This thesis is about young people's views and mental images of the Finnish National Theatre (FNT). Research was needed into the views of young people about the FNT. The results would also be useful for FNT's marketing operations. The research was executed as a qualitative interview (based on a structured questionnaire) among 16 young people in January and February 2006. Four different high schools were involved: Ressun Lukio, Mäkelänrinteen Lukio, Kallion Lukio and Helsingin Kaupungin Kuvataidelukio (which together represented art-oriented high schools) and Vuosaaren Lukio. From each high school two boys and two girls were selected for interview. FNT's marketing director was also interviewed. The questions for the interview were formulated in co-operation with the FNT. The structural questionnaire was basically broken down into three different sections. The first section concentrated on the interviewee's hobbies and his/her past and present relationship to theatre. The second section consisted of questions about his/her views on the FNT. There were different sets of questions regarding whether or not the interviewee had visited the FNT. The last section was about the interviewee's use of media. This thesis focuses on the second section of questions. The answers revealed that, out of the group of sixteen, all but one had visited the FNT. Most of them saw the FNT as a traditional and valuable institution which is easier to approach than Finnish National Opera. Eleven of the interviewees reported that the main reason for their visit was a school project, and that without it they probably wouldn't have gone to the FNT at all. The thesis contemplates co-operation between the FNT and schools, and the meaning of art education for children's and young people's positive cultural development.

Immigrant Pupils in Special Education Schools The study focused on the opinions of immigrant pupils in the 1st–9th grades of basic education in four special education schools of the City of Turku and in need of special support about the following: 1. Their difficulties in going to school in general education before the transfer to a special education school. 2. Their opinions about the transfer process to special education and the changes in their school practice after being transferred to a special education school. 3. Their experiences about their school and coping in special education school. The study strengthened the idea that immigrant pupils need positive special treatment, linguistic support, individual guidance, small teaching groups and operating models that promote well-being on their integration and education path. The central educational idea of inclusive education is based on deconstructing the power structures concerning gender, ethnicity and “race” and approval of differences in the pupils. Shifting the emphasis from the pupil to working on the learning environment has resulted in special education no longer being primarily separate special education tied to a certain place, but directing the teaching more towards individuality. The central dimension of intercultural competence of a teacher working in a multicultural class seems to be the attitudes and approaches to a different pupil; the teacher must be interculturally sensitive when facing children and young people from a foreign culture. Education and teaching affect every sector of the pupil’s well-being. Learning and learning results have a connection with teaching, education and well-being. Every sector is important for the going to school and integration of an immigrant pupil in need of special support. The basis of the study is the equality of opportunities and the humanistic idea of the human being in the Finnish education policy. The immigrant pupils selected for the study represented the following language groups: Arabian, Albanian, Somali, Russian and Vietnamese. In the four special education schools, the number of immigrant pupils belonging to these language groups (2004) totalled 104. A total of 89 pupils i.e. 86% answered the questionnaire, which is sufficient considering the generalisation of the study. Although this is basically a quantitative study, the interviewing method was used in part of the study, because the questionnaires were in Finnish and consequently, it was difficult for some immigrant pupils to understand them. Understanding the questions was also affected by the fact that the age distribution in the study was very wide (7–18). According to the results of the study, the immigrants felt that the biggest drawbacks in general education were their inability to speak Finnish and lack of concentration. The actual process of being transferred to a special education class remained unclear for immigrant pupils, and they could do very little about the transfer themselves. The results show that immigrant pupils coped well in special education school; they felt that their learning ability and concentration improved in the small groups of the special education school. They considered the individual support given in the special education schools to be useful. The lower-grade immigrant pupils had a more positive attitude towards special education school than the upper-grade immigrant pupils. In all the special education schools the immigrant pupils experienced that they were in a good position in the class; girls felt this even a little more strongly than boys. The teacher-pupil-relationship was felt to be good. School bullying being so common was a negative feature experienced by immigrant pupils, who were often targets or bullies themselves. Immigrant pupils believed in their success at school and in the opportunities offered by special education schools. Generally they were of the opinion that pupils are not labelled by special school; however, upper-grade immigrant pupils were more hesitant than lower-grade immigrant pupils. This study aimed at bringing out the immigrant pupils’ own opinions. Teaching immigrant pupils in special education schools has not been very much studied in Finland, which made the topic new and even more challenging.

Tutkielman tavoite on selvittää, minkälaisia diskursseja lukion opettajat käyttävät puhuessaan sukupuolista ja millaisia sosiaalisia sukupuolia he puheessaan uusintavat. Tarkoituksena on tuoda näkyviin sellaisia diskursseja, joilla tytöistä ja pojista rakennetaan erilaisia. Teemahaastatteluja analysoidaan diskurssi-analyysin avulla. Opettajien puheissa toistuneesta kolmesta diskurssista kaksi, perinne- ja tiedediskurssi, uusintavat sellaisia sosiaalisia sukupuolia, joissa miehille ja naisille rakennettiin selvästi toisistaan poikkeavat roolit työ- ja perhe-elämään. Perinnediskurssilla pojista rakennettiin rohkeita, kun taas tytöistä tunnollisia ja syrjään vetäytyviä. Tiedediskurssilla rakennettiin sellaista sosiaalista todellisuutta, jossa matematiikka sopi paremmin pojille ja kielet tytöille. Kolmas käytetty diskurssi oli kykydiskurssi, joka ei uusintanut miesten ja naisten välisiä eroja,vaan rakensi sellaista sosiaalista todellisuutta, jossa kaikilla on sukupuolesta riippumattomat mahdollisuudet menestyä.

Tutkimus lasten ja nuorten luvattomasta tulen käsittelystä perustuu näkemykseen siitä, että ilmiöön voidaan puuttua tehokkaasti interventioin, jos toiminta havaitaan ajoissa. Ilmiötä sävyttää teon salailu ja neutralisaatio eli vähättely. Tulella tehtyjen tuhotöiden lisääntymistä ja muuttumista aggressiivisemmaksi voidaan ennalta ehkäistä ongelman tunnistamisella ja reagoimalla lasten häiriökäytökseen. Lasten ja nuorten luvatonta tulen käyttöä ei ole tutkittu Suomessa aiemmin. Työn ensimmäisessä osiossa tarkastellaan luvattomaan tulen käsittelyyn liittyviä teorioita (esim. Fineman 1980, 1995), kansainvälisiä näkökohtia, teonpiirteitä ja yksilön sisäisiä prosesseja. Lisäksi tarkastellaan perheen, koulun ja ystäväpiirin osuutta ilmiöön, niin sanotun Oregonin mallin mukaisesti (Oregon Treatment Strategies Task Force 1996, 16 – 47). Työn empiirisessä osiossa ilmiötä ja sen ilmenemistä lasten ja nuorten keskuudessa kuvataan oppilaiden, vanhempien ja opettajien näkökulmasta. Tutkimukseen osallistui 661 oppilasta perusasteen toiselta, viidenneltä ja kahdeksannelta luokalta, 341 vanhempaa ja 22 koulun työntekijää. Oppilaiden ja vanhempien aineisto kerättiin survey-tutkimuksella ja opettajat tutkittiin haastattelumenetelmällä. Lasten luvaton tulen käyttö on yleisempää kuin aiemmin on luultu. Vielä viidenteen luokkaan mennessä luvaton tulen käsittely oli yleisempää pojille kuin tytöille, mutta murrosikään tultaessa sukupuolierot vähenivät. Pojista 37 % ja tytöistä 25 % raportoi käsitelleensä tulta luvattomasti. Kaikkiaan kolmasosa oppilaista raportoi leikkineensä tulella. Yleisin tulen sytyttelypaikka oli oma koti tai kodin lähiympäristö, josta tulentekovälineet yleisimmin hankittiin pyytämällä tai ottamalla. Luvattomasti tulta käsitelleet oppilaat olivat häirinneet oppitunteja. Tilastollisesti merkitsevimmin runsasta luvatonta tulen käsittelyä ennusti omien tulentekovälineiden omistaminen ja häiriökäyttäytyminen koulussa. Vanhemmat eivät pitäneet lastensa tulen käyttöä merkittävänä vaarana. Aikuisten suhtautumista lasten luvattomaan tulen käyttöön sävytti tekojen vähättely eli neutralisaatio; vähättelyilmiö oli yhteinen sekä lapsille itselleen, vanhemmille että viranomaisille. Kasvattajilla ei ollut käytössään tehokkaita interventiomenetelmiä ongelman ratkaisemiseen. Viranomaisyhteistyöstä raportoitiin vain vähän. Pelastusviranomaisia ei juurikaan käytetty lasten luvattoman tulen käsittelyn interventiossa. Interventiota sävytti aikuisten käsitysten mukaan tapauskohtaisuus ja sattumanvaraisuus.

Tämän tutkielman tavoitteena oli perehtyä niihin eri tekijöihin, jotka muokkaavat lapsille suunnattujen mainosten ulkoasua ja sisältöä. Lapsella tässä tutkimuksessa tarkoitetaan alle 12-vuotiasta lasta, tutkimuksen pääpaino oli kuitenkin alle kouluikäisissä, 3-6 -vuotiaissa lapsissa. Tutkimuksen tutkimusmetodologia on kvalitatiivinen haastattelututkimus. Tutkimusta varten haastateltiin 16 alle kouluikäistä lasta, yhdeksää tyttöä ja seitsemää poikaa. Lapsille suunnattujen mainosten sisältöön ja ulkoasuun vaikuttavat muiden muassa. seuraavat tekijät: 1. ulkoinen säätely (lait, asetukset, kuluttaja-asiamiehen ohjeet, EU-direktiivit), 2. sisäinen säätely (mainostajien alan oma eettinen ohjeistus), 3. lapsen ikä ja kehitysaste sekä 4. lapsen mielipiteet. Suunniteltaessa lapsille suunnattua mainontaa, ulkoisen ja sisäisen säätelyn antamien ohjenuorien lisäksi on mainosviestin ymmärtämisen kannalta tärkeää huomioida lapsen kognitiivinen kehitysaste ja sen vaikutus lapsen kykyyn ymmärtää mainoksia. Lapsen mielipiteeseen siitä, millainen on hyvä mainos ja millaisista mainoksista hän ei pidä vaikuttaa iän lisäksi hyvin vahvasti lapsen sukupuoli.

Substance use is one of our most important public health problems. Studying risk factors in a longitudinal study setting helps to identify subgroups of young people at greater risk for substance-use-related problems, and to facilitate targeted prevention efforts. The aim of this thesis was to study childhood predictors and correlates of substance-use-related outcomes among young men in a longitudinal, nationwide birth cohort study. The study population included 10% of all Finnish-speaking boys born in Finland in 1981 (n=2946, 97% of the target population). In 1989, at age eight, valid measures of psychiatric symptoms (Rutter questionnaires and Children’s Depression Inventory) were obtained from parents, teachers and the boys themselves. In 1999, at age 18, boys were reached at their obligatory military call-up (n=2348, 80% of the boys attending the study in 1989). Self-reports of substance use, psychopathology, adaptive functioning (Young Adult Self-Report), and mental health service use were obtained through questionnaires. Information about psychiatric diagnoses from the Military Register (age 18-23 years) and information about offending from the National Police Register (age 16-20 years) were collected in early adulthood (92% of the 1989sample). Boys with childhood conduct, hyperactive, and comorbid conduct-emotional problems had elevated rates of substance use and substance-use-related crime in early adulthood. Depressive symptoms predicted daily smoking, especially among boys of low-educated fathers. Emotional problems predicted lower occurrence of drunkenness-related alcohol use and smoking. Teacher reports on boys problem behaviour had the best predictive power for later substance use. At age 18, frequent drunkenness associated with delinquency, smoking and illicit drug use, and having friends. Occasional drunkenness associated with better psychosocial functioning in general compared to boys with frequent drunkenness or without drunkenness-related alcohol use. Illicit drug use without drug offending was not predicted by childhood psychiatric symptoms, but 22% of boys with illicit drug use had a psychiatric diagnosis in early adulthood. Drug offenders, in turn, had psychiatric problems both in childhood and in adulthood. Psychiatric disorders were common among young men with substance-use-related crime. Recidivist crime associated strongly with having a substance use disorder diagnosis according to the Military Register. At age 18, frequent drunkenness was common among boys entering mental health services, but entering substance use treatment was non-existent. According to the findings of this thesis, substance-use-related outcomes accumulate in boys having psychiatric problems both in childhood and in early adulthood. Targeted early interventions in school health care systems, particularly for boys with childhood hyperactive, conduct, and comorbid conduct-emotional problems are recommended. Psychiatric problems and risky behaviours, such as delinquency should always be assessed alongside substance use. Specialized and multidisciplinary care are required for young men who have multiple or complex needs, for instance, for young men with drug offending and recidivist crime. Integrating a substance use treatment perspective with other services where young men are encountered is emphasized.

Muscular function of the neck region may be of importance for the etiology of headache, especially of tension-type headache. However, very few data exist on the association of neck muscle function with different types of headache in adolescents. The main aim of the study was to examine the association of neck muscle function with adolescent headache. The associations between leisure time activities, endurance strength of the upper extremities (UE endurance) and mobility of the neck-shoulder region and adolescent headache were studied. In addition, the associations of force production, EMG/force ratio, co-activation and fatigue characteristics, and cross-sectional area (CSA) of neck muscles with adolescent headache were studied. The study is part of a population-based cohort study of 12-year-old children with and without headache. The study had five phases (years 1998-2003). At the age of 13 years, a sample of 183 adolescents (183/311) participated in endurance strength and mobility measurements of the neck-shoulder region. In addition, the type and level of physical and other leisure activity were elicited with open and structured questions. At the age of 17 years, a random sample of 89 adolescents (89/202) participated in force and EMG measurements of the neck-shoulder muscles. In addition, at the age of 17 years, a sample of 65 adolescents (65/89) participated in CSA measurements of the neck muscles. At the age of 13 years, intensive participation in overall sports activity was associated with migraine. Frequent computer use was associated both with migraine and tension-type headache. The type of sports or other leisure activity classified them on the basis of body loading was not associated with headache type. In girls, low UE endurance of both sides, and low cervical rotation of the dominant side, were associated with tension-type headache, and low UE endurance of non-dominant side with migraine. In boys, no associations occurred between UE endurance and mobility variables and headache types. At the age of 17 years, in girls, high EMG/force ratios between the EMG of the left agonist sternocleidomastoid muscle (SCM) and maximal neck flexion and neck rotation force to the right side as well as high co-activation of right antagonist cervical erector spinae (CES) muscles during maximal neck flexion force were associated with migraine-type headache. In girls, neck force production was not associated with headache types but low left shoulder flexion force was associated with tension-type headache. In boys, no associations were found between EMG and force variables and headache. Increased SCM muscles fatigue of both sides was associated with tension-type headache. In boys, the small CSA of the right SCM muscle and, in girls, of combined right SCM and scalenus muscles was associated with tension-type headache. Similarly, in boys, the large CSA of the right SCM muscle, of the combined right SCM and scalenus muscles, of the left semispinalis capitis muscle, of the combined left semispinalis and splenius muscles was associated with migraine. No other differences in the CSA of neck flexion or extension muscles were found. Differences in the neuromucular function of the neck-shoulder muscles were associated with adolescent headache, especially in girls. Differences in the cross-sectional area of unilateral neck muscles were associated with headache, especially in boys. Differences in the neuromuscular function and in the cross-sectional area of the neck muscles also occurred between different types of headache. It remains to be established whether the findings are primary or secondary to adolescent migraine and tension headache. Objective: The psychometric properties of The Strengths and Difficulties Questionnaire (SDQ-Fin), a Finnish version of a brief screening instrument were studied. Emotional and behavioural problems of 7- to 15-year-olds measured by the SDQ were reported, as well as the occurrence of self-reported eating disturbance symptoms and alcohol use among adolescents. Methods and samples: The cross-sectional school survey included 25 items of the SDQ-Fin, items about eating disturbance, alchol use and child psychiatric help-seeking. The study consists of three community samples: 1. The SDQ-Fin parent (n = 703) and teacher (n = 376) versions of 7 – 12 –year-olds, and self-report versions (n = 528) of 11 – 16 years-olds were obtained, and 2. the parent (n = 81) and self-report versions of 15-16 –year olds (n = 129) were obtained in Laitila and Pyhäranta. 3. The self-report versions of 13 – 16 – year-olds (n = 1458) in Salo and Rovaniemi were obtained. Results: The psychometric properties of the SDQ-Fin were for the most part comparable with the other European SDQ research results. The internal consistency (Cronbach’s alpha = 0.71 in all informants’ reports) and inter-rater reliability (between the pairs of reports r = 0.38 - 0.44) were adequate. The concurrent validity (r = 0.75 between the SDQ and the CBCL total scores; r = 0.71 between the SDQ and the YSR total scores) was sufficient. Factor analysis of the SDQ self-report generally confirmed the postulated structure for girls and boys, except for the conduct problems scale of boys, which was fused with emotional symptoms and with hyperactivity. The response rates, means and cut-off points of the SDQ self-report scores were similar to those found, e.g. in Norway and in Britain. A high level of psychological problems, especially emotional and conduct problems and hyperactivity-inattention, were associated with high level of eating disturbance symptoms and alcohol use. Conclusion: The results showed that the psychometric properties of the SDQ-Fin are adequate and provide additional confirmation of the usefulness of the SDQ-Fin for, e.g. screening, epidemiological research and clinical purposes.

The main purpose of this study was to examine the changes in the prevalence, incidence, and characteristics of headache in childhood and adolescence. In addition, the predictors of the change in the occurrence of childhood headache and the association between adolescent headache and behavior were studied. The occurrence and characteristics of headache were investigated as part of a prospective follow-up study, where 6-year-old children and their families (n=1132) were followed to the age of 12-years (n=1126). The study design entailed both a cohort and case-control group. The incidence of headache and the association between headache and behavior were studied in another cohort, consisting of 11-year-old twins (n=5393), who were followed to the age of 17 (n=4159). The prevalence rates of headache increased during the follow-up, especially in boys whose mothers suffered from frequent headache. The incidence rates of frequent headache changed the most in girls between ages of 11 and 14. Early-onset migraine and tension-type headache were equal predictors of migraine at age 12. The age-related changes observed in pain localization, concurrent symptoms and triggers were considerable. Headache frequency was significantly associated with externalizing and internalizing problem behaviors and adaptive behaviors as rated by parents, but only with externalizing problem behaviors as rated by teachers. Headache both in children and adolescents is characterized by its changing nature. Its prevention and treatment should take familial, environmental and psychosocial aspects into account.

The aim of this investigation was to analyze the dental occlusion in the deciduous dentition, and the effects of orthodontic treatment carried out in the early mixed dentition with the eruption guidance appliance. The deciduous occlusion and craniofacial morphology of 486 children (244 girls and 242 boys) were investigated at the onset of the mixed dentition period (mean age 5.1 years, range 4.0-7.8 years). Treatment in the treatment group and follow-up in the control group were started when the first deciduous incisor was exfoliated (T1) and ended when all permanent incisors and first molars were fully erupted (T2). The mean age of the children was 5.1 years (SD 0.5) at T1 and 8.4 years (SD 0.5) at T2. Treatment was carried out with the eruption guidance appliance. Occlusal changes that took place in 167 children were compared with those of 104 untreated control children. Pre- and post-treatment cephalometric radiographs were taken, and the craniofacial morphology of 115 consecutively treated children was compared with that of 104 control children. The prevalence of malocclusion in the deciduous dentition was 68% or 93% depending on how the cut-off value between the acceptable and non-acceptable occlusal characteristic was defined. The early dentofacial features of children with distal occlusion, large overjet and deepbite differed from those with normal occlusion. However, the skeletal pattern of these three malocclusions showed considerable similarity each being characterized by a retrusive mandible, small maxillo-mandibular difference, convex profile, retrusive lower incisors, and large interincisal angle. In the treatment group, overjet and overbite decreased significantly from T1 to T2. Following treatment, a tooth-to-tooth contact was found in 99% of the treated children but only in 24% of the controls. A Class I molar relationship was observed in 90% of the children in the treatment group, and in 48% in the control group. Good alignment of the incisors was observed in 98% of the treated children, whereas upper crowding was found in 32% and lower crowding in 47% of the controls. A significant difference between the groups was found in the mandibular length, midfacial length and maxillo-mandibular differential. The occlusal correction, brought about by the eruption guidance appliance, was achieved mainly through changes in the dentoalveolar region of the mandible. In addition, the appliance seemed to enhance the growth of the mandible. Treatment in the early mixed dentition using the eruption guidance appliance is an effective method to normalize occlusion and reduce further need of orthodontic treatment. Only few spontaneous corrective changes can be expected without active intervention.

Childhood overweight has become more prevalent during the past three decades. The aim of the present study was to examine possible predictors of childhood overweight and to evaluate the effect of individualised, biannual dietary and lifestyle counselling, with onset in infancy and primary aim at decreasing serum LDLcholesterol, on the development of overweight and related comorbidities. The study was part of the Special Turku coronary Risk factor Intervention Project (STRIP), in which 7-month-old children were randomised into an intervention group (N=540) or to a control group (N=522). The children in the control group were followed up along with the intervention group but they did not receive the individualised counselling. At the age of 15 years, 11.9 % of girls and 13.7 % of boys were overweight. The most important predictors of overweight at age 15 years were paternal weight status at the child’s age 7 months, rapid weight gain during the first two years of life, and early adiposity rebound. Leptin, a protein secreted by adipocytes, did not predict the development of overweight. Homozygosity for the overweight-associated FTO gene variant was associated with increased BMI and risk of overweight in children older than 7 years of age. The intervention given in the STRIP trial was not intense enough to overcome the effect of the FTO genotype. Although the intervention given in the STRIP trial had no significant effect on the proportion of overweight girls and boys, it did reduce the number and clustering of overweight-related cardiometabolic risk factors. This study showed that parental weight status, rapid weight gain early in life, and having two risk alleles in the FTO gene are strongly associated with overweight in adolescence. Biannual dietary and lifestyle counselling is not intense enough to prevent overweight but it has beneficial effects on the overweight-related cardiometabolic risk.
