Background Acute kidney injury (AKI) is a common but under-recognised disease process, which carries a high risk of mortality or chronic complications, such as chronic kidney disease and other organ dysfunction. Management of AKI, however, is suboptimal, both in developed settings and in Malawi. This is partly because of deficiencies in AKI education and training. Aim To establish current levels of AKI education in a range of healthcare workers in Malawi. Methods An AKI symposium was held in Blantyre in March 2015. Delegates were asked to complete a survey at the start of the symposium to assess their clinical experience and education in the management of AKI. Results From 100 delegates, 89 nurses, clinical officers, and physicians, originating from 11 different districts, responded to the survey. Twenty-two percent of healthcare workers (including 28% of district workers of the various cadres and 31% of nurses) had never received teaching on any aspect of renal disease, and 50% (including 63% of district workers and 61% of nurses) had never received teaching specifically on AKI. Forty-four percent did not feel confident managing AKI, and 98% wanted more support managing patients with renal disease. Thirty-four percent (including 55% of district workers) were unaware that haemodialysis was available at Queen Elizabeth Central Hospital (QECH) for the treatment of AKI and 53% (74% of district workers) were unaware that peritoneal dialysis was available for the treatment of AKI in children. Only 33% had ever referred a patient with AKI to QECH. Conclusions There are deficiencies in education about, and clinical experience in, the management of AKI among Malawian healthcare workers, in addition to limited awareness of the renal service available at QECH. Urgent action is required to address these issues in order to prevent morbidity and mortality from AKI in Malawi.

Non-invasive ventilation (NIV) is the application of a ventilatory support without resorting to invasive methods. Today it’s considered a credible therapeutic option, with enough scientiic evidence to support its application in various situations and clinical settings related to the treatment of acute respiratory disease, as well as chronic respiratory disease. Objectives: Characterize patients undergoing NIV admitted in Unit Intermediate Care (ICU) in the period from October 1st 2015 to June 30th 2016. Methods: Prospective study conducted in ICU between October 2015 and June 2016. In this study were included all patients hospitalized in this unit (ICU) and in that time period a sample of 57 participants was obtained. As data collection instruments we used a questionnaire for sociodemographic and clinical data and the Braden scale. Results: Participants were mostly male 38 (66.7%), the average age 69.5 ± 11.3 years, ranging between 43 and 92 years. They weighed on average 76.6 kg (52 and 150), with an average body mass index of 28.5 kg/m2 (20 to 58.5). With skin intact 28 (49.1%) with abnormal perfusion 12 (21.1%), with altered sensitivity 11 (19.3%) and a high risk of ulcer on the scale of Braden 37 (65%). The admission diagnosis was respiratory failure 33 (57.3%) and had different backgrounds. We used reused mask 53 (93.0%), the average time of NIV was 7.1 days (1-28), 4.8 days of hospitalization (1-18) and an average of 7.8 IPAP pressure. 11 (19.3%) of the participants developed face ulcer pressure.Conclusions: The NIV is used in patients with advanced age, obesity, respiratory failure and high risk of face ulcer development.

O presente estudo decorreu no serviço de urgência do Hospital do Espírito Santo de Évora – EPE, de Setembro de 2015 a Janeiro de 2016 e enquadra-se na área da prestação de cuidados especializados em enfermagem de reabilitação. É um estudo de caso, em que objeto de estudo é “A Intervenção do Enfermeiro Especialista em Enfermagem de Reabilitação no doente com Patologia Respiratória Crónica”. A abordagem metodológica é mista pois trata-se de um caso de estudo que não é puramente qualitativo. A utilização de metodologia quantitativa com indicadores quantitativos permitirá uma melhor compreensão e visão global do caso apresentado. Trata-se de um estudo de natureza descritiva, transversal e não experimental pois tem como objetivo a recolha de dados observáveis e quantificáveis, num determinado período de tempo e não o controlo de fenómenos. População alvo: todos os utentes com Patologia Respiratória Crónica internados no Serviço de Observação no período de tempo referido anteriormente. Amostra: 4 utentes com média de idades de 78,0 anos dos quais 75% eram do sexo masculino e 25% do sexo feminino. Foram implementadas intervenções de enfermagem de reabilitação, nomeadamente, reabilitação funcional respiratória e motora, que demonstraram ser eficazes tendo-se obtido saturações periféricas de O2 de 96%, em média. Estes resultados traduzem-se em ganhos ao nível da capacidade respiratória permitindo o desenvolvimento de atividades de vida diária normais. Também foi possível a manutenção e potenciação das capacidades de mobilidade nestes utentes com uma média etária elevada e com algumas limitações físicas e/ou cognitivas o que, só por si, também traduz ganhos em saúde no que concerne à sua independência e melhoria da qualidade de vida. O Enfermeiro Especialista em Enfermagem de Reabilitação é claramente uma mais-valia no serviço de urgência pois ao colocar em prática uma dinâmica de desempenho voltado para os conhecimentos especializados adquiridos, criando ferramentas que permitam um desempenho estruturado dos cuidados, a promoção da sua continuidade, a sua avaliação e a produção de conhecimento científico baseado na evidência contribui para a redução do tempo de internamento o que se repercute na maior eficácia na Gestão de Unidades de Saúde.

Aim: Amyotrophic lateral sclerosis (ALS) is a chronic, neurodegenerative disease, which leads to development of malnutrition. The main purpose of this research was to analyze the impact of malnutrition on the course of the disease and long-term survival. Material and methods: A retrospective analysis has been performed on 48 patients (22 F [45,83%] and 26 M [54,17%], the average age of patients: 66,2 [43-83]) in 2008-2014.The analysis of the initial state of nutrition was measured by body mass index (BMI), nutritional status according to NRS 2002, SGA and concentration of albumin in blood serum. Patients were divided into two groups, depending on the state of nutrition: well-nourished and malnourished. The groups were created separately for each of these, which allowed an additional comparative analysis of techniques used for the assessment of nutritional status. Results: Proper state of nutrition was interrelated with longer survival (SGA: 456 vs. 679 days, NRS: 312 vs. 659 vs. 835 days, BMI: respectively, 411, 541, 631 days, results were statistically significant for NRS and BMI). Concentration of albumin was not a prognostic factor, but longer survival was observed when level of albumin was increased during nutritional therapy. Conclusions: The initial nutrition state and positive response to enteral feeding is associated with better survival among patients with ALS. For this reason, nutritional therapy should be introduced as soon as possible.

Influenza is a major cause of morbidity and mortality; current estimates by the World Health Organization (WHO) are 3 to 5 million cases and 250,000 to 500,000 deaths worldwide every year. Most deaths associated with it occur among people age 65 or older, as well as among persons suffering a chronic debilitating disease regardless of age. The recent 2009 pandemic served to foster interest in this disease. An inactivated virus vaccine has been available since the late 1940´s but it only began to be used extensively when the influenza virus antigenic variability was taken into account. Aside from such variability, influenza viruses are capable of infecting a wide variety of vertebrates, including many avian species, both wild and domestic, thus it is essential to monitor the antigenic characteristics of influenza virus strains currently circulating, and so the vaccine formula has to be evaluated and modiied accordingly every year

Background: Renal cystic diseases are important causes of chronic kidney disease (CKD). Objectives: We report the pattern of renal cystic disease in children and evaluate the outcome of children with multicystic dysplastic kidney (MCDK). Patients and Methods: Retrospective study of all children with cystic kidney diseases at King Abdulaziz University hospital from 2006 to 2014. Results: Total of 55 children (30 males); 25 MCDK, 22 polycystic kidney diseases (PKD), 4 nephronophthises and 4 renal cysts. Consanguinity was positive in 96.2%. MCDK and simple renal cyst patients had good renal function while PKD and nephronophthisis developed renal impairment. Most MCKD were diagnosed ante-natally, 16 of them were followed up for 3.4 (1.97) year. Their last creatinine was 33.9 (13.5) umol/L. MCDK was spontaneously involuted at mean age of 2.6 (1.3) years in 56%. Conclusions: MCDK is the commonest cystic renal disease and diagnosed ante-natally in the majority of cases. It has a good prognosis.

Background: Respiratory distress syndrome (RDS) is one of the most common diseases in neonates admitted to NICU. For this important cause of morbidity and mortality in preterm neonates, several treatment methods have been used. To date, non-invasive methods are preferred due to fewer complications. Objectives: Herein, two non-invasive methods of ventilation support are compared: NCPAP vs. NIPPV. Patients and Methods: This is a randomized clinical trial. Premature neonates with less than 34 weeks gestation, suffering from RDS entered the study, including 151 newborns admitted to Vali-Asr NICU during 2012-2013. Most of these patients received surfactant as early rescue via INSURE method and then randomly divided into two NCPAP (73 neonates) and NIPPV (78 neonates) groups. Both early and late complications are compared including extubation failure, hospital length of stay, GI perforation, apnea, intraventricular hemorrhage (IVH) and mortality rate. Results: The need for re-intubation was 6% in NIPPV vs. 17.6% in NCPAP group, which was statistically significant (P = 0.031). The length of hospital stay was 23.92 ± 13.5 vs. 32.61 ± 21.07 days in NIPPV and NCPAP groups, respectively (P = 0.002). Chronic lung disease (CLD) was reported to be 4% in NCPAP and 0% in NIPPV groups (P = 0.035). The most common complication occurred in both groups was traumatization of nasal skin and mucosa, all of which fully recovered. Gastrointestinal perforation was not reported in either group. Conclusions: This study reveals the hospital length of stay, re-intubation and BPD rates are significantly declined in neonates receiving NIPPV as the treatment for RDS.

El objetivo de este estudio fue determinar el efecto de un programa de ejercicio aeróbico y un programa de circuito con pesas sobre la calidad de vida, disnea y resistencia cardiorrespiratoria en sujetos con Enfermedad Pulmonar Crónica (EPC). Metodología: el programa de rehabilitación se hizo durante ocho semanas, se realizó un pretest y un postest, se cumplió con una sesión semanal presencial, y las otras dos domiciliarias, se tuvo comunicación directa vía telefónica los días que realizaban el ejercicio en la casa. Instrumentos: el cuestionario St. George Respiratory Questonnaire para medir calidad de vida, la escala de Borg para medir la disnea y el test de caminata de los 6 min. para medir la resistencia cardiorrespiratoria. Sujetos: participaron 38 personas con diagnóstico previo de EPC, 18 hombres y 20 mujeres; 27 sujetos con patología obstructiva y 11 sujetos con restrictiva., con una edad promedio de 69.8 ± 9.34 años, divididos aleatoriamente en dos grupos, uno que realizó ejercicio aeróbico (22 sujetos) y otro que ejecutó ejercicio aeróbico y de contrarresistencia con pesas (16 sujetos). Análisis estadístico: ANOVA mixta de cuatro vías (2x2x2x2) para las variables mediciones, sexo, tratamiento y patología. Resultados: se encontraron diferencias significativas entre mediciones para la variable disnea (pre: 7.18 ± 0.69 puntos y post: 4.89 ± 0.68 puntos) (F = 228.770; p < 0,05), calidad de vida (pre: 59.68 ± 7.50 puntos y post 81.75 ± 7.33 puntos) (F = 228.770; p < 0,05), resistencia cardiorrespiratoria (pre 384,95 ± 51,02 mts. y post 432 ± 37,41 mts.) (F: 59.9; p<0.01), pero no se encontraron diferencias por sexo, actividad ni enfermedad (p>0,05), ni interacción entre las variable (p>0,05). Conclusión: la realización de ejercicio físico aeróbico y anaeróbico mejoran el grado de disnea, la calidad de vida y resistencia cardiorrespiratoria en pacientes con EPC.

Intradialytic hypotension (IDH) remains the most common severe side effect of hemodialysis despite numerous technological advancements. Recent evidence emphasises the significance of asymptomatic hypotensive episodes, as well as the hypoperfusive consequences of both relative blood pressure drops and repetitive, symptomatic events. This article reviews the physiological importance of rapid blood pressure decrease during hemodialysis, and highlights the pathological consequences of repeated asymptomatic and symptomatic hypoperfusive episodes. In proposing a view concerned with asymptomatic IDH, a practicalpre-emptive intervention is offered to improve the long-term outcomes of patients on hemodialysis. Ongoing monitoring of individual patient's mean arterial pressure (MAP) throughout the dialysis treatment can facilitate the identification of an asymptomatic hypotensive episode. A brief pause in ultrafiltration enables vascular refill and subsequent increase in MAP, allowing resumption of safe fluid removal. Such enhanced assessment results in a reduction off patient risk, allowing safe and optimal fluid removal.

Le rôle de l'inflammation dans le développement et la progression des maladies rénales chroniques (MRC) chez le chat a été peu étudié. L'hepcidine est une protéine de la phase aigue (PPA) de l'inflammation qui contribue au développement des anémies lors de MRC chez l'homme. Les objectifs de cette étude sont de comparer les concentrations en PPAs, en erythropoietine (EPO) ainsi que le statut en fer entre un groupe de chats sains et en MRC. 18 chats sains et 38 chats en MRC ont été recrutés de façon prospective. Les examens réalisés incluaient hématologie, biochimie, analyse d'urine, Serum amyloid A (SAA), haptoglobine (HAP), EPO, hepcidine,fer, TIBC et ferritinne. Nous avons observé une augmentation significative des concentrations en SAA et en hepcidine ainsi qu'une diminution significative du fer et du TIBC dans le groupe MRC (P < .05). Une corrélation positive entre la créatinine et certaines PPAs (SAA and hepcidin; P < .05) était présente. L'augmentation de SAA et hepcidine était significativement associé avec une diminution du TIBC et de l'hématocrite dans le groupe MRC. Les 14 (37%) chats anémiques du groupe MRC avaient une concentration significativement plus basse en fer et en TIBC (P < .05), changements compatibles avec une déficience fonctionelle en fer. Aucun chat n'avait un panel de fer compatible avec une carence en fer absolue. En conclusion, les résultats de cette étude suggèrent que les MRC chez le chat sont des conditions pro-inflammatoires, ayant un impact sur le métabolisme du fer.
