O sucesso educativo dos alunos é como facilmente se compreenderá, a essência do ato educativo nas suas múltiplas dimensões e formas. O percurso para a obtenção desse sucesso é variado e representa muitas vezes um enorme investimento das organizações educativas nomeadamente na gestão dos seus recursos humanos. Por outro lado, o modelo organizativo instituído pode ser o fator preponderante para a obtenção do sucesso procurado sobretudo quando ele é promotor de um clima harmonioso e motivador para o ato educativo. O projeto apresentado neste estudo estruturou-se numa nova construção dos grupos de trabalho, em que o conceito tradicional de turma foi substituído por grupos, constituídos por alunos cujo grau de conhecimento e competências no âmbito das disciplinas de Português e Matemática, fosse o mais aproximado possível não perdendo de vista o princípio de heterogeneidade do grupo mesmo que relativa ou diminuída, tão importante para promover a diversidade. Este sistema organizativo rompeu com o conceito tradicional da turma o que implicou uma alteração não só na organização de horários escolares dos alunos e docentes mas também obrigou estruturação e implementação de um processo de mobilidades dos alunos entre grupos em função do seu grau de aprendizagens ou de dificuldades na sua obtenção. Este projeto foi ainda promotor de uma cultura de trabalho colaborativo e partilha entre docentes de cada grupo disciplinar promovendo a sistematização de interações entre pares quer na preparação das atividades letivas quer na discussão e resolução dos constrangimentos que o desenvolvimento do projeto evidenciou.

O presente relatório de qualificação profissional, como requisito (parcial) para obtenção do grau de mestre em Educação Pré-Escolar e Ensino do 1.º Ciclo do Ensino Básico, é o reflexo dos saberes e competências profissionais adquiridas pelas experiências vividas durante a prática pedagógica supervisionada, desenvolvida em dois contextos de estágio: Educação Pré- Escolar e 1.º Ciclo do Ensino Básico. Ao longo desta formação inicial a mestranda adquiriu saberes científicos, pedagógicos, didáticos, culturais e de investigação que lhe possibilitaram o desenvolvimento de práticas sustentadas. A interação entre os saberes teóricos mobilizados e a reflexão sobre a prática, possibilitaram a construção de um quadro teórico próprio, com visão e pensamento pessoal, permitindo à futura profissional de educação melhorar e transformar a prática educativa, agindo nos contextos de forma adequada, visando a inclusão e equidade. Através da adoção de uma postura crítica, reflexiva e indagadora, apoiada nos processos da metodologia de investigação-ação – observação, planificação, ação, reflexão e avaliação – a mestranda desenvolveu a práxis de forma cíclica e articulada. Com efeito, a mestranda procurou desenvolver práticas que promovessem aprendizagens significativas e efetivas para as diferentes crianças e que lhes permitissem ser construtoras ativas do seu próprio conhecimento, numa perspetiva holística da educação e do desenvolvimento da criança, encarando-a como centro do processo de ensino e aprendizagem. Em conformidade, os estágios desenvolvidos em ambos os contextos permitiram a construção de um perfil generalista, através do desenvolvimento de competências marcadas por uma (re)construção pessoal e profissional, auxiliadas por processos colaborativos e reflexivos.

RESUMO - A profissão de enfermagem é definida por Virgínia Henderson (1966), como tendo por objeto "ajudar o indivíduo, saudável ou doente, na execução das atividades que contribuem para conservar a sua saúde ou a sua recuperação, de tal maneira, devendo desempenhar esta função no sentido de tornar o indivíduo o mais independente possível, ou seja, a alcançar a sua anterior independência". A atenção à saúde ao longo da história da humanidade vem sendo desenvolvida de muitos modos e por diversos atores sociais. O trabalho em saúde é essencial para a vida humana e envolve atividades realizadas por profissionais e equipas multidisciplinares que dominam os conhecimentos e técnicas essenciais para assistir o indivíduo com problemas de saúde ou em risco de adoecer, em atividades de investigação, de prevenção, curativo e de reabilitação. À Enfermagem tal como a outras profissões, são cada vez mais exigidos requisitos de eficiência, eficácia e polivalência na sua atividade, tendo a motivação um papel fundamental na atitude, postura e desempenho do profissional. Este trabalho descreve o estudo da motivação profissional, numa população de enfermeiros portugueses numa instituição privada. A versão adaptada de um inquérito como instrumento de autopreenchimento representa vários construtos implicados no processo motivacional, em contexto profissional. Pretendo com este estudo identificar a dimensão da motivação mais relevantes no processo motivacional dos enfermeiros, assim como as suas variações face a variáveis de características pessoais, profissionais e institucionais. A partir dos resultados obtidos pretendo ainda apresentar sugestões de maneira a oferecer oportunidade de transformação do seu ambiente organizacional em instituições similares.

RESUMO: O presente estudo, enquadra-se, no domínio do exercício profissional dos enfermeiros e na formação em serviço destes profissionais e teve por objectivo avaliar, num grupo de enfermeiros de um serviço de medicina, o impacto de uma intervenção educativa na melhoria dos procedimentos a executar na prática da terapêutica inalatória. A amostra do estudo (amostra por conveniência) foi constituída por oito enfermeiros do serviço de medicina do Hospital Curry Cabral (idades compreendidas entre os 24 a 43 anos de idade), sem anterior formação específica na área da terapêutica inalatória, mas com experiencia profissional prévia com doentes respiratórios. O estudo decorreu em dois momentos de avaliação: avaliação 0 (antes da intervenção educativa) e avaliação 1 (após a intervenção educativa). Entre estes dois momentos, foi ministrada, a todos os enfermeiros, uma acção de formação sobre terapêutica inalatória, com componente teórico-prática. Antes e depois da intervenção educativa foram aplicados dois instrumentos: para avaliação dos conhecimentos teóricos, construi-se e aplicou-se, um questionário de auto-avaliação de conhecimentos sobre terapêutica inalatória ao grupo dos enfermeiros, e para avaliação dos conhecimentos práticos e execução de procedimentos, uma grelha de observação da demonstração da técnica de inalação com três dispositivos inalatórios: MDI, MDI com câmara expansora ACE e DPI turbohaler®. Os resultados do estudo mostraram que o grupo dos enfermeiros da nossa amostra detinha já alguns conhecimentos de carácter teórico, de âmbito geral, sobre a terapêutica inalatória, mas conhecimentos pouco sólidos, nomeadamente no que concerne aos fármacos utilizados por via inalatória. Verificou-se, ainda, um deficiente domínio de conteúdos de carácter mais específico e prático sobre terapêutica inalatória, por exemplo a importância de determinados procedimentos - agitação do inalador, tempo de pausa entre cada inalação, cuidados de manutenção da câmara expansora, higienização da cavidade bucal após a inalação dos corticosteróides. Quanto à observação da técnica de inalação, o grupo dos enfermeiros revelou algumas lacunas, quer relativamente à técnica de inalação com MDI, quer do MDI com câmara expansora, observando-se a execução de passos incorrectos e mesmo omissão de passos por parte dos enfermeiros, antes da intervenção educativa.Não nos foi possível realizar a observação da técnica de inalação com dispositivo de pó seco turbohaler®, dado que nenhum dos enfermeiros da nossa amostra se sentiu capaz de manusear de forma correcta este dispositivo, antes e após a formação. Em termos globais, achamos que a formação dada sobre terapêutica inalatória, mostrou melhorar os conhecimentos teóricos e práticos dos enfermeiros neste âmbito. Esta melhoria ficou claramente demonstrada no aumento do número de respostas correctas dadas pelos enfermeiros no questionário efectuado, após a formação. Do mesmo modo, após a formação, registou-se uma melhoria considerável dos conhecimentos específicos e práticos, no que concerne à importância de determinados procedimentos na prática da terapêutica inalatória - agitação do inalador, tempo de pausa entre cada inalação, cuidados de manutenção da câmara expansora, higienização da cavidade bucal após a inalação dos corticosteróides - observando-se, um maior número de respostas correctas assinaladas no questionário. Em contrapartida, os conhecimentos sobre fármacos administrados por via inalatória, ficaram um pouco aquém das expectativas, pois o número de respostas correctas dadas pelos enfermeiros após a formação não foi significativo. Poderemos mesmo dizer que, a acção de formação com maior grau de especificidade sobre fármacos não se revelou eficaz e gerou até alguma ―confusão‖ neste grupo de enfermeiros, parecendo que os conhecimentos anteriores sobre fármacos por via inalatória não estariam consolidados antes da formação. De igual modo, após a formação, registou-se no grupo dos enfermeiros do estudo uma melhoria na performance da técnica de inalação com o MDI e MDI com a câmara expansora. Os enfermeiros do estudo, não só corrigiram os passos ou itens de avaliação da técnica inalatória, anteriormente executados incorrectamente, mas também, mostraram um melhor conhecimento dos passos ou itens de avaliação da técnica de inalação, com menor número de passos ou itens de avaliação omissos, após a formação. Em contrapartida, a acção de formação, no que diz respeito ao dispositivo de pó seco turbohaler®, não cumpriu o seu objectivo, dado que nenhum dos enfermeiros da amostra foi capaz de demonstrar a técnica de inalação com este dispositivo, antes e após a formação. Poderemos dizer que, não sendo um dispositivo da sua prática clínica, os enfermeiros não sentiram necessidade de adquirir estes conhecimentos. Em síntese, podemos concluir que na sua globalidade, a formação dada (com informação actualizada sobre as orientações mais adequadas neste domínio) e o treino formal dos enfermeiros neste âmbito, foi bastante proveitoso, uma vez que, mostrou melhorar os conhecimentos teóricos e práticos na prática da terapêutica inalatória do grupo de enfermeiros doserviço de medicina. Realça-se, no entanto, a importância de uma formação adequada, sentida como útil pelos enfermeiros, faseada, ligada à prática dos profissionais e alternando com momentos de exercício profissional. Os conhecimentos e os procedimentos dos enfermeiros deverão ainda ser monitorizados e avaliados ao longo do tempo, para a detecção de eventuais desvios que a rotina pode introduzir e para levantamento de novas necessidades de formação.-------------------ABSTRACT: This study fits in the domain of in-service training of professional nurses. It was aimed to assess the impact of an educational intervention and the consequent improvement of the practice of inhalation therapy in a group of nurses from a medical service The study sample (convenience sample) was consituted by eight nurses from the medical service, Hospital Curry Cabral (aged 24-43 years) without previous special training in the field of inhalation therapy, but with prior experience with patients with respiratory diseases. The study ran in two moments of evaluation: evaluation 0 (before the educational intervention) and evaluation 1 (post-intervention). Between these two moments, a training program about therapeutic inhalation was administered to all nurses, with a double component of theory and practice. Before and after the educational intervention two evaluation tools were applied: for the assessment of theoretical knowledge, the nurses had to fill a self-assessment questionnaire and in order to evaluate their knowledge related to procedural skills as well as their performance, they were observed (and their behaviour recorded on a check-list) during a demonstration of three inhalation techniques with devices: metered dose inhaler, metered dose inhaler with a spacer device ACE® and device turbohaler® The study results showed that the group of nurses in our sample had already some knowledge of theoretical nature (general scope of the inhalation therapy) but little solid knowledge, particularly in regard to drugs used in inhalation. In fact, a relatively weaker knowledge was registerd in what concerns specific and practical knowledge about inhalation therapy, for example, the importance of certain procedures - shaking the inhaler, pause between each inhalation, maintenance care of spacer device, mouth cavity hygiene after inhalation of corticosteroids. As for the observation of the inhaler technique, it was found that the group of nurses had a poor inhalation technique, in what concerns inhalation technique with MDI or MDI with spacer device, with a performance with several incorrect steps, or even omission of steps by nurses before the educational intervention. Finally, as the observation technique of the dry powder device turbohaler ®, we were unable to analyze the observation concerning this device, since none of the nurses in our sample, wast able to handlle it correctly, before and after training.In general terms, we found that the training given on inhalation therapy had a positive impact either in the theorical or the pratical knowledge on inhalation therapy (higher number of correct answers after training). Similarly, after training, there was the considerable improvement of specific and practical skills, namely the importance of certain procedures in the practice of inhalation therapy - shaking the inhaler, pause time between each inhalation, maintenance care of spacer device, sanitizing the buccal cavity after inhalation of corticosteroids. In contrast, knowledge about drugs administered by inhalation, were slightly below expectations, showing a lower number of correct answers given by the nurses after training. The training seemed to be a factor of "confusion" for this group of nurses, whose prior knowledge in this domais was not probably very solid to begin with. After training, the group of nurses in the study improved the performance of inhalation technique with MDI and MDI with spacer device. They not only correcty performed the steps or itens for the assessment of inhalation technique, previously performed incorrectly, but also showed a better understanding of the steps or itens for assessing the inhalation technique, with fewer steps missing (after training). In contrast, training with regard to the device turbohaler ® dry powder, was also below expectations, given that none of the nurses in the sample was able to demonstrate inhaler technique with this device before and after training. This was probably due to the fact that, the nurses did not feel the need to acquire this knowledge and the related practice. In summary, we can say that, overall, the training (with updated information on the appropriate policies in this field) showed an improvement in knowledge and performance in the practice of inhalation therapy. It is however crucial to underline the importance of in-service adequate training programmes, perceived as useful by the nurses, developped in different phases, linked to the nurses’ practice and combining with professional practices. The nurses’ knowledge and skills should also be further monitorized and evaluated in order to detect deviations introduced by the rotinization of procedures and to identify new training needs.

O presente estudo pretende descrever o programa de intervenção, baseado no desenvolvimento de conhe‑ cimentos e competências em educadores de infância relacionados com a alimentação, crescimento, desen‑ volvimento e adesão a novos alimentos na primeira infância. Os educadores de infância das creches (18 no total) participaram no programa de intervenção, que ocorreu entre dezembro de 2013 e fevereiro de 2014, acreditada pelo Ministério da Educação, Conselho Científico-Pedagógico da Formação Contínua. Globalmente, a oficina de formação constituiu-se como um espaço de reflexão, análise e exploração de temáticas relacionadas com os estilos de vida na primeira infância, num total de 25 horas de formação presencias e 25 horas de experimentação no contexto de trabalho das formandas. Procurámos centrar os momentos formativos numa pedagogia baseada na partilha, expo‑ sição de perspetivas singulares entre pares, colaboração e iniciativa conjunta. Explorámos as expetativas das formandas relativamente à oficina de formação e as suas representações sobre a alimentação em crianças. A experiência terminou, mas com um sentido expectável de continuidade, pois deixou marcas em todos os que participaram. As implicações para a prática foram testemunhadas por todas as formandas.

O presente conjunto de investigações pretendeu estudar o envolvimento parental na competição desportiva de crianças e jovens. Baseado no modelo do envolvimento parental no desporto (Teques & Serpa, 2009), o estudo permitiu concretizar dois objectivos fundamentais. Primeiro, desenvolver um conjunto de escalas válidas e fidedignas para aceder aos constructos incluídos no modelo teórico. Segundo, testar as hipóteses fundamentadas na estrutura conceptual do modelo com o propósito de compreender (1) a razão porque os pais se envolvem no desporto dos filhos, (2) quais os comportamentos utilizados pelos pais durante o envolvimento, e (3) como é que o envolvimento influencia o contexto de realização do jovem atleta. No total, participaram voluntariamente 1620 pais e 1665 jovens atletas de vários desportos individuais e coletivos, com idades compreendidas entre os 9 e os 18 anos. A prossecução dos objectivos teve por base uma série de três estudos independentes. Os resultados do primeiro estudo sugerem que as crenças do papel parental, a auto-eficácia, a perceção das invocações oriundas do treinador e do jovem atleta, o tempo e energia disponíveis, e os conhecimentos e competências relacionam-se com as atividades de envolvimento dos pais. No segundo estudo, os resultados demonstraram que as perceções dos comportamentos parentais de encorajamento, reforço, instrução, e modelagem medeiam a relação entre os comportamentos reportados pelos pais e as variáveis psicológicas de auto-eficácia, auto-eficácia social, motivação intrínseca, e estratégias de autorregulação dos jovens. Os resultados do terceiro estudo indicam que as perceções dos comportamentos dos pais relacionam-se com a realização desportiva através dos efeitos de mediação da auto-eficácia, autoeficácia social e das estratégias de autorregulação. Implicações para a intervenção, limitações e direções futuras para a investigação são também discutidas.

El presente trabajo pretende implantar un proyecto de formación ambiental y, a la vez, crear el marco físico y social para que así sea. De esta forma, se darán a conocer y mejorarán los conocimientos ambientales en la población del municipio de Concepción Las Minas, con el fin de que puedan mejorar sus técnicas a la hora de gestionar su territorio y, a la vez, ampliar sus conocimientos ambientales, útiles en el campo de la educación. Para realizar estas capacitaciones fue necesaria una descripción del territorio junto con un análisis de las principales problemáticas ambientales municipales. Se realizaron sondeos a la población para valorar su nivel de conocimientos ambientales. Con toda la información recogida, se planificaron una serie de sesiones formativas destinadas a maestros, niños y técnicos ambientales. Las sesiones trataban de formar y a la vez concienciar sobre temas generadores tales como el recurso hídrico, forestal, gestión de residuos, etc. Estas capacitaciones se dieron tanto a nivel teórico como a nivel práctico. La valoración global de las capacitaciones es positiva. Cabe destacar un alto grado de participación por parte de los maestros y los alumnos de las escuelas municipales, mostrando una gran implicación en las distintas sesiones. Contrariamente, no se obtuvo el mismo resultado con el colectivo de técnicos municipales, ya que faltó tiempo y planificación.

L'évolution des connaissances et des compétences en matière d'évaluation et de gestion du risque de violence a permis une indiscutable avancée. Néanmoins les instruments utilisés restent encore source de controverses et prêtent le flanc à de nombreuses critiques. Dans ce contexte le climat sécuritaire actuel fait peser une lourde pression sur les équipes soignantes qui ont aussi à faire face à la violence des patients. Une réflexion sur les possibilités et les limites autant du soin que de la privation de liberté permet, peut-être d'envisager ce qu'est un risque acceptable. Evolution of knowledge and skills in violence risk management and assessment allowed an indisputable progress. Nevertheless instruments used such as HCR-20 still remain source of controversy and gives occurrence to numerous criticisms. In this context, the current high security climate puts added pressure on the medical teams which also have to deal with the violence of patients. A reflection on the possibilities and the limits of care as well as freedom deprivation may allow us to envision what is an acceptable risk.

En el període 2006-2008, l'equip investigador de l'Observatori sobre la Didàctica de les Arts (ODAS) s'ha concentrat en la innovació i la investigació en el camp de la didàctica aplicada als estudis universitaris de les arts. Des del punt de vista propi de la investigació-acció, la finalitat ha estat la de plantejar i explorar una revisió integral de l'organització del treball a peu d'aula que fos extensible a d'altres assignatures i matèries. Així, prenent com a eix l'aprenentatge dels estudiants, les accions escomeses es fonamentaren en sis premisses: integració —de sessions de treball i activitats d'aprenentatge—, diversitat —d'escenaris, recursos i materials didàctics—, equilibri —entre coneixements i habilitats específiques i transversals—, modularitat —de les parts constitutives de la innovació proposada—, aplicabilitat —a d'altres assignatures i matèries— i progressió en la seva posada en marxa. I sobre aquestes bases, hom va establir cinc línies de treball: l'organització del treball a l'aula i del treball guiat de l'estudiant en diferents tipus de sessions, l'organització del treball autònom de l'alumne des dels pressupòsits d'una avaluació continuada, la incorporació de les TIC com autèntics recursos d'ensenyament-aprenentatge, la col·laboració amb d'altres unitats de la Universitat de Barcelona que tinguessin entre els seus objectius l'impuls de l'aprenentatge, i el seguiment del procés d'implantació de la iniciativa didàctica i l'anàlisi regular dels seus resultats. Pel que fa a aquest darrer punt, en aquest primer període, hom ha prioritzat l'estudi de les dades quantitatives i quasi-quantitatives derivades del judici dels alumnes que participaren en la nova proposta didàctica. En conseqüència, la investigació va acomplir una primera funció diagnòstica de la innovació docent duta a terme, i es va enquadrar en la categoria dels estudis descriptius transversals a través d'enquestes amb mostres probabilístiques.

It gives me great pleasure to accept the invitation to address this conference on “Meeting the Challenges of Cultural Diversity in the Irish Healthcare Sector” which is being organised by the Irish Health Services Management Institute in partnership with the National Consultative Committee on Racism and Interculturalism. The conference provides an important opportunity to develop our knowledge and understanding of the issues surrounding cultural diversity in the health sector from the twin perspectives of patients and staff. Cultural diversity has over recent years become an increasingly visible aspect of Irish society bringing with it both opportunities and challenges. It holds out great possibilities for the enrichment of all who live in Ireland but it also challenges us to adapt creatively to the changes required to realise this potential and to ensure that the experience is a positive one for all concerned but particularly for those in the minority ethnic groups. In the last number of years in particular, the focus has tended to be on people coming to this country either as refugees, asylum seekers or economic migrants. Government figures estimate that as many as 340,000 immigrants are expected in the next six years. However ethnic and cultural diversity are not new phenomena in Ireland. Travellers have a long history as an indigenous minority group in Ireland with a strong culture and identity of their own. The changing experience and dynamics of their relationship with the wider society and its institutions over time can, I think, provide some valuable lessons for us as we seek to address the more numerous and complex issues of cultural diversity which have arisen for us in the last decade. Turning more specifically to the health sector which is the focus of this conference, culture and identity have particular relevance to health service policy and provision in that The first requirement is that we in the health service acknowledge cultural diversity and the differences in behaviours and in the less obvious areas of values and beliefs that this often implies. Only by acknowledging these differences in a respectful way and informing ourselves of them can we address them. Our equality legislation – The Employment Equality Act, 1998 and the Equal Status Act, 2000 – prohibits discrimination on nine grounds including race and membership of the Traveller community. The Equal Status Act prohibits discrimination on an individual basis in relation to the nine grounds while for groups it provides for the promotion of equality of opportunity. The Act applies to the provision of services including health services. I will speak first about cultural diversity in relation to the patient. In this respect it is worth mentioning that the recognition of cultural diversity and appropriate responses to it were issues which were strongly emphasised in the public consultation process which we held earlier this year in the context of developing National Anti-Poverty targets for the health sector and also our new national health strategy. Awareness and sensitivity training for staff is a key requirement for adapting to a culturally diverse patient population. The focus of this training should be the development of the knowledge and skills to provide services sensitive to cultural diversity. Such training can often be most effectively delivered in partnership with members of the minority groups themselves. I am aware that the Traveller community, for example, is involved in in-service training for health care workers. I am also aware that the National Consultative Committee on Racism and Interculturalism has been involved in training with the Eastern Regional Health Authority. We need to have more such initiatives. A step beyond the sensitivity training for existing staff is the training of members of the minority communities themselves as workers in our health services. Again the Traveller community has set an example in this area with its Primary Health Care Project for Travellers. The Primary Health Care for Travellers Project was established in 1994 as a joint partnership initiative with the Eastern Health Board and Pavee Point, with ongoing technical assistance being provided from the Department of Community Health and General Practice, Trinity College, Dublin. This project was the first of its kind in the country and has facilitated The project included a training course which concentrated on skills development, capacity building and the empowerment of Travellers. This confidence and skill allowed the Community Health Workers to go out and conduct a baseline survey to identify and articulate Travellers’ health needs. This was the first time that Travellers were involved in this process; in the past their needs were assumed. The results of the survey were fed back to the community and they prioritised their needs and suggested changes to the health services which would facilitate their access and utilisation. Ongoing monitoring and data collection demonstrates a big improvement in levels of satisfaction and uptake and ulitisation of health services by Travellers in the pilot area. This Primary Health Care for Travellers initiative is being replicated in three other areas around the country and funding has been approved for a further 9 new projects. This pilot project was the recipient of a WHO 50th anniversary commemorative award in 1998. The project is developing as a model of good practice which could inspire further initiatives of this type for other minority groups. Access to information has been identified in numerous consultative processes as a key factor in enabling people to take a proactive approach to managing their own health and that of their families and in facilitating their access to health services. Honouring our commitment to equity in these areas requires that information is provided in culturally appropriate formats. The National Health Promotion Strategy 2000-2005, for example, recognises that there exists within our society many groups with different requirements which need to be identified and accommodated when planning and implementing health promotion interventions. These groups include Travellers, refugees and asylum seekers, people with intellectual, physical or sensory disability and the gay and lesbian community. The Strategy acknowledges the challenge involved in being sensitive to the potential differences in patterns of poor health among these different groups. The Strategic aim is to promote the physical, mental and social well-being of individuals from these groups. The objective of the Strategy on these issues are: While our long term aim may be to mainstream responses so that our health services is truly multicultural, we must recognise the need at this point in time for very specific focused responses particularly for groups with poor health status such as Travellers and also for refugees and asylum seekers. In the case of refugees and asylum seekers examples of targeted services are screening for communicable diseases – offered on a voluntary basis – and psychological support services for those who have suffered trauma before coming here. The two approaches of targeting and mainstreaming are not mutually exclusive. A combination of both is required at this point in time but the balance between them must be kept under constant review in the light of changing needs. A major requirement if we are to meet the challenge of cultural diversity is an appropriate data and research base. I think it is important that we build up our information and research data base in partnership with the minority groups themselves. We must establish what the health needs of diverse groups are; we must monitor uptake of services and how well we are responding to needs and we must monitor outcomes and health status. We must also examine the impact of the policies in other sectors on the health of minority groups. The National Health Information Strategy, currently being developed, and the recently published National Strategy for Health Research – Making Knowledge Work for Health provide important frameworks within which we can improve our data and research base. A culturally diverse health sector workforce – challenges and opportunities The Irish health service can benefit greatly from successful international recruitment. There has been a strong non-national representation amongst the medical profession for more than 30 years. More recently there have been significant increases in other categories of health service workers from overseas. The Department recognises the enormous value that overseas recruitment brings over a wide range of services and supports the development of effective and appropriate recruitment strategies in partnership with health service employers. These changes have made cultural diversity an important issue for all health service organisations. Diversity in the workplace is primarily about creating a culture that seeks, respects, values and harnesses difference. This includes all the differences that when added together make each person unique. So instead of the focus being on particular groups, diversity is about all of us. Change is not about helping “them” to join “us” but about critically looking at “us” and rooting out all aspects of our culture that inappropriately exclude people and prevent us from being inclusive in the way we relate to employees, potential employees and clients of the health service. International recruitment benefits consumers, Irish employees and the overseas personnel alike. Regardless of whether they are employed by the health service, members of minority groups will be clients of our service and consequently we need to be flexible in order to accommodate different cultural needs. For staff, we recognise that coming from other cultures can be a difficult transition. Consequently health service employers have made strong efforts to assist them during this period. Many organisations provide induction courses, religious facilities (such as prayer rooms) and help in finding suitable accommodation. The Health Service Employers Agency (HSEA) is developing an equal opportunities/diversity strategy and action plans as well as training programmes to support their implementation, to ensure that all health service employment policies and practices promote the equality/diversity agenda to continue the development of a culturally diverse health service. The management of this new environment is extremely important for the health service as it offers an opportunity to go beyond set legal requirements and to strive for an acceptance and nurturing of cultural differences. Workforce cultural diversity affords us the opportunity to learn from the working practices and perspectives of others by allowing personnel to present their ideas and experience through teamwork, partnership structures and other appropriate fora, leading to further improvement in the services we provide. It is important to ensure that both personnel units and line managers communicate directly with their staff and demonstrate by their actions that they intend to create an inclusive work place which doesn´t demand that minority staff fit. Contented, valued employees who feel that there is a place for them in the organisation will deliver a high quality health service. Your conference here today has two laudable aims – to heighten awareness and assist health care staff to work effectively with their colleagues from different cultural backgrounds and to gain a greater understanding of the diverse needs of patients from minority ethnic backgrounds. There is a synergy in these aims and in the tasks to which they give rise in the management of our health service. The creative adaptations required for one have the potential to feed into the other. I would like to commend both organisations which are hosting this conference for their initiative in making this event happen, particularly at this time – Racism in the Workplace Week. The Public Health Agency and safefood today (Wednesday 22 June) officially launched Eat, Taste and Grow, a new interactive curriculum based education resource to help increase awareness among primary school children of the origins of their food, local produce and the role this plays in healthy eating. As research in 2006 showed, 18% of children aged 2-15 years in Northern Ireland were reported to be obese; and provisional data in 2008-2009 showed that 22.5% of children entering Year 1 were already overweight (17%) or obese (5%).* The launch of Eat, Taste and Grow is an innovative collaboration between the PHA and safefood that will provide children with lifelong lessons about the food they eat, healthy eating options and the benefits of an active lifestyle.The free teacher-led CD-ROM resource will be sent to every primary school across Northern Ireland by September 2011 and will help increase awareness among primary school children of the origins of their food and local produce, and the role this plays in healthy eating.Speaking at the event, Health Minister Edwin Poots said: "Being obese as a child can store up problems for the future, leading to a reduction in life expectancy and potentially causing other health problems such as increased risk of coronary heart disease, cancer and Type 2 Diabetes."Currently around one in four girls and one in six boys in Primary One (Year 1) are overweight or obese."Many of our children are not as physically active as they should be, nor do they have a healthy, balanced diet."This new resource will help teachers in our primary schools educate children on how to choose what foods are healthier for them which hopefully they will carry with them into adulthood."Dr Eddie Rooney, Chief Executive, PHA said: "The Public Health Agency recognises the need to give every child a healthy start in life. Topics include 'Where does our food come from?', 'Who produces our food?', 'How food grows' and 'How food is produced and preserved'. Obesity is a modern lifestyle epidemic that is threatening our health and well-being.This was the key message delivered by Health Minister Edwin Poots at the launch of The Framework for Preventing and Addressing Overweight and Obesity in Northern Ireland 2012-2022: 'A Fitter Future for All'.This ten year strategy will seek to improve the health and wellbeing of people throughout their entire life, from newborns to seniors.Minister Poots said: "We need to face the issue of obesity head on. It's an issue that will require commitment and action from across all sectors, including other government departments and agencies. It is therefore my intention to invest more than £7 million towards tackling the problem of obesity over the next three years."The negative impact on health caused by obesity cannot be over stated. Being obese increases the risk of developing serious illnesses such as heart disease, stroke, some cancers and type 2 diabetes."It is a significant challenge facing modern society and if we don't tackle it now we are storing up a multitude of problems for ourselves in the future."The Minister continued: "More and more of our children and young people are becoming overweight or obese and are putting themselves at risk of developing a range of health problems in their later years."Evidence shows that it is more likely that an obese child will become an obese adult. This in turn will lead to a greater strain on our health and social care services, with more people requiring treatment for obesity related illnesses and specialist care."The proposed framework looks to address a number of key issues, including:-increasing levels of breastfeeding;increasing knowledge and skills about food and its preparationencouraging participation in physical activity;promoting walking and cycling; making sure how we live and where we live encourages and supports healthy eating and physical activity;encouraging and supporting more community involvement with these issues; and;continuation of reformulation of processed foods.The Minister added: "In Northern Ireland 59% of adults are either overweight (36%) or obese (23%). Another worrying statistic is that 8% of children aged 2-15 years were assessed as being obese. These figures demonstrate the scale of the problem and the enormous challenge we are facing."The new framework sets challenging targets. To date we have focussed on simply trying to stop the rise in the levels of obesity, however under A Fitter Future For All we are seeking to actually reduce the level of obesity by 4% and overweight and obesity by 3% among adults. In addition, we are seeking a 3% reduction of obesity and 2% reduction of overweight and obesity among our children and young people." "Meeting these targets will require changes in our lifestyles and behaviours. Most importantly, individuals need to be given the opportunity to make decisions that will benefit their own health and wellbeing".Referring to the 'Give It A Go!' initiative, to increase awareness of the range of nutritional and physical activity initiatives in the southern area, the Minister said: "The Give It A Go! Initiative is a great example of how collaborative work can make such a positive contribution to peoples' lives by providing opportunities for learning, participation in physical activity and for social interaction."Tackling obesity and seeing positive results throughout the life course of the entire population will take time but I strongly believe that the actions set out in this framework will inspire and enable people to improve their diets and be more active."Encouraging people to consider the framework and adopt a healthier lifestyle, the Minister concluded: "Government cannot tackle obesity on its own. We can encourage and promote healthy eating and physical activity but as a society, we must take more individual responsibility for our own health outcomes."Dr Tracy Owen, Consultant in Public Health Medicine with the PHA, said: "The PHA is already working with partner organisations across many of the areas included in the framework 'A Fitter Future for All' and is addressing issues such as developing people's skills and knowledge about healthier eating along with encouraging participation in physical activity. The framework gives us the opportunity to raise awareness of this important area and strengthen action."As the Minister has mentioned, a good example of this coordinated action is the PHA supported initiative Give it a Go! which is providing people in the Southern area with the opportunity to learn about food through supermarket tours and Cook it! classes and to get active through walks, spinning classes and many other activities, all of which are free. These taster sessions are aimed at raising awareness of healthier lifestyles which will ultimately make changes in behaviour more likely."These changes, no matter how small, can help people to lose weight, maintain a healthy weight and bring big benefits to their general health. Importantly, we have developed this joint programme by working closely with our partners, particularly local councils."
