Background and Purpose. A new method of dynamometry has been developed to measure the performance of the craniocervical (CC) flexor muscles by recording the torque that these muscles exert on the cranium around the CC junction. This report describes the method, the specifications of the instrument, and the preliminary reliability data. Subjects and Methods. For the reliability study, 20 subjects (12 subjects with a history of neck pain, 8 subjects without a history of neck pain) performed, on 2 occasions, maximal voluntary isometric contraction (MVIC) tests of CC flexion in 3 positions within the range of CC flexion and submaximal sustained tests (20% and 50% of MVIC) in the middle range of CC flexion (craniocervical neutral position). Reliability coefficients were calculated to establish the test-retest reliability of the measurements. Results. The method demonstrated good reliability over 2 sessions in the measurement of MVIC (intraclass correlation coefficient [ICC] =.79-.93, SEM=0.6-1.4 N-m) and in the measurement of steadiness (standard deviation of torque amplitude) of a sustained contraction at 20% of NMC (ICC=.74-.80, SEM=0.01 N-m), but not at 50% of MVIC (ICC=.07-.76, SEM=0.04-0.13 N-m). Discussion and Conclusion. The new dynamometry method appears to have potential clinical application in the measurement of craniocervical flexor muscle performance.

Predictors of outcome following whiplash injury are limited to socio-demographic and symptomatic factors, which are not readily amenable to secondary and tertiary intervention. This prospective study investigated the predictive capacity of early measures of physical and psychological impairment on pain and disability 6 months following whiplash injury. Motor function (ROM; kinaesthetic sense; activity of the superficial neck flexors (EMG) during cranio-cervical flexion), quantitative sensory testing (pressure, thermal pain thresholds, brachial plexus provocation test), sympathetic vasoconstrictor responses and psychological distress (GHQ-28, TSK, IES) were measured in 76 acute whiplash participants. The outcome measure was Neck Disability Index scores at 6 months. Stepwise regression analysis was used to predict the final NDI score. Logistic regression analyses predicted membership to one of the three groups based on final NDI scores (< 8 recovered, 10-28 mild pain and disability, > 30 moderate/severe pain and disability). Higher initial NDI score (1.007-1.12), older age (1.03-1.23), cold hyperalgesia (1.05-1.58), and acute post-traumatic stress (1.03-1.2) predicted membership to the moderate/severe group. Additional variables associated with higher NDI scores at 6 months on stepwise regression analysis were: ROM loss and diminished sympathetic reactivity. Higher initial NDI score (1.03-1.28), greater psychological distress (GHQ-28) (1.04-1.28) and decreased ROM (1.03-1.25) predicted subjects with persistent milder symptoms from those who fully recovered. These results demonstrate that both physical and psychological factors play a role in recovery or non-recovery from whiplash injury. A recent randomized controlled trial tested the effectiveness of therapeutic exercise and manipulative therapy on 200 subjects with cervicogenic headache. Although treatments were efficacious, 25% of patients did not achieve a clinically acceptable outcome - 50% reduction in headache frequency This study aimed to identify predictors from variables in subjects' demographics and headache history which might identify those who did or did not achieve a 50-79% or 80-100% reduction in headache immediately after the active treatments and 12 months postintervention. The results revealed no consistent pattern of predictors, although the absence of light-headedness indicated higher odds of achieving either a 50-79% [odds ratio (OR) = 5.45) or 80-100% (OR = 5.7) reduction in headache frequency in the long term. Headaches of at least moderate intensity, the patient's age and chronicity of headache did not mitigate against a successful outcome from physiotherapy intervention.

Cervical joint position error (JPE) has been used as a measure of cervical afferent input to detect disturbances in sensori-motor control as a possible contributor to a neck pain syndrome. This study aimed to investigate the relationship between cervical JPE, balance and eye movement control. It was of particular interest whether assessment of cervical ME alone was sufficient to signal the presence of disturbances in the two other tests. One hundred subjects with persistent whiplash-associated disorders (WADs) and 40 healthy controls subjects were assessed on measures of cervical JPE, standing balance and the smooth pursuit neck torsion test (SPNT). The results indicated that over all subjects, significant but weak-to-moderate correlations existed between all comfortable stance balance tests and both the SPNT and rotation cervical ME tests. A weak correlation was found between the SPNT and right rotation cervical JPE. An abnormal rotation cervical JPE score had a high positive prediction value (88%) but low sensitivity (60%) and specificity (54%) to determine abnormality in balance and or SPNT test. The results suggest that in patients with persistent WAD, it is not sufficient to measure ME alone. All three measures are required to identify disturbances in the postural control system. Persistent intermittent headache is a common disorder and is often accompanied by neck aching or stiffness, which could infer a cervical contribution to headache. However, the incidence of cervicogenic headache is estimated to be 14-18% of all chronic headaches, highlighting the need for clear criterion of cervical musculoskeletal impairment to identify cervicogenic headache sufferers who may benefit from treatments such as manual therapy. This study examined the presence of cervical musculoskeletal impairment in 77 subjects, 27 with cervicogenic headache, 25 with migraine with aura and 25 control subjects. Assessments included a photographic measure of posture, range of movement, cervical manual examination, pressure pain thresholds, muscle length, performance in the cranio-cervical flexion test and cervical kinaesthetic sense. The results indicated that when compared to the migraine with aura and control groups who scored similarly in the tests, the cervicogenic headache group had less range of cervical flexion/extension (P = 0.048) and significantly higher incidences of painful upper cervical joint dysfunction assessed by manual examination (all P < 0.05) and muscle tightness (P < 0.05). Sternocleidomastoid normalized EMG values were higher in the latter three stages of the cranio-cervical flexion test although they failed to reach significance. There were no between group differences for other measures. A discriminant analysis revealed that manual examination could discriminate the cervicogenic headache group from the other subjects (migraine with aura and control subjects combined) with an 80% sensitivity. The purpose of this presentation is to pay tribute to the life's work of Professor Vladimir Janda, a key figure in the 20th Century rehabilitation movement. An accomplished neurologist, he founded the rehabilitation department at Charles University Hospital in Prague, Czechoslovakia. He was one of the seminal members of the Prague school of manual medicine and rehabilitation that expanded its influence throughout Central and Eastern Europe. His observations regarding muscle imbalances, faulty posture and gait, and their association with chronic pain syndromes, etiologically, diagnostically, and therapeutically, influenced the rehabilitation world. The authors comprise a multinational, multiprofessional group representative of rehabilitation specialists around the world who would like to pay tribute and give a final word of thanks to this innovative educator, clinician, and author.

Accurate measurement of intervertebral kinematics of the cervical spine can support the diagnosis of widespread diseases related to neck pain, such as chronic whiplash dysfunction, arthritis, and segmental degeneration. The natural inaccessibility of the spine, its complex anatomy, and the small range of motion only permit concise measurement in vivo. Low dose X-ray fluoroscopy allows time-continuous screening of cervical spine during patient's spontaneous motion. To obtain accurate motion measurements, each vertebra was tracked by means of image processing along a sequence of radiographic images. To obtain a time-continuous representation of motion and to reduce noise in the experimental data, smoothing spline interpolation was used. Estimation of intervertebral motion for cervical segments was obtained by processing patient's fluoroscopic sequence; intervertebral angle and displacement and the instantaneous centre of rotation were computed. The RMS value of fitting errors resulted in about 0.2 degree for rotation and 0.2 mm for displacements. © 2013 Paolo Bifulco et al.

As escolas sofreram várias mudanças, ao longo dos anos, as quais geraram consequências a nível das condições de trabalho dos professores. Assim, os professores trabalham a um ritmo intenso, com uma enorme sobrecarga de trabalho, o que os leva a queixarem-se de dores de garganta, dores de pernas, dores de costas, rouquidão e cansaço. Enfrentam vários riscos psicossociais e profissionais, na sua profissão, e estes agrupam-se em seis dimensões, de acordo com Gollac & Bodier (2011), a saber: a intensificação de trabalho e tempo de trabalho, exigências emocionais, falta de autonomia, qualidade das relações sociais, conflito de valores e insegurança no trabalho. Esta investigação teve como objetivo a identificação dos riscos psicossociais e profissionais dos professores do 3.ºciclo e do ensino secundário, nela participando 89 professores. A identificação dos riscos psicossociais realizou-se através do INSAT e a entrevista aos docentes permitiu completar e comprovar estes mesmos dados. Esta investigação pretende alertar sobre as condições de trabalho dos professores e as consequências das mesmas para a saúde e bem-estar no trabalho.

Enquadramento: A prevalência da dor cervical crónica em adolescentes está a aumentar. Estudos recentes têm comprovado a eficácia de programas de educação com base na neurofisiologia da dor e exercício na diminuição da dor, incapacidade, medo e ansiedade associados à dor crónica. Contudo, apesar deste tipo de programas apresentar resultados promissores em adultos, a sua aplicação e efetividade em populações mais jovens tem sido pouco estudada. Objetivos: Avaliar a efetividade de um programa de educação com base na neurofisiologia da dor associado a exercícios na dor cervical crónica em adolescentes na 1) frequência e intensidade da dor, 2) incapacidade associada, 3) capacidade de resistência dos músculos flexores e extensores profundos da cervical e estabilizadores da omoplata, 4) ansiedade e 5) catastrofização. Métodos: Um total de 43 adolescentes com idade entre os 15 e os 18 anos da Escola Secundária Dr. João Carlos Celestino Gomes participaram neste estudo. Foram avaliadas a intensidade, duração e frequência da dor cervical, a incapacidade associada e a resistência dos músculos flexores e extensores profundos da cervical e estabilizadores da omoplata através dos testes dos flexores e extensores profundos e estabilizadores da omoplata, respetivamente. Foram também avaliados os níveis de ansiedade, catastrofização e perceção de mudança através do Inventário de Ansiedade Estado-Traço, da Escala de Catastrofização da Dor e da Escala de Perceção Global de Mudança. Resultados: O número de participantes a referir dor na semana que precedeu a avaliação no grupo experimental reduziu em 28,5%. Verificou-se uma interação significativa entre o momento de avaliação (antes da intervenção vs após a intervenção) e o grupo (experimental vs. controlo) para as variáveis resistência dos flexores profundos e catastrofização e um efeito do momento e do grupo (mas não uma interação) para a dor, incapacidade, resistência dos músculos extensores e estabilizadores da omoplata e ansiedade traço (p<0.05). Dos 21 participantes do grupo experimental, 85,7% referiu mudanças significativas na Escala de Perceção Global de Mudança. Conclusão: A educação em neurofisiologia da dor é uma intervenção que poderá ser utilizada em adolescentes com dor crónica, com resultados significativos na redução da dor, melhoria da resistência muscular dos músculos flexores e extensores profundos da cervical e estabilizadores da omoplata e diminuição da catastrofização.

Introducción. Los Trastornos Musculoesqueléticos (TME) son lesiones de músculos, tendones, nervios y articulaciones que tienen como principal síntoma el dolor y se presentan con gran frecuencia en trabajos que requieren una actividad física importante o como consecuencia de malas posturas sostenidas durante largos periodos de tiempo. Objetivo: Determinar la prevalencia de síntomas osteomusculares y su asociación con factores ergonómicos en una población de trabajadores administrativos de una empresa de servicios en Bogotá, Colombia, en el año 2015. Método: Se realizó un estudio de corte transversal. Se analizaron datos secundarios correspondientes a una base de datos de una población de 450 trabajadores pertenecientes al área administrativa de una empresa de servicios, de los cuales se incluyeron 150 registros que tenían la información completa de las variables del estudio. Se tuvieron en cuenta variables sociodemográficas, ocupacionales y la presencia de síntomas osteomusculares. Incluyó también el registro de condiciones de la estación de trabajo recolectada mediante inspecciones de puestos de trabajo (registro fotográfico y formato estandarizado para condiciones observadas). Se realizó un análisis descriptivo considerando las medidas de tendencia central para las variables cuantitativas y distribución de frecuencias para las variables cualitativas. Resultados: Predominó el género femenino (56%) dentro de la población estudiada, con una edad media para ambos géneros de 35 años (75%), (DS ± 7.0). El síntoma más frecuentemente reportado fue el dolor en cuello (17%) seguido por dolor en muñeca (11%) y dolor lumbar (8.7%). Del total de los trabajadores evaluados, el 80% correspondió al cargo ejecutivo, que reúne funciones administrativas especializadas complejas y/o de supervisión directa. De la población femenina objeto de estudio el 76% se desempeñaban en el cargo ejecutivo. Las condiciones ergonómicas por mejorar de mayor frecuencia fueron altura y distancia de la pantalla (42%) y cables sueltos (30%). La frecuencia de síntomas fue más alta en el rango de edad de 31 a 45 años. Conclusiones: En este estudio se encontró que los síntomas osteomusculares más frecuentes se ubicaron en el segmento axial (cuello y región lumbar) y en mano y que las condiciones de altura y distancia de la pantalla y cables sueltos presentaron las frecuencias más altas de condiciones por mejorar. Se hace necesaria la implementación de programas de prevención de TME, la vigilancia de casos y la intervención de las condiciones de los puestos de trabajo. Se debe promover la identificación y promoción de conductas saludables dentro del ámbito laboral y la prevención de condiciones que favorezcan la aparición del riesgo biomecánico.

Se realizó un estudio observacional retrospectivo longitudinal en una Institución prestadora de Servicios de Salud de la ciudad de Bogotá, con el objetivo de evaluar la efectividad en el manejo del dolor de la terapia con acupuntura en el tratamiento de lumbalgia. Se tomaron 150 historias clínicas de pacientes con lumbalgia atendidos de enero de 2014 a mayo de 2016, las cuales fueron sometidas a los criterios de inclusión definidos por los autores, arrojando 48 historias sometidas a la prueba de Friedman con el fin de identificar el impacto sobre el dolor del tratamiento con acupuntura en los pacientes seleccionados. Adicionalmente, bajo un muestreo aleatorio simple de distribución normal sobre las 48 historias clínicas evaluadas, se seleccionaron 25 casos a los cuales se les aplicó una encuesta no estructurada, con el fin de obtener información sobre el estado de la patología después de finalizar el tratamiento e identificar las posibles causas de deserción. Con este estudio se concluye que la terapia con acupuntura es efectiva en el manejo del dolor de pacientes con lumbalgia, y que es necesario realizar más estudios que puedan sustentar la inclusión de la terapéutica en el manejo de esta patología.

Introducción: El trabajador avícola presenta un alto riesgo de sufrir de Desórdenes Musculo esqueléticos, debido a la realización de trabajos manuales repetitivos; posición bípeda prolongada, posturas por fuera de ángulos de confort de miembros superiores Objetivo: Establecer las recomendaciones basadas en la evidencia de las intervenciones en salud para los Desórdenes Musculoesqueléticos (DME) en el trabajador avícola. Metodología: Se realizó una revisión de la literatura de los estudios primarios publicados en las bases de datos Medline, Scient Direct y Scielo desde 1990. Los artículos se clasificaron de acuerdo con: el tipo de estudio, la calidad de éste y el nivel de evidencia que aportaba. Resultados: Dentro de las recomendaciones de la evidencia disponible para el manejo integral de los pacientes de la industria avícola con riesgos o eventos asociados a DME se encuentran las siguientes: 1) incorporar un enfoque sistémico en la atención a dichos trabajadores, 2) incluir aspectos psicosociales en la identificación y explicación de los riesgos y eventos en salud, 3) permitir los descansos, microrupturas y pautas para el ejercicio, 4) facilitar la rotación y ampliación de puestos de trabajo, 5) mejorar las herramientas de trabajo - especialmente el corte de los cuchillos. Conclusiones: Las intervenciones descritas en la presente revisión, apuntan hacia el mejoramiento de la incidencia y la prevalencia de los DMS, la disminución de incapacidad temporal y definitiva por los DMS, el mejoramiento en la producción industrial y la reducción de costos tanto económicos como humanos. Sin embargo, se debe plantear la necesidad de continuar impulsando el desarrollo de investigaciones y estudios que permitan tener mayores elementos de juicio para poder realizar recomendaciones a los tipos de intervenciones propuestas. A pesar de lo anterior, las intervenciones en salud para los trabajadores de la industria avícola deben ser enfocadas desde la prestación integral de los servicios de salud.
