En aquellos pacientes que sufren un infarto de miocardio (IM), entender los procesos causantes de la muerte celular secundaria a la isquemia/reperfusión (I/R) es esencial para desarrollar estrategias capaces de prevenirla. La reciente disponibilidad de cepas de ratones transgénicos convierte al modelo murino con isquemia transitoria en una herramienta de gran interés para el estudio de los procesos causantes de esta muerte. El objetivo de este trabajo experimental ha sido establecer en nuestro laboratorio un modelo de I/R miocárdica in vivo en ratón mediante la oclusión de la arteria coronaria descendente anterior izquierda (ADA) con el fin de poder ser utilizado en futuros protocolos experimentales usando ratones transgénicos disponibles en el laboratorio.

En la pràctica diària hi ha síndromes coronàries amb elevació del segment ST que no compleixen de forma estricta els criteris diagnòstics de la definició universal de l'infart. Un 7,2% dels IAM de la nostra mostra presentaven una elevació del ST menor als criteris diagnòstics de l’IAM. El 70,4% d'aquests pacients no van rebre cap tractament de reperfusió en fase aguda. A la coronariografia el 73.4% dels pacients presentaven malaltia dels segments proximals o mitjos. Aquests IAM van presentar una baixa taxa de complicacions intrahospitalàries, però en el seguiment a 26,5 mesos destaca una important necessitat de noves revascularitzacions.

PGC-1α es un factor de transcripción maestro en la regulación mitocondrial de genes de protección frente a estrés oxidativo. Decidimos analizar el papel de la molécula en la regulación celular miocárdica tras infarto agudo. Evaluamos 38 pacientes con diagnóstico de SCACEST sometidos a estrategia de reperfusión. Encontramos que los pacientes con nivel basal de expresión reducido y mayor inducción de PGC-1α tras el evento presentaban infartos más extensos estimados por resonancia cardiaca. Concluimos que PGC-1α participa en la regulación de la respuesta celular frente a isquemia, en base a la activación de enzimas de protección mitocondrial.

L`electrocardiograma és la primera eina diagnòstica fàcilment disponible per la detecció de l´infart a la práctica clínica. El seu valor va ser donat principalment amb estudis antics anatomopatològics. La ressonància magnètica cardíaca actualment és la tècnica d`elecció per la detecció de l`infart. Aquest estudi investiga el valor de l`electrocardiograma ( sensibilitat i especificitat) per detectar infarts de la zona anteroseptal. Conclusiò: la sensibilitat y la especificitat de quatre patents electrocardiogràfiques de la zona anteroseptal va ser valorada. Així mateix, encara que s`observin extenses ones Q en les derivacions anteriors la necrosis és usualment limitada si VL no está afectat. 3

Background and Purpose Early prediction of motor outcome is of interest in stroke management. We aimed to determine whether lesion location at DTT is predictive of motor outcome after acute stroke and whether this information improves the predictive accuracy of the clinical scores. Methods We evaluated 60 consecutive patients within 12 hours of MCA stroke onset. We used DTT to evaluate CST involvement in the MC and PMC, CS, CR, and PLIC and in combinations of these regions at admission, at day 3, and at day 30. Severity of limb weakness was assessed using the m-NIHSS (5a, 5b, 6a, 6b). We calculated volumes of infarct and FA values in the CST of the pons. Results Acute damage to the PLIC was the best predictor associated with poor motor outcome, axonal damage, and clinical severity at admission (P&.001). There was no significant correlation between acute infarct volume and motor outcome at day 90 (P=.176, r=0.485). The sensitivity, specificity, and positive and negative predictive values of acute CST involvement at the level of the PLIC for 4 motor outcome at day 90 were 73.7%, 100%, 100%, and 89.1%, respectively. In the acute stage, DTT predicted motor outcome at day 90 better than the clinical scores (R2=75.50, F=80.09, P&.001). Conclusions In the acute setting, DTT is promising for stroke mapping to predict motor outcome. Acute CST damage at the level of the PLIC is a significant predictor of unfavorable motor outcome.

Registre per a estudiar l'impacte clínic de l'SCASEST a un hospital de tercer nivell. En un any van ingressar 310 pacients (54% Cardiologia, 46% Semicrítics/Unitat d'Estada Curta). La mortalitat hospitalaria va ser de l'1%; a 6 mesos va haver un 4,6% de morts i un 16% de reingressos. Edat, filtrat glomerular i FE van ser predictors de mortalitat; una troponina positiva i l'ingres a Cardiologia es van associar amb un menor risc de reingrés. Ingressar a Cardiologia es va associar a ser tributari de coronariografia i revascularització quirúrgica. El tipus de Servei pot tenir un impacte adicional en la història de l'SCASEST.

Aquest estudi té com a finalitat estudiar la repercussió clínica, bioquímica i electromecànica que pot implicar la pèrdua accidental de les branques auriculars durant l'angioplàstia coronària en l'home. Aquest propòsit es desenvolupa en dues parts. La primera d'elles consisteix en una revisió històrica del coneixement de la irrigació coronària auricular. En la segona d'elles, l'estudi pròpiament dit, es justifica el seu interès, es presenta la hipòtesi de treball i els objectius, s'exposa el seu disseny i es raona per què aquest escenari clínic pot resultar un model útil per analitzar les conseqüències de la isquèmia auricular aguda

El objetivo principal de este trabajo es evaluar el manejo intrahospitalario y al alta del SCA, para evaluar el grado de adherencia a las guías clínicas y ver su efecto en la evolución. Para ello realizamos registro continuo de pacientes consecutivos incluyendo los hospitalizados con diagnóstico de SCA y dolor torácico a estudio (DTE). Se ha realizado una primera evaluación durante el ingreso hospitalario y posteriormente al mes, 3 y 6 meses. Con respecto a los resultados y conclusiones destacar en primer lugar que la mayoría de los pacientes ingresados con el diagnóstico de dolor torácico a estudio muestran una baja probabilidad de cardiopatía isquémica. En el SCACEST la adherencia en cuanto a las recomendaciones de coronariografía y reperfusión son seguidas de acuerdo a otros registros publicados en la literatura. Se aprecia un manejo poco invasivo del SCASEST con porcentajes muy reducidos de cateterismo precoz en las primeras 24 horas en pacientes de riesgo moderado-alto. El tiempo de isquemia es uno de los aspectos claramente a mejorar en nuestro medio, en los dos tipos de SCA. En lo referido al manejo farmacológico, la adherencia a las recomendaciones es muy alta, incluso superior a las objetivadas en estudios publicados. En los pacientes con eventos cardiacos en el seguimiento se aprecia un manejo más conservador sin optar por una estrategia diagnóstico-terapéutica precoz, y un empleo menor de los fármacos de primera línea para la prevención secundaria de eventos coronarios.

Objetivos: 1. Evaluar la respuesta cardiovascular a dobutamina en la disfunción miocárdica secundaria a shock séptico. 2. Determinar si el análisis de la respuesta cronotrópica refleja la respuesta inotrópica. Material y métodos: pacientes con shock séptico y disfunción miocárdica (Fracción eyección ventrículo izquierdo, FEVI < 45%) que recibieron dobutamina (5 µg/kg/min) por persistencia de hipoperfusión tisular. Antes y 30 min después de su administración se recogieron parámetros ecocardiográficos y parámetros hemodinámicos. Datos: media y SD. Análisis estadístico mediante test de Wilcoxon /Mann Whitney. Resultados: Se incluyeron 22 pacientes. Dobutamina produjo un incremento significativo en la FC (90 ± 17 vs 107 ± 20 lpm, p < 0.0001), la FEVI (34 ± 7 vs 47 ± 10 %, p < 0.0001), el VSi eco (24.9 ± 5.9 vs 30 ± 7.4 ml/m2, p < 0.01) y el VSi td (28.6 ± 10.3 vs 34.7 ± 13.8 ml/m2, p < 0.005). Sólo tres pacientes no incrementaron ni FC ni VSi td. Respuesta cronotrópica significativa: sensibilidad 69% y especificidad 37% para detectar la respuesta inotrópica. Conclusiones: Dobutamina mejora la función cardiovascular en la disfunción miocárdica asociada a sepsis.

Little information is currently available from the various societies of cardiology on primary percutaneous coronary intervention (PCI) for acute myocardial infarction (AMI). Since primary PCI is the main method of reperfusion in AMI in many centres, and since of all cardiac emergencies AMI represents the most urgent situation for PCI, recommendations based on scientific evidence and expert experience would be useful for centres practising primary PCI, or those looking to establish a primary PCI programme. To this aim, a task force for primary PCI in AMI was formed to develop a set of recommendations to complement and assist clinical judgment. This paper represents the product of their recommendations.

Early repolarization, which is characterized by an elevation of the J-point on 12-lead electrocardiography, is a common finding that has been considered as benign for decades. However, in the last years, it has been related with vulnerability to idiopathic ventricular fibrillation and with cardiac mortality in the general population. Recently, 4 potential ECG predictors that could differentiate the benign from the malignant form of early repolarization have been suggested. Any previous study about early repolarization has been done in Spain. Aim. To ascertain whether the presence of early repolarization pattern in a resting electrocardiogram is associated with a major risk of cardiac death in a Spanish general population and to determine whether the presence of potential predictors of malignancy in a resting electrocardiogram increases the risk of cardiac mortality in patients with early repolarization pattern. Methods. We will analyse the presence of early repolarization and the occurrence of cardiac mortality in a retrospective cohort study of 4,279 participants aged 25 to 74 years in the province of Girona. This cohort has been followed during a mean of 9.8 years. Early repolarization will be stratified according to the degree of J-point elevation (≥0.1 mV or ≥0.2 mV), the morphology of the J-wave (slurring, notching or any of these two), the ST-segment pattern (ascending or descending) and the localization (inferior leads, lateral leads, or both). Association of early repolarization with cardiac death will be assessed by adjusted Cox-proportional hazards models

Objective: To determine the clinical characteristics of the orofacial pain of cardiac origin in patients visited when doing a treadmill exercise test, at the cardiology service of the Can Ruti Hospital in Badalona (Barcelona, Spain). Study design: The sample of that study included thirty patients visiteding when doing a treadmill exercise test, at the cardiology service. The questionnaire has been asked to a sample of 30 patients. Results: Eleven of the 30 patients included in this study presented craniofacial pain before or during the cardiac seizure. The location of the pain was bilateral, non-irradiated at the mandible in all cases. The intensity of the pain was from slight to severe. The frequency of the appearance of the pain was paroxysmal in 8 cases and constant in three cases, and the duration was from a few hours to a maximum of 14 days. Discussion: The cardiac pain in craniofacial structures is usually bilateral, compared to odontogenic pain which is always unilateral. The pain of cardiac origin is considered atypical because of its location, but about the 10 % of the cases, the cardiac ischemia has its primary manifestation in orofacial structures. Conclusions: Eleven patients referred a bilateral non-irradiated mandibular pain, with intensity from slight to severe, and with a paroxystic frequency in eight cases and a constant frequency in three cases. Just one patient referred pain during the treadmill exercise test. In all cases the pain disappeared after the cardiac surgery or the administration of vasodilators.

Background:The direct-acting platelet P2Y receptor antagonist ticagrelor can reduce the incidence of major adverse cardiovascular events when administered at hospital admission to patients with ST-segment elevation myocardial infarction (STEMI). Whether prehospital administration of ticagrelor can improve coronary reperfusion and the clinical outcome is unknown. Methods: We conducted an international, multicenter, randomized, double-blind study involving 1862 patients with ongoing STEMI of less than 6 hours' duration, comparing prehospital (in the ambulance) versus in-hospital (in the catheterization laboratory) treatment with ticagrelor. The coprimary end points were the proportion of patients who did not have a 70% or greater resolution of ST-segment elevation before percutaneous coronary intervention (PCI) and the proportion of patients who did not have Thrombolysis in Myocardial Infarction flow grade 3 in the infarct-related artery at initial angiography. Secondary end points included the rates of major adverse cardiovascular events and definite stent thrombosis at 30 days. Results: The median time from randomization to angiography was 48 minutes, and the median time difference between the two treatment strategies was 31 minutes. The two coprimary end points did not differ significantly between the prehospital and in-hospital groups. The absence of ST-segment elevation resolution of 70% or greater after PCI (a secondary end point) was reported for 42.5% and 47.5% of the patients, respectively. The rates of major adverse cardiovascular events did not differ significantly between the two study groups. The rates of definite stent thrombosis were lower in the prehospital group than in the in-hospital group (0% vs. 0.8% in the first 24 hours; 0.2% vs. 1.2% at 30 days). Rates of major bleeding events were low and virtually identical in the two groups, regardless of the bleeding definition used

Objectives: To examine the safety and effectiveness of cobalt-chromium everolimus eluting stents compared with bare metal stents. Design: Individual patient data meta-analysis of randomised controlled trials. Cox proportional regression models stratified by trial, containing random effects, were used to assess the impact of stent type on outcomes. Hazard ratios with 95% confidence interval for outcomes were reported. Data sources and study selection: Medline, Embase, the Cochrane Central Register of Controlled Trials. Randomised controlled trials that compared cobalt-chromium everolimus eluting stents with bare metal stents were selected. The principal investigators whose trials met the inclusion criteria provided data for individual patients. Primary outcomes: The primary outcome was cardiac mortality. Secondary endpoints were myocardial infarction, definite stent thrombosis, definite or probable stent thrombosis, target vessel revascularisation, and all cause death. Results: The search yielded five randomised controlled trials, comprising 4896 participants. Compared with patients receiving bare metal stents, participants receiving cobalt-chromium everolimus eluting stents had a significant reduction of cardiac mortality (hazard ratio 0.67, 95% confidence interval 0.49 to 0.91; P=0.01), myocardial infarction (0.71, 0.55 to 0.92; P=0.01), definite stent thrombosis (0.41, 0.22 to 0.76; P=0.005), definite or probable stent thrombosis (0.48, 0.31 to 0.73; P<0.001), and target vessel revascularisation (0.29, 0.20 to 0.41; P<0.001) at a median follow-up of 720 days. There was no significant difference in all cause death between groups (0.83, 0.65 to 1.06; P=0.14). Findings remained unchanged at multivariable regression after adjustment for the acuity of clinical syndrome (for instance, acute coronary syndrome v stable coronary artery disease), diabetes mellitus, female sex, use of glycoprotein IIb/IIIa inhibitors, and up to one year v longer duration treatment with dual antiplatelets. Conclusions: This meta-analysis offers evidence that compared with bare metal stents the use of cobalt-chromium everolimus eluting stents improves global cardiovascular outcomes including cardiac survival, myocardial infarction, and overall stent thrombosis.
