Vídeo produzido com o GoAnimate! que apresenta a visita da agente de saúde à Antonieta, que há cinco dias deu a luz a Leonardo. Após análise da caderneta de saúde da criança, a agente confirma a aplicação da vacinação adequada e a anotação correta dos índices antropométricos. A mãe ainda refere que seu filho apresentou odor pútrido no cordão umbilical, aumento da temperatura corporal e choro constante.

É durante o puerpério que o Enfermeiro ESMO cuida do RN de forma a promover o bem-estar e potenciar a sua saúde, incluindo os cuidados ao coto umbilical. Parece existirem diferentes práticas nos cuidados ao coto umbilical do RN, que se caraterizam pela não uniformização e nem sempre baseadas em evidência científica. Na prestação de cuidados surgem, às vezes, dúvidas. A primeira premissa para sanar estas questões, baseia-se no recurso à evidência, sendo a prática baseada na evidência a mais aceite e deverá ser praticada.

O Enfermeiro Especialista de Saúde Materna e 0bstetrícia é confrontado diariamente com a necessida de prestar cuidados ao coto umbilical do recém-nascido. Estes cuidados foram, ao longo dos anos, sofrendo alterações significativas devido ao impacto das infecções do colo umbilical na mortalidade neonatal. Parecem existir diferentes práticas nos cuidados ao coto umbilical do recém-nascido, que se caracterizam pela não uniformização nos cuidados e incidem sobretudo em técnicas enraizadas nas instituições, em vez de baseadas em evidência científica.

O Enfermeiro Especialista de Saúde Materna, Obstétrica e Ginecológica possui competências com responsabilidade em diversas áreas de atividade. The Rooming favors the permanence of the mother with the newborn, thus establishing a stronger link between the mother and baby, in addition, care assistance and guidance are essential to both through the health care team because they recognize the moments critical that their interventions are necessary to ensure the health of both. The educational lecture was an option to have the opportunity to assist, guide and answer possible questions that may arise for mothers, and also to assess the knowledge of puéperas regarding the care of babies and self care. There was participation from 40 patients. In the study methodology was developed a questionnaire containing two subjects (self-care and care for newborns) was given to patients before and after the lecture held by the author in a rooming Maternity HC de Botucatu. The results were analyzed statistically and studied by the author, allowing note that many significant results were obtained after the lecture, which we realize that the same intervention on self-care was significant in respect of: bleeding, infection in the genital tract and urinary tract, surgical dehiscence , care of the lower limbs and constipation, how to care for the newborn was the significance to the questions about the hygiene of the umbilical stump, cramps, hunger, weeping and pacifiers. In the pre-lecture questions 73.00% of 27.00% were right and wrong. Our conclusion is that the mothers have a high level of prior knowledge and educational lectures enriches their knowledge in a creative and participatory

O período de pré-natal é uma época onde a mulher passa por muitas modificações e tende a sofrer por insegurança, necessitando assim ser apoiada por profissionais da saúde capacitados para orientá-la quanto aos cuidados necessários com a criança. Com o objetivo de propor um guia de cuidados ao recém-nascido a ser incorporado ao material de treinamento das equipes de saúde da família e também utilizado nas orientações às puérperas, foi realizada uma revisão bibliográfica através de manuais do Ministério as Saúde e bases de dados do SciELO na área temática sobre os cuidados com o recém-nascido. Tal guia vem abordar cuidados importantes e necessários com o recém-nascido, tais como: crescimento e desenvolvimento da criança menor de um ano, prevenção de acidentes, saúde bucal e higiene oral, diarréia e desidratação, higiene pessoal e do ambiente, vacinação, amamentação e cuidados com o coto umbilical. Espera-se, com a presente pesquisa, estimular a organização do trabalho dos profissionais da saúde e auxiliar as puérperas para que os recém-nascidos recebam um atendimento com qualidade.

Buffalo is an important livestock resource, with a great participation in agricultural systems, providing milk, meat, and work power. Umbilical cord is responsible for maternal-fetal nutrients exchange during pregnancy, and its alterations can compromise the fetal development. We investigated ten pregnant uteruses collected from cross-bread buffaloes in different stages of gestation. Pregnancy and fetal age was determined by measuring the apex sacral length and development period was calculated by previously published formula. Umbilical cords were measured for length determination. Umbilical cord vascular net and anastomosis were observed by injection of Neoprene latex. Histological sections of the umbilical cord were studied after stain with HE, picrossirius, toluidine blue, orceine, and PAS reaction. Buffaloes' umbilical cord was formed by two central arteries, an allantois duct and two peripheral veins. The artery wall was composed by large quantity of collagen, elastic fibers, fibroblasts and large number of vasa vasorum. The allantois duct was located between the arteries and presented a great number of small nourishing vessels. Small nourishing vessels should be carefully considered to avoid to be mistaken to the arterials and veins vasa vasorum. Medium length of umbilical cord from buffalos was 11.8cm (minimum of 6.8cm and maximum of 17.4cm).

The canine model provides a large animal system to evaluate many treatment modalities using stem cells (SCs). However, only bone marrow ( BM) protocols have been widely used in dogs for preclinical approaches. BM donation consists of an invasive procedure and the number and differentiation potential of its mesenchymal stem cells (MSCs) decline with age. More recently, umbilical cord was introduced as an alternative source to BM since it is obtained from a sample that is routinely discarded. Here, we describe the isolation of MSCs from canine umbilical cord vein (cUCV). These cells can be obtained from every cord received and grow successfully in culture. Their multipotent plasticity was demonstrated by their capacity to differentiate in adipocytic, chondrocytic, and osteocytic lineages. Furthermore, our results open possibilities to use cUCV cells in preclinical trials for many well-characterized canine model conditions homologs to human diseases.

Umbilical cord mesenchymal stromal cells (MSC) have been widely investigated for cell-based therapy studies as an alternative source to bone marrow transplantation. Umbilical cord tissue is a rich source of MSCs with potential to derivate at least muscle, cartilage, fat, and bone cells in vitro. The possibility to replace the defective muscle cells using cell therapy is a promising approach for the treatment of progressive muscular dystrophies (PMDs), independently of the specific gene mutation. Therefore, preclinical studies in different models of muscular dystrophies are of utmost importance. The main objective of the present study is to evaluate if umbilical cord MSCs have the potential to reach and differentiate into muscle cells in vivo in two animal models of PMDs. In order to address this question we injected (1) human umbilical cord tissue (hUCT) MSCs into the caudal vein of SJL mice; (2) hUCT and canine umbilical cord vein (cUCV) MSCs intra-arterially in GRMD dogs. Our results here reported support the safety of the procedure and indicate that the injected cells could engraft in the host muscle in both animal models but could not differentiate into muscle cells. These observations may provide important information aiming future therapy for muscular dystrophies.

Background: Extrahepatic portal vein thrombosis (EHPVT) is an important cause of portal hypertension in children. Rex shunt has been used successfully to treat these patients. Methods: We report our experience in 19 infants and children (5 months to 14 years) with HPVT eligible for a mesenteric-portal surgical shunt with left internal jugular vein autograft. Eight children had idiopathic EHPVT, nine had post-umbilical catheterization EHPVT, one had portal vein agenesis, and one had posttransplant EHPVT. Results: It was possible to perform the Rex shunt in all patients except for 8 of 9 cases in the post-umbilical catheterization EHPVT group. A Warren procedure was performed in 4 of those patients and a proximal splenorenal shunt in 1. Current follow-up ranges from 3 to 26 months. Shunt thrombosis occurred in one patient with portal vein agenesis and associated cardiac anomaly. Portal hypertension has significantly improved after surgery. None of our patients have experienced new bleeding episodes until now. Conclusions: The Rex shunt should be considered in the treatment of children with idiopathic EHPVT experiencing repeated gastrointestinal bleeding episodes refractory to endoscopic treatment. Nevertheless, the role of this operation for children with post-umbilical catheterization EHPVT is yet to be clearly evaluated. (C) 2011 Elsevier Inc. All rights reserved.

Adherent umbilical cord blood stromal cells (AUCBSCs) are multipotent cells with differentiation capacities. Therefore, these cells have been investigated for their potential in cell-based therapies. Quantum Dots (QDs) are an alternative to organic dyes and fluorescent proteins because of their long-term photostability. In this study we determined the effects of the cell passage on AUCBSCs morphology, phenotype, and differentiation potential. QDs labeled AUCBSCs in the fourth cell passage were differentiated in the three mesodermal lineages and were evaluated using cytochemical methods and transmission electron microscopy (TEM). Gene and protein expression of the AUCBSCs immunophenotypic markers were also evaluated in the labeled cells by real-time quantitative PCR and flow cytometry. In this study we were able to define the best cellular passage to work with AUCBSCs and we also demonstrated that the use of fluorescent QDs can be an efficient nano-biotechnological tool in differentiation studies because labeled cells do not have their characteristics compromised.

Objective To determine accuracy of first trimester detection of single umbilical artery (SUA). Methods The number of vessels in the umbilical cord was examined in a prospective cohort of 779 singleton, low-risk, unselected pregnancies, in the first (11-13 weeks) and second (17-24 weeks) trimesters, using both power and color Doppler and after delivery, by placental histopathologic exam. Concordance between first and second trimester findings to postnatal diagnoses was compared by calculating kappa coefficients. Results There was medium concordance between the findings in the first trimester and the postnatal diagnoses (kappa = 0.52) and high concordance (kappa = 0.89) for the second trimester scan. Sensitivity, specificity, positive and negative predictive values for the findings in the first trimester were 57.1, 98.9, 50.0 and 99.2% and for the second trimester were 86.6, 99.9, 92.9 and 99.7%. Conclusion Sensitivity and positive predictive value of first trimester scan to identify an isolated SUA in a prospective unselected population was poor. Diagnosis of isolated SUA as well as a definitive judgment about the presence of associated anomalies would still require a scan in the second trimester. Copyright (C) 2011 John Wiley & Sons, Ltd.
