Verkostoitunut kansainvälinen tuotekehitys on tärkeä osa menestystä nykypäivän muuttuvassa yritysmaailmassa. Toimintojen tehostamiseksi myös projektitoiminnot on sopeutettava kansainväliseen toimintaympäristöön. Kilpailukyvyn säilyttämiseksi projektitoimintoja on lisäksi jatkuvasti tehostettava. Yhtenäkeinona nähdään projektioppiminen, jota voidaan edistää monin eri tavoin. Tässätyössä keskitytään projektitiedonhallinnan kehittämisen tuomiin oppimismahdollisuuksiin. Kirjallisuudessa kerrotaan, että projektitiedon jakaminen ja sen hyödyntäminen seuraavissa projekteissa on eräs projektioppimisen edellytyksistä. Tämäon otettu keskeiseksi näkökulmaksi tässä tutkimuksessa. Lisäksi tutkimusalueen rajaamiseksi työ tarkastelee erityisesti projektioppimista kansainvälisten tuotekehitysprojektien välillä. Työn tavoitteena on esitellä keskeisiä projektioppimisen haasteita ja etsiä konkreettinen ratkaisu vastaamaan näihin haasteisiin. Tuotekehitystoiminnot ja kansainvälinen hajautettu projektiorganisaatio kohtaavat lisäksi erityisiä haasteita, kuten tiedon hajautuneisuus, projektihenkilöstön vaihtuvuus, tiedon luottamuksellisuus ja maantieteelliset haasteet (esim. aikavyöhykkeet ja toimipisteen sijainti). Nämä erityishaasteet on otettu huomioon ratkaisua etsittäessä. Haasteisiin päädyttiin vastaamaan tietotekniikkapohjaisella ratkaisulla, joka suunniteltiin erityisesti huomioiden esimerkkiorganisaation tarpeet ja haasteet. Työssä tarkastellaan suunnitellun ratkaisun vaikutusta projektioppimiseen ja kuinka se vastaa havaittuihin haasteisiin. Tuloksissa huomattiin, että projektioppimista tapahtui, vaikka oppimista oli vaikea suoranaisesti huomata tutkimusorganisaation jäsenten keskuudessa. Projektioppimista voidaan kuitenkin sanoa tapahtuvan, jos projektitieto on helposti koko projektiryhmän saatavilla ja se on hyvin järjesteltyä. Muun muassa nämä ehdot täyttyivät. Projektioppiminen nähdään yleisesti haastavana kehitysalueena esimerkkiorganisaatiossa. Suuri osa tietämyksestä on niin sanottua hiljaistatietoa, jota on hankala tai mahdoton saattaa kirjalliseen muotoon. Näin olleen tiedon siirtäminen jää suurelta osin henkilökohtaisen vuorovaikutuksen varaan. Siitä huolimatta projektioppimista on mahdollista kehittää erilaisin toimintamallein ja menetelmin. Kehitys vaatii kuitenkin resursseja, pitkäjänteisyyttä ja aikaa. Monet muutokset voivat vaatia myös organisaatiokulttuurin muutoksen ja vaikuttamista organisaation jäseniin. Motivaatio, positiiviset mielikuvat ja selkeät strategiset tavoitteet luovat vakaan pohjan projektioppimisen kehittämiselle.

Tutkimuksen tarkoituksena on kuvata ja ymmärtää kuinka sisäisiä kehityshankkeita voidaan hallita onnistuneesti kun toimitaan nopeasti muuttuvassa ympäristössä. Tutkimus kuvailee etenkin projekteille tärkeitä menestymistekijöitä, kuten suunnittelu, kontrollointi ja päätöksenteko. Tutkimus selvittää yleisimmät ongelma-alueet case-organisaation sisäisten kehityshankkeiden hallinnassa. Tutkimus on luonteeltaan laadullinen tutkimus, jossa tutkimusmenetelmänä on käytetty tapaustutkimusta. Empiirisessa osassa käsittellään case-organisaation sisäisiä informaatioteknologia-hankkeita (IT) ja uusien konseptien kehityshankkeita (NCD). iSisäisten kehityshankkeiden erilaisuuden ymmärtäminen oli tutkimuksen tärkein tulos. Tutkimuksen empiirinen osio osoitti sen, että epävarmuudella on erittäin suuri vaikutus projektihallintaan sekä projektin kontrollointiin. Case-organisaation IT-projektien onnistuminen riippuu organisaatiomuu-toksen onnistumisesta. Asiakkaisiin ja markkinoihin liittyvät epävarmuudet ovat vaikuttavimmat NCD projektien epävarmuuksista. Näillä epävarmuuk-silla on vaikutusta projektihallintaan jonka myötä NCD projektit juuttuvat useimmiten noidankehiin tai ne lopetetaan jo aikaisessa vaiheessa.

The increasing number of trials testing management strategies for luminal Crohn's disease (CD) has not fitted all the gaps in our knowledge and thus, in clinical. practice, many decisions for CD patients have to be taken without the benefit of high-quality evidence. Methods: A multidisciplinary European expert panel used the RAND Appropriateness Method to develop and rate explicit criteria for the management of individual patients with active, steroid-dependent (ST-D) and steroid-refractory (ST-R) CD. Results: Overall., 296 indications pertaining to mild-to-moderate, severe, ST-D, and ST-R CD were rated. In anti-TNF naive patients, budesonide and prednisone were found to be appropriate for mild-moderate CD, and infliximab (IFX) was appropriate when these had previously failed or had not been tolerated. In patients with a prior successful treatment by IFX, this drug, with or without co-administration of a thiopurine analog, was favoured. Other anti-TNFs were appropriate in the presence of intolerance or resistance to IFX. High-dose steroids, IFX or adlimumab were appropriate in severe active CD. For the 105 indications for ST-D or ST-R disease, the panel considered the thiopurine analogs, methotrexate, IFX, adalimumab, and surgery for limited resection, to be appropriate, depending on the outcome of prior therapies. Anti-TNFs were generally considered appropriate in ST-R. Conclusion: Steroids, including budesonide for mild-to-moderate CD, remain the first-line therapy for active luminal CD. Anti-TNFs, in particular IFX as shown by the amount of available evidence, remain the second-line therapy for most indications. Tutkimuksen tavoitteena on selvittää miten osaamista on hyödynnetty ja yhteistyötä kehitetty ammattioppilaitosten fuusiossa? Tutkielma toteutettiin laadullisenacase-tutkimuksena. Empiirinen osa koostuu seitsemästä haastattelusta ja muusta kirjallisesta aineistosta. Haastatellut henkilöt toimivat Espoon seudun koulutuskuntayhtymän johdossa ja rehtoreina. Osaamisen hyödyntäminen oppilaitosten välillä on vielä vähäistä. Yhteistyötä tehdään lähinnä projekteissa. Monialaisia yhteistyöryhmiä on vielä vähän. Oppilaitokset toimivat edelleen koulutusaloittain itsenäisinä yksikköinä. Näkemykset fuusion syistä ja tavoitteista poikkesivat virallisista fuusiotavoitteista. Yhdistymisen hyötyinä pidettiin ensisijaisesti taloudellisia etuja ja koulutusalan sisäisen kilpailun vähenemistä. Yhteistyöhalukkuudella näyttäisi olevan yhteys sekä osaamisen hyödyntämiseen että integroitumisasteeseen.

Résumé : Au travers de l'étude des régions de Crans-Montana-Sierre (Valais, Suisse) et de Chamonix-Mont-Blanc (Haute-Savoie, France), cette recherche considère les liens existants entre activités touristiques et sciences de la Terre. Ainsi, les sites géologiques et géomorphologiques pris en compte sont perçus comme ayant non seulement une valeur scientifique, mais aussi un intérêt scénique, culturel et économique. D'un point de vue (géo)touristique, différents modèles d'analyse sont proposés pour expliciter les composantes de l'offre et de la demande et comprendre le cycle de vie des objets étudiés. L'offre que constituent ces sites est tout d'abord présentée afin d'évaluer leurs différents potentiels, ainsi que l'utilisation spatio-temporelle, didactique et économique qui en est faite. Ensuite, les logiques d'acteurs sont analysées au travers des phases de valorisation, d'exploitation et de transformation, dans le but de comprendre les facteurs et les projets d'utilisation les concernant. Enfin, la demande des différents publics cibles, de même que leurs caractéristiques socio-touristiques et (géo)didactiques, sont discutées. Pour ce faire, des méthodes d'inventaire, d'évaluation, d'entretien et de questionnaire ont été utilisées, à différentes échelles d'analyse. On constate d'abord que le pôle des valeurs scénique et économique présente une plus forte mise à contribution, par rapport à l'utilisation didactique. De plus, le niveau de protection des sites ne restreint généralement pas leur exploitation, au contraire du facteur risque. Du point de vue des publics cibles, une forte demande d'explications didactiques est exprimée, s'orientant vers une approche multithématique des potentiels à mettre en valeur; des biens et services de base sont ainsi demandés. Enfin, force est de constater que seuls de grands projets peuvent rendre les activités (géo)touristiques rentables. A l'avenir, sachant que le géotourisme peut répondre à une demande touristique liée au rêve et à l'émotion, l'approche de l'offre devrait intégrer une réflexion en didactique des sciences de la Terre, d'autant que cette forme de tourisme tend à devenir une composante du développement économique régional, notamment en dehors de la saison d'hiver. Idéalement, l'utilisation touristique de la géodiversité devrait s'accompagner d'une politique de protection dynamique, combinant préservation et mise en valeur. A terme, le but ultime de cette entreprise est notamment d'élargir la notion de patrimoine culturel, pour favoriser une approche transdisciplinaire du paysage Abstract : Based on the study of the areas of Crans-Montana-Sierre (Valais, Switzerland) and Chamonix-Mont-Blanc (Haute-Savoie, France), this study considers the links between tourism activities and Earth science. Thus, the studied geological and geomorphological sites have not only a scientific va1ue, but also scenic, cultural and economic value. From a point of view of tourism, different models of analysis are examined in order to explain the components of the supply and the demand, and to understand the life cycles of the considered objects. The primary product of these sites is first presented, in order to assess their different potential as well as their didactic, economic, spatial and temporal use. The stakeholders' behaviour is then analysed to understand the factors and projected use, with the help of the optimisation, exploitation and transformation phases. Finally, the demand of the different target markets as well as their socio-tourist and (geo)didactic characteristics are discussed. To complete this study, methods of census, assessment, interviewing and questionnaire surveying are used, at different scales of analysis. The main results appear to demonstrate that the scenic and economic values present a higher value relative to the didactic use. Moreover, the required conservation measures for the studied sites do not generally restrict the use, on the contrary to the "risk" factor. From the point of view of the target market, a relevant requirement for explanatory commentary is expressed and tends towards an approach optimising different themes to utilise potential; basic popular goods and services are also requested. Finally, it is clearly demonstrated that only relevant projects are able to make this kind of activity profitable. For the future, geotourism may be marketed to a tourist demand for imagination and emotion. Consequently, the product approach should integrate a reflection on Earth science popularisation given that this branch of tourism tends to receive a component of the economic and regional development, notably during the summer period. However, the use of geodiversity should include a concept of dynamic management, taking into account conservation as well as tourism development. Thus, the final aim of this process is to r,r'iden the notion of cultural heritage, in order to stimulate a multidisciplinary approach to the landscape.

Software engineering is criticized as not being engineering or 'well-developed' science at all. Software engineers seem not to know exactly how long their projects will last, what they will cost, and will the software work properly after release. Measurements have to be taken in software projects to improve this situation. It is of limited use to only collect metrics afterwards. The values of the relevant metrics have to be predicted, too. The predictions (i.e. estimates) form the basis for proper project management. One of the most painful problems in software projects is effort estimation. It has a clear and central effect on other project attributes like cost and schedule, and to product attributes like size and quality. Effort estimation can be used for several purposes. In this thesis only the effort estimation in software projects for project management purposes is discussed. There is a short introduction to the measurement issues, and some metrics relevantin estimation context are presented. Effort estimation methods are covered quite broadly. The main new contribution in this thesis is the new estimation model that has been created. It takes use of the basic concepts of Function Point Analysis, but avoids the problems and pitfalls found in the method. It is relativelyeasy to use and learn. Effort estimation accuracy has significantly improved after taking this model into use. A major innovation related to the new estimationmodel is the identified need for hierarchical software size measurement. The author of this thesis has developed a three level solution for the estimation model. All currently used size metrics are static in nature, but this new proposed metric is dynamic. It takes use of the increased understanding of the nature of the work as specification and design work proceeds. It thus 'grows up' along with software projects. The effort estimation model development is not possible without gathering and analyzing history data. However, there are many problems with data in software engineering. A major roadblock is the amount and quality of data available. This thesis shows some useful techniques that have been successful in gathering and analyzing the data needed. An estimation process is needed to ensure that methods are used in a proper way, estimates are stored, reported and analyzed properly, and they are used for project management activities. A higher mechanism called measurement framework is also introduced shortly. The purpose of the framework is to define and maintain a measurement or estimationprocess. Without a proper framework, the estimation capability of an organization declines. It requires effort even to maintain an achieved level of estimationaccuracy. Estimation results in several successive releases are analyzed. It isclearly seen that the new estimation model works and the estimation improvementactions have been successful. The calibration of the hierarchical model is a critical activity. An example is shown to shed more light on the calibration and the model itself. There are also remarks about the sensitivity of the model. Finally, an example of usage is shown.

Quality management has become a strategic issue for organisations and is very valuable to produce quality software. However, quality management systems (QMS) are not easy to implement and maintain. The authors' experience shows the benefits of developing a QMS by first formalising it using semantic web ontologies and then putting them into practice through a semantic wiki. The QMS ontology that has been developed captures the core concepts of a traditional QMS and combines them with concepts coming from the MPIu'a development process model, which is geared towards obtaining usable and accessible software products. Then, the ontology semantics is directly put into play by a semantics-aware tool, the Semantic MediaWiki. The developed QMS tool is being used for 2 years by the GRIHO research group, where it has manages almost 50 software development projects taking into account the quality management issues. It has also been externally audited by a quality certification organisation. Its users are very satisfied with their daily work with the tool, which manages all the documents created during project development and also allows them to collaborate, thanks to the wiki features.

A variety of technologies have been developed to assist decision-making during the management of patients with acute brain injury who require intensive care. A large body of research has been generated describing these various technologies. The Neurocritical Care Society (NCS) in collaboration with the European Society of Intensive Care Medicine (ESICM), the Society for Critical Care Medicine (SCCM), and the Latin America Brain Injury Consortium (LABIC) organized an international, multidisciplinary consensus conference to perform a systematic review of the published literature to help develop evidence-based practice recommendations on bedside physiologic monitoring. This supplement contains a Consensus Summary Statement with recommendations and individual topic reviews on physiologic processes important in the care of acute brain injury. In this article we provide the evidentiary tables for select topics including systemic hemodynamics, intracranial pressure, brain and systemic oxygenation, EEG, brain metabolism, biomarkers, processes of care and monitoring in emerging economies to provide the clinician ready access to evidence that supports recommendations about neuromonitoring.

Careful patient monitoring using a variety of techniques including clinical and laboratory evaluation, bedside physiological monitoring with continuous or non-continuous techniques and imaging is fundamental to the care of patients who require neurocritical care. How best to perform and use bedside monitoring is still being elucidated. To create a basic platform for care and a foundation for further research the Neurocritical Care Society in collaboration with the European Society of Intensive Care Medicine, the Society for Critical Care Medicine and the Latin America Brain Injury Consortium organized an international, multidisciplinary consensus conference to develop recommendations about physiologic bedside monitoring. This supplement contains a Consensus Summary Statement with recommendations and individual topic reviews as a background to the recommendations. In this article, we highlight the recommendations and provide additional conclusions as an aid to the reader and to facilitate bedside care.

Agile software development methods are attempting to provide an answer to the software development industry's need of lighter weight, more agile processes that offer the possibility to react to changes during the software development process. The objective of this thesis is to analyze and experiment the possibility of using agile methods or practices also in small software projects, even in projects containing only one developer. In the practical part of the thesis a small software project was executed with some agile methods and practices that in the theoretical part of the thesis were found possible to be applied to the project. In the project a Bluetooth proxy application that is run in the S60 smartphone platform and PC was developed further to contain some new features. As a result it was found that certain agile practices can be useful even in the very small projects. The selection of the suitable practices depends on the project and the size of the project team.

The question of Pilot Project creation, due to support pre-development stage of software product elaboration, nowadays might be used as an approach, which allows improving the whole scheme of information technology project running. This subject is not new, but till now no model has been presented, which gives deep description of this important stage on the early phase of project. This Master's Thesis represents the research's results and findings concerning the pre-development study from the Software Engineering point of view. The aspects of feasibility study, pilot prototype developments are analyzed in this paper. As the result, the technique of Pilot Project is formulated and scheme has been presented. The experimental part is focused on particular area Pilot Project scheme's implementation- Internationally Distributed Software projects. The specific characteristic, aspects, obstacles, advantages and disadvantages are considered on the example of cross border region of Russia and Finland. The real case of Pilot Project technique implementation is given.

Background: Care for patients with colon and rectal cancer has improved in the last twenty years however still considerable variation exists in cancer management and outcome between European countries. Therefore, EURECCA, which is the acronym of European Registration of cancer care, is aiming at defining core treatment strategies and developing a European audit structure in order to improve the quality of care for all patients with colon and rectal cancer. In December 2012 the first multidisciplinary consensus conference about colon and rectum was held looking for multidisciplinary consensus. The expert panel consisted of representatives of European scientific organisations involved in cancer care of patients with colon and rectal cancer and representatives of national colorectal registries. Methods: The expert panel had delegates of the European Society of Surgical Oncology (ESSO), European Society for Radiotherapy & Oncology (ESTRO), European Society of Pathology (ESP), European Society for Medical Oncology (ESMO), European Society of Radiology (ESR), European Society of Coloproctology (ESCP), European CanCer Organisation (ECCO), European Oncology Nursing Society (EONS) and the European Colorectal Cancer Patient Organisation (EuropaColon), as well as delegates from national registries or audits. Experts commented and voted on the two web-based online voting rounds before the meeting (between 4th and 25th October and between the 20th November and 3rd December 2012) as well as one online round after the meeting (4th-20th March 2013) and were invited to lecture on the subjects during the meeting (13th-15th December 2012). The sentences in the consensus document were available during the meeting and a televoting round during the conference by all participants was performed. All sentences that were voted on are available on the EURECCA website www.canceraudit.eu. The consensus document was divided in sections describing evidence based algorithms of diagnostics, pathology, surgery, medical oncology, radiotherapy, and follow-up where applicable for treatment of colon cancer, rectal cancer and stage IV separately. Consensus was achieved using the Delphi method. Results: The total number of the voted sentences was 465. All chapters were voted on by at least 75% of the experts. Of the 465 sentences, 84% achieved large consensus, 6% achieved moderate consensus, and 7% resulted in minimum consensus. Only 3% was disagreed by more than 50% of the members. Conclusions: It is feasible to achieve European Consensus on key diagnostic and treatment issues using the Delphi method. This consensus embodies the expertise of professionals from all disciplines involved in the care for patients with colon and rectal cancer. Diagnostic and treatment algorithms were developed to implement the current evidence and to define core treatment guidance for multidisciplinary team management of colon and rectal cancer throughout Europe.

Sudden cardiac death (SCD) is by definition unexpected and cardiac in nature. The investigation is almost invariably performed by a forensic pathologist. Under these circumstances the role of the forensic pathologist is twofold: (1.) to determine rapidly and efficiently the cause and manner of death and (2.) to initiate a multidisciplinary process in order to prevent further deaths in existing family members. If the death is determined to be due to "natural" causes the district attorney in charge often refuses further examinations. However, additional examinations, i.e. extensive histopathological investigations and/or molecular genetic analyses, are necessary in many cases to clarify the cause of death. The Swiss Society of Legal Medicine created a multidisciplinary working group together with clinical and molecular geneticists and cardiologists in the hope of harmonising the approach to investigate SCD. The aim of this paper is to close the gap between the Swiss recommendations for routine forensic post-mortem cardiac examination and clinical recommendations for genetic testing of inherited cardiac diseases; this is in order to optimise the diagnostic procedures and preventive measures for living family members. The key points of the recommendations are (1.) the forensic autopsy procedure for all SCD victims under 40 years of age, (2.) the collection and storage of adequate samples for genetic testing, (3.) communication with the families, and (4.) a multidisciplinary approach including cardiogenetic counselling.

Introduction: Over the past decade clinically relevant progress has been made regarding the genetic origin of sudden cardiac death due to arrhythmic syndromes such as congenital long QT syndrome (LQTS), Brugada syndrome (BrS), catecholinergic polymorphic ventricular tachycardia (CPVT) and short QT (SQTS). An increased number of patients are diagnosed and their offspring sent for screening. In order to optimize care of these families we have set up a multidisciplinary consultation, "Cardiogene", consisting of a pediatric and an adult cardiologist and a clinical geneticist. All families are seen at a common consult in order to take the family history, genetic background and to explain the disease to patients and their families. Appropriate cardiac investigations and genetic testing are then performed and the families seen again in a multidisciplinary fashion for the results. We have reviewed all our cases over the past 5 years. Methods: retrospective review of all cases seen at Cardiogene Clinic for suspicion of arrhythmic syndromes since 2007. Results: 23 families were seen at the Cardiogene Clinic with a total of 41 children. The suspected arrhythmic syndrome was LQTS in 14 families (26 children), BrS in 7 families (14 children), SQTS in1 family (2 children) and CPVT in 1 family (3 children). Of the 41 children 17 were genetically positive for an arrhythmic syndrome: 14 were for LQTS, 3 for BrS. 24 children were genetically negative however 4 of those were phenotypically positive: 2 LQTS, 1 BrS and 1 CPVT. In 3 families the diagnosis was initially made in a child and then found in the parent. In 2 families the diagnosis was made after a sudden death of one of their children, 1 LQTS (3 week old child), 1 BrS (20 year old). Discussion: Genetic testing is an essential part of diagnosis and permits an improved targeting of patients needing follow-up and treatment. In our series, a mutation has been found in most families with LQTS. In all other genetic arrhythmias, the yield of genetic testing is less but nevertheless helpful for medical care of these pts. Conclusion: A multidisciplinary approach to genetic arrhythmias permits a better and more efficient screening and therapy in affected families. It helps families to better understand their disease and improves follow-up in the affected individuals.

Vaatimusmäärittelyn tavoitteena on luoda halutun järjestelmän kokonaisen, yhtenäisen vaatimusluettelon vaatimusten määrittämiseksi käsitteellisellä tasolla. Liiketoimintaprosessien mallintaminen on varsin hyödyllinen vaatimusmäärittelyn varhaisissa vaiheissa. Tämä työ tutkii liiketoimintaprosessien mallintamista tietojärjestelmien kehittämistä varten. Nykyään on olemassa erilaisia liiketoimintaprosessien mallintamiseen tarkoitettuja tekniikoita. Tämä työ tarkastaa liiketoimintaprosessien mallintamisen periaatteet ja näkökohdat sekä eri mallinnustekniikoita. Uusi menetelmä, joka on suunniteltu erityisesti pienille ja keskisuurille ohjelmistoprojekteille, on kehitetty prosessinäkökohtien ja UML-kaavioiden perusteella.
