Toxoplasma gondii is an obligate intracellular parasite with a variety of hosts, responsible for reproductive problems and economic losses in sheep flocks. Neospora caninum was recently identified and its clinical presentation in sheep is similar to that of toxoplasmosis, which can cause repeated abortions, though less frequently in this species. In order to confirm the prevalence of these agents in the city of Mossoro, Rio Grande do Norte, Brazil, 409 serum samples from adult sheep (364 females and 45 males) were tested by the indirect immunofluorescence antibody test, using cut-off point at a dilution of 1:64 and 1:50 for T. gondii and N. caninum, respectively. From the 35 properties examined, 23 (65.7%)had at least one seropositive animal for T gondii and six (17.1%) for N. caninum. The prevalence of seropositive animals for T. gondii was 20.7% and for N. caninum 1.8%. There was no association between the presence of the agent`s antibody and gender, reports of reproductive problems and presence of dogs and/or cats in the properties. T. gondii is well distributed and N. caninum has low prevalence in sheep and in the properties of the studied region. (C) 2008 Elsevier B.V. All rights reserved.

A total of 57 captive neotropical felids (one Leopardus geoffroyi, 14 Leopardus pardalis, 17 Leopardus wiedii, 22 Leopardus tigrinus, and three Puma yagouaroundi) from the Itaipu Binacional Wildlife Research Center (Refugio Bela Vista, Southern Brazil) were anesthetized for blood collection. Feces samples were available for 44 animals, including one L. geoffroyi, eight L. pardalis, 14 L. wiedii, 20 L. tigrinus, and one P. yagouaroundi. Total DNA and RNA were extracted from blood and feces, respectively, using commercial kits. Blood DNA samples were evaluated by polymerase chain reaction (PCR) for feline leukemia virus (FeLV) proviral DNA, whereas reverse transcriptase-PCR was run on fecal samples for detection of coronavirus RNA. None of the samples were positive for coronaviruses. A male L. pardalis and a female L. tigrinus were positive for FeLV proviral DNA, and identities of PCR products were confirmed by sequencing. This is the first evidence of FeLV proviral DNA in these species in Southern Brazil.

P>To evaluate the prevalence of antibodies to Chlamydophila psittaci 364 serum samples were collected from veterinarians, biologists, animal scientists, veterinary students, animal keepers and others employees in 20 zoos, and from veterinary practitioners in 10 Brazilian states. Subjects ranged from 15 to 64 years of age, with 268 (74%) males and 96 (26%) females. Chlamydial antibodies were determined by the complement fixation test (CFT) and specific anti-C. psittaci IgG antibodies were determined by the microimmunoflurescence (MIF) test. Complement fixation test showed 23.9% (87/364) and MIF test showed 4.7% (17/364) positive serum samples. Titres ranged from 16 to 256 in both assays, demonstrating evidence of recent or current infection. Although chlamydial antibodies were detected in workers of seventeen zoos, MIF test only detected specific C. psittaci antibodies in seven of them. Previous psittacosis infection was suspected in eight workers of two zoos, five of whom reported having pneumonia, while employed at the zoos. However, diagnosis was not established in any of these cases in the past. Results indicated the occurrence of infection and previous contact of Brazilian zoo workers with C. psittaci, as well as the zoonotic potential of psittacosis in this risk population. Other studies are necessary to evaluate the risk factors of infection in this population. This seroepidemiological survey confirmed the need to adopt preventive measures to control avian chlamydiosis and protect the health of zoo workers in the country.

Objective. To investigate whether poor response to controlled ovarian stimulation (COS) is due to a qualitative decline in ovarian function. Methods. This retrospective cohort study included 436 patients younger than 35-years old, undergoing COS for intracytoplasmic sperm injection (ICSI). Patients with four or fewer MII oocytes after COS (poor-responder group, PR, n = 52) were age-matched with normoresponder patients (NR, n = 364). Results. Although similar duration of stimulation (10.5 +/- 0.4 and 9.3 +/- 0.8 days; p = 0.1358), increased doses of gonadotrophins (2510 +/- 865 and 2253 +/- 572 IU; p = 0.0061) were used in the PR. The results show a increased chance of cycle ending of PR (PR: 26.9% and NR: 3.1%; p < 0.0001). Although the lower total number of oocytes retrieved (2.4 +/- 1.4 and 16.2 +/- 9.3; p < 0.0001), equal rate of fertilization (70.2% and 72.0%, p = 0.1190) and high quality embryos were obtained (50.0% and 45.2%; p = 0.4895), resulting in similar implantation (14.5% and 19.7%; p = 0.2246) and abortion (10.0% and 15.4%; p = 1.00) rates, respectively. A trend towards increased pregnancy rate per embryo transfer in NR group was noted (PR: 26.3% and NR: 42.2%; p = 0.0818). Conclusions. Low ovarian response could be associated mainly with a quantitative rather than a qualitative decline in ovarian function. Therefore, even if the ovarian response to stimulation is low, patients aged <= 35 years should process to oocyte retrieval.

Objectives: To investigate the long-term effects of pharyngeal flap surgery (PFS) on nasal and nasopharyngeal dimensions of patients with velopharyngeal insufficiency (VPI) and to correlate the findings with the onset of respiratory complaints after surgery. Design/Participants: Prospective study in 58 nonsyndromic patients with repaired cleft palate and VPI, evaluated 2 days before and 5 months (POST1) and 1 year (POST2) after PFS, on average. Patients were divided into two groups: one consisting of patients with postoperative respiratory complaints (RC group) and the other without complaints (NRC group). Interventions: Superiorly based PFS. Main Outcome Measures: Respiratory complaints (self reports of mouth breathing, snoring, and other sleep obstructive events) assessed at POST1 and POST2, and minimum nasal (NCSA) and nasopharyngeal (NPA) cross-sectional areas assessed by rhinomanometry at POST2. Results: Respiratory complaints were reported by 55% and 36% of the patients evaluated at POST1 and POST2, respectively. Posterior rhinomanometry showed a significant postoperative reduction of mean NCSA in the RC and NRC groups (p < .05), to subnormal levels in some of them. The decrease was more pronounced in the RC group. No significant changes in NCSA were observed by anterior rhinomanometry. Similar results were obtained when NPA was assessed by modified anterior rhinomanometry. Conclusion: In the long-term, PFS yielded a significant reduction in upper airways dimensions beyond what should be expected and associated with persistent respiratory complaints in some cases.

Tonic immobility (TI) is a temporary state of profound motor inhibition induced by situations that generate intense fear, with the objective of protecting an animal from attacks by predators. A preliminary study by our group demonstrated that microinjection into the basolateral nucleus of the amygdala (BLA) of an agonist to 5-HT(1A) and 5-HT(2) receptors promoted a decrease in TI duration. In the current study, the effects of GABAergic stimulation of the BLA and the possible interaction between GABA(A) and 5-HT(2) receptors on TI modulation were investigated. Observation revealed that GABAergic agonist muscimol (0.26 nmol) reduced the duration of TI episodes, while microinjection of the GABAergic antagonist bicuculline (1 nmol) increased TI duration. Additionally, microinjection of 5-HT(2) agonist receptors (alpha-methyl-5-HT, 0.32 nmol) into the BLA decreased TI duration, an effect reversed by pretreatment with bicuculline (at the dose that had no effect per se, 0.2 nmol). Moreover, the activation of GABA(A) and 5-HT(2) receptors in the BLA did not alter the spontaneous motor activity in the open field test. These experiments demonstrated that the activation of GABA(A) and 5-HT(2) receptors of the BLA possibly produce a reduction in unconditioned fear that decreases the TI duration in guinea pigs, but this is not due to increased spontaneous motor activity, which could affect a TI episode nonspecifically. Furthermore, these results suggest an interaction between GABAergic and serotoninergic mechanisms mediated by GABA(A) and 5-HT(2) receptors. O artigo rev??, com base na bibliografia especializada, as reformas e tentativas de reformas administrativas realizadas no pa??s, desde o Estado Novo, passando pelas tentativas dos governos militares e pelas propostas mais recentes da Nova Rep??blica, e chegando at?? a reforma Bresser Pereira. O balan??o cr??tico ?? bastante negativo, sobretudo, porque as grandes reformas, precisamente pela sua abrang??ncia, nunca foram efetivamente implementadas. O sistema do m??rito no servi??o p??blico federal ?? um bom exemplo. As dificuldades de implementa????o das grandes reformas, advindas principalmente de sua grande complexidade, resultam tamb??m de sua intera????o negativa com o contexto pol??tico mais geral, pela carga ret??rica de que se fazem acompanhar, e pelo fato de subestimarem os aspectos t??cnicos necess??rios ?? sua implementa????o.

Este artigo estuda os instrumentos e mecanismos de transpar??ncia e accountability das ag??ncias reguladoras brasileiras. Por meio da caracteriza????o dos processos de controle, participa????o e acesso a informa????es da Ag??ncia Nacional de Vigil??ncia Sanit??ria (Anvisa), analisa-se como as ag??ncias t??m utilizado tais instrumentos e mecanismos para acolher e processar diversos interesses do processo regulat??rio, promover a estabilidade das regras do jogo e refor??ar a sua legitimidade no ambiente pol??tico e social em que elas est??o inseridas. Foram utilizados dados relativos aos diversos instrumentos de transpar??ncia e accountability, bem como ??s inst??ncias e mecanismos de participa????o da sociedade no processo regulat??rio da Anvisa. Conclui-se que a accountability das ag??ncias ?? um contrafluxo ?? tend??ncia de insulamento, ao mesmo tempo em que se pode configurar um esfor??o de reconhecimento pela sociedade da chegada de um novo aparato institucional no Estado brasileiro: as ag??ncias reguladoras.

Chronic lead exposure induces hypertension in humans and animals, affecting endothelial function. However, studies concerning acute cardiovascular effects are lacking. We investigated the effects of acute administration of a high concentration of lead acetate (100 µΜ) on the pressor response to phenylephrine (PHE) in the tail vascular bed of male Wistar rats. Animals were anesthetized with sodium pentobarbital and heparinized. The tail artery was dissected and cannulated for drug infusion and mean perfusion pressure measurements. Endothelium and vascular smooth muscle relaxation were tested with acetylcholine (5 µg/100 µL) and sodium nitroprusside (0.1 µg/100 µL), respectively, in arteries precontracted with 0.1 µM PHE. Concentration-response curves to PHE (0.001-300 µg/100 µL) were constructed before and after perfusion for 1 h with 100 µΜ lead acetate. In the presence of endothelium (E+), lead acetate increased maximal response (Emax) (control: 364.4 ± 36, Pb2+: 480.0 ± 27 mmHg; P < 0.05) and the sensitivity (pD2; control: 1.98 ± 0.07, 2.38 ± 0.14 log mM) to PHE. In the absence of endothelium (E-) lead had no effect but increased baseline perfusion pressure (E+: 79.5 ± 2.4, E-: 118 ± 2.2 mmHg; P < 0.05). To investigate the underlying mechanisms, this protocol was repeated after treatment with 100 µM L-NAME, 10 µM indomethacin and 1 µM tempol in the presence of lead. Lead actions on Emax and pD2 were abolished in the presence of indomethacin, and partially abolished with L-NAME and tempol. Results suggest that acute lead administration affects the endothelium, releasing cyclooxygenase-derived vasoconstrictors and involving reactive oxygen species.

As adenotonsilectomias são cirurgias freqüentes na especialidade de otorrinolaringologia, e sua principal indicação é a hipertrofia das tonsilas palatinas e faríngeas. Sua complicação mais comum é a hemorragia. Em nosso trabalho apresentamos a incidência de hemorragia pós-operatória das adenotonsilectomias realizadas no Hospital Geral de Pirajussara (HGP) e Hospital Estadual de Diadema (HED), objetivando a identificação das causas de sangramento. FORMA DE ESTUDO: Coorte transversal historica. MATERIAL E MÉTODO: Foram avaliados 397 pacientes submetidos a cirurgias de adenoidectomias, tonsilectomias palatinas e adenotonsilectomias. Estas últimas representaram 91,7% do total. A idade dos pacientes variou de 2 a 39 anos. Foi realizada triagem pré-operatória através de exames laboratoriais em todos os pacientes e os mesmos foram acompanhados em retornos semanais até o primeiro mês de pós-operatório. RESULTADOS: Foram realizadas nos dois hospitais (HGP e HED) 397 cirurgias, sendo 364 adenotonsilectomias (91,7%), 16 tonsilectomias palatinas (4,03%) e 17 adenoidectomias (4,28%). Houve 5 casos de sangramento no pós-operatório, sendo 3 no período imediato e 2 no período mediato. Não ocorreu nenhum caso de hemorragia no período tardio. A incidência de hemorragia, portanto, foi de 1,37% (5 casos em 364). CONCLUSÃO: a hemorragia pós-operatória parece estar relacionada com a dificuldade técnica da cirurgia e alterações da hemostasia encontradas em alguns pacientes.

Os hospitais públicos sofrem com a demanda reprimida de indicações de cirurgias de adenoidectomia e/ou tonsilectomia, fazendo com que haja uma fila de espera crescente. O otorrinolaringologista se acostumou com as filas de espera, talvez por entender que este é um problema exclusivo do estado. Achamos de fundamental importância a realização de mutirões dessas cirurgias. OBJETIVOS: Padronizar a organização de mutirões, sua eficácia e viabilidade para os hospitais públicos e comparar a hemorragia pós-operatória nos mutirões e em cirurgias de rotina. MATERIAL E MÉTODOS: Estudo clínico-prospectivo tipo coorte. Foram realizados mutirões de adenotonsilectomias no período de setembro de 2004 a junho de 2006, no Hospital Estadual de Diadema, analisando-se a equipe multiprofissional envolvida e comparando a complicação hemorragia no pós-operatório com um grupo controle de cirurgias realizadas na rotina. RESULTADOS: Foram realizados 22 mutirões no período (339 cirurgias), uma média de 15,4 cirurgias por mutirão. O índice de hemorragia pós-operatória que necessitou de revisão foi de 1,48% (5/339), não diferindo estatisticamente do grupo controle, 1,37% (5/364). CONCLUSÃO: Conseguimos padronizar a realização de mutirões de cirurgias de adenotonsilectomias, dentro dos parâmetros que consideramos mais seguros, diminuindo a fila de espera das cirurgias. O índice de hemorragia no pós-operatório entre as cirurgias nos mutirões e na rotina não mostrou diferença estatisticamente significante.
