Resumo:
The authors present a case report of a victim of high power electric shock. The main electric lesion was a penetrating abdominal wound with loss of substance of the abdominal wall and an electric lesion of the hepatic segment and of the gallbladder. The surgical treatment included hepatic segmentectomy, cholecystectomy, repair of the abdominal wall with Marlex prosthesis and skin graft, besides the debridment of the lesions of extremities. The postoperative evolution was satisfactory and the follow-up for 6 months didn't show any sequelae.

Purpose The aim of this thesis1 is to analyse theoretically how institutionalisation of competitive tendering2, governance and budgetary policies cannot be taken for granted to lead to accountability among institutional actors3. The nature of an institutionalised management accounting policy, its relevance as a source of power in organisational decision making, and in negotiating inter-organisational relationships, are also analysed. Practical motivation The practical motivation of the thesis is to show how practitioners and policy makers can institutionalise changes which improve the power of management accounting and control systems4 as a mechanism of accountability among institutional actors and in negotiating relationships with other organisations. Theoretical motivation and conceptual approach The theoretical motivation of the thesis is to extend the institutional framework of management accounting change proposed by Burns and Scapens (2000) by using the theories of critical realism, communicative action, negotiated order and the framework of circuits of power. The Burns and Scapens framework needs further theorisation to analyse the relationship between the institutionalisation of management accounting and accountability; and the relevance of management accounting information in negotiating in inter-organisational relationships. Methodology and field studies Field research took place in public and not-for-profit health care organisations and a municipality in Finland from 2008 to 2013. Data were gathered by document analysis, interviews, participation in meetings and observations. Findings The findings are explained in four different essays that show that institutionalisation of competitive tendering, governance and budgetary policies cannot be taken for granted to lead to accountability among institutional actors. The ways by which institutional actors think and act can be influenced by other institutional mechanisms, such as inter-organisational circuits of power and intraorganisational governance policies, independent of the institutional change process. The relevance of institutionalised management accounting policies in negotiating relationships between two or more organisations depends on processes and contexts through which institutional actors use management accounting information as a tool of communication, mutual understanding and power. Research limitations / implications The theoretical framework used can be applied validly in other studies. The empirical findings cannot be generalised directly to other organisations than the organisations analysed. Practical implications Competitive tendering and budgetary policies can be institutionalised to shape actions of institutional actors within an organisation. To lead to accountability, practitioners and policy makers should implement governance policies that increase the use of management accounting information in institutional actors’ thinking, actions and responsibility for their actions. To reach a negotiated order between organisations, institutionalised management accounting policies should be used as one of the tools of communication aiming to reach mutual agreement among institutional actors.

Työn tavoitteena on kehittää asiakkaiden käsinhitsausta kartoittamalla heidän hitsaavaa tuotantoa, sekä sen mahdollisia ongelmia. Järjestettävän hitsauskokeen avulla mitataan mekanisoidun hitsauksen tuottavuutta, laatua ja työergonomiaa. Työ sisältää teoreettisen ja käytännöllisen osuuden. Teoreettinen osuus pohjautuu kirjallisuuteen ja käytännöllinen osuus eri yrityksien kyselyihin ja haastatteluihin sekä hitsauskokeeseen. Teoreettinen osuus käsittelee käsinhitsausta ja siihen liittyviä mekanisointiratkaisuja. Käsinhitsauksessa käsitellään yleisimmät käsinhitsausprosessit, käsinhitsattava tuote ja tuotanto sekä sen tuottavuutta. Mekanisointiratkaisuissa käsitellään yleisimmät mekanisointiratkaisut ja laitteet sekä käytännön toteutusta. Käytännöllisessä osuudessa yritykset pitivät nykyisin hitsauksessa tärkeimpänä hitsaajien ammattitaitoa, tuottavuutta, työn mielekkyyttä, laatua sekä työturvallisuutta. Yrityksien tulisi tarkastella tuotannossaan hitsaussolujen sisältöä, sisäistä logistiikkaa ja kappaleiden kiinnitystä. Kappaleenkäsittelijään yrityksiltä tuli paljon kehitysideoita, kuten kiinnitykseen valmiita ratkaisuja sekä pöytälevyyn erilaisia vaihtoehtoja. Hitsauskokeen perusteella kappaleenkäsittelijällä työaika oli 29,4 % pienempi kuin työpöydällä. Laadullisesti koe antoi myös positiivisia tuloksia, käsittelypöydässä hitsattuja rakenteita ei tarvinnut korjata, mutta normaalissa työpöydässä piti, jotta C –luokan vaatimukset täyttyivät. Työturvallisuuden ja ergonomian kannalta suurimmat hyödyt olivat hyvä työasento, huurujen ja kurkottelun vähentyminen, mahdollisten palovammojen vähentyminen sekä vähemmän vaaraa osien putoamisesta.

"How can I improve my practice and contribute to the professional knowledge base through narrative-autobiographical self-study?" Through the use of Whitehead's (1989) living educational theory and examination of my stories, I identify the values and critical events that have helped me come to know my own learning and shape my professional self. Building on the premise that educational knowledge/theory is created, recreated, and lived through educational inquiry; I strive to make meaning of this data archive, collected over 7 years of teaching. I chart my journey to reexamine my beliefs and practices, to find a balance between traditional and progressive practices and to align my theory and practice. I retell, and, thus, in some way relive, my own "living contradictions." A reconceptualization of the KNOW, DO, BE model (Drake & Burns, 2004) is used to develop strategies to align my practice, including a six-step model of curriculum design that combines the backwards design process of Wiggins and McTighe (1998), the KNOW, DO, BE model (Drake & Burns) and Curry and Samara's (1995) differentiation planner.

The John O. McKellar was a ship that belonged to the Scott Misener fleet. The first ship named after McKellar was launched on Januaray 25, 1929, from Wallsend, England, and was bound for Sault St. Marie, Ontario. This ship became part of the Colonial Steamship Company in 1950, and in 1952 was renamed the J.G. Irwin when construction of a new John O. McKellar was completed. John Oscar McKellar was born on June 28, 1878 in Lobo Township, Middlesex County, west of London, Ont. He worked as a marine engineer, and became acquainted with Robert Scott Misener when the two were shipmates serving with the Algoma central fleet. In 1919, the two men joined forces to run a shipping company. Together, they purchased the wooden steamer "Simon Langell", and worked together on the ship for the next three years. Throughout his career with Misener's company, John McKellar served as Chief Engineer, then Marine Superintendent, and finally Secretary-Treasurer. He died on September 19, 1951.

The study purpose was to examine differences between competitive and recreational weight trainers on indices of motivation, goals and behaviour. Data was collected from a purposive sample of competitive (n = 177; Mage = 30.86; SDage = 11.35) and recreational (n = 196; Mage = 21.97; SDage = 6.05) weight trainers using a cross-sectional, non-experimental design. Participants completed the Behavioural Regulation in Exercise Questionnaire-2R, Exercise Motivations Inventory-2, assessment of weight training behaviour and demographic questions. Multivariate analyses of variance indicated higher endorsement of autonomous motives and mostly intrinsically-oriented goals, while independent samples t-tests indicated higher frequency of weight training behaviour among the competitive weight trainers. Group differences were independent of demographic factors. Findings suggest that autonomous motives and intrinsic goals may not be undermined by competition among competitive weight trainers. This study also provides support for the utility of organismic integration theory and goal contents theory in examining strength-based exercise.

An orderly book of the Second Regiment of U.S. Dragoons, New York, dated August 14, 1812-July 29, 1813. The book contains orders pertaining to day-to-day military matters, such as punishments for disobedience, court-martial proceedings, camp rules and regulations, and guidelines for interacting with civilians in the vicinity of the camp. The Regiment was stationed at various locations in upstate New York and Canada, including Greenbush, Albany, Sackets Harbor, Utica, Geneva, Fort Niagara and Fort George. General Henry Dearborn originally commanded the Regiment at Greenbush. Names noted in this book include:E. Beebe, Deputy Adjt. General; William King, Capt. 15th; John Chandler, General ;W. Gamewood, Major ;James Burns, Colonel;John Woodford, Major; Andrew McDowell, Capt.; Abm. Gustis, Major; C.W. The contract is dated May 20, 1812, between John Dodge, physician and surgeon, and Jonas Holland. The contract describes the services required of the physician and the salary to be paid. The bonus pay voucher is dated April 25, 1813, for $8.00 paid to Thomas Blunt by Captain Jonas Holland for 'enlisting into the army of the United States for five years'.

This paper derives optimal monetary policy rules in setups where certainty equivalence does not hold because either central bank preferences are not quadratic, and/or the aggregate supply relation is nonlinear. Analytical results show that these features lead to sign and size asymmetries, and nonlinearities in the policy rule. Reduced-form estimates indicate that US monetary policy can be characterized by a nonlinear policy rule after 1983, but not before 1979. This finding is consistent with the view that the Fed's inflation preferences during the Volcker-Greenspan regime differ considerably from the ones during the Burns-Miller regime.

INTRODUCTION. La guérison rapide des sites donneurs des greffes cutanées favorise la survie des victimes de brûlures graves (>50 % de superficie brûlée). La mortalité élevée de ces patients est attribuable au fait que la superficie des brûlures excède celle de la peau saine. Des cultures épithéliales autologues (CEA) sont des feuillets de kératinocytes produits en culture à partir de la peau du patient. Cette étude a évalué l’effet des CEA sur l'épithélialisation des sites donneurs chez les grands brûlés. MÉTHODES. Tous les patients recevant des CEA ont été prospectivement inclus. Les plaies des sites donneurs ont été recouvertes de CEA, sauf pour une région contrôle randomisée de 7 x 7 cm. Des biopsies faites sur la greffe de peau ont permis de contrôler la profondeur des plaies sur les sites donneurs. Il y avait deux types de contrôles, avec gaze non adhérente trempée dans le milieu de culture ou dans le salin. L’épithélialisation était quantifiée globalement (% d’épithélialisation par photographie) et histologiquement (par biopsie au poinçon) à simple insu. La guérison des zones de contrôle et CEA était comparée par analyse de variance et par le test de Student. RÉSULTATS. Entre 2008 et 2009, 6 patients furent recrutés avec un total de 11 sites donneurs. Ces patients avaient en moyenne 43.5 ans, 56 % de superficie brûlée, 45% de brûlure pleine épaisseur, 66% avaient une brûlure d’inhalation, 75 jours de séjour. Il n’y a aucune corrélation entre le pourcentage d’épithélialisation et l’épaisseur du prélèvement des greffes (Pearson 0.19). Le score photographique est significativement influencé par le traitement (CEA vs Contrôle; p = 0,039) et par le jour postopératoire (p < 0,001). Le temps moyen pour atteindre un score photographique de guérison pour les zones contrôles fut de 10.2 jours contre 8.6 jours pour le CEA (p = 0,021). A l’évaluation histologique, les sites donneurs traités par le milieu de culture ont évolué aussi favorablement que ceux traités par des feuillets de CEA. CONCLUSION. L’utilisation de CEA sur les sites donneurs semble accélérer leur épithélialisation chez les victimes de brûlures graves. Cet effet est probablement le résultat d’une stimulation de la réépithélialisation innée de la plaie, plutôt que par une adhérence des feuillets de kératinocytes cultivés à la surface de la plaie.

L’hypoalbuminémie est une trouvaille fréquente chez le patient brulé mais sa relation avec la morbidité et mortalité n’a pas été bien établie. Objectif : Déterminer si l’hypoalbuminémie dans les premières 24 heures suivant l’admission est associée avec la dysfonction d’organes (mesurée avec le score SOFA) chez les patients présentant des brûlures graves. Méthodologie : Nous avons révisé les dossiers médicaux des patients adultes avec de brûlures de 20% ou plus de surface corporelle admis pendant les premières 24 heures à l’unité de grands brulés du CHUM entre les années 2008 et 2009. Nous avons utilisé un modèle de régression linéaire multivariée pour déterminer si l’hypoalbuminémie était un prédicteur indépendant de la dysfonction d’organes. Résultats : 56 sujets ont été analysés. L’analyse de régression linéaire multiple a montré qu’en contrôlant pour l’âge, le sexe, la surface corporelle brûlée et les brûlures par inhalation, l’hypoalbuminémie pendant les premières 24 heures suivant l’admission est un prédicteur indépendant de la dysfonction d’organes. Une concentration d’albumine ≤30g/L est aussi associée à une augmentation de la dysfonction d’organes [score SOFA au jour 0 (p = 0.005), jour 1 (p = 0.005), moyenne de la première semaine (p = 0.004)], mais n’est pas associée avec la mortalité (p = 0.061). Conclusions : L’hypoalbuminémie est associée avec la dysfonction d’organes chez les patients brulés. À la différence de facteurs non modifiables comme l’âge, le sexe, la surface corporelle brûlée et la présence de brûlures par inhalation, la correction de l’hypoalbuminémie peut être un objectif intéressant pour un futur essai clinique.

An interdisciplinary team of a pediatric university teaching hospital in the Montreal area, who treats annually a considerable number of child burn victims, developed a behavioral observation scale on the comfort of children of 7 years of age and less during painful procedures «Échelle d’observation comportementale du confort d’enfants brûlés» (OCCEB- BECCO). The goal of this study was to initiate the validation of OCCEB-BECCO, a new tool for evaluation of comfort for child burn victims. With samples of 16 patients and 5 experts, we proceeded to the evaluation of content validity, internal consistency and criterion validity. Results have shown adequate content validity, internal consistency with (T1) r = 0,96 p < 0,0001, (T2) r = 0,95 p < 0,0001, (T3) r = 0,95 p < 0,0001 and criterion validity with Cronbach alpha at 0,82. A future study with a larger sample and on a longer period of time would be required to pursue validation of this new scale.

Sei $N/K$ eine galoissche Zahlkörpererweiterung mit Galoisgruppe $G$, so dass es in $N$ eine Stelle mit voller Zerlegungsgruppe gibt. Die vorliegende Arbeit beschäftigt sich mit Algorithmen, die für das gegebene Fallbeispiel $N/K$, die äquivariante Tamagawazahlvermutung von Burns und Flach für das Paar $(h^0(Spec(N), \mathbb{Z}[G]))$ (numerisch) verifizieren. Grob gesprochen stellt die äquivariante Tamagawazahlvermutung (im Folgenden ETNC) in diesem Spezialfall einen Zusammenhang her zwischen Werten von Artinschen $L$-Reihen zu den absolut irreduziblen Charakteren von $G$ und einer Eulercharakteristik, die man in diesem Fall mit Hilfe einer sogenannten Tatesequenz konstruieren kann. Unter den Voraussetzungen 1. es gibt eine Stelle $v$ von $N$ mit voller Zerlegungsgruppe, 2. jeder irreduzible Charakter $\chi$ von $G$ erfüllt eine der folgenden Bedingungen 2a) $\chi$ ist abelsch, 2b) $\chi(G) \subset \mathbb{Q}$ und $\chi$ ist eine ganzzahlige Linearkombination von induzierten trivialen Charakteren; wird ein Algorithmus entwickelt, der ETNC für jedes Fallbeispiel $N/\mathbb{Q}$ vollständig beweist. Voraussetzung 1. erlaubt es eine Idee von Chinburg ([Chi89]) umzusetzen zur algorithmischen Berechnung von Tatesequenzen. Dabei war es u.a. auch notwendig lokale Fundamentalklassen zu berechnen. Im höchsten zahm verzweigten Fall haben wir hierfür einen Algorithmus entwickelt, der ebenfalls auf den Ideen von Chinburg ([Chi85]) beruht, die auf Arbeiten von Serre [Ser] zurück gehen. Für nicht zahm verzweigte Erweiterungen benutzen wir den von Debeerst ([Deb11]) entwickelten Algorithmus, der ebenfalls auf Serre's Arbeiten beruht. Voraussetzung 2. wird benötigt, um Quotienten aus den $L$-Werten und Regulatoren exakt zu berechnen. Dies gelingt, da wir im Fall von abelschen Charakteren auf die Theorie der zyklotomischen Einheiten zurückgreifen können und im Fall (b) auf die analytische Klassenzahlformel von Zwischenkörpern. Ohne die Voraussetzung 2. liefern die Algorithmen für jedes Fallbeispiel $N/K$ immer noch eine numerische Verifikation bis auf Rechengenauigkeit. Den Algorithmus zur numerischen Verifikation haben wir für $A_4$-Erweiterungen über $\mathbb{Q}$ in das Computeralgebrasystem MAGMA implementiert und für 27 Erweiterungen die äquivariante Tamagawazahlvermutung numerisch verifiziert.
