Transportation and land-use are independent, inter-active systems. Land-use patterns shape local transportation demand, but transportation systems in turn influence land-use patterns. In attempting to satisfy transportation demand created by existing land-use patterns, transportation planners directly, if not always consciously or intentionally, influence future land-use patterns. This study examines that complex relationship. The purpose of the study was threefold: to compile the body of knowledge already existing; to apply this body of knowledge to the context of midsize cities in the Midwest; and, to make the knowledge accessible both to transportation planners and to public officials who make key decisions about land use.

Purpose To examine patient-reported outcome (PRO) in a selected group of Swedish patients about to receive anti-vascular endothelial growth factor (VEGF) treatment for diabetic macular edema (DME). Material and methods In this cross-sectional study, 59 patients with diabetes mellitus, who regularly visited the outpatient eye-clinics, were included. Sociodemographic and clinical data were collected and the patients completed PRO measures before starting anti-VEGF treatment. PRO measures assessed eye-specific outcomes (NEI-VFQ-25) and generic health-related quality of life (SF-36). Results The participants consisted of 30 men and 29 women (mean age, 68.5 years); 54 (92 %) patients had type 2 diabetes; Five (9%) patients had moderate or severe visual impairment; 28 (47 %) were classified as having mild visual impairment. Some of the patients reported overall problems in their daily lives, such as with social relationships, as well as problems with impaired sight as a result of reduced distance vision. Conclusions Further studies are needed to investigate PRO factors related to low perceived general health in this patient population. It is important to increase our understanding of such underlying mechanisms to promote improvements in the quality of patient care.

Tarsal coalition (a congenital fibrous, cartilaginous or bony connection between two bones) often leads to a flatfoot deformity in children. Usually it presents with recurrent ankle sprains or insidious onset of a painful rigid flatfoot and movement limitation of midtarsal and subtalar joints. Clinical diagnosis is confirmed by X-rays, computed axial tomography and nuclear magnetic resonance. The anteater nose sign is caused by a tubular elongation of the anterior process of the calcaneus that approaches or overlaps the tarsal scaphoid (navicular) and resembles the nose of an anteater on a lateral foot or ankle radiograph. The treatment of this union is primarily symptomatic but if the pain persists must be surgical .

Background Childhood cancers are rare and general practitioners (GPs) have limited experience in caring for these children and even less in providing their palliative care. Most families prefer that their child is cared for at home in the palliative phase of their illness, with professional support from those known to them (Chambers and Oakhill 1995, Vickers and Carlisle 2000, Craft and Killen 2007). A community based qualitative study examined the experiences of ten GPs following their involvement in the care of a child with cancer receiving palliative care within the family home. Methods Data collection was through 1:1 in-depth interviews and facilitated case discussion supported by field notes and grounded theory analysis (chronological comparative data analysis identifying generated themes). Social worlds theory was used as a framework to aid examination, and facilitate critical understanding, of the experiences of the GPs. Findings This presentation focuses on five of the findings relating to the experiences of the GP; the impact of minimal contact; lack of knowledge and experience, uncertain role, out of hours service provision and the emotional toll. Findings highlighted that GPs often have to re-establish their role at the child’s transition to palliative care. Factors hindering the GP in this process include a deficit of specialist knowledge and experience of paediatric palliative care and lack of role clarity. Conclusions/points of interest Strategies for enhancing the role of the Macmillan team in supporting GPs have been identified by this study, such as enhanced collaborative working. Findings have also provided further confirmation of the substantial variation in out of hours medical palliative care provision; with evidence that some GPs work beyond their remit in providing informal out of hours care. This presentation details the findings of one aspect (the experiences of GPs) of a wider study that explored the experiences of 54 community based health professionals (GPs, community nurses and allied health professionals) who had been involved in caring for a dying with cancer receiving palliative care at home (Neilson 2009).

O presente trabalho situa-se entre compreender o papel que a comunicação realizada pelo marketing da indústria de beleza desempenha, para o desenvolvimento e crescimento em suas vendas, por meio da influência constituída e disseminada pelos padrões de beleza, impostos pela mídia por meio da visão dos profissionais, que contribuem diretamente em sua atualização e divulgação de informações dessa mesma indústria da beleza. E por outro lado, os consumidores se mantêm informados acerca dos lançamentos de produtos e serviços. Nesta perspectiva, a influência apresentada por meio dos padrões de beleza, que vão sendo constituídos pela indústria de beleza e impostos por seu próprio marketing, mostra-se como o nosso objetivo de investigação, que em nossa pesquisa pretendemos apresentar um estudo a respeito da importância dada à influência desta comunicação do marketing para os profissionais de vários segmentos desta grande área da beleza e a relação com os seus profissionais e consumidores. Levamos em consideração aprioristicamente que a comunicação realizada pelo marketing é um recurso extremamente eficaz, para gerar influência a partir de sua divulgação. Estas informações por sua vez são reproduzidas pelos profissionais, que atuam na área da beleza por meio de seus vários segmentos. Eles constituem o objeto desta pesquisa. Há constatações evidentes da influência da comunicação constituída pelo marketing na indústria da beleza, porém, este trabalho investigativo toma como foco empírico, estes profissionais que estão diretamente ligados e atuantes na área da beleza e são agentes que materializam para os clientes o que há de mais inovador no mercado de produtos e serviços de beleza. Por ambicionar investigar a comunicação via o marketing, os profissionais da indústria da beleza e os clientes, esta investigação possui uma perspectiva transversal, que constitui o nosso objetivo.

The objective of this thesis was to better understand how parental factors influence feeding practices, how mothers experience feeding and what factors mothers perceive influencing feeding in different contexts. This study is largely based on STEPS Study (Steps to Healthy Development of Children), which is a longitudinal cohort of 1797 families. In addition, qualitative data was collected among mothers in Finland and Solomon Islands. The results of this study show that different parental determinants associate with infant and young child feeding behavior and practices. Mothers with high cognitive restraint of eating introduced complementary foods earlier and neophobic mothers’ breastfed exclusively for a shorter time than mothers who ranked lower in these behaviors. Fathers’ poor diet quality associated with shorter total breastfeeding duration. Mothers’ postnatal depressive symptoms associated with shorter duration of exclusive breastfeeding, earlier introduction of complementary foods and lower compliance of feeding recommendations. The higher amount of marital distress associated with longer duration of exclusive breastfeeding and better compliance with feeding recommendations. Mothers, who participated in qualitative interviews, described how complex interplay of individual perceptions, significant others and socio-cultural environment influenced feeding practices and behavior. This study showed that several parental factors influence infant and young child feeding practices as well as compliance with the feeding recommendations. Maternal experiences and perceptions on child feeding relate to the context where mother-infant pair lives in. These results highlight the importance of targeting feeding support and, if needed, specific interventions to mothers and families who are in risk of poor feeding practices.

The first section of this unfinished novel, titled Silk Butterflies is a diptych about a woman named Sarah, and her desire to acquire ancestral truth regarding her identity to negate the pain she feels from losing her unborn child. Her story, told in a guarded, first person point-of-view is paralleled with Ling’s story, an unconventional, ninety-two year old Shanghainese woman who, against her desires, had her feet bound in China during the early 1920’s. Ling’s story is also told from a lyrical first-person perspective that focuses especially on sensory details, and delves into the sacrifices we make to attain standards of beauty, and the loss Ling has never recovered from. As this historical fiction progresses, their stories overlap in an unexpected way, as both Sarah and Ling attempt to revitalize forgotten histories, including how Sarah’s grandparents fled to Shanghai in the 1930’s to escape Nazi persecution during World War II.

Tropical forests have decreased drastically especially in the Peruvian Amazon. In Peru deforestation is caused especially by migrant people; building of houses and infrastructure, clearing land for agricultural purposes and illegal logging and mining. Deforestation results in hindering ecosystem vitality, boosting climate change and decreasing livelihood possibilities. As a counterpoint to cutting down trees there is reforestation, which refers to re-establishment of forest cover. Deforestation and reforestation can be analysed in the light of Forest Transition theory. According to it, due to economic growth, the amount forest cover first diminishes but then starts to increase as the economy in general strengthens. Thus, the research framework is set to this theory. In this study the focus is on analysing socioeconomically sustainable reforestation possibilities in the community of Tingana, Peru. It is situated in a municipal conservation area around which deforestation has been heavy. Land cover change is analysed from LandsatTM satellite images covering a 15 year time period, 1995–2010, in the surroundings of the study area. Semi-structured interviews have been done with a sample size of 25 people and shed light on the perspectives on forests, reforestation and economical activities. The synthesis created from the two methods gives information about the possibilities to enforce reforestation in Tingana and the phase of forest transition in the area. The results show that forest cover has decreased around the surroundings of Tingana leaving the conservation area isolated from larger forest areas. Knowing that forest cover has also decreased inside the conservation area due to agricultural expansion it is certain that fragmentation harms biodiversity causing changes in local climate, which can have knock-on effects for farming and local livelihoods. Therefore reforestation is welcomed when it ensures both conservation and financial benefits and when carried out on locals’ terms. Regarding conservation and incomes the best option would be to plant native timber species together with fruit production species to create agroforestry systems. Economically the community should aim towards an economy that relies on ecotourism as it already practiced in the area. Reforestation could increase ecotourism, which then could in turn increase reforestation via revenues. Regarding forest transition it is likely that forest re-establishment will occur if reforestation along with ecotourism is implemented on long time scale.

Objective: To examine sociodemographic and dental factors for associations with dental sealant placement in children and adolescents aged 6-18 years old. Methods: Secondary data analysis of 2011-2012 NHANES data was conducted. Multiple logistic regression models were used to assess relationships between predictor variables and sealant presence. Results: More than a third (37.1%) of children and adolescents have at least one sealant present; 67.9% of children compared with 40.4% of adolescents. Racial/ethnic differences exist, with Non-Hispanic black youth having the lowest odds of having sealants. Sealant placement odds vary by presence of dental home; the magnitude of the odds varies by age group. Those with untreated decay have lower odds of having sealants than those who do not have untreated decay (child OR: 2.6, 95% CI: 1.83-3.72; adolescent OR: 3.9, 95% CI: 2.59-6.07). Conclusion: Disparities exist in odds of sealant prevalence across racial/ethnic groups, income levels, and dental disease and visit characteristics. Further research is necessary to understand the reasons for these differences and to inform future interventions.

The current study is a post-hoc analysis of data from the original randomized control trial of the Play and Language for Autistic Youngsters (PLAY) Home Consultation program, a parent-mediated, DIR/Floortime based early intervention program for children with ASD (Solomon, Van Egeren, Mahone, Huber, & Zimmerman, 2014). We examined 22 children from the original RCT who received the PLAY program. Children were split into two groups (high and lower functioning) based on the ADOS module administered prior to intervention. Fifteen-minute parent-child video sessions were coded through the use of CHILDES transcription software. Child and maternal language, communicative behaviors, and communicative functions were assessed in the natural language samples both pre- and post-intervention. Results demonstrated significant improvements in both child and maternal behaviors following intervention. There was a significant increase in child verbal and non-verbal initiations and verbal responses in whole group analysis. Total number of utterances, word production, and grammatical complexity all significantly improved when viewed across the whole group of participants; however, lexical growth did not reach significance. Changes in child communicative function were especially noteworthy, and demonstrated a significant increase in social interaction and a significant decrease in non-interactive behaviors. Further, mothers demonstrated an increase in responsiveness to the child’s conversational bids, increased ability to follow the child’s lead, and a decrease in directiveness. When separated for analyses within groups, trends emerged for child and maternal variables, suggesting greater gains in use of communicative function in both high and low groups over changes in linguistic structure. Additional analysis also revealed a significant inverse relationship between maternal responsiveness and child non-interactive behaviors; as mothers became more responsive, children’s non-engagement was decreased. Such changes further suggest that changes in learned skills following PLAY parent training may result in improvements in child social interaction and language abilities.

This chapter will start by providing an overview of current knowledge about young people with learning disabilities who sexually abuse. Research cited will, unless otherwise indicated, be limited to UK studies since international variations in the definitions of both learning disability and sexual abuse make the use of a wider literature base problematic – particularly that relating to prevalence and incidence. It will then go on to report key findings from a recent study (Fyson et al, 2003; Fyson, 2005) which examined how special schools and statutory child protection and youth offending services in four English local authorities responded to sexually inappropriate or abusive behaviours exhibited by young people with learning disabilities. It will conclude by highlighting areas of current practice which give cause for concern, and suggest some pointers for future best practice.
