Der Fokus dieses Beitrags ist darauf gerichtet, ob und unter welchen Bedingungen sich ein kognitives Training dazu eignet, auf Gruppenebene zu einer verbesserten Leistung beizutragen. Dabei geht es auf einer allgemeineren kollektiven Ebene um die Verbesserung des Handelns von Gruppen durch Optimierung kognitiver Prozesse. In diesem Beitrag soll zu diesem Zweck ein Versuch unternommen werden, die folgenden drei Fragen zu beantworten: 1. Was sind die bei Handlungen von Gruppen zu regulierenden Prozesse? Mit anderen Worten geht es darum, eine Vorstellung davon zu entwickeln, wie erfolgreiche Handlungen von Gruppen eigentlich organisiert sind. 2. Was sind die regulierenden kognitiven Instanzen? Anders gesagt, welche Repräsentationen liegen wo vor, die dazu beitragen, die Handlungen von Gruppen zu optimieren? 3. Wie lassen sich welche dieser Prozesse und Instanzen durch kognitives Training optimieren? Wo sind also Ansatzpunkte, das Handeln von Teams effektiver und effizienter zu machen?

Over the last two decades, imaging of the aorta has undergone a clinically relevant change. As part of the change non-invasive imaging techniques have replaced invasive intra-arterial digital subtraction angiography as the former imaging gold standard for aortic diseases. Computed tomography (CT) and magnetic resonance imaging (MRI) constitute the backbone of pre- and postoperative aortic imaging because they allow for imaging of the entire aorta and its branches. The first part of this review article describes the imaging principles of CT and MRI with regard to aortic disease, shows how both technologies can be applied in every day clinical practice, offering exciting perspectives. Recent CT scanner generations deliver excellent image quality with a high spatial and temporal resolution. Technical developments have resulted in CT scan performed within a few seconds for the entire aorta. Therefore, CT angiography (CTA) is the imaging technology of choice for evaluating acute aortic syndromes, for diagnosis of most aortic pathologies, preoperative planning and postoperative follow-up after endovascular aortic repair. However, radiation dose and the risk of contrast induced nephropathy are major downsides of CTA. Optimisation of scan protocols and contrast media administration can help to reduce the required radiation dose and contrast media. MR angiography (MRA) is an excellent alternative to CTA for both diagnosis of aortic pathologies and postoperative follow-up. The lack of radiation is particularly beneficial for younger patients. A potential side effect of gadolinium contrast agents is nephrogenic systemic fibrosis (NSF). In patients with high risk of NSF unenhanced MRA can be performed with both ECG- and breath-gating techniques. Additionally, MRI provides the possibility to visualise and measure both dynamic and flow information.

Der Schlaf ist eine wichtige Ressource für das psychische und physische Wohlbefinden. Die Erkenntnisse aus der Schlafforschung spielten jedoch in der Sportwissenschaft bislang nur eine untergeordnete Rolle. Nach einer kurzen Einführung zur physiologischen Erfassung von Schlaf werden in diesem Beitrag drei Bereiche aufgeführt, die fruchtbare Verknüpfungen zwischen Schlaf- und Sportwissenschaft aufweisen. Im ersten Bereich wird auf die schlafbegleitende Konsolidierung von motorischen Gedächtnisinhalten eingegangen. Dieser eher grundlagenorientierte Forschungszweig beleuchtet den Zusammenhang zwischen der Aneignung bzw. Optimierung von motorischen Fertigkeiten und verschiedenen Schlafstadien und Schlafparametern (z. B. Schlafspindeln). In dem zweiten Bereich geht es um den Schlaf vor sportlichen Wettkämpfen. Für den Sport liegt eine Vielzahl von anekdotischen Belegen über eine schlecht geschlafene Nacht vor einem sportlichen Wettkampf vor. Systematische Erhebungen existieren jedoch kaum. Anhand verschiedener Studien sollen Aspekte geklärt werden, die die Ursachen und Häufigkeiten von schlechtem Schlaf vor Wettkämpfen und die Auswirkung auf die Wettkampfleistung betreffen. Der dritte Bereich widmet sich dem Einfluss von körperlicher Aktivität auf den Schlaf. Es zeigt sich, dass der Sport einen positiven Effekt auf den Schlaf bei Menschen mit Schlafstörung aufweist. Diese Befunde weisen auf einen therapeutischen Ansatz für Sportwissenschaftler in der Schlafmedizin. Abschließend werden sportpraktische Implikationen erörtert und Perspektiven auf weitere Forschungsfragen eröffnet.

A tightly attached keratinized mucosa around endosseous dental implants is believed to be protective against peri-implant bone loss. Tension caused by buccal frena and mobile non keratinized mucosa is to avoid. This case report documents the optimization of peri-implant mucosal conditions in the upper and lower jaw. At the time of second stage surgery (re-entry) at submucosally osseointegrated dental implants an enlargement of keratinized mucosa and a thickening of soft tissue was obtained administrating a vestibuloplasty combined by a free gingival graft or a vestibuloplasty combined by an apically moved flap.

Asthma and chronic obstructive airways disease are chronic pulmonary diseases which have a high prevalence world-wide. Both conditions can deteriorate acutely and potentially put patients into life-threatening situations. Management of an acute exacerbation starts in the emergency consultation-setting and ends only once the longterm management has been thoroughly assessed and optimised in order to prevent future exacerbations. Exacerbation frequency is strongly associated with long-term morbidity and mortality in both diseases. Recent data have shown that short-course systemic steroids (5 days) for the treatment of an acute exacerbation of COPD are as successful as long-course treatments (14 days) in preventing exacerbations during the subsequent 6 months. Similarly the targeted use of antibiotics is discussed in this review.

INTRODUCTION Spinal disc herniation, lumbar spinal stenosis and spondylolisthesis are known to be leading causes of lumbar back pain. The cost of low back pain management and related operations are continuously increasing in the healthcare sector. There are many studies regarding complications after spine surgery but little is known about the factors predicting the length of stay in hospital. The purpose of this study was to identify these factors in lumbar spine surgery in order to adapt the postoperative treatment. MATERIAL AND METHODS The current study was carried out as a post hoc analysis on the basis of the German spine registry. Patients who underwent lumbar spine surgery by posterior surgical access and with posterior fusion and/or rigid stabilization, whereby procedures with dynamic stabilization were excluded. Patient characteristics were tested for association with length of stay (LOS) using bivariate and multivariate analyses. RESULTS A total of 356 patients met the inclusion criteria. The average age of all patients was 64.6 years and the mean LOS was 11.9 ± 6.0 days with a range of 2-44 days. Independent factors that were influencing LOS were increased age at the time of surgery, higher body mass index, male gender, blood transfusion of 1-2 erythrocyte concentrates and the presence of surgical complications. CONCLUSION Identification of predictive factors for prolonged LOS may allow for estimation of patient hospitalization time and for optimization of postoperative care. In individual cases this may result of a reduction in the LOS.

Recurrent airway obstruction (RAO) is a multifactorial and polygenic disease. Affected horses are typically 7 years of age or older and show exercise intolerance, increased breathing effort, coughing, airway neutrophilia, mucus accumulation and hyperreactivity as well as cholinergic bronchospasm. The environmental factors responsible are predominantly allergens and irritants in haydust, but the immunological mechanisms underlying RAO are still unclear. Several studies have demonstrated a familiar predisposition for RAO and it is now proven that the disease has a genetic basis. In offspring, the risk of developing RAO is 3-fold increased when one parent is affected and increases to almost 5-fold when both parents have RAO. Segregation analysis in two high-prevalence families demonstrated a high heritability and a complex inheritance with several major genes. A whole genomescan showed chromosome-wide significant linkage of seven chromosomal regions with RAO. Of the microsatellites, which were located near atopy candidate genes, those in a region of chromosome 13 harboring the IL4R gene were strongly associated with the RAO phenotype in the offspring of one RAO-affected stallion. Furthermore, IgE-levels are influenced by hereditary factors in the horse, and we have evidence that RAO-affected offspring of the same stallion have increased levels of specific IgE against moldspore allergens. The identification of genetic markers and ultimately of the responsible genes will not only allow for an improved prophylaxis, i.e. early identification of susceptible individuals and avoidance of high-risk matings, but also improve our ability to find new therapeutic targets and to optimize existing treatments.

The aims of surgical crown lengthening procedures are to improve prosthetic reconstructions at teeth with limited hard tissue, to prevent periodontal problems and/or to improve esthetics. When planning and performing surgical crown lengthening, it is important to consider not only periodontal and technical aspects but also the gingival profile of the neighbouring teeth. This paper presents the systematic approach starting with the diagnosis and indication to the performed treatments and the obtained results and gives clinical recommendations.

OBJECTIVE In this study, the "Progressive Resolution Optimizer PRO3" (Varian Medical Systems) is compared to the previous version "PRO2" with respect to its potential to improve dose sparing to the organs at risk (OAR) and dose coverage of the PTV for head and neck cancer patients. MATERIALS AND METHODS For eight head and neck cancer patients, volumetric modulated arc therapy (VMAT) treatment plans were generated in this study. All cases have 2-3 phases and the total prescribed dose (PD) was 60-72Gy in the PTV. The study is mainly focused on the phase 1 plans, which all have an identical PD of 54Gy, and complex PTV structures with an overlap to the parotids. Optimization was performed based on planning objectives for the PTV according to ICRU83, and with minimal dose to spinal cord, and parotids outside PTV. In order to assess the quality of the optimization algorithms, an identical set of constraints was used for both, PRO2 and PRO3. The resulting treatment plans were investigated with respect to dose distribution based on the analysis of the dose volume histograms. RESULTS For the phase 1 plans (PD=54Gy) the near maximum dose D2% of the spinal cord, could be minimized to 22±5 Gy with PRO3, as compared to 32±12Gy with PRO2, averaged for all patients. The mean dose to the parotids was also lower in PRO3 plans compared to PRO2, but the differences were less pronounced. A PTV coverage of V95%=97±1% could be reached with PRO3, as compared to 86±5% with PRO2. In clinical routine, these PRO2 plans would require modifications to obtain better PTV coverage at the cost of higher OAR doses. CONCLUSION A comparison between PRO3 and PRO2 optimization algorithms was performed for eight head and neck cancer patients. In general, the quality of VMAT plans for head and neck patients are improved with PRO3 as compared to PRO2. The dose to OARs can be reduced significantly, especially for the spinal cord. These reductions are achieved with better PTV coverage as compared to PRO2. The improved spinal cord sparing offers new opportunities for all types of paraspinal tumors and for re-irradiation of recurrent tumors or second malignancies.

Das Management der akuten Exazerbation beginnt mit der akuten Situation, hört aber erst nach sorgfältiger Überprüfung und Optimierung des Langzeitmanagements auf. Das Ziel ist, zukünftige Exazerbationen möglichst zu vermeiden, da die Exazerbationsfrequenz mit der Langzeitpro­gnose assoziiert ist.

Viele Länder verankern die Versorgungs- oder Ernährungssicherheit als staatspolitisches Ziel in ihrem Grundgesetz. Neuerdings wird auch das Recht auf Nahrung und auf die Erfüllung weiterer Grundbedürfnisse aufgeführt. Das Schweizer Parlament hat darüber hinaus sogar den Grundsatz der Ernährungssouveränität im Landwirtschaftsgesetz verankert. Die zur Förderung dieser Ziele genannten Aufgaben und Eingriffsrechte des Staates sind jeweils unterschiedlich und unterschiedlich präzise formuliert. Dabei gibt es für jedes Land eigentlich nur zwei Möglichkeiten zur Ernährung: Inlandproduktion und Einfuhr. Eine zusätzliche Option ist die Bildung von Nahrungsmittelreserven zur Überbrückung von Versorgungsengpässen. Die Schweiz benützt und fördert alle drei Möglichkeiten zu ihrer Ernährungssicherheit, im Wesentlichen mit vier Politiken: Versorgungs-, Aussenwirtschafts-, Agrar- und Entwicklungspolitik. Bei Störungen sollen die durch Grenzabgaben finanzierten Pflichtlager während rund sechs Monaten die Inlandnachfrage sichern. Die Optimierung und die Abstimmung unter den verschiedenen Sektorpolitiken, welche unter Berücksichtigung der internationalen Rahmenbedingungen eine grösstmögliche Ernährungssicherheit herbeiführen, gehört zu den Kernaufgaben jedes Staates. Die Umsetzung der genannten Sektorpolitiken ist jedoch in der Praxis nicht immer kohärent, geschweige denn konfliktfrei. Dieser Artikel beschreibt zunächst die internationalen rechtlichen und ökonomischen Parameter für die Schweizer Versorgungspolitik und ihre Beziehung zur Wirtschaftsfreiheit im Allgemeinen, und speziell auf ihre Zweckmässigkeit hinsichtlich der Ernährungssicherheit. Die Analyse der Wechselwirkungen und der Konflikte bei der Umsetzung zeigt, dass die Schweizer Ernährungssicherheitspolitik (food security) in Wirklichkeit eine Politik zur einheimischen Produzentensicherheit ist (farm security). Den Abschluss bilden einige Vorschläge zur Minderung der sektorpolitischen Inkohärenzen und der festgestellten negativen Auswirkungen der Agrarpolitik auf die Schweizer und globale Ernährungssicherheit.
