Introduction : Les guides de pratique recommandent que les patients avec une maladie artérielle périphérique (MAP) soient traités médicalement afin de réduire la survenue d’évènements cardiovasculaires majeurs. Objectif : Identifier les facteurs associés à la prescription des thérapies préventives recommandées. Méthode : Les patients avec une MAP (n=362) traités consécutivement par angiodilatation fémoropoplitée entre 2008 et 2010 dans un centre tertiaire (CHU de Québec, Canada) ont été inclus dans l’étude. L’issue clinique primaire était la prescription de trois thérapies combinées. Résultats : Au total, 52% des patients recevaient la thérapie combinée. La présence d’au moins trois facteurs de risque cardiovasculaire (Rapport de cotes (RC)=4,51; IC 95% : 2,76-7,37) était le facteur le plus fortement associé à la prise des thérapies combinées. Conclusion : La prise en charge du risque cardiovasculaire des patients avec une MAP est encore sous-optimale. Une meilleure compréhension des barrières et des facilitateurs à l’application des recommandations est toujours nécessaire.

OBJETIVOS: Identificar el nivel de sobrecarga y sus factores asociados en los cuidadores primarios de los /as pacientes del Centro de Atención Ambulatoria 302 del IESS. METODOLOGÍA: Es un estudio cuantitativo analítico-transversal, cuyo universo es 222 cuidadores primarios de adultos mayores, controlados en el Centro de Atención Ambulatoria 302 del Instituto Ecuatoriano de Seguridad Social, Cuenca Ecuador. Los datos se obtuvieron del formulario que consta de un apartado sociodemográfico, variables de estudio, Índice de Katz y Escala del Nivel de Sobrecarga de Zarit. Se tabuló y analizó la información obtenida exponiéndola en los resultados. RESULTADOS: En promedio los cuidadores tienen 53 años, en su mayoría de sexo femenino, 52% de ellos casados, casi el 60% son hijos y un 11% conyugues del adulto mayor. La mayoría los cuida 12 horas en el día, los 7 días de la semana y llevan cuidándolos al menos 5 años. Se observó que 26% de adultos mayores tiene una dependencia moderada y el 38% dependencia severa. De acuerdo al test de Zarit el 18,7% de cuidadores tienen sobrecarga leve y el 14% sobrecarga intensa. CONCLUSIONES: El 18,7% de los cuidadores primarios presentan una sobrecarga leve y el 14% perciben una sobrecarga intensa.

O presente relatório pretende descrever a intervenção psicomotora com população idosa em contexto de estágio profissionalizante no centro de dia do Centro Social da Trafaria da Santa Casa de Misericórdia de Almada, no ano de 2014-2015 no âmbito do Ramo de Aprofundamento de Competências Profissionais do Mestrado de Reabilitação Psicomotora da Faculdade de Motricidade Humana da Universidade de Lisboa. Encontra-se estruturado em duas partes relativas ao enquadramento e realização da prática profissional. Deste modo, são referidos os aspetos teóricos de envelhecimento, intervenção psicomotora, gerontopsicomotricidade, e é ainda realizada uma caracterização da instituição de forma a fundamentar e enquadrar a prática profissional. Referente à realização da prática profissional são abordadas as duas etapas da intervenção realizadas, especificando a intervenção psicomotora, na segunda etapa, realizada com um grupo de oito pessoas idosas e um estudo caso, verificando-se no geral uma manutenção e melhoria das competências psicomotoras. No final do relatório são referidas as conclusões finais, sendo uma análise crítica e reflexiva acerca da intervenção psicomotora neste contexto, limitações e propostas.

O presente documento, criado no âmbito da Unidade Curricular de Atividade de Aprofundamento de Competências Profissionais, do Mestrado em Reabilitação Psicomotora, apresenta o trabalho desenvolvido no Centro de Reabilitação Psicossocial, das Irmãs Hospitaleiras em Maputo – Moçambique. O processo de intervenção psicomotora foi desenvolvido com jovens/adultos com Dificuldades Intelectuais e Desenvolvimentais e crianças com Paralisia Cerebral ou Atraso Global do Desenvolvimento, durante 8 meses. A intervenção concretizou-se em sessões de grupo e individuais em contexto de atividade motora, terapias expressivas, desenvolvimento pessoal e social com os jovens/adultos e em contexto de promoção do desenvolvimento psicomotor, atividades psicopedagógicas e terapias expressivas com as crianças, com objetivo de promoção de bem-estar, qualidade de vida, funcionalidade e autonomia dos utentes, tendo em conta o contexto cultural, familiar e as potencialidades, dificuldades e interesses. No processo de intervenção estiveram presentes as fases próprias da prática terapêutica, observação, conhecimento e criação de relação empática com os utentes, avaliação inicial, processo de intervenção planeado, avaliação final e comparação dos resultados com avaliação inicial e reflexão sobre o mesmo. É ainda apresentado um projeto de investigação centrado no estudo das atitudes da comunidade do Bairro das Mahotas perante a população com Dificuldade Intelectual e Desenvolvimental.

In Spain, immigrant women have high rates for initiating breastfeeding. In contrast, the case of immigrant Chinese mothers stands out, due to the low rate. In China, breastfeeding has historically been the cultural norm. An ethnographic study was conducted to explore aspects related to the low rate of breastfeeding. Field observations and informal interviews were conducted in two hospitals and a primary care center. Semi-structured interviews were performed with Chinese mothers and health workers. Among Spanish health workers, there is a belief that Chinese women do not breastfeed due to cultural reasons. The rapid return to work and the popular phenomenon of sending babies back to China for grandparents to raise constitute barriers for breastfeeding. Although in China breastfeeding is common practice, in Madrid the prioritization of productivity over reproduction and the existence of the so-called “satellite babies” and transnational maternity make the establishment and maintenance of breastfeeding difficult.

This paper explores the role of information and communication technologies in managing risk and early discharge patients, and suggests innovative actions in the area of E-Health services. Treatments of chronic illnesses, or treatments of special needs such as cardiovascular diseases, are conducted in long-stay hospitals, and in some cases, in the homes of patients with a follow-up from primary care centre. The evolution of this model is following a clear trend: trying to reduce the time and the number of visits by patients to health centres and derive tasks, so far as possible, toward outpatient care. Also the number of Early Discharge Patients (EDP) is growing, thus permiting a saving in the resources of the care center. The adequacy of agent and mobile technologies is assessed in light of the particular requirements of health care applications. A software system architecture is outlined and discussed. The major contributions are: first, the conceptualization of multiple mobile and desktop devices as part of a single distributed computing system where software agents are being executed and interact from their remote locations. Second, the use of distributed decision making in multiagent systems, as a means to integrate remote evidence and knowledge obtained from data that is being collected and/or processed by distributed devices. The system will be applied to patients with cardiovascular or Chronic Obstructive Pulmonary Diseases (COPD) as well as to ambulatory surgery patients. The proposed system will allow to transmit the patient's location and some information about his/her illness to the hospital or care centre

Introducción: El dolor lumbar y los desórdenes músculo esqueléticos comprometen la salud y la calidad de vida de los trabajadores, pueden poner en riesgo el futuro laboral de las personas. bjetivo: Estimar la prevalencia de dolor lumbar y los posibles factores biomecánicos asociados en el personal operativo y administrativo en una empresa manufacturera de jabón en Bogotá, en el año 2016 Metodología: Estudio de corte transversal donde se evaluó el riesgo biomecánico y la prevalencia del dolor lumbar en personal administrativo (138) y operativo (165); se utilizó como instrumento el ERGOPAR validado en España. Se revisó la asociación utilizando la prueba Chi Cuadrado de Pearson, con un nivel de significación α 0.05 Resultados: 303 trabajadores de una empresa manufacturera de jabón en Bogotá, donde predominó el género masculino (51,82%) y la población adulta media entre 30-39 años (57,42%). La prevalencia del dolor lumbar en la población fue de 61,39% (186). La edad no se asoció estadísticamente al dolor lumbar. Se encontró asociación estadística entre el síntoma dolor lumbar y extensión de cuello (p=0,05 OR1.95 IC 1.33-2.88), así como con agarrar o sujetar objetos (p= 0,036. OR 2.3 IC 1.59-3.51) y con las exigencias físicas laborales (p= 0.001 OR 1.99 IC 1.31-3.02). Conclusiones: La población estudiada presentó una alta prevalencia de dolor lumbar, con predominio en personal que realiza labores operativas, y del género femenino. La adopción de posturas de extensión del cuello y la sujeción o agarre de objetos son factores asociados directamente con la aparición de lumbalgia.

The Education for All Handicapped Children Act of 1975, P.L. 94-142, created a new challenge for the nation's public school systems. During 1982-1983, a national study, called the "Collaborative Study of Children with Special Needs", was conducted in 5 metropolitan school districts to evaluate the effectiveness of education and health care services of children in kindergarten to 6th grade being provided under P.L. 94-142 programs. This dissertation (the Substudy) was undertaken to augment the findings of the Collaborative Study. The purpose of this study was to develop a database to provide descriptive information on the demographic, service and health characteristics of a small group of 3 and 4 year old handicapped children served by the Houston Independent School District (HISD) during 1982-1983.^ The study involved a stratified sample of 105 three and four year old children divided into 3 groups according to type of handicapping condition.^ The results of the study gave a clearer picture of the demographic characteristics of these Pre-K children. Specifically, sex ratio was approximately one, lower than the national norm. Family and socioeconomic characteristics were assessed.^ The study used an independence/dependence index composed of 11 items on the parent questionnaire to assess the level of functional independence of each child. An association was found between index scores and parent-reported effects of the child on family activity. Parents who said that their child's condition had affected the family's job situation, housing accomodations, vacation plans, marriage, choice of friends and social activities were also more likely to report less independence in the child. In addition, many of the Substudy children had extensive care-taking needs reflected in specific components of the index such as dressing, feeding, toileting or moving about the house.^ In general the results of the Pre-K Substudy indicate that at the early childhood level, the HISD special education program is functioning well in most areas and that parents are very satisfied with the program. Purpose: The primary goal of this exploratory study is to demonstrate that distress screening across the course of cancer treatment is possible and provides valuable information about patient needs over time. Distress screening is aligned with guidelines from national accrediting organizations and may lead to improved health-related quality of life, satisfaction with medical care, and possibly survival.Methods: Medical, surgical, and radiation oncology patients completed a screening instrument before their appointments during a six-month period. Patients indicated their level of distress on four domains (practical, emotional, health and social concerns). De-identified data was collected, aggregated and descriptive statistics were analyzed.Results: Approximately 3000 screens were collected and 1500 cancer patients were screened. Of patients who indicated distress, 54% demonstrated a distress level of five or greater. Distress level eight was the most frequent level of distress indicated. The Cancer Dietitian was the most commonly requested healthcare team provider. The Health Concern domain was most frequently endorsed.Conclusion: NCCN, IOM and COC guidelines recommend distress screening in all cancer treatment centers, however implementation has proven difficult. This study adds to the literature about distress in cancer patients, demonstrates the feasibility of repeated distress screening and provides a model program demonstrating the implementation of repeated distress screening at a community cancer center. Findings highlight the importance of supportive oncology services due to the prevalence of high levels of distress. Findings demonstrate the importance of the Cancer Dietitian in supportive cancer care. Additionally, the research reveals a potential perceived stigma in seeking psychosocial oncology services.

This study examined the joint effects of home environment and center-based child care quality on children’s language, communication, and early literacy development, while also considering prior developmental level. Participants were 95 children (46 boys), assessed as toddlers (mean age = 26.33 months;Time 1) and preschoolers (mean age = 68.71 months; Time 2) and their families. At both times, children attended center-based child care classrooms in the metropolitan area of Porto, Portugal. Results from hierarchical linear models indicated that home environment and preschool quality, but not center-based toddler child care quality, were associated with children’s language and literacy outcomes at Time 2. Moreover, the quality of preschool classrooms moderated the association between home environment quality and children’s language and early literacy skills – but not communication skills – at Time 2, suggesting the positive cumulative effects of home environment and preschool quality. Findings further support the existence of a detrimental effect of low preschool quality on children’s language and early literacy outcomes: positive associations among home environment quality and children’s developmental outcomes were found to reduce substantially when children attended low-quality preschool classrooms.

OBJECTIVE: Recognizing the potential impact of psychiatric and psychosocial factors on liver transplant patient outcomes is essential to apply special follow-up for more vulnerable patients. The aim of this article was to investigate the psychiatric and psychosocial factors predicted medical outcomes of liver transplanted patients. METHODS: We studied 150 consecutive transplant candidates, attending our outpatient transplantation clinic, including 84 who had been grafted 11 of whom died and 3 retransplanted. RESULTS: We observed that active coping was an important predictor of length of stay after liver transplantation. Neuroticism and social support were important predictors of mortality after liver transplantation. CONCLUSION: It may be useful to identify patients with low scores for active coping and for social support and high scores for neuroticism to design special modes of follow-up to improve their medical outcomes.

Birth centers are maternal care models that use appropriate technology when providing care to birthing women. This descriptive study aimed to characterize intrapartum care in a freestanding birth center, in light of the practices recommended by the World Health Organization (WHO), with 1,079 assisted births from 2006 to 2009 in the Sapopemba Birth Center, São Paulo, Brazil. Results included the use of intermittent auscultation (mean=7 controls); maternal positions during delivery: semi-sitting (82.3%), side-lying (16.0%), other positions (1.7%), oral intake (95.6%); companionship (93.3%); exposure to up to three vaginal examinations (85.4%), shower bathing (84.0%), walking (68.0%), massage (60.1%), exercising with a Swiss ball (51.7%); amniotomy (53.4%), oxytocin use during the first (31.0%) and second stages of labor (25.8%), bath immersion (29.3%) and episiotomy (14.1%). In this birth center, care providers used practices recommended by the WHO, although some practices might have been applied less frequently.

This study aimed to analyze the social representations in the professionals of technical staff, who work with children at USP daycare centers. Eight professionals of the nursing field underwent a semi-structured interview. The interviews were recorded and transcribed in their entirety and the content of the discourse was subjected to thematic-categorical analysis. The categories were transformed into variables and processed by the software Classification Hiérarchique Classificatoire et Cohésitive (CHIC®) and analyzed by the hierarchical similarity tree. The results indicate that actions to promote health are reported as educational and transformative, in which health care gains new meaning through contextualized conceptions in the field of child education. We conclude that professionals attribute new meanings to their practices in the health care environment of daycare centers as their representations shifts from the logic of the biomedical field to a logic of educational care. In this sense, they perceive themselves as being challenged to establish an interaction with the children in terms of their activities related to the promotion of health and in an educational act.
