We report a human case of polycystic hidatidosis due to Echinococcus vogeli from Contamana (Department of Loreto) village located in the central jungle of Peru. The patient is a 44 year-old lady, teacher, who carried a painless liver mass since a year ago. She was submitted to abdominal surgery and the liver mass was removed and showed multiple cysts containing colorless liquid as is showed in the polycystic hidatidosis. The morphology and measure of the hooks obtained from the liquid contained in the cysts are from Echinococcus vogeli. It is the first report of this parasitism in Perú.

The purpose of the present study is to compare the tomato juice agar, a well known medium employed to observe ascospore formation, with niger seed agar, casein agar and sunflower seed agar, applied to a differentiation between C. dubliniensis and C. albicans. After 48 hours of incubation at 30 ºC all 26 (100%) C. dubliniensis isolates tested produced chlamydospores on tomato juice agar as well as in the other three media evaluated. However, when we inoculated all media with C. albicans, the absence of chlamydospores became resulting in the following percents: tomato juice agar (92.47%), niger seed agar (96.7%), casein agar (91.39%), and sunflower seed agar (96.7%). These results indicate that tomato juice agar is another medium which can also be used in the first phenotypic differentiation between C. dubliniensis and C. albicans.

Candida dubliniensis is an opportunistic yeast that has been recovered from several body sites in many populations; it is most often recovered from the oral cavities of human immunodeficiency virus-infected patients. Although extensive studies on epidemiology and phylogeny of C. dubliniensis have been performed, little is known about virulence factors such as exoenzymatic and hemolytic activities. In this study we compared proteinase, hyaluronidase, chondroitin sulphatase and hemolytic activities in 18 C. dubliniensis and 30 C. albicans strains isolated from AIDS patients. C. albicans isolates produced higher amounts of proteinase than C. dubliniensis (p < 0.05). All the tested C. dubliniensis strains expressed hyaluronidase and chondroitin sulphatase activities, but none of them were significantly different from those observed with C. albicans (p > 0.05). Hemolytic activity was affected by CaCl2; when this component was absent, we did not notice any significant difference between C. albicans and C. dubliniensis hemolytic activities. On the contrary, when we added 2.5 g% CaCl2, the hemolytic activity was reduced on C. dubliniensis and stimulated on C. albicans tested strains (p < 0.05).

OBJECTIVES: 1) To determine trends in prevalence of neural tube defects and the impact of therapeutic abortion. 2) To review perinatal management of spina bifida. DESIGN: All spontaneous and therapeutic abortions, still births and live births affected by neural tube defects registered in Alfredo da Costa Maternity in Lisbon, from 1983 to 1992, were retrospectively analysed. RESULTS: Eighty-two cases with neural tube defects are reported and myelomeningocele and anencephaly++ were the most frequent ones. Total prevalence for all defects was 0.78:1000 births with a small upward trend during the last two years. Birth prevalence was 0.6:1000, with a clear downward trend, due to therapeutic abortion. Prenatal diagnosis improved significantly, from 9% of all defects detected in 1983-87 to 77.5% in 1988-92. Since 1989, all cases of anencephaly were detected before birth. Most cases of spina bifida were vaginally delivered, and elective cesarean section occurred in 4. Early closure of the defect was undertaken in 87.6% of the newborns with open spina bifida. CONCLUSION: While total prevalence of neural tube defects remained stable, with only a small upward trend, prenatal diagnosis and therapeutic abortion resulted in a 56.3% fall in birth prevalence. Optimal management of open spina bifida demands a multidisciplinary team with an individual program for each case.

SUMMARY Sporothrix schenckiiwas reclassified as a complex encompassing six cryptic species, which calls for the reassessment of clinical and epidemiological data of these new species. We evaluated the susceptibility of Sporothrix albicans (n = 1) , S. brasiliensis (n = 6) , S. globosa (n = 1), S. mexicana(n = 1) and S. schenckii(n = 36) to terbinafine (TRB) alone and in combination with itraconazole (ITZ), ketoconazole (KTZ), and voriconazole (VRZ) by a checkerboard microdilution method and determined the enzymatic profile of these species with the API-ZYM kit. Most interactions were additive (27.5%, 32.5% and 5%) or indifferent (70%, 50% and 52.5%) for TRB+KTZ, TRB+ITZ and TRB+VRZ, respectively. Antagonisms were observed in 42.5% of isolates for the TRB+VRZ combination. Based on enzymatic profiling, the Sporothrix schenckii strains were categorized into 14 biotypes. Leucine arylamidase (LA) activity was observed only for S. albicans and S. mexicana. The species S. globosaand S. mexicanawere the only species without β-glucosidase (GS) activity. Our results may contribute to a better understanding of virulence and resistance among species of the genus Sporothrixin further studies.

Os dados sobre diarreia por rotavírus em Portugal são limitados. Este estudo teve como objectivo estimar a proporção de gastroenterite aguda por este vírus em crianças observadas em serviços de urgência de vários hospitais do país e analisar as suas características clínicas e moleculares. Estudo prospectivo, multicêntrico, observacional, incluindo crianças como menos de 5 anos, com gastroenterite aguda, observadas em 10 serviços de urgência pediátricos, entre outubro de 2008 e setembro de 2009. Foram recolhidos dados demográfico e clínicos. as amostras positivas de rotavírus foram genotipadas por reacção em cadeia da polimerase. Foram incluídas 1846 crianças, 58% do sexo masculino, com idade média de 19,3 +- 14,4 meses. Foi identificado rotavírus nas fezes em 28,3% (intervalo de confiança 95%, 26,2-30,4%), com maior proporção no inverno e na primavera e em crianças com idade de 7-24 meses. Os genótipos mais frequentes foram G4P(8) (46%) e G1P(8) (37%), com variações de norte para sul. As crianças com gastroenterite por rotavírus tinham probabilidade significativamente superior (p<0,001) de ter febre, vómitos, perda de peso, desidratação e necessidade de internamento, comparativamente aos casos negativos para rotavírus. A gastroenterite aguda por rotavírus em crianças portuguesas com idade inferior a 5 anos associou-se a maior morbilidade e hospitalização do que nos casos sem identificação de rotavírus. Houve diferenças importantes na distribuição dos genótipos entre as regiões. Na era das vacinas contra o rotavírus, este conhecimento é importante para as decisões relativas à prevenção da doença e para monitorizar tendências da epidemiologia molecular do rotavírus.

Avaliou-se a capacidade do ágar suco de tomate (ágar V8) em diferenciar Candida dubliniensis de Candida albicans com base na produção de clamidoconídios. Noventa e três isolados de Candida albicans e vinte e seis de Candida dubliniensis foram incluídos; 100% de Candida dubliniensis formaram clamidoconídios e 92,5% de Candida albicans não evidenciaram estas estruturas. Estes resultados permitem sugerir este meio como recurso alternativo na identificação presuntiva de Candida dubliniensis.
