Introduzione: In seguito ad una distorsione di caviglia, circa il 40% dei soggetti tende a sviluppare sintomi ricorrenti e persistenti, come dolore, gonfiore, alterazione della percezione o recidive, che a lungo termine potrebbero portare ad una instabilità cronica di caviglia. Questa problematica potrebbe limitare i soggetti nelle varie attività della vita quotidiana e nelle attività sportive, riducendo la loro qualità della vita. Solitamente, il trattamento in seguito ad una distorsione o in soggetti affetti da instabilità cronica di caviglia è il training propriocettivo. Obiettivi: Confrontare gli studi raccolti e i risultati ottenuti da essi e valutare se il training propriocettivo potrebbe essere efficace o meno nei soggetti che presentano l’instabilità cronica di caviglia. Metodi di ricerca: Le ricerche per questa revisione sistematica sono state fatte sulle banche dati “PubMed”, “PEDro” e “Cochrane Library”. Le parole chiave utilizzate sono “ankle sprains”, “chronic ankle instability”, “ankle injuries”, “proprioceptive training”, “proprioception”, “pain”, “disability” e “quality of life”. Sono stati inclusi soltanto RCTs inerenti al quesito di ricerca (PICOS) e conformi ai criteri di eleggibilità. Risultati: In questa revisione sistematica sono stati inclusi 4 RCTs, che mettono a confronto il training propriocettivo con altre metodiche riabilitative. Si sono osservati miglioramenti per quanto riguarda la disabilità, la qualità di vita, l’equilibrio dinamico, l’equilibrio statico e gli outcome riportati dai partecipanti. Conclusioni: Il training propriocettivo risulta essere efficace nel migliorare la sintomatologia data dall’instabilità cronica di caviglia e anche la stabilità e propriocezione dell’articolazione. Si dovrebbero eseguire ulteriori studi per approfondire l'efficacia di questo trattamento sul dolore e anche la sua efficacia a lungo termine.

A articulação tíbio-társica (tornozelo) é considerada uma das articulações mais relevantes e mais susceptível à lesão, sendo que a entorse do tornozelo consiste na lesão mais comum em desportistas, com uma percentagem de ocorrência de 35% no andebol. Este trabalho centra-se, assim, na problemática das entorses do tornozelo nos jogadores de andebol e da sua relação com o treino proprioceptivo como medida preventiva. O objectivo do presente trabalho foi a realização de uma revisão sistemática que estuda o treino proprioceptivo na prevenção da entorse do tornozelo, para isso, tentámos identificar a possível relação entre entorse do tornozelo e treino proprioceptivo. A metodologia utilizada neste trabalho foi uma revisão da literatura, sendo realizada uma pesquisa electrónica na base de dados da B-On, utilizando combinações de palavras-chave. No total resultaram 758 artigos, tendo sido pela sua relevância selecionados 29. Neste trabalho ficou evidente a relação do treino proprioceptivo com a prevenção da entorse do tornozelo. De todas as medidas preventivas o treino proprioceptivo é o que apresenta melhores resultados, pois, este tipo de treino está direccionado para uma estruturação da zona lesionada, fortalecendo os músculos e ligamentos, restaurando a componente proprioceptiva das estruturas danificadas e permitindo uma resposta mais rápida do Sistema Nervoso Central, aos vários estímulos a que está sujeito o atleta. Foi possível descrever em que consiste o treino proprioceptivo e os seus constituintes, adaptando-o ao andebol. Com esta revisão da literatura não foi possível obter um consenso sobre o efeito do treino proprioceptivo na prevenção de entorses do tornozelo.

Objectivo: Descrever a intervenção em fisioterapia aplicada num paciente com o diagnóstico de entorse da Tíbio-társica de grau III. Participantes e Métodos: Paciente que em 1 de Dezembro de 2009 na sequência de uma disputa de bola, sofreu uma entorse da tíbio-társica. Iniciou fisioterapia a 4 de Janeiro de 2010, tendo sido utilizadas para o tratamento técnicas de Mulligan, nomeadamente MWM, mobilização articular, reforço muscular e treino proprioceptivo. O tratamento foi realizado diariamente durante 6 semanas. Resultados: Após a aplicação das técnicas de Mulligan MWM na 1ªsemana, o perónio encontra-se devidamente reposicionado, no entanto o calcâneo ainda se encontra posteriorizado. No fim da 2ªsemana, a amplitude articular aumentou significativamente, aproximando-se dos valores do membro contralateral, verificouse um ganho de força muscular progressivo. Na 3ª e 4ª semana progrediu-se o tratamento em termos de fortalecimento muscular e treino proprioceptivo. Na 5ª semana apresentava amplitudes de movimento de 17º de dorsiflexão, 43º de flexãoplantar, 15º de eversão e 18º de inversão. Relativamente à força, o músculo tibial anterior apresentava um grau 5 de força, longo e curto peronial e flexores plantares grau 4+. No fim de 6 semanas, terminada a sua recuperação encontrava-se sem qualquer limitação funcional, igual força muscular bilateral e proprioceptividade normal. Conclusão: Foi salientado o processo de raciocínio clínico desenvolvido pelo fisioterapeuta ao longo das 6 semanas de intervenção: a recolha e interpretação dos dados da história, levantamento das primeiras hipóteses, testes dessas hipóteses no exame objectivo, estabelecimento de um diagnóstico funcional e construção e implementação de um plano de intervenção com a colaboração do utente que permitiram uma recuperação funcional que foi de encontro aos objectivos traçados.

Propriocepção refere-se à percepção dos mecanorreceptores para discriminar a posição do corpo e movimentos articulares, bem como tensões sobre os tendões na fase estática ou dinâmica da marcha. Objetivou-se avaliar por eletromiografia a ativação muscular do gastrocnêmio e tibial anterior em diferentes exercícios de propriocepção do tornozelo em apoio unipodal, comparando graus de dificuldade. Foram selecionados 54 voluntários, sedentários, destros, do sexo masculino (20-35 anos). Exercícios foram feitos no balancinho, prancha de equilíbrio, cama elástica e solo, à razão de três repetições de 15 segundos cada, com intervalo de 15 segundos entre as repetições. Ao final dos testes os voluntários indicaram a maior dificuldade. A atividade elétrica de ambos os músculos foi significativamente maior durante o teste no balancinho. No solo, ambos os músculos apresentaram menor atividade, mas apenas no gastrocnêmio essa diferença foi significativa. No exercício na prancha de equilíbrio e na cama elástica não se encontrou diferença quanto à ativação dos músculos. Na análise intermúsculo foi observada maior atividade do tibial anterior, exceto no balancinho. Assim, para o treino do apoio unipodal na aquisição do ganho proprioceptivo, o equipamento adotado deve ser escolhido com cuidado: no balancinho é maior o recrutamento dos músculos tibial anterior e gastrocnêmio, assim como é maior o grau de dificuldade para manutenção do equilíbrio.

Single Limb Stance under visual and proprioceptive disturbances is largely used in clinical settings in order to improve balance in a wide range of functional disabilities. However, the proper role of vision and proprioception in SLS is not completely understood. The objectives of this study were to test the hypotheses that when ankle proprioception is perturbed, the role of vision in postural control increases according to the difficulty of the standing task. And to test the effect of vision during postural adaptation after withdrawal of the somesthetic perturbation during double and single limb stance. Eleven males were submitted to double (DLS) and single limb (SLS) stances under conditions of normal or reduced vision, both with normal and perturbed proprioception. Center of pressure parameters were analyzed across conditions. Vision had a main effect in SLS, whereas proprioception perturbation showed effects only during DLS. Baseline stability was promptly achieved independently of visual input after proprioception reintegration. In conclusion, the role of vision increases in SLS. After proprioception reintegration, vision does not affect postural recovery. Balance training programs must take that into account. © 2011 Elsevier Ltd.

The present study examined a wrist extension-to-flexion contraction pattern that was theorized to result in proprioceptive neuromuscular facilitation. However, the “reversal of antagonists” contraction pattern may have, alternatively, interfered with motor learning-related increases in strength. Participants (N=24) were matched on predicted strength and randomly assigned to either the control or experimental group. Training occurred during three test sessions within a one-week period. Retention and transfer (crossed-condition) tests were administered during a fourth test session two- weeks later. Both groups exhibited comparable increases in strength (20.2%) and decreases in muscle coactivation (35.2%), which were retained and transferred. Decreases in error and variability of the torque traces were associated with parallel decreases in variability of muscle activity. The reversal of antagonists technique did not interfere with motor learning-related increases in strength and decreases in variability. However, the more complex contraction pattern failed to result in proprioceptive neuromuscular facilitation of strength.

Proprioceptive neuromuscular facilitation (PNF) is an attractive method to increase strength and proprioception of elderly individuals. However, a major clinical concern about the prescription of PNF is the belief that it can cause a cardiovascular overload, because it involves close-to-maximal loads and isometric contractions. Yet the acute effect of a PNF training session on cardiovascular response in elderly individuals is still unknown. Hence, the objective of this study was to evaluate the effect of PNF on diastolic and systolic blood pressure of healthy elderly people. Fifteen older women (mean age 72.40±6.82 years) performed three sets (five repetitions each) of three different PNF techniques (rhythmic initiation, dynamic reversion, and isotonic combination), executing a single movement pattern. Diastolic and systolic blood pressure (DBP and SBP) were evaluated by means of a manual sphygmomanometer immediately before and during the last two repetitions (last set) of each technique. A two-way ANOVA test (time and technique) was performed to investigate the PNF effect on blood pressure. No time (preexercise to postexercise) (p=0.33 for DBP; p=0.06 for SBP) or PNF technique (p=0.75; p=0.81) effect were observed. In conclusion, we can state that the execution of these PNF techniques is safe for the cardiovascular system of healthy elderly women, because no blood pressure increases were found. Maximal strength, number of repetitions, and total volume are differently affected by static-, ballistic-, and proprioceptive neuromuscular facilitation stretching. J Strength Cond Res 26(9): 2432-2437, 2012-Stretching exercises have been traditionally incorporated into warm-up routines before training sessions and sport events. However, the effects of stretching on maximal strength and strength endurance performance seem to depend on the type of stretching employed. The objective of this study was to compare the effects of static stretching (SS), ballistic stretching (BS), and proprioceptive neuromuscular facilitation (PNF) stretching on maximal strength, number of repetitions at a submaximal load, and total volume (i.e., number of repetitions 3 external load) in a multiple-set resistance training bout. Twelve strength-trained men (20.4 +/- 4.5 years, 67.9 +/- 6.3 kg, 173.3 +/- 8.5 cm) volunteered to participate in this study. All of the subjects completed 8 experimental sessions. Four experimental sessions were designed to test maximal strength in the leg press (i.e., 1 repetition maximum [1RM]) after each stretching condition (SS, BS, PNF, or no-stretching [NS]). During the other 4 sessions, the number of repetitions performed at 80% 1RM was assessed after each stretching condition. All of the stretching protocols significantly improved the range of motion in the sit-and-reach test when compared with NS. Further, PNF induced greater changes in the sit-and-reach test than BS did (4.7 +/- 1.6, 2.9 +/- 1.5, and 1.9 +/- 1.4 cm for PNF, SS, and BS, respectively). Leg press 1RM values were decreased only after the PNF condition (5.5%, p < 0.001). All the stretching protocols significantly reduced the number of repetitions (SS: 20.8%, p < 0.001; BS: 17.8%, p = 0.01; PNF: 22.7%, p < 0.001) and total volume (SS: 20.4%, p < 0.001; BS: 17.9%, p = 0.01; PNF: 22.4%, p < 0.001) when compared with NS. The results from this study suggest that, to avoid a decrease in both the number of repetitions and total volume, stretching exercises should not be performed before a resistance training session. Additionally, strength-trained individuals may experience reduced maximal dynamic strength after PNF stretching.

Sexual dysfunction (SD) affects up to 80% of multiple sclerosis (MS) patients and pelvic floor muscles (PFMs) play an important role in the sexual function of these patients. The objective of this paper is to evaluate the impact of a rehabilitation program to treat lower urinary tract symptoms on SD of women with MS. Thirty MS women were randomly allocated to one of three groups: pelvic floor muscle training (PFMT) with electromyographic (EMG) biofeedback and sham neuromuscular electrostimulation (NMES) (Group I), PFMT with EMG biofeedback and intravaginal NMES (Group II), and PFMT with EMG biofeedback and transcutaneous tibial nerve stimulation (TTNS) (Group III). Assessments, before and after the treatment, included: PFM function, PFM tone, flexibility of the vaginal opening and ability to relax the PFMs, and the Female Sexual Function Index (FSFI) questionnaire. After treatment, all groups showed improvements in all domains of the PERFECT scheme. PFM tone and flexibility of the vaginal opening was lower after the intervention only for Group II. All groups improved in arousal, lubrication, satisfaction and total score domains of the FSFI questionnaire. This study indicates that PFMT alone or in combination with intravaginal NMES or TTNS contributes to the improvement of SD.

The present study investigated the effects of running at 0.8 or 1.2 km/h on inflammatory proteins (i.e., protein levels of TNF- α , IL-1 β , and NF- κ B) and metabolic proteins (i.e., protein levels of SIRT-1 and PGC-1 α , and AMPK phosphorylation) in quadriceps of rats. Male Wistar rats at 3 (young) and 18 months (middle-aged rats) of age were divided into nonexercised (NE) and exercised at 0.8 or 1.2 km/h. The rats were trained on treadmill, 50 min per day, 5 days per week, during 8 weeks. Forty-eight hours after the last training session, muscles were removed, homogenized, and analyzed using biochemical and western blot techniques. Our results showed that: (a) running at 0.8 km/h decreased the inflammatory proteins and increased the metabolic proteins compared with NE rats; (b) these responses were lower for the inflammatory proteins and higher for the metabolic proteins in young rats compared with middle-aged rats; (c) running at 1.2 km/h decreased the inflammatory proteins and increased the metabolic proteins compared with 0.8 km/h; (d) these responses were similar between young and middle-aged rats when trained at 1.2 km. In summary, the age-related increases in inflammatory proteins, and the age-related declines in metabolic proteins can be reversed and largely improved by treadmill training.

To investigate the effects of a specific protocol of undulatory physical resistance training on maximal strength gains in elderly type 2 diabetics. The study included 48 subjects, aged between 60 and 85 years, of both genders. They were divided into two groups: Untrained Diabetic Elderly (n=19) with those who were not subjected to physical training and Trained Diabetic Elderly (n=29), with those who were subjected to undulatory physical resistance training. The participants were evaluated with several types of resistance training's equipment before and after training protocol, by test of one maximal repetition. The subjects were trained on undulatory resistance three times per week for a period of 16 weeks. The overload used in undulatory resistance training was equivalent to 50% of one maximal repetition and 70% of one maximal repetition, alternating weekly. Statistical analysis revealed significant differences (p<0.05) between pre-test and post-test over a period of 16 weeks. The average gains in strength were 43.20% (knee extension), 65.00% (knee flexion), 27.80% (supine sitting machine), 31.00% (rowing sitting), 43.90% (biceps pulley), and 21.10% (triceps pulley). Undulatory resistance training used with weekly different overloads was effective to provide significant gains in maximum strength in elderly type 2 diabetic individuals.

Endurance exercise training as well as leucine supplementation modulates glucose homeostasis and protein turnover in mammals. Here, we analyze whether leucine supplementation alters the effects of endurance exercise on these parameters in healthy mice. Mice were distributed into sedentary (C) and exercise (T) groups. The exercise group performed a 12-week swimming protocol. Half of the C and T mice, designated as the CL and TL groups, were supplemented with leucine (1.5 % dissolved in the drinking water) throughout the experiment. As well known, endurance exercise training reduced body weight and the retroperitoneal fat pad, increased soleus mass, increased VO2max, decreased muscle proteolysis, and ameliorated peripheral insulin sensitivity. Leucine supplementation had no effect on any of these parameters and worsened glucose tolerance in both CL and TL mice. In the soleus muscle of the T group, AS-160(Thr-642) (AKT substrate of 160 kDa) and AMPK(Thr-172) (AMP-Activated Protein Kinase) phosphorylation was increased by exercise in both basal and insulin-stimulated conditions, but it was reduced in TL mice with insulin stimulation compared with the T group. Akt phosphorylation was not affected by exercise but was lower in the CL group compared with the other groups. Leucine supplementation increased mTOR phosphorylation at basal conditions, whereas exercise reduced it in the presence of insulin, despite no alterations in protein synthesis. In trained groups, the total FoxO3a protein content and the mRNA for the specific isoforms E2 and E3 ligases were reduced. In conclusion, leucine supplementation did not potentiate the effects of endurance training on protein turnover, and it also reduced its positive effects on glucose homeostasis.

The aim of this present study was to investigate on the effects of concurrent training with blood flow restriction (BFR-CT) and concurrent training (CT) on the aerobic fitness, muscle mass and muscle strength in a cohort of older individuals. 25 healthy older adults (64.7±4.1 years; 69.33±10.8 kg; 1.6±0.1 m) were randomly assigned to experimental groups: CT (n=8, endurance training (ET), 2 days/week for 30-40 min, 50-80% VO2peak and RT, 2 days/week, leg press with 4 sets of 10 reps at 70-80% of 1-RM with 60 s rest), BFR-CT (n=10, ET, similar to CT, but resistance training with blood flow restriction: 2 days/week, leg press with 1 set of 30 and 3 sets of 15 reps at 20-30% 1-RM with 60 s rest) or control group (n=7). Quadriceps cross-sectional area (CSAq), 1-RM and VO2peak were assessed pre- and post-examination (12 wk). The CT and BFR-CT showed similar increases in CSAq post-test (7.3%, P<0.001; 7.6%, P<0.0001, respectively), 1-RM (38.1%, P<0.001; 35.4%, P=0.001, respectively) and VO2peak (9.5%, P=0.04; 10.3%, P=0.02, respectively). The BFR-CT promotes similar neuromuscular and cardiorespiratory adaptations as CT.
