INTRODUCTION: Antifungal susceptibility testing assists in finding the appropriate treatment for fungal infections, which are increasingly common. However, such testing is not very widespread. There are several existing methods, and the correlation between such methods was evaluated in this study. METHODS: The susceptibility to fluconazole of 35 strains of Candida sp. isolated from blood cultures was evaluated by the following methods: microdilution, Etest, and disk diffusion. RESULTS: The correlation between the methods was around 90%. CONCLUSIONS: The disk diffusion test exhibited a good correlation and can be used in laboratory routines to detect strains of Candida sp. that are resistant to fluconazole.

At present, most Neisseria gonorrhoeae testing is done with ß-lactamase and agar dilution tests with common therapeutic agents. Generally, in bacteriological diagnosis laboratories in Argentina, study of antibiotic susceptibility of N.gonorrhoeae is based on ß-lactamase determination and agar dilution method with common therapeutic agents. The National Committee for Clinical Laboratory Standards (NCCLS) has recently described a disk diffusion test that produces results comparable to the reference agar dilution method for antibiotic susceptibility of N.gonorrhoeae, using a dispersion diagram for analyzing the correlation between both techniques. We obtained 57 gonococcal isolates from patients attending a clinic for sexually transmitted diseases in Tucumán, Argentina. Antibiotic susceptibility tests using agar dilution and disk diffusion techniques were compared. The established NCCLS interpretive criteria for both susceptibility methods appeared to be applicable to domestic gonococcal strains. The correlation between the MIC's and the zones of inhibition was studied for penicillin, ampicillin, cefoxitin, spectinomycin, cefotaxime, cephaloridine, cephalexin, tetracycline, norfloxacin and kanamycin. Dispersion diagrams showed a high correlation between both methods.

The correlation between the microdilution (MD), Etest (R) (ET), and disk diffusion (DD) methods was determined for amphotericin B, itraconazole and fluconazole. The minimal inhibitory concentration (MIC) of those antifungal agents was established for a total of 70 Candida spp. isolates from colonization and infection. The species distribution was: Candida albicans (n = 27), C. tropicalis (n = 17), C. glabrata (n = 16), C. parapsilosis (n = 8), and C. lusitaniae (n = 2). Non-Candida albicans Candida species showed higher MICs for the three antifungal agents when compared with C. albicans isolates. The overall concordance (based on the MIC value obtained within two dilutions) between the ET and the MD method was 83% for amphotericin B, 63% for itraconazole, and 64% for fluconazole. Considering the breakpoint, the agreement between the DD and MD methods was 71% for itraconazole and 67% for fluconazole. The DD zone diameters are highly reproducible and correlate well with the MD method, making agar-based methods a viable alternative to MD for susceptibility testing. However, data on agar-based tests for itraconazole and amphotericin B are yet scarce. Thus, further research must still be carded out to ensure the standardization to other antifungal agents. J. The CLSI M100-S19 document has recommended the disuse of vancomycin disks for staphylococci and informed that studies on the action of teicoplanin in disk-diffusion testing should be performed. We describe the comparison of two methods, disk diffusion and broth microdilution, for determining teicoplanin susceptibility in clinical isolates of staphylococci. Overall results showed an aggregation rate of 96.8%; Staphylococcus aureus showed total agreement while S. epidermidis showed 93.8% of agreement. According to these local results, disk diffusion can still be employed to teicoplanin susceptibility determination for staphylococci in our institution.

Antifungal activity of natural products has been tested by adapting methods designed for synthetic drugs. In this study, two methods for the determination of antifungal activity of natural products, agar diffusion and broth microdilution, the CLSI reference methods for synthetic drugs, are compared and discussed. The microdilution method was more sensitive. The minimal inhibitory concentrations (MIC) of crude extracts, fractions and pure substances from different species of the plant families Piperaceae, Rubiaceae, Clusiaceae, Fabaceae and Lauraceae, from the Biota project, were determined. Antifungal activities against Candida albicans, C.krusei, C.parapsilosis and Cryptococcus neoformans were produced by several samples.

Geographic differences in frequency and azole resistance among Candida glabrata may impact empiric antifungal therapy choice. We examined geographic variation in isolation and azole susceptibility of C. glabrata. We examined 23 305 clinical isolates of C. glabrata during ARTEMIS DISK global surveillance. Susceptibility testing to fluconazole and voriconazole was assessed by disk diffusion, and the results were grouped by geographic location: North America (NA) (2470 isolates), Latin America (LA) (2039), Europe (EU) (12 439), Africa and the Middle East (AME) (728), and Asia-Pacific (AP) (5629). Overall, C. glabrata accounted for 11.6% of 201 653 isolates of Candida and varied as a proportion of all Candida isolated from 7.4% in LA to 21.1% in NA. Decreased susceptibility (S) to fluconazole was observed in all geographic regions and ranged from 62.8% in AME to 76.7% in LA. Variation in fluconazole susceptibility was observed within each region: AP (range, 50-100% S), AME (48-86.9%), EU (44.8-88%), LA (43-92%), and NA (74.5-91.6%). Voriconazole was more active than fluconazole (range, 82.3-84.2% S) with similar regional variation. Among 22 sentinel sites participating in ARTEMIS from 2001 through 2007 (84 140 total isolates, 8163 C. glabrata), the frequency of C. glabrata isolation increased in 14 sites and the frequency of fluconazole resistance (R) increased in 11 sites over the 7-year period of study. The sites with the highest cumulative rates of fluconazole R were in Poland (22% R), the Czech Republic (27% R), Venezuela (27% R), and Greece (33% R). C. glabrata was most often isolated from blood, normally sterile body fluids and urine. There is substantial geographic and institutional variation in both frequency of isolation and azole resistance among C. glabrata. Prompt species identification and fluconazole susceptibility testing are necessary to optimize therapy for invasive candidiasis.

INTRODUCTION: Excessive group 2 carbapenem use may result in decreased bacterial susceptibility. OBJECTIVE: We evaluated the impact of a carbapenem stewardship program, restricting imipenem and meropenem use. METHODS: Ertapenem was mandated for ESBL-producing Enterobacteriaceae infections in the absence of non-fermenting Gram-negative bacilli (GNB) from April 2006 to March 2008. Group 2 carbapenems were restricted for use against GNB infections susceptible only to carbapenems and suspected GNB infections in unstable patients. Cumulative susceptibility tests were done for nosocomial pathogens before and after restriction using Clinical and Laboratory Standards Institute (CLSI) guide-lines.Vitek System or conventional identification methods were performed and susceptibility testing done by disk diffusion according to CLSI.Antibiotic consumption (t-test) and susceptibilities (McNemar's test) were determined. RESULTS: The defined daily doses (DDD) of group 2 carbapenems declined from 61.1 to 48.7 DDD/1,000 patient-days two years after ertapenem introduction (p = 0.027). Mean ertapenem consumption after restriction was 31.5 DDD/1,000 patient-days. Following ertapenem introduction no significant susceptibility changes were noticed among Gram-positive cocci. The most prevalent GNB were P. aeruginosa, Klebsiella pneumoniae, and Acinetobacter spp. There was no change in P. aeruginosa susceptibility to carbapenems. Significantly improved P. aeruginosa and K. pneumoniae ciprofloxacin susceptibilities were observed, perhaps due to decreased group 2 carbapenem use. K. pneumoniae susceptibility to trimethoprim-sulfamethoxazole improved. CONCLUSION: Preferential use of ertapenem resulted in reduced group 2 carbapenem use, with a positive impact on P. aeruginosa and K. pneumoniae susceptibility.

A Escherichia coli de aderência difusa (DAEC), um patotipo diarreiogênico de E. coli, corresponde a um grupo heterogêneo sem marcador de virulência comum a todos os isolados e de papel controverso na diarreia infantil. O objetivo deste estudo foi caracterizar genotipica e fenotipicamente amostras de DAEC, portadoras e não portadoras de adesinas Afa/Dr, isoladas de crianças com e sem diarreia. Em 70 amostras de DAEC, PCR foi realizado para pesquisa de genes descritos em DAEC, EAEC ou UPEC, que codificam: (i) oito adesinas fimbriais e afimbriais (fimH, papC, sfa, aggA, aafA, agg3A, aidA/aah, afaC); (ii) cinco toxinas (pet, astA, set1A, sat, hlyA); (iii) três proteínas captadoras/receptora de ferro (irp2, iucA, chuA/shuA); (iv) invasina (daaD) e; antígeno 43 (agn43). Ensaio de formação de biofilme foi realizado a partir da bactéria cultivada em caldo Luria-Bertani e inoculada em placas de poliestireno com DMEM suplementado com 0,4% glicose. A leitura da densidade ótica (DO490) foi realizada após coloração com safranina. Soroaglutinação para 23 antígenos O (Probac do Brasil) foi realizada em 50% das DAEC. Método de difusão de disco foi realizado para testar a suscetibilidade a 13 antimicrobianos. A presença de pelo menos um gene que codifica adesinas, toxinas, proteínas captadoras/receptora de ferro, invasina ou antígeno 43 foram encontrados em 58,6%, 51,4%, 80%, 48,6% e 57,1%, respectivamente, com os genes fimH, irp2, agn43, iucA, chuA/shuA, presentes em mais de 50% das amostras. Gene afaC+ (PCR) e/ou sonda afaBC+ (hibridização de colônias) classificou 50% das DAEC como Afa/Dr, sendo pet, sat, irp2, iucA, chuA/shuA e agn43 significantes nessas amostras (p<0,05). Do total das DAEC, 44,3% foram formadoras de biofilme, igualmente distribuídas entre as Afa/Dr e não Afa/Dr, e nenhum gene foi associado com esse fenótipo. Sorologia de 35 amostras evidenciou os seguintes sorogrupos: 1 O29, 2 O125, 2 O127 e 7 O86. Todas as O86 foram de DAEC Afa/Dr. Maiores frequências de resistência antimicrobiana foram encontradas para ampicilina (55,7%), sulfametoxazol/trimetoprim (35,7%) e tetraciclina (28,6%) e o perfil resistente/intermediário para amoxicilina/ácido clavulânico, ampicilina, sulfametoxazol/trimetoprim foi significante nas DAEC Afa/Dr, assim como a multi-droga resistência (p<0,05). Em conclusão, observou-se: (i) alta frequência de fimH e pet e presença de agn43, até então não descrito em DAEC, em frequências similares àquelas encontradas em EAEC, UPEC e EAEC/UPEC, respectivamente; (ii) que as amostras de DAEC Afa/Dr e não Afa/Dr constituíram grupos com perfis genéticos diferenciados entre si; (iii) poucos sorogrupos foram encontrados entre as DAEC; (iv) frequências de resistência menores quando comparado com as poucas descrições em DAEC, sugerindo uma menor pressão seletiva da população do presente estudo e; (v) amostras de DAEC Afa/Dr podem representar um importante reservatório de genes de resistência a antimicrobianos, além de diversos fatores de virulência.

Fiber meshes of poly(hydroxybutyrate) (PHB) and poly(hydroxybutyrate)/ poly(ethylene oxide) (PHB/PEO) with different concentrations of chlorhexidine (CHX) were prepared by electrospinning, for assessment as a polymer based drug delivery system. The electrospun fibers were characterized at morphological, molecular and mechanical levels. The bactericidal potential of PHB and PHB/PEO electrospun fibers with and without CHX was investigated against Escherichia coli (E. coli) and Staphylococcus aureus (S. aureus) by disk diffusion susceptibility tests. Electrospun fibers containing CHX exhibited bactericidal activity. PHB/PEO-1%CHX displayed higher CHX release levels and equivalent antibacterial activity when compared to PHB/PEO with 5 and 10 wt% CHX. Bactericidal performance of samples with 1 wt% CHX was assessed by Colony Forming Units (CFU), where a reduction of 100 % and 99.69 % against E. coli and S. aureus were achieved, respectively.

O objetivo deste estudo consiste em avaliar a atividade antimicrobiana da quinoxalina 1,4-dióxido e alguns dos seus derivados em estirpes bacterianas e leveduras. Os compostos estudados foram a quinoxalina 1,4-dióxido (QNX), 2-metilquinoxalina-1,4-dióxido (2MQNX), 2-metil-3-Benzoilquinoxalina-1,4-dióxido (2M3BenzoilQNX), 2-metil-3-benzilquinoxalina-1,4-dióxido (2M3BQNX), 2-amino-3-cianoquinoxalina-1,4-dióxido (2A3CQNX), 3-metil-2-quinoxalinacarboxamida-1,4-dióxido (3M2QNXC), 2-hidroxifenazina–N-dióxido (2HF) e 3-metil-N-(2-metilphenil)quinoxalinacarboxamida-1,4-dioxido (3MN(2MF)QNXC). Os modelos procariotas selecionados para este estudo foram o Staphylococcus aureus ATCC 6538, Staphylococcus aureus ATCC 6538P, Staphylococcus aureus ATCC 29213, Escherichia coli ATCC 25922, Escherichia coli S3R9, Escherichia coli S3R22, Escherichia coli TEM CTX-M9, Escherichia coli TEM-1, Escherichia coli AmpC MOX-2, Escherichia coli CTX-M2 e Escherichia coli CTX-M9. A Candida albicans ATCC 10231 e a Saccharomyces cerevisiae PYCC 4072 constituíram os modelos eucariotas deste estudo. Para os compostos químicos que apresentem atividade pelo método de difusão em disco, será determinada a Concentração Mínima Inibitória (CMI), bem como a viabilidade e o crescimento (na presença e na ausência dos compostos químicos). Os resultados deste estudo mostram atividade antimicrobiana para a maioria dos compostos estudados em todos os modelos procariotas Gram negativos, à exceção da E.coli CTX-M2 e CTX-M9 e nenhuma atividade nos modelos eucariotas. O estudo da viabilidade/curvas de morte em bactérias e num modelo eucariota (S.cerevisiae) sugerem que alguns destes compostos constituem potenciais drogas para a quimioterapia antibacteriana.

A água é um recurso escasso e indispensável à vida, podendo ser um importante veículo de microrganismos patogénicos com origem fecal. A matéria fecal é também uma fonte de microrganismos resistentes a antimicrobianos e contribui para a sua disseminação e dos seus genes de resistência no ambiente e entre as comunidades microbianas comensais e microrganismos patogénicos humanos e animais. A qualidade microbiológica da água é monitorizada recorrendo à utilização de bioindicadores como Escherichia coli, enterococos e microrganismos totais. O presente estudo apresentou como principal objetivo determinar a prevalência de ESBLs e AmpCs e ainda avaliar a prevalência de estirpes de enterococos com resistência à vancomicina (VRE) em águas do rio Douro e da orla costeira da cidade do Porto. As amostragens de água foram realizadas em quatro locais localizados no estuário do rio Douro e orla costeira da cidade do Porto entre o mês de Abril e Julho. A deteção e quantificação dos bioindicadores foram realizadas pelo método de filtração por membrana. A suscetibilidade das estirpes de E. coli e enterococos foi testada pelo método de difusão em disco relativamente a várias classes de antimicrobianos. As contagens microbianas mais elevadas foram determinadas entre Abril e Junho e em amostras de água doce. Foram isoladas 62 estirpes de E. coli e 49 estirpes de enterococos que apresentaram prevalências de resistência a antimicrobianos de 90,3% (56/62) e 83,7% (41/49), respetivamente. As estirpes de E. coli apresentaram altas frequências de resistência à ampicilina (74,2%) e tetraciclina (61,3%). Nestas estirpes verificou-se ainda fenótipos associados a multirresistência a um mínimo de três classes de antimicrobianos em 56,5% (35/62) dos isolados. Verificou-se que as estirpes de enterococos apresentaram altos níveis de resistência à rifampicina (34,7%) e azitromicina (40,8%), detetando-se ainda a manifestação de fenótipo de resistência à vancomicina em 26,5% das estirpes. Observou-se uma prevalência de 36,7% (18/49) de estirpes de enterococos associadas a fenómenos de multirresistência antimicrobiana. Ana Martins vi Os resultados obtidos sugerem que o rio Douro e orla costeira, bem como os ambientes aquáticos, constituem reservatórios de bactérias e genes de resistência a antimicrobianos e possuem um papel preponderante na sua disseminação.

In the present study were evaluated the DNA macrorestriction profile and SCCmec types for nine multi-resistant MRSA selected. Also antimicrobial susceptibility testing by disk diffusion method was evaluated for 68 MRSA isolates against 12 antimicrobial agents. The isolates were recovered from blood culture collected from hospitalized patients in three hospitals of Porto Alegre, Brazil. PFGE and PCR for mecA and SCCmec I, II, III, IV types genes were done on selected nine isolates with susceptibility only to vancomycin, teicoplanin and linezolid. Two clone profiles, with five subtypes, were demonstrated among multi-resistant MRSA analyzed. Eight isolates showed harbor SCCmec type III and one isolate was not typeable. The knowledge of SCCmec type, clone and antimicrobial profiles among S. aureus is essential mainly to prevention and control of dissemination of the antimicrobial resistance.

We describe an in vivo evolution of an antimicrobial profile from susceptibility to full-resistance to carbapenems, with heteroresistance as an intermediate stage, in an Acinetobacter baumannii strain. Heteroresistance was characterized by the growth of sub-populations within the susceptibility halo in both disk-diffusion and Etest. PCRs for the main A. baumannii carbapenemases were negative. The exact resistance mechanism, diagnostic methods and clinical relevance of heteroresistance in A. baumannii warrant further investigations. This is the first description of such phenomenon in vivo and the second report of heteroresistance to carbapenems in A. baumannii.
