Em 1992, durante um ensaio com mosquiteiros impregnados com deltametrina, conduzido em Porto Murtinho, município de Costa Marques, Rondônia, na Amazônia Brasileira, analisou-se a aderência e os fatores econômicos capazes de reduzir o impacto do uso de mosquiteiros impregnados sobre a morbidade da malária. Os mosquiteiros impregnados tiveram excelente receptividade pela população local. No entanto, a aderência foi muito baixa, principalmente pelos adultos maiores de 15 anos de idade e, durante a estiagem. O preço do mosquiteiro impregnado foi calculado em US$18,83 para casal e, em US$13,82, para solteiro. Apesar das vantagens econômicas do emprego dos mosquiteiros impregnados em larga escala, desde que subsidiados pelas agências governamentais, a baixa aderência verificada, aliada aos fatores epidemiológicos locais, não permitem indicar o uso de mosquiteiros impregnados como medida de proteção em massa na Amazônia.

Em resposta a denúncias de triatomíneos em ilhas do Alto Rio Paraná foram investigados 145 ecótopos artificiais e 4 (2,8%) deles encontravam-se infestados: residência, "clube", ex-escola e monte de madeira. Foram analisados 17 de 35 P. megistus coletados; 12 (70,6%) apresentavam-se infectados por Trypanosoma cruzi. Ave e roedor constituíram as fontes alimentares mais comuns. Todos os exames sorológicos (56 de humanos, 18 de cães e 10 de gatos) foram negativos.

No período de novembro de 2000 a novembro de 2001, foi realizado o estudo dos flebotomíneos nos municípios de Alto Caparaó e Caparaó, com o objetivo de determinar a sua variação sazonal, comparar os pontos de estudo quanto à ocorrência destes insetos e determinar os ambientes de prevalência destes. Foram realizadas coletas mensais, com armadilhas luminosas de Falcão em nove pontos, sendo quatro armadilhas por ponto, nos seguintes ambientes: abrigo de animais, mata, cafezal e parede externa das casas. Lutzomyia intermedia (Lutz & Neiva, 1912) foi a espécie predominante, e o abrigo de animais o ambiente com maior ocorrência de flebotomíneos. Encontrou-se diferença significativa na composição de espécies de flebotomíneos nos pontos estudados. Lutzomyia intermedia é a espécie suspeita de transmitir a Leishmania na região.

Foram analisados os achados epidemiológicos, clínicos, laboratoriais e terapêuticos de 23 casos de síndrome cardiopulmonar por hantavírus, identificados sorologicamente ou por imunohistoquímica em hospitais do município de Uberlândia, Minas Gerais. Febre (100%), dispnéia (100%) e mialgias (78%) foram os sintomas mais frequentemente observados nesta casuística. Os sinais físicos mais prevalentes foram hipotensão (65%) e taquicardia (65%). Achados laboratoriais mais comuns incluíram trombocitopenia (96%), hemoconcentração (83%) e leucocitose (74%). Valores anormais de enzimas hepáticas foram encontrados em todos os pacientes testados e alterações em radiografias de tórax foram muito (95,6%) freqüentes. Em 55,5% dos pacientes, foi necessário intubação orotraqueal e suporte hemodinâmico. O presente estudo confirmou o padrão sazonal da síndrome cardiopulmonar por hantavírus na região de Uberlândia e o envolvimento, no ciclo de transmissão da doença, de grupos profissionais considerados de baixo risco de infecção. A alta (39%) taxa de letalidade e a gravidade da doença observadas neste estudo podem estar associadas ao atendimento tardio dos pacientes.

Impact of the vector control program was evaluated eight years after implantation of epidemiological surveillance for Chagas disease in Berilo, a municipality in the Jequitinhonha Valley of the Brazilian State of Minas Gerais. In all 5,242 domiciliary units (96% of the total) were inspected and 10 found to be infested by the triatomine bug Triatoma pseudomaculata. Triatomines were found associated with bats inside one house and in the peridomiciles of the other nine. None of the 111 Triatoma pseudomaculata captured was infected with Trypanosoma cruzi. Noireau et al16 traps were installed in (n=8) and around (n=100) the infested house but no Trypanpsoma cruzi-infected triatomines were found. None bat, opossums (Didelphis albiventris) and rat captured in the peridomicile were infected with Trypanosoma cruzi although 24% of the inhabitants of the house infested by Triatoma pseudomaculata were seropositive for the parasite, based on ELISA, IHA and IIF.

This study aimed to evaluate the Chagas Disease Control Program which has operated since 1982 in the municipality of Berilo in the Jequitinhonha Valley, Minas Gerais, Brazil, based on evaluation of 5,242 domiciliary units (DUs) and 7,807 outbuildings over an eight-year period of epidemiological surveillance implanted in 1997. A total of 391 triatomines (280 Panstrongylus megistus and 111 Triatoma pseudomaculata) were captured, indicating the continued predominance of the former species. However, Triatoma pseudomaculata is clearly becoming more important in this region, with intradomiciliary colonies being detected in recent years. Entomological parameters, such as dispersion (17%) and intradomiciliary infestation (0.15%) indices, are compatible with the results of the epidemiological surveillance. The majority of DUs were of construction type A (plaster over bricks) or C (plaster over adobe). Twenty-five percent of the inhabitants of the DUs infested by triatomines were reactive in ELISA, IHA and IIF tests for Trypanosoma cruzi antigens.

The type of pulmonary histoplasmosis presents limited lesions to the lungs, with symptoms that are clinically and radiological similar to chronic pulmonary tuberculosis. This paper describes the clinical features of four cases of pulmonary histoplasmosis. Aspects of diagnostic and clinical, epidemiological, laboratorial and imaging exams are discussed, in addition to the clinical status of the individuals five years after disease onset. The treatment of choice was oral medication, following which all the patients improved. It is important to understand the clinical status and the difficulties concerning the differential diagnosis of histoplasmosis, to assist the proper indication of cases, thus reducing potential confusion with other diseases.

This study deals with investigating the groundwater quality for irrigation purpose, the vulnerability of the aquifer system to pollution and also the aquifer potential for sustainable water resources development in Kobo Valley development project. The groundwater quality is evaluated up on predicting the best possible distribution of hydrogeochemicals using geostatistical method and comparing them with the water quality guidelines given for the purpose of irrigation. The hydro geochemical parameters considered are SAR, EC, TDS, Cl-, Na+, Ca++, SO4 2- and HCO3 -. The spatial variability map reveals that these parameters falls under safe, moderate and severe or increasing problems. In order to present it clearly, the aggregated Water Quality Index (WQI) map is constructed using Weighted Arithmetic Mean method. It is found that Kobo-Gerbi sub basin is suffered from bad water quality for the irrigation purpose. Waja Golesha sub-basin has moderate and Hormat Golena is the better sub basin in terms of water quality. The groundwater vulnerability assessment of the study area is made using the GOD rating system. It is found that the whole area is experiencing moderate to high risk of vulnerability and it is a good warning for proper management of the resource. The high risks of vulnerability are noticed in Hormat Golena and Waja Golesha sub basins. The aquifer potential of the study area is obtained using weighted overlay analysis and 73.3% of the total area is a good site for future water well development. The rest 26.7% of the area is not considered as a good site for spotting groundwater wells. Most of this area fall under Kobo-Gerbi sub basin.

Este estudo pretendeu caracterizar as constantes elásticas de bronzes com alto teor em estanho e, em particular, da fase intermetálica delta (~32 wt.% Sn) à temperatura ambiente. Esta caracterização é importante para a previsão das propriedades acústicas dos sinos, pelo que especial atenção foi também dada à composição média das ligas Cu-Sn usadas na fundição sineira. Estes bronzes, devido ao caráter frágil da fase delta, não são processados termo-mecanicamente, apresentando-se na condição de vazados. Para o efeito, produziram-se provetes com teores de Sn variáveis (até 32 wt.% Sn), fundidos a 1150 ºC e arrefecidos em condições próximas das de fundição em moldação em areia. A composição e fases presentes foram estudadas com recurso a radiação X, enquanto que a microestrutura foi analisada com recurso a técnicas de microscopia. As constantes elásticas foram determinadas pela velocidade de propagação de impulsos ultra-sónicos, recorrendo a ondas longitudinais e transversais. Individualmente para a fase alfa e delta estas foram medidas por nanoindentação. Em conclusão, determinaram-se as constantes elásticas para as ligas de alto teor em Sn. Nas ligas hipo-eutectóides a estrutura grosseira de grão inviabilizou a técnica utilizada. Verificou-se também que a velocidade das ondas longitudinais pouco varia com o teor em Sn, contrariamente às ondas transversais.

The prolonged wait times may arguably put into question the Canadian Health Act of 1984. Statistics show throughput wait times are 5.5 hours and output wait times for admitted patients are 32.4 hours. After probing and analyzing best practices through a qualitative/quantitative Value Stream Mapping and a qualitative SWOT Analysis; Team Triage and an Overcapacity Protocol is suggested to improve non-admitted patients wait times by 1.89 hours and admitted patients wait times by 16 hours by eliminating wasteful steps in the patient process and upon overcapacity, effectively sharing already stabilized and admitted patients with all wards in the hospital.

INTRODUCTION: Hepatitis B virus (HBV) infection is one of the most serious public health problems in the world. In Brazil, HBV endemicity is heterogeneous, with the highest disease prevalence in the North region. METHODS: A total of 180 samples were analyzed and subjected to polymerase chain reaction (PCR) and semi-nested PCR of the HBV S-gene, with the aim of determining the prevalence of HBV-DNA (deoxyribonucleic acid) in indigenous groups inhabiting the areas near the Curuçá and Itaquaí Rivers in the Javari Valley, State of Amazonas, Brazil. RESULTS: The prevalence of the HBV-DNA S-gene was 51.1% (92/180). The analysis found 18 of 49 (36.7%) samples from the Marubo tribe, 68 of 125 (54.4%) from the Kanamary, and 6 of 6 (100%) from other ethnic groups to be PCR positive. There was no statistically significant difference in gender at 5% (p=0.889). Indigenous people with positive PCR for HBV-DNA had a lower median age (p<0.001) of 23 years. There was no statistical difference found in relation to sources of contamination or clinical aspects with the PCR results, except for fever (p<0.001). The high prevalence of HBV-DNA of 75% (15/20) in pregnant women (p=0.009) demonstrates an association with vertical transmission. CONCLUSIONS: The results confirm the high prevalence of HBV-DNA in the Javari Valley, making it important to devise strategies for control and more effective prevention in combating the spread of HBV.

Introduction The biological diversity of Trypanosoma cruzi strains plays an important role in the clinical and epidemiological features of Chagas disease. Methods Eight T. cruzi strains isolated from children living in a Chagas disease vector-controlled area of Jequitinhonha Valley, State of Minas Gerais, Brazil, were genetically and biologically characterized. Results The characterizations demonstrated that all of the strains belonged to T. cruzi II, and showed high infectivity and a variable mean maximum peak of parasitemia. Six strains displayed low parasitemia, and two displayed moderate parasitemia. Later peaks of parasitemia and a predominance of intermediate and large trypomastigotes in all T. cruzi strains were observed. The mean pre-patent period was relatively short (4.2±0.25 to 13.7±3.08 days), whereas the patent period ranged from 3.3±1.08 to 34.5±3.52 days. Mortality was observed only in animals infected with strain 806 (62.5%). Histopathological analysis of the heart showed that strains 501 and 806 caused inflammation, but fibrosis was observed only in animals infected with strain 806. Conclusions The results indicate the presence of an association between the biological behavior in mice and the genetic characteristics of the parasites. The study also confirmed general data from Brazil where T. cruzi II lineage is the most prevalent in the domiciliary cycle and generally has low virulence, with some strains capable of inducing inflammatory processes and fibrosis.
