A prostatectomia radical (PR) é um dos procedimentos mais utilizados para o tratamento do câncer de próstata (CaP) localizado, porém apesar da maior compreensão da anatomia local e do desenvolvimento tecnológico, esta cirurgia permanece associada à elevada morbidade na esfera sexual. A redução do comprimento peniano após a PR é uma queixa freqüente na prática urológica, porém não há dados na literatura a respeito da variação deste comprimento em um longo período de acompanhamento. A determinação da história natural do comprimento peniano após PR, assim como possíveis fatores de risco ou de proteção é de fundamental importância para o aconselhamento e tratamento dos pacientes submetidos a esta cirurgia. O objetivo deste estudo é determinar a história natural do comprimento peniano após a PR em um acompanhamento de cinco anos, assim como avaliar o papel da função erétil na variação do comprimento peniano destes pacientes. Foram avaliados prospectivamente os comprimentos penianos de 105 pacientes com câncer de próstata localizado submetidos PR aberta. Participação em programas de reabilitação peniana e deformidades anatômicas do pênis foram considerados critérios de exclusão. A medição do comprimento real peniano sob máxima tração (CRTmax) foi realizada antes da PR e aos 3, 6, 12, 24, 36, 48 e 60 meses no pós-operatório. O domínio da função erétil do índice internacional de função erétil (IIEF-EF) foi utilizado para avaliar a função erétil. Houve redução média de 1 cm no CRTmax em 3 meses após a PR e essa diferença permaneceu até 24 meses (p<0,001). Após este período, a diferença reduziu gradativamente, deixando de ser estatisticamente significativa em 48 meses (-0,3 cm, p=0,080) e 60 meses (+0,4 cm, p=0,065). A função erétil foi um preditor para o retorno precoce do comprimento do pênis. Um encurtamento peniano médio de 1 cm é esperado nos primeiros 24 meses após PR. No entanto, há uma tendência para a recuperação deste comprimento após 24 meses de pós-operatório, com retorno ao comprimento original em 48 meses. A função erétil preservada após a PR é um preditor para a recuperação precoce do comprimento do pênis

Ill-health prevails in the workplace. A key problem encountered in the area of stress management is a lack of research into the way job burnout turns into mental problems, especially depressive symptoms, the most prevalent and costly psychiatric condition in the workplace. This research belongs to a cross-discipline area of industrial psychiatry and organizational behavior, which has seldom been investigated before. This research will contribute to the theoretical development of organizational behavior, especially to stress management and industrial psychiatry. This study aims to explore etiological factors and mechanisms of depressive symptoms of workers in the financial industry. By using literature review, semi-structured interviews and surveys as the major research methods, this Ph.D. study systematically investigated the risk factors of workers’ depressive symptoms within and outside of the work area. These risk factors are worker-work environment fits, work family conflicts, and workers’ psychological vulnerabilities to depression. A thorough literature review and 20 semi-structured interviews of brokers in different kinds of financial markets show the feasibility and necessity of this Ph.D. study when it comes to the issue of financial workers’ depressive symptoms. Two surveys of workplace-etiological factors of depressive symptoms were conducted among 244 financial workers and 1024 financial workers. This cross-sample verification showed that worker-work environment fit was a good framework to study risk factors of workers’ depressive symptoms. Results revealed that job demands-abilities misfit could lead to job burnout which in turn contributed to worker’s depressive symptoms; besides this, work effort-reward imbalance could directly cause workers’ depressive symptoms. Emotional labor enhanced the positive effect of job burnout on workers’ depressive symptoms. In the third study, a prominent risk factor outside of the work area, namely work family conflict, and workers’ psychological vulnerabilities of depression were included with workplace etiological factors to investigate the overall predictive model of depressive symptoms of financial workers. The survey was conducted among the same 1024 financial workers. Results indicated that work effort-reward imbalance, job burnout and work interfering in family life were three external etiological factors of workers’ depressive symptoms. Neuroticism, autonomy and low emotional intelligence were three individual etiological factors which had a positive effect on workers’ depressive symptoms. Moreover, neuroticism enhanced the relationship between job burnout and depressive symptoms as well as between work interfering in family life and depressive symptoms. Autonomy aggravated the relationship between job burnout and depressive symptoms. However, emotional intelligence attenuated the relationship between job burnout and depressive symptoms as well as between work effort-reward imbalance and depressive symptoms. Besides, workers’ dysfunctional attitudes played a partial mediating role in the relationships between above etiological factors and depressive symptoms. In the same sample, research evidence of impairments of workers’ depressive symptoms to their work-life quality was also obtained. Specifically, depressive symptoms could predict workers’ presenteeism, absenteeism and turnover intention. Their subjective well-being was also lowered when suffering more severe depressive symptoms. This research provides a theoretical basis to management practices targeted to set up the Employee Assistance Program or even more specilised rehabilitation programs for workers with depressive symptoms so as to improve their work-life quality and and establish a harmonious enterprise.

BACKGROUND: Stroke is one of the most disabling and costly impairments of adulthood in the United States. Stroke patients clearly benefit from intensive inpatient care, but due to the high cost, there is considerable interest in implementing interventions to reduce hospital lengths of stay. Early discharge rehabilitation programs require coordinated, well-organized home-based rehabilitation, yet lack of sufficient information about the home setting impedes successful rehabilitation. This trial examines a multifaceted telerehabilitation (TR) intervention that uses telehealth technology to simultaneously evaluate the home environment, assess the patient's mobility skills, initiate rehabilitative treatment, prescribe exercises tailored for stroke patients and provide periodic goal oriented reassessment, feedback and encouragement. METHODS: We describe an ongoing Phase II, 2-arm, 3-site randomized controlled trial (RCT) that determines primarily the effect of TR on physical function and secondarily the effect on disability, falls-related self-efficacy, and patient satisfaction. Fifty participants with a diagnosis of ischemic or hemorrhagic stroke will be randomly assigned to one of two groups: (a) TR; or (b) Usual Care. The TR intervention uses a combination of three videotaped visits and five telephone calls, an in-home messaging device, and additional telephonic contact as needed over a 3-month study period, to provide a progressive rehabilitative intervention with a treatment goal of safe functional mobility of the individual within an accessible home environment. Dependent variables will be measured at baseline, 3-, and 6-months and analyzed with a linear mixed-effects model across all time points. DISCUSSION: For patients recovering from stroke, the use of TR to provide home assessments and follow-up training in prescribed equipment has the potential to effectively supplement existing home health services, assist transition to home and increase efficiency. This article explores the use of restorative justice as a response to sexual crime. The management of high risk sex offenders, particularly in the community post-release, has been a key focus of contemporary popular and political debates on sexual offending. Many offenders fail to come to the attention of the criminal justice system. For those that do, there is the almost blanket application of recent control in the community measures such as sex offender registries and community notification which have failed to prevent reoffending. The response by the media and the public to the presence of sex offenders in the community may also impede offender rehabilitation. The use of punishment alone via formal criminal justice is, therefore, an inadequate deterrent for sexual crimes. Although controversial, this article advocates the use of restorative practices with sexual crime as a proactive, holistic response to the problem and ultimately as a more effective means of reducing the incidence of sexual offences and sex offender recidivism.

Introdução: A Lesão Medular (LM) é um dos mais devastadores e traumáticos eventos que um Ser Humano pode vivenciar do ponto de vista clínico e emocional, demonstrando-se fundamental a disponibilização de recursos específicos para que o indivíduo possa enfrentar e gerir a sua nova realidade da melhor maneira possível. Alguns estudos têm vindo a demonstrar os benefícios de programas de reabilitação com estimulação elétrica funcional (EEF). Portanto, é de importante relevância perceber os reais efeitos da intervenção na recuperação de indivíduos com este diagnóstico. Objetivo: Analisar as evidências de abordagens de aplicação de correntes de estimulação elétrica funcional (EEF) para coadjuvar na reabilitação em adultos com lesão medular completa. Métodos: Foi conduzida uma pesquisa dos artigos preferencialmente estudos randomized controlled trials RCT´s e estudos quasi-experimentais com os mesmos participantes foram admitidos complementarmente aos experimentais compreendidos entre 2004 e 2013, bem como as citações e as referências bibliográficas de cada estudo nas principais bases de dados de ciências da saúde (Elsevier – Science Direct, Highwire Press, PEDro, PubMed, Scielo Portugal, Clinical Key, B-on, Biomed Central, LILACS- Literatura Latino-Americana e do Caribe em Ciências da Saúde) com as palavras-chave: “spinal cord injuries”, “rehabilitation, electric stimulation funtional”, “FES”, “therapy” em todas as combinações possíveis. Os estudos RCT’s foram analisados independentemente por dois revisores quanto aos critérios de inclusão e qualidade dos estudos. Resultados: Dos 857 estudos identificados apenas sete foram incluídos. Destes, dois apresentaram um score 3/10, um apresentou 4/10, um apresentou um score 5/10. O score total bem como o preenchimento ou não de cada critério encontram-se detalhados na tabela 1 e organizados por ordem alfabética de autores. Todos os estudos incluíram indivíduos com Lesão Medular Completa, idades entre 16 e 68 anos com diagnóstico de acordo com a American Spinal Injury Association (ASIA).Os programas de intervenção dividiram-se em programas de programas de força, densidade mineral óssea, cardiorrespiratório e de atividade física. Dos estudos incluídos, cinco apresentaram melhorias na reabilitação funcional para o grupo experimental, demonstrando assim uma influência positiva da estimulação elétrica funcional em lesões medulares completas. Apenas dois estudos não apresentaram diferenças estatisticamente significativas com relevância clínica. Conclusão: Há uma tendência notória do benefício dos programas com EEF em pacientes com lesões medulares completas parece melhorar a capacidade cardiorrespiratória, a densidade mineral óssea, a força e atividade física, dos indivíduos. Contudo, mais estudos com elevada qualidade metodológica serão essenciais para conceber o real efeito da sua aplicação. Introdução: nos pacientes que sofreram um Acidente Vascular Encefálico são incididos programas de reabilitação que visam principalmente o hemicorpo contralesional, negligenciando o lado ipsilesional. Porém, estão descritas alterações sensitivas e motoras do membro superior ipsilesional neste grupo populacional. Objetivo: avaliar os défices de sensibilidade, destreza grossa e fina e força de preensão da mão ipsilesional em indivíduos com diagnóstico de Acidente Vascular Encefálico, comparando o sexo, o hemisfério cerebral onde se localiza a lesão, a fase aguda e crónica, tendo como referência um grupo controlo. Metodologia: este estudo observacional de carácter analítico transversal foi constituído por 34 indivíduos em que 18 deles tem diagnóstico de Acidente Vascular Encefálico e 16 sem défices neurológicos. A avaliação foi iniciada com um questionário para recolha de informações dos participantes e de seguida foram aplicados instrumentos de avaliação num só momento com o mesmo examinador. Para avaliar a sensibilidade foi usado o Moving Touch-Pressure Test, para a força o Dinamómetro Baseline®, na avaliação da destreza grossa o Teste Caixa e Blocos e por fim a destreza fina com o Purdue Pegboard Test. Estatisticamente recorreu-se ao teste T-student para amostras independentes com nível de significância de 0,05. Resultados: verificou-se que o membro superior ipsilesional dos indivíduos que sofreram um AVE apresenta um défice na força de preensão manual com nível se significância de p=0,001, e de p=0,000 para a destreza fina e grossa e sensibilidade, quando comparados com um grupo controlo. Constatou-se também que no grupo de indivíduos com diagnóstico de Acidente Vascular Encefálico, o sexo masculino obteve melhores resultados em todos os parâmetros avaliados. Observou-se ainda que tanto os homens como as mulheres após sofrerem um Acidente Vascular Encefálico apresentam défices funcionais na mão ipsilesional, mas no entanto os homens apresentam apenas alterações na destreza grossa (p=0,017) e na força (p=0,001). Na comparação dos hemisférios cerebrais lesados e das fases aguda e crónica do AVE verificou-se que não houve diferenças significativas. Conclusão: Existem défices no membro superior ipsilesional após Acidente Vascular Encefálico, sendo então pertinente a sua inclusão em programas de reabilitação, podendo assim melhorar a qualidade de vida dos indivíduos com esta patologia.

People with intellectual disability who sexually offend commonly live in community-based settings since the closing of all institutions across the province of Ontario. Nine (n=9) front line staff who provide support to these individuals in three different settings (treatment setting, transitional setting, residential setting) were interviewed. Participants responded to 47 questions to explore how sex offenders with intellectual disability can be supported in the community to prevent re-offenses. Questions encompassed variables that included staff attitudes, various factors impacting support, structural components of the setting, quality of life and the good life, staff training, staff perspectives on treatment, and understanding of risk management. Three overlapping models that have been supported in the literature were used collectively for the basis of this research: The Good Lives Model (Ward & Gannon, 2006; Ward et al., 2007), the quality of life model (Felce & Perry, 1995), and variables associated with risk management. Results of this research showed how this population is being supported in the community with an emphasis on the following elements: positive and objective staff attitude, teamwork, clear rules and protocols, ongoing supervision, consistency, highly trained staff, and environments that promote quality of life. New concepts arose which suggested that all settings display an unequal balance of upholding human rights and managing risks when supporting this high-risk population. This highlights the need for comprehensive assessments in order to match the offender to the proper setting and supports, using an integration of a Risk, Need, Responsivity model and the Good Lives model for offender rehabilitation and to reduce the likelihood of re-offenses.

La réadaptation pulmonaire est une intervention dont l’efficacité est largement reconnue. Cette efficacité a été établie grâce à l’utilisation d’instruments de mesure d’impact global. Les patients bénéficiant des programmes de réadaptation pulmonaire possèdent des caractéristiques variées et souffrent généralement de maladie pulmonaire obstructive chronique à différents degrés. En fonction de leurs différents besoins, les patients répondent de façon variable aux composantes d’un programme de réadaptation pulmonaire. Il est recommandé d’individualiser les programmes en fonction des besoins des patients afin d’en optimiser les effets. À cette fin, l’évaluation des besoins de réadaptation des patients est nécessaire. Comme il n’existe actuellement aucun instrument standardisé pour procéder à cette évaluation, nous avons entrepris d’en développer un à l’aide de méthodes qualitatives et quantitatives de recherche. Un modèle conceptuel d’évaluation des besoins de réadaptation des patients a été élaboré suite aux résultats tirés de groupes de discussion, de la consultation de dossiers médicaux et d’une recension des écrits. À partir de ce modèle, des items devant être sélectionnés de façon individualisée parmi cinq domaines (reconnaissance des besoins, connaissance, motivation, attentes et buts) ont été pré-testés. Les tendances générales concernant la validité des items contenus dans le prototype d’instrument ont été vérifiées lors d’une étude pilote auprès de 50 répondants en réadaptation. Les pistes d’investigation dégagées dans ce mémoire serviront aux études de validation plus approfondies dont devrait faire l’objet ce prototype d’instrument dans le futur.

Depuis les dernières années, la prévalence de personnes souffrant de dégénérescence des cartilages articulaires, communément appelée ostéoarthrite (OA), ne cesse d’augmenter. Les douleurs articulaires et les raideurs musculaires associées à cette pathologie mènent à des limitations des capacités fonctionnelles, à une perte de mobilité et d’autonomie affectant grandement la qualité de vie de ces personnes. Afin de soulager les personnes souffrant de cette pathologie, l’arthroplastie de la hanche est une procédure chirurgicale fréquemment utilisée. À la suite de cette chirurgie, une amélioration de la qualité de vie et une reprise des capacités fonctionnelles sont souvent observées. Cependant, comparativement à des sujets sains, la vitesse de marche est diminuée, une faiblesse des muscles abducteurs de la hanche est constatée et des mouvements compensatoires au niveau du tronc sont persistants. L’objectif de cette thèse est d’évaluer le patron locomoteur chez des patients qui subiront une arthroplastie de la hanche. Plus spécifiquement, les adaptations locomotrices pré et post-opératoires seront quantifiées dans le but d’apporter des modifications aux programmes de réhabilitation pour ainsi favoriser un patron locomoteur sans déficit. Afin de répondre à cet objectif, trois études distinctes ont été effectuées. Dans le cadre de la première étude, l’impact de l’implantation d’une prothèse totale de la hanche avec une tête fémorale de large diamètre et une prothèse de resurfaçage a été évalué par rapport aux sujets sains lors de la locomotion. Au cours de cette étude, le contrôle du tronc a été analysé en utilisant la distance entre le centre de masse corporel et le centre articulaire de la hanche opérée. Suite aux résultats obtenus, aucune différence majeure n’existe entre les deux types de prothèses en ce qui a trait au contrôle du tronc et ce, à un an post-opératoire. Lors de la deuxième étude, la symétrie des paramètres biomécaniques des membres inférieurs lors de la locomotion chez des patients ayant bénéficié de l’implantation d’une prothèse de la hanche a été caractérisée suite à un programme d’exercices péri-opératoires (pré et post-opératoire). Lors de cette étude, le programme d’exercices péri-opératoires était complémentaire au protocole de réadaptation du centre hospitalier. D’après les résultats obtenus lors de cette étude exploratoire, ce programme d’exercices péri-opératoires semble permettre d’améliorer la symétrie de la puissance et du travail musculaire au niveau de la hanche, du genou et de la cheville favorisant ainsi un patron de marche avec de minimes compensations. Finalement, dans le cadre de la troisième étude, l’approche prédictive et l’approche fonctionnelle, utilisées pour localiser le centre articulaire de la hanche, ont été comparées aux mesures radiographiques, chez des patients à la suite d’un remplacement articulaire de la hanche. À la suite de cette étude, les résultats démontrent que l’utilisation de l’approche fonctionnelle est plus appropriée chez des patients ayant bénéficié d’une arthroplastie de la hanche. En effet, cette approche individualisée est plus précise ce qui, par conséquent, permettra d’obtenir des résultats de plus grande qualité lors d’analyses biomécaniques de la locomotion.

Introduction et recension des écrits : Récemment, les suicides de vétérans et d’athlètes professionnels ont attiré l’attention sur l’association entre le TCC et le suicide. Les lignes directrices concernant la prise en charge en santé mentale dans cette population demeurent fragmentaires. Les objectifs de cette thèse sont de 1) déterminer si une association existe entre le TCC subi dans l’enfance et le suicide futur, 2) explorer si les personnes qui se sont suicidées ont consulté un psychiatre dans l’année précédant le suicide et évaluer si cela diffère selon que la personne ait eu un TCC ou non, 3) décrire et qualifier l’offre québécoise de santé mentale offerte en réadaptation aux enfants et aux adultes ayant subi un TCC. Méthodologie : Le volet épidémiologique consiste en une étude de cohorte rétrospective sur un échantillon de 135 703 enfants ayant reçu des services médicaux au Québec en 1987 et suivis jusqu’en 2008. Le volet qualitatif comprend un sondage auprès des gestionnaires des programmes de réadaptation TCC du Québec, des groupes de discussion avec des cliniciens et des entrevues avec des survivants de TCC et leurs proches. Résultats : Notre étude épidémiologique confirme une association significative entre le TCC subi dans l’enfance (HR 1,49 IC95% 1,04- 2,14), dans l’adolescence (HR 1,57, IC 95% 1,09-2,26) et à l’âge adulte (HR 2,53, IC95% 1,79-3,59) et le suicide. Malgré un risque de suicide plus élevé, les personnes avec un TCC et qui se sont suicidées n’ont pas consulté de psychiatre plus fréquemment que les personnes sans TCC (OR 1,29, IC 95% 0,75- 2,24). Par ailleurs, notre étude qualitative révèle que les forces du système actuel incluent une bonne qualité des services, mais qu’il existe des faiblesses au niveau de l’accès aux médecins spécialisés, du dépistage systématique et de l’accès aux services à long terme. Nos recommandations incluent le développement d’une approche coordonnée en santé mentale, l’implication automatique d’un gestionnaire de cas et l’amélioration des mécanismes d’accès après le congé.

Introducción: La enfermedad pulmonar obstructiva crónica (EPOC), está caracterizada por la limitación del flujo aéreo, de forma progresiva y casi irreversible, asociada a la reacción inflamatoria atribuida a diferentes factores, principalmente a la exposición al humo de tabaco. Es considerada un problema de salud pública en Colombia y en el mundo, con un aumento acelerado de la condición crónica en la actualidad. Objetivo: Identificar las diferencias sociodemográficas, clínicas y de tratamiento, entre los pacientes con diagnóstico clínico y espirométricos de EPOC vs los pacientes con diagnóstico clínico y descartados por espirometría en el Hospital de Suba. Material y Métodos: Estudio observacional, descriptivo, retrospectivo como un componente exploratorio para comparar los grupos con diagnóstico de EPOC clínico y confirmado o descartado por espirometría, entre Enero y Agosto del 2011. Se utilizó estadística descriptiva para calcular las medidas de tendencia central, los datos cuantitativos se expresaron como la media de la variable ± desviación estándar, y los cualitativos como porcentaje, la t de Student para analizar diferencia de las variables cuantitativas de medias entre grupos y la prueba de Pearson para analizar la relación entre los datos cualitativos para aquellos con valores esperados menores a 5 se aplicó test exacto de Fisher, tuvimos en cuenta un α de 0.05 para el análisis bivariado y medidas de asociación. Todos los análisis se realizaron con el paquete estadístico SPSS 19,0 Versión corporativa. Resultados: De los 398 pacientes, solo 287 cumplían con criterios de inclusión. El promedio de edad del total de los pacientes fue de 70,29 + 11,18 años, y 59,5% de la población fue de sexo femenino. Del total de pacientes evaluados, 171 pacientes (59.6%) se descartó el diagnóstico de EPOC (VEF1/ VEC > 0,70). Al comparar los grupos de pacientes a los que se les confirmo el diagnóstico de EPOC contras los descartados por espirometría se encontró que no hay diferencias estadísticamente significativas entre la edad; en los pacientes con EPOC predomino el sexo femenino (p 0.02); en los factores de riesgo existe clara asociación entre EPOC y la exposición a humo de leña (p <0.001), y en cuanto al tabaquismo solo se encontró asociación con ex fumador (p 0,011). Para analizar las diferencias en el tratamiento se estratifico por las posible combinaciones de inhaladores con o sin teofilina, encontrando una diferencia estadísticamente significativa para los tratamientos de tres inhaladores (p 0,015), dos inhaladores + teofilina (p 0,05), tres inhaladores + teofilina (p <0.001), y en los pacientes no tratados (p <0,001).

Objetivo. Analizar el concepto del usuario con EPOC y de los médicos en un hospital público de Bogotá respecto a educación, tratamiento farmacológico y no farmacológico, de acuerdo con la GOLD. Método. Estudio descriptivo cualitativo, exploratorio, desarrollado mediante entrevistas semiestructuradas a 8 médicos y 61 pacientes de consulta externa. Resultados. El 72% de los pacientes no ha recibido explicación acerca del tratamiento, recomendaciones de manejo, ni ha sido interrogado sobre aspectos personales, sociales, o nutricionales. El 70% manifiesta dificultades para acceder a citas médicas y obtener medicamentos. El 90% de los médicos conoce la GOLD; refieren que el tiempo, el volumen de consulta, la disponibilidad de recursos institucionales, el bajo nivel sociocultural, económico y de compromiso de los pacientes dificulta su aplicación. Conclusiones. Existen desventajas en la atención del paciente, ya sea por escasez de recursos, infraestructura, ausencia de programas de prevención, promoción y educación, características del Sistema de Seguridad Social, personales y del entorno.

Occupational therapists are equipped to promote wellbeing through occupation and to enable participation and meaningful engagement of people in their social and physical environments (WFOT, 2012). As such, the role of the occupational therapists is profoundly linked to the social, cultural and environmental characteristics of the contexts in which occupations take place. The central role that context plays in occupational performance creates an interesting dichotomy for the occupational therapist: on one hand, a profound understanding of cultural and social factors is required from the Occupational Therapy (OT) in order to develop a meaningful and successful collaboration with the person; on the other hand, the ability of the occupational therapists to recognize and explore the contextual factor of an occupation-person dyad transcends cultural and spatial barriers. As a result, occupational therapists are equipped to engage in international collaboration and practice, and as such face unique and enriching challenges. International fieldwork experiences have become a tool through which occupational therapists in training can develop the critical skills for understanding the impact of cultural and social factors on occupation. An OT student in an international fieldwork experience faces numerous challenges in leading a process that is both relevant and respectful to the characteristics of the local context: language, cultural perceptions of occupation and personhood, religious backgrounds, health care access, etc. These challenges stand out as ethical considerations that must be considered when navigating an international fieldwork experience (AOTA, 2009). For more than five years now, the Faculty of Rehabilitation Medicine (FRM) of the University of Alberta (UoFA) and the School of Medicine and Health Sciences at the Universidad del Rosario (UR), Bogota, Colombia, have sustained a productive and meaningful international collaboration. This collaboration includes a visit by Dr. Albert Cook, professor of the FRM and former dean, to the UR as the main guest speaker in the International Congress of Technologies for Disability Support (IBERDISCAP) in 2008. Furthermore, Dr. Cook was a speaker in the research seminar of the Assistive Technology Research Group of the Universidad del Rosario. Following Dr. Cook’s visit, Professors Liliana Álvarez and Adriana Ríos travelled to Edmonton and initiated collaboration with the FRM, resulting in the signing of an agreement between the FRM and the UR in 2009, agreement that has been maintained to this day. The main goal of this agreement is to increase academic and cultural cooperation between the UR and the UofA. Other activities have included the cooperation between Dr. Kim Adams (who has largely maintained interest and effort in supporting the capacity building of the UR rehabilitation programs in coordinating the provision of research placement opportunities for UR students at the UofA), an Assistive Technology course for clinicians and students led by Dr. Adams, and a research project that researched the use of basic cell phones to provide social interaction and health information access for people with disabilities in a low-income community in Colombia (led by Tim Barlott, OT, MSc, under the supervision of Dr. Adams). Since the beginning, the occupational therapy programs of the Universidad del Rosario and the University of Alberta have promoted this collaboration and have strived to engage in interactions that provide further development opportunities for students and staff. As part of this process, the international placement experience of UofA OT students was born under the leadership of: Claudia Rozo, OT program director at UR, placement and academic leadership of Elvis Castro and Angélica Monsalve, professors of the occupational therapy program at UR; and Dr. Lili Liu, OT department director at UofA, Cori Schmitz, Academic coordinator of clinical education at the UofA; and Tim Barlott and Liliana Álvarez leading the international and cross-cultural aspect of this collaboration.This publication summarizes and illustrates the process of international placement in community settings in Colombia, undertaken by occupational therapy students of the University of Alberta. It is our hope that this document can provide and document the ethical considerations of international fieldwork experience, the special characteristics of communities and the ways in which cultural and social competences are developed and help international students navigate the international setting. We also hope that this document will stimulate discussion among professional and academic communities about the importance and richness of international placement experiences and encourage staff and students to articulate their daily efforts with the global occupational therapy agenda.
