Glaucoma, optic neuropathy with excavation in the optic nerve head and corresponding visual field defect, is one of the leading causes for blindness worldwide. However, visual disability can often be avoided or delayed if the disease is diagnosed at an early stage. Therefore, recognising the risk factors for development and progression of glaucoma may prevent further damage. The purpose of the present study was to evaluate factors associated with visual disability caused by glaucoma and the genetic features of two risk factors, exfoliation syndrome (ES) and a positive family history of glaucoma. The present study material consisted of three study groups 1) deceased glaucoma patients from the Ekenäs practice 2) glaucoma families from the Ekenäs region and 3) population based families with and without exfoliation syndrome from Kökar Island. For the retrospective study, 106 patients with open angle glaucoma (OAG) were identified. At the last visit, 17 patients were visually impaired. Blindness induced by glaucoma was found in one or both eyes in 16 patients and in both eyes in six patients. The cumulative incidence of glaucoma caused blindness for one eye was 6% at 5 years, 9% at 10 years, and 15% at 15 years from initialising the treatment. The factors associated with blindness caused by glaucoma were an advanced stage of glaucoma at diagnosis, fluctuation in intraocular pressure during treatment, the presence of exfoliation syndrome, and poor patient compliance. A cross-sectional population based study performed in 1960-1962 on Kökar Island and the same population was followed until 2002. In total 965 subjects (530 over 50 years) have been examined at least once. The prevalence of exfoliation syndrome (ES) was 18% among subjects older than 50 years. Seventy-five of all 78 ES-positives belonged to the same extended pedigree. According to the segregation and family analysis, exfoliation syndrome seemed to be inherited as an autosomal dominant trait with reduced penetrance. The penetrance was more reduced for males, but the risk for glaucoma was higher in males than in females. To find the gene or genes associated with exfoliation syndrome, a genome wide scan was performed for 64 members (28 ES affected and 36 controls) of the Kökar pedigree. A promising result was found: the highest two-point LOD score of 3.45 (θ=0.04) in chromosome18q12.1-21.33. The presence of mutations in glaucoma genes TIGR/MYOC (myocilin) and OPTN (optineurin) was analysed in eight glaucoma families from the Ekenäs region. An inheritance pattern resembling autosomal dominant mode was detected in all these families. Primary open angle glaucoma or exfoliation glaucoma was found in 35% of 136 family members and 28% were suspected to have glaucoma. No mutations were detected in these families.

In anisometropia, the two eyes have unequal refractive power. Anisometropia is a risk factor for amblyopia. The visual deficiencies are thought to be irreversible after the first decade of life. There is, however, accumulating evidence that neural plasticity exists also in adult brains. The aim of this study was to investigate functional outcome of excimer laser refractive surgery in adult anisometropic and visually impaired patients. Additional goal was to examine changes in the primary visual cortex (V1) using multifocal functional magnetic resonance imaging (mffMRI) after laser refractive surgery. Study I comprised of 57 anisometropic patients (anisometropia of ≥3.25 diopters) and 174 isometropic myopic subjects formed the control group. A significant improvement in best-spectacle-corrected visual acuity (BSCVA) among myopic control subjects was evident 3 months postoperatively. The improvement in BSCVA was significantly slower for anisometropic patients and the improvement appeared to persist to the end of the follow-up (24 months). In study II we found that refractive surgery may be also successfully used for iathrogenic anisometropia. In Study III we evaluated mildly visually impaired adult patients after refractive surgery. There was a statistically significant improvement in BSCVA among visually impaired patients and the difference in the mean BSCVA between visually impaired patients and isometropic myopic control subjects diminished during follow-up. Study IV was a prospective follow-up trial examining the changes in the primary visual cortex after refractive surgery. Two anisometropic patients and two isometropic myopic patients were examined with a 61-region mffMRI before refractive surgery and at three, six, nine and twelve months postoperatively. In this study, a dramatic decrease in the number of active voxels in the fovea was found among anisometropic patients. The results presented in this thesis revealed that refractive surgery may be successfully used for the treatment of anisometropic adults with both congenital and iatrogenic anisometropia and for mildly visually impaired adults. The findings in conclusion strengthen our hypothesis of plastic changes in the visual cortex of adult anisometropic and mildly visually impaired patients after refractive surgery.

O estudo epidemiológico transversal randomizado objetivou avaliar condições de saúde bucal nos competidores dos XV Jogos Pan-Americanos (JPA) e III Jogos Parapan-Americanos (JPPA), 2007. Foram enviados convites para 5.662 atletas (JPA) e 1.300 (JPPA). Radiografias panorâmicas digitais (RPD) foram utilizadas para o exame de triagem nos 2 eventos, e nos JPPA, os atletas também foram submetidos à avaliação do sangramento gengival interdental (SI) através de uma versão modificada do Índice de Sangramento Interdental de Eastman (EIBI). Foram obtidas RPDs de 410 atletas dos JPA, média de idade 24,38 (dp5,35), 55% homens; e de 118 dos atletas dos JPPA, média de idade de 32,3 (dp9,53), 77,97% homens. 121 competidores (JPPA) foram avaliados para SI: 78,51% homens, média de idade 32,6(dp9,6), e foram separados em grupos (G), conforme sua deficiência física: GI c/ deficiência visual (DV), com 2 subgrupos: GI-a: DV tardia e GI-c-: DV congênita/precoce; GII- deficiência de membro superior; com 1 subgrupo: GII-t: deficiência/ausência bilateral; GIII- deficiência de membro inferior (grupo controle). As RPDs foram examinadas por 1 examinador com o Kodak Dental Imaging(v6.7). A frequência e a distribuição do SI foram calculadas, e os grupos foram comparados. Resultados da triagem com RPDs, representados por número de observações(média por atleta) JPA//número de observações(média por atleta JPPA: Dentes erupcionados/ hígidos: 9097(22,19)//2451(20,77); Ausentes: 803(1,96 //405(3,43); Não erupcionados ou impactados: 330(0,80)//52(0,44); Parcialmente erupcionados e/ou hígidos: 109(0,27)//20(0,17); Cárie extensa: 261(0,64)//62(0,53); Cárie extensa e lesão periapical: 96(0,23)//50(0,42); Tratamento endodôntico e lesão periapical: 24(0,06)//13(0,11); Restaurados: 2298(5,60)//670(5,68); Imagens radiolúcidas patológicas circunscritas: 23(0,06)//0; Raízes-residuais: 27(0,07)//22(0,19); Implantes:6(0,01)//5(0,04); Dentes anteriores fraturados: 13 (0,03)//3(0,03); Molares bandados: 26(0,06)//11(0,09); Dentes anômalos: 7(0,02)//12(0,10). Resultados para SI: G-I>G-III (p=0.0002);GI-c>GI-a (p=0,042). Homens exibiram > freqüência de SI (3,6%+1,7) que mulheres (0,8%+0,5), p<0,01. Conclusões: Os dados das 2 populações de atletas mostraram que há uma grande variação na saúde bucal entre os indivíduos avaliados. Diversas condições com potencial de influenciar o desempenho esportivo dos atletas foram detectadas através de radiografias panorâmicas digitais, sugerindo que um programa de saúde bucal deve ser incluído como parte da preparação destes indivíduos.A avaliação da frequência e distribuição de sangramento gengival interdental em uma população de atletas que competiu nos III Jogos Parapan-Americanos, revelou que o tipo de deficiência ou limitação física dos competidores é um fator que influencia na saúde gengival desses indivíduos. O planejamento de um programa de saúde bucal para esta população deve ser adaptado às diferentes limitações de cada atleta.

Este trabalho apresenta uma pesquisa sobre o ensino de Trigonometria para portadores de visão subnormal, desenvolvida com três jovens estudantes do 1 ano do Ensino Médio do Colégio Pedro II Unidade São Cristóvão - Rio de Janeiro, possuidores de diferentes tipos de deficiência visual. A intenção desta pesquisa é colaborar para uma verdadeira inclusão, onde estudantes normovisuais e deficientes visuais possam compartilhar o mesmo currículo e o mesmo ambiente de aprendizagem. Esta pesquisa traz algumas informações sobre as deficiências da visão e sobre o ensino da Trigonometria, além de apresentar o Multiplano Pedagógico, uma excelente ferramenta facilitadora da aprendizagem Matemática. Com o auxílio do Multiplano foram realizadas experiências com atividades direcionadas para o ensino de Matemática, em especial o conteúdo de Trigonometria, para esse público alvo, resgatando conteúdos inerentes ao bom acompanhamento do curso. Os jovens participantes da pesquisa participaram de forma ativa desde o direcionamento da ação até sua conclusão. Ela mostra que diversos conceitos podem ser melhor introduzidos quando auxiliados por materiais didáticos adaptados às necessidades dos educandos, porém ainda falta um bom caminho para que haja uma educação inclusiva de fato, com profissionais e escolas capacitados a atenderem de forma qualificada aos estudantes portadores de necessidades educacionais especiais.

The World Health Organization estimates that 13 million children aged 5-15 years worldwide are visually impaired from uncorrected refractive error. School vision screening programs can identify and treat or refer children with refractive error. We concentrate on the findings of various screening studies and attempt to identify key factors in the success and sustainability of such programs in the developing world. We reviewed original and review articles describing children's vision and refractive error screening programs published in English and listed in PubMed, Medline OVID, Google Scholar, and Oxford University Electronic Resources databases. Data were abstracted on study objective, design, setting, participants, and outcomes, including accuracy of screening, quality of refractive services, barriers to uptake, impact on quality of life, and cost-effectiveness of programs. Inadequately corrected refractive error is an important global cause of visual impairment in childhood. School-based vision screening carried out by teachers and other ancillary personnel may be an effective means of detecting affected children and improving their visual function with spectacles. The need for services and potential impact of school-based programs varies widely between areas, depending on prevalence of refractive error and competing conditions and rates of school attendance. Barriers to acceptance of services include the cost and quality of available refractive care and mistaken beliefs that glasses will harm children's eyes. Further research is needed in areas such as the cost-effectiveness of different screening approaches and impact of education to promote acceptance of spectacle-wear. School vision programs should be integrated into comprehensive efforts to promote healthy children and their families.

The goal of the project "SmartVision: active vision for the blind" is to develop a small and portable but intelligent and reliable system for assisting the blind and visually impaired while navigating autonomously, both outdoor and indoor. In this paper we present an overview of the prototype, design issues, and its different modules which integrate a GIS with GPS, Wi-Fi, RFID tags and computer vision. The prototype addresses global navigation by following known landmarks, local navigation with path tracking and obstacle avoidance, and object recognition. The system does not replace the white cane, but extends it beyond its reach. The user-friendly interface consists of a 4-button hand-held box, a vibration actuator in the handle of the cane, and speech synthesis. A future version may also employ active RFID tags for marking navigation landmarks, and speech recognition may complement speech synthesis.

O presente relatório descreve a atividade realizada na Biblioteca de Arte da Fundação Calouste Gulbenkian, no âmbito do Mestrado em Ciências da Informação e da Documentação, tendo por tema o acesso à informação pelos leitores com deficiência visual. Nele procura-se abordar que medidas têm sido tomadas para assegurar a integração destes leitores, tendo em vista alcançar a sua autonomia a nível de utilização dos serviços disponíveis. Nesse sentido, explicitam-se os conceitos inerentes a esta temática (biblioteca inclusiva e acessibilidade), destaca-se a importância da interação com outros utilizadores como forma de integração, refere-se a importância da cooperação interinstitucional, salienta-se o papel do bibliotecário e das tecnologias de informação e da comunicação, como meio de permitir ao deficiente visual aceder a um conjunto diversificado de serviços, através da utilização de equipamentos e software específicos. Através da aplicação de uma metodologia de pesquisa bibliográfica, observação direta e entrevista, efetua-se um estudo de caso com uma análise detalhada ao nível da acessibilidade física e dos recursos materiais assim como da atitude e preparação dos profissionais para lidar com a deficiência. Conclui-se que a Biblioteca de Arte deve continuar a investir na exploração de novos recursos e potencialidades, de modo a disponibilizar todo o tipo de meios humanos e tecnológicos para apoiar os leitores com deficiência visual, promovendo a sua inclusão e participação na sociedade da informação e do conhecimento.

ABSTRACT: Background. In India, prevalence rates of dementia and prodromal amnestic Mild Cognitive Impairment (MCI) are 3.1% and 4.3% respectively. Most Indians refer to the full spectrum of cognitive disorders simply as ‘memory loss.’ Barring prevention or cure, these conditions will rise rapidly with population aging. Evidence-based policies and practices can improve the lives of affected individuals and their caregivers, but will require timely and sustained uptake. Objectives. Framed by social cognitive theories of health behavior, this study explores the knowledge, attitudes and practices concerning cognitive impairment and related service use by older adults who screen positive for MCI, their primary caregivers, and health providers. Methods. I used the Montreal Cognitive Assessment to screen for cognitive impairment in memory camps in Mumbai. To achieve sampling diversity, I used maximum variation sampling. Ten adults aged 60+ who had no significant functional impairment but screened positive for MCI and their caregivers participated in separate focus groups. Four other such dyads and six doctors/ traditional healers completed in-depth interviews. Data were translated from Hindi or Marathi to English and analyzed in Atlas.ti using Framework Analysis. Findings. Knowledge and awareness of cognitive impairment and available resources were very low. Physicians attributed the condition to disease-induced pathology while lay persons blamed brain malfunction due to normal aging. Main attitudes were that this condition is not a disease, is not serious and/or is not treatable, and that it evokes stigma toward and among impaired persons, their families and providers. Low knowledge and poor attitudes impeded help-seeking. Conclusions. Cognitive disorders of aging will take a heavy toll on private lives and public resources in developing countries. Early detection, accurate diagnosis, systematic monitoring and quality care are needed to compress the period of morbidity and promote quality of life. Key stakeholders provide essential insights into how scientific and indigenous knowledge and sociocultural attitudes affect use and provision of resources.

La navigation repose en majeure partie sur la vision puisque ce sens nous permet de rassembler des informations spatiales de façon simultanée et de mettre à jour notre position par rapport à notre environnement. Pour plusieurs aveugles qui se fient à l’audition, le toucher, la proprioception, l’odorat et l’écholocation pour naviguer, sortir à l’extérieur de chez soi peut représenter un défi considérable. Les recherches sur le circuit neuronal de la navigation chez cette population en particulier s’avèrent donc primordiales pour mieux adapter les ressources aux handicapés visuels et réussir à les sortir de leur isolement. Les aveugles de naissance constituent aussi une population d’intérêt pour l’étude de la neuroplasticité. Comme leur cerveau s’est construit en absence d’intrant visuel, la plupart des structures reliées au sens de la vue sont réduites en volume par rapport à ceux de sujets voyants. De plus, leur cortex occipital, une région normalement dédiée à la vision, possède une activité supramétabolique au repos, ce qui peut représenter un territoire vierge pouvant être recruté par les autres modalités pour exécuter diverses tâches sensorielles. Plusieurs chercheurs ont déjà démontré l’implication de cette région dans des tâches sensorielles comme la discrimination tactile et la localisation auditive. D’autres changements plastiques de nature intramodale ont aussi été observés dans le circuit neuronal de la navigation chez ces aveugles. Par exemple, la partie postérieure de l’hippocampe, impliquée dans l’utilisation de cartes mentales, est réduite en volume alors que la section antérieure est élargie chez ces sujets. Bien que ces changements plastiques anatomiques aient bel et bien été observés chez les aveugles de naissance, il reste toutefois à les relier avec leur aspect fonctionnel. Le but de la présente étude était d’investiguer les corrélats neuronaux de la navigation chez l’aveugle de naissance tout en les reliant avec leurs habiletés spatio-cognitives. La première étude comportementale a permis d’identifier chez les aveugles congénitaux une difficulté d’apprentissage de routes tactiles construites dans des labyrinthes de petite échelle. La seconde étude, employant la technique d’imagerie par résonance magnétique fonctionnelle, a relié ces faiblesses au recrutement de régions cérébrales impliquées dans le traitement d’une perspective égocentrique, comme le lobule pariétal supérieur droit. Alors que des sujets voyants aux yeux bandés excellaient dans la tâche des labyrinthes, ces derniers recrutaient des structures impliquées dans un traitement allocentrique, comme l’hippocampe et le parahippocampe. Par ailleurs, la deuxième étude a confirmé le recrutement du cortex occipital dans une tâche de navigation chez les aveugles seulement. Ceci confirme l’implication de la plasticité intermodale dans des tâches cognitives de plus haut niveau, comme la navigation.
TactoColor : conception et évaluation d’une interface d’exploration spatiale du web pour malvoyants.

Nous nous intéressons, dans le cadre de cette recherche, à l’accès à l’internet des personnes malvoyantes. Plusieurs types d’outils destinés à ce public sont disponibles sur le marché, comme les lecteurs et les agrandisseurs d’écran, en fonction de l’acuité visuelle de la personne. Bien que ces outils soient utiles et régulièrement utilisés, les malvoyants (ainsi que les aveugles) évoquent souvent leur aspect frustrant. Plusieurs raisons sont citées, comme le manque d’organisation spatiale du contenu lu avec les lecteurs d’écran ou le fait de ne solliciter qu’un seul sens. La présente recherche consiste à adapter pour les malvoyants un système en développement le TactoWeb (Petit, 2013) qui permet une exploration audio-tactile du Web. TactoWeb a été conçu pour les handicapés ayant une cécité complète et n’offre donc aucune propriété visuelle. Nous proposons ici une adaptation du système pour les handicapés n’ayant qu’une déficience visuelle partielle. Nous espérons fournir à cette population des outils performants qui leur permettront de naviguer sur l’internet de façon efficace et agréable. En effet, grâce à une exploration non-linéaire (qui devrait améliorer l’orientation spatiale) et une interface multimodale (qui sollicite la vue, l’ouïe et le toucher), nous pensons réduire fortement le sentiment de frustration qu’évoquent les malvoyants. Nous avons posé l’hypothèse qu’une exploration non-linéaire et trimodale d’un site internet avec TactoColor est plus satisfaisante et efficace qu’une exploration non-linéaire bimodale avec TactoWeb (sans retour visuel). TactoColor a été adapté pour les malvoyants en ajoutant des indices visuels traduisant les composantes de la page (liens, menus, boutons) qui devraient rendre l’exploration plus aisée. Pour vérifier notre hypothèse, les deux versions du logiciel ont été évaluées par des malvoyants. Ainsi, les participants ont commencé soit avec TactoWeb, soit avec TactoColor afin de ne pas favoriser une des versions. La qualité de la navigation, son efficacité et son efficience ont été analysées en se basant sur le temps nécessaire à l’accomplissement d’une tâche, ainsi que la facilité ou la difficulté évoquée par le participant. Aussi, à la fin de chaque session, nous avons demandé leur avis aux participants, grâce à un questionnaire d’évaluation, ce qui nous a permis d’avoir leur retour sur notre logiciel après leur brève expérience. Tous ces relevés nous ont permis de déterminer que l’ajout des couleurs entraine une exploration plus rapide des pages web et une meilleure orientation spatiale. Par contre les performances très différentes des participants ne permettent pas de dire si la présence des couleurs facilite la complétion des tâches.

A presente dissertação de mestrado, centra-se no desenvolvimento teórico de uma ideia de projecto de investigação, relacionada com o conceito de Ambiente Pessoal de Aprendizagem (APA) que em conexão com as tecnologias digitais, aplicadas nos contextos educativos actuais, providencia maiores possibilidades integração comunicacional e social, em crianças com deficiência visual. Grande ênfase de parte conceptual do estudo, assenta na ideia de Promoção de Espaços Inclusivos de Cooperação Comunicativa em Cegueira Infantil (PEICC-CI). Um dos objectivos do projecto, será o desenvolvimento de um espaço inclusivo de comunicação em rede que possa promover uma maior integração entre ensino formal e informal. Esta ideia de projecto, integra uma metodologia qualitativa e descritiva, a aplicar nas interacções sociais de um pequeno grupo de alunos participantes, que serão estudados de acordo com as suas capacidades comunicativas durante o plano de estudo. A ideia de projecto apresentada, tentará defender a importância de criação de sistemas tecnológicos mais inclusivos e dinâmicos, com base na aprendizagem informal - que possam proporcionar a crianças cegas, uma igual participação democrática de comunicação e convivência social em meios escolares, dentro das novas sociedades de conhecimento e informação.

This dissertation examines the relationship between frequency response and word-discrimination performance of hearing-impaired persons. Three questions are addressed: does the restoration of the normal field-to-eardrum transfer function improve word discrimination; is the restoration of the normal shape of the audibility curve (uniform hearing level at all frequencies) beneficial to hearing-impaired listeners; and can speech discrimination be improved by an extension of the present narrow-band response in hearing aids.
