Nicotine, the addictive compound of tobacco products, exerts its effects in the brain by binding to neuronal acetylcholine nicotinic receptors (nAChRs). The aim of the present study was to increase the knowledge of nicotine s complex effects, the focus being on homomeric alpha7-nAChRs that are widely expressed in the brain. Nicotinic regulation of differential signalling molecules including transcriptional regulators was also studied. We found that the number of alpha7-nAChRs is increased in specific brain regions in mice, in a time-dependent manner after chronic oral nicotine administration. Our results suggest that in addition to alpha4beta2-nAChRs, the other major nAChR subtype expressed in the brain, the number of alpha7-nAChRs is affected by chronic presence of nicotine. We suggest that when studying the long-term effects of nicotine, the duration on administration is of great importance. Next, we observed that nicotine exposure induces accumulation of cAMP in cell cultures expressing nAChRs. Furthermore, nicotine-induced alpha7-nAChR upregulation was potentiated by treatments enhancing cAMP-signalling, suggesting a role for cAMP in the upregulation process. Protein kinase C (PKC) was found essential for the basal regulation of alpha7-nAChR number. The nicotine-evoked alpha7-nAChR upregulation could be further increased by PKC overexpression. Thirdly, the effects of nicotine on dopamine and cAMP regulated phosphoprotein (DARPP-32) were characterised in rat brain. The results show that DARPP-32 is regulated by both acute and long-term nicotine treatment in the striatal subdivisions. The effect of acute nicotine is dose-dependent and the three striatal regions display differential sensitivities to nicotine. Chronic nicotine is also able to regulate DARPP-32 signalling with prominent effect seen in the nucleus accumbens (NAc), suggesting a role for DARPP-32 in the mediation of long-term effects of nicotine. Finally, the regulation of transcription factors Elk-1 and FosB/deltaFosB by nicotine was investigated. We found that Elk-1 is activated by acute nicotine selectively in the NAc core and hippocampal area CA1, whereas acute nicotine does not affect FosB/deltaFosB. Long-term intermittent or continuous nicotine increases the level of total Elk-1 in the same brain regions as acute nicotine. FosB/deltaFosB is also affected by chronic nicotine. Thus, similarly to other drugs of abuse, nicotine regulates transcriptional regulators Elk-1 and FosB/deltaFosB. These results bring further support for a common mechanism underlying the development of addiction. Nicotine s positive effects on learning and memory might involve the transcription factor Elk-1 based on the changes seen in the hippocampus, the key area in cognitive functions.

Fumonisin B1 (FB1) is a mycotoxin produced by the fungus Fusarium verticillioides, which commonly infects corn and other agricultural products. Fusarium species can also be found in moisture-damaged buildings, and therefore there may also be human exposure to Fusarium mycotoxins, including FB1. FB1 affects the metabolism of sphingolipids by inhibiting the enzyme ceramide synthase. It is neuro-, hepato- and nephrotoxic, and it is classified as possibly carcinogenic to humans. This study aimed to clarify the mechanisms behind FB1-induced neuro- and immunotoxicity. Four neural and glial cell lines of human, rat and mouse origin were exposed to graded doses of FB1 and the effects on the production of reactive oxygen species, lipid peroxidation, intracellular glutathione levels, cell viability and apoptosis were investigated. Furthermore, the effects of FB1, alone or together with lipopolysaccharide (LPS), on the mRNA and protein expression levels of different cytokines and chemokines were studied in human dendritic cells (DC). FB1 induced oxidative stress and cell death in all cell lines studied. Generally, the effects were only seen after prolonged exposure at 10 and 100 µM of FB1. Signs of apoptosis were also seen in all four cell lines. The sensitivities of the cell lines used in this study towards FB1 may be classified as human U-118MG glioblastoma > mouse GT1-7 hypothalamic > rat C6 glioblastoma > human SH-SY5Y neuroblastoma cells. When comparing cell lines of human origin, it can be concluded that glial cells seem to be more sensitive towards FB1 toxicity than those of neural origin. After exposure to FB1, significantly increased levels of the cytokine interferon-γ (IFNγ) were detected in human DC. This observation was further confirmed by FB1-induced levels of the chemokine CXCL9, which is known to be regulated by IFNγ. During co-exposure of DC to both LPS and FB1, significant inhibitions of the LPS-induced levels of the pro-inflammatory cytokines interleukin-6 (IL-6) and IL-1β, and their regulatory chemokines CCL3 and CCL5 were observed. FB1 can thus affect immune responses in DC, and therefore, it is rather likely that it also affects other types of cells participating in the immune defence system. When evaluating the toxicity potential of FB1, it is important to consider the effects on different cell types and cell-cell interactions. The results of this study represent new information, especially about the mechanisms behind FB1-induced oxidative stress, apoptosis and immunotoxicity, as well as the varying sensitivities of different cell types towards FB1.

The UDP-glucuronosyltransferases (UGTs) are enzymes of the phase II metabolic system. These enzymes catalyze the transfer of α-D-glucuronic acid from UDP-glucuronic acid to aglycones bearing nucleophilic groups affording exclusively their corresponding β-D-glucuronides to render lipophilic endobiotics and xenobiotics more water soluble. This detoxification pathway aids in the urinary and biliary excretion of lipophilic compounds thus preventing their accumulation to harmful levels. The aim of this study was to investigate the effect of stereochemical and steric features of substrates on the glucuronidation catalyzed by UGTs 2B7 and 2B17. Furthermore, this study relates to the design and synthesis of novel, selective inhibitors that display high affinity for the key enzyme involved in drug glucuronidation, UGT2B7. The starting point for the development of inhibitors was to assess the influence of the stereochemistry of substrates on the UGT-catalyzed glucuronidation reaction. A set of 28 enantiomerically pure alcohols was subjected to glucuronidation assays employing the human UGT isoforms 2B7 and 2B17. Both UGT enzymes displayed high stereoselectivity, favoring the glucuronidation of the (R)-enantiomers over their respective mirror-image compounds. The spatial arrangement of the hydroxy group of the substrate determined the rate of the UGT-catalyzed reaction. However, the affinity of the enantiomeric substrates to the enzymes was not significantly influenced by the spatial orientation of the nucleophilic hydroxy group. Based on these results, a rational approach for the design of inhibitors was developed by addressing the stereochemical features of substrate molecules. Further studies showed that the rate of the enzymatic glucuronidation of substrates was also highly dependent on the steric demand in vicinity of the nucleophilic hydroxy group. These findings provided a rational approach to turn high-affinity substrates into true UGT inhibitors by addressing stereochemical and steric features of substrate molecules. The tricyclic sesquiterpenols longifolol and isolongifolol were identified as high-affinity substrates which displayed high selectivity for the UGT isoform 2B7. These compounds served therefore as lead structures for the design of potent and selective inhibitors for UGT2B7. Selective and potent inhibitors were prepared by synthetically modifying the lead compounds longifolol and isolongifolol taking stereochemical and steric features into account. The best inhibitor of UGT2B7, β-phenyllongifolol, displayed an inhibition constant of 0.91 nM.

A 26-hour English reading comprehension course was taught to two groups of second year Finnish Pharmacy students: a virtual group (33 students) and a teacher-taught group (25 students). The aims of the teaching experiment were to find out: 1.What has to be taken into account when teaching English reading comprehension to students of pharmacy via the Internet and using TopClass? 2. How will the learning outcomes of the virtual group and the control group differ? 3. How will the students and the Department of Pharmacy respond to the different and new method, i.e. the virtual teaching method? 4. Will it be possible to test English reading comprehension learning material using the groupware tool TopClass? The virtual exercises were written within the Internet authoring environment, TopClass. The virtual group was given the reading material and grammar booklet on paper, but they did the reading comprehension tasks (written by the teacher), autonomously via the Internet. The control group was taught by the same teacher in 12 2-hour sessions, while the virtual group could work independently within the given six weeks. Both groups studied the same material: ten pharmaceutical articles with reading comprehension tasks as well as grammar and vocabulary exercises. Both groups took the same final test. Students in both groups were asked to evaluate the course using a 1 to 5 rating scale and they were also asked to assess their respective courses verbally. A detailed analysis of the different aspects of the student evaluation is given. Conclusions: 1.The virtual students learned pharmaceutical English relatively well but not significantly better than the classroom students 2. The overall student satisfaction in the virtual pharmacy English reading comprehension group was found to be higher than that in the teacher-taught control group. 3. Virtual learning is easier for linguistically more able students; less able students need more time with the teacher. 4. The sample in this study is rather small, but it is a pioneering study. 5. The Department of Pharmacy in the University of Helsinki wishes to incorporate virtual English reading comprehension teaching in its curriculum. 6. The sophisticated and versatile TopClass system is relatively easy for a traditional teacher and quite easy for the students to learn. It can be used e.g. for automatic checking of routine answers and document transfer, which both lighten the workloads of both parties. It is especially convenient for teaching reading comprehension. Tutkielman tavoitteena on saada yleiskuva senegalilaisesta populaarimusiikista tarkastellen senegalilaisen perinteen ja modernien vaikutteiden kohtaamista tässä musiikissa. Moderni käsitetään siis työssä perinteen vastakohdaksi, vaikka perinteinen–moderni-jako onkin varsin ongelmallinen. Tutkimus perustuu kirjallisten lähteiden ja äänitteiden lisäksi tekijän musiikki- ja tanssiopintoihin sekä Senegalissa tehtyyn kenttätyöhön. Työssä käytetään musiikkiantropologisia ja musiikkianalyyttisiä metodeja. Tarkastelun taustaksi työssä luodaan katsaus senegalilaiseen musiikkiperinteeseen ja senegalilaisten populaarimusiikkityylien kehittymiseen. Varsinaista aihetta käsitellään sekä muusikkohaastatteluja että esimerkkikappaleita analysoimalla. Tutkimuksen pohjalta senegalilainen populaarimusiikki näyttäytyy musiikilliselta materiaaliltaan hyvin perinteisenä, lähinnä käytetyt soittimet ja teknologia ovat moderneja piirteitä. Tosin myös äänitetuotannon ja modernien medioiden vaatimukset ovat vaikuttaneen muun muassa kappaleiden rakenteisiin. Populaarimusiikki on kuitenkin kehittynyt ennemminkin länsimaista ja latinalaisamerikkalaista musiikkia "afrikkalaistamalla" kuin senegalilaista perinnemusiikkia "länsimaistamalla", vaikka perinnettä on myös pyritty modernisoimaan. Vaikka senegalilaisen populaarimusiikin voikin katsoa jatkavan alueen musiikkiperinteitä, se nähdään kuitenkin modernina musiikin muotona, ja muusikot mielellään korostavat tätä mielikuvaa haastatteluissa. Toisaalta populaarimusiikissa tuodaan esille myös mielikuvia autenttisesta senegalilaisuudesta, idealisoidusta perinteisestä yhteiskunnasta, jota ei ehkä koskaan ole ollut olemasta. Näiden mielikuvien avulla, toisaalta moderniutta, toisaalta perinnettä korostamalla. senegalilainen populaarimusiikki rakentaa ja tuo esiin modernia senegalilaista identiteettiä.

Soturit olivat keskeinen sosiaalinen ryhmä keskiajan läntisessä Euroopassa ja Meiji-kautta (1868-1912) edeltäneessä Japanissa. Japanin avauduttua 1800-luvun puolivaiheilla maan historiaa alettiin kirjoittaa eurooppalaisen mallin mukaan, ja soturiperinteiden rinnastaminen ja vertailu yleistyivät. Vertailun taustalla vaikuttivat käsitykset alueiden samankaltaisesta feodaalisesta historiasta. Feodalismi on säilynyt keskeisenä teemana vertailuissa ja Japanin-tutkimuksessa, vaikka keskustelu siitä onkin Euroopan keskiajantutkimuksessa pitkälti hiipunut. Myös Japanin-tutkimuksessa on viime aikoina alettu esittää kritiikkiä feodalismi-termin käyttöä, rinnastuksia ja jopa pelkkää Euroopan historiaan vertaamistakin kohtaan. Feodalismin ohella muita keskeisiä vertailuteemoja ovat Japanin modernisoituminen ja sodankäynnin teknologia. Ensimmäiset vertailut olivat etupäässä yksittäisten joskus hyvin ylimalkaisten rinnastusten hakemista. Myös systemaattisia sivilisaatiohistoriallisia vertailuja alettiin tehdä jo varhain. Japanin-tutkimuksen ensisijaiseksi vertailukohteeksi ovat kuitenkin nousseet Euroopan historian sijaan teoriat feodalismista. Tarkastelu keskittyy nykyisin lähinnä eurooppalaisten termien käyttökelpoisuuteen Japanin historiasta kirjoitettaessa. Japanin modernisoitumista käsittelevät vertailut sivuavat keskusteluita feodalismista, mutta sotureiden rooli jää niissä usein hyvin vähäiseksi. Sodankäynnin teknologiaan keskittyvät vertailut ovat ilmiönä varsin tuore, sillä japanilaisen ja eurooppalaisen sodankäynnin pääteknologiat ovat olleet ilmeisen erilaisia lukuunottamatta 1500-luvun jälkipuoliskoa ja 1600-luvun alkua sekä nykyaikaa. Uuden ajan alun Euroopan ja saman ajan Japanin sotateknologiset yhtäläisyydet rajoittuvat jalkaväen tuliaseiden käyttöönoton mukanaan tuomiin muutoksiin maasodankäynnissä ja linnoittamiseen. Merisodankäynnin ja tykistön kehitys oli alueilla erilaista. Ritareiden ja samuraiden historioissa vaikuttavat edellä mainitun varhaisten tuliaseiden aikakauden rinnalla yhtäläisimmiltä kehityskuluilta niin sanotut varhais- ja täysfeodaaliset kaudet. Näistä ensimmäisellä tarkoitetaan Euroopan karolinkivaltakunnan aikaa suhteessa Kamakura-bakufuun (1185-1333) Japanissa. Jälkimmäisellä viitataan puolestaan ensimmäisen vuosituhannen vaihteen tienoille ajoittuvasta murroksesta noin 1300-1400-luvulle ulottuvaan ajanjaksoon Euroopassa ja sisällissotien kauteen 1300-luvun lopulta 1600-luvun alkuun Japanissa. Soturiperinteiden historioissa lähimmin toisiaan vastaavat feodaaliset piirteet ovat sotureiden yhteiskunnallinen asema ja heidän arvomaailmansa. Ilmeisin ongelma Euroopan ja Japanin vertailemisessa on se, että Eurooppa on laajempi ja historialtaan monimuotoisempi kuin Japani. Kuitenkaan tätä mittakaavaongelmaa eikä muitakaan metodologisia kysymyksiä ole vertailuissa juurikaan pohdittu. Osasyynä tähän lienee se, että muutamaa poikkeusta lukuunottamatta vertailijoiden asiantuntemus on keskittynyt vain toisen soturiperinteen historiaan. Sotureiden historiat tarjoavat antoisan vertailuparin. Suurista yhtäläisyyksistä huolimatta ritareita ja samuraita ei tulisi summittaisesti samaistaa toisiinsa, vaan rinnastettaessa tulisi mieluummin käyttää yleisempää soturin käsitettä. This study addresses four issues concerning technological product innovations. First, the nature of the very early phases or "embryonic stages" of technological innovation is addressed. Second, this study analyzes why and by what means people initiate innovation processes outside the technological community and the field of expertise of the established industry. In other words, this study addresses the initiation of innovation that occurs without the expertise of established organizations, such as technology firms, professional societies and research institutes operating in the technological field under consideration. Third, the significance of interorganizational learning processes for technological innovation is dealt with. Fourth, this consideration is supplemented by considering how network collaboration and learning change when formalized product development work and the commercialization of innovation advance. These issues are addressed through the empirical analysis of the following three product innovations: Benecol margarine, the Nordic Mobile Telephone system (NMT) and the ProWellness Diabetes Management System (PDMS). This study utilizes the theoretical insights of cultural-historical activity theory on the development of human activities and learning. Activity-theoretical conceptualizations are used in the critical assessment and advancement of the concept of networks of learning. This concept was originally proposed by the research group of organizational scientist Walter Powell. A network of learning refers to the interorganizational collaboration that pools resources, ideas and know-how without market-based or hierarchical relations. The concept of an activity system is used in defining the nodes of the networks of learning. Network collaboration and learning are analyzed with regard to the shared object of development work. According to this study, enduring dilemmas and tensions in activity explain the participants' motives for carrying out actions that lead to novel product concepts in the early phases of technological innovation. These actions comprise the initiation of development work outside the relevant fields of expertise and collaboration and learning across fields of expertise in the absence of market-based or hierarchical relations. These networks of learning are fragile and impermanent. This study suggests that the significance of networks of learning across fields of expertise becomes more and more crucial for innovation activities.

In the 1990 s the companies utilizing and producing new information technology, especially so-called new media, were also expected to be forerunners in new forms of work and organization. Researchers anticipated that new, more creative forms of work and the changing content of working life were about to replace old industrial and standardized ways of working. However, research on actual companies in the IT sector revealed a situation where only minor changes to existing organizational forms were seen .Many of the independent companies faced great difficulties trying to survive the rapid changes in the products and production forms in the emerging field. Most of the research on the new media field has been conducted as surveys, and an understanding of the actual everyday work process has remained thin. My research is a longitudinal study of the early phases of one new media company in Finland. The study is an analysis of the challenges the company faced in a rapidly changing business field and the attempts to overcome these challenges. The two main analyses in the study focus on the developmental phases of the company and the disturbances in the production process. Based on these analyses, I study changes and learning at work using the methodological framework of developmental work research. Developmental work research is a Finnish variant of the cultural-historical activity theory applied to the study of learning and transformations at work. The data was gathered over a three-year period of ethnographic fieldwork. I documented the production processes and everyday life in the company as a participant observer. I interviewed key persons, video and audio-taped meetings, followed e-mail correspondence and collected various documents, such as agreements and memos. I developed a systematic method for analyzing the disturbances in the production process by combining the various data sources. The systematic analysis of the disturbances depicted a very complex and only partly managed production process. The production process had a long duration, and no single actor had an understanding of it as a whole. Most of the disturbances had to do with the customer relationships. The nature of the disturbances was latent; they were recognized but not addressed. In the particular production processes that I analyzed, the ending life span of a particular product, a CD-ROM, became obvious. This finding can be interpreted in relation to the developmental phase of the production and the transformation of the field as a whole. Based on the analysis of the developmental phases and the disturbances, I formulate a hypothesis of the contradictions and developmental potentials of the activity studied. The conclusions of the study challenge the existing understanding of how to conceptualize and study organizational learning in production work. Most theories of organizational learning do not address qualitative changes in production nor historical challenges of organizational learning itself. My study opens up a new horizon in understanding organizational learning in a rapidly changing field where a learning culture based on craft or mass production work is insufficient. There is a need for anticipatory and proactive organizational learning. Proactive learning is needed to anticipate the changes in production type, and the life cycles of products.

In this research the technical functionality, quality and demands made on endurance runners` running suits in winter conditions were studied. The aim was also to find out how smart clothing and wearable technology are adopted in endurance runners` practise. Referring to previous studies the subject was approached in the theoretical part by studying the profile of endurance running, sports wear and the technology to wear as well as the smart clothing from the point of view of endurance running. The basis of subject was the interest of smart materials and the connection between technical structures and functionality. In the science of handicrafts smart clothing is rarely researched which made it even more interesting for the author. This research was carried out by the principles of the usability research. Usability means the suitability of product to its intended meaning. In the research both quantitative and qualitative methods were used. Researched persons were active competitive long-distance runners and also the long-distance runners doing it as a hobby, 35 male and 12 female runners. User information was gathered by the internet forms which mainly was based on the multiple choices but also included few open questions. Gathered information was considered by using both quantitative and content analysing methods. The functional long-distance running practice suit in winter conditions consisted of layered look which considered the possibilities of functional and smart materials. The Practise suit was humid transformable, easy care and light also comfortable to wear. These suits were hoped to be more functional than the current ones. The future running suit was described to not to feel or notice during running. It will not be too tight or sweltering. The functional abilities of clothing materials were believed to be developed further more. Even if the new technical materials are adopted for the running suits the technology to wear is not even though half of the researched runners used pulse indicators. The runners hoped the technology to wear to change more invisible and easier to use. Some of the runners wished the technology to wear to be integrated straight to the clothes which would reduce the number of devices carried with while running. The rare use of Polar Adidas AdiStar Fusion practise system and some other similar systems for endurance running was surprising. According to the results the smart clothing would not make a big brake through in the near future. In the point of view of the researched persons developing of clothing materials was a good and necessary thing, but integrating too much technology to the hobby smears the main purpose of sports and focuses wrongly on the metres and others minors .

Tutkimus kuuluu Helsingin yliopiston opettajankoulutuslaitoksen luonnontieteen ja teknologian opetusta ja opetusvälineitä kehittävään LUONTI-projektiin. Tutkimuksen tarkoituksena on teknologiakasvatukseen liittyvän käsitteistön selkiyttäminen ja projektissa kehitetyn laitteiston, Empirica Controlin, käyttöönotto ja sen tutkiminen. Teoreettisessa taustassa tarkastellaan teknologiaa ja teknologiakasvatusta oppivelvollisuuskoulun näkökulmasta. Erilaisia teknologiamäärittelyjä ja ajatuksia teknologian olemuksesta verrataan tutkimuksessa kehitetyillä malleilla. Teknologia on luonteeltaan integroiva aihepiiri, jolle on ainakin tässä tutkimuksessa esitetyn teorian valossa vaikea määritellä omaa tiedonalaa ja siten luontevaa paikkaa omana oppiaineenaan. Avoimessa oppimisympäristössä tapahtuvaa kognitiivisen teorian mukaista oppimista on hahmoteltu muokkaamalla Lehtisen (1988) esittämää mallia. Aikaisempien tutkimus- ja kokeiluraporttien perusteella määritellään kolme luovaa ongelmanratkaisua ja avointa oppimisympäristöä hyödyntävää työtapaa: mallintava menetelmä, ongelmanratkaisu ja vapaa kokeilu. Empirica Controlia verrataan tekstipohjaiseen Logoon ja muihin markkinoilla oleviin laitteistoihin sekä tarkastellaan sen ominaisuuksia didaktiikan ja konstruktivistisen oppimisteorian näkökulmasta. Tutkimuksen empiirinen osuus koostuu kolmesta opetuskokeilusta. Laajin kokeilu järjestettiin kahdella helsinkiläisellä peruskoulun ala-asteen 2. luokalla. Menetelmällisenä viitekehyksenä oli neljä sykliä käsittävä opetus- ja arviointikäytäntöjen kehittämiseen pyrkivä toimintatutkimus, jossa jokainen sykli pitää sisällään kvantitatiivisesti suoritetun oppimisen tarkastelun. Tiedonhankintamenetelminä käytettiin oppilaiden itsearviointia, erilaisia haastatteluja ja ääninauhan tukemaan strukturoitua päiväkirjaa pyrkien menetelmälliseen triangulaatioon. Näitä tuloksia varmennettiin kahdella lyhyemmällä opetuskokeilulla, joista toisessa tutkittavat olivat viidesluokkalaisia ja toisessa luokan- ja lastentarhanopettajiksi opiskelevia. Tutkimus tuotti kokemuksia Empirica Controlin opetuskäytöstä sekä laitteiston kehittämisehdotuksia. Tulosten mukaan oppilailla oli työskentelyn vaatiman teknologiaan, tietotekniikkaan ja rakentamisee nliittyvät perusvalmiudet. He kokivat työskentelyn positiivisena ja oppivat käyttämään ohjelmointikielen yksinkertaisimpia käskyjä. Lyhyemmät opetuskokeilut antoivat samansuuntaisia tuloksia. Teknologian oppimista tutkittiin automaatioteknologisten laitteiden toiminnasta annettujen selitysten kautta. Selityksissä havaittiin vain hienoista paranemista. Suun kautta annosteltava kalsiumherkistäjä parantaa sydämen vajaatoimintaan liittyvää pumppausvajetta kokeellisissa sydämen vajaatoimintamalleissa Huolimatta viime vuosikymmenien lääketieteellisestä kehityksestä krooninen sydämen vajaatoiminta on silti edelleen vakava, elämänlaatua voimakkaasti rajoittava sairaus. Kalsiumherkistäjät ovat uusi, sydämen pumppausvoimaa lisäävä lääkeryhmä. Levosimendaani, kotimaista alkuperää oleva kalsiumherkistäjä, on kliinisessä käytössä akuutin vajaatoiminnan hoitoon suonensisäisesti ja lyhytaikaisesti annosteltavana valmisteena. Levosimendaanilla on aktiivinen metaboliitti, OR-1896, jonka oletetaan olevan vuorokauden mittaisen levosimendaani-infuusion jälkeen havaittujen useita päiviä kestävien hyödyllisisten vaikutuksisten takana. Levosimendaanin kroonisen, suun kautta tapahtuvan annostelun vaikutuksista tieto on vähäisempää, mutta sillä näyttää olevan positiivisia vaikutuksia potilaiden raportoimana. FM Marjut Louhelainen on selvittänyt väitöskirjassaan suun kautta annosteltavan levosimendaanin ja sen pitkäkestoisen aktiivisen metaboliitin vaikutuksia kroonisen vajaatoiminnan hoidossa käyttämällä sekä hypertensiivisen sydäntaudin että 2 tyypin diabeteksen komplisoimaan sydäninfarktin kokeellisia malleja. Tutkimuksessa selvitettiin lisäksi vajaatoimintaan johtavia molekyylitason tapahtumia sydänlihaksessa. Tutkimuksessa osoitettiin, että krooninen suun kautta annosteltu hoito sekä kalsiumherkistäjä levosimendaanilla että sen aktiivisella metaboliitilla estää hypertensiiviseen sydämen vajaatoiminnan aikaasaamaa sydämen uudelleenmuovaantumista ja siihen liittyvää kuolleisuutta. Nämä vaikutukset välittyivät vähentyneen sydänlihassoluhypertrofian, solukuolleisuuden ja neurohumaraalisen aktivaation kautta. Levosimendaanin ja OR-1896:n osoitettiin myös parantavan sydämen pumppausfunktiota tyyppi 2 diabeteksen komplisoimassa sydäninfarktissa. Ei-diabeettiseen tilanteeseen verrattuna diabetekseen liittyvä infarktin jälkeinen vajaatoiminnan kehitys oli yhteydessä lisääntyneeseen tulehdukseen, fibroosiin, solukuolemaan, neurohumoraaliseen aktivaatioon ja ennenaikaiseen kudoksen vanhenemiseen. Sekä levosimendaani, että OR-1869 vähensivät tulehduksen, fibroosin ja solukuoleman merkkejä ja vaimensi neurohumoraalista aktivaatiota. OR-1896 myös vähensi solujen vanhenemiseen liittyvien merkkiaineiden ilmentymistä. Väitöskirjassa todettiin, että suun kautta annosteltuna sekä levosimendaani, että sen aktiivinen metaboliitti OR-1896, omaavat terapeuttista potentiaalia sekä hypertensiivisen sydäntaudin hoitoon että sydäninfarktin jälkeisen vajaatoiminnan estoon. FM Marjut Louhelaisen farmakologian alaan kuuluva väitöskirja Effects of oral calcium sensitizers on experimental heart failure tarkastetaan Helsingin yliopiston Lääketieteellisessä tiedekunnassa perjantaina 29.01.2010 klo 12 (Biomedicum Helsinki, luentosali 2, Haartmaninkatu 8, Helsinki).
