Le Programme cantonal Diabète souhaite collecter des indicateurs afin d'objectiver son suivi et produire un Baromètre Diabète Vaud qui serait publié et distribué périodiquement à un public large. Ce rapport présente les indicateurs suggérés par l'IUMSP, mandaté afin d'établir une liste raisonnée d'indicateurs sur le diabète pour le canton de Vaud. La sélection d'indicateurs s'est faite d'après plusieurs critères, parmi lesquels la disponibilité des indicateurs actuelle et à long terme, ainsi que leur pertinence avérée par la littérature. Ce rapport décrit les résultats pour les indicateurs sélectionnés, ainsi que les informations nécessaires à leur compréhension : source, fréquence de mise à disposition, méthode de calcul, limites, références éventuelles. Parmi les indicateurs décrits dans ce rapport, certains sont proposés comme spécialement pertinents à inclure dans le futur Baromètre Diabète Vaud. Aussi, une suggestion de présentation de ces indicateurs (fiches descriptives) est articulée pour ce dernier.

Emotion regulation plays a key role in mental health and psychopathology. Therefore, it seems important to develop effective forms of emotion regulation. Implementation intentions are if-then plans that help people attain their self-regulatory goals. Perspective-taking and response-focused implementation intentions have been shown to reduce feelings of unpleasantness and arousal, respectively, in response to briefly presented disgusting pictures. The present study addressed the open research questions whether forming these types of implementation intentions is effective in regulating affect during prolonged presentation of disgusting pictures, and whether it is associated with changes in physiological arousal. Eighty-one participants viewed disgusting, neutral, and pleasant pictures of 6 s duration under four instructions: the goal intention to not get disgusted, this goal intention furnished with a perspective-taking or a response-focused implementation intention, and no emotion regulation instructions. The dependent variables were ratings of disgust, valence, arousal, and electrodermal activity. Only perspective-taking implementation intention participants significantly reduced their disgust and unpleasantness as compared to goal-intention and control participants. Arousal and skin conductance did not significantly differ between conditions. The effectiveness of response-focused but not perspective-taking implementation intentions seems to be substantially reduced during sustained exposure duration.

The main objective of this dissertation is to create new knowledge on an administrative innovation, its adoption, diffusion and finally its effectiveness. In this dissertation the administrative innovation is approached through a widely utilized management philosophy, namely the total quality management (TQM) strategy. TQM operationalizes a self-assessment procedure, which is based on continual improvement principles and measuring the improvements. This dissertation also captures the theme of change management as it analyzes the adoption and diffusion of the administrative innovation. It identifies innovation characteristics as well as organisational and individual factors explaining the adoption and implementation. As a special feature, this study also explores the effectiveness of the innovation based on objective data. For studying the administrative innovation (TQM model), a multinational Case Company provides a versatile ground for a deep, longitudinal analysis. The Case Company started the adoption systematically in the mid 1980s in some of its units. As part of their strategic planning today, the procedure is in use throughout the entire global company. The empirical story begins from the innovation adoption decision that was made in the Case Company over 22 years ago. In order to be able to capture the right atmosphere and backgrounds leading to the adoption decision, key informants from that time were interviewed, since the main target was to clarify the dynamics of how an administrative innovation develops. In addition, archival material was collected and studied, available memos and data relating to the innovation, innovation adoption and later to the implementation contained altogether 20500 pages of documents. A survey was furthermore conducted at the end of 2006 focusing on questions related to the innovation, organization and leadership characteristics and the response rate totalled up to 54%. For measuring the effectiveness of the innovation implementation, the needed longitudinal objective performance data was collected. This data included the profit unit level experience of TQM, the development of the self assessment scores per profit unit and performance data per profit unit measured with profitability, productivity and customer satisfaction. The data covered the years 1995-2006. As a result, the prerequisites for the successful adoption of an administrative innovation were defined, such as the top management involvement, support of the change agents and effective tools for implementation and measurement. The factors with the greatest effect on the depth of the implementation were the timing of the adoption and formalization. The results also indicated that the TQM model does have an effect on the company performance measured with profitability, productivity and customer satisfaction. Consequently this thesis contributes to the present literature (i) by taking into its scope an administrative innovation and focusing on the whole innovation implementation process, from the adoption, through diffusion until its consequences, (ii) because the studied factors with an effect on the innovation adoption and diffusion are multifaceted and grouped into individual, organizational and environmental factors, and a strong emphasis is put on the role of the individual change agents and (iii) by measuring the depth and consistency of the administrative innovation. This deep analysis was possible due to the availability of longitudinal data with triangulation possibilities.

Tason ovet voivat olla joko itsesuunniteltuja ja -tehtyjä, tai itse malli ja tuote voidaan ostaa myös yrityksen ulkoa. Tätä make-or-buy kysymystä pohdittaessa ilmenee monta tekijää, jotka vaikuttavat päätökseen. Jos yritys käsittää ostamisen parempana vaihtoehtona, tuote tarvitsee myösimplementointisuunnitelman. Tämän työn päätavoite oli luoda arviointi- ja implementointiprosessi kaupallisille hissin tason oville. Muut tavoitteet olivat: selvittää syyt tälle erikoiselle toiminnolle, löytää kaupallisten ovien käyttöön liittyvät edut ja haitat, sekä luoda kriteerit tason ovien arvioinnille. Lopputuloksena saatiin, että tämän prosessin suorittajan pitää sitoutua arviointi- ja implementointiprosessiin,ja että myös virtuaalitehdas tarvitaan täydentämään arviointivaihetta. Mutta tärkein havainto oli, että tulevaisuuden ovistrategia ei ole vielä täysin päätetty ja siitä syystä yrityksen omat ovimallit pitäisi testata ja verrata niitä kaupallisiin ovimalleihin käyttämällä tämän työn arviointiprosessia.

This paper analyses how banking regulation was introduced in Switzerland - one of the world's most prominent financial centres - which remained in place until the beginning of the twenty-first century. It shows that the law adopted on 8 November 1934 is a perfect example of capture of the regulator by the regulated. Essentially a political response in the context of the economic crisis of the 1930s, it largely reflected the interests of banking circles by limiting the intervention of the State as much as possible. The introduction of the new legislation was facilitated by the temporary weakness of Swiss banking circles, as they depended on the State to delay or prevent the collapse of many major credit institutions. They did not manage to derail the law as they had two decades earlier when they scuppered the federal bill on banks drawn up between 1914 and 1916. But this time they were better organized and more united, and intervened all the more effectively in the legislative process itself. The 1934 law is thus distinctive in that it made no structural changes to the architecture of the financial centre but merely codified its practices through flexible legislation meant to reassure the public. The law was aimed less at controlling banking activity than at keeping - thanks to skilfully calibrated political concessions - the State from having to intervene more directly in the internal management of banks or in the fixing of interest rates and the export of capital.

Työssä tutkittiin IFC (Industrial Foundation Classes)-tietomallin mukaisen tiedoston jäsentämistä, tiedon jatkoprosessointia ja tiedonsiirtoa sovellusten välillä. Tutkittiin, mitä vaihtoehtoja tiedon siirron toteuttamiseksi ohjelmallisesti on ja mihin suuntaan tiedon siirtäminen on menossa tulevaisuudessa. Soveltavassa osassa toteutettiin IFC-standardin mukaisen ISO10303-tiedoston (Osa 21) jäsentäminen ja tulkitseminen XML-muotoon. Sovelluksessa jäsennetään ja tulkitaan CAD-ohjelmistolla tehty IFC-tiedosto C# -ohjelmointikielellä ja tallennetaan tieto XML-tietokantaan kustannuslaskentaohjelmiston luettavaksi.

The strategy process is a method for strategy formulation and implementation. The strategy process is commonly used especially within bigger companies. It is important to link the strategy formulation and implementation. The objective of this thesis has been to find out improvement areas for the case company’s strategy process. The theoretical framework based on literature emphasizes on strategy process as a method for strategy formulation and implementation. The theoretical framework, several mainly ad hoc interviews and author’s observation were used as tools to analyze the case company’s strategy process. The hierarchy in between the various corporate levels provides the foundation to formulate and implement the strategies. These strategies include the corporate and strategic business area level strategies. The recommendations to improve the case company’s strategy process were formulated at corporate and strategic business area levels. These recommendations were formulated based on research and experience gained throughout the work. The role of strategic projects to implement the strategies more efficiently and organizational control over distribution were found as potential improvement areas. The resource allocation prioritizing towards the most important strategic projects was also an important improvement area.

A Nurse's Preceptorship Skills of Guiding Students and the Need for and Benefit from Preceptorship Education Guided practical training in units of health services is an essential part of the education of nurses. Nurses take care of a student's preceptorship during these periods, and their role in supporting a student's learning has been shown to be the most important factor in the learning environment of guided practical training. Education for preceptors in various educational units has been organized to develop their skills of guiding students. The aim of this study was both to investigate preceptors' skills of guiding students, the differences in the student guiding skills of those who have and who have not received preceptorship education and to describe their experiences of their own need for preceptorship education and of the benefit of such education. Any activities that promote a student's learning were included in nurses' preceptorship skills. On the basis of research knowledge, the preceptorship skills were divided into the following subdomains: proficiency in nursing; creation of a preceptorship relationship; planning of preceptorship; implementation of preceptorship; combination of theory and practice; and evaluation. The target group comprised all those nurses (n=128) in a hospital in southern Finland who guided future nurses. The material was gathered by means of a questionnaire with structural and open questions. Preceptorship skills were studied with the structured questions and the need for and benefit from preceptorship education with the open questions. The material was interpreted by means of a statistical SAS programme and qualitative content analysis. The preceptorship skills in all domains of guiding skills proved good. Those who had received preceptorship education had better skills than those who had not received such education in all domains but “creation of a preceptorship relationship”. However, the differences were not statistically significant. The need for preceptorship education was highest in evaluation and in setting goals for a student. To support these, the preceptors wanted information on education and students' requirements. Most of all, preceptorship education had benefited the creation of a preceptorship relationship and the implementation of evaluation. The preceptors were of the opinion that their skills of guiding students were good. However, education is needed, which makes the results inconsistent in this respect. The results can be used in developing preceptorship skills and in planning preceptorship education.

Le programme cantonal vaudois de dépistage du cancer colorectal vise à faciliter ce dépistage pour la population de 50 à 69 ans. Les deux modalités retenues sont la recherche immunologique de sang dans les selles (FIT) et la coloscopie. La décision de réaliser un test de dépistage et la modalité de dépistage s'appuient sur une consultation individuelle avec un médecin de famille. L'assurance de base prend en charge le remboursement. Le programme vaudois permet l'exemption de la franchise pour la consultation médicale d'information et les deux modalités de dépistage, ainsi que pour la coloscopie de confirmation en cas de test FIT positif. La quote-part de 10 % reste à charge des participants. Des outils de communication ont été développés pour faciliter un entretien de décision partagée dans le cadre d'une consultation médicale. The colorectal cancer screening program of the canton of Vaud aims to facilitate screening for this cancer for the population aged 50 to 69 years old. The two screening modalities offered are fecal immunochemical testing (FIT) and colonoscopy. The decision to undergo screening and the screening modality is based on an individual medical encounter with a primary care physician. Both screening modalities are reimbursed through basic health coverage in Switzerland. The participation to the screening program allows the exemption of the deductible for the medical encounter and the chosen screening modality. A copay of 10% is maintained for all costs. Communication tools were developed on the basis of recommendations in the literature to facilitate shared decision-making in a medical encounter.

Being highly discussed the problem of climate change and global warming has been keeping importance for several of decades. As a response to the world’s need in solution for climate change disasters, the United Nations Framework Convention on Climate Change was adopted in 1992 and supplemented with the Kyoto protocol in 1997. This work is aimed to give better understanding of the Convention, Kyoto Protocol with its mechanisms and their function, related to energy projects in such case countries, as Russia and China, in order to assist evaluation of projects cost-effectiveness. It provides basic information about the Convention and the Protocol with their regulations, overview of present situation and future post-Kyoto forecasts, while the most attention is concentrated on the clean development mechanism and joint implementation step-by-step project cycles and specific regulations in given countries. The current study disclosed that CDM and JI project cycles are resulting in a complicated process. By the moment it requires step-by-step following of a number of methodologies, spending time and finance to particular project development. Uncertainties about post-Kyoto period bring additional risk to the projects and complicate any business decision concerning Kyoto Protocol.
