Este trabajo tiene como meta el desarrollo de un sistema de neurorrehabilitación mediante realidad virtual para pacientes con problemas motores en las extremidades superiores provocados por un daño cerebral. Para entender y enfrentarse de manera correcta al problema, se han revisado todos los aspectos concernientes al daño cerebral y déficits motores derivados así como las terapias empleadas para su rehabilitación, como las que utilizan tecnologías asistidas como la realidad virtual. Se han expuesto las ventajas que han llevado a utilizar esta terapia en el proyecto y se han mencionado algunos estudios de investigación previos y con resultados favorables. Por último, se ha hecho hincapié en las novedades (embodiment y neuronas espejo) que aporta este proyecto con las que se podría acelerar la rehabilitación de los pacientes incidiendo en la plasticidad cerebral. En lo que se refiere al desarrollo del entorno, se ha utilizado el motor de videojuegos Unity 3D, con el que se ha integrado el dispositivo Myo Armband, para capturar el movimiento, y las gafas de realidad virtual Oculus Rift DK2. Durante la terapia, el paciente tiene que superar una serie de niveles definidos por el terapeuta y, para ello, debe realizar un alcance y tocar un botón un número de veces por cada nivel. En cada nivel, el esfuerzo que tiene que realizar el paciente es distinto, dependiendo de la distancia a la que se encuentre el botón (relativa al calibrado previo del paciente) e igualmente, está definido por el terapeuta. También se lleva a cabo un registro de las aceleraciones y del porcentaje de acierto del paciente en un fichero, para llevar un control de su progreso. Como conclusión, cabe mencionar que el entorno desarrollado es adaptable a las posibilidades de cada paciente, permitiendo una recuperación óptima. Se trata de un sistema de bajo coste, intuitivo y que lleva a cabo un registro de datos de la actividad del paciente, lo que posibilita, como trabajo futuro, el control de forma remota por parte del terapeuta. Todos estos aspectos dejan abierta la posibilidad de que el paciente lo utilice a domicilio, con las ventajas que conlleva. Del mismo modo, el sistema se podría adaptar fácilmente para la rehabilitación de las extremidades inferiores. Tras su análisis, los investigadores expertos en la materia del Consejo Superior de Investigaciones Científicas afirman que el sistema satisface con creces su propósito. Con todo esto, se concluye que este entorno tiene un excelente presente y un futuro muy prometedor.

Stroke is a prevalent disorder with immense socioeconomic impact. A variety of chronic neurological deficits result from stroke. In particular, sensorimotor deficits are a significant barrier to achieving post-stroke independence. Unfortunately, the majority of pre-clinical studies that show improved outcomes in animal stroke models have failed in clinical trials. Pre-clinical studies using non-human primate (NHP) stroke models prior to initiating human trials are a potential step to improving translation from animal studies to clinical trials. Robotic assessment tools represent a quantitative, reliable, and reproducible means to assess reaching behaviour following stroke in both humans and NHPs. We investigated the use of robotic technology to assess sensorimotor impairments in NHPs following middle cerebral artery occlusion (MCAO). Two cynomolgus macaques underwent transient MCAO for 90 minutes. Approximately 1.5 years following the procedure these NHPs and two non-stroke control monkeys were trained in a reaching task with both arms in the KINARM exoskeleton. This robot permits elbow and shoulder movements in the horizontal plane. The task required NHPs to make reaching movements from a centrally positioned start target to 1 of 8 peripheral targets uniformly distributed around the first target. We analyzed four movement parameters: reaction time, movement time (MT), initial direction error (IDE), and number of speed maxima to characterize sensorimotor deficiencies. We hypothesized reduced performance in these attributes during a neurobehavioural task with the paretic limb of NHPs following MCAO compared to controls. Reaching movements in the non-affected limbs of control and experimental NHPs showed bell-shaped velocity profiles. In contrast, the reaching movements with the affected limbs were highly variable. We found distinctive patterns in MT, IDE, and number of speed peaks between control and experimental monkeys and between limbs of NHPs with MCAO. NHPs with MCAO demonstrated more speed peaks, longer MTs, and greater IDE in their paretic limb compared to controls. These initial results qualitatively match human stroke subjects’ performance, suggesting that robotic neurobehavioural assessment in NHPs with stroke is feasible and could have translational relevance in subsequent human studies. Further studies will be necessary to replicate and expand on these preliminary findings.

RESUMO Objetivos: O Acidente Vascular Cerebral (AVC) potencia o desenvolvimento de disfunção executiva, conduzindo a défice no desempenho das tarefas do quotidiano. A avaliação neuropsicológica das funções executivas é importante para desenvolver estratégias de reabilitação adequadas. Assim, são objetivos descrever os dados normativos, precisão de diagnóstico, propriedades psicométricas e análise fatorial da Bateria de Avaliação Frontal (FAB), instrumento breve e de rápida administração, numa amostra de idosos com AVC. Métodos: Inserida no projeto Trajetórias do Envelhecimento de Idosos em Resposta Social, esta investigação conta com uma amostra de 112 pessoas idosas com diagnóstico médico de AVC e 157 pessoas idosas de um subgrupo de controlo sem AVC. Os sujeitos apresentam idades compreendidas entre os 60 e os 100 anos (M = 78,20; DP = 7,57) sendo maioritariamente do sexo feminino (n = 194). A avaliação inclui entrevistas e testes neuropsicológicos agrupados em medidas de funcionamento executivo, medidas cognitivas de referência e medidas clínicas de controlo. Resultados: As variáveis idade e escolaridade interferiram nas pontuações obtidas na amostra clínica, não sendo verificado impacto da variável sexo. Para um ponto de corte de 7, a FAB teve uma sensibilidade de 83,4% e especificidade de 66,1 % (AUC = 0,64); revelou um alfa de Cronbach de 0,79 e correlações fortes com os testes executivos (teste de Stroop, Figura Complexa de Rey, fator Atencional-Executivo do Montreal Cognitive Assessment e Alternância nos testes de Fluência verbal). A análise fatorial confirmatória apontou uma estrutura com um fator. Conclusões: A FAB apresenta boa consistência interna, validade convergente e validade de constructo, aparentando ser uma escala útil para avaliar o défice executivo em pessoas idosas com AVC. Dadas algumas limitações do estudo, que poderão explicar a fraca precisão diagnóstica da FAB, são incentivadas investigações futuras pois a FAB revelou-se um instrumento com propriedades psicométricas promissoras. ABSTRACT Goals: Stroke potentiates the development of executive dysfunction, leading to impairment in performance of daily activities. The neuropsychological assessment of executive functions is important to develop adequate rehabilitation strategies. Thus, describing the normative data, diagnostic accuracy, psychometric properties and factor analysis of the Frontal Assessment Battery (FAB), a brief and easy to administer instrument, in a clinical sample with stroke are objectives of this study. Methods: Being part of the Aging Trajectories of Institutionalized Elderly, this research has a sample 112 elderly people with a medical diagnosis of stroke and a control subgroup of 157 elderly people. The subjects have ages between 60 and 100 years old (M = 78.20, SD = 7.57), mostly females (n = 194). The measurements used include interviews and neuropsychological tests grouped in executive functioning measures, cognitive measures of reference and clinical measures of control. Results: The variables age and education affect the scores obtained in the clinical subgroup, having the variable gender no impact on these. Using a cutoff score of 7, the FAB had a sensitivity of 83.4% and a specificity of 66.1% for screening stroke (AUC = 0.64); showed a Cronbach's α of 0.79, and strong correlations with executive tests (Stroop test, Rey Complex Figure, Attentional-Executive factor of Montreal Cognitive Assessment and Switching in the verbal fluency tests). The confirmatory factor analysis supported a one-factor structure. Conclusions: The FAB presents good internal consistency, convergent, and construct when used for elderly with stroke. Due to some limitations of the study, which may explain the weak discriminant validity, further investigations are encouraged because FAB has showed promising psychometric properties.

Obesity is a major challenge to human health worldwide. Little is known about the brain mechanisms that are associated with overeating and obesity in humans. In this project, multimodal neuroimaging techniques were utilized to study brain neurotransmission and anatomy in obesity. Bariatric surgery was used as an experimental method for assessing whether the possible differences between obese and non-obese individuals change following the weight loss. This could indicate whether obesity-related altered neurotransmission and cerebral atrophy are recoverable or whether they represent stable individual characteristics. Morbidly obese subjects (BMI ≥ 35 kg/m2) and non-obese control subjects (mean BMI 23 kg/m2) were studied with positron emission tomography (PET) and magnetic resonance imaging (MRI). In the PET studies, focus was put on dopaminergic and opioidergic systems, both of which are crucial in the reward processing. Brain dopamine D2 receptor (D2R) availability was measured using [11C]raclopride and µ-opioid receptor (MOR) availability using [11C]carfentanil. In the MRI studies, voxel-based morphometry (VBM) of T1-weighted MRI images was used, coupled with diffusion tensor imaging (DTI). Obese subjects underwent bariatric surgery as their standard clinical treatment during the study. Preoperatively, morbidly obese subjects had significantly lower MOR availability but unaltered D2R availability in several brain regions involved in reward processing, including striatum, insula, and thalamus. Moreover, obesity disrupted the interaction between the MOR and D2R systems in ventral striatum. Bariatric surgery and concomitant weight loss normalized MOR availability in the obese, but did not influence D2R availability in any brain region. Morbidly obese subjects had also significantly lower grey and white matter densities globally in the brain, but more focal changes were located in the areas associated with inhibitory control, reward processing, and appetite. DTI revealed also signs of axonal damage in the obese in corticospinal tracts and occipito-frontal fascicles. Surgery-induced weight loss resulted in global recovery of white matter density as well as more focal recovery of grey matter density among obese subjects. Altogether these results show that the endogenous opioid system is fundamentally linked to obesity. Lowered MOR availability is likely a consequence of obesity and may mediate maintenance of excessive energy uptake. In addition, obesity has adverse effects on brain structure. Bariatric surgery however reverses MOR dysfunction and recovers cerebral atrophy. Understanding the opioidergic contribution to overeating and obesity is critical for developing new psychological or pharmacological treatments for obesity. The actual molecular mechanisms behind the positive change in structure and neurotransmitter function still remain unclear and should be addressed in the future research.

Amphetamine enhances recovery after experimental ischaemia and has shown promise in small clinical trials when combined with motor or sensory stimulation. Amphetamine, a sympathomimetic, might have haemodynamic effects in stroke patients, although limited data have been published. Subjects were recruited 3-30 days post ischaemic stroke into a phase II randomised (1:1), double blind, placebo-controlled trial. Subjects received dexamphetamine (5mg initially, then 10mg for 10 subsequent doses with 3 or 4 day separations) or placebo in addition to inpatient physiotherapy. Recovery was assessed by motor scales (Fugl-Meyer, FM), and functional scales (Barthel index, BI and modified Rankin score, mRS). Peripheral blood pressure (BP), central haemodynamics and middle cerebral artery blood flow velocity were assessed before, and 90 minutes after, the first 2 doses. 33 subjects were recruited, age 33-88 (mean 71) years, males 52%, 4-30 (median 15) days post stroke to inclusion. 16 patients were randomised to placebo and 17 amphetamine. Amphetamine did not improve motor function at 90 days; mean (standard deviation) FM 37.6 (27.6) vs. control 35.2 (27.8) (p=0.81). Functional outcome (BI, mRS) did not differ between treatment groups. Peripheral and central systolic BP, and heart rate, were 11.2 mmHg (p=0.03), 9.5 mmHg (p=0.04) and 7 beats/minute (p=0.02) higher respectively with amphetamine, compared with control. A non-significant reduction in myocardial perfusion (Buckberg Index) was seen with amphetamine. Other cardiac and cerebral haemodynamics were unaffected. Amphetamine did not improve motor impairment or function after ischaemic stroke but did significantly increase BP and heart rate without altering cerebral haemodynamics.

El trastorno orgánico de la personalidad supone una alteración significativa de las formas habituales del comportamiento premórbido, afectando particularmente a la expresión de las emociones, necesidades e impulsos. Se presenta el caso de una mujer de 49 años de edad que, tras padecer un infarto cerebral, presentó una transformación de su personalidad y de sus valores previos. Anteriormente era una persona con múltiples miedos, cuidadosa, metódica en su vida diaria. Tras el infarto comenzó a presentar conductas de provocación y una mayor desinhibición e impulsividad, menor tolerancia a la frustración, así como mínima capacidad de planificación y resolución de problemas. Además, fue incapaz de conservar ningún trabajo y perdió las relaciones sociales. Se realizaron estudios de imagen (tomografía computarizada (TC) y resonancia magnética (RM)), electroencefalograma (EEG) y una evaluación neuropsicológica. Los resultados obtenidos son compatibles con un trastorno orgánico de la personalidad subtipo desinhibido, caracterizado clínicamente por la tendencia a la labilidad emocional, la desinhibición conductual y la falta de control de impulsos. El trastorno orgánico de la personalidad es una afectación crónica e irreversible que está influyendo en la capacidad de autogobierno de la paciente, y por ello deben valorarse los aspectos psiquiátrico-forenses pertinentes.

La parálisis cerebral (PC) se puede presentar como consecuencia de una hipoxia perinatal severa, la cual deja una daño cerebral permanente en 50% de los recién nacidos prematuros que sobreviven este evento. La terapia celular es una opción terapéutica potencial para la PC, la cual se cree que funciona a través diversos mecanismos, los cuales incluyen la inmunomodulación a través de citocinas (C) y de la secreción de factores de crecimiento.Se incluyeron 18 pacientes con PC con el fin de evaluar la seguridad de la aplicación intratecal (IT) e intravenosa (IV) de células nucleadas totales (CNT) derivadas de médula ósea (MO) autóloga después de la estimulación con factor estimulador de colonias de granulocitos (FEC-G). Se evaluó clínicamente las áreas motora gruesa y fina, cognitiva, de comunicación, personal-social y adaptativa por medio de una prueba validada llamada Inventario del Desarrollo de Battelle (IDB). Se realizaron tres evaluaciones en cada paciente, la primera previa al procedimiento, y después a los 30 y a los 180 días. También se llevaron a cabo estudios de resonancia magnética nuclear (RMN) antes del procedimiento y uno de control 6 meses después del tratamiento.

Se entendemos o Acidente vascular cerebral como um acontecimento incapacitante da vida dos pacientes, poderemos compreender as alterações comportamentais e emocionas que este provoca. No entanto, poucos estudos têm sido realizados acerca desta temática, tendo assim este estudo como objetivo verificar se existe uma associação clara entre a ansiedade e um determinado tipo de AVC. Aplicou-se a escala de Montereal, o State-Trait Anxiety lnventory e o Inventario Neuropsiquiátrico numa amostra constituída por pacientes que tenham sofrido AVC no período de três meses a um ano, e num grupo controlo. Os resultados mostram que não existem diferenças significativas no que respeita à ansiedade nos pacientes com AVC anterior. Conclui-se que não existe um aumento de ansiedade, mas que os défices visuo-espaciais e atencionais são mais significativos no AVC anterior. / ABSTRACT: lf we recognize Stroke as an event that can incapacitate the patients' life, we can comprehend the behaviour and emotional changes that this provokes. One of the psychiatric symptoms usually associated to Stroke is anxiety. However, few studies have been made concerning this matter, being the purpose of this study to verify if a clear association exists between anxiety and a certain type of Stroke. The Montereal Cognitiva Assessment, State-Trait Anxiety lnventory, and Neuropsychiatric lnventory were applied in a sample consisting of patients that have suffered Stroke in the period of three months to one year, and in a control group. The results show no significant differences in what regards to anxiety in patients with previous Stroke. We conclude that there is no increase in anxiety after stroke, but cognitive deficits in visual-spatial and attentional are most significant in subjects with previous stroke.

Purpose: To evaluate the clinical efficacy and safety of edaravone in the treatment of acute cerebral haemorrhage (ACH). Methods: This study recruited 120 patients who developed ACH. The patients were divided into control and treatment groups with 60 patients per group. The control group underwent conventional treatment and the treatment group also received intravenous edaravone. The volumes of cerebral edema and cerebral hematoma, high-sensitivity C-reactive protein (hs-CRP) and interleukin-6 (IL-6) levels, and Chinese Stroke Scale (CSS) score before and after treatment were compared between the two groups. Results: The respective cerebral edema volumes of the control and treatment groups decreased from 20.99 ± 12.09 and 21.80 ± 12.01 mL on day 0 to 11.23 ± 6.34 and 12.11 ± 5.98 mL at day 7 and 4.69 ± 4.03 and 4.64 ± 3.9 mL on day 14 (P < 0.05). The respective cerebral hematoma volumes of the control and treatment groups decreased from 18.98 ± 12.04 and 18.97 ± 12.07 mL on day 0 to 12.34 ± 6.57 and 11.89 ± 4.01 mL at day 7 and 9.49 ± 3.95 and 9.52 ± 3.96 mL on day 14. Compared with pretreatment, hs-CRP and IL-6 levels and CSS score of the two groups decreased significantly following treatment (p < 0.05); the differences in the cerebral edema and hematoma volumes of the two groups on days 7 and 14 were not significant (p > 0.05). The hs-CRP and IL-6 levels and CSS scores of the treatment group decreased appreciably (p < 0.05), while the incidence of adverse reactions in the treatment and control groups was 16.67 and 13.33 %, respectively, but the difference was not significant (p > 0.05). Conclusion: Edaravone shows remarkable clinical efficacy and safety with no obvious adverse reactions in the treatment of ACH. Therefore, its use is recommended.

An unstructured mesh �nite volume discretisation method for simulating di�usion in anisotropic media in two-dimensional space is discussed. This technique is considered as an extension of the fully implicit hybrid control-volume �nite-element method and it retains the local continuity of the ux at the control volume faces. A least squares function recon- struction technique together with a new ux decomposition strategy is used to obtain an accurate ux approximation at the control volume face, ensuring that the overall accuracy of the spatial discretisation maintains second order. This paper highlights that the new technique coincides with the traditional shape function technique when the correction term is neglected and that it signi�cantly increases the accuracy of the previous linear scheme on coarse meshes when applied to media that exhibit very strong to extreme anisotropy ratios. It is concluded that the method can be used on both regular and irregular meshes, and appears independent of the mesh quality.
