Weight loss normally stimulates hunger, through mechanisms that include falls in circulating leptin and insulin, leading to stimulation of hypothalamic neuropeptide Y (NPY). Here, we investigated the leptin, insulin and NPY to clarify why hunger is suppressed in mice with severe cachexia due to the MAC16 adenocarcinoma. MAC16-bearing mice progressively lost weight (19% below controls) and fat (-61%) over 16 days after tumour transplantation, while total food intake fell by 10%. Pair-fed mice showed less wasting, with final weight being 9% and fat mass 25% below controls. Plasma leptin fell by 85% in MAC16 and 51% in pair-fed mice, in proportion to loss of fat. Plasma insulin was also reduced by 49% in MAC16 and 53% in pair-fed groups. Hypothalamic leptin receptor (OB-Rb) mRNA was significantly increased in both MAC16 (+223%) and pair-fed (+192%) mice. Hypothalamic NPY mRNA was also significantly raised in MAC16 (+152%) and pair-fed (+99%) groups, showing negative correlations with plasma leptin and insulin, and a positive association with OB-Rb mRNA. In MAC16-induced cachexia, leptin production and hypothalamic OB-Rb and NPY expression are regulated appropriately in response to fat depletion. Therefore, suppression of hunger is probably due to tumour products that inhibit NPY transport or release, or that interfere with neuronal targets downstream of NPY.

Sin dal suo ingresso nel mercato della traduzione, la machine translation (MT) è stata impiegata in numerosi settori professionali e ha suscitato l’interesse del grande pubblico. Con l’avvento della traduzione automatica neurale, la MT ha avuto un ulteriore impulso e ci si è iniziati a interrogare sulla possibilità di estenderne il raggio d’azione all’ambito letterario, che sembra tuttora non toccato dagli automatismi delle macchine in virtù della funzione espressiva e del contenuto creativo dei testi. Il presente elaborato nasce quindi con l’obiettivo di valutare l’effettiva applicabilità della traduzione automatica in ambito letterario attraverso un’attenta analisi dell’output prodotto dalla MT supportata da un confronto con la traduzione svolta da un professionista. Questa analisi si baserà sulla traduzione di un estratto del romanzo Hunger Games di Suzanne Collins e le traduzioni verranno effettuate da due dei migliori software di traduzione automatica neurale attualmente sul mercato: ModernMT e DeepL. Una volta terminata la valutazione dei singoli output verranno mostrati i risultati e determinati i limiti e i vantaggi della machine translation.

This article analyzes food insecurity and hunger in Brazilian families with children under five years of age. This was a nationally representative cross-sectional study using data from the National Demographic and Health Survey on Women and Children (PNDS-2006), in which the outcome variable was moderate to severe food insecurity, measured by the Brazilian Food Insecurity Scale (EBIA). Prevalence estimates and prevalence ratios were generated with 95% confidence intervals. The results showed a high prevalence of moderate to severe food insecurity, concentrated in the North and Northeast regions (30.7%), in economic classes D and E (34%), and in beneficiaries of conditional cash transfer programs (36.5%). Multivariate analysis showed that the socioeconomic relative risks (beneficiaries of conditional cash transfers), regional relative risks (North and Northeast regions), and economic relative risks (classes D and E) were 1.8, 2.0 and 2.4, respectively. Aggregation of the three risks showed 48% of families with moderate to severe food insecurity, meaning that adults and children were going hungry during the three months preceding the survey.

Prevalence of severe food insecurity was estimated for Brazilian municipalities based on the 2004 National Household Sample Survey (PNAD). First, a logistic regression model was developed and tested with this database. The model was then applied to the 2000 census data, generating severe food insecurity estimates for the Brazilian municipalities, which were subsequently analyzed according to the proportion of families exposed to severe food insecurity. Severe food insecurity was mainly concentrated in the North and Northeast regions, where 46.1% and 65.3% of municipalities showed high prevalence of severe food insecurity, respectively. Most municipalities in the Central West region showed intermediate prevalence of severe food insecurity. There was wide intra-regional variation in severe food insecurity, while the South of Brazil showed the most uniform distribution. In conclusion, Brazil displays wide inter and intra-regional variations in the occurrence of severe food insecurity. Such variations should be identified and analyzed in order to plan appropriate public policies.

OBJETIVO: analisar a insegurança alimentar e o vínculo inadequado mãe-filho como dois potenciais determinantes da desnutrição em crianças de quatro a seis anos de idade. MÉTODOS: estudo de caso-controle desenvolvido em Escolas Municipais de Educação Infantil (EMEIs) no Jardim Jaqueline, área de alta vulnerabilidade social do município de São Paulo, Brasil. Foram aplicados a Escala Brasileira de Insegurança Alimentar e o Protocolo de Avaliação do Vínculo Mãe-filho, além de coletadas informações biológicas e socio-econômicas. Para verificação dos efeitos de cada variável independente e controle dos efeitos das demais variáveis incluídas no modelo, foi utilizado o modelo de regressão logística múltipla. RESULTADOS: verificou-se que tanto a insegurança alimentar familiar (OR=3,6) como o vínculo inadequado mãe-filho (OR=9,4) estiveram associados com a desnutrição infantil (p<0,05), mesmo após o controle para o peso ao nascimento da criança e idade, estado conjugal e trabalho maternos. CONCLUSÕES: tanto a insegurança alimentar familiar (OR=3,6) como o vínculo mãe-filho inadequado (OR=9,4) mostraram-se fatores determinantes da ocorrência da desnutrição na população estudada.

Estimou-se a prevalência de insegurança alimentar grave para os municípios brasileiros, com base na Pesquisa Nacional por Amostra de Domicílios (PNAD) 2004. Inicialmente, foi gerado e testado um modelo por regressão logística multivariada com base nesse banco de dados. O modelo foi aplicado à amostra do Censo Demográfico de 2000, gerando estimativas de prevalências de insegurança alimentar grave para os municípios brasileiros, que foram analisadas de acordo com o percentual de famílias em condição de insegurança alimentar grave. Essa insegurança alimentar está mais concentrada nas regiões Norte e Nordeste, onde 46,1 por cento e 65,3 por cento dos municípios, respectivamente, apresentam altas prevalências de insegurança alimentar grave. Predominam nas regiões Sudeste e Sul municípios com baixa exposição à insegurança alimentar grave. No Centro-oeste a maior parte dos municípios mostra estimativas de insegurança alimentar grave classificadas como médias. Verificou-se grande variação intra-regional na ocorrência da insegurança alimentar, sendo a Região Sul a mais uniforme. Conclui-se que o Brasil apresenta grandes variações inter e intra-regionais na ocorrência da insegurança alimentar, sendo importante realçá-las e analisá-las, no intuito de subsidiar políticas públicas

Objective. To study the AIDS epidemic in Brazil`s border areas, from the spatial and temporal perspective. Methods. This was an ecological study in which the cases of AIDS reported to the Ministry of Health of Brazil from 1990-2003 were grouped according to ""hunger areas"" as defined by Josue de Castro in the 1940s and according to 19 cultural subregions. Spatial assessment was based on incidence rates for border municipalities; temporal assessment considered the absolute number of cases occurring quarterly from 1990-2003 in each of the hunger areas studied (Extreme South, Midwest, and Amazon). Results. During the study period, 7 973 cases of AIDS were reported from the Brazilian border areas: 648 in the Amazon area, 1 579 in the Midwest, and 5 746 in the Extreme South (populations of 668 098, 895 489, and 2 769 361, respectively). The subregions with the highest AIDS incidence rates in each of the three border areas were those near triple-borders, between more than two Latin American countries. Sexual transmission was predominant, with heterosexual transmission being the most frequent, followed by transmission by male homosexuality. These two categories accounted for 87.2% of the cases reported. The estimates of the trend parameter in the temporal analysis were 0.53 (P < 0.0001), 0.83 (P < 0.0001), and 3.47 (P < 0.0001), respectively, for the Amazon, Midwest, and Extreme South areas. Conclusion. The improvement of health care services along Brazil`s borders may be a strategy for territorial integration and for dealing with the AIDS epidemic, as long as social, economic, and cultural differences are taken into account.

Background: Alterations in gastrointestinal tract physiology after gastrectomy may affect appetite and energy balance. Objective: The objective of this study was to examine energy balance, appetite, and gastrointestinal transit in subjects with gastrectomy. Design: Seven subjects with total gastrectomy (TG) and 14 subjects with partial gastrectomy (PG), who were free from signs of recurrent disease, and 10 healthy control subjects were studied. Resting energy expenditure (REE) was measured by indirect calorimetry and compared with REE predicted by the Harris-Benedict equation (mREE/pREE%). Gastrointestinal transit was measured by scintigraphy. Habitual food intake was assessed, and appetite was measured during scintigraphy after ingestion of a test meal (361 kcal). Results: Body mass index was not different among the groups. mREE/pREE% was higher in patients with PG (P < 0.01) than in control subjects. The TG group showed higher energy intake (P < 0.05) than the PG group and control subjects. Gastric emptying was faster in the PG group than in control subjects, and gastrointestinal transit was accelerated in both PG and TG groups. An intense, precocious postprandial fullness and a relatively early recovery of hunger and prospective consumption sensations were seen in these patients. Conclusions: Patients with PG or TG have higher than predicted energy expenditure, which in TG seems to be compensated for by increased energy intake. These patients have preserved postprandial appetite responses and precocious postprandial fullness, which seem to be associated with disturbances in gastrointestinal transit of the ingested meal and are likely to be independent of vagal fiber integrity or stomach-released ghrelin.
