This dissertation documents health and illness in the context of daily life circumstances and structural conditions faced by African American families living in Clover Heights (pseudonym), an inner city public housing project in the Third Ward, Houston, Texas. Drawing from Kleinman's (1980) model of culturally defined health care systems and using the holistic-content approach to narrative analysis (Lieblich, Tuval- Mashiach, & Zilber, 1998) the purpose of this research was to explore the ways in which social and health policy, economic mobility, the inner city environment, and cultural beliefs intertwined with African American families' health related ideas, behaviors, and practices. I recruited six families using a convenience sampling method (Schensul, Schensul, & LeCompte, 1999) and followed them for fourteen months (2010–2011). Family was defined as a household unit, or those living in the same residence, short or long-term. Single, African American women ranging in age from 29–80 years headed all families. All but one family included children or grandchildren 18 years of age and younger, or children or other relative 18 years of age and older. I also recruited six residents with who I became acquainted over the course of the project. I collected data using traditional ethnographic methods including participant-observation, archive review, field notes, mapping, free-listing, in-depth interviews, and life history interviews. ^ Doing ethnography afforded the families who participated in this project the freedom to construct their own experiences of health and illness. My role centered on listening to, learning from, and interpreting participants' narratives, exploring similarities and differences within and across families' experiences. As the research progressed, a pattern concerning diagnosis and pharmacotherapy for children's behavioral and emotional problems, particularly attention-deficit hyperactivity disorder (ADHD) and pediatric bipolar disorder (PBD), emerged from my formal interactions with participants and my informal interactions with residents. The findings presented in this dissertation document this pattern, focusing on how mothers and families interpreted, organized, and ascribed meaning to their experiences of ADHD and PBD. ^ In the first manuscript presented here, I documented three mothers' narrative constructions of a child's diagnosis with and pharmacotherapy for ADHD or PBD. Using Gergen's (1997) relational perspective I argued that mothers' knowledge and experiences of ADHD and PBD were not individually constructed, but were linguistically and discursively constituted through various social interactions and relationships, including family, spirituality and faith, community norms, and expert systems of knowledge. Mothers' narratives revealed the complexity of children's behavioral and emotional problems, the daily trials of living through these problems, how they coped with adversity and developed survival strategies, and how they interacted with various institutional authorities involved in evaluating, diagnosing, and encouraging pharmaceutical intervention for children's behavior. The findings highlight the ways in which mothers' social interactions and relationships introduced a scientific language and discourse for explaining children's behavior as mental illness, the discordances between expert systems of knowledge and mothers' understandings, and how discordances reflected mothers' ‘microsources of power’ for producing their own stories and experiences. ^ In the second manuscript presented here, I documented the ways in which structural factors, including gender, race/ethnicity, and socioeconomic status, coupled with a unique cultural and social standpoint (Collins, 1990/2009) influenced the strategies this group of African American mothers employed to understand and respond to ADHD or PBD. The most salient themes related to mother-child relationships coalesced around mothers' beliefs about the etiology of ADHD and PBD, ‘conceptualizing responsibility,’ and ‘protection-survival.’ The findings suggest that even though mothers' strategies varied, they were in pursuit of a common goal. Mothers' challenged the status quo, addressing children's behavioral and emotional problems in the ways that made the most sense to them, specifically protecting their children from further marginalization in society more so than believing these were the best options for their children.^

La mayor parte delas personas que conviven con algún tipo de enfermedad, tienden a adoptar un mayor número de hábitos saludables, pudiendo crear nuevas maneras de ver la vida y a sí mismos. El objetivo de este estudio fue verificar la relación entre índices de calidad de vida y percepción de la imagen corporal de los pacientes incluidos en un programa de rehabilitación cardiovascular en Florianópolis-Brasil. La muestra estuvo compuesta por 24 sujetos varones con una edad de 62 ± 1,3 años, portadores de Enfermedad Arterial Coronaria. Para evaluar la calidad de vida, se utilizó el cuestionario Minnesota Living With Heart Failure Questionnaire (MLHFQ) y para identificar el grado de descontento de la muestra con la imagen corporal, se aplicó el cuestionario de Stunkard y Sorensen (1993). El análisis de las pruebas fue hecho a través de un programa de estadísticas utilizando para tal fin, el software SPSS 11.0. El grado de asociación entre variables fue estudiado a partir del test de Kendall. Se verificó que cuanto mayor es el IMC y la silueta actual, mayor el grado de insatisfacción con la imagen corporal. Los síntomas emocionales también parecen estar correlacionados significativamente con un deseo de obtener una menor silueta corporal y con indicadores de menor calidad de vida (r= 0,474 y r= 0,735; p mayor 0,05). Los síntomas físicostambién se encuentran correlacionados de manera significativa con los síntomas emocionales. Estos resultados sugieren que las variables referentes a la calidad de vida poseen un peso significativo en la imagen corporal y la satisfacción con ésta, parece correlacionar con una menor cantidad de problemas emocionales y en un mejor afrontamiento de la enfermedad. Los programas de rehabilitación cardiovascular que implementan actividad física en los hábitos diarios se muestran adecuados como herramienta para la mejora de dichas dolencias en esta fase post aguda

Enquadramento: Embora o ciclo menstrual seja um fenômeno normal na vida de mulheres em idade fértil, a ele podem se associar síndromes de intensidade variável. Objectivo: Determinar o impacto da síndrome pré-menstrual e da síndrome disfórica pré-menstrual na qualidade de vida de mulheres em idade fértil, comparadas a mulheres sem essas síndromes. Material e Métodos: Procedeu-se a estudo quantitativo, descritivo, correlacional, transversal, com 60 participantes do sexo feminino (16 com síndrome pré-menstrual, 28 com sintomatologia compatível com SDPM e 16 não apresentavam sintomas compatíveis com diagnóstico), A colheita de dados foi no Ambulatório de Ginecologia do Hospital Militar de nível terciário no período de janeiro a fevereiro de 2015, apresentando idade entre 18 e 56 anos, menarca há mais de um ano; ciclos menstruais em ausência de uso de terapia de reposição hormonal, com queixas associadas ao período menstrual. Três instrumentos de recolha de dados foram empregados: um questionário de identificação da paciente e caracterização de vida reprodutiva; o Questionário de Triagem de Sintomas Pré-menstruais e o Short Form Health-Survey com 36 itens. Para organização e análise dos dados, foi empregado o programa Statistical Package for Social Sciences, na versão 21.0. As variáveis foram dicotomizadas, para análise de contingência com o teste exato de Fisher. Para comparação de médias, foram utilizados o teste de diferença de médias de amostras independentes e o teste Kruskal Wallis, todos em nível de significância de 0,05. Resultados: A prevalência de síndrome menstrual igualou-se a 76,3%, sendo 46,6% compatível com síndrome disfórica e 29,7% com síndrome pré-menstrual. Esses grupos diferiram significantemente nos sintomas de ansiedade/tensão, fadiga/perda de energia, dificuldade de concentração, fome excessiva ou anorexia e sentimento de opressão. A síndrome disfórica pré-menstrual associou-se a maior comprometimento de relações sociais, familiares e domésticas, bem como à perda de saúde geral e de saúde mental, nos domínios de vitalidade, funções sociais, limitações devidas a problemas emocionais e saúde mental. Essas alterações não interferiram na percepção da libido e de orgasmo, mas alguns domínios da qualidade de vida estiveram associados a características reprodutivas. Conclusão: As síndromes pré-menstruais comprometem a qualidade de vida das utentes e requerem assistência voltada à melhoria do conhecimento das mulheres sobre seu problema de saúde. Este trabalho tem como tema “Crianças em Situação de Risco Social: Perceção dos Docentes. Um contributo para a proteção e promoção dos direitos das crianças” e tem como objetivo tentar conhecer a frequência de alunos do Agrupamento de Escolas do Crato em situação de risco social, bem como os tipos de risco que poderão estar a viver. O Agrupamento de Escolas do Crato é constituído por duas escolas que são elas: a Escola Básica Integrada com Jardim de Infância Professora Ana Maria Ferreira Gordo e a Escola Básica 1 de Gáfete. Neste agrupamento encontra-se em funcionamento o jardim-de-infância, o 1º. ciclo, 2º. ciclo e o 3º. ciclo. A amostra é constituída pelos docentes do 1.º, 2.º e 3.º ciclos do ensino básico, os quais responderam a um questionário por cada aluno, num total de 232 alunos, ou seja, todos os alunos a frequentar o 1.º ou o 2.º ou o 3.ºciclos do ensino básico, excluindo-se apenas as crianças do pré-escolar, abrangendo-se assim toda a população do ensino básico. O instrumento utilizado neste estudo quantitativo foi adaptado do questionário de Díaz-Aguado e Arias (1999) validado na Universidade de Complutense de Madrid, e permitiu identificar vários tipos de risco social (maltrato ativo, problemas emocionais, negligência e condutas anti-sociais) bem como caraterizar algumas das variáveis que se associam a estes problemas no concelho do Crato. Algumas das principais conclusões foram as seguintes: as crianças que eram percecionadas como estando em maior risco social frequentavam o 3.º Ciclo, eram beneficiárias de ação social escolar e tinham mais insucesso escolar.

Background. Previous research has found links between being a victim of bullying and reporting more unhealthy eating behaviours and cognitions, particularly in girls. However, little is known about the factors that might mediate these relationships. Aim. The present study compared the relationships between bullying, emotional adjustment, restrained eating, and body dissatisfaction in adolescent boys and girls. Sample/method. Self-report data were collected from a sample of 11- to 14-year-olds (N= 376) on experiences of bullying, emotional symptoms, and unhealthy eating and shape-related attitudes and behaviours. Results. Bullying, emotional symptoms, restrained eating, and body dissatisfaction were all correlated. Emotional symptoms were found to significantly mediate the relationships between verbal bullying with body dissatisfaction in girls but not in boys. Conclusions. Findings suggest that the experience of being verbally bullied places adolescent girls at risk of developing emotional problems which can then lead to body dissatisfaction. Longitudinal research is necessary to disentangle these pathways in more detail to facilitate the development of informed interventions to support children who are being bullied.

During the past few years there has been a drastic shortage of registered nurses in the field. The shortage appears to have affected the field of psychiatric mental health nursing most intensely. The psychiatric nursing shortage is a multifaceted problem grounded in decreasing federal funds for advanced clinical training, inadequate undergraduate psychiatric experiences, lack of a well prepared articulate role model, the integrated curriculum and the confusion and blurring associated with the roles and functions of the psychiatric mental health nurse.^ This dissertation will describe the current nursing shortage; the decline in enrollment to nursing programs; the history of psychiatric nursing as a discipline; the shortage of psychiatric mental health nurses; factors contributing to the psychiatric nursing shortage and a plan for a solution to the nursing shortage in psychiatry.^ The paper focuses on an evaluation conducted on an internship curriculum designed to facilitate effective nursing care in the treatment of clients who exhibit emotional problems. The purpose of this study was to attract and retain nurses to employment opportunities in four Hospital Corporation of America (HCA) facilities, using a six week internship program.^ The study will yield an analysis of the effect of combining psychodynamic principles and knowledge with skills in the clinical area. The demands of educational practice have been merged with the discipline of psychiatric nursing in the development of this curriculum. ^

The current study was designed to explore the salience of social support, immigrant status, and risk in middle childhood and early adolescence across two time periods as indicated by measures of school adjustment and well-being. Participants included 691 children of public elementary schools in grades 4 and 6 who were interviewed in 1997 (Time 1) and reinterviewed two years later (Time 2); 539 were U.S.-born, and 152 were foreign-born. ^ Repeated measures multivariate analyses of variance (MANOVA's) were conducted to assess the effects of immigrant status and risk on total support, well-being, and school adjustment from Time 1 to Time 2. Follow-up analyses, including Student-Newman-Keuls post hoc tests, were used to test the significance of the differences among the means of support categories (low and high), immigrant status (U.S. born and non-U.S. born), risk (low and high) and time (time 1 and time 2). ^ Results showed that immigrant participants in the high risk group reported significantly lower levels of support than their peers. Further, children of low risk at Time 2 indicated the highest levels of support. Second, immigrant preadolescents, preadolescents who reported low levels of social support, and preadolescents of the high risk reported lower levels of emotional well-being. There was also an interaction of support by risk by time, indicating that children who are at risk and had low levels of social support reported more emotional problems at Time 1. Finally, preadolescents who are at risk and preadolescents who reported lower levels of support were more likely to show school adaptation problems. Findings from this study highlight the importance of a multivariable approach to the study of support, emotional adjustment, and academic adjustment of immigrant preadolescents. ^

Aim: This study is going to assess the prevalance of prolonged grief diagnoses and it will evaluate the severity of the symptoms of depression, anxiety and complicated grief two months after a loved one is lost. We also intend to study which variables associated with the risk of grief could be more decisive when diagnosing it, its symptoms and the consequent emotional distress.Method: A total of 66 families of patients in the Palliative Care Unit (PCU) at Hospital San Cecilio in Granada have been evaluated. Measurements were taken two months after the death. This investigation has explored the existing emotional distress using the following questionnaires: Beck Depression Inventory (BDI-II), Beck Anxiety Inventory (BAI), Inventory of Complicated Grief (ICG) and Prolongued Grief Disorder (PG-12).Results: The results show that 33.3% and 21.21% of the sufferers had high levels of depression and clinical anxiety two months after the death. The prevalence of prolongued grief diagnoses, according to the PG-12, is 10.6% and 53.03% of the participants showed symptoms of complicated grief according to the ICG. Additionally, statistically significant differences are found in the sufferers with and without a prolongued grief diagnosis and scores in the ICG and BDI-II. The family’s financial situation is linked to the presence of symptoms of anxiety and depression and complicated grief, with the most determining variable being the risk of grief. Finally, the greater the age of the deceased and the longer the time spent in the PCU is linked to fewer symptoms of grief. However, important links have been found between the sufferers who have experienced stressful critical events prior to losing their loved one, with symptoms of depression, anxiety and complicated grief.Conclusions: The high numbers of cases of symptoms of complicated grief and levels of anxiety and clinical depression two months after a death suggests that early interventions should be carried out in those individuals with greater vulnerability.

Background: Although it is well-established that children with language impairment (LI) and children with autism spectrum disorders (ASD) both show elevated levels of emotional and behavioural problems, the level and types of difficulties across the two groups have not previously been directly compared. Aims: To compare levels of emotional and behavioural problems in children with LI and children with ASD recruited from the same mainstream schools. Methods & Procedures: We measured teacher-reported emotional and behavioural problems using the Strengths and Difficulties Questionnaire (SDQ) in a sample of 5-to-13-year old children with LI (N=62) and children with ASD (N=42) attending mainstream school but with identified special educational needs. Outcomes & Results: Both groups showed similarly elevated levels of emotional, conduct and hyperactivity problems. The only differences between the LI and ASD groups were on subscales assessing peer problems (which were higher in the ASD group) and prosocial behaviours (which were higher in the LI group). Overall, there were few associations between emotional and behavioural problems and child characteristics, reflecting the pervasive nature of these difficulties in children with LI and children with ASD, although levels of problems were higher in children with ASD with lower language ability. However, in the ASD group only, a measure of family social economic status was associated with language ability and attenuated the association between language ability and emotional and behavioural problems. Conclusions & Implications: Children with LI and children with ASD in mainstream school show similarly elevated levels of emotional and behavioural problems, which require monitoring and may benefit from intervention. Further work is required to identify the child, family and situational factors that place children with LI and children with ASD at risk of emotional and behavioural problems, and whether these differ between the two groups. This work can then guide the application of evidence-based interventions to these children.
