Background Estimates of the disease burden due to multiple risk factors can show the potential gain from combined preventive measures. But few such investigations have been attempted, and none on a global scale. Our aim was to estimate the potential health benefits from removal of multiple major risk factors. Methods We assessed the burden of disease and injury attributable to the joint effects of 20 selected leading risk factors in 14 epidemiological subregions of the world. We estimated population attributable fractions, defined as the proportional reduction in disease or mortality that would occur if exposure to a risk factor were reduced to an alternative level, from data for risk factor prevalence and hazard size. For every disease, we estimated joint population attributable fractions, for multiple risk factors, by age and sex, from the direct contributions of individual risk factors. To obtain the direct hazards, we reviewed publications and re-analysed cohort data to account for that part of hazard that is mediated through other risks. Results Globally, an estimated 47% of premature deaths and 39% of total disease burden in 2000 resulted from the joint effects of the risk factors considered. These risks caused a substantial proportion of important diseases, including diarrhoea (92%-94%), lower respiratory infections (55-62%), lung cancer (72%), chronic obstructive pulmonary disease (60%), ischaemic heart disease (83-89%), and stroke (70-76%). Removal of these risks would have increased global healthy life expectancy by 9.3 years (17%) ranging from 4.4 years (6%) in the developed countries of the western Pacific to 16.1 years (43%) in parts of sub-Saharan Africa. Interpretation Removal of major risk factors would not only increase healthy life expectancy in every region, but also reduce some of the differences between regions, The potential for disease prevention and health gain from tackling major known risks simultaneously would be substantial.

Agriculture in limited resource areas is characterized by small farms which an generally too small to adequately support the needs of an average farm family. The farming operation can be described as a low input cropping system with the main energy source being manual labor, draught animals and in some areas hand tractors. These farming systems are the most important contributor to the national economy of many developing countries. The role of tillage is similar in dryland agricultural systems in both the high input (HICS) and low input cropping systems (LICS), however, wet cultivation or puddling is unique to lowland rice-based systems in low input cropping systems. Evidence suggest that tillage may result in marginal increases in crop yield in the short term, however, in the longer term it may be neutral or give rise to yield decreases associated with soil structural degradation. On marginal soils, tillage may be required to prepare suitable seedbeds or to release adequate Nitrogen through mineralization, but in the longer term, however, tillage reduces soil organic matter content, increases soil erodibility and the emission of greenhouse gases. Tillage in low input cropping systems involves a very large proportion of the population and any changes: in current practices such as increased mechanization will have a large social impact such as increased unemployment and increasing feminization of poverty, as mechanization may actually reduce jobs for women. Rapid mechanization is likely to result in failures, but slower change, accompanied by measures to provide alternative rural employment, might be beneficial. Agriculture in limited resource areas must produce the food and fiber needs of their community, and its future depends on the development of sustainable tillage/cropping systems that are suitable for the soil and climatic conditions. These should be based on sound biophysical principles and meet the needs of and he acceptable to the farming communities. Some of the principle requirements for a sustainable system includes the maintenance of soil health, an increase in the rain water use efficiency of the system, increased use of fertilizer and the prevention of erosion. The maintenance of crop residues on the surface is paramount for meeting these requirements, and the competing use of crop residues must be met from other sources. These requirements can be met within a zonal tillage system combined with suitable agroforestry, which will reduce the need for crop residues. It is, however, essential that farmers participate in the development of any new technologies to ensure adoption of the new system. (C) 2001 Elsevier Science B.V. All rights reserved.

Objective: To review the outcome of acute liver failure (ALF) and the effect of liver transplantation in children in Australia. Methodology: A retrospective review was conducted of all paediatric patients referred with acute liver failure between 1985 and 2000 to the Queensland Liver Transplant Service, a paediatric liver transplant centre based at the Royal Children's Hospital, Brisbane, that is one of three paediatric transplant centres in Australia. Results: Twenty-six patients were referred with ALF. Four patients did not require transplantation and recovered with medical therapy while two were excluded because of irreversible neurological changes and died. Of the 20 patients considered for transplant, three refused for social and/or religious reasons, with 17 patients listed for transplantation. One patient recovered spontaneously and one died before receiving a transplant. There were 15 transplants of which 40% (6/15) were < 2 years old. Sixty-seven per cent (10/15) survived > 1 month after transplantation. Forty per cent (6/15) survived more than 6 months after transplant. There were only four long-term survivors after transplant for ALF (27%). Overall, 27% (6/22) of patients referred with ALF survived. Of the 16 patients that died, 44% (7/16) were from neurological causes. Most of these were from cerebral oedema but two patients transplanted for valproate hepatotoxicity died from neurological disease despite good graft function. Conclusions: Irreversible neurological disease remains a major cause of death in children with ALF. We recommend better patient selection and early referral and transfer to a transplant centre before onset of irreversible neurological disease to optimize outcome of children transplanted for ALF.

OBJECTIVE - To assess the effect of age on glucose metabolism by examining 1) glucose metabolism in young and middle-aged subjects when total or regional adiposity is taken into account and 2) in vitro glucose transport in adipose tissue explants from young and middle-aged women paired for total and abdominal adiposity. RESEARCH DESIGN AND METHODS - Study 1: body composition, subcutaneous abdominal and visceral adipose tissue areas, and fasting and oral glucose-stimulated glucose and insulin were measured in 84 young and 81 middle-aged men and in 110 young and 91 middle-aged women. Study 2: glucose uptake in subcutaneous abdominal and visceral adipose tissue explants were measured in eight young and eight middle-aged women. RESULTS - Study 1: young and middle-aged men showed similar subcutaneous abdominal tissue area, whereas fat mass and visceral adipose tissue were greater in middle-aged than in young men (P < 0.01). Fat mass and subcutaneous and visceral adipose tissue areas were greater in middle-aged as compared with young women (P < 0.01). Fasting plasma glucose and the glucose response to an oral glucose tolerance test were significantly higher in middle-aged than in young men and women (P < 0.001). Statistical control for visceral adipose tissue area eliminated the difference seen in glucose response in men and women. Study 2: glucose transport in subcutaneous and omental adipose tissue did not differ between young and middle-aged women. CONCLUSIONS - 1) Visceral obesity, more than age per se, correlates with glucose intolerance in middle-aged subjects; 2) aging does not influence in vitro adipose tissue glucose uptake.

The left ventricular response to dobutamine may be quantified using tissue Doppler measurement of myocardial velocity or displacement or 3-dimensional echocardiography to measure ventricular volume and ejection fraction. This study sought to explore the accuracy of these methods for predicting segmental and global responses to therapy. Standard dobutamine and 3-dimensional echocardiography were performed in 92 consecutive patients with abnormal left ventricular function at rest. Recovery of function was defined by comparison with follow-up echocardiography at rest 5 months later. Segments that showed improved regional function at follow-up showed a higher increment in peak tissue Doppler velocity with dobutamine therapy than in nonviable segments (1.2 +/- 0.4 vs 0.3 +/- 0.2 cm/s, p = 0.001). Similarly, patients who showed a > 5% improvement of ejection fraction at follow-up showed a greater displacement response to dobutamine (6.9 +/- 3.2 vs 2.1 +/- 2.3 mm, p = 0.001), as well as a higher rate of ejection fraction, response to dobutamine (9 +/- 3% vs 2 +/- 2%, p = 0.001). The optimal cutoff values for predicting subsequent recovery of function at rest were an increment of peak velocity > 1 cm/s, >5 mm of displacement, and a >5% improvement of ejection fraction with low-dose dobutamine. Recent studies have revealed marked regional variation in pyramidal cell morphology in primate cortex. In particular, pyramidal cells in human and macaque prefrontal cortex (PFC) are considerably more spinous than those in other cortical regions. PFC pyramidal cells in the New World marmoset monkey, however, are less spinous than those in man and macaques. Taken together, these data suggest that the pyramidal cell has become more branched and more spinous during the evolution of PFC in only some primate lineages. This specialization may be of fundamental importance in determining the cognitive styles of the different species. However, these data are preliminary, with only one New World and two Old World species having been studied. Moreover, the marmoset data were obtained from different cases. In the present study we investigated PFC pyramidal cells in another New World monkey, the owl monkey, to extend the basis for comparison. As in the New World marmoset monkey, prefrontal pyramidal cells in owl monkeys have relatively few spines. These species differences appear to reflect variation in the extent to which PFC circuitry has become specialized during evolution. Highly complex pyramidal cells in PFC appear not to have been a feature of a common prosimian ancestor, but have evolved with the dramatic expansion of PFC in some anthropoid lineages.

The branching structure of neurones is thought to influence patterns of connectivity and how inputs are integrated within the arbor. Recent studies have revealed a remarkable degree of variation in the branching structure of pyramidal cells in the cerebral cortex of diurnal primates, suggesting regional specialization in neuronal function. Such specialization in pyramidal cell structure may be important for various aspects of visual function, such as object recognition and color processing. To better understand the functional role of regional variation in the pyramidal cell phenotype in visual processing, we determined the complexity of the dendritic branching pattern of pyramidal cells in visual cortex of the nocturnal New World owl monkey. We used the fractal dilation method to quantify the branching structure of pyramidal cells in the primary visual area (V1), the second visual area (V2) and the caudal and rostral subdivisions of inferotemporal cortex (ITc and ITr, respectively), which are often associated with color processing. We found that, as in diurnal monkeys, there was a trend for cells of increasing fractal dimension with progression through these cortical areas. The increasing complexity paralleled a trend for increasing symmetry. That we found a similar trend in both diurnal and nocturnal monkeys suggests that it was a feature of a common anthropoid ancestor.

Objective: Current prevalence of smoking, even where data are available, is a poor proxy for cumulative hazards of smoking, which depend on several factors including the age at which smoking began, duration of smoking, number of cigarettes smoked per day, degree of inhalation, and cigarette characteristics such as tar and nicotine content or filter type. Methods: We extended the Peto-Lopez smoking impact ratio method to estimate accumulated hazards of smoking for different regions of the world. Lung cancer mortality data were obtained from the Global Burden of Disease mortality database. The American Cancer Society Cancer Prevention Study, phase 11 (CPS-II) with follow up for the years 1982 to 1988 was the reference population. For the global application of the method, never-smoker lung cancer mortality rates were chosen based on the estimated use of coal for household energy in each region. Results: Men in industrialised countries of Europe, North America, and the Western Pacific had the largest accumulated hazards of smoking. Young and middle age males in many regions of the developing world also had large smoking risks. The accumulated hazards of smoking for women were highest in North America followed by Europe. Conclusions: In the absence of detailed data on smoking prevalence and history, lung cancer mortality provides a robust indicator of the accumulated hazards of smoking. These hazards in developing countries are currently more concentrated among young and middle aged males.

Desde o in??cio de 2000, a Canada School of Public Service (CSPS) e a Escola Nacional de Administra????o P??blica (ENAP) demonstraram interesse na possibilidade de coopera????o para desenvolver capacidades de gest??o de servidores p??blicos brasileiros. Um acordo assinado em 2003, inicialmente foi planejado para ser uma parceria de tr??s anos entre as duas escolas. No entanto, a parceria continua at?? hoje e agora estamos no final do segundo, e ??ltimo projeto de coopera????o t??cnica financiado pela Canadian International Development Agency (CIDA) entre Brasil e Canad??, denominado ???Desenvolvimento da Capacidade de Governan??a???. Este relat??rio representa o resultado deste segundo projeto, o qual ampliou o esfor??o de capacita????o da primeira parceria, focando agora em funcion??rios p??blicos mais pr??ximos da "linha de frente" da presta????o de servi??os, aumentando assim a efic??cia do governo nessas ??reas. O projeto mostrou ser bem-sucedido, principalmente, porque seguiu de perto os princ??pios do desenvolvimento eficaz e sustent??vel. Considerando a sua miss??o de desenvolver compet??ncias de servidores p??blicos, a ENAP deu prioridade, dentro deste projeto de coopera????o com o Canad??, a atividades que visam refor??ar a capacidade de governan??a do pa??s, trabalhando em conjunto com escolas em cinco regi??es: Norte, Nordeste, Centro-Oeste, Sudeste e Sul, com especial ??nfase nas ??reas mais pobres do pa??s, localizadas nas regi??es Norte e Nordeste. Foram participantes-chave neste projeto: i) seis escolas brasileiras regionais de administra????o p??blica; ii) uma das principais institui????es acad??micas do Canad?? - Queen's University, e iii) o Minist??rio do Desenvolvimento Social e Combate ?? Fome e tr??s Secretarias de Ra??a, Direitos Humanos e Pol??ticas para as Mulheres. Neste projeto, a CSPS compartilhou suas metodologias em educa????o a dist??ncia, lideran??a e gest??o p??blica. Trabalhando com parceiros brasileiros, ela tamb??m transferiu e adaptou curr??culos e conte??dos de seus cursos. A CIDA forneceu $1.845.000,00 por meio da Programa de Interc??mbio de Conhecimentos para a Promo????o da Eq??idade (PIPE) Brasil-Canad??. A ENAP e as EPBs contribu??ram com $993,752 e a CSPS contribuiu com material de curso, conhecimento e apoio t??cnico no valor de $ 1.000.000

Reconhecendo a import??ncia estrat??gica dessa regi??o detentora de inestim??veis patrim??nios em s??cio e biodiversidades, o Governo Federal declara, em 2003, a Amaz??nia uma prioridade para o combate ??s desigualdades e ao desenvolvimento socioecon??mico, articulando, desde ent??o, f??runs com minist??rios e institui????es regionais, embasados no Programa de Desenvolvimento Sustent??vel da Amaz??nia. Com o objetivo de implementar a????es para produ????o de saber cient??fico voltado ?? realidade local e para a integra????o da pesquisa b??sica e aplicada, ensino e tecnologia, incentiva-se a coopera????o com outras institui????es afins regionais, nacionais e internacionais. Uma dessas a????es resultou, em 2004, na formaliza????o do Acordo Multilateral de Coopera????o T??cnico-Cient??fica em Sa??de das Institui????es da Amaz??nia, cuja proposta baseia-se na constitui????o de uma rede de pesquisa, forma????o de recursos humanos, coopera????o t??cnica com gestores do SUS e coopera????o internacional em sa??de, objetivando desenvolver atividades de investiga????o conjunta para conhecer as realidades s??cios sanit??rias e epidemiol??gicas da regi??o e implementar respostas do sistema de sa??de e de ci??ncia, tecnologia e inova????o.

Este artigo procura explicar as raz??es do colapso na gest??o do sistema de planejamento regional no Brasil, debru??ando-se sobre a extin????o da Sudene no final da d??cada de 1990, como estudo de caso. Para tanto, utiliza-se o modelo de sa??da, voz e lealdade de Hirschman, para destacar o funcionamento dos mecanismos promotores do slack organizacional nessa ag??ncia de governo, e sua subsequente extin????o. Assim, sustenta-se o argumento de que a implementa????o de modelo federativo fortemente descentralizado, a partir da Constitui????o de 1988, produziu disfun????es operacionais em uma organiza????o dotada de estrutura decis??ria colegiada, deslegitimando-a, e conduzindo-a ?? extin????o.

Analisa como a questão do professor se apresenta na produção científica brasileira. Toma como base o discurso apresentado na SBPC, por, entre outros fatores, ser originário de entidade que congrega cientistas de todas as áreas de conhecimento e ser representativo da produção docente e discente de graduação e pós-graduação das várias regiões e instituições do país. Usa como metodologia a abordagem histórico-documental. Utiliza como fontes os resumos publicados nos anais de 2001 e da década de 1980. Os resultados evidenciam: 1) um aumento extraordinário do número de trabalhos sobre o professor, a permanência da origem institucional (universidade pública) e territorial (Sudeste); 2) alteração do predomínio do enfoque temático, da formação do professor, em nível superior e médio, para a prática pedagógica exercida no cotidiano escolar do ensino fundamental; 3) alteração no enfoque metodológico, passando dos estudos exploratório-descritivos para a pesquisa-ação crítica voltada para a intervenção no cotidiano escolar do ensino fundamental. Conclui pela negação dos espaços/tempos da produção científica sobre o professor, visto que os espaços/tempos são ações de sujeitos históricos, que exibem operações de troca, intercâmbios, compartilhamentos coletivos e não a determinação do "lugar próprio" do pesquisador e/ou da "autoria marcada". Os discursos expressos pareceram, cada um, ocupar um "lugar próprio" e isolado, não permitindo a sua acepção como conjunto da obra sobre a questão do professor, não se vislumbrando uma tessitura temática coletiva, com gênese nos espaços/tempos da academia em sua relação com a realidade educacional e social do Brasil.
