The sternal end of the clavicle has been illustrated to be useful in aging young adults, however, no studies have investigated what age-related changes occur to the sternal end post epiphyseal fusion. In this study, three morphological features (i.e., surface topography, porosity, and osteophyte formation) were examined and scored using 564 clavicles of individuals of European ancestry (n = 318 males; n = 246 females), with known ages of 40+ years, from four documented skeletal collections: Hamann-Todd, Pretoria, St. Bride's, and Coimbra. An ordinal scoring method was developed for each of the three traits. Surface topography showed the strongest correlation with age, and composite scores (formed by summing the three separate trait scores) indicated progressive degeneration of the surface with increasing chronological age. Linear regression analyses were performed on the trait scores to produce pooled-sample age estimation equations. Blind tests of the composite score method and regression formulae on 56 individuals, aged 40+ years, from Christ Church Spitalfields, suggest accuracies of 96.4% for both methods. These preliminary results display the first evidence of the utility of the sternal end of the clavicle in aging older adult individuals. However, in the current format, these criteria should only be applied to individuals already identified as over 40 years in order to refine the age ranges used for advanced age. These findings do suggest the sternal end of the clavicle has potential to aid age estimates beyond the traditional "mature adult" age category (i.e., 46+ years), and provides several suggestions for future research.

Clavicle reconstruction is a rare operation. In most cases a mid-shaft defect of the clavicle is bridged by using different grafting techniques or musculo-osteous flaps. In some clinical situations where reconstruction is not a suitable option claviculectomy as a salvation procedure has proven to be an acceptable solution. In the paediatric population the challenge of both the cosmetic and the functional result attempting reconstruction of large bone defects is of higher demand. To our knowledge, this is the first case of a successful clavicle reconstruction with a sufficient follow-up using a free vascularised fibula graft in a child. This case provides a technique description, considerations in the paediatric population, an overview of other techniques used, and a long-term follow-up.

Rhesus macaques and stump-tailed macaques are sympatric in western Yunnan (China), coexisting or occupying habitats that show little difference. This paper tests hypotheses based on theoretical expectation from the differing biomechanical demands of terrestrial and arboreal quadrupedalism in stump-tailed macaques and rhesus macaques, respectively. Individuals of these two macaque taxa were markedly separated by the first two principal components and discriminant analyses based on 18 variables of the upper limb. The rhesus macaques appear to be more adapted for arboreal quadruped habits because of elongation of the clavicle and forearm, a larger humeral head and greater midshaft sagittal diameters of the radius and ulna.

L'épaule est un complexe articulaire formé par le thorax, la clavicule, la scapula et l'humérus. Alors que les orientation et position de ces derniers la rendent difficile à étudier, la compréhension approfondie de l'interrelation de ces segments demeure cliniquement importante. Ainsi, un nouveau modèle du membre supérieur est développé et présenté. La cinématique articulaire de 15 sujets sains est collectée et reconstruite à l'aide du modèle. Celle-ci s'avère être généralement moins variable et plus facilement interprétable que le modèle de référence. Parallèlement, l'utilisation de simplifications, issues de la 2D, sur le calcul d'amplitude de mouvement en 3D est critiquée. Cependant, des cas d'exception où ces simplifications s'appliquent sont dégagés et prouvés. Ainsi, ils sont une éventuelle avenue d'amélioration supplémentaire des modèles sans compromission de leur validé.

Introducción: el tratamiento usual para fracturas del tercio medio de la clavícula es el ortopédico, sin embargo recientemente se ha propuesto que el tratamiento quirúrgico podría ser una mejor opción para los pacientes jóvenes laboralmente activos afectados por este tipo de fractura. Métodos: este es un estudio retrospectivo analítico de corte transversal comprendido en un periodo de cuatro años (2006-2009) en el servicio de Ortopedia del Hospital Kennedy E.S.E. Los desenlaces que se evaluaron fueron la función del hombro, el reintegro laboral, lesión neurológica y alteración cosmética. Resultados: en total se evaluaron 204 pacientes, de los cuales solo 103 cumplieron con los criterios de inclusión (17 mujeres y 86 hombres), de los cuales 83 recibieron tratamiento ortopédico y 20 requirieron manejo quirúrgico con promedio de edades de 28.4 años manejados ortopédicamente y 30.4 años en tratamiento quirúrgico predominando el género masculino. Conclusión: para los pacientes con fractura de clavícula de tercio medio, sin importar el tipo de tratamiento al que fueron sometidos, el resultado desde el punto de vista funcional, lesión neurológica y reintegro laboral fueron similares con excepción de la alteración cosmética.

Introducción:. Para el tratamiento de pacientes con fractura del tercio medio de clavícula encontramos el ortopédico y quirúrgico, se desconoce a nivel local cuál alternativas ofrece un mejor desenlace. Materiales y Métodos: Estudio de cohorte retrospectiva, en pacientes con fractura del tercio medio de la clavícula comparando pacientes expuestos a tratamiento quirúrgico versus tratamiento ortopédico evaluando alteración estética, dolor, sesiones de fisioterapia, reintegro laboral y puntaje del DASH a los 3 meses de tratamiento. Resultados: 91 pacientes tratados ortopédicamente y 24 tratados quirúrgicamente. Mediana de edad 34.1 años para ambos tratamientos. El 20.2% de los pacientes sometidos al tratamiento quirúrgico presentaron dolor mientras que ningún paciente del grupo ortopédico presentó dolor después del tratamiento. El 79.1% de pacientes tratados ortopédicamente y el 20.9% tratados quirúrgicamente referían alteración estética. La medina de sesiones de fisioterapia realizadas para el tratamiento ortopédico fue 8.04 y 9.04 del quirúrgico. El promedio de reintegro laboral fue de 5 semanas para ambos tratamientos Discusión: Tanto el tratamiento ortopédico como el quirúrgico obtuvieron resultados muy similares para todas las variables medidas sin que se pueda concluir que uno sea mejor que el otro. Los pacientes con desplazamientos iniciales menores de 2 cm son candidatos para manejo ortopédico con buenos resultados con puntajes del DASH de 35 a los 3 meses tratamiento, los pacientes con desplazamientos iniciales mayores de 2 cm son los que con mayor frecuencia son sometidos a tratamientos quirúrgicos también encontrándose buenos resultados con puntajes del DASH de 38 a los 3 meses de tratamiento.

The forelimbs of higher vertebrates are composed of two portions: the appendicular region (stylopod, zeugopod and autopod) and the less prominent proximal girdle elements (scapula and clavicle) that brace the limb to the main trunk axis. We show that the formation of the muscles of the proximal limb occurs through two distinct mechanisms. The more superficial girdle muscles (pectoral and latissimus dorsi) develop by the “In–Out” mechanism whereby migration of myogenic cells from the somites into the limb bud is followed by their extension from the proximal limb bud out onto the thorax. In contrast, the deeper girdle muscles (e.g. rhomboideus profundus and serratus anterior) are induced by the forelimb field which promotes myotomal extension directly from the somites. Tbx5 inactivation demonstrated its requirement for the development of all forelimb elements which include the skeletal elements, proximal and distal muscles as well as the sternum in mammals and the cleithrum of fish. Intriguingly, the formation of the diaphragm musculature is also dependent on the Tbx5 programme. These observations challenge our classical views of the boundary between limb and trunk tissues. We suggest that significant structures located in the body should be considered as components of the forelimb.

The action potential level for shoulder muscles deltoid-anterior portion (DA) and pectoralis major-clavicular portion (PMC) determined by four different modalities of execution of rowing exercises, each one with two different grips, was recorded. These were compared with the action potential level determined for the same muscles by four different modalities of execution of the frontal-lateral cross, dumbbells exercises. Twenty-four male volunteers were examined using a 2 channel TECA TE4 electromyograph and Hewlett Packard surface electrodes. The statistic analysis showed significant (p<0,05) superiority for all the frontal-lateral cross, dumbbells exercises in comparison to all rowing exercises for the PMC, for the DA this generalized supremacy was not observed.

The muscles deltoid-anterior portion (DA) and pectoralis major-clavicular portion (PMC) were analysed to establish the muscular behavior and intensity patterns, as well to evaluate the connected participation of these muscles during supine and frontal elevation exercises. Twenty-four male volunteers were examined using a 2-channel TECA TE4 electromyograph and Hewlett Packard surface electrodes. Our results showed low levels of activity to PMC in frontal elevation exercises, whereas to DA the levels were very high. In the supine exercise, the action potential levels developed by the PMC were always lower than those presented by DA, however, with action simultaneity. Some suggestions to the use of the tested exercises are presented.

The electromyographic activity of the deltoideus (anterior portion) and pectoralis major (clavicular portion) muscles was analyzed in 24 male volunteers in two different modalities of pull-over exercises. The PMC activity varied from weak to moderate in both modalities, while the DA activity was moderate in the pull-over and strong in the pull-over with bent arms exercises.

Maxillomandibular reconstructions are traditionally performed by means of autogenous bone grafts collected from intraoral donor areas and extraoral donor areas such as clavicle, iliac bone, rib, and tibia. The calvarial bone has been studied as an alternative donor area, with a low incidence of complications and minimal postoperative morbidity. Complications such as dural lacerations associated with cerebrospinal fluid leakage and extradural and subdural bleeding were minimized due to the use of surgical trepan, allowing the diploic layer delimitation before the osteotomy, preserving the internal calvarial cortical. The purpose of this article is to suggest a new technique for the obtainment of calvarial bone grafts with surgical trepan.

Ewing sarcoma is a common primary bone malignancy occurring in childhood and adolescence. This case report describes a 4-year-old female patient who had Ewing sarcoma in the left clavicular region. The patient underwent total excision of the left clavicle and subsequently developed periodontitis and multiple carious lesions after chemotherapy. Caries risk and salivary flow rate tests were performed, followed by periodontal treatment, topical fluoride application, restoration of caries, and oral hygiene instruction. The care of this patient demonstrates that an interdisciplinary approach is essential to eliminate all foci of infection, minimize morbidity, and improve the patient's general health before, during, and after oncological treatment. © 2012 Special Care Dentistry Association and Wiley Periodicals, Inc.

Fractures of the growing bone require fixation techniques, which preclude any injury to the growth plate regions. This requirement is met by Elastic Stable Intramedullary Nails (ESIN) which are positioned between both metaphyseal regions. Pronounced malposition and/or shortening, open fractures and fractures with impending skin perforation are indications for clavicle nailing in adolescents. Retrograde nailing with two elastic nails, inserted from lateral, is the method of choice for stabilization of humerus fractures. In radial neck fractures with severe tilting of the radial head, a retrograde nail may reduce and fix the head. In Monteggia lesions, the ulna fracture is reduced and fixed with an antegrade nail. Forearm fractures with unacceptable axial deviation are reduced and fixed with one antegrade nail in the ulna and a retrograde nail in the radius. Ascending elastic nailing is done for femur shaft and proximal femur fractures. The medial and lateral entry sites are located above the distal physis. End caps are used to prevent shortening in spiral and multiple segment fractures. Fractures of the distal third of the femur are nailed in a descending technique. The entry sites of two nails are located on the lateral cortex below the greater trochanter. Combined tibia and fibula fractures, open fractures and unstable fracture types such as spiral and multifragmental tibia fractures are good indications for ESIN. Descending nailing is the method of choice. The nail entry points are medially and laterally distal to the apophysis of the proximal tibia. Thorough knowledge of each fracture type, fracture location and age specific healing pattern is necessary for safe and effective treatment of pediatric fractures

Background and Study Aim: Understanding injury incidence rates will be a great help with regards to preventing potential future damages. It is for this reason that this study suggests studying a large number of variables. The purpose of research is the relationship of events (empirical variables) that are usually taken into account in developing injury prevention programs during the battles and training in judo tournament. Material and methods: In this research project, 57 male judokas taking part in the Spanish National University Championship in 2009 were asked to complete a retrospective questionnaire. We analysed the following events: the most commonly injured body regions, the medical diagnosis, how and when the injury happened, the type of injury, the side of the body and the type of medical attention received. For the statistical analysis, we used the SPSS statistics programme to apply the Chi-square test in order to determine the significance levels for non-parametric tests from p<.05. Results: Significant differences were found in the most commonly injured body region, the shoulder/clavicle (p<.05), and in the most common diagnosis, the sprain (p<.05). Impact injuries (p<.05) are the most common and training (p<.05) is the most dangerous time. About the type of injury, 78.38% are new injuries (p<.05) and 69.05% affect the right hand side of the body (p<.05). Doctors are the most consulted specialists, but the physiotherapists obtained the best marks. Have been out due to injury for over 21 days 36.36% of the participants, but not for the entire season. Conclusions: The most common diagnosis in university student judokas coincides with those of elite judokas, with the sprain being the most common. University student judokas have a higher rate of shoulder/clavicle injuries, while professional judokas are prone to a higher rate of knee injuries. Training is the most common moment in which injuries occur, both in university student judokas and professional judokas. New injuries are the most common types of injuries in university student judokas and, while doctors are the most consulted specialists, the physiotherapists obtained the best marks.
