Introduction Cette recherche constitue une étude clinique descriptive, visant à évaluer la douleur durant le traitement orthodontique avec boîtiers auto-ligaturants passifs et actifs. Matériel et méthode L'étude a été effectuée chez 39 patients (18 garçons, 21 filles), âge moyen 14 (entre 11 et 19 ans). Deux types de boîtiers auto-ligaturants ont été utilisés (SPEED n=20 et Damon n=19). Pour évaluer la douleur, un questionnaire a été élaboré par l'équipe de recherche. L’étude comportait 4 phases, c’est-à-dire l’évaluation de la douleur suite à l’insertion des 4 premiers fils orthodontiques du traitement de chaque patient (0.016 Supercable, 016 CuNiTi, 016X022 CuNiTi, 019X025 CuNiTi). Le même questionnaire était utilisé lors de chaque phase et le questionnaire comprenait 6 différents temps (T0: avant l’insertion du fil orthodontique, T1: immédiatement suite à l’insertion du fil, T2: 5h après l’insertion, T3: 24h après l’insertion, T4: 3 jours après l’insertion, T5: une semaine après l’insertion, T6: 4 semaines après l’insertion) suite à l’insertion de chaque fil. L’échelle visuelle analogue (EVA) et la version courte du questionnaire de Saint-Antoine ont été utilisés afin d’évaluer la douleur. Les données des EVA entre les groupes ont été comparées en utilisant le U test Mann-Whitney. Résultats et discussion Pour les deux premiers fils et pour tous les temps étudiés, il n’y avait pas de différence statistiquement significative entre les deux groupes (SPEED et Damon). Cependant, au moment de l’insertion (T0) du troisième fil (016X022 CuNiTi), parmi les patients ayant rapporté de la douleur (SPEED 47.1%, Damon 55.6%), le groupe Damon a rapporté une douleur significativement plus élevée que le groupe SPEED (p=0.018), (EVA moyenne SPEED=14.14±8.55, Damon=33.85±19.64). Trois jours après l’insertion du troisième fil, toujours parmi les patients ayant rapporté de la douleur (SPEED 23.5%, Damon 33.4%), la douleur était significativement plus élevée chez le groupe Damon que chez le groupe SPEED (p=0.008), (EVA moyenne SPEED=8.74±4.87, Damon=25.15±9.69). La plupart des analgésiques ont été pris suite à la pose du premier fil au temps T2 (5h) et T3 (24h). Il n’y avait pas de différence statistiquement significative entre les groupes en ce qui a trait au nombre de patients qui prenaient des analgésiques. La douleur n’a pas affecté le style de vie pour la grande majorité des patients. Les mots descriptifs sensoriels « tiraillement », « étau » et « élancement » et le mot affectif « énervant » étaient le plus souvent utilisés. Conclusion Les patients du groupe Damon ont ressenti significativement plus de douleur que les patients du groupe SPEED à l’insertion du troisième fil et trois jours suite à l’insertion. Plus de patients ont pris des médicaments pour la douleur avec le premier fil et le style de vie n’était pas affecté pour une majorité de patients. « Tiraillement », « étau », « élancement » et « énervant » étaient les mots descriptifs les plus utilisés par les patients pour décrire leur douleur.

Endodontic treatment is an important step of tooth replantation protocols, but the ideal moment for definitive obturation of replanted teeth has not yet been established. In this study, a histomorphometric analysis was undertaken to evaluate the repair process on immediate replantation of monkeys teeth after calcium hydroxide (CH) therapy for 1 and 6 months followed by root canal filling with a CH-based sealer (Sealapex (R)). The maxillary and mandibular lateral incisors of five female Cebus apella monkeys were extracted, kept in sterile saline for 15 min, replanted and splinted with stainless steel orthodontic wire and composite resin for 10 days. In Group I (control), definitive root canal filling was performed before tooth extraction. In Groups II and III, CH therapy started after removal of splint, and definitive root canal filling was performed 1 and 6 months later, respectively. The animals were euthanized 9 months after replantation, and specimens were processed for histomorphometric analysis. In all groups, epithelial attachment occurred at the cementoenamel junction or very close to this region; the areas of resorption on root surface had small extension and depth and were repaired by newly formed cementum; and the periodontal ligament was organized. Statistical analysis of the scores obtained for the histomorphometric parameters did not show any statistically significant difference (P = 0.1221) among the groups. The results suggests that when endodontic treatment is initiated 10 days after immediate replantation and an antibiotic regimen is associated, definitive root canal filling can be performed after a short-term CH therapy.

This paper reports a case of tooth replantation in which the splinting was performed with stainless steel orthodontic wire and a widely marketed brand of quick-setting cyanoacrylate ester adhesive (Super Bonder((R))). The long-term clinical and radiographic success of the case reported in this paper indicates that the splinting technique using a quick-setting cyanoacrylate ester adhesive may be a feasible option for making a rapid, simple and efficient contention of replanted teeth in situations where the routinely used materials are not readily available.

The interpretation of the set of radiographs taken during the follow-up period after tooth replantation might pose several difficulties, especially the inability to adequately reproduce the projection geometry of the exposures. This article describes a method for the geometric standardization of intraoral radiographs using a custom-made apparatus comprising a film-holder attached to an occlusal splint for the long-term follow up of dentoalveolar trauma. The method was applied in a patient who suffered an avulsion of the maxillary central incisors and had the teeth replanted after 4 h in saline storage. Endodontic treatment started 7 days after the trauma with changes of a calcium hydroxide intracanal medication every 15 days in the first 2 months and thereafter at 30-day intervals for 8 months. Root canal filling was carried out after this period. The radiographic exposures taken at the follow-up visits were standardized to identify the possible alterations during the repair process, such as root resorptions. A maxillary arch impression was made with alginate, and the model was cast in stone for fabrication of an acetate occlusal splint. The custom-made apparatus used for standardization of the radiographic exposures was fabricated by fixing a Rinn X-C-P film-holder and a 5-mm-long piece of 0.7-mm orthodontic wire to the occlusal splint with autopolymerized acrylic resin. Radiographs were taken at 4-month intervals, starting 10 months after replantation up to 76 months. The images were digitized and analysed using the Digora system. The length of the central incisors was determined to verify the reproduction of the projection geometry of the exposures and the orthodontic wire served to assess accuracy during length estimations in the radiographs. The method described in this article for geometric standardization of intraoral radiographs provided a consistent reproduction of the geometric exposure parameters, being indicated for use in the radiographic follow up of cases of dentoalveolar trauma. © 2012 John Wiley & Sons A/S.

Several alloys present the shape memory effect and among them, the equiatomic NiTi alloy, nitinol, is the most important one. It is usually used in several engineering applications and also in biomedical devices, in the fabrication of orthodontic wire, stents and Judet staples. Although a considerable amount of these biomedical devices is utilized in Brazil and a fraction of it is already made here, all nitinol used is bought abroad. Thus, it is important to develop the necessary know-how to fabricate NiTi wire and sheet. It would mean less importation with job creation and wealth generation for the country. In this work nitinol was obtained powder metallurgy from elemental powders of Ti and Ni using uniaxial compression and uniaxial compression followed by isostatic compression. The final densities achieved were determined by the Archimedes method. The precipitation of intermetallic secondary phases was studied and the samples were characterized by metallographic analysis, optical microscopy and X-ray diffraction. Results indicated that 50 hours sintering route showed a low amount of intermetallics, and no trace of unreacted powder. XRD and metallography at room temperature indicated B19’ as the predominant phase, which corresponds to martensite. Although density results showed little dispersion, the most dense sample was compacted under uniaxial compression and presented 4.8 g/cm3, corresponding to 20.84% porosity. O objetivo do presente estudo consistiu em comparar as forças de deflexão in vitro entre fios estéticos e fio NiTi, para verificar a força máxima e da região platô e determinar sua correlação com o diâmetro do fio com e sem revestimento. Foram realizados dez ensaios de sete marcas comerciais de arcos précontornados, sendo seis fios estéticos (Invu (I), Optis (O), Flexy Super Elastic Esthetic (FSEE), Niticosmetic (N), Orthocosmetic Elastinol (OE) e Pro Form Coated Nitanium (PFCN)) e um fio NiTi superelástico (Nitinol Super Elastic (NSE)), aplicando-se o teste de curvatura de três pontos, associado a máquina de ensaios universal EMIC. Desenvolveu-se um dispositivo com braquetes cerâmicos (Transcend), cujos fios foram presos por elásticos modulares, sendo obtidos os valores da força de deflexão em 3,0, 2,0 e 1,0mm. O diâmetro dos fios com e sem revestimento foram mensurados com o especímetro digital Micromaster. A análise de variância a um critério (p<0,05) mostrou diferença significante entre os fios e o teste post-hoc de Tukey determinou que a força de deflexão em 3mm para o O

O objetivo deste estudo foi avaliar a topografia de superfície dos fios estéticos, antes e após teste de deflexão. A amostra foi composta por 70 corpos de prova de fios 0,014 redondos, sendo 10 de cada uma das marcas comerciais avaliadas: Orthocosmetic Elastinol (Masel), Flexy Super Elastic Esthetic (Orthometric), InVu (TP Orthodontics) e ProForm Nitanium (Ortho Organizers) fios de NiTi revestidos por Teflon®; Optis (TP Orthodontics) fio de resina reforçado por fibra de vidro ou FRP; Niticosmetic (Tecnident) fio de NiTi revestido por resina epoxídica; e Nitinol SE (3M Unitek) fio de NiTi superelástico, usado para controle. A topografia de superfície de cada fio foi avaliada por rugosímetro e por microscópio óptico, antes e após ser submetido a ensaio de deflexão, no lado em que a força foi aplicada e no lado oposto a este. Cada fio foi defletido em 3,1mm, a uma velocidade de 1mm/min, com célula de carga de 5N a 36⁰C + 1⁰C. A análise de variância a três critérios (p<0,05) mostrou diferença significante entre os fios e o teste de Tukey mostrou que o fio Optis (TP Orthodontics) apresentou aumento nos parâmetros de rugosidade Ra, Rt e Rz, após a deflexão. O fio Niticosmetic (Tecnident) apresentou aumento na rugosidade média (Ra). O fio InVu (TP Orthodontics) foi o único que mostrou aumento na rugosidade no lado em que a força foi aplicada. A análise visual por meio de microscopia óptica revelou alterações na superfície em todos os fios estéticos após o teste de deflexão, desde delaminações do revestimento, observadas nos fios Orthocosmetic Elastinol e InVu, riscos permanentes na superfície, como visto nos fios Flexy Super Elastic Esthetic, Niticosmetic e ProForm Nitanium, e até mesmo fratura incompleta, no fio Optis. Concluiu-se que o fio Niticosmetic apresentou topografia de superfície similar ao fio metálico, e os demais fios estéticos apresentaram maior rugosidade e alterações visuaisna superfície.

Since the 1980s, different devices based on superelastic alloys have been developed to fulfill orthodontic applications. Particularly in the last decades several researches have been carried out to evaluate the mechanical behavior of Ni-Ti alloys, including their tensile, torsion and fatigue properties. However, studies regarding the dependence of elastic properties on residence time of Ni-Ti wires in the oral cavity are scarce. Such approach is essential since metallic alloys are submitted to mechanical stresses during orthodontic treatment as well as pH and temperature fluctuations. The goal of the present contribution is to provide elastic stress-strain results to guide the orthodontic choice between martensitic thermal activated and austenitic superelastic Ni-Ti alloys. From the point of view of an orthodontist, the selection of appropriate materials and the correct maintenance of the orthodontic apparatus are essential needs during clinical treatment. The present work evaluated the elastic behavior of Ni-Ti alloy wires with diameters varying from 0.014 to 0.020 inches, submitted to hysteresis tensile tests with 8% strain. Tensile tests were performed after periods of use of 1, 2 and 3 months in the oral cavity of patients submitted to orthodontic treatment. The results from the hysteresis tests allowed to exam the strain range covered by isostress lines upon loading and unloading, as well as the residual strain after unloading for both superelastic and thermal activated Ni-Ti wires. Superelastic Ni-Ti wires exhibited higher load isostress values compared to thermal activated wires. It was found that such differences in the load isostress values can increase with increasing residence time.

The aim of this study was to evaluate the tensile strength of orthodontic wires bonded onto the enamel with cyanoacrylate ester. To obtain the specimens, 120 human premolars (extracted for orthodontic or periodontal reasons) were included in acrylic blocks of rapid polymerization with three teeth each. Four groups were formed with ten specimens each. In the specimens, a dental splint model was made with cyanoacrylate ester and round stainless steel wire. In groups I, II and III, cyanoacrylate ester was used with round steel wires, with variation in diameter: 0.014 inches; 0.016 inches and 0.018 inches, respectively. In group IV, round steel wire 0.018 inches was used with photo polymerizing resin composite with previous acid etching. The adhesive force of the materials was measured in two points under the action of the tensiometer (ETM-USA). The number of loose wires was counted along with those that remained fixed according to the different levels of force applied because of the direction of the tensile force (vertical or horizontal) and the diameter of the wire used. The data obtained were first submitted to a descriptive analysis and then submitted to a statistical analysis (Friedman's Test and Dunn's Test of Multiple Comparison - Epi-info 3.2). Within the limitations of the experimental conditions presented, the cyanoacrylate ester or 'Super Bonder (R)' maintained bonded to enamel and steel wires (0.016 and 0.018 inches) during the tensile strength tests under different levels of applied forces.

The purpose of this study was to evaluate histologically, in dogs, the periodontal healing of 1-walled intraosseous defects in teeth that were subjected to orthodontic movement toward the defects. The defects were surgically created bilaterally at the mesial aspects of the maxillary second premolars and distal aspects of the mandibular second premolars of 4 mongrel dogs. One week after creating the defects, an orthodontic appliance was installed, and the teeth were randomly assigned to 1 of 2 treatment groups: those in the test group received a titanium-molybdenum alloy rectangular wire spring that performed a controlled tipping root movement, and those in the control group received a passive stainless steel wire. Active orthodontic movement of the test teeth lasted 2 months and was followed by a stabilization period of another 2 months, after which the animals were killed. Throughout the study, routine daily plaque control was performed on the dogs with a topical application of a 2% chlorhexicline gel. The results showed no difference between the groups, with some regularization of the defects and periodontal regeneration limited to the apical portion of the defects. Histometric analysis showed a significant difference in bone height; on average, it was 0.53 mm smaller in the test group. It was concluded that orthodontic movement does not interfere with the healing of 1-walled intraosseous defects, with the exception of the linear extent of new bone apposition.

Objective: To determine the mechanical characteristics of teardrop loop with and without helix fabricated using different metal alloy compositions (stainless steel and beta-titanium), submitted to different intensities of bends preactivation (0° and 40°), and with different cross-sectional dimension of the wire used to build these loops (0.017 x 0.025-in and 0.019 x 0.025-in). Methods: Eighty loops used to close spaces were submitted to mechanical tests. The magnitudes of horizontal force, the moment/force ratio, and the load/deflection ratio produced by the specimens were quantified. Loops were submitted to a total activation of 5.0 mm and the values were registered for each 1.0 mm of activation. For statistic data analysis, a analysis of variance was performed and a Tukey's Multiple Comparison test was used as supplement, considering a 5% level of significance. Results: In general, teardrop loops with helix produced lower magnitudes of horizontal force and load/deflection ratio, and higher moment/force ratio than teardrop loops without helix. Among all analyzed variables, metal alloy composition presented greater influence in the horizontal force and in the load/deflection ratio. The moment/force ratio showed to be more influenced by the preactivation of loops for space closure. © 2013 Dental Press Journal of Orthodontics.

INTRODUCTION: The opinion on the 'straight-wire' concept has been evolving since its origin, characterized by faithful followers or absolute skepticism. Currently, it seems reasonable to state that most professionals have a more realistic and critical viewpoint, with an attitude that reveals Orthodontics' maturity and greater knowledge on the technique. The most relevant criticisms refer to the impossibility of the both the Straight-Wire and the Standard systems to completely express the characteristics related to the brackets due to mechanical deficiencies, such as bracket/wire play. OBJECTIVES: A critical analysis of this relationship, which is unclear due to lack of studies, was the scope of this paper. METHODS: The compensatory treatment of two patients, using Capelozza's individualized brackets, works as the scenery for cephalometric evaluation of changes in incisor inclination produced by different dimensions of leveling archwires. RESULTS: The evaluation of these cases showed that, while the introduction of a 0.019 x 0.025-in stainless steel archwire in a 0.022 x 0.030-in slot did not produce significant changes in incisor inclination, the 0.021 x 0.025-in archwire was capable of changing it, mainly in mandibular incisors, and in the opposite direction to the compensation. CONCLUSION: Considering compensatory treatments, even when using an individualized prescription according to the malocclusion, the bracket/wire play seems to be a positive factor for malocclusion correction, without undesirable movements. Therefore, it seems reasonable to admit that, until a bracket system can have absolute individualization, the use of rectangular wires that still have a certain play with the bracket slot is advisable.
