The objectives of this study were (1) to determine the sensitivity (Se) and specificity (Sp) of somatic cell count (SCC) thresholds to identify subclinical mastitis in Gyr cows caused by major and minor pathogens; (2) to study the effects of month of sampling, rear or front mammary quarters, herd, intramammary infection (IMI), and bacterial species on SCC at quarter level; and (3) to describe the prevalence of IMI in Gyr cows in commercial dairy herds. In total, 221 lactating Gyr cows from 3 commercial dairy farms were selected. Milk samples were collected from individual quarters once a month for 1 yr from all lactating cows for SCC and bacteriological analysis. Mammary quarters were considered the experimental units and the SCC results were log(10)-transformed. Four SCC thresholds (100, 200, 300 and 400 x 10(3) cells/mL) were used to determine Se and Sp to identify infected mammary quarters. The overall prevalence of IMI in quarter milk samples of Gyr cows was 49.8%, and the prevalence of minor pathogens was higher (31.9%) than that of major pathogens (17.8%). Quarter samples with microbial isolation presented higher SCC compared with negative samples. Sensitivity and Sp of selected SCC thresholds varied according to the group of pathogen (major and minor) involved in the IMI definition. Sensitivity increased and Sp decreased when mammary quarters with only major pathogens isolation were considered positive. The use of a single SCC analysis to classify quarters as uninfected or infected in Gyr cows may not be a useful test for this breed because Se and Sp of SCC at the studied thresholds were low. The occurrence of IMI and the bacterial species are the main factors responsible for SCC variation in mammary quarters of Gyr cows. Milk samples with major pathogens isolation elicited higher SCC than those with minor pathogens.

Purpose: The objective of this in vitro study was to compare the degree of microleakage of composite restorations performed by lasers and conventional drills associated with two adhesive systems. Materials and Methods: Sixty bovine teeth were divided into 6 groups (n = 10). The preparations were performed in groups 1 and 2 with a high-speed drill (HID), in groups 3 and 5 with Er:YAG laser, and in groups 4 and 6 with Er,Cr:YSGG laser. The specimens were restored with resin composite associated with an etch-and-rinse two-step adhesive system (Single Bond 2 [SB]) (groups 1, 3, 4) and a self-etching adhesive (One-Up Bond F [OB]) (groups 2, 5, 6). After storage, the specimens were polished, thermocycled, immersed in 50% silver nitrate tracer solution, and then sectioned longitudinally. The specimens were placed under a stereomicroscope (25X) and digital images were obtained. These were evaluated by three blinded evaluators who assigned a microleakage score (0 to 3). The original data were submitted to Kruskal-Wallis and Mann-Whitney statistical tests. Results: The occlusal/enamel margins demonstrated no differences in microleakage for all treatments (p > 0.05). The gingival/dentin margins presented similar microleakage in cavities prepared with Er:YAG, Er,Cr:YSGG, and HD using the etch-and-rinse two-step adhesive system (SB) (p > 0.05); otherwise, both Er:YAG and Er,Cr:YSGG lasers demonstrated lower microleakage scores with OB than SB adhesive (p < 0.05). Conclusion: The microleakage score at gingival margins is dependent on the interaction of the hard tissue removal tool and the adhesive system used. The self-etching adhesive system had a lower microleakage score at dentin margins for cavities prepared with Er:YAG and Er,Cr:YSGG than the etch-and-rinse two-step adhesive system.

Purpose: To evaluate the influence of dentin moisture on bond strengths of an etch-and-rinse bonding agent to primary dentin clinically and in the laboratory. Methods: The sample consisted of two groups of 20 caries-free primary second molars: molars in exfoliation period (clinical group) and extracted molars (laboratory group). Class I cavities were prepared in all specimens leaving a flat dentin surface on the pulpal floor. A two-step etch-and-rinse adhesive was vigorously rubbed on either dry (n= 5) or wet demineralized dentin (n= 5) under clinical or laboratory conditions. After restorative procedures, the teeth from the clinical group were extracted after 20 minutes. All samples were processed and underwent microtensile bond strength test and silver nitrate uptake evaluation under scanning electron microscopy. Results: Statistically higher bond strength values were observed when the bonding was performed under laboratory conditions and on a wet demineralized dentin. Most of the failures were adhesive and mixed irrespective of the experimental condition. Silver nitrate uptake occurred in all groups irrespective of the experimental condition. Resin-dentin bond strengths produced in the laboratory in primary teeth may overestimate those produced under clinical circumstances. (Am J Dent 2011;24:221-225).

Objective: Verify the influence of radiant exposure (H) on composite degree of conversion (DC) and mechanical properties. Methods: Composite was photoactivated with 3, 6, 12, 24, or 48 J/cm(2). Properties were measured after 48-h dry storage at room temperature. DC was determined on the flat surfaces of 6 mm x 2 mm disk-shaped specimens using FTIR. Flexural strength (FS) and modulus (FM) were accessed by three-point bending. Knoop microhardness number (KHN) was measured on fragments of FS specimens. Data were analyzed by one-way ANOVA/Tukey test, Student`s t-test, and regression analysis. Results: DC/top between 6 and 12 J/cm(2) and between 24 and 48 J/cm(2) were not statistically different. No differences between DC/top and bottom were detected. DC/bottom, FM, and KHN/top showed significant differences among all H levels. FS did not vary between 12 and 24 J/cm(2) and between 24 and 48 J/cm(2). KHN/bottom at 3 and 6 J/cm(2) was similar. KHN between top and bottom was different up to 12 J/cm(2). Regression analyses having H as independent variable showed a plateau region above 24 J/cm(2). KHN increased exponentially (top) or linearly (bottom) with DC. FS and FM increased almost linearly with DC/bottom up to 55% conversion. Conclusions: DC and mechanical properties increased with radiant exposure. Variables leveled off at high H levels. A unilateral hearing loss is characterized by reduced hearing in one ear. The problems caused by sensory deprivation can be minimized with the use of hearing aids (HA). Aim: To analyze the correlation between the prescribed grain and the insertion gain difference and with the results obtained regarding the benefit and satisfaction with the use of hearing aids in unilateral hearing impaired patients. Materials and Methods: Prospective study with 15 subjects, mean age of 41.6 years, of both genders, users of hearing aids effectively. We used the International Questionnaire Results for hearing aids (International Outcome Inventory for Hearing Aids - IOI-HA), measured with a probe microphone. Results: The mean values in the analyses of the IOI-HA per item were positive and higher than four points. In relation to the objective measures, the frequencies in which we obtained the gain values which were closer to the target were: 1K Hz, 2K Hz and 500 Hz, respectively. Conclusion: The satisfaction of individuals using hearing aid unilaterally is not completely correlated to the prescribed gain, because even if the target is not being reached in some frequencies, the individuals were pleased as to the use of their hearing aids.

Correcting a Class III subdivision malocclusion is usually a challenge for an orthodontist, especially if the patient`s profile does not allow for any extractions. One treatment option is to use asymmetric intermaxillary elastics to correct the unilateral anteroposterior discrepancy. However, the success of this method depends on the individual response of each patient and his or her compliance in using the elastics. The objectives of this article were to present a successful treatment of a Class III subdivision patient with this approach and to illustrate and discuss the dentoskeletal changes that contributed to the correction. Objectives: To compare the population modelling programs NONMEM and P-PHARM during investigation of the pharmacokinetics of tacrolimus in paediatric liver-transplant recipients. Methods: Population pharmacokinetic analysis was performed using NONMEM and P-PHARM on retrospective data from 35 paediatric liver-transplant patients receiving tacrolimus therapy. The same data were presented to both programs. Maximum likelihood estimates were sought for apparent clearance (CL/F) and apparent volume of distribution (V/F). Covariates screened for influence on these parameters were weight, age, gender, post-operative day, days of tacrolimus therapy, transplant type, biliary reconstructive procedure, liver function tests, creatinine clearance, haematocrit, corticosteroid dose, and potential interacting drugs. Results: A satisfactory model was developed in both programs with a single categorical covariate - transplant type - providing stable parameter estimates and small, normally distributed (weighted) residuals. In NONMEM, the continuous covariates - age and liver function tests - improved modelling further. Mean parameter estimates were CL/F (whole liver) = 16.3 1/h, CL/F (cut-down liver) = 8.5 1/h and V/F = 565 1 in NONMEM, and CL/F = 8.3 1/h and V/F = 155 1 in P-PHARM. Individual Bayesian parameter estimates were CL/F (whole liver) = 17.9 +/- 8.8 1/h, CL/F (cutdown liver) = 11.6 +/- 18.8 1/h and V/F = 712 792 1 in NONMEM, and CL/F (whole liver) = 12.8 +/- 3.5 1/h, CL/F (cut-down liver) = 8.2 +/- 3.4 1/h and V/F = 221 1641 in P-PHARM. Marked interindividual kinetic variability (38-108%) and residual random error (approximately 3 ng/ml) were observed. P-PHARM was more user friendly and readily provided informative graphical presentation of results. NONMEM allowed a wider choice of errors for statistical modelling and coped better with complex covariate data sets. Conclusion: Results from parametric modelling programs can vary due to different algorithms employed to estimate parameters, alternative methods of covariate analysis and variations and limitations in the software itself.

A primeira relaciona-se ao direito de manuten????o dos v??nculos afetivos dos presidi??rios, possibilitando o contato deles com seus familiares e amigos; e a segunda refere-se ?? realiza????o de audi??ncias judiciais por videoconfer??ncia. Desde a implanta????o do Projeto Visita Virtual e Videoconfer??ncia Judicial, em maio de 2010, 509 presos participaram da visita virtual e puderam conversar e visualizar seus familiares e amigos por meio desse recurso, conferindo, assim, um resultado extremamente satisfat??rio, visto que h?? pelo menos um ano n??o recebiam visitas. De mar??o de 2011 at?? julho deste ano, foram realizadas 160 videoconfer??ncias

Em raz??o disso, o Poder Executivo Federal patrocinou ampla reforma legislativa da mat??ria previdenci??ria p??blica, mediante aprova????o da Emenda Constitucional n?? 20/98 e da Lei 9.717/98, a chamada Lei Geral da Previd??ncia Social, calcada em preceitos que possam assegurar equil??brio financeiro e atuarial dos Regimes Pr??prios de Previd??ncia Social - RPPS. A situa????o, ??quela altura, da previd??ncia gerida por Estados e Munic??pios, apresentava-se preocupante, especialmente quanto aos aspectos estruturais, operacionais e t??cnicos, incluindo base de informa????es desatualizadas, dispersas e pulverizadas nos diversos ??rg??os de cada ente federado, bem como gestores e t??cnicos desqualificados para atender aos novos preceitos estabelecidos pela Reforma Previdenci??ria. Ressalta, a partir da??, o conjunto de medidas constante no ???Pacto em favor de um Judici??rio mais r??pido e republicano???, firmados pelos representantes m??ximos dos tr??s poderes. A primeira delas constitui a realiza????o de um diagn??stico do Judici??rio. A segunda a????o diz respeito ?? moderniza????o de sua gest??o, por meio, entre outros, da capacita????o de servidores no exerc??cio das atividades administrativas e da incorpora????o de novas tecnologias. Por fim, integram-se a esse conjunto iniciativas de altera????o legislativa.

O presente trabalho utiliza a metodologia DEA (Data Envelopment Analysis ), para avaliar a efici??ncia das 22 Ag??ncias da Previd??ncia Social ??? Ger??ncia Executiva Fortaleza (APS-GEXFOR). DEA utiliza programa????o linear cujo n??cleo das estruturas anal??ticas ?? formado a partir do modelo original CCR (Charnes, Cooper, e Rhodes). Aplicada ??s DMU (Decision Making Units), define uma Fronteira de Efici??ncia identificando Unidades Eficientes e Ineficientes. Foi utilizado o modelo DEA-CCR implementado pelo software DEA Solver??. A Previd??ncia Social (INSS) disp??e de indicadores de desempenho. Algumas vari??veis utilizadas no modelo implementado derivam desses indicadores, outras informa????es foram disponibilizadas pelos sistemas de informa????o da institui????o. A avalia????o de efici??ncia DEA das APS-GEXFOR permitiu identificar as melhores pr??ticas, mensurar a participa????o de cada vari??vel envolvida na avalia????o da unidade e projetar as unidades ineficientes na fronteira de efici??ncia, identificando metas a serem atingidas para torn??-las eficientes no conjunto observado.

O presente artigo pretende analisar a quest??o da qualidade da programa????o na televis??o brasileira a partir da proposta de um novo marco regulat??rio para o setor de comunica????o social eletr??nica. Essa nova lei, entre outras disposi????es, ir?? regulamentar o artigo 221 da Constitui????o Federal, que trata dos princ??pios pelos quais o conte??do televisivo deve pautar-se. Com isso, define-se qualidade levando-se em considera????o dois aspectos: diversidade e ressalvas ?? liberdade de express??o, ambos previstos na Constitui????o Federal. A partir dessa conceitua????o, prop??e-se a instrumentaliza????o do controle social sobre o conte??do televisivo e a garantia de meios para a diversidade da programa????o. Com rela????o ao primeiro aspecto, recomenda-se a atua????o transparente de uma futura ag??ncia reguladora e a implementa????o de mecanismo de controle individual da programa????o. No que tange ?? diversidade, ressalta-se a import??ncia do fortalecimento das televis??es p??blicas e medidas governamentais no sentido de estimular a multiprograma????o propiciada pelo advento da tecnologia digital.
