São conhecidos alguns ensaios acerca da administração de contrastes orais alternativos, de origem natural, nos estudos de Colangiopancreatografia por Ressonância Magnética (CPRM). Através da administração dum contraste oral negativo (chá preto), o objetivo deste estudo foi a otimização do protocolo de aquisição da CPRM com vista à melhor evidência das estruturas anatómicas em estudo, relativamente à supressão das imagens indesejadas de fundo causadas pelos fluídos do sistema gastrointestinal. Pretendeu-se também minimizar os artefactos causados pelos movimentos respiratórios ocorridos durante a aquisição das imagens.

FUNDAMENTO: Penicilina G benzatina a cada 3 semanas é o protocolo padrão para a profilaxia secundária para febre reumática recorrente. OBJETIVO: Avaliar o efeito da penicilina G benzatina em Streptococcus sanguinis e Streptococcus oralis em pacientes com doença valvular cardíaca, devido à febre reumática com recebimento de profilaxia secundária. MÉTODOS: Estreptococos orais foram avaliados antes (momento basal) e após 7 dias (7º dia) iniciando-se com penicilina G benzatina em 100 pacientes que receberam profilaxia secundária da febre reumática. Amostras de saliva foram avaliadas para verificar a contagem de colônias e presença de S. sanguinis e S. oralis. Amostras de saliva estimulada pela mastigação foram serialmente diluídas e semeadas em placas sobre agar-sangue de ovelhas seletivo e não seletivo a 5% contendo penicilina G. A identificação da espécie foi realizada com testes bioquímicos convencionais. Concentrações inibitórias mínimas foram determinadas com o Etest. RESULTADOS: Não foram encontradas diferenças estatísticas da presença de S. sanguinis comparando-se o momento basal e o 7º dia (p = 0,62). No entanto, o número existente de culturas positivas de S. oralis no 7º dia após a Penicilina G benzatina apresentou um aumento significativo em relação ao valor basal (p = 0,04). Não houve diferença estatística existente entre o momento basal e o 7º dia sobre o número de S. sanguinis ou S. oralis UFC/mL e concentrações inibitórias medianas. CONCLUSÃO: O presente estudo mostrou que a Penicilina G benzatina a cada 3 semanas não alterou a colonização por S. sanguinis, mas aumentou a colonização de S. oralis no 7º dia de administração. Portanto, a susceptibilidade do Streptococcus sanguinis e Streptococcus oralis à penicilina G não foi modificada durante a rotina de profilaxia secundária da febre reumática utilizando a penicilina G. We present a double-blind study in two groups afflicted with oral lichen planus erythematous of long evolution and resistant to other treatments. We tested on it a treatment with Cyclosporine A (CyA) which had been successfully used before by many dermatologists. In the group A we used mouthwashes with a 5 ml Cyclosporine A solution to a 10% in olive oil of 0.4 degrees of acidity for five minutes, three times a day for eight weeks. In the control group we used acetonide of triamcinolone 01% in aqueous solution. Patients in group A improved considerably in their symptomatology in a 90% against a 60% in group B. In group A we could appreciate a disappearance of the symptomatology after two weeks of treatment in 60% of patients against 30% in group B. CyA can be an alternative to the conventional treatments in the acute period of lichen planus although it can not be considered as a first option drug because of the high cost of the treatment. For long term, results are not so good and we consider that extensive studies are necessary

Aunque el manejo nutricional de los pacientes con Pancreatitis Aguda Severa ha sido bien establecido por la evidencia disponible, el inicio de la vía oral en Pancreatitis Leve no ha sido igualmente estudiado. El objetivo de este estudio es evaluar el efecto del inicio temprano de la nutrición por vía oral en estos pacientes. Métodos: Realizamos un descriptivo serie de comparación de casos en los cuales comparamos la evolución y resultados del manejo de los pacientes con pancreatitis aguda antes y después de iniciar un protocolo de inicio temprano de nutrición por vía oral. Resultados: La serie incluyó 13 pacientes manejados con restitución convencional y 9 pacientes a quienes se restituyó tempranamente la nutrición oral. No existieron diferencias estadísticamente significativas en cuanto las características clínicas, etiológicas y demográficas entre los grupos. No se presentaron casos de progresión a enfermedad severa en ninguno de los grupos. La aparición de síntomas gastrointestinales como vomito y dolor después del inicio de la dieta fue similar en los dos grupos. Conclusiones: La restitución de la nutrición por vía oral basada criterios preestablecidos (la intensidad del dolor menor de 4 y un periodo mayor a 6 horas desde el último episodio emético) es segura y disminuye el periodo de ayuno inicial mas no el de estancia hospitalaria.

El propósito de este estudio fue analizar los resultados de una intervención primaria en salud oral de una población infantil entre edades de 7 a 12 años y establecer el comportamiento presentado en la población estudiada con respecto al índice internacional de salud oral COE Y COP, determinando aumento o disminución luego de la intervención. Para ello se utilizó un instrumento de recolección de datos, posterior a la observación de los registros clínicos, logrando conocer las características de las variables planteadas en el estudio en relación al Índice de Salud Oral COE y COP – D al inicio, final y comparativo de la intervención; número de eventos requeridos para la intervención, adherencia y cambios en patrón de morbilidad luego de la intervención. De acuerdo con los resultados se encontró que el comportamiento presentado en la población estudiada con respecto al índice de salud oral COE Y COP muestra una disminución que luego del análisis de resultados concluye con el cumplimiento de la meta internacional establecida por la OMS, estableciendo como objetivo para América Latina un COP-D de 1 a 2.9 para el año 2015; lo cual para esta investigación demuestra efectividad de la intervención al obtener un índice COP-D de 2,67.

La salud bucodental de los niños y niñas con autismo es, en muchos casos, deficiente. La asociación CEPRI, en colaboración con la Facultad de Odontología de la Universidad Complutense, elabora este proyecto con el objetivo de crear un programa de atención bucodental a niños autistas o con trastornos relacionados, y proporcionarles los apoyos necesarios para que tengan una adecuada salud que redunde en una mayor calida de vida. Este programa consta de varias fases: proporcionar a los niños el conocimiento directo de la intervención dental; prepararlos para que acepten un reconocimiento oral; ofrecer a los profesionales de la odontología la oportunidad de conocer niños autistas y de cómo ayudarles; y dar a las familias información que mejore la salud oral de sus hijos. En anexos se incluyen cuestionarios sobre atención y cuidado oral en autismo y fichas de observación y registro en la situación dental..

Base Teórica. Embora vários relatos de casos tenham sugerido uma associação entre a ingesta de vitamina K e a instabilidade da anticoagulação oral, o impacto clínico de diferentes quantidades de vitamina K dietética na anticoagulação oral crônica nunca foi prospectivamente estabelecido. Métodos. Características clínicas e estimativas semi-quantitativas sobre a ingesta de vitamina K foram avaliadas, prospectivamente, em 230 visitas ambulatoriais de pacientes anticoagulados em um hospital público universitário (protocolo observacional). Treze pacientes ambulatoriais anticoagulados cronicamente e estáveis foram arrolados em um ensaio clínico randomizado cruzado de intervenção dietética de 4 dias com aumento e diminuição de 5 vezes da sua ingesta habitual de vitamina K (protocolo experimental randomizado). Resultados. Protocolo observacional: Na análise univariada, identificamos uma associação estatisticamente significativa, progressiva e inversa entre um escore global de ingesta de vitamina K e diferentes níveis de anticoagulação. Na análise multivariada, a ingesta de vitamina K dietética foi independentemente associada com níveis subterapêuticos e anticoagulação excessiva (ambos valores de p =0,04), após ajuste para escolaridade, renda, uso de novos fármacos, aderência à anticoagulação oral e intercorrências clínicas. Protocolo randomizado: Após a dieta restrita em vitamina K, observamos um aumento significativo nos valores do INR (de 2,6 ± 0,5 para 3,3 ± 1,0, p=0,03; do início para o dia 7) enquanto, após a dieta enriquecida, identificamos uma diminuição significativa e mais precoce no INR (de 3,2 ± 0,9 para 2,8 ± 0,7, p=0,005; do início para o dia 4). O efeito da ingesta de vitamina K sobre a variação do INR foi particularmente influenciada pela ingesta média habitual da vitamina. Conclusões. Nossos dados prospectivos reforçam o conceito de que a interação vitamina K e fármacos cumarínicos é de fato real e clinicamente relevante, devendo ser reconhecida como um fator importante e independente que interfere com a estabilidade da anticoagulação oral crônica.

The therapeutic possibilities for chronic renal failure closely are related to the biological and social condition, and in this perspective the renal transplant is considered the best option, for providing quality of life better. This study it objectified to apprehend, by means of the rescue of the verbal history of life of the kidney transplant patients, the experiences lived since the diagnosis of the renal to the current with the current therapeutic modality. One is about a study of qualitative boarding, exploratory and descriptive, having the verbal history of life as a technique and method. The colony was formed with the ten first kidney transplant patients of the Rio Grande of the North, taken care of in the clinic of the Nephrology in the University Hospital Onofre Lopes-UFRN, located in the city of Christmas-RN. The net was composed for collaborators of both the sex, in the age band between 21 and 56 years of age, submitted it more than has one year to the renal transplant and that, in some cases, together of its familiar ones, they had voluntarily accepted to participate of the study. The first collaborator interviewed for this was excluded not to desire to participate the study more than. After approval for the Committee of Ethics in Search of the UFRN, we carry through the collection of data, by means of a halfstructuralized interview, recorded individually, in environment chosen for the collaborators. We carry through the transcription of the interviews and later we return to the interviewed ones so that the same ones made the conference, what it made possible in them to carry through the transcriation, after consecutive readings. We analyze the stories by means of the analysis of content of Bardin. Guiding the analysis of the stories of the collaborators, we find three axles thematic: Impact in the social relations, Impact in the social condition and Behavior front the illness and treatment. We conclude in this study that the loss of the renal function reed-echo drastically in the life of the collaborators, but that the acceptance of the pathological condition emerged, mainly for the support of the family and in the belief in God. The renal transplant was seen as the chance for a new life, however, so that they reach an improvement in the quality of life, other questions must be optimized, as the guarantee of constitutional laws, the rescue of the citizenship and offer of bigger support of the professionals of the health, family and society for the confrontation of the problems generated for the chronic renal illness and its treatments

Family Health Program (FHP) presents itself as a restructured model on basic cares besides having prevention practices and promotion in health as essential parts. According to that, the purpose of this study is to perceive which prevention practices in oral health are accomplished by the dentists from the FHP of Natal-RN as well as the way they have been developed, the knowledge basis to execute the procedure and the use of evaluation instruments by the dentists and by the Oral Health Co-Ordinator. A list of dentists participating on the FHP of Natal in March, 2006 (n= 91) was obtained so it was possible to formulate a structured interview. After excluding those with less than six months attending to the program, and considering the loss, eighty dentists got to be interviewed. It had questions about individual and group preventive procedures, sources of basement to develop the activities and to verify the impact and evaluation instruments. Besides that, an interview was made with the Oral Health Co-Ordinator about the evaluating process and the existence of a protocol as well as about document analysis in the Ambulatory Informations System Unique Health System (AIS-UHS), Information of Basic Care System and Pact of Recorder. The main individual activities consist on Oral Hygiene Orientation (87,5%) and Fluoridization (95%). Group activities remain at scholar groups acts(91,25%) being fluoridization done by 91,25% and educative activities by 86,25% mainly through speeches (61,25%).Orientations about oral cancer were detached on the aged group (39,96%)and hypertensive and diabetic group (19,51%), as well as integration with the health team was respectively 21,93% and 39,02%. Speeches and talk rounds are the main activities for aged, hypertensive and diabetics, and are also expressive to pregnant women. For pregnant women and babies group predominate hygiene and diet orientation. About the evaluation instruments 73,75% of the dentists do not consider their existence, despite 73,75% consider that actions have impact specially due to a increase of hygiene and decrease of caries index on the children. The main sources of basement are clinical experience (42,5%) and preparative courses (33,75%), only 7,5% mentioned population need and local reality. The Oral Health Co-Ordinator related the Attention Pact Recorder and records from the AIS - UHS as evaluation source but also detached the lack of epidemiologic surveying. She mentioned the increase of supervised brushing as a positive result and detached the hardness of the professional to act inside the family and as a team. The AIS-UHS records showed that fluoridization is more used individually and the supervised brushing is predominant as a group action, besides that educative activities at the Health Establishment predominate due to actions at the community. Facing the results, it was verified the act in different groups, special attention given to scholars, as well as focus the activities on caries giving less emphasis to other oral problems. Despite that, there was no epidemiologic instruments or data of the local reality to guide the actions and to be part of the planning and evaluation process

The most common malignant neoplasm of the oral cavity and oropharynx are squamous cell carcinoma. Injuries to the same stage and subjected to the same treatment protocol have sometimes different evolutionary courses. The scope of this study was to investigate, through a retrospective cohort, associations between the number of CD8 + T cells and natural killer, identified immunohistochemically in the inflammatory infiltrate in a series of cases of oral squamous cell carcinoma and orofaringeano, and the level of tumor response to radiotherapy and chemotherapy, overall survival and relapse-free survival of patients. We identified 54 patients with unresectable disease were treated exclusively with radiotherapy and chemotherapy. The median follow-up was 22 months. The sample was characterized by the predominance of male subjects, median age 60 years, all were smokers. The most frequent site was the tongue and 81.5% were in stage IV. Patients with disease in the oral cavity had a worse response to treatment (p = 0.006), worse relapse-free survival (p = 0.007), worse overall survival (p = 0.007). The advanced T stage was shown a negative prognostic factor (p= 0.006) for the clinical treatment response made. Immunohistochemistry was performed to select CD8 + cells (anti-CD8) and NK cells (anti-CD57). Lymphocytes positive and negative markings were counted using the program ImageJ ®. Two groups were created for each marking evaluated: Group I patients with more than 50% cells positive, Group II: less than 50% of labeled cells. For CD8 + cells detected in 38 (70.3%) of Group I were CD8 + and 16 (29.7%) Group II CD8 +. For NK cells, 26 (48.15%) Group I NK and 28 (51.85%) Group II NK. Regarding the clinical response to treatment, we observed that 39% of patients achieved a complete response and 25.9% remained without recurrence at the end of follow-up. These results were better in Group I CD8 + (p = 0.2). Identified that 72.2% of patients progressed to death, this finding had no association with the immunohistochemical data. There was no statistically significant differences between the number of CD8 + and NK cells and the ability of tumor response to radiotherapy and chemotherapy, or with overall survival and relapse-free survival of patients. However, especially in relation to a learned response, we found that this group of patients with advanced disease have a low count of CD8 + T cells active. Believing in the role that the immune response plays in the local fight against neoplastic cells, however, our results do not support the use of quantitative analysis of CD8 + T cells and NK cells as a prognostic factors for oral squamous cell carcinoma and oropharynx
