Background: Asthma prevalence is high (>10%) in developed countries and although data is still missing for most of Africa, rates are increasing in developing regions as they become more westernized. We investigated the prevalence of asthma in school children in Gaborone, Botswana. Methods: This was a cross sectional descriptive study. ISAAC methodology was used. A representative proportionate size random sample of two age groups of children (13-14 year olds and 6-7 year olds) was consecutively enrolled from 10 schools. The schools were selected using a table of random numbers. A minimum sample size of 924 individuals (462 from each group) was adequate to achieve a precision of 3 % around our estimated prevalence of asthma of 10% with 95% confidence assuming a non-response rate of 20%. Data was collected using the validated International study of Asthma and Allergies in children (ISAAC) questionnaire. In accordance with the ISAAC criteria, Asthma was defined as wheezing in the previous 12 months. Data was captured in microsoft excel and analysed using SPSS version 23. Results: The prevalence of asthma (wheezing in the previous 12 months) was 16.5% (194/1175). Among the 6-7 year olds, the prevalence of asthma (wheezing in the previous 12 months) was 15.9%, while among the 13-14 years olds it was 16.8 %. The prevalence school type was 22.3 % in private schools versus 14.5 % in public schools. More severe asthma was associated with older children, 13 -14 years. The older children reported more limited speech due to wheezing (OR= 2.0, 95% CI =1.034, 3.9, p-value=0.043), ever had asthma (OR= 1.5, 95% CI=1.031, 2.3, p-value=0.034) and wheezing during exercise (OR=3.4, 95% CI= 2.5, 4.9, p-value= <0.001) compared to the younger children 6-7 years. Children from private schools had more wheezing symptoms. They were more likely to have ever wheezed (OR=2.2, .95% CI=1.7,2.9, p-value < 0.0001), wheezed in the previous twelve months (have asthma) (OR=1.7,95%CI=1.2,2.4, p-value = 0.001), ever had asthma (OR=2.4, 95% CI=1.7,3.5, pvalue< 0.0001), and wheezed during exercise (OR=1.8, 95% CI=1.4,2.4, p-value < 0.0001). Conclusion: The prevalence of asthma amongst school children in Gaborone, Botswana is high with older children experiencing more severe symptoms of asthma.

Objectives: To describe the frequency of feared discrimination in various social situations and of perceived discrimination in clinical settings, as well as to study the relationship between discrimination and depression and anger in women living with human immunodeiciency virus (HIV). Material and methods: The scale of Feared and Perceived Discrimination for Women with HIV (DTP-40-MV), the Beck Depression Inventory (BDI-2), and the Anger Expression scale of State-Trait-anger expression inventory (STaXi-2-aX/eX) were applied to a random sample of 200 women living with HIV. Results: These women feared being discriminated against, perceived discrimination upon the review of medical records, but perceived little discrimination in clinical care. a model with good adjustment to the data showed that the fear of being discriminated against creates a disposition toward perception of discrimination in the clinical settings (latent variable with 2 indicators: review of the medical records and clinical care) and increases cognitive/affective depressive symptoms; higher anger control decreases the anger manifestation; greater discrimination perceived in the clinical settings decreases anger control, which facilitates the expression of anger and slows cognitive/affective depressive symptoms; and these latter symptoms sensitize the perception of discrimination before the clinical records. Conclusion: Feared discrimination is a clinically relevant aspect due to its frequency and effect on depressive symptoms and perception of discrimination before the review of medical records.

An alarming rise of obesity in adolescents has been observed, placing them at risk of developing resistance to insulin (IR) and its adverse metabolic consequences such as diabetes mellitus type 2 (DM2), metabolic syndrome and cardiovascular disease. Objective: To know the prevalence of obesity, and the levels of glucose, insulin and HOMA-IR and its association with the BMIz, waist circumference and the pubertal stage in a random sample of adolescents. Materials and methods: 292 adolescents between 12 and 15 years (152 female and 140 male), whose anthropometric measurements (weight, height and waist circumference) were taken, body mass index calculated and Z-score determined. Blood glucose and insulin levels were evaluated from a sample of blood and their HOMA-IR was determined. Results: The results showed that 32.5% were obese, 22.3% were overweight and 23.6% showed adolescent central obesity. Glucose levels (p = 0.016), insulin (p = 0.0001) and HOMA-IR (p = 0.0001) showed significant differences in the group with obesity. Values of the three parameters were increased with the stage of puberty. We found significant differences in the levels of glucose (p = 0.0388), insulin (p = 0.0005) and HOMA-IR (p = 0.0001) between the prepubertal and postpubertal stages.

O objetivo deste trabalho de investigação é o de procurar compreender o impacto da estrutura organizacional de dois hospitais públicos portugueses com diferentes figuras jurídicas, na execução dos seus objetivos, através da perceção dos seus colaboradores. De modo a atingirmos o nosso objetivo, recorremos à metodologia do estudo de caso, efetuado a cada um dos hospitais escolhidos. O principal instrumento de recolha de dados é o questionário, efetuado a uma população que é constituída por uma amostra aleatória simples e estratificada em classes de cada um dos hospitais. Para além de não terem sido encontradas diferenças significativas, nas áreas analisadas, entre os dois hospitais, este estudo revelou-nos ainda que a sua estrutura organizacional deve ser mais orgânica, no sentido de uma estrutura matricial, mantendo, no entanto, a parte burocrática que advêm da elevada profissionalização do seu pessoal e os principais mecanismos de coordenação. Este trabalho pretende dar um contributo no conhecimento da forma como os gestores dos hospitais podem controlar e modificar a estrutura e o desenho da organização e de que forma essas modificações podem vir a influenciar a execução dos seus objetivos. ABSCTRAT: The purpose of this research work is to seek to understand the organizational structure’s impact of two portuguese public hospitals, with different legal forms, in the implementation of their objectives through its collaborators' perception. ln order to achieve our purpose, we have used the case study methodology performed in each one of the chosen hospitals. The main instrument for data collection is the questionnaire, made to a population consisting of a simple and stratified into classes random sample from each one of the hospitals. Besides not having been found any significant differences between the two hospitals, this study has also revealed to us that its organizational structure should be more organic, towards a matrix structure, although maintaining the bureaucratic part arising from the high professionalization of its personnel and the main coordination mechanisms. This work aims to contribute to the knowledge of how hospital managers can control and modify the organization's structure and design and how those modifications may come to influence the implementation of their objectives.

Las atopias constituyen un problema de salud que puede afectar a adolescentes y adultos, llegando incluso a ser mortal en ciertos casos. La investigación en este tema es escasa en nuestro medio, es por ello que este estudio tiene como objetivo determinar la prevalencia de atopias y los alérgenos inhalantes más comunes entre los adolescentes de octavo y noveno de básica de las áreas urbana y rural del cantón Cuenca, mediante la aplicación de un “skin prick test”. Para ello fueron evaluados los estudiantes de octavo y noveno de básica de los colegios del cantón Cuenca, incuyendo una muestra aleatoria representativa de 991 alumnos. Los resultados demostraron que más de la mitad de los participantes fueron atópicos para al menos uno de los alérgenos probados, situación que refleja un problema de salud pública que debe ser considerado. La prevalencia de atopia no fue diferente de acuerdo al sexo de los participantes; de los 20 alérgenos probados, los alérgenos intra-domiciliarios y las gramíneas estuvieron entre los más prevalentes, los cinco más comunes fueron los ácaros D. pteronyssinus y blomia tropicalis, epitelio de cucaracha, mezcla de pólenes de gramíneas y ecynodon. No se encontró una diferencia significativa en la prevalencia global de atopia según la ubicación geográfica de los colegios. Sin embrago, el análisis por alérgeno reveló que la atopia al epitelio de cucaracha fue mayor entre los alumnos que asisten a colegios rurales y la atopia al salix fue mayor entre los adolescentes de los colegios urbanos

Bowel cancer is the third most common cancer in Australia. The Australian Cancer Society has set a national goal 'to exploit prevention opportunities and to increase early detection'. To address this goal, information about community perceptions is required. Through the use of a Delphi process, a questionnaire was developed to investigate community knowledge, attitudes and beliefs regarding primary prevention and early detection activities. This paper describes the results of a postal survey of a random sample of 855 Queensland adults selected from electoral rolls. Results indicate that there is a high level of uncertainty about bowel cancer and a belief that many behaviours are related to bowel cancer. While 74 per cent believed that tests could detect bowel cancer, only 52 per cent were confident that a doctor could accurately diagnose it. Only 35 per cent agreed that the public should be screened for bowel cancer and only 10 per cent would get a check-up. Perhaps reflecting the degree of uncertainty among health professionals about bowel cancer, there appears to be a high level of misinformation and confusion in the community. County jurisdictions in America are increasingly exercising self-government in the provision of public community services through the context of second order federalism. In states exercising this form of contemporary governance, county governments with “reformed” policy-making structures and professional management practices, have begun to rival or surpass municipalities in the delivery of local services with regional implications such as environmental protection (Benton 2002, 2003; Marando and Reeves, 1993). The voter referendum, a form of direct democracy, is an important component of county land preservation and environmental protection governmental policies. The recent growth and success of land preservation voter referendums nationwide reflects an increase in citizen participation in government and their desire to protect vacant land and its natural environment from threats of over-development, urbanization and sprawl, loss of open space and farmland, deterioration of ecosystems, and inadequate park and recreational amenities. The study’s design employs a sequential, mixed method. First, a quantitative approach employs the Heckman two-step model. It is fitted with variables for the non-random sample of 227 voter referendum counties and all non-voter referendum counties in the U.S. from 1988 to 2009. Second, the qualitative data collected from the in-depth investigation of three South Florida county case studies with twelve public administrator interviews is transformed for integration with the quantitative findings. The purpose of the qualitative method is to complement, explain and enrich the statistical analysis of county demographic, socio-economic, terrain, regional, governance and government, political preference, environmentalism, and referendum-specific factors. The research finds that government factors are significant in terms of the success of land preservation voter referendums; more specifically, the presence of self-government authority (home rule charter), a reformed structure (county administrator/manager or elected executive), and environmental interest groups. In addition, this study concludes that successful counties are often located coastal, exhibit population and housing growth, and have older and more educated citizens who vote democratic in presidential elections. The analysis of case study documents and public administrator interviews finds that pragmatic considerations of timing, local politics and networking of regional stakeholders are also important features of success. Further research is suggested utilizing additional public participation, local government and public administration factors.

Introduction: The focus of the community pharmacist’s (CP’s) activities continues to move away from traditional dispensing activities towards the provision of health services. Current functions of CPs cover a combination of roles including prescription matters, counselling and service provision. These expanding roles, along with raised prescription volume, have increased CP workload. Therefore, it has become commonplace to delegate certain activities to other pharmacy staff (PS). This research aimed to examine public perceptions of CPs and other PS functions. Methodology: A self-completion postal questionnaire was sent to a random sample of 9769 members of the general public in England. Participants were asked to indicate which functions they believed CPs and other PS perform. Data were imported into SPSS 22 for analysis. Results: A response rate of 15.7% (n = 1537) was achieved. The roles most commonly attributed to CPs were monitoring prescription appropriateness (90.4%, n = 1390) and counselling patients on prescribed medicines (90.4%, n = 1389). The role most commonly attributed to other PS was sales transactions (92.4%, n = 1420). Similar numbers of responders agreed that the delivery of health services was the role of both CPs and other PS (58.9%, n = 906; 57.0%, n = 876). Conclusion: Despite a move towards more service based practice, the public still primarily associate the CP’s role with activities centred on dispensing. The provision of health services was seen to be equally carried out by CPs and other PS. As the CP’s service-based activities continue to develop, promotional activities may be required to ensure developments in CP functions are recognised by the public
