Background: Minority ethnic groups in the UK are reported to have a poor experience of mental health services, but comparative information is scarce. Aims: To examine ethnic differences in patients’ experience of community mental health services. Method: Trusts providing mental health services in England conducted surveys in 2004 and 2005 of users of community mental health services. Multiple regression was used to examine ethnic differences in responses. Results: About 27 000 patients responded to each of the surveys, of whom 10% were of minority ethnic origin. In the 2004 survey, age, living alone, the 2004 survey, age, living alone, detention and hospital admissions were stronger predictors of patient experience than ethnicity. Self-reported mental health status had the strongest explanatory effect. In the 2005 survey, the main negative differences relative to the White British were for Asians. Conclusions: Ethnicity had a smaller effect on patient experience than other variables. Relative to the White British, the Black group did not report negative experiences whereas the Asian group were most likely to respond negatively. However, there is a need for improvements in services for minority ethnic groups, including access to talking therapies and better recording of ethnicity.

Since remote times, certain sectors of society have been exposed to inequality and vulnerability, where adequate intervention processes have become conspicuous because of their absence. Nowadays, current societies have the responsibility of contributing, based on their experience and knowledge, with more efficient policies and programs that improve the life quality of the most disadvantaged. It is here where art and its different tools play a very important role, not only on a physical level, but also as an education tool that allows the development of emotional, mental and communicative skills. The aim of this paper is to make clear the potential of art as an instrument of social and educational intervention. It starts by showing worldwide-collected experience related to education and arts, and then, it acquaints the reader with two parallel intervention projects that worked with youths under social vulnerability conditions. These interventions were developed based on a qualitative research (Grounded theory), using as methodology “The Artistic Mediation” with emphasis on body language. This methodology helped researchers to get close to the participants and to know their experiences and emotions. At the same time, it was possible to evidence the positive effects of educative interventions through art. These workshops were based on an artistic methodology especially focused on body language. Data in this work is qualitative, and as such, it permits a special approach to the personal and emotional experiences of the participants; clearly showing the positive effects of the referenced practice on them.

In mental health services over recent decades, the positive move away from hospital-based care to community-based services has entailed that people with higher levels of need are being supported by community mental health services. This paper begins by reviewing the literature on coercion in the field of community-based mental health care and treatment. It is argued that the lack of a critical understanding of the concept and how it is used by practitioners and agencies can have serious repercussions for the rights of service users. Using a quasi-experimental, longitudinal design, the authors then seek to test some of the ideas about coercion by comparing the activities of assertive outreach and community mental health teams in Northern Ireland, particularly the key ideas of perceived coercion, workers’ strategies and engagement with services. Key findings were that assertive outreach appeared to be more successful at reducing perceived coercion, minimizing the need for coercive strategies, engaging high-risk clients and reducing inpatient bed use. These findings are compared with other studies in this area. The authors also argue that there is a need for greater transparency in the way that practitioners use coercive measures and more explicit guidance is required in this crucial area of mental health practice.

En este artículo tratamos, por medio de Suenan timbres de Luis Vidales, el idiotismo de la sociedad tradicionalista tecnificada de principios de siglo XX en Colombia. Estudio que realizamos desde el concepto de idiotismo de Vidales, en estrecha relación con el de cultura de viñeta de Rafael Gutiérrez Girardot, para plantear la burla del poeta a la sociedad capitalista, que cuestiona la presencia de la Iglesia católica y la política conservadora en todos los estadios de la vida nacional.

This paper describes a study that used a mixed method approach to elicit the views of a range of stakeholders about experiences of compulsory admission to psychiatric hospital, and the use of the Mental Health Review Tribunal (MHRT). The paper begins with an introduction to the background of the study, one that took place in Northern Ireland, a region in the UK with its own mental health legislation and policy. A review of literature is then presented. This highlights some of the disadvantages that service users and carers face when dealing with professionals during and following compulsory admission to hospital. This section concludes with an overview of literature on the MHRT in the UK. A range of methods was used to gather data from the following stakeholders: five service user and carer focus group interviews (n = 44); interviews with four lawyers experienced in Tribunal work; an interview with a legal member of the Tribunal; a survey of solicitors who identified themselves as equipped to carry out Tribunal work; interviews with three managers of organisations that provided patient advocacy services; letters to hospital managers requesting information provided to patients and carers. The findings reveal a number of themes associated with these experiences of compulsory admission to hospital and subsequent use of the Tribunal. Service users and carers generally found it difficult to access relevant information about rights, information provided by hospital managers was uneven and lawyers were often not familiar with processes associated with compulsory admission. There was a range of views about the Tribunal. Most respondents felt that the Tribunal was necessary and mostly satisfactory in the way it carried out its functions, but stakeholders raised a number of issues. Carers in particular felt that they should be more involved in decision-making processes, whereas lawyers tended to be focused on more technical, legal issues. Problems of regrading prior to the Tribunal and in examining medical evidence were highlighted by lawyers. There was an appeal for better information and advice by service users and carers, and recognition of the need for better training and education for lawyers. The paper concludes with a brief discussion about current mental health law in the UK, arguing that, in this context, professionals should more proactively use information and advice that can enable service users and carers to defend their rights. An exploration of the pages of two psychiatric hospital magazines, Speedwell from Holywell Hospital, Antrim, and The Sketch from Downshire Hospital, Downpatrick, reveals the activity filled lives of patients and staff during the 1960s and 1970s. This was a time of great change in mental health care. It was also a time of political turbulence in Northern Ireland. With large in-patient populations, both hospitals had a range of occupational and sporting activities available to patients and staff. The magazines formed part of the effort to promote the ethos of a therapeutic community. While hospital magazines may be viewed as one aspect of an institutional system that allowed people to cut themselves from the wider society, they also provided opportunities for budding writers to express their views on life in a hospital from the service user perspective. As such they offer some valuable insights into the lives of psychiatric patients.

Civic participation is important for peacebuilding and democratic development; however, the role of mental health has been largely overlooked by policymakers aiming to stimulate engagement in civil society. This study investigated antecedents of civic participation in Colombia, a setting of protracted political conflict, using bootstrapped mediation in path analysis. Past exposure to violence, experience with community antisocial behavior, and perceived social trust were all significantly related to civic participation. In addition, depression mediated the impact of past exposure to political violence and perceived social trust, but not community antisocial behavior, on civic participation. In this context, findings challenged depictions of helpless victims and instead suggested that when facing greater risk (past violence exposure and community antisocial behavior), individuals responded in constructive ways, taking on agency in their communities. Social trust in one’s neighbors and community also facilitated deeper engagement in civic life. Relevant to the mediation test, interventions aiming to increase civic participation should take mental health into account. Limitations and possible future research are discussed.

Introdução: O Bullying é um tema que tem despertado a atenção da comunidade científica e pedagógica. Os estudos desenvolvidos nas últimas 3 décadas, têm associado o Bullying a sofrimento psíquico dos seus intervenientes e como precursor de delinquência e criminalidade na idade adulta. Não existe consenso quanto à especificidade do tema, no entanto o conceito pretende caracterizar um tipo particular de violência e agressividade entre pares. A definição do fenómeno implica, segundo alguns autores, a intencionalidade e repetição do comportamento com desequilíbrio de poder entre os intervenientes, provocador e vítima. Existe uma grande variabilidade nas taxas de prevalência, esta diminui com a idade, é mais frequente nos rapazes, acontece no perímetro escolar, principalmente no recreio e alguns estudos sugerem uma diminuição nos últimos 15 anos. Os estudos têm permitido delinear os perfis social e psicológico dos diferentes intervenientes no Bullying, provocador, vítima, vítima-provocador e audiência, assim como das suas famílias, o que pode ter um impacto na prevenção, detecção e intervenção precoces. Casos Clínicos: As autoras expõem dois casos de crianças do sexo masculino, ambas com 11 anos de idade, residentes em distritos diferentes, uma vítima e a outra vítima-provocador de Bullying, em que se observam consequências ao nível da psicopatologia das crianças. Os perfis sócio-psicológicos das crianças e das suas famílias são ilustrativos dos apresentados nos estudos. Conclusão: A compreensão e maior conhecimento por parte dos clínicos e professores sobre o Bullying permitem uma maior protecção das crianças em idade escolar contra algumas formas de violência e agressividade, evitando situações de reactividade excessiva, como as medidas punitivas, geradoras de mais violência.

Este relatório descreve o trabalho realizado no âmbito do estágio do Mestrado em Reabilitação Psicomotora da Faculdade de Motricidade Humana, no Ramo de Aprofundamento de Competências Profissionais. O estágio foi realizado na Clínica da Encarnação, Hospital Dona Estefânia. A Intervenção Psicomotora foi desenvolvida com crianças entre os 3 e os 12 anos. A intervenção da estagiária desenvolveu-se, essencialmente com 32 crianças. Inicialmente é apresentada uma revisão teórica sobre saúde mental, psicomotricidade e aspetos teóricos relativos às principais psicopatologias observadas. Segue-se a caracterização a instituição, bem como do serviço de psicomotricidade. São descritos dois estudos-caso, de modo a exemplificar o trabalho desenvolvido. Para cada um deles é apresentado o processo de avaliação, estruturação do processo terapêutico, resultados e a sua respetiva discussão. Em ambos os casos foram observadas melhorias, verificando-se assim o contributo da intervenção através da psicomotricidade. Por fim, é apresentada uma reflexão, no sentido de compreender e integrar as vivências relativas ao percurso realizado, destacando a importância destas na formação de uma identidade e crescimento a nível profissional e pessoal.

The purpose of this project was to raise awareness surrounding child and adolescent mental health in an effort to reduce preconceived stigmas in relation to this specialized field. This project presented a literature review of the current state of child and adolescent mental health in Canada today, including the prevalence and several treatment options for young people confronting mental health challenges. Consideration of the powerful role of the education system upon youth with mental health issues became evident, specifically regarding early identification and prevention. A needs assessment was conducted to gather feedback from the clinical practitioners of a Section 23 classroom within a Southern Ontario hospital. This assessment was used to develop an informational and pedagogical workshop resource to extend practitioner understanding of this pertinent issue and support the social and emotional needs of young people confronting mental heath challenges. Results of the assessment indicated the significant need for such a workshop resource, and these responses were used to guide the development of Group Chat: A Workshop to Support the Emotional and Social Needs of Youth. The latter was subsequently presented to participants, whereby evaluative questionnaires indicated the efficacy and usefulness of this workshop resource to both practitioners and students alike.

Les problèmes de santé mentale représentent un pourcentage important du fardeau de morbidité mondiale. Cela, ajouté aux ressources limitées disponibles pour le fonctionnement des établissements, encourage l’intérêt pour l’évaluation de leur performance. Ce projet propose d’utiliser le modèle ÉGIPSS pour évaluer la performance de deux hôpitaux psychiatriques dans deux systèmes de santé différents, le « Hospital Psiquiátrico Universitario del Valle » (HPUV) situé en Colombie et l’Institut universitaire en santé mentale Douglas au Québec. Le modèle a été choisi en raison de la richesse des dimensions qu’il comprend et son caractère global. Les données ont été recueillies à travers des entrevues et de la documentation disponible dans les établissements. L’analyse des résultats montre que le Douglas a une meilleure performance générale, même tenant compte qu’il se trouve dans un pays à haut revenu avec des standards d’évaluation supérieures. De vingt-et-une sous-dimensions évaluées, l’HPUV a présente six avec un état préoccupante et le Douglas aucune. L’analyse des résultats a aussi servi pour identifier des facteurs de causalité critiques des problèmes de performance, soit l’organisation à l’intérieur des établissements, la communication interne, les plans de santé mentale et la disponibilité des professionnels. La comparaison a permis apprécier l’influence de ces éléments dans les deux contextes.

Au Canada, les Commissions d'Examen des Troubles Mentaux de chaque province ont la responsabilité de déterminer les conditions de prise en charge des personnes déclarées Non Criminellement Responsables pour cause de Troubles Mentaux (NCRTM) et de rendre, sur une base annuelle une des trois décisions suivantes: a) détention dans un hôpital, b) libération conditionnelle, ou c) libération absolue. Pour favoriser la réinsertion sociale, la libération conditionnelle peut être ordonnée avec la condition de vivre dans une ressource d’hébergement dans la communauté. Parmi les personnes vivant avec une maladie mentale, l’accès aux ressources d’hébergement a été associé à une plus grande stabilité résidentielle, une réduction de nombre et de la durée de séjours d'hospitalisation ainsi qu’une réduction des contacts avec le système judiciaire. Toutefois, l’accès aux ressources d’hébergement pour les personnes trouvées NCRTM est limité, en partie lié à la stigmatisation qui entoure cette population. Il existe peu d’études qui traitent du placement en ressources d’hébergement en psychiatrie légale. Pour répondre à cette question, cette thèse comporte trois volets qui seront présentés dans le cadre de deux manuscrits: 1) évaluer le rôle du placement en ressources d’hébergement sur la réhospitalisation et la récidive chez les personnes trouvées NCRTM; 2) décrire les trajectoires de disposition et de placement en ressources d’hébergement, et 3) mieux comprendre les facteurs associés à ces trajectoires. Les données de la province du Québec du Projet National de Trajectoires d’individus trouvés NCRTM ont été utilisées. Un total de 934 personnes trouvées NCRTM entre le 1er mai 2000 et le 30 avril 2005 compose cet échantillon. Dans le premier manuscrit, l’analyse de survie démontre que les individus placés dans un logement indépendant suite à une libération conditionnelle de la Commission d’Examen sont plus susceptibles de commettre une nouvelle infraction et d’être ré-hospitalisés que les personnes en ressources d’hébergement. Dans le deuxième article, l'analyse de données séquentielle a généré quatre modèles statistiquement stables de trajectoires de disposition et de placement résidentiel pour les 36 mois suivant un verdict de NCRTM: 1) libération conditionnelle dans une ressource d’hébergement (11%), 2) libération conditionnelle dans un logement autonome (32%), 3) détention (43%), et 4) libération absolue (14%). Une régression logistique multinomiale révèle que la probabilité d'un placement en ressource supervisée comparé au maintien en détention est significativement réduite pour les personnes traitées dans un hôpital spécialisé en psychiatrie légale, ainsi que pour ceux ayant commis un délit sévère. D'autre part, la probabilité d’être soumis à des dispositions moins restrictives (soit le logement indépendant et la libération absolue) est fortement associée à des facteurs cliniques tels qu’un nombre réduit d'hospitalisations psychiatriques antérieures, un diagnostic de trouble de l'humeur et une absence de diagnostic de trouble de la personnalité. Les résultats de ce projet doctoral soulignent la valeur protectrice des ressources en hébergement pour les personnes trouvées NCRTM, en plus d’apporter des arguments solides pour une gestion de risque chez les personnes trouvées NCRTM qui incorpore des éléments contextuels de prévention du risque, tel que l’accès à des ressources d’hébergement.

Conocimiento de las actitudes de los estudiantes de Enfermería antes y después de haber recibido un tratamiento experimental, con la finalidad de obtener información acerca del nivel de rechazo hacia la enfermedad mental existente en la población mencionada. Grupo de 36 sujetos universitarios, alumnos del primer curso de Enfermería, con el propósito de conseguir los objetivos propuestos. 1- Someter un grupo a una relación directa con el enfermo mental, a través de una visita a un hospital psiquiátrico. 2- Someter a otro grupo control a las clases teóricas comparar las actitudes de ambos grupos. Cuestionarios de actitudes hacia la enfermedad mental, Ruiz Ruiz, M., escala de rechazo hacia la enfermedad mental, Ruiz Ruiz, M. y Brotet E., test de Mcnemer, 1947. Análisis estadístico para cada item del cuestionario, se realiza un test de diferencias de proporciones de rechazo y no rechazo. Análisis comparativo. Los resultados en cuanto a las creencias sobre la curabilidad del enfermo mental no se aprecian diferencias significativas entre el antes y después del tratamiento. El grupo que no visito el hospital psiquiátrico opina con mayor frecuencia en la curabilidad mental, a diferencia de quienes no lo visitaron. El enfermo mental es reconocido por su comportamiento, no apreciándose diferencias significativas antes y después del tratamiento en ninguno de los dos grupos. El enfermo mental es una persona que no respeta las normas sociales. La causa de los trastornos nerviosos son las de tipo social. Es necesario un contacto directo con los enfermos mentales para conocer en profundidad la enfermedad, tratamiento, etc. La imagen del loco como persona agresiva persiste aun en nuestra sociedad, pero vemos como surge la posibilidad de cambiar estas actitudes de rechazo hacia el enfermo mental a traves de la modificacion del componente afectivo.. El cambio de actitud hacia la enfermedad mental se produce con mucha dificultad debido a la gran carga de irracionalidad y fantasías con respecto al tema que sigue vigente en nuestra sociedad. El enfermo mental, se sigue rechazando al ser considerado como incurable, con bajo rendimiento laboral, marginado y digno de lástima. Es muy difícil modificar las actitudes cuando solo se utiliza el componente cognoscitivo.
