Aims: The HR-NBL1 Study of the European SIOP Neuroblastoma Group (SIOPEN) randomised two high dose regimens to learn about potential superiority and toxicity profiles.Patients and Methods: At interim analysis 1483 high risk neuroblastoma patients (893 males) were included since 2002 with either INSS stage 4 disease (1383 pts) above 1 year, or as infants (59 pts) and stage 2&3 of any age (145 pts) with MYCN amplification. The median age at diagnosis was 2.9 years (1 month-19.9 years) with a median follow up of 3 years. Response eligibility criteria prior randomisation after Rapid Cojec Induction (J Clin Oncol, 2010) ± 2 courses of TVD (Cancer, 2003) included complete bone marrow remission and at least partial response at skeletal sites with no more than 3, but improved mIBG positive spots and a PBSC harvest of at least 3x10E6 CD34/kgBW. The randomised regimens were BuMel [busulfan oral till 2006, 4x150mg/m² in 4 ED; or intravenous use according to body weight as licenced thereafter; melphalan 140mg/m²/day) and CEM [carboplatinum ctn. infusion (4x AUC 4.1mg/ml.min/day, etoposid ctn. infusion (4x 338mg/m²/day or [4x 200mg/m²/day]*, melphalan (3x70mg/m²/day; 3x60mg/m²/day*;*reduced dosis if GFR< 100ml/min/1.73m²). Supportive care followed institutional guidelines. VOD prophylaxis included ursadiol, but randomised patients were not eligible for the prophylactic defibrotide trial. Local control included surgery and radiotherapy of 21Gy.Results: Of 1483 patients, 584 were being randomised for the high dose question at data lock. A significant difference in event free survival (3-year EFS 49% vs. 33%, p<0.001) and overall survival (3-year OS 61% vs. 48%, p=0.003) favouring the BuMel regimen over the CEM regimen was demonstrated. The relapse/progression rate was significantly higher after CEM (0.60±0.03) than after BuMel (0.48±0.03)(p<0.001). Toxicity data had reached 80% completeness at last analysis. The severe toxicity rate up to day 100 (ICU and toxic deaths) was below 10%, but was significantly higher for CEM (p= 0.014). The acute toxic death rate was 3% for BuMel and 5% for CEM (NS). The acute HDT toxicity profile favours the BuMel regimen in spite of a total VOD incidence of 18% (grade 3:5%).Conclusions: The Peto rule of P<0.001 at interim analysis on the primary endpoint, EFS was met. Hence randomization was stopped with BuMel as recommended standard treatment in the HR-NBl1/SIOPEN trial which is still accruing for the randomised immunotherapy question.

The Goliath grouper, Epinephelus itajara, a large-bodied (similar to 2.5 m TL, > 400 kg) and critically endangered fish (Epinephelidae), is highly Vulnerable to overfishing. Although protected from fishing in many countries, its exploitation in Mexico is unregulated; a situation that puts its populations at risk. Fishery records of E. itajara are scarce, which prevents determination of its fishery status. This work aimed to elucidate the E itajara fishery in the northern Yucatan Peninsula by 1) analyzing available catch records and 2) interviewing veteran fishermen (local ecological knowledge) from two traditional landing sites: Dzilam de Bravo and Puerto Progreso. Historic fishery records from two fishing cooperatives were analyzed in order to elucidate the current situation and offer viable alternatives for conservation and management. Catches have decreased severely. Local knowledge obtained from fishermen represented a very important source of information for reconstructing the fisheries history of this species. Conservation measures that incorporate regional and international regulations on critically endangered fish species are suggested

La crisi actual ha agreujat la situació socioeconòmica de nombroses persones i famílies , esdevenint una realitat social cada vegada més complexa . Aquest nou context, ha portat a que aquesta nova bossa de persones hagin de recórrer a demanar suport als serveis socials bàsics. Persones que fins aquest moment havien estat en una situació normalitzada i en aquests moments estan en el llindar de l’exclusió social. El treball Sorgeix de la necessitat del món local, concretament de l’Àrea Bàsica de Serveis Socials del Consell Comarcal de l’Alt Penedès, de disposar d’eines d’inserció social per a les persones perceptores de la renda mínima d’inserció de la comarca, que formen part d’aquest col·lectiu en risc d’exclusió. Planteja la importància que des del món local es creïn aliances amb entitats , associacions diverses i de diferents àmbits , per tal d’adequar les respostes i les mesures d’inserció social segons la realitat de cada persona i família a través de diferents tasques col·laboratives, dins la seva comunitat, el seu entorn més immediat. Presenta la renda mínima com una via útil i necessària per lluitar contra la pobresa i a favor de la inclusió social

OBJECTIVE: To investigate whether better management of chronic conditions by family practices reduces mortality risk. DATA: Two random samples of 5 million patients registered with over 8,000 English family practices followed up for 4 years (2004/5-2007/8). Measures of the quality of disease management for 10 conditions were constructed for each family practice for each year. The outcome measure was an indicator taking the value 1 if the patient died during a specified year, 0 otherwise. STUDY DESIGN: Cross-section and multilevel panel data multiple logistic regressions were estimated. Covariates included age, gender, morbidity, hospitalizations, attributed socio-economic characteristics, and local health care supply measures. PRINCIPAL FINDINGS: Although a composite measure of the quality of disease management for all 10 conditions was significantly associated with lower mortality, only the quality of stroke care was significant when all 10 quality measures were entered in the regression. CONCLUSIONS: The panel data results suggest that a 1 percent improvement in the quality of stroke care could reduce the annual number of deaths in England by 782 [95 percent CI: 423, 1140]. A longer study period may be necessary to detect any mortality impact of better management of other conditions.

Allergy has been on the rise for half a century and concerns nearly 30% of children; it has now become a real public health problem. The guidelines on prevention of allergy set up by the French Society of Paediatrics (SFP) and the European Society of Paediatric Allergology and Clinical Immunology (ESPACI) are based on screening children at risk through a systematic search of the family history and recommend, for children at risk, exclusive breastfeeding whenever possible or otherwise utilization of hypoallergenic infant formula, which has demonstrated efficacy. The AllerNaiss practice survey assessed the modes of screening and prevention of allergy in French maternity units in 2012. The SFP guidelines are known by 82% of the maternity units that took part in the survey, and the ESPACI guidelines by 55% of them. A screening strategy is in place in 59% of the participating maternity wards, based on local consensus for 36% of them, 13% of the units having a written screening procedure. Screening is based on the search for a history of allergy in first-degree relatives (99%) during pregnancy (51%), in the delivery room (50%), and after delivery (89%). A mode of prevention of the risk of allergy exists in 62% of the maternity units, most often in writing (49%). A hypoallergenic infant formula is prescribed for non-breastfed children in 90% of the units. The survey shows that there is a real need for formalization of allergy risk screening and prevention of allergy in newborns in French maternity units.

Top predator loss is a major global problem, with a current trend in biodiversity loss towards high trophic levels that modifies most ecosystems worldwide. Most research in this area is focused on large-bodied predators, despite the high extinction risk of small-bodied freshwater fish that often act as apex consumers. Consequently, it remains unknown if intermittent streams are affected by the consequences of top-predators' extirpations. The aim of our research was to determine how this global problem affects intermittent streams and, in particular, if the loss of a small-bodied top predator (1) leads to a 'mesopredator release', affects primary consumers and changes whole community structures, and (2) triggers a cascade effect modifying the ecosystem function. To address these questions, we studied the topdown effects of a small endangered fish species, Barbus meridionalis (the Mediterranean barbel), conducting an enclosure/exclosure mesocosm experiment in an intermittent stream where B. meridionalis became locally extinct following a wildfire.We found that top predator absence led to 'mesopredator release', and also to 'prey release' despite intraguild predation, which contrasts with traditional food web theory. In addition, B. meridionalis extirpation changed whole macroinvertebrate community composition and increased total macroinvertebrate density. Regarding ecosystem function, periphyton primary production decreased in apex consumer absence. In this study, the apex consumer was functionally irreplaceable; its local extinction led to the loss of an important functional role that resulted in major changes to the ecosystem's structure and function. This study evidences that intermittent streams can be affected by the consequences of apex consumers' extinctions, and that the loss of small-bodied top predators can lead to large ecosystem changes. We recommend the reintroduction of small-bodied apex consumers to systems where they have been extirpated, to restore ecosystem structure and function.

Techniques of evaluation of risks coming from inherent uncertainties to the agricultural activity should accompany planning studies. The risk analysis should be carried out by risk simulation using techniques as the Monte Carlo method. This study was carried out to develop a computer program so-called P-RISCO for the application of risky simulations on linear programming models, to apply to a case study, as well to test the results comparatively to the @RISK program. In the risk analysis it was observed that the average of the output variable total net present value, U, was considerably lower than the maximum U value obtained from the linear programming model. It was also verified that the enterprise will be front to expressive risk of shortage of water in the month of April, what doesn't happen for the cropping pattern obtained by the minimization of the irrigation requirement in the months of April in the four years. The scenario analysis indicated that the sale price of the passion fruit crop exercises expressive influence on the financial performance of the enterprise. In the comparative analysis it was verified the equivalence of P-RISCO and @RISK programs in the execution of the risk simulation for the considered scenario.

The objective of this research was to describe how Nordic companies manage hazard risks in their operations in Russia and how the local business environment is considered to affect the hazard risks. Research methods used in this research were literature review and expert interviews. Twelve Nordic industrial companies operating in different fields of industry were interviewed. Large Nordic companies typically guide risk management centralized from the parent company on behalf of the whole company group and the risk management standards and policies are integrated in all subsidiaries. Parent companies typically control hazard risk management in Russia by regular risk management reporting, auditing the Russian sites and by training local managers and employees to risk management work. Many companies have experienced several losses in the first years of operating in Russia before the risk management policies have been implemented in Russian subsidiaries. The companies have learned to take local characteristics better into account by experience and most companies are quite satisfied with their current risk management standards in Russia. The interviews indicate that companies experience especially the poor quality of infrastructure, some features in Russian organizational culture and high level of criminality to increase hazard risks in Russia. However, understanding these features and risks in the business environment makes the management of these risks possible. Risks related to infrastructure can be managed in advance by decreasing dependencies of infrastructure and considering the infrastructure quality already when planning the business operations. Also good local network is often considered critical in order to overcome the complications related to infrastructure. Russian personnel has typically different attitude towards risk management than Nordic personnel and neglecting safety and maintenance and concealing losses is more typical in Russia. By training and guiding the local personnel risk management and safety work and desired ways of actions these risks can be decreased. Criminality risks are often managed to certain extent by investing in security, increasing supervising and paying attention to reliability of the employees and other interest groups of the company.

Tämän työn tavoitteena on antaa kuvaus riskinhallintamenetelmistä viidelle välituotekemikaalille, joita käytetään Stora Enson Imatran tehtailla. Välituotekemikaalit ovat mustalipeä, viherlipeä, valkolipeä, natriumbisulfiitti ja natriumsulfiitti. Nämä kemikaalit ovat jo rekisteröityjä ECHA:an ja rekisteröintiin liittyen ECHA:an on toimitettava myös kuvaus riskinhallintamenetelmistä. Työn alussa kuvaillaan työn kannalta olennaiset säädökset ja viranomaiset, jotka valvovat kemikaalien käyttöä ja valmistusta Euroopan Unionin alueella. Tämän jälkeen kerrotaan yleisesti välituotekemikaalien rekisteröintikriteereistä. Työn loppuosa käsittää kuvauksen riskinhallintamenetelmistä jokaiselle kemikaalille. Riskinhallintamenetelmät sisältävät eristyksen teknisin keinoin, menettelytapa- ja valvontatekniikat, johtamistavat ja henkilökunnan koulutuksen ja välituotekemikaalien kuljetuksen. Myös jokaisen kemikaalin ominaisuudet on kuvattu ja lyhyt prosessikuvaus kemikaalien valmistuksesta ja käytöstä on esitetty helpottamaan ymmärtämistä.

The evolving antimicrobial resistance coupled with a recent increase in incidence highlights the importance of reducing gonococcal transmission. Establishing novel risk factors associated with gonorrhea facilitates the development of appropriate prevention and disease control strategies. Sexual Network Analysis (NA), a novel research technique used to further understand sexually transmitted infections, was used to identify network-based risk factors in a defined region in Ontario, Canada experiencing an increase in the incidence of gonorrhea. Linear network structures were identified as important reservoirs of gonococcal transmission. Additionally, a significant association between a central network position and gonorrhea was observed. The central participants were more likely to be younger, report a greater number of risk factors, engage in anonymous sex, have multiple sex partners in the past six months and have sex with the same sex. The network-based risk factors identified through sexual NA, serving as a method of analyzing local surveillance data, support the development of strategies aimed at reducing gonococcal spread.

Recent work shows that a low correlation between the instruments and the included variables leads to serious inference problems. We extend the local-to-zero analysis of models with weak instruments to models with estimated instruments and regressors and with higher-order dependence between instruments and disturbances. This makes this framework applicable to linear models with expectation variables that are estimated non-parametrically. Two examples of such models are the risk-return trade-off in finance and the impact of inflation uncertainty on real economic activity. Results show that inference based on Lagrange Multiplier (LM) tests is more robust to weak instruments than Wald-based inference. Using LM confidence intervals leads us to conclude that no statistically significant risk premium is present in returns on the S&P 500 index, excess holding yields between 6-month and 3-month Treasury bills, or in yen-dollar spot returns.

Résumé Le but de cette thèse est de comprendre les représentations du risque chez les jeunes de la rue. Plus précisément, elle s’intéresse à appréhender les constructions du risque que font les jeunes de la rue eux-mêmes, d’autant plus que ces jeunes sont définis comme un groupe à risque. Si le risque est plus souvent défini de manière stricte comme le mal éventuel, dans cette étude, il est défini plus largement intégrant l’idée des opportunités et prises de risque. Ancrée dans une perspective double du constructionnisme social et de l’interactionnisme symbolique, cette recherche a exploré les savoirs des jeunes sur les risques qu’ils vivaient dans les contextes observés et la manière dont ils les appréhendaient. Pour y parvenir, cette recherche s’inscrit dans une approche ethnographique pour mieux comprendre le monde des jeunes de la rue, utilisant des méthodes d'observation participante et dévoilée et des entrevues informelles variées. Cette approche globale permet de saisir comment les jeunes définissent leur capacité à estimer, gérer, éviter ou prendre des risques. L’utilisation d’une perspective longitudinale (de un à deux ans) et les relations de confiance bâties avec ces jeunes, ont permis de suivre comment la construction identitaire des jeunes observés a influencé leurs perception du risque et leurs pratiques de débrouillardise. En outre, les liens établis ont permis de révéler les points de vue singuliers des jeunes mais aussi leurs savoirs expérientiels relatifs aux risques. Il s’agit dans cette étude de montrer à partir des théories générales qui définissent nos sociétés comme des sociétés du risque, comment des individus, identifiés comme appartenant à un groupe à risque, définissent et gèrent leurs risques à partir de leur propre expérience et point de vue afin de révéler la diversité et la complexité des expériences et savoirs des jeunes de la rue à l’endroit du risque. En effet, cette thèse montre qu’un ancrage dans une sociologie du risque permet de sortir de l’image de victime ou de déviance associée généralement aux jeunes de la rue mais qu’elle demeure marquée par la promotion de la sécurité légitimant intervention et régulation de la situation des jeunes de la rue tout en ignorant l’expérience même des jeunes. Les discours sur les risques associés à la rue sont alors inscrits dans une logique d’expertise. Cette étude vise à sortir de ces préconceptions des risques pris par les jeunes de la rue pour au contraire s’attarder à comprendre comment se définit le risque à partir du sens que les jeunes accordent et les expériences qu’ils en ont. Introducción: El carcinoma de mama es el tumor maligno más frecuente entre las mujeres y representa una significativa mortalidad en los países en vías de desarrollo. Según datos del Instituto Nacional de Cancerología en el 2010 se reportaron 672 nuevos casos de cáncer de mama, lo que representó el 18% de todos los tumores malignos en mujeres. Durante las últimas 3 décadas las técnicas quirúrgicas para el tratamiento del cáncer de mama han presentado un cambio significativo y proponen disminución de procedimientos agresivos y radicales, intervenciones como: mastectomía radical modificada, cirugía conservadora y la disección de ganglio centinela son ejemplos claros de esta evolución asociado al incremento de la reconstrucción mamaria inmediata. Metodología: Estudio observacional tipo cohorte retrospectivo en el cual se revisó una base de datos de pacientes con cáncer de mama de las cuales 632 fueron sometidas a mastectomía radical con preservación de piel y complejo areola-pezón y mastectomía radical con preservación de piel sin preservación del complejo areola-pezón, los dos procedimientos asociados a reconstrucción mamaria inmediata y se comparó la frecuencia de recaída local entre los dos grupos. Resultados: De las 632 pacientes estudiadas al 30.5% se les realizo preservación del complejo areola pezón. Las mujeres a quienes se les realizó preservación del complejo areola pezón presentaron menor sobrevida a la recaída local a 10 años (80.51%) comparado con las mujeres a quienes no se les preservó el complejo areola pezón (87.40%), sin embargo no se encontró diferencia estadísticamente significativa para determinar que las probabilidades de sobrevida sean diferentes. Discusión: No se evidenció diferencia estadísticamente significativa entre los 2 procedimientos quirúrgicos (con y sin preservación del complejo areola pezón) en relación a la recaída local, estudios retrospectivos no han evidenciado una mayor tasa de recaídas locales en pacientes a quienes se les preserva el complejo areola-pezón, sin embargo hacen falta estudios prospectivos y aleatorizados que puedan otorgar un mayor sustento científico que garantice la seguridad de la preservación del complejo areola-pezón.

Many common bird species have declined as a result of agricultural intensification and this could be mitigated by organic farming. We paired sites for habitat and geographical location on organic and nonorganic farms in Ontario, Canada to test a priori predictions of effects on birds overall, 9 guilds and 22 species in relation to candidate models for farming practices (13 variables), local habitat features (12 variables), or habitat features that influence susceptibility to predation. We found that: (1) Overall bird abundance, but not richness, was significantly (p < 0.05) higher on organic sites (mean 43.1 individuals per site) than nonorganic sites (35.8 individuals per site). Significantly more species of birds were observed for five guilds, including primary grassland birds, on organic vs. nonorganic sites. No guild had higher richness or abundance on nonorganic farms; (2) Farming practice models were the best (ΔAIC < 4) for abundance of birds overall, primary grassland bird richness, sallier aerial insectivore richness and abundance, and abundance of ground nesters; (3) Habitat models were the best for overall richness, Neotropical migrant abundance, richness and abundance of Ontario-USA-Mexico (short-distance) migrants and resident richness; (4) Predation models were the best for richness of secondary grassland birds and ground feeders; (5) A combination of variables from the model types were best for richness or abundance overall, 13 of 18 guilds (richness and abundance) and 16 of 22 species analyzed. Five of 10 farming practice variables (including herbicide use, organic farm type) and 9 of 13 habitat variables (including hedgerow length, proportion of hay) were significant in best models. Risk modeling indicated that herbicide use could decrease primary grassland birds by one species (35% decline from 3.4 to 2.3 species) per site. Organic farming could benefit species of conservation concern by 49% (an increase from 7.6 to 11.4 grassland birds). An addition of 63 m of hedgerow could increase abundance and richness of short distance migrants by 50% (3.0 to 4.8 and 1.3 to 2.0, respectively). Increasing the proportion of hay on nonorganic farms to 50% could increase abundance of primary grassland bird by 40% (6.7 to 9.4). Our results provide support for alternative farmland designs and agricultural management systems that could enhance select bird species in farmland.

In recent years, the eastern foothills of the Rocky Mountains in northeastern British Columbia have received interest as a site of industrial wind energy development but, simultaneously, have been the subject of concern about wind development coinciding with a known migratory corridor of Golden Eagles (Aquila chrysaetos). We tracked and quantified eagle flights that crossed or followed ridgelines slated for one such wind development. We found that hourly passage rates during fall migration peaked at midday and increased by 17% with each 1 km/h increase in wind speed and by 11% with each 1°C increase in temperature. The propensity to cross the ridge tops where turbines would be situated differed between age classes, with juvenile eagles almost twice as likely to traverse the ridge-top area as adults or subadults. During fall migration, Golden Eagles were more likely to cross ridges at turbine heights (risk zone, < 150 m above ground) under headwinds or tailwinds, but this likelihood decreased with increasing temperature. Conversely, during spring migration, eagles were more likely to move within the ridge-top area under eastern crosswinds. Identifying Golden Eagle flight routes and altitudes with respect to major weather systems and local topography in the Rockies may help identify scenarios in which the potential for collisions is greatest at this and other installations.
