High risk of recurrence/progression bladder tumours is treated with Bacillus Calmette-Guérin (BCG) immunotherapy after complete resection of the tumour. Approximately 75% of these tumours express the uncommon carbohydrate antigen sialyl-Tn (Tn), a surrogate biomarker of tumour aggressiveness. Such changes in the glycosylation of cell-surface proteins influence tumour microenvironment and immune responses that may modulate treatment outcome and the course of disease. The aim of this work is to determine the efficiency of BCG immunotherapy against tumours expressing sTn and sTn-related antigen sialyl-6-T (s6T). METHODS: In a retrospective design, 94 tumours from patients treated with BCG were screened for sTn and s6T expression. In vitro studies were conducted to determine the interaction of BCG with high-grade bladder cancer cell line overexpressing sTn. RESULTS: From the 94 cases evaluated, 36 had recurrence after BCG treatment (38.3%). Treatment outcome was influenced by age over 65 years (HR=2.668; (1.344-5.254); P=0.005), maintenance schedule (HR=0.480; (0.246-0.936); P=0.031) and multifocality (HR=2.065; (1.033-4.126); P=0.040). sTn or s6T expression was associated with BCG response (P=0.024; P<0.0001) and with increased recurrence-free survival (P=0.001). Multivariate analyses showed that sTn and/or s6T were independent predictive markers of recurrence after BCG immunotherapy (HR=0.296; (0.148-0.594); P=0.001). In vitro studies demonstrated higher adhesion and internalisation of the bacillus to cells expressing sTn, promoting cell death. CONCLUSION: s6T is described for the first time in bladder tumours. Our data strongly suggest that BCG immunotherapy is efficient against sTn- and s6T-positive tumours. Furthermore, sTn and s6T expression are independent predictive markers of BCG treatment response and may be useful in the identification of patients who could benefit more from this immunotherapy.

Little is known on the expression of the tumour-associated carbohydrate antigen sialyl-Tn (STn), in bladder cancer. We report here that 75% of the high-grade bladder tumours, presenting elevated proliferation rates and high risk of recurrence/progression expressed STn. However, it was mainly found in non-proliferative areas of the tumour, namely in cells invading the basal and muscle layers. STn was also found in tumour-adjacent mucosa, which suggests its dependence on a field effect of the tumour. Furthermore, it was not expressed by the normal urothelium, demonstrating the cancer-specific nature of this antigen. STn expression correlated with that of sialyltransferase ST6GalNAc.I, its major biosynthetic enzyme. The stable expression of ST6GalNAc.I in the bladder cancer cell line MCR induced STn expression and a concomitant increase of cell motility and invasive capability. Altogether, these results indicate for the first time a link between STn expression and malignancy in bladder cancer. Hence, therapies targeting STn may constitute new treatment approaches for these tumours.

«O Cadáver de James Joyce», conto publicado por José Luís Peixoto na revista Ficções em 2004, é um exercício que explora os territórios ambíguos que se situam na intersecção da “realidade” com a “imaginação”, da biografia com a ficção. O narrador-protagonista, José Luís Peixoto – epónimo do autor, portanto -, narra em poucas páginas a viagem impossível que faz a Zurique e a Dublin com o propósito único de secretamente devolver os restos mortais de James Joyce à cidade natal do escritor irlandês. A brevidade da narrativa acolhe a magnitude do feito narrado operando uma redimensionação do mundo, que por um lado o miniaturiza e por outro o esquematiza e, por essa via, irrealiza. A própria linguagem do conto acompanha este movimento de esquematização e síntese: a frase tende a encurtar-se, essencializa-se – e esta compressão da linguagem reforça o efeito da compressão do mundo representado. Pela forma como problematiza as relações entre a linguagem e o mundo, entre a literatura e a realidade, «O Cadáver de James Joyce» encena, em última instância, a dimensão simultaneamente grandiosa e precária da criação literária.

Este seminário pretendeu actualizar o estado da arte na abordagem da (dis)função respiratória da criança, uma vez que, nesta área, o fisioterapeuta amplificou os limites de actuação, para além do modelo biomédico. Num dos extremos encontra-se a intervenção comunitária, cujo modelo propicia a educação e promoção de saúde, privilegiando estratégias que capacitem o cuidador; no outro extremo inclui-se a intervenção do fisioterapeuta nos cuidados intensivos e na agudização da doença. A intervenção do fisioterapeuta centra-se cada vez mais numa abordagem holística, tendo como objectivo não só o tratamento da afecção do sistema que está na base da (dis)função mas também das repercussões fisiológicas, funcionais e sociais para a criança e cuidadores, sendo também responsável pela actuação sobre os factores ambientais. Desta forma, este seminário tornou-se pertinente não só para fisioterapeutas que trabalhem directamente com crianças com patologia cárdio-respiratória, mas também àqueles cujo âmbito de intervenção seja mais direccionado para outras áreas ou condições de saúde, de forma a potenciar o modelo de transporte de O2. De facto, a abordagem da (dis) função respiratória da criança deve estar implícita em qualquer processo de (re)educação, sendo fundamental em casos como a paralisia cerebral, doenças neuromusculares, alterações posturais, doenças auto-imunes, entre outras. Pelas suas importantes repercussões no sistema cárdiorespiratório da criança, torna-se fundamental alertar o fisioterapeuta para um conjunto de estratégias que potenciem a sua intervenção. O seminário pretendeu também ter um carácter multidisciplinar, contando com a presença de preletores nacionais e internacionais, referências nas várias áreas das profissões de saúde.

Aspergillus fumigatus (Af) and Pseudomonas aeruginosa (Pa) are leading fungal and bacterial pathogens, respectively, in many clinical situations. Relevant to this, their interface and co-existence has been studied. In some experiments in vitro, Pa products have been defined that are inhibitory to Af. In some clinical situations, both can be biofilm producers, and biofilm could alter their physiology and affect their interaction. That may be most relevant to airways in cystic fibrosis (CF), where both are often prominent residents. We have studied clinical Pa isolates from several sources for their effects on Af, including testing involving their biofilms. We show that the described inhibition of Af is related to the source and phenotype of the Pa isolate. Pa cells inhibited the growth and formation of Af biofilm from conidia, with CF isolates more inhibitory than non-CF isolates, and non-mucoid CF isolates most inhibitory. Inhibition did not require live Pa contact, as culture filtrates were also inhibitory, and again non-mucoid>mucoid CF>non-CF. Preformed Af biofilm was more resistant to Pa, and inhibition that occurred could be reproduced with filtrates. Inhibition of Af biofilm appears also dependent on bacterial growth conditions; filtrates from Pa grown as biofilm were more inhibitory than from Pa grown planktonically. The differences in Pa shown from these different sources are consistent with the extensive evolutionary Pa changes that have been described in association with chronic residence in CF airways, and may reflect adaptive changes to life in a polymicrobial environment.

Informal learning is becoming more and more important: Nowadays people learn more this way, through the Internet, than in schools or normal trainings. But they don’t get any certificateswhich attest this fact. So they can't show the employer or teacher etc. that they have learned something. TRAILER project aim is to solve this problem by developing a special tool for managing of all competences and skills acquired through informal learning experiences. Both from the perspective of the user and the perspective of an institution or a company. We’ll present the IT tool to show how people can make their informal learning outcomes visible. TRAILER helps users to gather all information about process and outcomes of their informal learning. Users can share this with friends, colleagues or their employees, teachers and so on. They can create an interactive e-portfolio which can be attached to their CV, cover letter or Knowledge Management system etc. After the presentation of the tool we will discuss possible areas and fields to use this tool. Also we would like to discuss all possible use of the tool by the participants and another needs in this area. Moreover we want to discuss other problems in informal learning process, ways to solve the problems and discuss other ideas of different IT tools which could help in informal learning process. During the discussion we’ll use an interactive respond system which can be used on mobile devices: it makes possible for participants to share their opinions individually before knowing another persons' opinion.

People do not learn only in formal educational institutions, but also throughout their lives, from their experiences, conversations, observations of others, exploration of the Internet, meetings and conferences, and chance encounters etc. However this informal and non-formal learning can easily remain largely invisible, making it hard for peers and employers to recognize or act upon it. The TRAILER project aims to make this learning visible so that it can benefit both the individual and the organization. The proposed demonstration will show a software solution that (i) helps the learners to capture, organize and classify a wide range of ’informal’ learning taking place in their lives, and (ii) assists the organization in recognizing this learning and use it to help managing human resources (benefiting both parts). This software tool has recently been used in two phases of pilot studies, which have run in four different European countries.

The evolution of new technology and its increasing use, have for some years been making the existence of informal learning more and more transparent, especially among young and older adults in both Higher Education and workplace contexts. However, the nature of formal and non-formal, course-based, approaches to learning has made it hard to accommodate these informal processes satisfactorily, and although technology bring us near to the solution, it has not yet achieved. TRAILER project aims to address this problem by developing a tool for the management of competences and skills acquired through informal learning experiences, both from the perspective of the user and the institution or company. This paper describes the research and development main lines of this project.
