Abstract Background: Preventable mortality is a good indicator of possible problems to be investigated in the primary prevention chain, making it also a useful tool with which to evaluate health policies particularly public health policies. This study describes inequalities in preventable avoidable mortality in relation to socioeconomic status in small urban areas of thirty three Spanish cities, and analyses their evolution over the course of the periods 1996–2001 and 2002–2007. Methods: We analysed census tracts and all deaths occurring in the population residing in these cities from 1996 to 2007 were taken into account. The causes included in the study were lung cancer, cirrhosis, AIDS/HIV, motor vehicle traffic accidents injuries, suicide and homicide. The census tracts were classified into three groups, according their socioeconomic level. To analyse inequalities in mortality risks between the highest and lowest socioeconomic levels and over different periods, for each city and separating by sex, Poisson regression were used. Results: Preventable avoidable mortality made a significant contribution to general mortality (around 7.5%, higher among men), having decreased over time in men (12.7 in 1996–2001 and 10.9 in 2002–2007), though not so clearly among women (3.3% in 1996–2001 and 2.9% in 2002–2007). It has been observed in men that the risks of death are higher in areas of greater deprivation, and that these excesses have not modified over time. The result in women is different and differences in mortality risks by socioeconomic level could not be established in many cities. Conclusions: Preventable mortality decreased between the 1996–2001 and 2002–2007 periods, more markedly in men than in women. There were socioeconomic inequalities in mortality in most cities analysed, associating a higher risk of death with higher levels of deprivation. Inequalities have remained over the two periods analysed. This study makes it possible to identify those areas where excess preventable mortality was associated with more deprived zones. It is in these deprived zones where actions to reduce and monitor health inequalities should be put into place. Primary healthcare may play an important role in this process. Gold-mining may play an important role in the maintenance of malaria worldwide. Gold-mining, mostly illegal, has significantly expanded in Colombia during the last decade in areas with limited health care and disease prevention. We report a descriptive study that was carried out to determine the malaria prevalence in gold-mining areas of Colombia, using data from the public health surveillance system (National Health Institute) during the period 2010-2013. Gold-mining was more prevalent in the departments of Antioquia, Córdoba, Bolívar, Chocó, Nariño, Cauca, and Valle, which contributed 89.3% (270,753 cases) of the national malaria incidence from 2010-2013 and 31.6% of malaria cases were from mining areas. Mining regions, such as El Bagre, Zaragoza, and Segovia, in Antioquia, Puerto Libertador and Montelíbano, in Córdoba, and Buenaventura, in Valle del Cauca, were the most endemic areas. The annual parasite index (API) correlated with gold production (R2 0.82, p < 0.0001); for every 100 kg of gold produced, the API increased by 0.54 cases per 1,000 inhabitants. Lack of malaria control activities, together with high migration and proliferation of mosquito breeding sites, contribute to malaria in gold-mining regions. Specific control activities must be introduced to control this significant source of malaria in Colombia.

Background. Collagen-induced arthritis (CIA), a murine experimental disease model induced by immunization with type II collagen (CII), is used to evaluate novel therapeutic strategies for rheumatoid arthritis. Adult stem cell marker Musashi-1 (Msi1) plays an important role in regulating the maintenance and differentiation of stem/precursor cells. The objectives of this investigation were to perform a morphological study of the experimental CIA model, evaluate the effect of TNFα-blocker (etanercept) treatment, and determine the immunohistochemical expression of Msi1 protein. Methods. CIA was induced in 50 male DBA1/J mice for analyses of tissue and serum cytokine; clinical and morphological lesions in limbs; and immunohistochemical expression of Msi1. Results. Clinically, TNFα-blocker treatment attenuated CIA on day 32 after immunization (P < 0.001). Msi1 protein expression was significantly higher in joints damaged by CIA than in those with no lesions (P < 0.0001) and was related to the severity of the lesions (Spearman's rho = 0.775, P = 0.0001). Conclusions. Treatment with etanercept attenuates osteoarticular lesions in the murine CIA model. Osteoarticular expression of Msi1 protein is increased in joints with CIA-induced lesion and absent in nonlesioned joints, suggesting that this protein is expressed when the lesion is produced in order to favor tissue repair.

En la actualidad resulta complicado encontrar algún documento (legislación, artículo académico o demanda proveniente de las ONG's) en que los términos coherencia y cooperación para el desarrollo no aparezcan relacionados. Sin embargo ésta concurrencia no era común hasta que a principios de la década de los noventa tanto el Comité de Ayuda al Desarrollo de la OCDE como la Unión Europea incluyeron la coherencia como una de las directrices de las políticas de cooperación para el desarrollo. Precisamente la Unión, y sus diferentes políticas de acción exterior, han sido el objeto de análisis predilecto de aquellos que han tratado de examinar la coherencia. Indudablemente la obligación jurídica, contenida en los artículos C y 130V del Tratado de Maastricht,2 así como el proceso político europeo, complejo y fragmentado, han supuesto un fructífero caldo de cultivo para estos estudios. No obstante irrupciones tan violentas tienen sus costes, descontando algunas notables excepciones3 diversos análisis se desarrollaron sin haber realizado una previa, y necesaria, clarificación del concepto. El previsible resultado ha sido la pluralidad de análisis aplicados a una pluralidad de coherencias. A fin de superar este escollo inicial la primera sección del presente trabajo tratará de definir y establecer una tipología de los análisis de coherencia, es decir ¿Qué es exactamente coherencia? ¿Cómo podemos aplicar este concepto al análisis de políticas?. Posteriormente repasaré los recientes cambios en la política de cooperación para el desarrollo, entre los cuales destaca la inclusión, de forma generalizada, de la coherencia entre las prioridades de los donantes. La cuarta sección tiene como objeto la aplicación de la tipología de coherencia a la política de cooperación para el desarrollo de la Unión Europea. BACKGROUND Most textbooks contains messages relating to health. This profuse information requires analysis with regards to the quality of such information. The objective was to identify the scientific evidence on which the health messages in textbooks are based. METHODS The degree of evidence on which such messages are based was identified and the messages were subsequently classified into three categories: Messages with high, medium or low levels of evidence; Messages with an unknown level of evidence; and Messages with no known evidence. RESULTS 844 messages were studied. Of this total, 61% were classified as messages with an unknown level of evidence. Less than 15% fell into the category where the level of evidence was known and less than 6% were classified as possessing high levels of evidence. More than 70% of the messages relating to "Balanced Diets and Malnutrition", "Food Hygiene", "Tobacco", "Sexual behaviour and AIDS" and "Rest and ergonomics" are based on an unknown level of evidence. "Oral health" registered the highest percentage of messages based on a high level of evidence (37.5%), followed by "Pregnancy and newly born infants" (35%). Of the total, 24.6% are not based on any known evidence. Two of the messages appeared to contravene known evidence. CONCLUSION Many of the messages included in school textbooks are not based on scientific evidence. Standards must be established to facilitate the production of texts that include messages that are based on the best available evidence and which can improve children's health more effectively.

BACKGROUND Although the painful shoulder is one of the most common dysfunctions of the locomotor apparatus, and is frequently treated both at primary healthcare centres and by specialists, little evidence has been reported to support or refute the effectiveness of the treatments most commonly applied. According to the bibliography reviewed, physiotherapy, which is the most common action taken to alleviate this problem, has not yet been proven to be effective, because of the small size of sample groups and the lack of methodological rigor in the papers published on the subject. No reviews have been made to assess the effectiveness of acupuncture in treating this complaint, but in recent years controlled randomised studies have been made and these demonstrate an increasing use of acupuncture to treat pathologies of the soft tissues of the shoulder. In this study, we seek to evaluate the effectiveness of physiotherapy applied jointly with acupuncture, compared with physiotherapy applied with a TENS-placebo, in the treatment of painful shoulder caused by subacromial syndrome (rotator cuff tendinitis and subacromial bursitis). METHODS/DESIGN Randomised controlled multicentre study with blind evaluation by an independent observer and blind, independent analysis. A study will be made of 465 patients referred to the rehabilitation services at participating healthcare centres, belonging to the regional public health systems of Andalusia and Murcia, these patients presenting symptoms of painful shoulder and a diagnosis of subacromial syndrome (rotator cuff tendinitis and subacromial bursitis). The patients will be randomised into two groups: 1) experimental (acupuncture + physiotherapy); 2) control (TENS-placebo + physiotherapy); the administration of rescue medication will also be allowed. The treatment period will have a duration of three weeks. The main result variable will be the change produced on Constant's Shoulder Function Assessment (SFA) Scale; as secondary variables, we will record the changes in diurnal pain intensity on a visual analogue scale (VAS), nocturnal pain intensity on the VAS, doses of non-steroid anti-inflammatory drugs (NSAIDs) taken during the study period, credibility scale for the treatment, degree of improvement perceived by the patient and degree of improvement perceived by the evaluator. A follow up examination will be made at 3, 6 and 12 months after the study period has ended. Two types of population will be considered for analysis: per protocol and per intention to treat. DISCUSSION The discussion will take into account the limitations of the study, together with considerations such as the choice of a simple, safe method to treat this shoulder complaint, the choice of the control group, and the blinding of the patients, evaluators and those responsible for carrying out the final analysis.

INTRODUCTION Familial history of melanoma is a well-known risk factor for the disease, and 7% melanoma patients were reported to have a family history of melanoma. Data relating to the frequency and clinical and pathological characteristics of both familial and non-familial melanoma in Spain have been published, but these only include patients from specific areas of Spain and do not represent the data for the whole of Spain. PATIENTS AND METHODS An observational study conducted by the Spanish Group of Melanoma (GEM) analyzed the family history of patients diagnosed with melanoma between 2011 and 2013 in the dermatology and oncology departments. RESULTS In all, 1047 patients were analyzed, and 69 (6.6%) fulfilled criteria for classical familial melanoma (two or more first-degree relatives diagnosed with melanoma). Taking into account other risk factors for familial melanoma, such as multiple melanoma, pancreatic cancer in the family or second-degree relatives with melanoma, the number of patients fulfilling the criteria increased to 165 (15.8%). Using a univariate analysis, we determined that a Breslow index of less than 1 mm, negative mitosis, multiple melanoma, and a history of sunburns in childhood were more frequent in familial melanoma patients, but a multivariate analysis revealed no differences in any pathological or clinical factor between the two groups. CONCLUSIONS Similar to that observed in other countries, familial melanoma accounts for 6.6% of melanoma diagnoses in Spain. Although no differences in the multivariate analysis were found, some better prognosis factors, such as Breslow index, seem more frequent in familial melanoma, which reflect a better early detection marker and/or a different biological behavior.

Objective: To determine the values of, and study the relationships among, central corneal thickness (CCT), intraocular pressure (IOP), and degree of myopia (DM) in an adult myopic population aged 20 to 40 years in Almeria (southeast Spain). To our knowledge this is first study of this kind in this region. Methods: An observational, descriptive, cross-sectional study was done in which a sample of 310 myopic patients (620 eyes) aged 20 to 40 years was selected by gender- and age-stratified sampling, which was proportionally fixed to the size of the population strata for which a 20% prevalence of myopia, 5% epsilon, and a 95% confidence interval were hypothesized. We studied IOP, CCT, and DM and their relationships by calculating the mean, standard deviation, 95% confidence interval for the mean, median, Fisher’s asymmetry coefficient, range (maximum, minimum), and the Brown-Forsythe’s robust test for each variable (IOP, CCT, and DM). Results: In the adult myopic population of Almeria aged 20 to 40 years (mean of 29.8), the mean overall CCT was 550.12 μm. The corneas of men were thicker than those of women (P = 0.014). CCT was stable as no significant differences were seen in the 20- to 40-year-old subjects’ CCT values. The mean overall IOP was 13.60 mmHg. Men had a higher IOP than women (P = 0.002). Subjects over 30 years (13.83) had a higher IOP than those under 30 (13.38) (P = 0.04). The mean overall DM was −4.18 diopters. Men had less myopia than women (P < 0.001). Myopia was stable in the 20- to 40-year-old study population (P = 0.089). A linear relationship was found between CCT and IOP (R2 = 0.152, P ≤ 0.001). CCT influenced the IOP value by 15.2%. However no linear relationship between DM and IOP, or between CCT and DM, was found. Conclusions: CCT was found to be similar to that reported in other studies in different populations. IOP tends to increase after the age of 30 and is not accounted for by alterations in CCT values.

Objectives: To identify factors that correlate with insulin values and to examine its independent associations among adolescents. Methods: A cross-sectional population-based study was conducted among adolescents aged 12-16,9 years old. A multi-stage stratified cluster random sampling method was employed. Anthropometric measurements and nutritional survey were performed, and fasting blood samples for insulin were obtained. Statistics: Multiple lineal regression. Results: 379 adolescents were included. Mean age was 14.08 ± 1.30 years. Factors associated with higher fasting insulin levels were puberty [ 4.55 (95% IC 0.42-8.69)], abdominal obesity [ 6.11 (95% IC 3.93-8.29)] and to be born small for gestational age (SGA) [ 7.45 (95% IC 2.47-12.44)]. It was observed a negative association between the regular intake of olive oil at home and insulin values [ -4.14 (95% IC -7.31- -0.98)]. Conclusions: Abdominal obesity and SGA were factors associated with higher fasting insulin values. In contrast, the regular intake of olive oil at home was an independent protective factor.

Este trabajo busca identificar los elementos de cambio y continuidad que los ataques terroristas del 11 de septiembre tuvieron en la discusión sobre la redefinición de la arquitectura interamericana de seguridad. En particular, se revisan las posturas de Brasil, Chile, Estados Unidos y México, en tanto que se considera que las posiciones de estos países tendrán un peso importante en la Conferencia Especial sobre Seguridad hemisférica que celebrará la Organización de los Estados Americanos en el 2003. Asimismo, de cara a esta Conferencia, se enumeran algunos temas sobre los que hay un amplio consenso y otros en los cuales se tendrá que seguir trabajando para acordar posiciones comunes. Este análisis concluye que después del 11 de septiembre se generaron importantes cambios en la voluntad de varios países para avanzar en la redefinición de la seguridad hemisférica. Si embargo, esto no se tradujo en cambios importantes sobre las posiciones que los cuatro países analizados tenían sobre algunos temas específicos del debate, ni tampoco sobre el conjunto de fenómenos que consideraban como las principales amenazas a la seguridad en el Continente Americano.

El presente volumen es una nueva contribución del Observatorio de Política Exterior Europea (Obs) al análisis de la interacción entre la política exterior española y la acción exterior de la Unión Europea. En esta ocasión, el lector se encuentra frente a un ejercicio de balance de la Presidencia española de la Unión Europea del primer semestre de 2010, una presidencia única, que sus mismos protagonistas han definido como presidencia de transición. Una transición entre el viejo y el nuevo modelo, impuesto por el Tratado de Lisboa, que entró en vigor poco antes del inicio de la presidencia rotatoria de España. El volumen se compone de más de 20 capítulos, que abordan de manera sistematizada y sucinta los temas fundamentales para entender cuál ha sido el cometido de España a la hora de gestionar el papel de Europa en el mundo. El Obs ya ha realizado este tipo de ejercicio en tres ocasiones desde 2002. De ahí que este “Entre la irrelevancia internacional y el aprendizaje institucional: La presidencia española de la Unión Europea (2010)” deba ser entendido como un balance de la presidencia española al frente de la agenda exterior de la UE, pero también como un referente para los estudiosos de la política europea de España, por un lado, o de la acción exterior de la UE, por el otro, que se suma a la línea de análisis seguida por el Obs a lo largo de casi una década de trabajo.

Con la Europa del 2004 acercándose rápidamente, esta obra no pretende re-explorar el proceso de ampliación una vez más. Este trabajo pretende aportar una perspectiva aproximativa de aquello que le espera a la Unión Europea tras su ampliación. El acento se pone en las distintas oportunidades y desafíos que la acción exterior de la Unión tendrá que afrontar en su futuro ampliado, centrándose especialmente en la UE-25 como una actor en política exterior a la vez que un proveedor de seguridad. El objetivo de los capítulos de la primera parte de este volumen es reflejar cómo se producirá la adaptación de los nuevos miembros a las cuatro áreas de impacto exterior de la UE (PESC, PESD, minorías y asilo), y consecuentemente, de qué modo influirán éstos nuevos actores. En la última parte de la obra, se explora la naturaleza de las nuevas fronteras de la UE-25.

Durante el primer semestre de 2002, España ejerció la Presidencia del Consejo de la Unión Europea, por tercera vez. Las relaciones exteriores de la Unión son cada vez más complejas y amplias. España afrontó esta Presidencia con una agenda, en la que sumaba sus tradicionales áreas de influencia a la gestión continuista de la política exterior de la Unión. En este sentido, el Observatorio de Política Exterior Europea del Institut Universitari d’Estudis Europeus, que se presenta como un grupo de investigadores cuyo principal interés es el análisis de la dimensión exterior de la UE, hace balance en este Quadern de las prioridades y resultados efectivos del programa de la Presidencia.
