This article seeks to provide an in depth review about one of the most revolutionary and influential methods used in understanding the variables and processes that explain human health. Based on a new vision in the analysis of the consequences of theNazi Holocaust, a doctor-sociologist—Aaron Antonovsky— managed to influence medicine and behavioral science by facilitating the keys for the optimal development of public health today. Despite the fact that this theory began appearing in the1970s in the 20th century, its real development and expansion have been seen in recent years. In fact, in Spain, there is littlescientific literature that analyses the theoretic keys of the model in depth. This work seek to cover this gap; to achieve this objective, it first presents how the construct of salutogenesis arose, the social-cultural context that promoted it, as well as the importance public health acquires today. This is the aim of this work, which analyses the theoretical bases of the salutogenesis model,with specific emphasis on its background and precursors, aswell as its inception, development and current expansion

Background: Most of the studies on sychological distress in Spain have been conducted in small geographical areas or specific population groups. However, there are no studies that provide representative data for each Autonomous Community (AC). The objectives of this paper are to determine, both in Spain and in the AC, the prevalence of psychological distress, diagnosis, use of psychoactive medication, social support and self-perceived health, as well as to study the association between psychological distress and the rest of the variables. Methods: Cross-sectional study, using data from the 2006 National Health Survey, that was completed by 29,478 persons. Variables studied: sociodemographics, psychological distress (GHQ-12), self-perceived health, mental disorder diagnosis, functional social support (Duke) and use and prescription of psychoactive medication. Results: The prevalence of psychological distress in Spain was 20,1%; the highest prevalence was found in Canary Islands (28,2%) and the lowest in La Rioja (12,2%). Among those who presented psychological distress, 62,4% had never received a mental disorder diagnosis, and 71,6% had not used psychoactive medication in the last year. The highest prevalences of non-diagnosed cases (81,8%) and cases non-treated with psychoactive medication (83,1%) were found in La Rioja, whereas the lowest prevalences were found in Asturias. Eight percent of the persons who presented psychological distress had low social support and 63,8% reported bad self-perceived health. Conclusions: Psychological distress is a prevalent phenomenon, and more than half of the persons who suffer it receive neither a diagnosis nor psychoactive medication. Moreover, there are considerable differences between the AC.

The EMECAM Project demonstrated the short-term effect of air pollution on the death rate in 14 cities in Spain throughout the 1990-1995 period. The Spanish Multicentre Study on Health Effects of Air Pollution (EMECAS) is broadening these objectives by incorporating more recent data, information on hospital disease admissions and totaling 16 Spanish cities. This is an ecological time series study in which the response variables are the daily deaths and the emergency hospitalizations due to circulatory system diseases and respiratory diseases among the residents in each city. Pollutants analyses: suspended particles, SO2, NO2, CO and O3. Control variables: meteorological, calendar, seasonality and influenza trend and incidence. Statistical analysis: estimate of the association in each city by means of the construction of generalized additive Poisson regression models and metanalysis for obtaining combined estimators. The EMECAS Project began with the creation of three working groups (Exposure, Epidemiology and Analysis Methodology) which defined the protocol. The average levels of pollutants were below those established under the current regulations for sulfur dioxide, carbon monoxide and ozone. The NO2 and PM10 values were around those established under the regulations (40 mg/m3). This is the first study of the relationship between air pollution and disease rate among one group of Spanish cities. The pollution levels studied are moderate for some pollutants, although for others, especially NO2 and particles, these levels could entail a problem with regard to complying with the regulations in force.

BACKGROUND It has been identified differences of medical care practice in primary care related to physician's sex. Simultaneously, there are gender inequalities in the assignment of health resources. Both aspects give rise to an increasing growing interest in the management and provision of health services. OBJECTIVES To examine the differences in the referral practice made by female and male primary care physicians working in health centers in Andalusia, to consider whether there are disparities in referrals received by men and women, and to examine the interaction between patient's sex and physician's sex. METHODS Observational, cross-sectional, and multicenter study. POPULATION 4 health districts in Andalucía and their physicians. SAMPLE 382 physicians. MEASUREMENTS referral rate per visit (RV), referral rate per patient quota (RQ), patient's sex, physician: sex, age, postgraduate family medicine specialty, size of the patient quota by sex, mean number of patients/day by sex, mean age of the patient quota by sex, and proportion of men in the quota. Health center: urban / rural, size of the team, enrolled population, and postgraduate family medicine specialty's accreditation. SOURCES databases of health districts. PERIOD OF STUDY 2010. ANALYSIS Bivariate and multivariate multilevel analysis of the referral rate per visit with mixed Poisson model. RESULTS In 2010 382 physicians made 129,161 referrals to specialized care. The RQ was 23.47 and the RV was 4.92. The RQ in women and men was 27.23 and 19.78 for women physicians, being 27.37 and 19.51 for male physicians. The RV in women and men was 4.92 and 5.48 for women physicians, being 4.54 and 4.93 for male physicians. CONCLUSION There are no differences in referral according to physician's sex. However, there are signs that might indicate the existence of gender inequality, and women patient received less referrals. There are no physician-patient's sex interaction.

The objective of this study is to review the health promotion interventions on spanish youths published in Spanish in the 1995-2000 period and to describe them in accordance with the scope within which they have been carried out, the topics addressed, the methodology, the evaluation design used and the results obtained. Two hundred and fourteen intervention were identified. Illegal drugs were the topic on which the largest number of interventions were focused (29.8%), followed by alcohol (15.9%), the risk-related sexual behaviours (14.6%) and leisure time (12.6%). The activities carried out most often were: participation-based educational methods (30.7%), explanatory education methods (11.5%) and the preparation of educational materials (11%). In 80.8% of the cases, some evaluation of the health promotion activities was found. The type of evaluation employed most often was the process evaluation (73.7%), and the medium-long term results evaluation being those employed the least (2.2%). The evaluation methodologies used most often were questionnaires (28.2%). A combination of qualitative and quantitative methods was employed in 13.2% of the cases. The four interventions in which the medium-long term impact was gauged had the purpose of preventing and reducing cigarette smoking and/or drinking and or marijuana smoking, all of these initiatives achieving a reduction in cigarette smoking. In conclusion, it seems necessary to improve the design of the evaluations of the health promotion initiatives addressed to young people in Spain and to increase the dissemination of the same by way of their publication.

In this second article of a series of three, we will discuss using the Metaplan technique on controversial issues of health outcomes in economic evaluation of health care interventions. The four-discussion areas focus on: choice of health outcomes measures, where any outcome measure is superior to another; extrapolation and transferability of health outcomes measures, which should not be assumed the results of an EEIS of one country to another without making certain adjustments; appropriate instruments to measure quality of life in Spain, where the EQ-5D was indicated as convenient due to its widespread international use; and, indirect comparisons, where the combination of both comparisons, direct and indirect, it would be advisable if the test for indirect estimates is consistent and has been validated. Finally, research lines to try to overcome the identified discrepancies were identified in each of these areas, some of those are: doing studies of correlation between scores of specific and generic instruments measuring quality of life; update or create a database of economic evaluations in Spain; estimating utilities for the Spanish population by existing generic and specific instruments; or, establish a common way to show the results of a meta-analysis network.

La viabilidad de la estrategia de municipios saludables es un producto conjunto entre el trabajo de investigación realizado por el autor en la Maestría de Administración Pública, de la Escuela Superior de Administración Pública, y las reflexiones realizadas en el Grupo de Investigación de Rehabilitación e Integración Social de la Persona con Discapacidad, dentro del proyecto de Políticas Públicas y Municipios Saludables, de la Facultad de Rehabilitación y Desarrollo Humano de la Universidad del Rosario. Este texto expone, desde una mirada conceptual y crítica, la viabilidad de la estrategia de municipios saludables como política pública en nuestro contexto; analiza la importancia de las políticas internacionales de promoción de la salud, la reforma en dicho sector y, finalmente, muestra un estudio de caso en el municipio de Zipaquirá. La obra desarrolla los factores y elementos que hacen posible la estrategia de municipios saludables y presenta los instrumentos de recolección de la información que pueden ser teóricos e investigativos para que desde lo local se implemente la estrategia, con el objetivo de mejorar la calidad de vida de los ciudadanos y las ciudadanas.

Esta monografía busca analizar el papel del Estado colombiano con respecto a la protección de los Derechos de sus ciudadanos que han sido víctimas de la explotación sexual. En especial procura determinar los alcances y los logros de las políticas públicas colombianas encaminadas a reducir el número de niños, niñas y adolescentes víctimas de la explotación sexual como modalidad del delito de Trata de Personas durante el período 2000 – 2007. A partir de lo anterior, se trazaron como objetivos específicos: definir el delito de Trata de Personas y analizar las causas que conllevan a ser víctimas de ésta con la finalidad de comprender de qué es lo que se está hablando, y las condiciones en que se ubica la mayoría de sus víctimas. En segundo lugar intenta exponer la política pública colombiana contra el delito de Trata de Personas y especificar las instituciones que respaldan la elaboración de la misma. Igualmente busca analizar los programas implementados a partir de la política para entender qué ha hecho el Estado al respecto y con qué institucionalidad cuenta para la realización de sus proyectos. Para finalizar, busca analizar los alcances y logros de la política implementada por el gobierno colombiano. La presente investigación sobre el fenómeno de la Trata de Personas pretende mostrar que este delito no solamente afecta a las mujeres, como se ha presumido comúnmente, sino que por el contrario, hoy en día vincula diferentes sectores de la sociedad.

El tema del sistema de salud en Colombia involucra discursos jurídicos, políticos, sociales, económicos y empresariales que se enfrentan constantemente creando inestabilidad e incertidumbre en el proceder del sistema y violentando a la Ley 100 de 1993. Bajo este marco de desequilibrio se hace necesario que la entidad promotora de salud SALUD TOTAL EPS-S realice una investigación de futuros basada en la planeación estratégica por escenarios para disminuir la incertidumbre sobre la perdurabilidad de la compañía y el funcionamiento del sistema de salud entendido como un sector relevante de la economía nacional.

La Constitución Política de 1991 introdujo el concepto de participación como dato fundamental de su estructura. Un estudio ligero permite evidenciar una cantidad de artículos que regulan la materia participativa sin embargo, dichos preceptos se encuentran sin una relación de convexidad precisa. En cuanto al tema de la participación del administrado en la administración pública, como género de la especie que engloba las normas precisas sobre el ejercicio de funciones administrativas por particulares, el problema es de mayor complejidad; ello debido a que el problema participativo supone el estudio de teorías sociológicas que explican el fenómeno desde perspectivas que superan el ámbito normativo de los preceptos que consagran dicho fenómeno. El presente artículo tiene como propósito estudiar desde un punto de vista teórico el problema de la participación como fundamento del ejercicio de funciones administrativas por particulares con el objeto de explicar su fundamento último. Para este efecto, el artículo propone un panorama general de los fundamentos tradicionales del derecho administrativo y su eficacia actual, dentro del marco de una sociedad compleja en permanente conexión con el Estado Social de Derecho. A su vez expone teorías explicativas de la relación mencionada y sus efectos en la producción de normas jurídicas. Por último, expone un estudio de caso para la aplicación de los fundamentos teóricos expuestos en este artículo.
