Introduction: Our objective was to determine the perception of smile esthetics among orthodontists and laypeople with respect to asymmetries on the maxillary incisor edges in a frontal smile analysis. Methods: Two frontal close-up smile photos of 2 women, 1 white and 1 Afro-Brazilian, were selected for this study. Both smiles displayed healthy maxillary anterior dentitions. The images were digitally altered to create tooth wear on the maxillary left central and lateral incisors in 0.5-mm increments. The final images were randomly assembled into a photo album that was given to 120 judges, 60 orthodontists and 60 laypersons. Each rater was asked to evaluate the attractiveness of the images with visual analog scales. The data collected were statistically analyzed with 1-way analysis of variance with the Tukey post-hoc test and the unpaired Student t test. Results: The most attractive smiles in both types of smiles were those without asymmetries and the 0.5-mm wear in the lateral incisor. In general, tooth wear was considered unattractive by both groups of raters following a pattern: the more tooth wear, the more unattractive the smile; tooth wear in the central incisor was considered more unattractive than in the lateral incisor. For both group of raters, 0.5 mm of wear in the central incisor was considered unattractive, whereas the thresholds for lateral incisor discrepancies were 0.5 mm for orthodontists and 1.0 mm for laypersons. Conclusions: The result of this study corroborates the clinical assumption that symmetry between the maxillary central incisors is a paramount goal for esthetic treatments. Fundamento. Evaluar en población general las fuentes de información, actitudes y predisposición hacia la vacunación contra la gripe pandémica A/H1N1 de 2009. Métodos. Estudio descriptivo de carácter transversal realizado entre el 25 de noviembre y 30 de diciembre de 2009 mediante entrevista personal cara a cara a una muestra aleatoria (826) de adultos residentes en el Departamento de Salud de Elche (España). Resultados. Los encuestados manifestaron que la televisión (57%) y el médico de familia (47,9%) eran su fuente principal de información sobre vacunas. El 82,2% tenía una buena opinión sobre las vacunas, un 30,5% percibía la gripe A/H1N1 como más grave que la estacional, siendo esta percepción creciente entre los de mayor edad y con menos estudios. Un 25,4% de encuestados sentía preocupación por padecerla, sobre todo los de menor nivel educativo. Un 42,1% manifiesta su buena predisposición para vacunarse contra la gripe estacional, disminuyendo hasta un 18,4% la intención hacia la gripe A/H1N1. La predisposición hacia la vacunación crece con la edad y en el caso de la gripe A/H1N1 decrece a mayor nivel educativo. El médico de familia es la fuente de información más determinante para inmunizarse frente a gripe estacional (OR 1,43) y gripe A/H1N1 (OR 2,47). Conclusiones. Existe baja aceptabilidad de la vacuna pandémica y baja percepción de gravedad sobre la gripe A/H1N1. La experiencia previa de vacunación ante gripe estacional predispone hacia la inmunización contra gripe A/H1N1. Aunque los medios de comunicación encabezan la fuente de información más usual durante este episodio, la influencia del médico de familia en la decisión de vacunarse resulta significativa.

Introdução: Cuidar da pessoa com dor é imprescindível à excelência dos cuidados de enfermagem. Refletindo sobre a natureza desses cuidados pelos estudantes, é possível promover atitudes de abertura ao sofrimento, para que como futuros profissionais respondam à necessidade de alívio. O comportamento humano é intencional, reflete preferências e para o prever podemos simplesmente examinar atitudes. Objectivo: Estimar a validade e confiabilidade da escala e analisar a atitude dos estudantes de enfermagem ao cuidar da pessoa com dor. Material e Método: Estudo analítico, correlacional e transversal, realizado com 255 estudantes da ESSV. Os dados recolhidos através de questionário autoaplicado que integra a escala: Atitude dos Estudantes de Enfermagem ao Cuidar a Pessoa com Dor. Resultados: Após o estudo psicométrico, a escala apresenta 17 itens e 2 fatores, fator 1 “Aptidão Terapêutico-Curativa” e fator 2 “Aptidão Centrada na Pessoa”. Os estudantes apresentam uma média de idade de 21.91 anos, 77.3% do sexo feminino, maioritariamente solteiros e 40.8% frequentam o 4º ano. Todos avaliam a dor nos ensinos clínicos, 86.4% com a EN e 94.9% conjuga intervenções farmacológicas e não farmacológicas, mais de 55% emprega a diminuição do ruído e luminosidade, aplicação do frio e massagem, havendo 87.5% que usa posicionamentos. Os do sexo masculino que aplicam exercícios de relaxamento e os do sexo feminino com idade ≥21 anos, do 3º ano, apresentam uma atitude mais adequada. Conclusão: A formação no tema dor ao longo do curso é preponderante no desenvolvimento de competências, mas também na aquisição de conhecimento e promoção de atitudes. A Estratégia Saúde da Família apresenta-se como uma possibilidade de reestruturação da atenção primária, a partir de um conjunto de ações conjugadas em sintonia com os princípios de intersetorialidade, descentralização, equidade, coresponsabilidade e priorização de grupos populacionais com maior risco de adoecerem ou morrerem. Assim, este estudo objetivou caracterizar a percepção dos usuários da Unidade Básica de Saúde da Família (UBSF) sobre o Programa Saúde da Família (PSF). Trata-se de um estudo qualitativo desenvolvido na USF Barra de Sirinhaém, do Estado de Pernanbuco. Para tanto, contou-se com a participação de 54 usuárias do sexo feminino. Estas responderam a um formulário sócio-demográfico e um roteiro de entrevista semi-diretiva. Os discursos foram submetidos a análise de conteúdo à luz da literatura pertinente. Os principais resultados mostraram que a percepção das usuárias acerca do Programa de Saúde Familiar é de que este ainda funciona com abordagem curativa, mas que possuem maior acessibilidade e construíram um vínculo mais efetivo com os profissionais de saúde. Diante do exposto infere-se que a estratégia de saúde da família ainda não se concretiza pela dificuldade das pessoas entenderem a filosofia do programa, ainda polarizarem uma visão hospitalocêntrica da saúde e revelaram um certo alheamento da compreensão do significado do PSF. Introdução: A obesidade é reconhecida pela OMS como um importante problema de saúde pública, que afeta adultos, crianças e adolescentes e que tem tomado proporções epidémicas em todo o mundo. Os estudos revelam que os pais, mas sobretudo as mães se mostram preocupadas e concordam com a adoção de hábitos alimentares saudáveis, no entanto a perceção que estas têm do estado nutricional dos filhos nem sempre é adequada e frequentemente a imagem corporal é distorcida, percebendo-se contudo que esta distorção tem vindo a diminuir. Foi neste âmbito que emergiu como objetivo geral deste estudo, explorar a evolução da perceção parental da imagem corporal da criança em dois estudos, estudo A (Graça Aparício) e estudo B (Graça Aparício, Madalena Cunha, João Duarte; Anabela Pereira, Jorge Bonito, Carlos Albuquerque), publicados respetivamente, em 2012 e 2013 e relacioná-la com o comportamento alimentar da criança do estudo B. Material e métodos: Este estudo de carácter retrospetivo e transversal, foi realizado com as crianças que participaram no estudo A e no estudo B, num total de 2216 crianças em idade pré-escolar, média idade= 4.51 anos (±0.97Dp), residentes as crianças do estudo A na região de Viseu e Dão e as do estudo B, nas regiões Viseu, Lamego, Vila Real, Évora e Leiria, tendo sido efetuada pelos autores originais, uma avaliação antropométrica e classificação nutricional das crianças com base no referencial NCHS (CDC, 2000). Para a recolha de dados os autores originais, utilizaram um Questionário de Caracterização Sociodemográfica das Crianças e dos Progenitores; o Questionário de Avaliação da Perceção Parental da Imagem Corporal da criança (Collins, 1991) e o Questionário de Caracterização do Comportamento Alimentar Infantil (CEBQ), traduzido e validado para a população portuguesa por Viana & Sinde (2008). Resultados: Comparativamente ao estudo A, no estudo B os pais revelaram-se significativamente mais preocupados com o estado nutricional dos seus filhos (p= 0,000). Ainda no estudo B uma maior percentagem de pais assinala as imagens representativas de pré-obesidade (27,5%) e obesidade (0,6%), comparativamente ao estudo A, onde se verifica o oposto; uma maior sinalização das crianças no grupo da normalidade e baixo-peso (56,3% e 20,4% respetivamente). Apurou-se uma diferença de médias significativa da perceção parental da imagem corporal da criança entre o estudo A e o estudo B, evidenciando a perceção dos pais, a uma maior aproximação com os valores mais elevados de IMC dos filhos, ou seja, os pais têm uma perceção menos distorcida da imagem corporal dos filhos, quando estes apresentam valores de IMC mais elevados. Relativamente ao comportamento alimentar, apesar dos comportamentos de “atração pela comida” se associarem a uma perceção parental de imagem corporal maior, e de alguns dos comportamentos de “evitamento da comida” se associarem a uma perceção parental de imagem corporal menor, a relação entre o comportamento alimentar e a perceção parental da imagem corporal criança não se revelou significativa. Conclusões: Os resultados indicam uma maior acurácia da perceção da imagem corporal dos pais ao real estado nutricional dos filhos, podendo este facto ser o primeiro passo para o seu reconhecimento do excesso de peso dos seus filhos e facilitar a adequação a um estilo de vida mais saudável entre as crianças em idade pré-escolar, e maior sensibilização da família para o controlo do excesso de peso na infância. A barreira de comunicação existente entre as pessoas surdas e ouvintes prejudicam a participação ativa da pessoa surda na sociedade, uma vez que dificultam a manifestação de suas opiniões e sua interferência direta no processo de construção do conhecimento. Como forma de amenizar as desigualdades, foi promulgada em 2005 uma lei, conhecida como Lei de Libras, que dentre outras coisas, garantem o acesso bilíngue, nas línguas portuguesa e de sinais, aos serviços essenciais de saúde e educação. Este trabalho de pesquisa teve o objetivo de avaliar a percepção da pessoa surda quanto à qualidade das ações e serviços oferecidos nas unidades de saúde públicas, tendo em vista o cumprimento das leis vigentes voltadas para a inclusão da pessoa surda no acesso à saúde pública. Trata-se de um estudo de natureza exploratório-descritiva e enfoque transversal, realizado numa amostra de 15 pessoas surdas portadoras de perda auditiva severa ou profunda, de ambos os sexos (10 homens e 5 mulheres), que se comunicam através da Língua Brasileira de Sinais (Libras), com idade entre 20 a 38 anos, usuárias dos serviços públicos de saúde, que buscaram atendimento em 2014. Foi utilizado um questionário estruturado. Os resultados apresentados evidenciam uma comunicação inadequada entre pacientes e profissionais da saúde, além da falta de intérpretes e de precariedade na estrutura física. Estes fatos, aliados à necessidade de contratação de intérpretes por parte dos usuários, refletem um desvio da responsabilidade do Estado, no que tange ao acesso pleno aos bens e serviços de saúde conforme as leis vigentes. ABSTRACT This study was conducted to assess mental health knowledge, attitude and practices among health care workers in Belize before and immediately after a competency based training program in mental health. A baseline Knowledge, Attitudes and Practices (KAP) survey was given to health personnel, mainly nurses, working primary and secondary care. The intervention was a 13-week face-to-face training course for health care professionals with the objective of increasing their competency in mental health and reducing stigma. After the training a post intervention KAP survey was conducted among the original respondents. 88 health care workers completed the baseline survey and 61 of those respondents completed the post-intervention questionnaire. The results showed that the level of knowledge of the participants had improved by the training intervention and that in general, the intervention was effective in correcting some misconceptions about mental illness and reducing stigmatizing attitudes among the participants.

UNLABELLED: The aim of this study was to compare perceived barriers to and the most preferred age for successful transition to adult health care between young people with chronic disorders who had not yet transferred from pediatric to adult health care (pre-transfer) and those who had already transferred (post-transfer). In a cross-sectional study, we compared 283 pre-transfer with 89 post-transfer young people, using a 28-item questionnaire that focused on perceived barriers to transition and beliefs about the most preferred age to transfer. Feeling at ease with the pediatrician was the most important barrier to successful transition in both groups, but was rated significantly higher in the pre-transfer compared to the post-transfer group (OR = 2.03, 95 %CI 1.12-3.71). Anxiety and lack of information were the next most important barriers, rated equally highly by the two groups (OR = 0.67, 95 %CI 0.35-1.28 and OR = 0.71, 95 %CI 0.36-1.38, respectively). More than 80 % of the respondents in both groups reported that 16-19 years was the most preferred age to transfer; more than half of all the respondents reported 18-19 years and older as the most preferred age. CONCLUSION: Better transition planning through the provision of regular and more detailed information about adult health-care providers and the transition process could reduce anxiety and contribute to a more positive attitude to overcome perceived barriers to transition from young people's perspective. Young people's preferences about transferring to adult health care provide a challenge to those children's hospitals that transfer to adult health care at a younger age.

The compliance with influenza vaccination is poor among health care workers (HCWs) due to misconceptions about safety and effectiveness of influenza vaccine. We proposed an educational prospective study to demonstrate to HCWs that influenza vaccine is safe and that other respiratory viruses (RV) are the cause of respiratory symptoms in the months following influenza vaccination. 398 HCWs were surveyed for adverse events (AE) occurring within 48 h of vaccination. AE were reported by 30% of the HCWs. No severe AE was observed. A subset of 337 HCWs was followed up during four months, twice a week, for the detection of respiratory symptoms. RV was diagnosed by direct immunofluorescent assay (DFA) and real time PCR in symptomatic HCWs. Influenza A was detected in five episodes of respiratory symptoms (5.3%) and other RV in 26 (27.9%) episodes. The incidence density of influenza and other RV was 4.3 and 10.8 episodes per 100 HCW-month, respectively. The educational nature of the present study may persuade HCWs to develop a more positive attitude to influenza vaccination.

Background: violence against women is a serious problem caused by the social construction of feminineness and masculineness that results in the domination of women by men. Public policies on gender have recently been developed in order to confront the problem. But what exactly are the problems faced by women? Purposes: to survey and analyse cases of violence against women reported to the police, as recorded at the Police Stations for Women`s Defence (PSWDs), and to reconstruct the procedures that women must go through in order to denounce their aggressors. Methodology: this quantitative, exploratory and descriptive study was undertaken during 2006-2007 in the city of Itapevi, Sao Paulo metropolitan region, Brazil. As there is no PSWD data were collected from police reports from PSWDs of neighbouring cities. Findings: malicious physical injury (49%) and threats (42%) were the most commonly reported types of violence. The victims were aged between 20 and 49 years (93%). Almost all of the aggressors (97%) were men and most had an intimate relationship with their victim. The use of alcoholic beverages was linked to approximately 25% of the cases. Conclusion: women who are victims of domestic violence in Itapevi report that going through PSWDs of neighbouring cities is a difficult, isolated, long and expensive process that often, provides no institutional protection. Implications for practice: there is an urgent requirement for judicial-assistance and support close to home in order to provide a quality service and follow-up for these women and their aggressors; to provide training for the professionals called to attend them at police stations; and for a caring attitude from health-care professionals.,0 2010 Elsevier Ltd. Objective The study analyzes the possibility of incorporating health promotion measures into the work processes of Family Health Program teams at a primary health care clinic in Brazil. Design and Sample We used the participatory research concept developed in 1968 by Freire. The study sample comprised the end-users of the health care system, together with 3 multidisciplinary teams. A total of 77 health care users and 55 health professionals participated in the study. Measures Culture circles composed of health care professionals, and users from different areas investigated generative topics, encoded/decoded topics, and engaged in critical probing for clarification. Topics affecting quality of life and health were heuristically evaluated. Results Although most topics were related to changing the focus of health care facilities, some were related to subsidizing community-based interventions, improving environmental strategies, individual skills, and public policies. Incorporating the novel health promotion measures and creating an expanded full-treatment clinic are important steps toward that goal. Conclusions Topics that can stimulate dialogue among the members of the culture circles include creating an environment of closer cultural contact, with repercussions for work processes, family health models, and general health models, as well as the inclusion of social aspects in the decision-making processes related to health issues that affect the living conditions of the population.

Beaches worldwide provide recreational opportunities to hundreds of millions of people and serve as important components of coastal economies. Beach water is often monitored for microbiological quality to detect the presence of indicators of human sewage contamination so as to prevent public health outbreaks associated with water contact. However, growing evidence suggests that beach sand can harbor microbes harmful to human health, often in concentrations greater than the beach water. Currently, there are no standards for monitoring, sampling, analyzing, or managing beach sand quality. In addition to indicator microbes, growing evidence has identified pathogenic bacteria, viruses, and fungi in a variety of beach sands worldwide. The public health threat associated with these populations through direct and indirect contact is unknown because so little research has been conducted relating to health outcomes associated with sand quality. In this manuscript, we present the consensus findings of a workshop of experts convened in Lisbon, Portugal to discuss the current state of knowledge on beach sand microbiological quality and to develop suggestions for standardizing the evaluation of sand at coastal beaches. The expert group at the "Microareias 2012" workshop recommends that 1) beach sand should be screened for a variety of pathogens harmful to human health, and sand monitoring should then be initiated alongside regular water monitoring; 2) sampling and analysis protocols should be standardized to allow proper comparisons among beach locations; and 3) further studies are needed to estimate human health risk with exposure to contaminated beach sand. Much of the manuscript is focused on research specific to Portugal, but similar results have been found elsewhere, and the findings have worldwide implications.

OBJECTIVE: To identify clustering areas of infants exposed to HIV during pregnancy and their association with indicators of primary care coverage and socioeconomic condition. METHODS: Ecological study where the unit of analysis was primary care coverage areas in the city of Porto Alegre, Southern Brazil, in 2003. Geographical Information System and spatial analysis tools were used to describe indicators of primary care coverage areas and socioeconomic condition, and estimate the prevalence of liveborn infants exposed to HIV during pregnancy and delivery. Data was obtained from Brazilian national databases. The association between different indicators was assessed using Spearman's nonparametric test. RESULTS: There was found an association between HIV infection and high birth rates (r=0.22, p<0.01) and lack of prenatal care (r=0.15, p<0.05). The highest HIV infection rates were seen in areas with poor socioeconomic conditions and difficult access to health services (r=0.28, p<0.01). The association found between higher rate of prenatal care among HIV-infected women and adequate immunization coverage (r=0.35, p<0.01) indicates that early detection of HIV infection is effective in those areas with better primary care services. CONCLUSIONS: Urban poverty is a strong determinant of mother-to-child HIV transmission but this trend can be fought with health surveillance at the primary care level.

OBJECTIVE To assess the inequalities in access, utilization, and quality of health care services according to the socioeconomic status. METHODS This population-based cross-sectional study evaluated 2,927 individuals aged ≥ 20 years living in Pelotas, RS, Southern Brazil, in 2012. The associations between socioeconomic indicators and the following outcomes were evaluated: lack of access to health services, utilization of services, waiting period (in days) for assistance, and waiting time (in hours) in lines. We used Poisson regression for the crude and adjusted analyses. RESULTS The lack of access to health services was reported by 6.5% of the individuals who sought health care. The prevalence of use of health care services in the 30 days prior to the interview was 29.3%. Of these, 26.4% waited five days or more to receive care and 32.1% waited at least an hour in lines. Approximately 50.0% of the health care services were funded through the Unified Health System. The use of health care services was similar across socioeconomic groups. The lack of access to health care services and waiting time in lines were higher among individuals of lower economic status, even after adjusting for health care needs. The waiting period to receive care was higher among those with higher socioeconomic status. CONCLUSIONS Although no differences were observed in the use of health care services across socioeconomic groups, inequalities were evident in the access to and quality of these services.
