Background: Adverse drug reactions (ADRs) cause significant morbidity and mortality and account for around 6.5% of hospital admissions. Patient experiences of serious ADRs and their long-term impact on patients' lives, including their influence on current attitudes towards medicines, have not been previously explored. Objective: The aim of the study was to explore the experiences, beliefs, and attitudes of survivors of serious ADRs, using drug-induced Stevens-Johnson syndrome (SJS) and Toxic Epidermal Necrolysis (TEN) as a paradigm. Methods: A retrospective, qualitative study was undertaken using detailed semi-structured interviews. Fourteen adult survivors of SJS and TEN, admitted to two teaching hospitals in the UK, one the location of a tertiary burns centre, were interviewed. Interview transcripts were independently analysed by three different researchers and themes emerging from the text identified. Results: All 14 patients were aware that their condition was drug induced, and all but one knew the specific drug(s) implicated. Several expressed surprise at the perceived lack of awareness of the ADR amongst healthcare professionals, and described how the ADR was mistaken for another condition. Survivors believed that causes of the ADR included (i) being given too high a dose of the drug; (ii) medical staff ignoring existing allergies; and (iii) failure to monitor blood tests. Only two believed that the reaction was unavoidable. Those who believed that the condition could have been avoided had less trust in healthcare professionals. The ADR had a persisting impact on their current lives physically and psychologically. Many now avoided medicines altogether and were fearful of becoming ill enough to need them. © 2011 Adis Data Information BV. All rights reserved. Conclusions: Life-threatening ADRs continued to affect patients’ lives long after the event. Patients’ beliefs regarding the cause of the ADR differed, and may have influenced their trust in healthcare professionals and medicines. We propose that clear communication during the acute phase of a serious ADR may therefore be important.

Background: In December 2007, the National Institute for Health and Clinical Excellence and the National Patient Safety Agency in the UK (NICE-NPSA) published guidance that recommends all adults admitted to hospital receive medication reconciliation, usually by pharmacy staff. A costing and report tool was provided indicating a resource requirement of d12.9 million for England per year. Pediatric patients are excluded from this guidance. Objective: To determine the clinical significance of medication reconciliation in children on admission to hospital. Methods: A prospective observational study included pediatric patients admitted to a neurosurgical ward at Birmingham Childrens Hospital, Birmingham, England, between September 2006 and March 2007. Medication reconciliation was conducted by a pharmacist after the admission of each of 100 consecutive eligible patients aged 4 months to 16 years. The clinical significance of prescribing disparities between pre-admission medications and initial admission medication orders was determined by an expert multidisciplinary panel and quantified using an analog scale. The main outcome measure was the clinical signficance of unintentional variations between hospital admission medication orders and physician-prescribed pre-admission medication for repeat (continuing) medications. Results: Initial admission medication orders for children differed from prescribed pre-admission medication in 39%of cases. Half of all resulting prescribing variations in this setting had the potential to cause moderate or severe discomfort or clinical deterioration. These results mirror findings for adults. Conclusions: The introduction of medication reconciliation in children on admission to hospital has the potential to reduce discomfort or clinical deterioration by reducing unintentional changes to repeat prescribed medication. Consequently, there is no justification for the omission of children from the NICENPSA guidance concerning medication reconciliation in hospitals, and costing tools should include pediatric patients. © 2010 Adis Data Information BV. The purpose of the present dissertation was to evaluate the internal validity of symptoms of four common anxiety disorders included in the Diagnostic and Statistical Manual of Mental Disorders fourth edition (text revision) (DSM-IV-TR; American Psychiatric Association, 2000), namely, separation anxiety disorder (SAD), social phobia (SOP), specific phobia (SP), and generalized anxiety disorder (GAD), in a sample of 625 youth (ages 6 to 17 years) referred to an anxiety disorders clinic and 479 parents. Confirmatory factor analyses (CFAs) were conducted on the dichotomous items of the SAD, SOP, SP, and GAD sections of the youth and parent versions of the Anxiety Disorders Interview Schedule for DSM-IV (ADIS-IV: C/P; Silverman & Albano, 1996) to test and compare a number of factor models including a factor model based on the DSM. Contrary to predictions, findings from CFAs showed that a correlated model with five factors of SAD, SOP, SP, GAD worry, and GAD somatic distress, provided the best fit of the youth data as well as the parent data. Multiple group CFAs supported the metric invariance of the correlated five factor model across boys and girls. Thus, the present study’s finding supports the internal validity of DSM-IV SAD, SOP, and SP, but raises doubt regarding the internal validity of GAD.^

This article analyzes the process of deterioration of the work as a source of social rights and as a social integration element. The context of this process is the passage of a wage-labour society with stable employment to other where the labour relations are deregulated. This aim was tackled by means of quantitative and qualitative techniques. Secondary sources of statistical information were used together with interviews to experts of the institutional sphere. The results of this research show the emergence of a new exclusive model of social cohesion based on intensification and generalization of social inequalities and job insecurity. In this new model of social cohesion, it is noted that the integration strategies of people have less and less support from Government and people are neglected. This process appears in the four spheres that classify the social exclusion risks factors: the structural, the institutional, the relational and the individual.

Este trabajo tiene como objetivo estudiar los diferentes estados mentales de los personajes de las novelas de Tu rostro mañana, de Javier Marías. Estudiando las reflexiones del narrador sobre J. Deza, Peter Wheeler o Francisco Rico observamos que su decadencia mental se muestra a través de una suerte de ―presciencia‖ o lucidez momentánea que puede servir para mostrar el silencio como única tendencia de todo discurso. Desde el momento en que toda historia de ficción se cimenta sobre un discurso –no importa su cauce de presentación, ni su fuente– este es falsificado por el tiempo, la gente y cualquier otra herramienta que pueda ser utilizada para contar nada. Las conclusiones de este trabajo muestran la quimera que implica tratar de mantener una contención absoluta sobre lo acaecido, pues dicho vacío de narrativas será ocupado por una suplantación que suele ser el reverso más infame de sus actores. Es por ello que el narrador J. Deza sigue conminado a explicar sus historias, incluso allí donde uno diría que ya no puede haber ni palabras suficientes para traducir un hecho en ficción.

Está de nuevo reunida la comunidad del ICESI para cumplir el último acto con sus alumnos y para recibir la última lección que la da siempre el Rector de la Institución, así como su despedida como estudiantes del claustro. Sin embargo, es esta sólo un adiós ritual ya que esperamos continuar en contacto con todos ustedes y engrosar la cada vez más amplia familia deIICESI, que poco a poco va convirtiéndose en una gran fuerza intelectual y moral, por su preparación y vocación de servicio. La de hoyes una fecha de gran importancia para la entidad, pues marca una etapa más de progreso. Damos el título hoy al mayor número de estudiantes de postgrado, de Magister y especialistas en Mercadeo efectuado dentro del convenio Eafit Icesi. Han sido estudios serios con tesis investigativas en los del Magister y trabajos especiales en los de especializaciones. Para Eafit por intermedio de su Rector, nuestra más sincera gratitud de esta institución y de la comarca vallecaucana. Ellos han contribuido ya con un contingente importante para el desarrollo de esta región de la patria, con gran sentido de amplia mi misión colombianista y sin mirar egoístamente sólo su tierra.

O presente trabalho tem como temática: O papel do Auxiliar de Desenvolvimento Infantil da Secretaria de Educação do Recife: das normativas do cargo a prática profissional. Este profissional desenvolve suas funções dentro das creches e centros de educação infantil da Secretaria de Educação do Recife, trabalhando com crianças de 0 a 3 anos. O objetivo deste estudo é identificar as relações entre o pré-requisito exigido para o cargo, as atribuições conferidas à função e a prática profissional do Auxiliar de Desenvolvimento Infantil, tomando para análise o último concurso realizado em 2014. A partir dessa configuração e para entender de fato o papel do Auxiliar de Desenvolvimento Infantil, pontuando a sua função como pedagógica ou não, considerando que se atribui como pré-requisito para o provimento do cargo a exigência de formação no Ensino Médio. O estudo parte da revisão de literatura sobre a concepção de infância, criança e educação passando pelas contribuições teóricas de vários autores, sobretudo os que abordam as questões da infância e da educação infantil de maneira contextualizada. Procedeu-se também a abordagem da evolução do perfil profissional daqueles que atuam na Educação Infantil no Brasil, pois, ao longo dos anos, estes profissionais vêm passando por transformações e exigências em relação a sua atuação, sobretudo em decorrência das mudanças de concepção de infância, de criança e de educação. A metodologia é qualitativa e como instrumento de coleta de dados utilizou-se questionários e entrevista. Os sujeitos da pesquisa são os Auxiliares de Desenvolvimento Infantil, os professores de Educação Infantil e a Divisão de Educação Infantil da Secretaria de Educação do Recife. Os resultados apontaram que o trabalho desenvolvido pelos ADIs dentro das instituições de educação infantil é pedagógico. Estes profissionais realizam atividades educativas e desta forma, pode-se dizer, que esta função exige que o profissional tenha conhecimentos pedagógicos para exercê-la. Finalmente, este estudo poderá contribuir para que a Secretaria de Educação do Recife redirecione a política pública de Educação Infantil no que se refere aos pré-requisitos de formação dos profissionais concursados para o trabalho nas instituições de educação infantil, exigindo a formação mínima no Normal Médio (Magistério) para a função de Auxiliar de Desenvolvimento Infantil, entendendo que esta função exige que o profissional tenha conhecimentos pedagógicos para atuar junto às crianças de 0 a 3 anos.
