the elderly had a deficiency using serum cobalamin as a criterion, and 45-52% had a deficiency when using serum holotranscobalamin as a criterion. E) 500 μg of cyanocobalamin administered orally during 28 days significantly improved vitamin B12 status and significantly decreased total homocysteine concentrations in institutionalized elderly. No effect of the intervention was observed on functional and cognitive parameters. F) The relative change (%) of improvement of vitamin B12 status was higher when using serum holo-transcobalamin as a criterion than serum cobalamin. G) Antiaenemic drug intake normalized cobalamin, urologic drugs and corticosteroids serum folate, and psychoanaleptics holo-transcobalamin levels. Drugs treating pulmonary obstruction increased total homocysteine concentration significantly. H) The daily mean drug intake was 5.1. Fiftynine percent of the elderly took medication belonging to 5 or more different ATC groups. The most prevalent were psycholeptic (53%), antiacid (53%) and antithrombotic (47%) drugs.

El presente proyecto surge como resultado de un proyecto real, consistente en la elaboración de un estudio energético a un edificio perteneciente al Sector Terciario. Pretende llevarse a cabo la realización de una auditoría energética en una Residencia de Mayores con el objetivo principal de obtener un conocimiento óptimo del perfil de los consumos energéticos, identificando y valorando las posibilidades de ahorro de energía según cada sistema, proponiendo medidas de mejora y realizando su evaluación técnica y económica. Para la consecución de este objetivo principal se ha necesitado realizar una campaña de medidas que ha consistido en la obtención de mediciones y registros de parámetros eléctricos, térmicos y de confort, inventariar equipos e instalaciones existentes, conocer las condiciones normales de funcionamiento del centro, analizar y estudiar los datos recogidos durante las visitas y la información aportada por el personal del centro. Con las medidas de mejora energética propuestas se reduce el consumo del edificio un 4,39 %, dando como resultado un periodo de retorno de 4,40 años. ABSTRACT The following project is the result of a real project, involving the development of an energy study to a building belonging to the Tertiary Sector. Energetic audit is performed at a home for the elderly with the main aim of getting knowledge on its optimal energy consumption profile. For this purpose, the possibilities in the energy savings for each system are identified and evaluated. To achieve our goal, we have carried out a measurement campaign consisting of: obtaining and recording measurements of electrical, thermal and comfort parameters, inventory of existing equipment and facilities, acquisition of knowledge about the normal operating conditions of the center, and analysis and study of the data collected during visits and information provided by the staff. As a conclusion of this study it should be noted that the proposed improvement measures will minimize energy consumption of the building 4.39%, resulting in a payback period of 4,40 years.

Los beneficios de la actividad física han sido ampliamente abordados en multitud de estudios con personas mayores. Sin embargo, los modelos de actividad física seguidos no siempre han tenido en cuenta las roles de participación activa, estando por lo general más influidos por metodologías directivas y propuestas de entrenamiento de la carga física, que ofrecen mayor control de grupo y menor esfuerzo organizativo. El objetivo principal de este trabajo fue comparar dos programas de ejercicio físico para personas mayores, y a continuación poder determinar el alcance sobre el ánimo, la autoestima, y el disfrute con la actividad física. Participaron 72 mujeres entre 55-70 años (M=64.10; DT=9.40), de los municipios gallegos de Arousa-Norte. Los resultados principales indicaron que los participantes en los programas de ejercicio físico supervisado recogen beneficios psicológicos, diferentes en función del tipo de programa. No se debe hablar de un único modelo de práctica física en edad adulta, porque el rol del participante y la forma de interactuar tienen importante efecto sobre la salud, con utilidad para la vida cotidiana. Por tanto, es necesario seguir desarrollando nuevas propuestas, a la vez utilitarias y pedagógicas, combinando bajo el denominador común motriz, lo físico, lo mental, y lo social.

Introducción: A nivel mundial la población de adultos mayores se incrementa, Ecuador no se queda fuera de esta transición demográfica y así aumentan también todas las enfermedades asociadas con la edad, principalmente la hipertensión arterial. Objetivo: Determinar la frecuencia de hipertensión arterial en adultos mayores de las parroquias urbanas de la ciudad de Cuenca, 2015. Metodología: Se realizó un estudio descriptivo en una muestra de 378 adultos mayores. Fueron hipertensos aquellos pacientes que tengan dos tomas de presión arterial separadas como mínimo una semana en las cuales la presión arterial sistólica sea >o= a 140 mmHg y presión arterial diastólica >o= a 90 mmHg. Se clasificó según parámetros sociodemográficos, antecedentes familiares, estado nutricional y actividad física. El muestreo fue de las zonas censales de cada parroquia, se usó un muestreo aleatorio simple. Los datos fueron analizados en el programa SPSS versión 15.0, utilizando distribución de frecuencias, medidas de tendencia central: media y medidas de dispersión: desvió estándar. Resultados: la frecuencia de hipertensión arterial fue 52,5%. El 65,1% de hipertensos fueron mujeres y el 34.9% hombres. El 57,6% de hipertensos fueron estuvieron entre los 65 y 74 años. El 41,2% tenía instrucción primaria completa. El 76,4% no tenía una ocupación laboral. El 51,2% no tenía antecedentes familiares de HTA. El 40% realizaba actividad física moderada. El 41,9% presentó obesidad. Conclusión: la frecuencia de hipertensión arterial en los adultos mayores de las parroquias urbanas de la ciudad de Cuenca fue del 52,5%

Very high field (29)Si-NMR measurements using a fully (29)Si-enriched URu(2)Si(2) single crystal were carried out in order to microscopically investigate the hidden order (HO) state and adjacent magnetic phases in the high field limit. At the lowest measured temperature of 0.4 K, a clear anomaly reflecting a Fermi surface instability near 22 T inside the HO state is detected by the (29)Si shift, (29)K(c). Moreover, a strong enhancement of (29)K(c) develops near a critical field H(c) ≃ 35.6 T, and the ^{29}Si-NMR signal disappears suddenly at H(c), indicating the total suppression of the HO state. Nevertheless, a weak and shifted (29)Si-NMR signal reappears for fields higher than H(c) at 4.2 K, providing evidence for a magnetic structure within the magnetic phase caused by the Ising-type anisotropy of the uranium ordered moments.

Datos de humedad de equilibrio de polvo de pulpa de lulo (PL) con y sin aditivos - 58% de maltodextrina (MD) o 58% de goma Arábiga (GA) - fueran determinados en las temperaturas de 20, 30, 40 y 50 °C utilizando el método estático gravimétrico para un rango de actividades de agua entre 0.06 y 0.90. Las isotermas presentaran formato sigmoidal del tipo III, y el modelo de Guggenhein-Anderson-de Boer (GAB) fue ajustado satisfactoriamente a los datos experimentales de humedad de equilibrio en función de la actividad de agua. La adición de encapsulantes afecto las isotermas de tal manera que en la misma actividad de agua, las muestras PL + GA y PL + MD presentaron un menor contenido de humedad de equilibrio y no fueron afectadas por la variación de temperatura. Los calores isostéricos de sorción de las pulpas en polvo con encapsulantes fueron mayores (menos negativos) con relación a las pulpas de lulo en polvo, sugiriendo la existencia de sitios polares más activos en el producto sin adición de GA o MD. Una relación exponencial empírica fue utilizada para describir la dependencia del calor de sorción con el contenido de humedad del material.

A clinical Klebsiella pneumoniae isolate carrying the extended-spectrum beta-lactamase gene variants bla(SHV-40), bla(TEM-116) and bla(GES-7) was recovered. Cefoxitin and ceftazidime activity was most affected by the presence of these genes and an additional resistance to trimethoprim-sulphamethoxazole was observed. The bla(GES-7) gene was found to be inserted into a class 1 integron. These results show the emergence of novel bla(TEM) and bla(SHV) genes in Brazil. Moreover, the presence of class 1 integrons suggests a great potential for dissemination of bla(GES) genes into diverse nosocomial pathogens. Indeed, the bla(GES-7) gene was originally discovered in Enterobacter cloacae in Greece and, to our knowledge, has not been reported elsewhere

Chronic infusion of human amyloid-beta 1-40 (A beta) in the lateral ventricle (LV) of rats is associated with memory impairment and increase of kinin receptors in cortical and hippocampal areas. Deletion of kinin B1 or B2 receptors abolished memory impairment caused by an acute single injection of A beta in the LV. As brain tissue and kinin receptors could unlikely react to acute or chronic administration of a similar quantity of A beta, we evaluated the participation of B1 or B2 receptors in memory impairment after chronic infusion of A beta. Male C57BI/6 J (wt), knock-out B1 (koB1) or B2 (koB2) mice (12 weeks of age) previously trained in a two-way shuttle-box and achieving conditioned avoidance responses (CAR, % of 50 trials) were infused with AB (550 pmol, 0.12 mu L/h, 28 days) or vehicle in the LV using a mini-osmotic pump. They were tested before the surgery (TO), 7 and 35 days after the infusion started (T7; T35). In T0, no difference was observed between CAR of the control (Cwt = 59.7 +/- 6.7%; CkoB1 = 46.7 +/- 4.0%; CkoB2 = 64.4 +/- 5.8%) and A beta (A beta wt = 66.0 +/- 3.0%; A beta koB1 = 66.8 +/- 8.2%; A beta koB2 = 58.7 +/- 5.9%) groups. In T7, A beta koB2 showed a significant decrease in CAR (41.0 +/- 8.6%) compared to the control-koB2 (72.8 +/- 2.2%, P <0.05). In T35, a significant decrease (P <0.05) was observed in A beta wt (40.7 +/- 3.3%) and A beta koB2 (41.2 +/- 10.7%) but not in the A beta koB1 (64.0 +/- 14.0%) compared to their control groups. No changes were observed in the controls at T35. We suggest that in chronic infusion of BA, B1 receptors could playan important role in the neurodegenerative process. Conversely, the premature memory impairment of koB2 suggests that it may be a protective factor. (C) 2009 Elsevier Ltd. All rights reserved.

Continuing our series of papers on the three-dimensional (3D) structure and accurate distances of planetary nebulae (PNe), we present here the results obtained for PN NGC 40. Using data from different sources and wavelengths, we construct 3D photoionization models and derive the physical quantities of the ionizing source and nebular gas. The procedure, discussed in detail in the previous papers, consists of the use of 3D photoionization codes constrained by observational data to derive the 3D nebular structure, physical and chemical characteristics, and ionizing star parameters of the objects by simultaneously fitting the integrated line intensities, the density map, the temperature map, and the observed morphologies in different emission lines. For this particular case we combined hydrodynamical simulations with the photoionization scheme in order to obtain self-consistent distributions of density and velocity of the nebular material. Combining the velocity field with the emission-line cubes we also obtained the synthetic position-velocity plots that are compared to the observations. Finally, using theoretical evolutionary tracks of intermediate-and low-mass stars, we derive the mass and age of the central star of NGC 40 as (0.567 +/- 0.06) M(circle dot) and (5810 +/- 600) yr, respectively. The distance obtained from the fitting procedure was (1150 +/- 120) pc.
