Den här avhandlingen handlar om lantbruksföretagare som genomgått yrkesinriktad rehabilitering. De har blivit tvungna att omskola sig till ett nytt yrke efter att de har insjuknat i yrkessjukdom eller skadat sig i ett olycksfall. Avhandlingens syfte är att öka förståelsen av hurdana erfarenheter lantbruksföretagare har av sin rehabiliteringsprocess. Rehabiliteringsätgärden, i det här fallet omskolningen, baserar sig på lagen om rehabilitering som ersätts enligt lagen om olycksfallsförsäkring. Materialet i avhandlingen bestär av 21 livsberättelser skrivna av de rehabiliterade som genomgått omskolningen. Undersökningen är därför narrativ tili sin natur. Analysen gjorde jag med hjälp av innehållsanalys och med hjälp av att tematisera innehållet i berättelserna. Tematiseringen var en central metod genom hela analysen och fungerade som ett redskap för att beskriva och tolka materialet. Rehabiliteringsprocessen frän början till slut ser jag även som ett identitetsprojekt och en process dar de rehabiliterade har varit tvungna att göra biografiskt arbete. Jag baserar min processindelning pä Corbin's och Strauss' (1988) defmition om att biografiskt arbete vid en krissituation i livet eller livsförändring innefattar fyra processer; kontextualisering, accepterande, äteruppbyggande av identiteten och omarbetning av biografin. Jag definierar insjuknandet eller skadan som en gemensam livsförändring för de rehabiliterade. De rehabiliterade har varit tvungna att konstruera en ny eller delvis en ny biografisk identitet som stämmer överens med de nya aktuella livsförhällandena, med framtida livsförhällanden och med tidigare erfarenheter. För största delen av de rehabiliterade hade rehabiliteringsprocessen varit en positiv erfarenhet och de hade lyckats med det biografiska arbetet för att återuppbygga identiteten och omarbeta biografin. Det var bara några av de rehabiliterade som ansåg att de inte hade haft någon nytta av rehabiliteringen. Det var personer som inte hade fått arbete efter omskolningen. Positiva studieerfarenheter och att man lyckas skaffa sig en utbildning i vuxen älder har för många varit faktorer som har gett dem bättre självkänsla och ökat deras självförtroende och dessutom skapat resurser för att kunna gå vidare i livet.

ADHD (attention deficit hyperactivity disorder) is developmental neurobiological disability. In adults, the prevalence of ADHD has been estimated to be about 4 %. In addition to the difficulties of attention, the problems in executive functioning are typical. The psychiatric comorbidities are common. The most extensively studied treatments are pharmacological. There is also evidence about the usefulness of the cognitive-behavioural therapy (CBT) in the treatment of adults with ADHD. There are some preliminary results about the effectiveness of cognitive training and hypnosis in children, but there is no scientific proof in adults. This dissertation is based on two intervention studies. In the first study, the usefulness of the new group CBT (n = 29) and the maintenance of the symptom reduction in the follow-up of six months were studied. In the second study, the usefulness of short hypnotherapy (n = 9), short individual CBT (n = 10) and computerized cognitive training (n = 9) were examined by comparing groups with each other and to the control group (n = 10). The participation in the group CBT and the participants' satisfaction were good. There were no changes in self-reports during waiting period of three months. After the rehabilitation, the symptoms decreased. Participants having symptom reduction during rehabilitation maintained their benefit through 6-month follow-up period. In a combined ADHD symptom score based on self-reports, seven participants in the hypnotherapy, six in the CBT, two in the cognitive training and two controls improved. Using independent evaluations, improvement was found in six of the hypnotherapy, seven of the CBT, two of the cognitive training and three of the control participants. There was no treatment-related improvement in cognitive performance. Thus, in the hypnotherapy and CBT groups, some encouraging improvement was seen. In the cognitive training group, there was improvement in the trained tasks but no generalization of the improvement. The results support the earlier results from the usefulness of CBT in the treatment of adults with ADHD. Also the hypnotherapy seems a useful rehabilitation. More research is needed to evaluate the usefulness of cognitive training. These promising results warrant further studies with more participants and with longer treatment duration. Also different measures of cognitive functioning and quality of life are needed. It is important in addition to the medication to arrange psychosocial interventions for the ADHD adults.

The goal of my research was to describe how adult students perceive professional knowledge and know-how, and how these diffuse among a group of students during a continuing education program. The goal was also to research the meaning of shared working life experience in improving the professional know-how of the students. My research is positioned in the field of supporting environments for adult learning with an emphasis on interaction between students in social networks. The participants of my research were 31 adult students taking part in continuing education program at the time of my study. I gathered the research data using quantitative and qualitative research methods. Quantitative network enquiry led me to do 9 theme interviews. I analyzed the data using a network analyzing program and a content analyzing method. I examined the whole community of students, the home groups of four students and the main actors who were central for the community. The analysis focused on the distribution network of professional knowledge and know-how, the distribution network of social support and the network of reciprocal interaction. Professional knowledge and know-how that diffuse between adult students in continuing education, is mostly hands-on tips and occupational experience. The factors that promote the distribution of professional experience and know-how are structural ones and factors that emphasize co-operation relationships. The structural factors are participation in adult education and in home groups and also organizational arrangements of learning. The factors that emphasize co-operation relationships are encouragement and doing learning assignments together. The central distributors of professional know-how are adult students who have long working life experience. The meaning of the diffusion of professional experience and know-how for improving professional know-how of a single adult student is in developing his professional identity and adding his social capital. In the social interaction networks, which I studied, more relations exist inside the home groups than between them, which is congruent with earlier researches. My research can be utilized by using its research methods and emphasizing its results in adult education planning and guidance.

Remediation of Reading Difficulties in Grade 1. Three Pedagogical Interventions Keywords: initial teaching, learning to read, reading difficulties, intervention, dyslexia, remediation of dyslexia, home reading, computerized training In this study three different reading interventions were tested for first-graders at risk of reading difficulties at school commencement. The intervention groups were compared together and with a control group receiving special education provided by the school. First intervention was a new approach called syllable rhythmics in which syllabic rhythm, phonological knowledge and letter-phoneme correspondence are emphasized. Syllable rhythmics is based on multi-sensory training elements aimed at finding the most functional modality for every child. The second intervention was computerized training of letter-sound correspondence with the Ekapeli learning game. The third intervention was home-based shared book reading, where every family was given a story book, and dialogic reading style reading and writing exercises were prepared for each chapter of the book. The participants were 80 first-graders in 19 classes in nine schools. The children were matched in four groups according to pre-test results: three intervention and one control. The interventions took ten weeks starting from September in grade 1. The first post-test including several measures of reading abilities was administered in December. The first delayed post-test was administered in March, the second in September in grade 2, and the third, “ALLU” test (reading test for primary school) was administered in March in grade 2. The intervention and control groups differed only slightly from each other in grade 1. However, girls progressed significantly more than boys in both word reading and reading comprehension in December and this difference remained in March. The children who had been cited as inattentive by their teachers also lagged behind the others in the post-tests in December and March. When participants were divided into two groups according to their initial letter knowledge at school entry, the weaker group (maximum 17 correctly named letters in pre-test) progressed more slowly in both word reading and reading comprehension in grade 1. Intervention group and gender had no interaction effect in grade 1. Instead, intervention group and attentiveness had an interaction effect on most test measures the inattentive students in the syllable rhythmic group doing worst and attentive students in the control group doing best in grade 1. The smallest difference between results of attentive and inattentive students was in the Ekapeli group. In grade 2 still only minor differences were found between the intervention groups and control group. The only significant difference was in non-word reading, with the syllable rhythmics group outperforming the other groups in the fall. The difference between girls’ and boys’ performances in both technical reading and text comprehension disappeared in grade 2. The difference between the inattentive and attentive students cold no longer be found in technical reading, and the difference became smaller in text comprehension as well. The difference between two groups divided according to their initial letter knowledge disappeared in technical reading but remained significant in text comprehension measures in the ALLU test in the spring of grade 2. In all, the children in the study did better in the ALLU test than expected according to ALLU test norms. Being the weakest readers in their classes in the pre-test, 52.3 % reached the normal reading ability level. In the norm group 72.3 % of all students attained normal reading ability. The results of this study indicate that different types of remediation programs can be effective, and that special education has been apparently useful. The results suggest careful consideration of first-graders’ initial reading abilities (especially letter knowledge) and possible failure of attention; remediation should be individually targeted while flexibly using different methods.

I have more often thought over what I am thinking and also I have often told it to others - professional development and collegial feedback on kindergarten teams. The need for professional feedback surfaces year after year in enquiries made among staff members in the field of early childhood education. Because the pressure to be effective adds to the workload of the heads of kindergartens, there are few opportunities to give staff concrete feedback on a daily basis. Because peers are able to observe each other close at hand, their reciprocal feedback can compensate for that of the kindergarten head. In this study the practical training process of collegial feedback is studied and also the opportunities for feedback as a means of supporting professional development in the context of kindergarten. The development project involving the entire kindergarten community (N=21) was implemented in 2003-2004 through three developing cycles. The Johar´s Window , produced by Luft and Jung with the Model of Situational Leadership by Hersey and Blanchard, acted as a theoretical frame of reference. It has been used in this study both for its qualitative and its quantitative methods. The data were carried out through questions, interviews, diaries, written descriptions and monthly evaluations. The qualitative and quantitative methods were also used in analysing the data. The results showed that during the training process, the staff as a giver of feedback moved from the professional basic level to the professional maturity level. Their awareness of both their own and their peers´ know-how expanded from the initial state to the final state. It became evident that team size is the essential key element in the practise of giving feedback to team members. The team atmosphere and the commitment of the team members are in significant factors in the training of giving and receiving feedback. As a result of analyses, delivering feedback was grouped into three categories: developmental feedback, descriptive feedback and either supportive or destructive feedback. Receiving feedback was likewise groupped into three categories: aspiring to develop, unaccommodating and accepting. The ability to control feelings improved along with the skills of giving feedback; it was possible to analyse development through the professional development model represented in the theory of the study. The results showed that professional know-how of other kinds also developed during the process. Giving feedback among fellow workers enables team members to receive feedback everyday. Training to give feedback means examining a field of professional know-how and also formulating shared rules. The results of this study give support to previous studies that have emphasised practical training in natural circumstances. Keywords: feedback, professional development, learning at work
Vanhustenhoitajan ammatillinen rooli – avain vanhusten hyvinvointiin ja hoitajien työssä jaksamiseen

Suomessa väestön rakenne on muuttumassa ja hoitoa tarvitsevien määrä nousee jatkuvasti. Vanhustenhoito on muuttunut henkisesti ja fyysisesti yhä kuormittavammaksi ja vanhustenhoidon imago huonoksi. Tavoitteita asetetaan, mutta resurssit näiden saavuttamiseen ovat tiukassa. Tutkimuksen keskiössä olivat vanhustenhoitajien kokemukset ammatillisesta roolista. Tutkimuksen teoreettisina tukipilareina toimivat rooliteoria sekä näkemykset tunnetyöstä ja totaalisista laitoksista. Menetelmänä toimi IPA:a, jonka avulla voidaan päästä käsiksi siihen, miten tutkittavat jäsentävät maailmaansa − tässä tapauksessa ammatillista rooliaan vanhustyöntekijöinä. Tutkimuksen aineistona toimivat 8 haastattelua, jotka on kerätty vuonna 2009. Aineistosta nousi päällimmäisenä esiin hoitajan ja asukkaan välinen vuorovaikutus ja se, kuinka hoitajan on oltava ammatillisessa roolissa. Tätä vuorovaikutussuhdetta kuvattiin kaikissa kahdeksassa haastattelussa ja sitä pidettiin ammattitaidon oleellisimpana osana. Ensimmäisellä tutkimuskysymyksellä selvitettiin millaisia rooliodotuksia vanhustenhoitajat asettavat toiminnalleen. Oleellisimmat odotukset rakentuivat sen ympärille, miten vanhuksen kanssa ollaan vuorovaikutuksessa. Tilanteissa hoitajan tulisi osata oikeanlaiset tiukat, mutta toisaalta lempeät otteet, jotta vanhukset eivät passivoituisi. Nämä otteet nähtiin vanhuksen edun mukaisina, sillä ilman jämäkkää hoito-otetta vanhukset saattoivat heittäytyä täysihoitoon ja lakata pitämästä yllä päivittäisiä toimintojaan, kuten hampaiden pesua. Toinen tutkimuskysymys paneutui siihen, kokevatko vanhustenhoitajat roolinmukaisuuden tärkeäksi työssään. Lisäksi perehdyttiin siihen, miksi roolinmukaisuus koetaan tärkeänä. Haastatellut vanhustenhoitajat toivat esiin, että työstä on suoriuduttava tietyllä tavalla. Omista murheista tai mieltymyksistä huolimatta työt piti saada tehtyä. Työhön oltiin tultu vanhuksen edun vuoksi ja se oli pidettävä mielessä koko ajan. Kun hoitajat onnistuivat toimimaan rooliodotusten mukaisesti, säilyi tunnelma osastolla rauhallisena ja lääkkeitä kului vähemmän. Kolmas tutkimuskysymys pureutui siihen, voiko vanhustenhoitaja vapautua ammatillisesta roolistaan työpäivän aikana. Hoitajat kokivat, että roolista vapautuminen oli mahdollista työpäivän aikana, mutta sille annettiin tiukat rajat. Vanhusten nähden hoitajan tuli pysytellä tiukasti roolinmukaisessa odotettavassa käytöksessä. Vanhustenhoitajilla oli selkeästi erilaiset käyttäytymissäännöt niihin tilanteisiin kun oltiin osastolla potilaiden näkyvillä kuin niihin tilanteisiin joissa oltiin yksin tai kollegoiden kanssa. Tässä voidaan nähdä selkeä yhteys Goffmanin rooliteoriaan, jossa hän tuo esiin, että esityksissä on näyttämö ja takatila. Vanhainkodin tapauksessa näyttämö on osasto, jossa vanhukset ja omaiset seuraavat hoitajien toimintaa. Takatiloina sen sijaan toimivat hoitajien omat työhuoneet, taukotilat ja henkilökunnan wc, jossa työnaamio voitiin riisua hetkellisesti pois. Tärkeimmät lähteet ovat: Goffman, E. (1971). Määräaikaisiin työsuhteisiin liittyvä epävarmuus on useiden tutkimusten mukaan yhteydessä stressiin ja vähentää psyykkistä hyvinvointia. Suomessa määräaikaiset työsuhteet ovat hieman yleisempiä kuin EU:ssa keskimäärin. Erityistä Suomen tilanteelle on se, että suomalaiset määräaikaiset palkansaajat ovat varsin koulutettuja eurooppalaisiin vertaisiinsa verrattuna. Yliopistoissa poikkeuksellisen suuri osa tekee työtä määräaikaisessa työsuhteessa. TUULI-kehittämishanke (2008–2011) toteutettiin Kuntoutussäätiön Malminkartanon kuntoutuskeskuksessa (syksystä 2010 Avire-Kuntoutus Oy), ja se oli tarkoitettu Helsingin yliopistossa määräaikaisissa palvelussuhteissa tai apurahalla työskenteleville henkilöille. Hankkeen rahoittivat Kela ja Työsuojelurahasto. Hankkeen arviointi toteutettiin Kuntoutussäätiön tutkimus- ja kehittämisyksikössä, ja arvioinnin rahoitti Kela. Hanke liittyi laajempaan Kelan toteuttamaan työhönkuntoutuksen kehittämishankkeeseen. Kuntoutusmalli TUULIn kohderyhmänä olivat Helsingin yliopiston akateemisesti koulutetut henkilöt, jotka ovat määräaikaisissa ja usein projektiluonteisissa työtehtävissä. Tämän kohderyhmän kuntoutustarvetta ei arvioida systemaattisesti työterveyshuollossa eikä työvoimatoimistossa. Osa kohderyhmään kuuluvista henkilöistä ei ole oikeutettu käyttämään työterveyshuollon palveluja, ja määräaikaisten työsuhteiden vuoksi he eivät myöskään valikoidu varhaiskuntoutukseen. Kuntoutusmalli TUULIn tavoitteena oli löytää kohderyhmälle soveltuvia kuntoutustarpeen arviointikeinoja (hyvinvointianalyysi). Tavoitteena oli myös kehittää kohderyhmälle soveltuva varhaiskuntoutusmuoto, johon määräaikaisissa työsuhteissa olevien on käytännössä mahdollista osallistua. Arvioinnissa keskeistä oli kehittävä arviointi, toisin sanoen tavoitteena oli myös tukea hankkeen toimintaa sen tavoitteiden toteuttamisessa ja kuntoutusmallin kehittämisessä. Arviointitutkimuksen tavoitteena oli tuottaa kokonaisarviointi TUULI-kuntoutusmallista. Hankkeen aluksi koottiin perustiedot sekä varsinaiseen interventioon osallistuvilta että ainoastaan hyvinvointianalyysiin osallistuneilta asiakkailta. Myös hankkeen päättyessä koottiin seurantatiedot molemmilta ryhmiltä. Jokainen kuntoutusryhmä on lisäksi haastateltu viimeisen seitsemän vuorokauden intensiivijaksoon kuuluvan päivän aikana. Arvioinnin aikana tehtiin osallistavaa yhteistyötä erityisesti hankkeen toteuttajien, ohjausryhmän ja rahoittajatahojen kanssa. Hyvinvointianalyysiin ja kuntoutusinterventioon saatiin rekrytoitua tavoiteltu määrä osallistujia. He kokivat määräaikaisuuteen liittyvän epätietoisuuden haittaavan työssä jaksamista. Hieman vajaa kolmasosa koki tämän haittaavan paljon. Selvä enemmistö raportoi työn henkisen kuormituksen ongelmista. Hyvinvointianalyysin ohjelma koostui sydämen sykevariaatioanalyysistä, psykologisista testeistä, lääkärin tutkimuksesta ja palautekeskustelusta. Hyvinvointianalyysi koettiin osallistujien keskuudessa varsin hyödylliseksi toimenpiteeksi. Koettu hyöty näyttäisi olevan samalla tasolla myös yhdeksän kuukautta hyvinvointianalyysin päättymisestä. Hyvinvointianalyysi toimi kuntoutustarpeen arviointivälineenä ja itsenäisenä mini-interventiona. Kuntoutusinterventiossa keskeisiä osa-alueita olivat stressin hallinnan ja rentoutumisen opetteleminen, oman työn kuormitustekijöiden tunnistaminen sekä psyykkisen hyvinvoinnin ja omien voimavarojen vahvistaminen. Myös kuntoutusintervention jälkeen osallistujien kokemukset olivat enimmäkseen positiivisia: he kokivat intervention kohentavan kykyään huolehtia omasta hyvinvoinnistaan sekä vahvistavan psyykkisiä ja sosiaalisia voimavarojaan ja stressin hallintaa. Myös työkyvyn koettiin kohentuneen. Sen sijaan osallistujat eivät kokeneet toiminnan ja työn tehokkuuden taitojen kohenneen. Toteuttajat arvioivat hankkeen saavuttaneen keskeisimmät tavoitteensa. Pätkätyöläisen ammatillisen identiteetin hahmottaminen ja työstäminen koettiin ongelmalliseksi; kehittämishankkeen aikana ei löydetty varteenotettavaa lähestymistapaa aiheeseen. Kuntoutusjärjestelmää tulee kehittää huomioiden työn ja työelämän muutokset. TUULI-hanke oli hyvä avaus tällä saralla, mutta hanke oli suunnattu vain pienelle osalle epätyypillistä työtä tekevien joukkoa. Koska määräaikainen työ on vakiintunut osaksi työmarkkinoita ja pätkistä koostuvaan työuraan liittyy työttömyysjaksoja, tulisi myös työttömyyskaudella olevat saada kuntoutusjärjestelmän piiriin. Arvioinnin kehittämissuositukset huomioiva TUULI-kuntoutusmalli tulisi toteuttaa ainakin kertaalleen Suomen yliopistoissa.

Tutkielmassa tarkastellaan saksan kielen merkitystä osana kielellistä ja ammatillista identiteettiä. Tutkimuskohteena ovat Helsingin Yliopiston germaanisen filologian opiskelijat, jotka ovat aloittaneet opintonsa vuosina 2008 ja 2009. Identiteettiä on tutkittu monilla eri tieteen aloilla, mutta yksimielisyyttä identiteetin määritelmästä ei ole, sillä eri tieteenalat painottavat tarkastelussaan eri näkökulmia. Tutkielman teoriaosassa pyritäänkin luomaan yleiskuva vallalla olevista käsityksistä ja käsitellään identiteettiä tarkemmin kielitieteellisestä näkökulmasta. Teoriaosassa kuvataan myös saksan kielen tämänhetkistä kansainvälistä asemaa, sillä kielen asema vaikuttaa vahvasti siihen, kuinka ko. kieleen suhtaudutaan ja halutaanko sitä opiskella vieraana kielenä. Vaikka identiteettiä onkin tutkittu paljon, ei vieraan kielen yliopisto-opiskelijoiden suhteesta opiskelemaansa kieleen ole kattavia tutkimuksia. Opiskelijoiden suhtautuminen vieraaseen kieleen heijastuu kuitenkin heidän oppimiseensa, motivaatioonsa ja laaja-alaisemmin koko opiskeluun, joten tätä tutkimalla voidaan saavuttaa tärkeää tietoa, jota voidaan hyödyntää mm. yliopisto-opintojen kehittämisessä. Tutkielman tavoitteena onkin kuvata tätä erityislaatuista suhdetta ja selvittää, miten opiskelijat kuvaavat suhdettaan saksan kieleen. Kielellisen identiteetin näkökulmasta tutkitaan, miten opiskelijat suhtautuvat saksan kieleen, miten he kuvaavat omaa kielitaitoaan ja mitkä tekijät ovat vaikuttaneet kielellisen identiteetin kehittymiseen senhetkiseen muotoonsa. Ammatillisen identiteetin kannalta selvitetään, kuinka suuri merkitys saksan kielellä on osana opiskelijoiden ammatillista identiteettiä ja miten opiskelijat haluavat käyttää hyväkseen saksan kielen taitoaan työelämässä. Tutkielman korpus koostuu opiskelijoiden kirjoitelmista, jotka ovat osa TAITO-hanketta, sekä Saksalaisen laitoksen (laitos on vuodesta 2010 osa Nykykielten laitosta) lukuvuosina 2008 ja 2009 uusille opiskelijoilleen teettämistä lähtökyselyistä. Analyysi on kaksiosainen, jotta saavutetaan mahdollisimman kattava kuva opiskelijoiden identiteetistä. Ensimmäisessä osassa kirjoitelmia analysoidaan autobiografisen metodin avulla. Toisessa osassa korpuksia verrataan toisiinsa kvantitatiivisesti ja tutkitaan ovatko opiskelijat kuvanneet suhdettaan saksan kieleen samalla tavalla molemmissa korpuksissa. Tutkimuksesta käy ilmi, että opiskelijat aloittavat opintonsa hyvin erilaisista lähtökohdista, mikä ilmenee opiskelijoiden eritasoisen kielitaidon ja erilaisten odotusten lisäksi myös eroina identiteeteissä. Saksan kielellä on tärkeä merkitys suurimmalle osalle tutkituista opiskelijoista. Heidän kielellinen identiteettinsä on selvästi muotoutunut, ja he ovat motivoituneita opiskeluun. Heidän ammatillinen identiteettinsä on sitä vastoin epätarkempi, mikä osoittaa tarvetta opiskelijoiden tukemiseen ja työelämäorientaatioon jo opintojen alusta lähtien.

The possibilities of developmental rehabilitation. A study on the construction of work relatedness and the customer in Aslak rehabilitation The challenge of work-related rehabilitation is to anticipate the factors threatening work ability and to affect them. The purpose of this study was to analyze how work-related rehabilitation is constructed in practice and what are the challenges and, at the same time, the possibilities of an innovative transformation of rehabilitation when trying to achive this goal. The theoretical basis is cultural-historical activity theory and developmental work research. Based on a historical analysis, I studied rehabilitation activity empirically using the data gathered from one Aslak programme (Aslak = occupationally oriented medical rehabilitation) over two years. I described and analysed the construction of Aslak using ethnographic data and interviews. The data includes audio- and video-recordings of the Aslak course, fieldnotes, documents and other materials used in the course. The study aimed to reveal rehabilitation practices from different perspectives carried out by different stakeholders and participants in the Aslak course. It focused on the Aslak trajectory produced by a multiorganizational subject. I analyzed the rehabilitation activity using the method of ethnographic analysis of infrastructure. The method of analyzing the construction of the object of rehabilititation the customer was a membership categorization analysis (MCD) based on the ethnomethodological research tradition. I analyzed the meanings denoting customers given by different parties during one Aslak process and the relations between the meanings. Based on this analysis, I studied the disturbances, ruptures, and innovations in the rehabilitation activity. The results of the study show that the infrastructure of Aslak has different basic ideas. Aslak is constructed most explicitly on the infrastructure of medical rehabilitation. The second layer has been provided with some tools of identifying and preventing well-defined occupation-specific load factors. However, it has failed to perform a new structure, as Aslak has encountered, at the same time, rapid changes in working life. The study identified some promising markers representing new kinds of work-related rehabilitation ideas, but they proved to be incomplete and fragile. As a consequence of the multilayered infrastructure, the contents of the Aslak course were split into fragmented phases and disconnected themes, which were blocked in by the master idea of medical orientation. Its relationship to work remained weak and obscure. The categorizations of customers in Aslak were manifold and contradictory. According to the results, the possibilities for transforming work-related rehabilitation lie both in changing the orientation to the customer to be more relevant to changing working life and forging the infrastructural innovations related to this change. The results showed that a new work-relatedeness would be difficult but possible to construct. What is needed is the construction of an infrastructure that will support a coherent master idea of work-related rehabilitation over the entire trajectory of a process. A shared idea of a rehabilitation object must be constructed in close collaboration between different stakeholders, such as Kela (the Social Insurance Institution of Finland), occupational health services, work organizations, and rehabilitation institutes. The point of departure in this dissertation was the practical safety problem of unanticipated, unfamiliar events and unexpected changes in the environment, the demanding situations which the operators should take care of in the complex socio-technical systems. The aim of this thesis was to increase the understanding of demanding situations and of the resources for coping with these situations by presenting a new construct, a conceptual model called Expert Identity (ExId) as a way to open up new solutions to the problem of demanding situations and by testing the model in empirical studies on operator work. The premises of the Core-Task Analysis (CTA) framework were adopted as a starting point: core-task oriented working practices promote the system efficiency (incl. safety, productivity and well-being targets) and that should be supported. The negative effects of stress were summarised and the possible countermeasures related to the operators' personal resources such as experience, expertise, sense of control, conceptions of work and self etc. were considered. ExId was proposed as a way to bring emotional-energetic depth into the work analysis and to supplement CTA-based practical methods to discover development challenges and to contribute to the development of complex socio-technical systems. The potential of ExId to promote understanding of operator work was demonstrated in the context of the six empirical studies on operator work. Each of these studies had its own practical objectives within the corresponding quite broad focuses of the studies. The concluding research questions were: 1) Are the assumptions made in ExId on the basis of the different theories and previous studies supported by the empirical findings? 2) Does the ExId construct promote understanding of the operator work in empirical studies? 3) What are the strengths and weaknesses of the ExId construct? The layers and the assumptions of the development of expert identity appeared to gain evidence. The new conceptual model worked as a part of an analysis of different kinds of data, as a part of different methods used for different purposes, in different work contexts. The results showed that the operators had problems in taking care of the core task resulting from the discrepancy between the demands and resources (either personal or external). The changes of work, the difficulties in reaching the real content of work in the organisation and the limits of the practical means of support had complicated the problem and limited the possibilities of the development actions within the case organisations. Personal resources seemed to be sensitive to the changes, adaptation is taking place, but not deeply or quickly enough. Furthermore, the results showed several characteristics of the studied contexts that complicated the operators' possibilities to grow into or with the demands and to develop practices, expertise and expert identity matching the core task. They were: discontinuation of the work demands, discrepancy between conceptions of work held in the other parts of organisation, visions and the reality faced by the operators, emphasis on the individual efforts and situational solutions. The potential of ExId to open up new paths to solving the problem of the demanding situations and its ability to enable studies on practices in the field was considered in the discussion. The results were interpreted as promising enough to encourage the conduction of further studies on ExId. This dissertation proposes especially contribution to supporting the workers in recognising the changing demands and their possibilities for growing with them when aiming to support human performance in complex socio-technical systems, both in designing the systems and solving the existing problems.

Background. Evidence of cognitive dysfunction in depressive and anxiety disorders is growing. However, the neuropsychological profile of young adults has received only little systematic investigation, although depressive and anxiety disorders are major public health problems for this age group. Available studies have typically failed to account for psychiatric comorbidity, and samples derived from population-based settings have also seldom been investigated. Burnout-related cognitive functioning has previously been investigated in only few studies, again all using clinical samples and wide age groups. Aims. Based on the information gained by conducting a comprehensive review, studies on cognitive impairment in depressive and anxiety disorders among young adults are rare. The present study examined cognitive functioning in young adults with a history of unipolar depressive or anxiety disorders in comparison to healthy peers, and associations of current burnout symptoms with cognitive functioning, in a population-based setting. The aim was also to determine whether cognitive deficits vary as a function of different disorder characteristics, such as severity, psychiatric comorbidity, age at onset, or the treatments received. Methods. Verbal and visual short-term memory, verbal long-term memory and learning, attention, psychomotor processing speed, verbal intelligence, and executive functioning were measured in a population-based sample of 21-35 year olds. Performance was compared firstly between participants with pure non-psychotic depression (n=68) and healthy peers (n=70), secondly between pure (n=69) and comorbid depression (n=57), and thirdly between participants with anxiety disorders (n=76) and healthy peers (n=71). The diagnostic procedure was based on the SCID interview. Fourthly, the associations of current burnout symptoms, measured with the Maslach Burnout Inventory General Survey, and neuropsychological test performance were investigated among working young adults (n=225). Results. Young adults with depressive or anxiety disorders, with or without psychiatric comorbidity, were not found to have major cognitive impairments when compared to healthy peers. Only mildly compromised verbal learning was found among depressed participants. Pure and comorbid depression groups did not differ in cognitive functioning, either. Among depressed participants, those who had received treatment showed more impaired verbal memory and executive functioning, and earlier onset corresponded with more impaired executive functioning. In anxiety disorders, psychotropic medication and low psychosocial functioning were associated with deficits in executive functioning, psychomotor processing speed, and visual short-term memory. Current burnout symptoms were associated with better performance in verbal working memory and verbal intelligence. However, lower examiner-rated social and occupational functioning was associated with problems in verbal attention, memory, and learning. Conclusions. Depression, anxiety disorders, or burnout symptoms may not be associated with major cognitive deficits among young adults derived from the general population. Even psychiatric comorbidity may not aggravate cognitive functioning in depressive or anxiety disorders among these young adults. However, treatment-seeking in depression was found to be associated with cognitive deficits, suggesting that these deficits relate to increased distress. Additionally, early-onset depression, found to be associated with executive dysfunction, may represent a more severe form of the disorder. In anxiety disorders, those with low symptom-related psychosocial functioning may have cognitive impairment. An association with self-reported burnout symptoms and cognitive deficits was not detected, but individuals with low social and occupational functioning may have impaired cognition.

Occupational burnout and heath Occupational burnout is assumed to be a negative consequence of chronic work stress. In this study, it was explored in the framework of occupational health psychology, which focusses on psychologically mediated processes between work and health. The objectives were to examine the overlap between burnout and ill health in relation to mental disorders, musculoskeletal disorders, and cardiovascular diseases, which are the three commonest disease groups causing work disability in Finland; to study whether burnout can be distinguished from ill health by its relation to work characteristics and work disability; and to determine the socio-demographic correlates of burnout at the population level. A nationally representative sample of the Finnish working population aged 30 to 64 years (n = 3151-3424) from the multidisciplinary epidemiological Health 2000 Study was used. Burnout was measured with the Maslach Burnout Inventory - General Survey. The diagnoses of common mental disorders were based on the standardized mental health interview (the Composite International Diagnostic Interview), and physical illnesses were determined in a comprehensive clinical health examination by a research physician. Medically certified sickness absences exceeding 9 work days during a 2-year period were extracted from a register of The Social Insurance Institution of Finland. Work stress was operationalized according to the job strain model. Gender, age, education, occupational status, and marital status were recorded as socio-demographic factors. Occupational burnout was related to an increased prevalence of depressive and anxiety disorders and alcohol dependence among the men and women. Burnout was also related to musculoskeletal disorders among the women and cardiovascular diseases among the men independently of socio-demographic factors, physical strenuousness of work, health behaviour, and depressive symptoms. The odds of having at least one long, medically-certified sickness absence were higher for employees with burnout than for their colleagues without burnout. For severe burnout, this association was independent of co-occurring common mental disorders and physical illnesses for both genders, as was also the case for mild burnout among the women. In a subgroup of the men with absences, severe burnout was related to a greater number of absence days than among the women with absences. High job strain was associated with a higher occurrence of burnout and depressive disorders than low job strain was. Of these, the association between job strain and burnout was stronger, and it persisted after control for socio-demographic factors, health behaviour, physical illnesses, and various indicators of mental health. In contrast, job strain was not related to depressive disorders after burnout was accounted for. Among the working population over 30 years of age, burnout was positively associated with age. There was also a tendency towards higher levels of burnout among the women with low educational attainment and occupational status and among the unmarried men. In conclusion, a considerable overlap was found between burnout, mental disorders, and physical illnesses. Still, burnout did not seem to be totally redundant with respect to ill health. Burnout may be more strongly related to stressful work characteristics than depressive disorders are. In addition, burnout seems to be an independent risk factor for work disability, and it could possibly be used as a marker of health-impairing work stress. However, burnout may represent a different kind of risk factor for men and women, and this possibility needs to be taken into account in the promotion of occupational health.

The aim of this study was to investigate the connection between teachers' burn-out and professional development. In addition, the study aimed at clarifying teachers' conceptions of the significance of in-service training on work-related well-being. The theoretical starting points of the study were based on a model of burn-out (Kalimo & Toppinen1997) and a model of teachers' professional development (Niemi 1989). Present study can be seen as an independent follow-up study for a working ability project called "Uudistumisen eväät" that was followed through in Kuopio. The study was carried out in two phases. First, the connection between teachers' burn-out and professional development was charted with the help of a quantitative survey study. 131 teachers participated in the survey. Some of them were from schools that participated in the working ability project and the remainder were from other schools in Kuopio. The questionnaire consisted of self-constructed instruments of burn-out and professional development. According to the results, burn-out and professional development were strongly correlated with each other. Burn-out was summed up in three factors: emotional exhaustion, feelings of depersonalization and low feelings of personal accomplishment. Professional development was summed up in four factors: personality and pedagogical skills, learning-orientation, social skills and confronting change. Personality and pedagogical skills and skills of confronting change were correlated strongest with burn-out and its symptoms. A teacher, who has not found his/her own personal way of acting as a teacher and who considers change as something negative, is more likely to become exhausted than a teacher, who has developed his/her own pedagogical identity and who regards change more positively. In the second phase of this study, teachers' conceptions of the significance of in-service training on well-being was investigated with the help of group interviews (n=12). According to the results, the importance of in-service training was significant on the well-being of teachers. It appeared that in-service training promotes well-being by providing teachers with motivation, professional development and the possibility of taking a break from teaching and cooperating with other teachers. It has to be based on teachers' own needs. It has to be offered to teachers frequently and early enough. If teachers are already exhausted, they will neither have enough resources to participate in training, nor will they have the strength to make good use of it in practice. Both professional development and well-being are becoming more and more essential now that society is changing rapidly and the demands set on teachers are growing. Professional development can promote well-being, but are teachers too exhausted to develop themselves? Professional development demands resources and teachers may regard it as a threat and an additional strain. When the demands are so high that teachers cannot cope with them, they are likely to suffer stress and see reduction of commitment to their work and its development as a means to survive. If teachers stop caring about their work and their own development, how can we expect them to promote pupils' learning and development? It should be considered in the planning and implementation of in-service training and in arranging teachers' working conditions, that teachers have enough time and resources to develop themselves. Tutkimuksen tavoitteena on ollut kuvata pian valmistuvien sairaanhoitaja- ja laboratoriohoitajaopiskelijoiden teoreettista työnhallintaa kliinisen mikrobiologian alaan kuuluvien potilasnäytteiden ottamisessa. Tutkimuksessa haluttiin selvittää, miten peruskoulutus ja työelämän harjoittelujaksot ovat vaikuttaneet näytteenottovalmiuksien syntymiseen sekä minkälainen käsitys valmistuvilla hoitajilla on omista mikrobiologisista näytteenottotaidoistaan. Syy tutkimuksen tekemiseen oli tarve arvioida ammatillisen koulutuksen tuloksellisuutta sekä hakea mikrobiologian opetusalueelta tutkimustietoa opetuksen kehittämiseen ja sisältöjen suuntaamiseen. Tutkimukseen osallistui 21 laboratoriohoitajaopiskelijaa ja 27 sairaanhoitajaopiskelijaa. Opiskelijat olivat viimeiseltä lukukaudelta ja valmistuivat keväällä 1998. Tiedonkeruumenetelmänä oli kyselylomake, jonka alkuosa koostui neljästä mikrobiologisesta näytteenottotilanteesta. Opiskelija vastasi toimintatapansa kertoen ja perustellen, miten hän ottaisi näytteet näissä tilanteissa. Vastaukset ryhmiteltiin käyttäen luokitteluun mukaeltua Solo-taksonomiaa. Koulutusta ja opiskelijan käsityksiä kartoittavat kysymykset olivat joko puolistrukturoituja tai avoimia. Laboratoriohoitajaopiskelijoiden tiedollinen työnhallinta sijoittuu tulosten perusteella tyydyttävän ja hyvän välimaastoon. Sairaanhoitajaopiskelijoiden tietotaso oli heikko ja osin hyvin heikko. Laboratoriohoitajaopiskelijat arvioivat koulutuksen antamia mikrobiologisen näytteenoton valmiuksia pääasiassa hyviksi tai tyydyttäviksi; sairaanhoitajaopiskelijat pitivät opetuksesta saamiaan valmiuksia heikkoina. Näytteenoton arvostusta pidettiin kaiken kaikkiaan matalana, ja tietoa laadukkaasta näytteenotosta arvioitiin puuttuvan työelämässä laajasti. Tutkimuksen perusteella näyttää siltä, ettei valmistuvilla sairaanhoitajilla ole tiedollisia ja taidollisia valmiuksia suoriutua edes perusnäytteenotosta niin, että saatu näyte edustaisi laadukkaasti potilaan tilaa näytteenottohetkellä. Näytteenotto on koko laboratoriotyön prosessin kriittisin vaihe. Jos näyte on otettu, säilytetty tai kuljetettu väärin, paraskaan analyysitekniikka ei pysty tuottamaan siitä potilaan hoitoa oikeaan suuntaan ohjaavaa tulosta. Kaikkien laboratorionäytteiden oton siirtäminen laboratoriohoitajille ja runsaan lisäkoulutuksen anto tälle ammattiryhmälle voisi tuottaa parannuksen vallitsevaan tilanteeseen. Most musicians choose a career in music based on their love of the art and a desire to share it with others. However, being a performing musician is highly demanding. Despite considerable evidence of the great frequency of performance-related problems (e.g. debilitating performance anxiety) among professional musicians or aspiring musicians in the current Western classical music tradition these problems are seldom discussed openly. The existing system offers musicians very little help in learning how to build sustainable performance success into their musical career. This study it is first of its kind in Finland which addresses the issue on larger scale in a systematic way. I devised the HOPE intervention (Holistically-Oriented Top Performance and Well-Being Enhancement), in order to learn how to integrate professional peak performance and a sense of personal well-being into the lives and careers of musicians. Unlike most interventions in previous research, the HOPE intervention is explicitly holistic and aims at enhancing the whole musician, not just alleviating performance anxiety. Earlier research has not in principle focused on musicians´ psychological well-being or on their subjective perceptions. The main purpose of the study is to understand the perceived impacts of the specially devised HOPE intervention on the participants and particularly in four key areas: performing, playing or singing well-being, and overall (performing, playing or singing and well-being combined). Furthermore, it is hoped that a deeper understanding of performers´ development will be gained. The research method is interdisciplinary and mainly qualitative. The primary data consist of a series of linked questionnaires (before and after the intervention) and semi-structured follow-up interviews collected during action research-oriented HOPE intervention courses for music majors in the Sibelius Academy. With the longitudinal group called Hope 1, the core data were collected during a nine month HOPE intervention course and from follow-up interviews conducted six months later in 2003-2004. The core data of Hope 1 (nine participants) are compared with the perceived impacts on fifty-three other participants in the HOPE courses during the period since their inception, 2001-2006. The focus is particularly on participants´ subjective perceptions. Results of the study suggest that the HOPE intervention is beneficial in enhancing overall performance capacity, including music performance, and a personal sense of well-being in a music university setting. The findings indicate that within all key areas significant positive changes take place between the beginning and the end of a HOPE intervention course. The longitudinal data imply that the perceived positive changes are still ongoing six months after the HOPE intervention course is finished. The biggest change takes place within the area of performing and the smallest, in participants´ perception of their playing or singing. The main impacts include reduced feelings of stress and anxiety (an enhanced sense of well-being) as well as increased sense of direction and control in one's life. Since the results of the present research gave no other reason to believe otherwise, it is to be expected that the HOPE intervention and the results of the study can be exploited in other areas of human activity as well, especially where continuous professional top performance is a prerequisite such as in business or sports.
