Evaluación de la respuesta cardiovascular a dobutamina en la disfunción miocárdica asociada al shock séptico

Objetivos: 1. Evaluar la respuesta cardiovascular a dobutamina en la disfunción miocárdica secundaria a shock séptico. 2. Determinar si el análisis de la respuesta cronotrópica refleja la respuesta inotrópica. Material y métodos: pacientes con shock séptico y disfunción miocárdica (Fracción eyección ventrículo izquierdo, FEVI < 45%) que recibieron dobutamina (5 µg/kg/min) por persistencia de hipoperfusión tisular. Antes y 30 min después de su administración se recogieron parámetros ecocardiográficos y parámetros hemodinámicos. Datos: media y SD. Análisis estadístico mediante test de Wilcoxon /Mann Whitney. Resultados: Se incluyeron 22 pacientes. Dobutamina produjo un incremento significativo en la FC (90 ± 17 vs 107 ± 20 lpm, p < 0.0001), la FEVI (34 ± 7 vs 47 ± 10 %, p < 0.0001), el VSi eco (24.9 ± 5.9 vs 30 ± 7.4 ml/m2, p < 0.01) y el VSi td (28.6 ± 10.3 vs 34.7 ± 13.8 ml/m2, p < 0.005). Sólo tres pacientes no incrementaron ni FC ni VSi td. Respuesta cronotrópica significativa: sensibilidad 69% y especificidad 37% para detectar la respuesta inotrópica. Conclusiones: Dobutamina mejora la función cardiovascular en la disfunción miocárdica asociada a sepsis.

Minimax lower bounds for the two-armed bandit problem

We obtain minimax lower bounds on the regret for the classicaltwo--armed bandit problem. We provide a finite--sample minimax version of the well--known log $n$ asymptotic lower bound of Lai and Robbins. Also, in contrast to the log $n$ asymptotic results on the regret, we show that the minimax regret is achieved by mere random guessing under fairly mild conditions on the set of allowable configurations of the two arms. That is, we show that for {\sl every} allocation rule and for {\sl every} $n$, there is a configuration such that the regret at time $n$ is at least 1 -- $\epsilon$ times the regret of random guessing, where $\epsilon$ is any small positive constant.

IL-2/anti-IL-2 antibody complexes show strong biological activity by avoiding interaction with IL-2 receptor alpha subunit CD25.

IL-2 is crucial to T cell homeostasis, especially of CD4(+) T regulatory cells and memory CD8(+) cells, as evidenced by vigorous proliferation of these cells in vivo following injections of superagonist IL-2/anti-IL-2 antibody complexes. The mechanism of IL-2/anti-IL-2 antibody complexes is unknown owing to a lack of understanding of IL-2 homeostasis. We show that IL-2 receptor alpha (CD25) plays a crucial role in IL-2 homeostasis. Thus, prolongation of IL-2 half-life and blocking of CD25 using antibodies or CD25-deficient mice led in combination, but not alone, to vigorous IL-2-mediated T cell proliferation, similar to IL-2/anti-IL-2 antibody complexes. These data suggest an unpredicted role for CD25 in IL-2 homeostasis.

Human large granular lymphocytes contain an esterase activity usually considered as specific for the myeloid series.

A cytochemical marker such as alpha-naphthyl acetate esterase (ANAE) has been found useful for the morphological identification of the subset of T lymphocytes having receptors for Fcμ (TM cells). ANAE reaction on TM cells gives a typical pattern of one to four positive spots, whereas this pattern is not found on T cells with receptors for Fcγ (TG cells). ANAE is abundant in monocytes but not detectable in granulocytes. Herein another type of esterase activity, naphthol-AS-D chloroacetate esterase (NCAE), is described; it is known to be abundant in granulocytes and was found to give a specific pattern of reactivity with the subpopulation of large granular lymphocytes (LGL). This pattern of fine granular staining was observed not only on LGL present in the TG cell subpopulation but also in LGL present in the non-T, non-B cells. Fractions of peripheral blood mononuclear cells which were ènriched up to 80% in LGL by Percoll discontinuous density gradient gave a similar percentage of specific NCAE pattern. In addition, among the different fractions from Percoll gradient, there was a good correlation (r = 0.94) between the number of NCAE-positive cells and the natural killer activity against the natural killer susceptible K562 target cells. It will be important to determine whether or not this enzymatic activity plays a role in the cytotoxic activities of LGL.

ATP release due to Thy-1-integrin binding induces P2X7-mediated calcium entry required for focal adhesion formation.

Thy-1, an abundant mammalian glycoprotein, interacts with αvβ3 integrin and syndecan-4 in astrocytes and thus triggers signaling events that involve RhoA and its effector p160ROCK, thereby increasing astrocyte adhesion to the extracellular matrix. The signaling cascade includes calcium-dependent activation of protein kinase Cα upstream of Rho; however, what causes the intracellular calcium transients required to promote adhesion remains unclear. Purinergic P2X7 receptors are important for astrocyte function and form large non-selective cation pores upon binding to their ligand, ATP. Thus, we evaluated whether the intracellular calcium required for Thy-1-induced cell adhesion stems from influx mediated by ATP-activated P2X7 receptors. Results show that adhesion induced by the fusion protein Thy-1-Fc was preceded by both ATP release and sustained intracellular calcium elevation. Elimination of extracellular ATP with Apyrase, chelation of extracellular calcium with EGTA, or inhibition of P2X7 with oxidized ATP, all individually blocked intracellular calcium increase and Thy-1-stimulated adhesion. Moreover, Thy-1 mutated in the integrin-binding site did not trigger ATP release, and silencing of P2X7 with specific siRNA blocked Thy-1-induced adhesion. This study is the first to demonstrate a functional link between αvβ3 integrin and P2X7 receptors, and to reveal an important, hitherto unanticipated, role for P2X7 in calcium-dependent signaling required for Thy-1-stimulated astrocyte adhesion.

Les organisations Freies Deutschland en France et en Suisse : nouveaux partenaires allemands à la fin de la Seconde Guerre mondiale ?

Cet article analyse le processus qui mène à la légalisation des organisations Freies Deutschland en France et en Suisse. Celles-ci ont été fondées en automne 1943, à la suite du National Komitee « Freies Deutschland », leur organisation mère créée par des prisonniers de guerre de la Wehrmacht et des réfugiés politiques allemands. Le but de ces organisations est d'abattre le régime nazi et de rassembler les exilés et militaires allemands qui partagent ses buts. L'article analyse également comment les gouvernements français et suisse ont reçu ces organisations et dans quelle mesure ils leur ont permis d'exister et de développer leurs activités dans les deux pays. Leurs réactions face à ces organisations sont en grande partie déterminées par leur propre politique dans les domaines dans lesquels Freies Deutschland se propose également d'intervenir.

Prévention de la violence commise par les spectateurs lors de manifestations sportives : étude des mesures préventives et de la responsabilité de l'organisateur à la lumière du droit comparé, du droit suisse et du droit associatif

Introduction 1. Généralités : Le sport occupe une place importante dans notre société, de manière active par la pratique d'une ou de plusieurs activités sportives, ou de manière passive, au travers de la presse, de la radio et de la télévision. Le sport est ainsi un acte de participation, d'appartenance, de revendication et d'intégration à la société en général ou à un groupe. Il stimule l'imagination et permet de rêver aux héros sportifs. Enfin, non seulement il améliore la santé de ceux qui le pratique, mais il a une dimension éducative et joue un rôle social, culturel et récréatif. Toutefois, le sport est également un spectacle qui provoque des passions et engendre des émotions de la part des supporters, dont certains s'exaltent pour leur équipe fétiche. Il arrive que ce supportérisme soit à tel point exacerbé qu'il mène à des dérives pouvant aboutir à des actes de violence dans et en dehors des stades, ceci tant avant, pendant qu'après le match. A titre d'exemple tragique, les téléspectateurs garderont longtemps en mémoire les scènes auxquelles ils ont assisté le 29 mai 1985, en direct, lorsque, avant le début de la rencontre, des hooligans anglais ont attaqué des supporters italiens dans les gradins du bloc Z du stade du Heysel à Bruxelles, lors de la finale de la Coupe d'Europe des champions, opposant le FC Liverpool à la Juventus de Turin; 39 personnes en sont mortes et 600 ont été blessées. La Suisse, longtemps épargnée par le phénomène, en regard de la situation qui a prévalu dans d'autres Etats européens, ne peut échapper, depuis quelques années, au triste constat selon lequel les stades constituent désormais des environnements propices à des actions de violence, de racisme et, plus rarement, d'extrémisme. Le cas le plus révélateur a eu lieu le 13 mai 2006, lorsque des fauteurs de trouble ont envahi le terrain du Parc Saint-Jacques de Bâle après le coup de sifflet final du match de championnat opposant le FC Bâle au FC Zurich, match dont l'enjeu était la première place du classement du championnat de Super League, pour attaquer à coups de pied et à coups de poing des joueurs, des accompagnants et des personnes chargées de la sécurité. Les affrontements ont continué dans la rue jusque tard dans la soirée. Il s'en est suivi une centaine de blessés et des dégâts d'un demi million de francs. De tels débordements mettent en danger la sécurité du public, des équipes et des arbitres. Il s'agit de tout mettre en oeuvre afin que les spectateurs qui assistent à une manifestation sportive puissent prendre du plaisir aux performances des sportifs sans devoir craindre pour leur sécurité. De même, les acteurs sur le terrain doivent pouvoir exercer leur sport sans craindre un envahissement de l'aire de jeu. Ainsi, les Etats et les associations sportives ont élaboré des textes juridiques afin d'éviter des débordements ou tout autre événement qui pourraient mettre en danger des personnes ou des biens matériels lors de manifestations sportives. Sous l'angle du droit étatique helvétique, cela s'est traduit, notamment en vue du déroulement en Suisse du Championnat d'Europe de football de l'UEFA en 2008 (EURO 2008) et du Championnat du monde de hockey sur glace en 2009, par l'adoption de mesures préventives permettant de lutter contre les actes de violence lors de manifestations sportives, introduites dans la Loi fédérale du 21 mars 1997 instituant des mesures visant au maintien de la sûreté intérieure (LMSI). Elles se concrétisent par l'inscription d'individus ayant commis des actes de violence dans une banque de données nationale, ainsi que par le recours au périmètre d'exclusion, à l'interdiction de sortie du territoire, à l'obligation de s'annoncer à la police et, en dernier ressort, à la garde à vue; enfin, il est également possible de saisir, séquestrer ou confisquer du matériel de propagande5. La mise en place de telles mesures relève de l'Etat, garant de la sécurité et de l'ordre publics à l'extérieur des enceintes sportives. L'organisateur, chargé quant à lui d'assurer la sécurité à l'intérieur du stade, n'est toutefois pas en marge, puisque les fédérations et associations sportives ont édicté des règlements dont il est le destinataire. Ces textes prévoient, à sa charge, notamment les mesures suivantes: le prononcé d'interdictions de stade à l'encontre de supporters violents, la fouille accrue des spectateurs, l'engagement d'un service de sécurité privé, l'obligation de désigner un responsable de la sécurité, la séparation des différents groupes de supporters, etc.. Il appartient ainsi aux associations sportives, aux organisateurs, aux chargés de la sécurité au sein des clubs et aux forces de l'ordre public d'appliquer de la meilleure façon que ce soit les mesures proposées et de collaborer afin de combattre les débordements des spectateurs de manière effective. Prévenir et supprimer la violence dans les manifestations sportives exige ainsi la mobilisation et la collaboration de tous les protagonistes concernés.

Protection from lethal gram-negative bacterial sepsis by targeting Toll-like receptor 4.

Toll-like receptor 4 (TLR4), the signal-transducing molecule of the LPS receptor complex, plays a fundamental role in the sensing of LPS from gram-negative bacteria. Activation of TLR4 signaling pathways by LPS is a critical upstream event in the pathogenesis of gram-negative sepsis, making TLR4 an attractive target for novel antisepsis therapy. To validate the concept of TLR4-targeted treatment strategies in gram-negative sepsis, we first showed that TLR4(-/-) and myeloid differentiation primary response gene 88 (MyD88)(-/-) mice were fully resistant to Escherichia coli-induced septic shock, whereas TLR2(-/-) and wild-type mice rapidly died of fulminant sepsis. Neutralizing anti-TLR4 antibodies were then generated using a soluble chimeric fusion protein composed of the N-terminal domain of mouse TLR4 (amino acids 1-334) and the Fc portion of human IgG1. Anti-TLR4 antibodies inhibited intracellular signaling, markedly reduced cytokine production, and protected mice from lethal endotoxic shock and E. coli sepsis when administered in a prophylactic and therapeutic manner up to 13 h after the onset of bacterial sepsis. These experimental data provide strong support for the concept of TLR4-targeted therapy for gram-negative sepsis.

Aspectos reproductivos de la anchoveta peruana durante el periodo 1992-2000

Se colectaron y se analizaron con métodos histológicos, ovarios de anchoveta adulta(> 12 cm LT), para comparar algunos parámetros reproductivos: fracción desovante parcial por mes (FDPM),fecundidad parcial (FP), factor de condición (FC) e índice gonadosomático (IGS), durante años normales y años El Niño. El material se obtuvo del monitoreo de las pesquerías en los puertos de desembarque desde mayo 1992 hasta abril 2000, además de once cruceros de evaluación de biomasa de anchoveta entre 1997 y 2000. La FDPM mostró una buena correlación directa con elGS (R = 0,89), pero una correlación inversa con el FC (R=0,7). Este factor comparado con la temperatura superficial del mar (TSM) mensual presentó una baja correlación (R =0,4), pero ésta fue mayor cuando se comparó con la desviación estándar de la TSM mensual (R = -0,7) . Este resultado confirmó que la estación de desove coincidió con el periodo de menor variabilidad de la TSM. En relación a El Niño 1997-98,la anchoveta respondió con una estación de desove más corta, un retraso en máximo de desove una reducción de la FP.

Rostrum length, mandible serration, and food and salivary canals areas of selected species of stink bugs (Heteroptera, Pentatomidae)

Laboratory studies were conducted to compare rostrum length morphology of mandible serration and area of food and salivary canals of Dichelops melacanthus (Dallas) (Dm), Euschistus heros (F.) (Eh), Nezara viridula (L.) (Nv), and Piezodorus guildinii (Westwood) (Pg) (Heteroptera: Pentatomidae). Nv showed the longest (5.9 mm) and Pg the shortest (3.5 mm) rostrum length; Dm and Eh were intermediate. Length and width of mandible tip areas holding serration was bigger for Nv (106.0 and 30.2 µm, respectively) and smaller for Pg (71.1 and 23.7 µm), with all species having four central teeth and three pairs of lateral teeth. The inner mandible surface showed squamous texture. Cross-section of food and salivary canals (Fc and Sc) indicated greater area for Nv and Dm compared to Eh and Pg; however, the ratio Fc/Sc, yielded the highest relative area for Pg.

Decision making in uncertain and changing environments

We consider an agent who has to repeatedly make choices in an uncertainand changing environment, who has full information of the past, who discountsfuture payoffs, but who has no prior. We provide a learning algorithm thatperforms almost as well as the best of a given finite number of experts orbenchmark strategies and does so at any point in time, provided the agentis sufficiently patient. The key is to find the appropriate degree of forgettingdistant past. Standard learning algorithms that treat recent and distant pastequally do not have the sequential epsilon optimality property.

Identification of optimal structural connectivity using functional connectivity and neural modeling.

The complex network dynamics that arise from the interaction of the brain's structural and functional architectures give rise to mental function. Theoretical models demonstrate that the structure-function relation is maximal when the global network dynamics operate at a critical point of state transition. In the present work, we used a dynamic mean-field neural model to fit empirical structural connectivity (SC) and functional connectivity (FC) data acquired in humans and macaques and developed a new iterative-fitting algorithm to optimize the SC matrix based on the FC matrix. A dramatic improvement of the fitting of the matrices was obtained with the addition of a small number of anatomical links, particularly cross-hemispheric connections, and reweighting of existing connections. We suggest that the notion of a critical working point, where the structure-function interplay is maximal, may provide a new way to link behavior and cognition, and a new perspective to understand recovery of function in clinical conditions.

Aspectos reproductivos de Vinciguerria lucetia (Garman, 1899)

En el Perú, los estudios sobre peces mesopelágicos, fueron orientados hacia aspectos de taxonomía, distribución espacial y estimación acústica de la biomasa de Vinciguerria lucetia “vinciguerria”. Debido a su importancia tanto en el aspecto nutricional, económico como ecológico, la presente tesis pretende aportar una base científica para posteriores investigaciones, estableciendo una escala histológica de madurez gonadal que permita validar una escala macroscópica para facilitar el manejo de la obtención de datos sin tener que recurrir al uso de sofisticada maquinaria que actualmente es requerida para este tipo de investigación. El sistema reproductor de las hembras de V. lucetia consta de un par de ovarios, los cuales en su parte anterior están unidos por un solo oviducto y van adoptando una forma tubular en su parte media y posterior, terminando en bordes redondeados. Microscópicamente, los ovarios están conformados por una pared, lamelas y un estroma que los sostiene. El sistema reproductor de los machos consta de un par de testículos, de forma ligeramente laminar, con un túbulo de colección común que se engrosa en estadios maduros, microscópicamente, se diferenció una zona de crecimiento (periférica) y una zona de colección (central), en la primera se presentó los túbulos seminíferos y en la segunda los ductos eferentes. En el desarrollo ovocitario se caracterizó por presentar ovogonias y 5 tipos de ovocitos (inmaduro, pre-vitelogénico, vitelogénico, maduro e hidratado). Además de estructuras como folículos post – ovulatorios y atresia. El desarrollo espermatogénico presentó 4 tipos de células espermatogénicas (espermatogonia, espermatocito, espermátida y espermatozoide). La escala de madurez gonadal para hembras validada microscópicamente consta de 5 estadios (inmaduro, en maduración, maduro, desovante y recuperación) y macroscópicamente de 4 estadios (inmaduro, en maduración, maduro y desovante). Para machos, la escala macroscópica de madurez gonadal validada microscópicamente consta de 4 estadios (inmaduro, en maduración, maduro y expulsante). La proporción sexual de la fracción adulta fue favorable para hembras en el periodo de estudio y la talla de primera madurez gonadal para hembras a considerar es de 52 mm. El IGS promedio mostró un mayor valor para las hembras que para los machos, con un promedio de 5.19 ± 2.76 % y 4.38 ± 2.62 % respectivamente. El FC varió para las hembras de 0.20 - 0.87 %, con un promedio de 0.49 ± 0.09 % y para los machos de 0.28 - 0.87 %, con un promedio de 0.51 ± 0.09 %.

Indirect hydrogen monitoring by chemi-ionisation following an associative ionisation pathway after metastable helium atoms production by electron impact ionisation: Protonation and deuteration of carrier gas

Gas chromatography (GC) is an analytical tool very useful to investigate the composition of gaseous mixtures. However, hydrogen (H2) detection after a GC separation is only possible with a Thermal Conductivity Detector (TCD), a Helium Ionisation Detector (HID) or expensive Atomic Emission Detector (AED). Recently, indirect H2 detection by GC coupled to mass spectrometry (MS) was demonstrated but the mechanism of carrier gas protonation remained unclear. With electron impact as ionisation source of MS and helium (He) as GC carrier gas, H2 is not ionised according the expected Penning ionisation neither according to the Associative ionisation. Rearrangement ionisation (RI) was found to be the main channel for H2 and D2 ionisation under GC-MS conditions used in most of laboratories using GC-MS, leading to the formation of [He−H]+ and [He−D]+ ions.
