La política social y el mercado de trabajo son dimensiones del sistema social mutuamente implicadas en las sociedades industriales capitalistas. Sin embargo, una interpretación apresurada sobre el mundo social podría opacar los procesos que hacen que en un grupo de la población se yuxtapongan: una inserción laboral precaria dentro del mercado de trabajo informal junto con la (re) construcción de ese grupo como sujeto-objeto de políticas sociales. El artículo tiene por propósito problematizar las representaciones dominantes sobre el trabajo y su relación con las prácticas de organización de los hogares, considerando que en su interior la dimensión laboral se articula con otros recursos (provenientes de las políticas sociales). De modo más específico, el objetivo se orienta a la descripción y análisis de prácticas y representaciones sobre la situaci162n ocupacional de jefas/es y cónyuges que realizan actividades laborales en la informalidad de subsistencia. Se plantea como hipótesis exploratoria que en el período actual las condiciones de trabajo y representaciones de los trabajadores ocupados en ese tipo de actividades expresarían la existencia de un tipo de informalidad fuertemente segregada respecto de las actividades productivas, un en un contexto de crecimiento del empleo. También se sostiene que las formas bajo las cuales estas inserciones laborales se articulan con los planes sociales (como un nivel específico de política social) evidenciarían el carácter de la contribución estatal a la reproducción de las condiciones de vida de los trabajadores insertos en este tipo de informalidad. Los hallazgos presentados constituyen un avance de investigación sobre la reproducción de la marginalidad urbana en el Conurbano Bonaerense. Parte del trabajo de campo se realizó durante el año 2008, en un barrio del Partido de Almirante Brown. Se organizaron dos grupos de discusión: uno con mujeres perceptoras de planes y otro con hombres que realizaban changas como actividad principal. Con el objeto de aportar datos acerca de las condiciones materiales de los hogares, se aplicó un cuestionario a los participantes. Junto con esta información, se analizaron registros de campo tomados en observaciones realizadas en diversos espacios del barrio

BACKGROUND AND AIMS: Data from the literature reveal the contrasting influences of family members and friends on the survival of old adults. On one hand, numerous studies have reported a positive association between social relationships and survival. On the other, ties with children may be associated with an increased risk of disability, whereas ties with friends or other relatives tend to improve survival. A five-year prospective, population-based study of 295 Swiss octogenarians tested the hypothesis that having a spouse, siblings or close friends, and regular contacts with relatives or friends are associated with longer survival, even at a very old age. METHODS: Data were collected through individual interviews, and a Cox regression model was applied to assess the effects of kinship and friendship networks on survival, after adjusting for socio-demographic and health-related variables. RESULTS: Our analyses indicate that the presence of a spouse in the household is not significantly related to survival, whereas the presence of siblings at baseline improves the oldest old's chances of surviving five years later. Moreover, the existence of close friends is a central component in the patterns of social relationships of oldest adults, and one which is significantly associated with survival. Overall, the protective effect of social relationships on survival is more related to the quality of those relationships (close friends) than to the frequency of relationships (regular contacts). CONCLUSIONS: We hypothesize that the existence of siblings or close friends may beneficially affect survival, due to the potential influence on the attitudes of octogenarians regarding health practices and adaptive strategies.

BACKGROUND Persistence of anti-tumor necrosis factor (TNF) therapy in rheumatoid arthritis (RA) is an overall marker of treatment success. OBJECTIVE To assess the survival of anti-TNF treatment and to define the potential predictors of drug discontinuation in RA, in order to verify the adequacy of current practices. DESIGN An observational, descriptive, longitudinal, retrospective study. SETTING The Hospital Clínico Universitario de Valladolid, Valladolid, Spain. PATIENTS RA patients treated with anti-TNF therapy between January 2011 and January 2012. MEASUREMENTS Demographic information and therapy assessments were gathered from medical and pharmaceutical records. Data is expressed as means (standard deviations) for quantitative variables and frequency distribution for qualitative variables. Kaplan-Meier survival analysis was used to assess persistence, and Cox multivariate regression models were used to assess potential predictors of treatment discontinuation. RESULTS In total, 126 treatment series with infliximab (n = 53), etanercept (n = 51) or adalimumab (n = 22) were administered to 91 patients. Infliximab has mostly been used as a first-line treatment, but it was the drug with the shortest time until a change of treatment. Significant predictors of drug survival were: age; the anti-TNF agent; and the previous response to an anti-TNF drug. LIMITATION The small sample size. CONCLUSION The overall efficacy of anti-TNF drugs diminishes with time, with infliximab having the shortest time until a change of treatment. The management of biologic therapy in patients with RA should be reconsidered in order to achieve disease control with a reduction in costs.

Gene expression data from microarrays are being applied to predict preclinical and clinical endpoints, but the reliability of these predictions has not been established. In the MAQC-II project, 36 independent teams analyzed six microarray data sets to generate predictive models for classifying a sample with respect to one of 13 endpoints indicative of lung or liver toxicity in rodents, or of breast cancer, multiple myeloma or neuroblastoma in humans. In total, >30,000 models were built using many combinations of analytical methods. The teams generated predictive models without knowing the biological meaning of some of the endpoints and, to mimic clinical reality, tested the models on data that had not been used for training. We found that model performance depended largely on the endpoint and team proficiency and that different approaches generated models of similar performance. The conclusions and recommendations from MAQC-II should be useful for regulatory agencies, study committees and independent investigators that evaluate methods for global gene expression analysis.

Les usines récupérées par les ouvriers en Argentine sont devenues un mouvement social emblématique symbolisant l'un des aspects de la révolte sociale entourant la crise économique de 2001-2002. Les usines récupérées sont des entreprises abandonnées par leurs propriétaires originaux ou déclarées faillite, laissant derrières elles des salaires et des dettes impayés. Par conséquence, les ouvriers ont commencé à récupérer leurs usines; reprenant la production sans leurs anciens patrons, sous, et au profit de la gestion collective des ouvriers. Le mouvement est remarquable pour sa rémunération égalitaire et sa gestion horizontale. Ce travail examine la continuité des usines récupérées et ceci à travers l'évolution sociale, politique et économique du paysage de l'Argentine. Il évalue également l'impact du mouvement en tant que défi aux modes économiques de production hégémoniques et orientés vers le marché. En supposant que l'avenir du mouvement dépend de deux ensembles de facteurs, le rapport analyse les facteurs internes à travers le prisme de la théorie de mobilisation des ressources, ainsi que les facteurs externes à travers la perspective de la théorie de la structure de l'opportunité politique. Le travail conclut que la situation actuelle se trouve dans une impasse dans laquelle le mouvement a gagné l'acceptation institutionnelle, mais a échoué d'effectuer le changement structurel favorisant ses pratiques et garantissant la sécurité à long terme. Il argumente que le mouvement doit consolider certains aspects combatifs. Il doit consolider sa nouvelle identité en tant que mouvement social et forger des alliances stratégiques et tactiques tout en préservant son autonomie.

El concepto de organización saludable cada vez toma más fuerza en el ámbito empresarial y académico, a razón de su enfoque integral y al impacto generado en distintos grupos de interés. Debido a su reciente consolidación como concepto, existe un limitado cuerpo de investigación en torno al tema. Para contribuir a la generación de conocimiento en este sentido, se desarrolló un estudio exploratorio el cual tenía como objetivo identificar la relación existente entre la implementación de prácticas saludables en las organizaciones y los valores culturales. En el estudio participaron 66 sujetos a quienes se les administró un cuestionario compuesto por nueve variables, cinco provenientes del modelo de Hofstede (1980) y cuatro más que evaluaban la implementación de prácticas organizacionales saludables. Los resultados obtenidos muestran que los valores culturales predicen la implementación de prácticas saludables.

Native grasslands have been altered to a greater extent than any other biome in North America. The habitats and resources needed to support breeding performance of grassland birds endemic to prairie ecosystems are currently threatened by land management practices and impending climate change. Climate models for the Great Plains prairie region predict a future of hotter and drier summers with strong multiyear droughts and more frequent and severe precipitation events. We examined how fluctuations in weather conditions in eastern Colorado influenced nest survival of an avian species that has experienced recent population declines, the Mountain Plover (Charadrius montanus). Nest survival averaged 27.2% over a 7-yr period (n = 936 nests) and declined as the breeding season progressed. Nest survival was favored by dry conditions and cooler temperatures. Projected changes in regional precipitation patterns will likely influence nest survival, with positive influences of predicted declines in summer rainfall yet negative effects of more intense rain events. The interplay of climate change and land use practices within prairie ecosystems may result in Mountain Plovers shifting their distribution, changing local abundance, and adjusting fecundity to adapt to their changing environment.

About 45 palm species occur in the Atlantic forest of Brazil, and most of them are affected by loss of seed dispersers resulting from forest fragmentation and hunting. Here we report the effects of habitat loss and defaunation on the seed dispersal system of an endemic palm, Astrocaryum aculeatissimum. We evaluated seed removal, insect and rodent seed predation, and scatter-hoarding in nine sites, ranging from 19 ha to 79 000 ha. We report the seedling, juvenile and adult palm densities in this range of sites. Endocarps remaining beneath the parent palm had a higher probability of being preyed upon by insects in small, mostly fragmented and more defaunated sites. The frequency of successful seed removal, scatter-hoarding and consumption by rodents increased in the larger, less defaunated sites. Successful removal and dispersal collapsed in small (< 1000 ha), highly defaunated sites and frequently resulted in low densities of both seedlings and juveniles. Our results indicate that a large fraction of Atlantic forest palms that rely on scatter-hoarding rodents may become regionally extinct due to forest fragmentation and defaunation. Current management practices including palm extraction and hunting pressure have a lasting effect on Atlantic forest palm regeneration by severely limiting successful recruitment of prereproductive individuals.(c) 2006 the Linnean Society of London.

CONTEXT Robot-assisted surgery is increasingly used for radical cystectomy (RC) and urinary reconstruction. Sufficient data have accumulated to allow evidence-based consensus on key issues such as perioperative management, comparative effectiveness on surgical complications, and oncologic short- to midterm outcomes. OBJECTIVE A 2-d conference of experts on RC and urinary reconstruction was organized in Pasadena, California, and the City of Hope Cancer Center in Duarte, California, to systematically review existing peer-reviewed literature on robot-assisted RC (RARC), extended lymphadenectomy, and urinary reconstruction. No commercial support was obtained for the conference. EVIDENCE ACQUISITION A systematic review of the literature was performed in agreement with the PRISMA statement. EVIDENCE SYNTHESIS Systematic literature reviews and individual presentations were discussed, and consensus on all key issues was obtained. Most operative, intermediate-term oncologic, functional, and complication outcomes are similar between open RC (ORC) and RARC. RARC consistently results in less blood loss and a reduced need for transfusion during surgery. RARC generally requires longer operative time than ORC, particularly with intracorporeal reconstruction. Robotic assistance provides ergonomic value for surgeons. Surgeon experience and institutional volume strongly predict favorable outcomes for either open or robotic techniques. CONCLUSIONS RARC appears to be similar to ORC in terms of operative, pathologic, intermediate-term oncologic, complication, and most functional outcomes. RARC consistently results in less blood loss and a reduced need for transfusion during surgery. RARC can be more expensive than ORC, although high procedural volume may attenuate this difference. PATIENT SUMMARY Robot-assisted radical cystectomy (RARC) is an alternative to open surgery for patients with bladder cancer who require removal of their bladder and reconstruction of their urinary tract. RARC appears to be similar to open surgery for most important outcomes such as the rate of complications and intermediate-term cancer-specific survival. Although RARC has some ergonomic advantages for surgeons and may result in less blood loss during surgery, it is more time consuming and may be more expensive than open surgery.

BACKGROUND Potentially avoidable risk factors continue to cause unnecessary disability and premature death in older people. Health risk assessment (HRA), a method successfully used in working-age populations, is a promising method for cost-effective health promotion and preventive care in older individuals, but the long-term effects of this approach are unknown. The objective of this study was to evaluate the effects of an innovative approach to HRA and counselling in older individuals for health behaviours, preventive care, and long-term survival. METHODS AND FINDINGS This study was a pragmatic, single-centre randomised controlled clinical trial in community-dwelling individuals aged 65 y or older registered with one of 19 primary care physician (PCP) practices in a mixed rural and urban area in Switzerland. From November 2000 to January 2002, 874 participants were randomly allocated to the intervention and 1,410 to usual care. The intervention consisted of HRA based on self-administered questionnaires and individualised computer-generated feedback reports, combined with nurse and PCP counselling over a 2-y period. Primary outcomes were health behaviours and preventive care use at 2 y and all-cause mortality at 8 y. At baseline, participants in the intervention group had a mean ± standard deviation of 6.9 ± 3.7 risk factors (including unfavourable health behaviours, health and functional impairments, and social risk factors) and 4.3 ± 1.8 deficits in recommended preventive care. At 2 y, favourable health behaviours and use of preventive care were more frequent in the intervention than in the control group (based on z-statistics from generalised estimating equation models). For example, 70% compared to 62% were physically active (odds ratio 1.43, 95% CI 1.16-1.77, p = 0.001), and 66% compared to 59% had influenza vaccinations in the past year (odds ratio 1.35, 95% CI 1.09-1.66, p = 0.005). At 8 y, based on an intention-to-treat analysis, the estimated proportion alive was 77.9% in the intervention and 72.8% in the control group, for an absolute mortality difference of 4.9% (95% CI 1.3%-8.5%, p = 0.009; based on z-test for risk difference). The hazard ratio of death comparing intervention with control was 0.79 (95% CI 0.66-0.94, p = 0.009; based on Wald test from Cox regression model), and the number needed to receive the intervention to prevent one death was 21 (95% CI 12-79). The main limitations of the study include the single-site study design, the use of a brief self-administered questionnaire for 2-y outcome data collection, the unavailability of other long-term outcome data (e.g., functional status, nursing home admissions), and the availability of long-term follow-up data on mortality for analysis only in 2014. CONCLUSIONS This is the first trial to our knowledge demonstrating that a collaborative care model of HRA in community-dwelling older people not only results in better health behaviours and increased use of recommended preventive care interventions, but also improves survival. The intervention tested in our study may serve as a model of how to implement a relatively low-cost but effective programme of disease prevention and health promotion in older individuals. Whether anticoagulation management practices are associated with improved outcomes in elderly patients with acute venous thromboembolism (VTE) is uncertain. Thus, we aimed to examine whether practices recommended by the American College of Chest Physicians guidelines are associated with outcomes in elderly patients with VTE. We studied 991 patients aged ≥65 years with acute VTE in a Swiss prospective multicenter cohort study and assessed the adherence to four management practices: parenteral anticoagulation ≥5 days, INR ≥2.0 for ≥24 hours before stopping parenteral anticoagulation, early start with vitamin K antagonists (VKA) ≤24 hours of VTE diagnosis, and the use of low-molecular-weight heparin (LMWH) or fondaparinux. The outcomes were all-cause mortality, VTE recurrence, and major bleeding at 6 months, and the length of hospital stay (LOS). We used Cox regression and lognormal survival models, adjusting for patient characteristics. Overall, 9% of patients died, 3% had VTE recurrence, and 7% major bleeding. Early start with VKA was associated with a lower risk of major bleeding (adjusted hazard ratio 0.37, 95% CI 0.20-0.71). Early start with VKA (adjusted time ratio [TR] 0.77, 95% CI 0.69-0.86) and use of LMWH/fondaparinux (adjusted TR 0.87, 95% CI 0.78-0.97) were associated with a shorter LOS. An INR ≥2.0 for ≥24 hours before stopping parenteral anticoagulants was associated with a longer LOS (adjusted TR 1.2, 95% CI 1.08-1.33). In elderly patients with VTE, the adherence to recommended anticoagulation management practices showed mixed results. In conclusion, only early start with VKA and use of parenteral LMWH/fondaparinux were associated with better outcomes.

This article examines current career thinking and employability management practices within the Polish Information and Communication Technology (ICT) sector. The aim of this contribution is to identify career management problems and to determine obstacles for implementing employability management practices at a company level. Semi-structured interviews aimed at establishing company needs were conducted with 18 managers of small and medium-sized ICT enterprises in Poland. These firms appear to apply various developmental approaches to stimulate competitive advantage. Faced with a more demanding environment, firms aim for versatility rather than adopting simplified solutions. Managing the careers and employability of ICT professionals is acknowledged as vitally important for the survival and development of ICT companies.

It is widely believed that wading birds in the Everglades have declined as a result of historic water management practices. I determined growth rates for Snowy Egret (Egretta thula) chicks by assessed the nestling body condition through measurement of body weight and skeletal traits. A growth index was calculated as a residual of body weight regressed on age. A body condition index was calculated as the residual of body weight regressed on a skeletal trait (tarsus). Growth was significantly related to water level and hatch date. Survival rates were calculated to day 14, 21, and 50. Survival to 50 days of age was significantly related to hatch date and order. Survival to 21 days of age was significantly related to water level and hatching order. Survival to 14 days of age was marginally related to hatching order. Growth and survival is greatly influenced by water level and hatch date.
