Clinical and demographic presurgical variables may be associated with unfavorable postsurgical neurological outcome in patients with mesial temporal lobe epilepsy with hippocampal sclerosis (MTLE-HS). However, few reports include preoperative psychiatric disorders as a factor predictive of long-term post-surgical MTLE-HS neurological Outcome. We used Engel`s criteria to follow 186 postsurgical patients with MTLE-HS for an average of 6 years. DSM-IV criteria and psychiatric comorbidity criteria specific to epilepsy (interictal dysphoric disorder, postictal and interictal psychosis) were used to assess presurgical psychiatric disorders. Kaplan-Meier event-free Survival and adjusted hazard ratios were estimated with unconditional logistic regression. Seventy-seven (41.4%) patients had a preoperative Axis I psychiatric diagnosis. Thirty-six patients had depression, I I interictal dysphoric disorder, 14 interictal psychosis, 6 postictal psychosis, and 10 anxiety disorders. Twenty-three (12.4%) patients had Axis 11 personality disorders. Regarding seizure outcome, preoperative anxiety disorders (P = 0.009) and personality disorders (P = 0.003) were positively Correlated with Engel class I B (remaining auras) or higher. These findings emphasize the importance of presurgical psychiatric evaluation, counseling, and Postsurgical follow-up of patients with epilepsy and psychiatric disorders. (C) 2009 Elsevier Inc. La migraine de l'enfant est paradoxale à plus d'un titre : lors des crises, la douleur est sévère, les enfants sont souvent livides, « cadavériques », incapables de bouger, ils sont obligés de s'allonger dans la pénombre, vomissent parfois massivement, voient, sentent, entendent des « choses bizarres » - correspondant aux auras ; mais ils se réveillent en pleine forme après avoir dormi quelques heures... et tous les examens sont normaux. Alors que 5 à 10 % des enfants sont migraineux, le diagnostic n'est posé que pour une petite partie d'entre eux. La migraine est pourtant la première cause de céphalées intenses récurrentes chez l'enfant, la plupart des professionnels cherchant vainement une cause, et évoquant « les yeux, une gastro, les sinus ou bien une origine psy... ». Cet ouvrage a pour double objectif de présenter l'état de la science (épidémiologie, physiopathologie, génétique...) qui s'est largement enrichi ces dix dernières années et d'apporter aux professionnels des outils concrets pour identifier les céphalées et améliorer la prise en charge des enfants et des adolescents migraineux. Les céphalées de tension, les céphalées chroniques, les syndromes épisodiques associés à la migraine sont aussi détaillés. Illustré de nombreuses vignettes cliniques, cet ouvrage précise les critères diagnostiques, les pièges et les idées fausses ; il décrit comment rechercher les facteurs déclenchants et particulièrement ceux d'origine psychosociale, comment utiliser et combiner les traitements médicamenteux et non médicamenteux (notamment les approches psychothérapeutiques et psychocorporelles). Telle est l'ambition de ce livre, destiné à tous les professionnels confrontés à des enfants et des adolescents migraineux, mais également accessible en grande partie aux familles.

We report on two patients with recurrent syncope secondary to ictal bradyarrhythmias, triggered by partial epileptic seizures with atypical, stereotyped auras. Ictal bradyarrhythmias are potentially lethal, and likely originate from the involvement of limbic autonomic regions. The appropriate treatment is double-headed, including an antiepileptic drug and the implantation of a pacemaker.

Posterior epilepsies are relatively rare, mainly suspected clinically by the presence of visual auras. Functional near-infrared spectroscopy (fNIRS) is an emerging non-invasive imaging technique that has the potential to monitor hemodynamic changes during epileptic activity. Combined with electroencephalography (EEG), 9 patients with posterior epilepsies were recorded using EEG-fNIRS with large sampling (19 EEG electrodes and over 100 fNIRS channels). Spikes and seizures were carefully marked on EEG traces, and convolved with a standard hemodynamic response function for general linear model (GLM) analysis. GLM results for seizures (in 3 patients) and spikes (7 patients) were broadly sensitive to the epileptic focus in 7/9 patients, and specific in 5/9 patients with fNIRS deoxyhemoglobin responses lateralized to the correct lobe, and to plausible locations within the occipital or parietal lobes. This work provides evidence that EEG-fNIRS is a sensitive technique for monitoring posterior epileptic activity.

The calcite compensation depth (CCD) fluctuates as a result of changes in the water-mass system, thereby producing a distinct dissolution pattern. Differential dissolution changes the composition of the foraminiferal assemblages, reflecting the depositional environment in respect to the fluctuating CCD. The dissolution pattern for the comparatively shallow Site 541 on the Barbados Ridge indicates a depositional environment mostly above the CCD, but below the foraminiferal lysocline during the late Miocene to early Pleistocene. In contrast, sediments of the deeper-water Site 543 indicate a depositional environment above the CCD during the late Pliocene to early Pleistocene only. Furthermore, similarities in the dissolution pattern of corresponding time intervals of Site 541 (represented by superimposed faulted intervals termed Tectonic Units A and B) are recognizable. Sediments deposited clearly above the foraminiferal lysocline are rare
