[ES]The purpose of this paper was to use threedimensional computed tomographic reconstruction as another tool to teach in the veterinary colleges. 2-millimeters thick transverse images of two foals and one dog were obtained. Images provided excellent detail of relevant anatomic structures and detailed information regarding the extent of disease and accurate discrimination of neoplastic versus non-neoplastic diseases. Tridimensional reconstruction can be a valuable diagnostic aid for clinical evaluation of several disturbances and could be used as a tool for teaching anatomy in veterinary schools.

Evolving interfaces were initially focused on solutions to scientific problems in Fluid Dynamics. With the advent of the more robust modeling provided by Level Set method, their original boundaries of applicability were extended. Specifically to the Geometric Modeling area, works published until then, relating Level Set to tridimensional surface reconstruction, centered themselves on reconstruction from a data cloud dispersed in space; the approach based on parallel planar slices transversal to the object to be reconstructed is still incipient. Based on this fact, the present work proposes to analyse the feasibility of Level Set to tridimensional reconstruction, offering a methodology that simultaneously integrates the proved efficient ideas already published about such approximation and the proposals to process the inherent limitations of the method not satisfactorily treated yet, in particular the excessive smoothing of fine characteristics of contours evolving under Level Set. In relation to this, the application of the variant Particle Level Set is suggested as a solution, for its intrinsic proved capability to preserve mass of dynamic fronts. At the end, synthetic and real data sets are used to evaluate the presented tridimensional surface reconstruction methodology qualitatively.

OBJECTIVE: Long-term follow-up after endovascular aneurysm repair (EVAR) is very scarce, and doubt remains regarding the durability of these procedures. We designed a retrospective cohort study to assess long-term clinical outcome and morphologic changes in patients with abdominal aortic aneurysms (AAAs) treated by EVAR using the Excluder endoprosthesis (W. L. Gore and Associates, Flagstaff, Ariz). METHODS: From 2000 to 2007, 179 patients underwent EVAR in a tertiary institution. Clinical data were retrieved from a prospective database. All patients treated with the Excluder endoprosthesis were included. Computed tomography angiography (CTA) scans were retrospectively analyzed preoperatively, at 30 days, and at the last follow-up using dedicated tridimensional reconstruction software. For patients with complications, all remaining CTAs were also analyzed. The primary end point was clinical success. Secondary end points were freedom from reintervention, sac growth, types I and III endoleak, migration, conversion to open repair, and AAA-related death or rupture. Neck dilatation, renal function, and overall survival were also analyzed. RESULTS: Included were 144 patients (88.2% men; mean age, 71.6 years). Aneurysms were ruptured in 4.9%. American Society of Anesthesiologists classification was III/IV in 61.8%. No patients were lost during a median follow-up of 5.0 years (interquartile range, 3.1-6.4; maximum, 11.2 years). Two patients died of medical complications ≤ 30 days after EVAR. The estimated primary clinical success rates at 5 and 10 years were 63.5% and 41.1%, and secondary clinical success rates were 78.3% and 58.3%, respectively. Sac growth was observed in 37 of 142 patients (26.1%). Cox regression showed type I endoleak during follow-up (hazard ratio, 3.74; P = .008), original design model (hazard ratio, 3.85; P = .001), and preoperative neck diameter (1.27 per mm increase, P = .006) were determinants of sac growth. Secondary interventions were required in 32 patients (22.5%). The estimated 10-year rate of AAA-related death or rupture was 2.1%. Overall life expectancy after AAA repair was 6.8 years. CONCLUSIONS: EVAR using the Excluder endoprosthesis provides a safe and lasting treatment for AAA, despite the need for maintained surveillance and secondary interventions. At up to 11 years, the risk of AAA-related death or postimplantation rupture is remarkably low. The incidences of postimplantation sac growth and secondary intervention were greatly reduced after the introduction of the low-permeability design in 2004.

OBJETIVO: Determinar através de cortes tomográficos e reconstrução tridimensional a incidência de mal posicionamento de parafusos em pacientes submetidos ao tratamento cirúrgico da Escoliose idiopática do adolescente. MÉTODOS: Foram analisados exames tomográficos de 8 pacientes, tratados cirurgicamente no Hospital de Base de São José do Rio Preto-SP, realizada instrumentação posterior partindo de T2 /T4 a L4/L5 totalizando 164 parafusos. RESULTADOS: 32,9% (n=54) apresentavam posicionamento com risco potencial,ou seja desvio acima de 2 milímetros, sendo 20,1% (n=33) com invasão lateral, 9,1% (n=15) com invasão medial, 3,6 %(n=6) com invasão anterior. Dos parafusos que ofereciam risco potencial a relação com aspecto da curva foi de 46% (n=25) na concavidade, 35% (n=19) na convexidade e 19% (n=10) em vértebras adjacentes a curva. CONCLUSÃO: Os limites de penetração aceitáveis, assim como os métodos de mensuração ainda não foram padronizados, a técnica free hand' mostrou-se segura, apesar da violação dos pedículos. A tomografia computadorizada pré-operatória, auxilia no planejamento cirúrgico e na redução das complicações.

This research work analyzes the theme of the architecture of the city and aims at establishing, by studying the urban project of the new town hall in Ljubljana made by the Slovenian architect Jože Plečnik, the idea that the construction of the city must be carried out through a type of architecture directed at the planning of collective urban spaces. The plan for the new town hall building, drew in three versions – 1932, 1939, and 1940-41 –, is part of a large set of plans concerning the area that Plečnik defines to be the “osrčje” (heart) of Ljubljana, that is, the central area within the castle hill and the distinctive arc of the Ljubljanica River, on the eastern boundary of the old “mesto” (town). Among the Plečnik's projects on urban scale for Ljubljana, the above-mentioned plans, unbuilt and scarcely published, must be considered to be ones of the least known, despite their importance in the professional activity of the architect. The work consists of three parts: the first part describes the background of theories and projects which shaped Plečnik's urban culture, during the years of his education in Vienna and before the beginning of the planning activities this work focuses on; the second part studies the plans for the “heart” of the city; the third part investigates the plan for the new town hall building by means of the graphical reconstruction of the three plan versions made by Plečnik, and it provides insights into the relationships among form, significance and motivation of his work. Since the plans have never been built, the digital tridimensional reconstruction of the building models allowed to show unknown spaces and confirm that Architecture has a particular significance when its goal is the planning of collective urban spaces.

Il presente lavoro di tesi ha riguardato una riformulazione teorica, una modellazione numerica e una serie di applicazioni della Generalized Beam Theory per lo studio dei profili in parete sottile con particolare riguardo ai profili in acciaio formati a freddo. In particolare, in questo lavoro è proposta una riscrittura della cinematica GBT che introduce in una forma originale la deformabilità a taglio della sezione. Tale formulazione consente di conservare il formato della GBT classica e introducendo uno spostamento di warping variabile lungo lo spessore della generica parete della sezione trasversale, garantisce perfetta coerenza tra la componente flessionale e tagliante della trave. E' mostrato, come tale riscrittura consente in maniera agevole di ricondursi alle teorie classiche di trave, anche deformabili a taglio. Inoltre, in tale contesto, è stata messa a punto una procedura di ricostruzione dello sforzo tridimensionale in grado ricostruire la parte reattiva delle componenti di tensioni dovuta al vincolamento interno proprio di un modello a cinematica ridotta. Sulla base di tali strumenti, è stato quindi proposto un approccio progettuale dedicato ai profili in classe 4, definito ESA (Embedded Stability Analysis), in grado di svolgere le verifiche coerentemente con quanto prescritto dalle normative vigenti. Viene infine presentata una procedura numerica per la progettazione di sistemi di copertura formati a freddo. Tale procedura permette di effettuare in pochi semplici passi il progetto dell'arcareccio e dei dettagli costruttivi relativi alla copertura.

Reconstruction of the nipple-areola complex (NAC) is the last stage of breast reconstruction and represents the search for symmetry in regard to the contralateral breast. The objective of this study was to present an areola reconstruction technique with local skin graft to improve the texture and aspect of the reconstructed areola, searching for a natural look. This technique was performed on 122 patients who in the period from January 2000 to December 2005 were submitted to nipple and areola reconstruction. Once the position of the new nipple was determined, markings were made for the reconstruction of the areola. Then the external limit of the new areola was incised and the skin was centripetally deepidermized up to 85% of its diameter. After this procedure the detached skin was repositioned in its bed as a graft and sutured with 4.0 mononylon thread. Incisions with an 11-blade scalpel were then made in V and C forms associated with the detachment of this skin of the receptor area along the local graft so that at the end of the healing process they would determine alterations in the areolar texture mimicking the texture of a normal areola. All patients underwent tattooing 3 months after reconstruction of the NAC taking into account the different shades of the contralateral areola and nipple colors. The use of a local skin graft associated with C and V incisions allowed alteration in the texture of the reconstructed areola. The use of different ink shades for tattooing helped to give a tridimensional aspect to this areola. These factors determined a good aesthetic result in these patients. This areola reconstruction using a local skin graft allows change in the areola texture and a tridimensional aspect similar to that of a normal areola without the inconvenience of grafting from a distance.

La patología de la válvula mitral tiene gran prevalencia dentro de la enfermedad cardiaca. Con nuevas técnicas diagnósticas se perfecciona la caracterización de la válvula mitral y la ecocardiografía tridimensional tras esofágica, ha permitido obtener mejor información acerca de la patología valvular mitral. Objetivo principal : describir estructural y anatómicamente la válvula mitral, sus medidas y relaciones espaciales tridimensionales, en pacientes catalogados con válvula normal comparándolos con pacientes con insuficiencia mitral; en estudios realizados mediante ecocardiografía tras esofágica tridimensional. Materiales y métodos : estudio descriptivo, prospectivo con una serie de casos de válvulas mitrales normales comparadas con insuficientes : Obtención - Imagen tras esofágica 3D en tiempo real - Análisis y procesamiento de la imagen - Reconstrucción Tridimensional. Obtención de las diferentes medidas anatómicas estructurales que servirán para la tipificación de la válvula mitral en 3D. Análisis descriptivo : se utilizarán distribuciones de frecuencia y distribuciones porcentuales y en las variables de tipo cuantitativo medidas de tendencia central y medidas de variabilidad y dispersión. Resultados : se evaluaron durante el periodo de tiempo comprendido entre junio de 2008 y agosto de 2009 un total de 113 pacientes en total, encontrando claras diferencias en la estructura de las insuficiencias por prolapso. No hubo diferenciación en las cardiopatía isquémica vs dilatada. Conclusión : en el prolapso mitral aporta datos en la identificación etiológica ya sea degenerativa fibroelástica o enfermedad de Barlow. No hay diferencia significativa en la estructura que ayude caracterizar cardiopatía isquémica vs cardiopatía dilatada.
