Background Timely assessment of the burden of HIV/AIDS is essential for policy setting and programme evaluation. In this report from the Global Burden of Disease Study 2015 (GBD 2015), we provide national estimates of levels and trends of HIV/AIDS incidence, prevalence, coverage of antiretroviral therapy (ART), and mortality for 195 countries and territories from 1980 to 2015. Methods For countries without high-quality vital registration data, we estimated prevalence and incidence with data from antenatal care clinics and population-based seroprevalence surveys, and with assumptions by age and sex on initial CD4 distribution at infection, CD4 progression rates (probability of progression from higher to lower CD4 cell-count category), on and off antiretroviral therapy (ART) mortality, and mortality from all other causes. Our estimation strategy links the GBD 2015 assessment of all-cause mortality and estimation of incidence and prevalence so that for each draw from the uncertainty distribution all assumptions used in each step are internally consistent. We estimated incidence, prevalence, and death with GBD versions of the Estimation and Projection Package (EPP) and Spectrum software originally developed by the Joint United Nations Programme on HIV/AIDS (UNAIDS). We used an open-source version of EPP and recoded Spectrum for speed, and used updated assumptions from systematic reviews of the literature and GBD demographic data. For countries with high-quality vital registration data, we developed the cohort incidence bias adjustment model to estimate HIV incidence and prevalence largely from the number of deaths caused by HIV recorded in cause-of-death statistics. We corrected these statistics for garbage coding and HIV misclassifi cation. Findings Global HIV incidence reached its peak in 1997, at 3·3 million new infections (95% uncertainty interval [UI] 3·1–3·4 million). Annual incidence has stayed relatively constant at about 2·6 million per year (range 2·5–2·8 million) since 2005, after a period of fast decline between 1997 and 2005. The number of people living with HIV/AIDS has been steadily increasing and reached 38·8 million (95% UI 37·6–40·4 million) in 2015. At the same time, HIV/AIDS mortality has been declining at a steady pace, from a peak of 1·8 million deaths (95% UI 1·7–1·9 million) in 2005, to 1·2 million deaths (1·1–1·3 million) in 2015. We recorded substantial heterogeneity in the levels and trends of HIV/AIDS across countries. Although many countries have experienced decreases in HIV/AIDS mortality and in annual new infections, other countries have had slowdowns or increases in rates of change in annual new infections. Interpretation Scale-up of ART and prevention of mother-to-child transmission has been one of the great successes of global health in the past two decades. However, in the past decade, progress in reducing new infections has been slow, development assistance for health devoted to HIV has stagnated, and resources for health in low-income countries have grown slowly. Achievement of the new ambitious goals for HIV enshrined in Sustainable Development Goal 3 and the 90-90-90 UNAIDS targets will be challenging, and will need continued eff orts from governments and international agencies in the next 15 years to end AIDS by 2030.

Biochemical agents, including bacteria and toxins, are potentially dangerous and responsible for a wide variety of diseases. Reliable detection and characterization of small samples is necessary in order to reduce and eliminate their harmful consequences. Microcantilever sensors offer a potential alternative to the state of the art due to their small size, fast response time, and the ability to operate in air and liquid environments. At present, there are several technology limitations that inhibit application of microcantilever to biochemical detection and analysis, including difficulties in conducting temperature-sensitive experiments, material inadequacy resulting in insufficient cell capture, and poor selectivity of multiple analytes. This work aims to address several of these issues by introducing microcantilevers having integrated thermal functionality and by introducing nanocrystalline diamond as new material for microcantilevers. Microcantilevers are designed, fabricated, characterized, and used for capture and detection of cells and bacteria. The first microcantilever type described in this work is a silicon cantilever having highly uniform in-plane temperature distribution. The goal is to have 100 μm square uniformly heated area that can be used for thermal characterization of films as well as to conduct chemical reactions with small amounts of material. Fabricated cantilevers can reach above 300C while maintaining temperature uniformity of 2−4%. This is an improvement of over one order of magnitude over currently available cantilevers. The second microcantilever type is a doped single crystal silicon cantilever having a thin coating of ultrananocrystalline diamond (UNCD). The primary application of such a device is in biological testing, where diamond acts as a stable, electrically isolated reaction surface while silicon layer provides controlled heating with minimum variations in temperature. This work shows that composite cantilevers of this kind are an effective platform for temperature-sensitive biological experiments, such as heat lysing and polymerase chain reaction. The rapid heat-transfer of Si-UNCD cantilever compromised the membrane of NIH 3T3 fibroblast and lysed the cell nucleus within 30 seconds. Bacteria cells, Listeria monocytogenes V7, were shown to be captured with biotinylated heat-shock protein on UNCD surface and 90% of all viable cells exhibit membrane porosity due to high heat in 15 seconds. Lastly, a sensor made solely from UNCD diamond is fabricated with the intention of being used to detect the presence of biological species by means of an integrated piezoresistor or through frequency change monitoring. Since UNCD diamond has not been previously used in piezoresistive applications, temperature-denpendent piezoresistive coefficients and gage factors are determined first. The doped UNCD exhibits a significant piezoresistive effect with gauge factor of 7.53±0.32 and a piezoresistive coefficient of 8.12×10^−12 Pa^−1 at room temperature. The piezoresistive properties of UNCD are constant over the temperature range of 25−200C. 300 μm long cantilevers have the highest sensitivity of 0.186 m-Ohm/Ohm per μm of cantilever end deflection, which is approximately half that of similarly sized silicon cantilevers. UNCD cantilever arrays were fabricated consisting of four sixteen-cantilever arrays of length 20–90 μm in addition to an eight-cantilever array of length 120 μm. Laser doppler vibrometry (LDV) measured the cantilever resonant frequency, which ranged as 218 kHz−5.14 MHz in air and 73 kHz−3.68 MHz in water. The quality factor of the cantilever was 47−151 in air and 18−45 in water. The ability to measure frequencies of the cantilever arrays opens the possibility for detection of individual bacteria by monitoring frequency shift after cell capture.

Background: The nitration of tyrosine residues in proteins is associated with nitrosative stress, resulting in the formation of 3-nitrotyrosine (3-NT). 3-NT levels in biological samples have been associated with numerous physiological and pathological conditions. For this reason, several attempts have been made in order to develop methods that accurately quantify 3-NT in biological samples. Regarding chromatographic methods, they seem to be very accurate, showing very good sensibility and specificity. However, accurate quantification of this molecule, which is present at very low concentrations both at physiological and pathological states, is always a complex task and a target of intense research. Objectives: We aimed to develop a simple, rapid, low-cost and sensitive 3-NT quantification method for use in medical laboratories as an additional tool for diagnosis and/or treatment monitoring of a wide range of pathologies. We also aimed to evaluate the performance of the HPLC-based method developed here in a wide range of biological matrices. Material and methods: All experiments were performed on a Hitachi LaChrom Elite® HPLC system and separation was carried out using a Lichrocart® 250-4 Lichrospher 100 RP-18 (5μm) column. The method was further validated according to ICH guidelines. The biological matrices tested were serum, whole blood, urine, B16 F-10 melanoma cell line, growth medium conditioned with the same cell line, bacterial and yeast suspensions. Results: From all the protocols tested, the best results were obtained using 0.5% CH3COOH:MeOH:H2O (15:15:70) as the mobile phase, with detection at wavelengths 215, 276 and 356 nm, at 25ºC, and using a flow rate of 1 mL/min. By using this protocol, it was possible to obtain a linear calibration curve (correlation coefficient = 1), limits of detection and quantification in the order of ng/mL, and a short analysis time (<15 minutes per sample). Additionally, the developed protocol allowed the successful detection and quantification of 3-NT in all biological matrices tested, with detection at 356 nm. Conclusion: The method described in this study, which was successfully developed and validated for 3-NT quantification, is simple, cheap and fast, rendering it suitable for analysis in a wide range of biological matrices.

A large proportion of human populations suffer memory impairments either caused by normal aging or afflicted by diverse neurological and neurodegenerative diseases. Memory enhancers and other drugs tested so far against memory loss have failed to produce therapeutic efficacy in clinical trials and thus, there is a need to find remedy for this mental disorder. In search for cure of memory loss, our laboratory discovered a robust memory enhancer called RGS14(414). A treatment in brain with its gene produces an enduring effect on memory that lasts for lifetime of rats. Therefore, current thesis work was designed to investigate whether RGS14(414) treatment can prevent memory loss and furthermore, explore through biological processes responsible for RGS-mediated memory enhancement. We found that RGS14(414) gene treatment prevented episodic memory loss in rodent models of normal aging and Alzheimer´s disease. A memory loss was observed in normal rats at 18 months of age; however, when they were treated with RGS14(414) gene at 3 months of age, they abrogated this deficit and their memory remained intact till the age of 22 months. In addition to normal aging rats, effect of memory enhancer treatment in mice model of Alzheimer´s disease (AD-mice) produced a similar effect. AD-mice subjected to treatment with RGS14(414) gene at the age of 2 months, a period when memory was intact, showed not only a prevention in memory loss observed at 4 months of age but also they were able to maintain normal memory after 6 months of the treatment. We posit that long-lasting effect on memory enhancement and prevention of memory loss mediated through RGS14(414) might be due to a permanent structural change caused by a surge in neuronal connections and enhanced neuronal remodeling, key processes for long-term memory formation. A neuronal arborization analysis of both pyramidal and non-pyramidal neurons in brain of RGS14(414)-treated rats exhibited robust rise in neurites outgrowth of both kind of cells, and an increment in number of branching from the apical dendrite of pyramidal neurons, reaching to almost three times of the control animals. To further understand of underlying mechanism by which RGS14(414) induces neuronal arborization, we investigated into neurotrophic factors. We observed that RGS14 treatment induces a selective increase in BDNF. Role of BDNF in neuronal arborization, as well as its implication in learning and memory processes is well described. In addition, our results showing a dynamic expression pattern of BDNF during ORM processing that overlapped with memory consolidation further support the idea of the implication of this neurotrophin in formation of long-term memory in RGS-animals. On the other hand, in studies of expression profiling of RGS-treated animals, we have demonstrated that 14-3-3ζ protein displays a coherent relationship to RGS-mediated ORM enhancement. Recent studies have demonstrated that the interaction of receptor for activated protein kinase 1 (RACK1) with 14-3-3ζ is essential for its nuclear translocation, where RACK1-14-3-3ζ complex binds at promotor IV region of BDNF and promotes an increase in BDNF gene transcription. These observations suggest that 14-3-3ζ might regulate the elevated level of BDNF seen in RGS14(414) gene treated animals. Therefore, it seems that RGS-mediated surge in 14-3-3ζ causes elevated BDNF synthesis needed for neuronal arborization and enhanced ORM. The prevention of memory loss might be mediated through a restoration in BDNF and 14-3-3ζ protein levels, which are significantly decreased in aging and Alzheimer’s disease. Additionally, our results demonstrate that RGS14(414) treatment could be a viable strategy against episodic memory loss.

Every space launch increases the overall amount of space debris. Satellites have limited awareness of nearby objects that might pose a collision hazard. Astrometric, radiometric, and thermal models for the study of space debris in low-Earth orbit have been developed. This modeled approach proposes analysis methods that provide increased Local Area Awareness for satellites in low-Earth and geostationary orbit. Local Area Awareness is defined as the ability to detect, characterize, and extract useful information regarding resident space objects as they move through the space environment surrounding a spacecraft. The study of space debris is of critical importance to all space-faring nations. Characterization efforts are proposed using long-wave infrared sensors for space-based observations of debris objects in low-Earth orbit. Long-wave infrared sensors are commercially available and do not require solar illumination to be observed, as their received signal is temperature dependent. The characterization of debris objects through means of passive imaging techniques allows for further studies into the origination, specifications, and future trajectory of debris objects. Conclusions are made regarding the aforementioned thermal analysis as a function of debris orbit, geometry, orientation with respect to time, and material properties. Development of a thermal model permits the characterization of debris objects based upon their received long-wave infrared signals. Information regarding the material type, size, and tumble-rate of the observed debris objects are extracted. This investigation proposes the utilization of long-wave infrared radiometric models of typical debris to develop techniques for the detection and characterization of debris objects via signal analysis of unresolved imagery. Knowledge regarding the orbital type and semi-major axis of the observed debris object are extracted via astrometric analysis. This knowledge may aid in the constraint of the admissible region for the initial orbit determination process. The resultant orbital information is then fused with the radiometric characterization analysis enabling further characterization efforts of the observed debris object. This fused analysis, yielding orbital, material, and thermal properties, significantly increases a satellite’s Local Area Awareness via an intimate understanding of the debris environment surrounding the spacecraft.

RESUMO - Enquadramento: A infeção por Chlamydia trachomatis (CT) é considerada um grave problema de Saúde Pública. É causa de infeções sintomáticas tanto no homem como na mulher, bem como, de infeções assintomáticas que podem ter consequências muito graves a longo prazo. A presença de CT sem tratamento aumenta o risco de transmissão do vírus da imunodeficiência humana. Existem vários estudos publicados de prevalência da CT por todo o mundo, no entanto, estudos de prevalência da CT em populações assintomáticas, envolvendo homens que fazem sexo com homens (HSH) são raros na Europa. Assim, o presente estudo, ao estimar a prevalência e ao descrever as caraterísticas epidemiológicas e de conduta dos HSH, pretende contribuir para o plano de ação contra as IST´s e VIH na Catalunha, através de estratégias concretas para deteção e prevenção da CT. Métodos: Trata-se de um estudo observacional, transversal de prevalência da CT em HSH utentes de um serviço comunitário em Barcelona, com recurso a técnicas de diagnóstico de biologia molecular e a um questionário. Durante Março e Junho de 2015, foram recrutados 200 voluntários de um serviço comunitário em Barcelona que foram testados para CT e NG em três locais anatómicos através de uma PCR em tempo real utilizando o ensaio Anyplex™ CT/NG Real-time Detection. Resultados: O presente estudo permitiu uma caraterização da situação atual quanto á prevalência e aos fatores de risco associados á infeção por CT em HSH. A prevalência nesta população específica foi de 12,6% e os fatores risco associados foram o facto de serem VIH e praticarem sexo anal insertivo/recetivo com parceiro estável. Conclusões: Os resultados obtidos reforçam a necessidade do desenvolvimento de estratégias adequadas de controlo e prevenção da CT nesta população de risco, tais como: rastreios frequentes e tratamento dos casos positivos para quebrar a cadeia de transmissão, promoção da saúde, educação e notificação dos parceiros sexuais. Também demonstram a importância destes rastreios nos três locais anatómicos: uretra, reto e faringe.

Vulnerability and psychic illness Based on a sample of 1701 college and university students from four different sites in Switzerland, the U.S., and Argentina, this study investigated the interrelationships between insufficient coping skills under chronic stress and impaired general health. We sought to develop standardised means for "early" identification of students at risk of mental health problems, as these students may benefit from "early" interventions before psychiatric symptoms develop and reach clinically relevant thresholds. All students completed two self-report questionnaires: the Coping Strategies Inventory "COPE" and the Zurich Health Questionnaire "ZHQ", with the latter assessing "regular exercises", "consumption behavior", "impaired physical health", "psychosomatic disturbances", and "impaired mental health". This data was subjected to structure analyses based on neural network approaches, using the different study sites' data subsets as independent "learning" and "test" samples. We found two highly stable COPE scales that quantified basic coping behaviour in terms of "activity-passivity" and "defeatism-resilience". The excellent reproducibility across study sites suggested that the new scales characterise socioculturally independent personality traits. Correlation analyses for external validation revealed a close relationship between high scores on the defeatism scale and impaired physical and mental health, hence underlining the scales' clinical relevance. Our results suggested in particular: (1.) the proposed method to be a powerful screening tool for early detection and prevention of psychiatric disorders; (2.) physical activity like regular exercises to play a critical role not only in preventing health problems but also in contributing to early intervention programs.

Background: One third of hospitalized older-adults who developed a delirium are discharged prior to complete resolution of their symptoms. Others may develop symptoms shortly after their hospital discharge or an acute illness. Therefore, strategies for early detection and prevention of delirium at home must be created and implemented. Aims: The aim of the pilot study was two-fold. First, to develop and test the effectiveness of a nursing intervention to detect and prevent delirium among older- adults who were recently hospitalized or had an acute illness. Second, we assessed the feasibility and acceptability of this nursing intervention strategy with this specific population. Methods: A total of 114 patients age 65 and older were recruited in a home health service to participate between February and November 2012. Participants were randomized into an experimental group (n=56) or a control group (n=58). In addition to the control group which only receives standard home care, nursing interventions tailored to detect/prevent delirium were delivered to the experimental group at 5 time points following discharge (at 48 hours, 72 hours, 7 days, 14 days, and 21 days). Socio-demographic characteristics, body mass index, medications, comorbidities, delirium symptoms (Confusion Assessment Method), cognitive impairment (Mini- Mental State Examination) and functional status (Activities of Daily Living) were assessed at study entry (TT). Outcomes of delirium symptoms, cognitive impairment and functional status were assessed after one month (T2). Descriptive and bivariate methods were used to analyse the data. Results: The two groups were similar at baseline. At one month following discharge no statistical differences were observed between groups in terms of symptoms of delirium (p= 0.085), cognitive impairment (p= 0.151) and functional status (p= 0.235). However in the nursing intervention group, significant improvements in cognitive functioning (p= 0,005) and functional status (p= 0,000) as well as decreased delirium symptoms (p=0,003) were observed. The nursing intervention strategy was feasible and well received by the participants. Conclusion: Nursing intervention strategy to detect/prevent delirium appears to be effective but a larger clinical study is needed to confirm these preliminary findings. - Introduction : Un tiers des personnes âgées hospitalisées développent un état confusionnel aigu (ECA) et quittent l'hôpital sans que les symptômes ne soient résolus. D'autres peuvent développer des symptômes d'ECA à domicile après une hospitalisation ou une maladie aiguë. Pour ces raisons, des stratégies de détection et prévention précoces d'ECA doivent être développées, implantées et évaluées. But : Cette étude pilote avait pour but de développer et tester les effets d'une stratégie d'interventions infirmières pour détecter et prévenir l'ECA chez des personnes âgées à domicile après une hospitalisation ou une maladie récente. Dans un deuxième temps, la faisabilité et l'acceptabilité de l'implantation de cette stratégie auprès de cette population spécifique ainsi que de l'étude ont été évaluées. Méthode : Au total 114 personnes 65 ans) ont été recrutées entre février et novembre 2012. Les participants ont été randomisés, soit dans le groupe expérimental (GE, n=56), soit dans le groupe témoin (GT, n=58). En complément des soins usuels, une stratégie d'interventions de détection/prévention d'ECA a été dispensée au GE à 48 heures, 72 heures, 7 jours, 14 jours et 21 jours après le retour à domicile ou une maladie récente. Des données sociodémographiques et de santé (Indice de Masse Corporelle, relevé de la médication, comorbidités), la présence de symptômes d'ECA (Confusion Assessment Method), de troubles cognitifs (Mini évaluation de l'état mental) et de déficit fonctionnel (Activités de la vie quotidienne et instrumentales) ont été évalués à l'entrée de l'étude (T,). L'effet de la stratégie d'interventions a été mesuré sur le nombre de symptômes d'ECA, du déficit/état cognitif (Mini évaluation de l'état mental) et du déficit/état fonctionnel (Activités de la vie quotidienne) après un mois (T2). Des analyses descriptives et bivariées ont été effectuées. Résultats : Les deux groupes étaient équivalents au début de l'étude. Aucune différence significative n'a été retrouvée après un mois entre le GE et le GT par rapport au nombre de symptômes d'ECA (p= 0,085), au déficit cognitif (p= 0,151) et fonctionnel (p= 0,235). Toutefois, une amélioration significative a été observée dans le GE par rapport aux symptômes d'ECA (p= 0,003), aux déficits cognitifs (p= 0,005) et fonctionnels (p= 0,000) à un mois. La stratégie d'interventions s'avère faisable et a été bien acceptée par les participants. Conclusion : La stratégie d'interventions infirmières de détection/prévention d'ECA à domicile semble prometteuse, mais des études cliniques à large échelle sont nécessaires pour confirmer ces résultats préliminaires.

School counselors have a duty to formulate strategies that aid in the detection and prevention of child sexual abuse (American School Counselor Association, 2003). School counselors are charged with helping sexually abused children by recognizing sexual abuse indicators based on a child's symptomology and/or behavior, and understanding how this trauma may affect children in the school setting. Mandated reporting issues, talking with children and adolescents about sexual abuse suspicions, and understanding trauma symptoms and their contribution to the difficulties that sexually abused children have in school are highlighted. In addition, how school counselors can collaborate with clinicians treating sexually abused children through role-appropriate advocacy, intervention, and aftercare strategies is described.

El ausentismo laboral es un problema para la economía de las empresas y para la sociedad en general, su principal causa son las enfermedades de origen común. Los costos totales son difíciles de cuantificar por la cantidad de factores que influyen en él. METODOLOGIA: Estudio descriptivo de corte transversal que caracterizó el ausentismo laboral teniendo como fuente la base de datos de una empresa del sector terciario de la economía Colombiana, dedicada a las telecomunicaciones a nivel nacional. Se tomó como población universo 3.647 trabajadores a mitad de año de 2013. La población objeto de estudio la conformaron 889 trabajadores que se incapacitaron desde el 1 de enero de 2013 al 31 de diciembre de 2013. RESULTADOS: Se presentaron por enfermedad común 1.097 (83,87%)incapacidades, 740 (83,24%) incapacitados y 7.526 (57,28%) de días perdidos. Las mujeres son las que presentaron mayor número de incapacidades 63,3% (694). Según la clasificación de grupos diagnósticos del CIE 10, el sistema que generó mayor número de días perdidos fue el osteomuscular con el 13,40%. El diagnostico individual que genero mayor número de días perdidos fue el trastorno mixto de ansiedad y depresión 4% (301). Los trabajadores con cinco o más incapacidades durante el año fueron 26 (3,51%) que generaron el 33,3% (2.506) de los días perdidos. Los accidentes de trabajo no son una causa importante de ausentismo. RECOMENDACIONES: Implementar un sistema de recolección de información del ausentismo que se alimente de manera sistemática y continua para tener información suficiente, oportuna y confiable que permita analizarla y generar planes de acción. revisar las actividades que la empresa ha implementado para identificar, evaluar, gestionar los factores de riesgo psicolaborales; optimizar la detección temprana y prevención de desórdenes musculo esqueléticos. Por último realizar monitoreo evaluación y seguimiento a los trabajadores que presentan más de cinco incapacidades durante el año, con un grupo de salud interdisciplinario que permita implementar acciones de promoción y prevención de salud.

Las mujeres son las que presentaron mayor número de incapacidades 63,3% (694). Según la clasificación de grupos diagnósticos del CIE 10, el sistema que generó mayor número de días perdidos fue el osteomuscular con el 13,40%. El diagnostico individual que genero mayor número de días perdidos fue el trastorno mixto de ansiedad y depresión 4% (301). Los trabajadores con cinco o más incapacidades durante el año fueron 26 (3,51%) que generaron el 33,3% (2.506) de los días perdidos. Los accidentes de trabajo no son una causa importante de ausentismo. RECOMENDACIONES: Implementar un sistema de recolección de información del ausentismo que se alimente de manera sistemática y continua para tener información suficiente, oportuna y confiable que permita analizarla y generar planes de acción. revisar las actividades que la empresa ha implementado para identificar, evaluar, gestionar los factores de riesgo psicolaborales; optimizar la detección temprana y prevención de desórdenes musculo esqueléticos. Por último realizar monitoreo evaluación y seguimiento a los trabajadores que presentan más de cinco incapacidades durante el año, con un grupo de salud interdisciplinario que permita implementar acciones de promoción y prevención de salud.
