In this study we examined the shape of the association between temperature and mortality in 13 Spanish cities representing a wide range of climatic and socio demographic conditions. The temperature value linked with minimum mortality (MMT) and the slopes before and after the turning point (MMT) were calculated. Most cities showed a V-shaped temperature-mortality relationship. MMTs were generally higher in cities with warmer climates. Cold and heat effects also depended on climate: effects were greater in hotter cities but lesser in cities with higher variability. The effect of heat was greater than the effect of cold. The effect of cold and MMT was, in general, greater for cardio-respiratory mortality than for total mortality, while the effect of heat was, in general, greater among the elderly

BACKGROUND A catheter-based approach after fibrinolysis is recommended if fibrinolysis is likely to be successful in patients with acute ST-elevation myocardial infarction. We designed a 2x2 randomized, open-label, multicenter trial to evaluate the efficacy and safety of the paclitaxel-eluting stent and tirofiban administered after fibrinolysis but before catheterization to optimize the results of this reperfusion strategy. METHODS AND RESULTS We randomly assigned 436 patients with acute ST-elevation myocardial infarction to (1) bare-metal stent without tirofiban, (2) bare-metal stent with tirofiban, (3) paclitaxel-eluting stent without tirofiban, and (4) paclitaxel-eluting stent with tirofiban. All patients were initially treated with tenecteplase and enoxaparin. Tirofiban was started 120 minutes after tenecteplase in those patients randomly assigned to tirofiban. Cardiac catheterization was performed within the first 3 to 12 hours after inclusion, and stenting (randomized paclitaxel or bare stent) was applied to the culprit artery. The primary objectives were the rate of in-segment binary restenosis of paclitaxel-eluting stent compared with that of bare-metal stent and the effect of tirofiban on epicardial and myocardial flow before and after mechanical revascularization. At 12 months, in-segment binary restenosis was similar between paclitaxel-eluting stent and bare-metal stent (10.1% versus 11.3%; relative risk, 1.06; 95% confidence interval, 0.74 to 1.52; P=0.89). However, late lumen loss (0.04+/-0.055 mm versus 0.27+/-0.057 mm, P=0.003) was reduced in the paclitaxel-eluting stent group. No evidence was found of any association between the use of tirofiban and any improvement in the epicardial and myocardial perfusion. Major bleeding was observed in 6.1% of patients receiving tirofiban and in 2.7% of patients not receiving it (relative risk, 2.22; 95% confidence interval, 0.86 to 5.73; P=0.14). CONCLUSIONS This trial does not provide evidence to support the use of tirofiban after fibrinolysis to improve epicardial and myocardial perfusion. Compared with bare-metal stent, paclitaxel-eluting stent significantly reduced late loss but appeared not to reduce in-segment binary restenosis. OBJECTIVE Zinc-α(2) glycoprotein (ZAG) stimulates lipid loss by adipocytes and may be involved in the regulation of adipose tissue metabolism. However, to date no studies have been made in the most extreme of obesity. The aims of this study are to analyze ZAG expression levels in adipose tissue from morbidly obese patients, and their relationship with lipogenic and lipolytic genes and with insulin resistance (IR). METHODS mRNA expression levels of PPARγ, IRS-1, IRS-2, lipogenic and lipolytic genes and ZAG were quantified in visceral (VAT) and subcutaneous adipose tissue (SAT) of 25 nondiabetic morbidly obese patients, 11 with low IR and 14 with high IR. Plasma ZAG was also analyzed. RESULTS The morbidly obese patients with low IR had a higher VAT ZAG expression as compared with the patients with high IR (p = 0.023). In the patients with low IR, the VAT ZAG expression was greater than that in SAT (p = 0.009). ZAG expression correlated between SAT and VAT (r = 0.709, p<0.001). VAT ZAG expression was mainly predicted by insulin, HOMA-IR, plasma adiponectin and expression of adiponectin and ACSS2. SAT ZAG expression was only predicted by expression of ATGL. CONCLUSIONS ZAG could be involved in modulating lipid metabolism in adipose tissue and is associated with insulin resistance. These findings suggest that ZAG may be a useful target in obesity and related disorders, such as diabetes.

El ciment ossi es prepara barrejant dos components, un sòlid (en format de pols) i l'altre líquid. En general, en la major part de productes comercials el constituent en pols conté polimetacrilat de metil (PMMA), un agent radioopac i peròxid de benzoïl (BPO) com a iniciador, mentre que el component líquid conté metacrilat de metil (MMA) com a monòmer i com activador la (N,Ndimetil- p-toluidina). Els continguts de cada constituent poden variar lleugerament d'una marca comercial a una altra. En barrejar els dos components es produeix la polimerització i té lloc l'enduriment del ciment. Es planteja la modificació de la formulació convencional del ciment ossi acrílic amb el component inorgànic majoritari dels ossos, és a dir, la hidroxiapatita (HA) (fosfat doble de sodi i calci) amb una relació molar (Ca/P) de 1,67. La HA i particularment la d’origen mineral o de síntesi, no és un component innovador en aquesta disciplina científica, però l’ús de la HA d’origen natural (procedent d’ossos de porc) introdueix la possibilitat d’incorporar un material molt econòmic que pot millorar la resistència mecànica del material sense perjudici de millorar la biocompatibilitat amb teixit ossi viu. Es planteja un treball de cerca d’un material que permeti la fixació de pròtesis articulades o bé pugui ser considerat com a sistema de farciment de cavitat òssies derivades de procesos osteoporòtics. Per arribar a trobar aquest material amb les propietats desitjades es preparen ciments amb diferents continguts en HA natural. Posteriorment es caracteritzen diferents propietats fisico-químiques dels materials les quals permetran prendre decisions a l’hora de determinar la viabilitat tècnica dels materials (tèrmic, mecànic i morfològic). Finalment, es realitza un estudi “in vitro” amb cultius cel•lulars per tal de determinar la biocompatibilitat del material

L'objectiu d'aquest projecte és analitzar com optimitzar el temps total d'escala d'una aeronau comercial, per tal de reduir els costos que suposa que l'aeronau estigui en terra. L'anàlisi es realitza a partir de conèixer quines són les operacions mínimes necessàries que cal dur a terme durant el tumaround i en quin ordre convé realitzar- les perquè el temps total sigui mínim. També s'estudien quins són els mètodes científics ja proposats i quins són els usats en l'actualitat. Fruit de l'estudi es proposa un model que permeti reduir els costos derivats de retards, afegint un temps addicional a l'escala, a partir de dades recollides de l'activitat comercial diària a l’aeroport de Barcelona. Dels resultats obtinguts es conclou que interessa afegir un buffer que absorbeixi possibles incidents, però convé que sigui dividit segons la franja horària del dia, per tal que s'adapti al màxim a les necessitats de cada vol, enlloc d'afegir un mateix buffer a tots els vols de qualsevol hora.

In this paper, we reflect about the broadening of the field of application of CRM from the business domain to a wider context of relationships in which the inclusion of non-profit making organizations seems natural. In particular, we focus on analyzing the suitability of adopting CRM processes by universities and higher educational institutions dedicated to e-learning. This is an issue that, in our opinion, has much potential but has received little attention in research so far.
