Realistic Evaluation of EWS and ALERT: factors enabling and constraining implementation Background The implementation of EWS and ALERT in practice is essential to the success of Rapid Response Systems but is dependent upon nurses utilising EWS protocols and applying ALERT best practice guidelines. To date there is limited evidence on the effectiveness of EWS or ALERT as research has primarily focused on measuring patient outcomes (cardiac arrests, ICU admissions) following the implementation of a Rapid Response Team. Complex interventions in healthcare aimed at changing service delivery and related behaviour of health professionals require a different research approach to evaluate the evidence. To understand how and why EWS and ALERT work, or might not work, research needs to consider the social, cultural and organisational influences that will impact on successful implementation in practice. This requires a research approach that considers both the processes and outcomes of complex interventions, such as EWS and ALERT, implemented in practice. Realistic Evaluation is such an approach and was used to explain the factors that enable and constrain the implementation of EWS and ALERT in practice [1]. Aim The aim of this study was to evaluate factors that enabled and constrained the implementation and service delivery of early warnings systems (EWS) and ALERT in practice in order to provide direction for enabling their success and sustainability. Methods The research design was a multiple case study approach of four wards in two hospitals in Northern Ireland. It followed the principles of realist evaluation research which allowed empirical data to be gathered to test and refine RRS programme theory. This approach used a variety of mixed methods to test the programme theories including individual and focus group interviews, observation and documentary analysis in a two stage process. A purposive sample of 75 key informants participated in individual and focus group interviews. Observation and documentary analysis of EWS compliance data and ALERT training records provided further evidence to support or refute the interview findings. Data was analysed using NVIVO8 to categorise interview findings and SPSS for ALERT documentary data. These findings were further synthesised by undertaking a within and cross case comparison to explain the factors enabling and constraining EWS and ALERT. Results A cross case analysis highlighted similarities, differences and factors enabling or constraining successful implementation across the case study sites. Findings showed that personal (confidence; clinical judgement; personality), social (ward leadership; communication), organisational (workload and staffing issues; pressure from managers to complete EWS audit and targets), educational (constraints on training; no clinical educator on ward) and cultural (routine task delegated) influences impact on EWS and acute care training outcomes. There were also differences noted between medical and surgical wards across both case sites. Conclusions Realist Evaluation allows refinement and development of the RRS programme theory to explain the realities of practice. These refined RRS programme theories are capable of informing the planning of future service provision and provide direction for enabling their success and sustainability. References: 1. Physical inactivity is the fourth leading risk factor for global mortality, with most of these deaths occurring in low and middle-income countries (LMICs) like India. Research from developed countries has consistently demonstrated associations between built environment features and physical activity levels of populations. The development of culturally sensitive and reliable measures of the built environment is a necessary first step for accurate analysis of environmental correlates of physical activity in LMICs. This study systematically adapted the Neighborhood Environment Walkability Scale (NEWS) for India and evaluated aspects of test-retest reliability of the adapted version among Indian adults. Cultural adaptation of the NEWS was conducted by Indian and international experts. Semi-structured interviews were conducted with local residents and key informants in the city of Chennai, India. At baseline, participants (N = 370; female = 47.2%) from Chennai completed the adapted NEWS-India surveys on perceived residential density, land use mix-diversity, land use mix-access, street connectivity, infrastructure and safety for walking and cycling, aesthetics, traffic safety, and safety from crime. NEWS-India was administered for a second time to consenting participants (N = 62; female = 53.2%) with a gap of 2–3 weeks between successive administrations. Qualitative findings demonstrated that built environment barriers and constraints to active commuting and physical activity behaviors intersected with social ecological systems. The adapted NEWS subscales had moderate to high test-retest reliability (ICC range 0.48–0.99). The NEWS-India demonstrated acceptable measurement properties among Indian adults and may be a useful tool for evaluation of built environment attributes in India. Further adaptation and evaluation in rural and suburban settings in India is essential to create a version that could be used throughout India.

BACKGROUND: The needs of children with autism spectrum disorder (ASD) are complex and this is reflected in the number and diversity of outcomes assessed and measurement tools used to collect evidence about children's progress. Relevant outcomes include improvement in core ASD impairments, such as communication, social awareness, sensory sensitivities and repetitiveness; skills such as social functioning and play; participation outcomes such as social inclusion; and parent and family impact.

OBJECTIVES: To examine the measurement properties of tools used to measure progress and outcomes in children with ASD up to the age of 6 years. To identify outcome areas regarded as important by people with ASD and parents.

METHODS: The MeASURe (Measurement in Autism Spectrum disorder Under Review) research collaboration included ASD experts and review methodologists. We undertook systematic review of tools used in ASD early intervention and observational studies from 1992 to 2013; systematic review, using the COSMIN checklist (Consensus-based Standards for the selection of health Measurement Instruments) of papers addressing the measurement properties of identified tools in children with ASD; and synthesis of evidence and gaps. The review design and process was informed throughout by consultation with stakeholders including parents, young people with ASD, clinicians and researchers.

RESULTS: The conceptual framework developed for the review was drawn from the International Classification of Functioning, Disability and Health, including the domains 'Impairments', 'Activity Level Indicators', 'Participation', and 'Family Measures'. In review 1, 10,154 papers were sifted - 3091 by full text - and data extracted from 184; in total, 131 tools were identified, excluding observational coding, study-specific measures and those not in English. In review 2, 2665 papers were sifted and data concerning measurement properties of 57 (43%) tools were extracted from 128 papers. Evidence for the measurement properties of the reviewed tools was combined with information about their accessibility and presentation. Twelve tools were identified as having the strongest supporting evidence, the majority measuring autism characteristics and problem behaviour. The patchy evidence and limited scope of outcomes measured mean these tools do not constitute a 'recommended battery' for use. In particular, there is little evidence that the identified tools would be good at detecting change in intervention studies. The obvious gaps in available outcome measurement include well-being and participation outcomes for children, and family quality-of-life outcomes, domains particularly valued by our informants (young people with ASD and parents).

CONCLUSIONS: This is the first systematic review of the quality and appropriateness of tools designed to monitor progress and outcomes of young children with ASD. Although it was not possible to recommend fully robust tools at this stage, the review consolidates what is known about the field and will act as a benchmark for future developments. With input from parents and other stakeholders, recommendations are made about priority targets for research.

FUTURE WORK: Priorities include development of a tool to measure child quality of life in ASD, and validation of a potential primary outcome tool for trials of early social communication intervention.

Concetualmente ancorado em referenciais teóricos que demonstram o papel fundamental da narrativa e a ubiquidade dos produtos dos esquemas narrativos nos vários domínios da vida humana, bem como a importância do desenvolvimento de competências narrativas adequadas enquanto processo determinante no sentido da construção da literacia, este trabalho procurou compreender os processos de estruturação e a estrutura de histórias contadas por crianças do 1º Ciclo do Ensino Básico (CEB), numa abordagem morfológica suportada pela teoria e modelo de Vladimir Propp, no sentido da concetualização e prototipagem de uma aplicação multimédia de suporte ao conto de histórias. Os estudos empíricos conduzidos, de natureza comparativo-exploratória, permitiram que as crianças assumissem o papel de contadoras de histórias e informantes, fornecendo-nos o corpus e conteúdo para análises morfológicas e de conteúdo, sobretudo qualitativas e com objetivos fundamentalmente descritivos. Os resultados do estudo suportaram o delineamento de linhas orientadoras para o desenvolvimento do modelo concetual e requisitos funcionais de uma aplicação multimédia de suporte ao conto de histórias, de natureza educativa, que veio a designar-se por Curucucu©, Conto eu ou contas tu?. O processo de prototipagem da aplicação multimédia implementou uma abordagem iterativa e centrada no aluno para efeitos de concetualização, desenho, implementação e testes da ferramenta e tendo como preocupação central o desenvolvimento de interfaces adequadas, quer às crianças, quer aos objetivos e contextos de utilização, integrando crianças e professores do 1.º CEB, bem como outros informantes (na qualidade de peritos) em fases específicas do processo. Para além das limitações encontradas no estudo, avançam-se propostas de investigação futura.

As alterações no panorama social português que ocorreram no virar do milénio, marcadas pela chegada massiva de imigrantes provenientes de países que não tinham laços históricos e linguísticos com Portugal – com destaque para as comunidades do Leste europeu –, vieram colocar novos desafios à sociedade e à escola portuguesas. Com o propósito de caracterizar o perfil de apropriação da Língua Portuguesa (LP) de aprendentes adultos eslavófonos residentes em Portugal, foram recolhidos 87 questionários e testes linguísticos junto de adultos falantes de línguas eslavas que frequentavam cursos de LP em contexto formal/ não formal. Complementarmente, foram realizadas 17 entrevistas a aprendentes e professores. A análise de dados, de carácter quantitativo e qualitativo, centrou-se em três eixos fundamentais: a) o perfil de apropriação da LP; b) a gestão dos repertórios linguísticos; e c) as dificuldades manifestadas na apropriação, compreensão e uso do sistema temporal-aspetual em LP. Para cada um destes eixos, salientamos as seguintes conclusões: a) o contexto formal/ não formal é aquele que é considerado o mais eficaz; a apropriação da LP decorre ainda em contexto de autoaprendizagem e em contexto informal. A expressão oral afigura-se como uma competência de intervenção prioritária para este público. b) os repertórios linguísticos dos informantes deste estudo são limitados em número e diversidade de línguas, estando quase circunscritos às línguas eslavas, com amplo destaque para o Russo. A influência das Línguas Maternas na apropriação da LP é percecionada, de uma forma geral, como negativa, e é reduzida a adoção de estratégias de rentabilização interlinguística. c) as principais dificuldades no que se refere ao Tempo e Aspeto prendem- -se com o modo conjuntivo, os tempos compostos, os valores secundários de alguns tempos verbais, o pretérito perfeito composto do indicativo, a distinção entre o pretérito perfeito simples e o pretérito imperfeito do indicativo e a distinção entre os verbos ‘ser’ e ‘estar’. De uma forma global, apontam-se como pistas de intervenção a criação de cursos de LP de nível mais avançado, uma maior rentabilização do capital linguístico presente em sala de aula e a frequência, por parte dos docentes, de formação em línguas eslavas. A nível de recursos didáticos, sugere-se a elaboração de materiais bilingues para autoaprendizagem e de materiais de apoio ao professor. Seria igualmente útil a promoção de mais oportunidades de interação entre imigrantes e nativos.

A introdução de novas exigências na Avaliação do Desempenho Docente (ADD) em Portugal (Decretos-Lei 15/2007 e 75/2010; Decretos Regulamentares 2/2008 e 2/2010) tem gerado nas escolas e nos professores, desde 2008, situações organizacionais complexas, que urge analisar e compreender. As políticas educativas têm vindo a apontar para a necessidade de um maior investimento nos processos de avaliação de professores, dando relevância ao papel a desempenhar pela supervisão pedagógica nesse contexto. A supervisão da prática letiva passou a ser considerada procedimento fundamental, na medida em que se tornou difícil conceber uma avaliação dos docentes que não incluísse sessões de observação em sala de aula. Fruto destas novas políticas de avaliação de professores, a ADD apresentou-se, do ponto de vista normativo, com base em duas funções: sumativa e formativa. Estas funções exigem maiores investimentos, novas práticas e uma reflexão sobre o papel do professor ancorada numa perspetiva holística, transformadora e emancipatória do seu desenvolvimento profissional. O principal objetivo do nosso estudo consiste em compreender as perceções e práticas desenvolvidas pelos docentes face às alterações introduzidas no sistema da ADD no geral, e, em particular, o papel desempenhado pela supervisão pedagógica (SP) na avaliação entre pares, no segundo ciclo avaliativo (2009-2011). A investigação empírica tem por base um estudo de caso desenvolvido num Agrupamento de escolas do Distrito de Aveiro. A recolha de dados foi concretizada através da administração de um questionário a todos os docentes do Agrupamento e da condução de entrevistas a 26 informadores privilegiados do processo avaliativo. Os principais resultados do estudo de caso apontam para: i) a opinião desfavorável dos professores face ao atual modelo de ADD, dada a sua natureza burocrática, complexa, injusta e a sua escassa utilidade prática, preenchendo apenas requisitos legais e de controlo; ii) um processo de SP, concretamente, a observação de aulas, que não contribui para o desenvolvimento profissional dos professores e tem impacte negativo no trabalho docente, acentuando a artificialidade dos procedimentos e aumentando o clima de competição e de conflito; iii) a sensação de desconforto entre os intervenientes, fazendo emergir sentimentos de injustiça, insegurança e ansiedade que, em seu entender, constituem constrangimentos à melhoria das aprendizagens dos alunos e à qualidade da escola; iv) uma escassa implicação na melhoria das práticas futuras e do desenvolvimento profissional dos docentes. Estes resultados constituem um desafio colocado aos investigadores no sentido de identificarem alternativas de ação para poderem lidar com a complexidade inerente ao processo de avaliação na sua relação com a supervisão pedagógica, com vista à melhoria de desempenho das escolas e dos professores.

RESUMO - Pretende-se com este projecto fazer uma reflexão sobre a problemática dos modelos de governação na gestão pública de hospitais e sobre a forma de incrementar a capacidade estratégica da gestão, contextualizando a governação dos hospitais num quadro mais amplo e comunitário, integrando os interesses e expectativas dos diversos interessados no funcionamento dos hospitais públicos. Sinteticamente, a questão fundamental de investigação é: Como conseguirá o sistema de governação de um hospital público incorporar os interesses de todos os seus stakeholders e shareholders? A resposta a esta questão integra duas fases, uma primeira dedicada ao estudo de um novo modelo de configuração para o órgão de governação de topo do hospital público e uma segunda fase dedicada à construção de um questionário para discussão e validação do modelo proposto através da técnica Delphi. A metodologia utilizada passou pela condução prévia de entrevistas exploratórias a informadores privilegiados e pela divisão do projecto em duas partes – uma primeira parte de enquadramento que segue a metodologia clássica da revisão bibliográfica e uma segunda parte prática que segue a metodologia Delphi, precedida de um levantamento das questões críticas para integrar no questionário que servirá de base à discussão dos peritos. Como resultado, o actual modelo pareceu-nos dificultar a consideração das expectativas e dos interesses dos proprietários/accionistas e dos stakeholders em geral do hospital público, concluindo-se pela necessidade de um novo modelo que estabeleça, inequivocamente, os papéis e as funções inerentes ao órgão de governação, garantindo a internalização da perspectiva de todos os interessados. -------------------------------ABSTRACT - The objective of this project is to reflect on the problematic of models of governance in the public management of hospitals and on the means for incrementing strategic management capacity. It does so by contextualizing the governance of public hospitals on a more ample community–wide scale, integrating the interests and expectations of different parties. The main research question is: how can the system of governance of a public hospital incorporate the interests of all its stakeholders and shareholders? The answer to this question involves two phases, one dedicated to the study of the configuration of the top-level governing body of the public hospital and, a second, dedicated to the construction of a questionnaire for discussion and validation of the proposed model by means of a Delphi technique. The methodology involved, first of all, exploratory interviews with key- informants and by the structuring of the project in two parts – the first, dedicated to contextualization by means of a standard literature review and the second, essentially practical by means of the Delphi technique, preceded by the raising of critical questions that integrate the questionnaire that will form the basis of expert discussion. The present model of public hospital governance appears to limit the possibility of integrating the expectations and interests of stakeholders and owner/shareholders. Acquired behavioral changes have essentially been described in advanced multiple sclerosis (MS). The present study was designed to determine whether behavioral modifications specifically related to the MS pathological process could be identified in the initial phase of the disease, as compared to control patients with chronic, relapsing and progressive inflammatory disorders not involving the central nervous system (CNS). Eighty-eight early MS patients (Expanded Disability Status Scale score <or= 2.5) and 48 controls were tested. Perceived changes by informants in behavioral control, goal-directed behavior, decision making, emotional expression, insight and interpersonal relationships were assessed using the Iowa Scale of Personality Change (ISPC). Executive behavioral disturbances were screened using the Dysexecutive Questionnaire (DEX). The mean change between the premorbid and postmorbid ISPC ratings was similar in the MS [12.2 (SD 15.6)] and in the control [11.5 (SD 15.1)] group. The perceived behavioral changes (PBCs) most frequently reported in both groups were lack of stamina, lability/moodiness, anxiety, vulnerability to stress and irritability. Pathological scores in the DEX were also similar in both groups. Correlations between PBCs and DEX scores were different in MS and control groups. MS patients with cognitive impairment had a marginally higher number of PBCs than control patients (p=0.056) and a significantly higher DEXp score (p=0.04). These results suggest that (1) PBCs occurring in early MS patients were not different from those induced by comparable chronic non-CNS disorders, (2) qualitative differences in the relationship between behavioral symptoms and executive-behavioral changes may exist between MS and control groups, and (3) behavioral symptoms seem associated with cognitive deficits in MS. We further plan to assess these observations longitudinally.

Abstract: In Imperial Eyes Mary Louise Pratt (1992: 7, emphasis original) defines autoethnography as "instances in which colonized subjects undertake to represent themselves in ways that engage with the colonizer's own terms ... in response to or in dialogue with . . . metropolitan representations." Although Pratt's conceptualization of autoethnography has much to offer post-colonial studies, it has received little attention in the field. In this thesis, I interrogate Pratt's notion of autoethnography as a theoretical tool for understanding the self-representations of subordinate peoples within transcultural terrains of signification. I argue that autoethnography is a concept that allows us to move beyond some theoretical dualisms, and to recognize the (necessary) coexistence of subordinate peoples' simultaneous accommodation of and resistance to dominant representations of themselves. I suggest that even when autoethnographic expressions seem to rely on or to reproduce dominant knowledges, their very existence as speech acts implicitly resists dominant discourses which objectify members of oppressed populations and re-create them as Native Informants. I use Pratt's concept to analyze two books by Islamic feminist sociologist Fatima Memissi. Memissi's Dreams ofTrespass and Scheherazade Goes West illustrate the simultaneity of accommodation and disruption evident in autoethnographic communication. Across the two books, Memissi shows herself renegotiating the discourses which discipline her (and her speech). She switches back and forth between the positions of reader and author, demonstrates the reciprocity of the disciplinary gaze (she looks back at her dominants, reading their own reading of her representation of her social group), and provides a model of autoethnographic dialogue.

Discussions concerning the challenges of combining work and family are certainly not new, and still actively continue. There is, however, a silence in the related literature regarding a comprehensive description of integrating specifically university academic ~. work and family responsibilities. This silence is especially evident for men who are parents as well as academics. With the participation of 4 key informants, this qualitative research study gave voice to men and women who participate in the academic labour of a Canadian university as professors, and as graduate students, along with the parenting labour of at least 1 child under the age of7. Methodology was developed to reveal in-depth perspectives regarding the work practices employed by 4 key informants as they combined intellectual and child-care responsibilities. Multiple data collection methods included journal reflections, day time observation sessions, a focus group, and a final evaluation questionnaire. Using research findings, together with information extrapolated from Three Models of the Family (Eichler, 1997), this study also took steps toward developing a Proposed "Three Models of the University," to offer explanation for the work practices of the key informants as academics/parents, and also for future consideration in university policy formation.
