Introduction Poor medication adherence is common in children and adolescents with chronic illness, but there is uncertainty about the best way to enhance medication adherence in this group. The authors conducted a systematic review of controlled trials examining interventions that aim to improve medication adherence. Method A comprehensive literature search was undertaken to locate controlled trials that described specific interventions aiming to improve adherence to long-term medication, where participants were aged 18 years and under, medication adherence was reported as an outcome measure, and which could be implemented by individual health practitioners. Studies were reviewed for quality and outcome. Results 17 studies met inclusion criteria: seven studies examined educational strategies, seven studies examined behavioural interventions and three studies examined educational intervention combined with other forms of psychological therapies. Only two of seven studies reported a clear benefit for education on medication adherence, whereas four of seven trials indicated a benefit of behavioural approaches on medication adherence. One trial reported that combining education with behavioural management may be more effective than education alone. Studies which combined education with other non-medication specific psychological interventions failed to demonstrate a beneficial effect on medication adherence. Only two studies examined adherence-promoting interventions in young people with established adherence problems. Conclusion These findings suggest that education interventions alone are insufficient to promote adherence in children and adolescents, and that incorporating a behavioural component to adherence interventions may increase potential efficacy. Future research should examine interventions in high-risk groups.

This article explains the essence of the context-sensitive parameters and dimensions in play at the time of an intervention, through the application of Rog’s (2012) model of contextual parameters. Rog’s model offers evaluators a structured approach to examine an intervention. The initial study provided a systematic way to clarify the scope, variables, timing, and appropriate evaluation methodology to evaluate the implementation of a government policy. Given that the government implementation of an educational intervention under study did not follow the experimental research approach, nor the double cycle of action research approach, the application of Rog’s model provided an in-depth understanding of the context-sensitive environment; it is from this clear purpose that the broader evaluation was conducted. Overall, when governments or institutions implement policy to invoke educational change (and this intervention is not guided by an appropriate evaluation approach), then program evaluation is achievable post-implementation. In this situation, Rog’s (2012) model of contextual parameters is a useful way to achieve clarity of purpose to guide the program evaluation.

Varhaislapsuuden karies ja sen ehkäisy kehittyvän terveydenhuollon maassa Varhaislapsuuden karies on merkittävä kansanterveysongelma varsinkin lapsirikkaissa maissa ja väestöissä. Karieksen hoitaminen vie paljon voimavaroja ja aiheuttaa mittavia taloudellisia seuraamuksia. Karies voi ilmaantua lapselle jo vauvaikäisenä, pian ensimmäisten maitohampaiden puhjettua suuhun. Alle 3-vuotiaiden karieksesta on kuitenkin niukasti tilastotietoja. Maailman terveysjärjestökin suosittaa tietojen keräämistä vasta 3-vuotiaiden ikäryhmästä. Heistä kariesta sairastaa Suomessa 16 %, Yhdysvalloissa 25 %, Englannissa 30 %, Iranissa 46 % ja Saudi-Arabiassa 61 %. Tämä väitöstutkimus selvitti karieksen esiintymistä ja sen vaaratekijöitä 1─3-vuotiailla Teheranissa. Lisäksi tutkimus arvioi perusterveydenhuoltoon sisällytetyn karieksen ehkäisyn tuloksellisuutta. Tutkimuskohteiksi arvottiin Teheranista 18 neuvolaa. Jokaisessa oltiin 4 päivää, jolloin kaikkia rokotuksiin tulleita 1─3-vuotiaita äiteineen pyydettiin osallistumaan tutkimukseen. Kahta lukuun ottamatta kaikki äidit suostuivat, ja aineistoon tuli kaikkiaan 504 lasta äiteineen. Kaikki 1-vuotiaat, 242 lasta äiteineen, valittiin karieksen ehkäisykokeiluun. Sitä varten neuvolat jaettiin kolmeen ryhmään, joista kaksi (A ja B) oli koeryhmiä ja yksi (C) oli vertailuryhmä. Tutkimus alkoi äidin haastattelulla. Siinä selvitettiin perheen koulutus- ja tulotaso sekä lapsen ruokinnasta imetyksen kesto, yösyötöt ja päiväaikaan nautitut makeat. Vielä kysyttiin lapsen ja äidin suuhygieniatavoista ja äidin kokemuksista lapsen suun puhdistamisessa. Sitten hammaslääkäri tutki lapsen suun ja kirjasi karieksen ja hammasplakin esiintymät. Suun tutkimuksen jälkeen äiti ja lapsi siirtyivät rokotushuoneeseen. Koeryhmissä (A ja B) äidit saivat terveydenhoitajalta suunterveyttä koskevan esitteen ja kehotuksen lukea se huolellisesti. Lisäksi ryhmässä A terveydenhoitaja kertoi suun ja hampaiden terveydenhoidosta saman esitteen avulla, ja neuvolan henkilökunta muistutti suunhoidon tärkeydestä puhelimitse kahdesti seuraavan puolen vuoden kuluessa. Vertailuryhmässä äideille ei annettu suunhoidon ohjeita. Kaikissa ryhmissä äitejä muistutettiin seuraavan rokotuskerran ajankohdasta, muttei mainittu tulevaa toista hammastarkastusta. Varhaislapsuuden kariesta sairasti ikäryhmästä riippuen 3─26 % tutkituista 1─3-vuotiaista, ja 65─76 %:lla oli hammasplakkia. Äideistä 68 % harjasi hampaansa päivittäin ja 39 % puhdisti lapsensa suun päivittäin. Mitä useammin äiti harjasi omat hampaansa, sitä paremmin hän huolehti lapsen suun puhtaudesta. Rintaruokinta oli yleistä eikä lisännyt kariesvaaraa. Yöllä pullomaitoa saavilla karies oli 5 kertaa yleisempää kuin muilla. Neuvolassa saatu ohjeistus ehkäisi selvästi karieksen syntyä puolen vuoden kokeessa.

The aim of the present study was to assess dental health and its determinants among 15-year-olds in Tehran, Iran and to evaluate the impact of a school-based educational intervention on their oral cleanliness and gingival health. The total sample comprised 506 students. Data collection was performed through a clinical dental examination and a self-administered structured questionnaire. This questionnaire covered the student s background information, socio-economic status, self-perceived dental health, tooth-brushing, and smoking. The clinical dental examination covered caries experience, gingival status, dental plaque status, and orthodontic treatment needs. Participation was voluntary, and all students responded to the questionnaire. Only three students refused the clinical dental examination. The intervention was based on exposing students to dental health education through a leaflet and a videotape designed for the present study. The outcome examinations took place 12 weeks after the baseline among the three groups of the intervention trial (leaflet, videotape, and control). High participation rates at the baseline and scanty drop-outs (7%) in the intervention speak for reliability of the results. Mean value of the DMFT (D=decayed, M=missing, and F=filled teeth) index of the 15-year-olds was 2.1, which comprised DT=0.9, MT=0.2, and FT=1.0 with no gender differences. Dental plaque existed on at least one index tooth of all students, and healthy periodontium (Community Periodontal Index=0) was found in less than 10% of students. Need for caries treatment existed in 40% of students, for scaling in 24%, for oral hygiene instructions in all, and for orthodontic treatment in 26%. Students with the highest level of parents education had fewer dental caries (36% vs. 48%) and less dental plaque (77% vs. 88%). Of all students, 78% assessed their dental health as good or better. Even more of those with their DMFT=0 (73% vs. 27%) and DT=0 (68% vs. 32%) assessed their dental health as good or better. Smokers comprised 5% of the boys and 2% of the girls. Smoking was common among students of less-educated parents (6% vs. 3%). Of all students, 26% reported twice-daily tooth-brushing; girls (38% vs. 15%) and those of higher socio-economic background (33% vs. 17%) did so more frequently. The best predictors for a good level of oral cleanliness were female gender or twice-daily tooth-brushing. The present study demonstrated that a school-based educational intervention can be effective in the short term in improving the oral cleanliness and gingival health of adolescents. At least 50% reduction in numbers of teeth with dental plaque compared to baseline was achieved by 58% of the students in the leaflet group, by 37% in the videotape group, and by 10% of the controls. Corresponding figures for gingival bleeding were 72%, 64%, and 30%. For improving the oral cleanliness and gingival health of adolescents in countries such as Iran with a developing oral health system, school-based educational intervention should be established with focus on oral self-care and oral health education messages. Emphasizing the immediate gains from good oral hygiene, such as fresh breath, clean teeth, and attractive appearance should be key aspects for motivating these adolescents to learn and maintain good dental health, whilst in planning school-based dental health intervention, special attention should be given to boys and those with lower socio-economic status. Fatigue fracture is an overuse injury commonly encountered in military and sports medicine, and known to relate to intensive or recently intensified physical activity. Bone responds to increased stress by enhanced remodeling. If physical stress exceeds bone s capability to remodel, accumulation of microfractures can lead to bone fatigue and stress fracture. Clinical diagnosis of stress fractures is complex and based on patient s anamnesis and radiological imaging. Bone stress fractures are mostly low-risk injuries, healing well after non-operative management, yet, occurring in high-risk areas, stress fractures can progress to displacement, often necessitating surgical treatment and resulting in prolonged morbidity. In the current study, the role of vitamin D as a predisposing factor for fatigue fractures was assessed using serum 25OHD level as the index. The average serum 25OHD concentration was significantly lower in conscripts with fatigue fracture than in controls. Evaluating TRACP-5b bone resorption marker as indicator of fatigue fractures, patients with elevated serum TRACP-5b levels had eight times higher probability of sustaining a stress fracture than controls. Among the 154 patients with exercise induced anterior lower leg pain and no previous findings on plain radiography, MRI revealed a total of 143 bone stress injuries in 86 patients. In 99% of the cases, injuries were in the tibia, 57% in the distal third of the tibial shaft. In patients with injury, forty-nine (57%) patients exhibited bilateral stress injuries. In a 20-year follow-up, the incidence of femoral neck fatigue fractures prior to the Finnish Defence Forces new regimen in 1986 addressing prevention of these fractures was 20.8/100,000, but rose to 53.2/100,000 afterwards, a significant 2.6-fold increase. In nineteen subjects with displaced femoral neck fatigue fractures, ten early local complications (in first postoperative year) were evident, and after the first postoperative year, osteonecrosis of the femoral head in six and osteoarthritis of the hip in thirteen patients were found. It seems likely that low vitamin D levels are related to fatigue fractures, and that an increasing trend exists between TRACP-5b bone resorption marker elevation and fatigue fracture incidence. Though seldom detected by plain radiography, fatigue fractures often underlie unclear lower leg stress-related pain occurring in the distal parts of the tibia. Femoral neck fatigue fractures, when displaced, lead to long-term morbidity in a high percentage of patients, whereas, when non-displaced, they do not predispose patients to subsequent adverse complications. Importantly, an educational intervention can diminish the incidence of fracture displacement by enhancing awareness and providing instructions for earlier diagnosis of fatigue fractures.

Background: The national resuscitation guidelines were published in Finland in 2002 and are based on international guidelines published in 2000. The main goal of the national guidelines, available on the Internet free of charge, is early defibrillation by nurses in an institutional setting. Aim: To study possible changes in cardiopulmonary resuscitation (CPR) practices, especially concerning early defibrillation, nurses and students attitudes of guideline implementation and nurses and students ability to implement the guideline recommendations in clinical practices after publication of the Current Care (CC) guidelines for CPR 2002. Material and methods: CPR practices in Finnish health centres; especially concerning rapid defibrillation programmes, as well as the implementation of CC guidelines for CPR was studied in a mail survey to chief physicians of every health centre in Finland (Study I). The CPR skills using an automated external defibrillator (AED) were compared in a study including Objective stuctured clinical examination (OSCE) of resuscitation skills of nurses and nursing students in Finnish and Swedish hospital and institution (Studies II, III). Attitudes towards CPR-D and CPR guidelines among medical and nursing students and secondary hospital nurses were studied in surveys (Studies IV, V). The nurses receiving different CPR training were compared in a randomized trial including OSCE of CPR skills of nurses in Finnish Hospital (Study VI). Results: Two years after the publication, 40.7% of Finnish health centres used national resuscitation guidelines. The proportion of health centres having at least one AED (66%) and principle of nurse-performed defibrillation without the presence of a physician (42%) had increased. The CPR-D training was estimated to be insufficient regarding basic life support and advanced life support in the majority of health centres (Study I). CPR-D skills of nurses and nursing students in two specific Swedish and Finnish hospitals and institutions (Study II and III) were generally inadequate. The nurses performed better than the students and the Swedish nurses surpassed the Finnish ones. Geriatric nurses receiving traditional CPR-D training performed better than those receiving an Internet-based course but both groups failed to defibrillate within 60 s. Thus, the performance was not satisfactory even two weeks after traditional training (Study VI). Unlike the medical students, the nursing students did not feel competent to perform procedures recommended in the cardiopulmonary resuscitation guidelines including the defibrillation. However, the majority of nursing students felt confident about their ability to perform basic life support. The perceived ability to defibrillate correlated significantly with a positive attitude towards nurse-performed defibrillation and negatively with fear of damaging the patient s heart by defibrillation (Study IV). After the educational intervention, the nurses found their level of CPR-D capability more sufficient than before and felt more confident about their ability to perform defibrillation themselves. A negative attitude toward defibrillation correlated with perceived negative organisational attitudes toward cardiopulmonary resuscitation guidelines. After CPR-D education in the hospital, the majority (64%) of nurses hesitated to perform defibrillation because of anxiety and 27 % hesitated because of fear of injuring the patient. Also a negative personal attitude towards guidelines increased markedly after education (Study V). Conclusions: Although a significant change had occurred in resuscitation practices in primary health care after publication of national cardiopulmonary resuscitation guidelines the participants CPR-D skills were not adequate according to the CPR guidelines. The current way of teaching is unlikely to result in participants being able to perform adequate and rapid CPR-D. More information and more frequent training are needed to diminish anxiety concerning defibrillation. Negative beliefs and attitudes toward defibrillation affect the nursing students and nurses attitudes toward cardiopulmonary resuscitation guidelines. CPR-D education increased the participants self-confidence concerning CPR-D skills but it did not reduce their anxiety. AEDs have replaced the manual defibrillators in most institutions, but in spite of the modern devices the anxiety still exists. Basic education does not provide nursing students with adequate CPR-D skills. Thus, frequent training in the workplace has vital importance. This multi-professional program supported by the administration might provide better CPR-D skills. Distance learning alone cannot substitute for traditional small-group learning, tutored hands-on training is needed to learn practical CPR-D skills. Standardized testing would probably help controlling the quality of learning. Training of group-working skills might improve CPR performance.

ABSTRACT Bakhshandeh, Soheila. Periodontal and dental health and oral self-care among adults with diabetes mellitus. The aim of the present study was to assess oral health and treatment needs among Iranian adults with diabetes according to socio-demographic status, oral hygiene, diabetes related factors, and to investigate the relation between these determinants and oral health. Moreover, the effect of an educational oral health promotion intervention on their oral health and periodontal treatment needs was studied. The target population comprised adults with diabetes in Tehran, Iran. 299 dentate patients with diabetes, who were regular attendants to a diabetic clinic, were selected as the study subjects. Data collection was performed through a clinical dental examination and self-administered structured questionnaire. The questionnaire covered information of the subject s social background, medical history, oral health behaviour and smoking. The clinical dental examinations covered the registration of caries experience (DMFT), community periodontal index (CPI) and plaque index (PI). The intervention provided the adults with diabetes dental health education through a booklet. Reduction in periodontal treatment needs one year after the baseline examination was used as the main outcome. A high prevalence of periodontal pockets among the study population was found; 52% of the participants had periodontal pockets with a pocket depth of 4 to 5 mm and 35% had periodontal pockets with pocket depth of 6 mm or more. The mean of the DMFT index was 12.9 (SD=6.1), being dominated by filled teeth (mean 6.5) and missing teeth (mean 5.0). Oral self-care among adults with diabetes was inadequate and poor oral hygiene was observed in more than 80% of the subjects. The educational oral health promotion decreased periodontal treatment needs more in the study groups than in the control group. The poor periodontal health, poor oral hygiene and insufficient oral self-care observed in this study call for oral health promotion among adult with diabetes. An educational intervention showed that it is possible to promote oral health behaviour and to reduce periodontal treatment needs among adults with diabetes. The simplicity of the model used in this study allows it to be integrated to diabetes programmes in particular in countries with a developing health care system.

A formação inicial de professores se constitui como objeto desta tese. Na presente pesquisa pretendi planejar, implementar e avaliar a eficácia de um programa de formação inicial de professores para atuar com Tecnologia Assistiva, principalmente com a Comunicação Alternativa e Ampliada, por meio de metodologia problematizadora. Para responder à pergunta principal do estudo A formação inicial de professores para atuar com TA, principalmente com a CAA, através de metodologia problematizadora favorece a modificação das preconcepções sobre deficiência e práticas desses futuros professores? foi necessário conhecer as concepções dos alunos de Pedagogia, assim como envolvê-los no planejamento do curso. Outro elemento fundamental na proposta de formação foi a Metodologia da Problematização aplicada a questões educacionais , que estimula o trabalho reflexivo, criativo e colaborativo. A pesquisa foi desenvolvida em dois estudos, durante o período de agosto de 2008 a dezembro de 2010. Uma pesquisa ação foi desenvolvida com 37 alunos da graduação do curso de Pedagogia, duas professoras responsáveis pela disciplina, 26 alunos com deficiência sem fala articulada, com idades entre 8 e 32 anos, cinco professoras de uma escola especial da rede pública de ensino. Os estudos foram desenvolvidos na Faculdade de Educação da UERJ em salas de aula e no Laboratório de Tecnologia Assistiva/Comunicação Alternativa (Lateca) e numa escola especial. Foram utilizados questionários, filmagens, vídeos e vários recursos de tecnologia assistiva. Os procedimentos metodológicos foram os seguintes: 1. Aplicação de questionário com objetivo duplo caracterizar os graduandos de Pedagogia e apreender suas concepções a respeito de Educação Inclusiva, deficiência, TA e CAA. 2. Com base na análise dos questionários, levantei o perfil dos alunos, suas expectativas em relação a essa formação e as sugestões que direcionaram o planejamento e a implementação da oferta das aulas expositivas e das atividades práticas. 3. Oferta de aulas expositivas, acompanhadas de farto material audiovisual e do desenvolvimento de atividades práticas, que versaram sobre os seguintes temas: linguagem, comunicação e interação, deficiência, TA e CAA. 4. Os graduandos observaram, planejaram e desenvolveram uma proposta de intervenção direta para alunos com deficiência. Os estudos apontaram que houve modificação das concepções dos graduandos em relação aos conceitos de deficiência, TA e CAA. Os dados revelaram ainda que esse trabalho proporcionou aos graduandos uma oportunidade real de exercício da prática, com as seguintes características: partindo da observação da realidade de uma sala de aula ou sala de atendimento especializado, identificar problemas pedagógicos e escolher um deles como foco de uma investigação; refletir sobre os possíveis fatores e determinantes principais do problema selecionado e definir os pontos-chave do estudo; investigar esses pontos-chave; buscar informações em diversas fontes e analisá-las para responder ao problema, compondo, assim, a teorização; elaborar hipóteses de solução para o problema; e, por fim, aplicar uma ou mais das hipóteses de solução, como um retorno do estudo à realidade investigada. Verifiquei também, ao final do estudo, a imensa gama de recursos e estratégias de CAA, a adequação de material pedagógico e os recursos de acesso ao computador que foram criados pelos graduandos nesse atendimento aos alunos com deficiência. O estudo beneficiou, ainda, os professores e alunos da escola especial que, além de conhecer os recursos de TA e CAA, puderam vivenciar essa abordagem educacional diferenciada, proposta na formação inicial. Além disso, observei que os alunos com deficiência ampliaram as oportunidades de comunicação e interação social.

[es] Este trabajo presenta una intervención educativa realizada en una Ikastola del País Vasco. Su propósito es atender las necesidades que en el ámbito de la educación emocional presentan los niños y niñas de la escuela de primaria. Con este punto de partida, el objeto de estudio se sitúa en la técnica del mindfulness como herramienta educativa para el desarrollo de las competencias emocionales. El proceso de elaboración de la propuesta ha requerido buscar información sobre el objeto de estudio y elaborar un diagnóstico de necesidades a partir del cual concretar los objetivos de la intervención. Entre los resultados obtenidos cabe destacar que el alumnado participante ha sido capaz de identificar y verbalizar diferentes sentimientos y emociones con las que ha entrado en contacto en la práctica del mindfulness.

Contraceptive prevalence in Haiti remains low despite extensive foreign aid targeted at improving family planning. [1] Earlier studies have found that peer-informed learning have been successful in promoting sexual and reproductive health. [2-5] This pilot project was implemented as a three-month, community-based, educational intervention to assess the impact of peer education in increasing contraceptive knowledge among women in Fondwa, Haiti. Research investigators conducted contraceptive information trainings to pre-identified female leaders of existing women’s groups in Fondwa, who were recruited as peer educators (n=4). Later, these female leaders shared the knowledge from the training with the test participants in the women’s group (n=23) through an information session. Structured surveys measuring knowledge of contraceptives were conducted with all participants before the intervention began, at the end of the intervention, and four weeks after the intervention. The surveys measured general contraceptive knowledge, knowledge about eight selected types of modern contraceptives and contraceptive preferences and attitudes. Only test participants showed significant improvement in their general contraceptive knowledge score (p<0.001), but both test participants and peer educators showed significant improvement in overall knowledge scores for identifying the types and uses of modern contraceptive methods. Assessment for knowledge retention remained significantly higher four weeks after the intervention than prior to the intervention. Therefore, a one-time, three-hour peer-based educational intervention using existing social structures is effective, and might be valuable in a population with minimal access to education and little to no knowledge about contraceptives.

Since remote times, certain sectors of society have been exposed to inequality and vulnerability, where adequate intervention processes have become conspicuous because of their absence. Nowadays, current societies have the responsibility of contributing, based on their experience and knowledge, with more efficient policies and programs that improve the life quality of the most disadvantaged. It is here where art and its different tools play a very important role, not only on a physical level, but also as an education tool that allows the development of emotional, mental and communicative skills. The aim of this paper is to make clear the potential of art as an instrument of social and educational intervention. It starts by showing worldwide-collected experience related to education and arts, and then, it acquaints the reader with two parallel intervention projects that worked with youths under social vulnerability conditions. These interventions were developed based on a qualitative research (Grounded theory), using as methodology “The Artistic Mediation” with emphasis on body language. This methodology helped researchers to get close to the participants and to know their experiences and emotions. At the same time, it was possible to evidence the positive effects of educative interventions through art. These workshops were based on an artistic methodology especially focused on body language. Data in this work is qualitative, and as such, it permits a special approach to the personal and emotional experiences of the participants; clearly showing the positive effects of the referenced practice on them.

Peace education initiatives in schools are often based on social psychological theories assuming that social identity affects ingroup and outgroup attitudes and, in turn, behaviors relating to relevant outgroups. However, research evidence on the role of young people’s social identity has often failed to take account of different social identity dimensions or to conceptualise outgroup behavior in the context of young people’s understandings of the social world. While recent research relating to bullying and bystander behavior amongst young people has addressed the latter point, this has rarely been considered in conjunction with a differentiated view of social identity. This paper is therefore distinctive as it will address the role of social identity dimensions with regards to behavior as captured in bystander scenarios relating to intergroup discrimination. This is particularly important in the context of divided societies, where peace education initiatives are crucial in promoting positive community relations for the future and where such initiative may be hampered by communities’ concerns about loss of identity and hardened intergroup attitudes. Furthermore, previous research frequently highlighted teachers’ fears to tackle outgroup attitudes in the classroom, especially in contexts where pupils and the wider school community are seen as entrenched in community divisions (Hughes, Donnelly, Hewstone, Gallagher & Carlisle, 2010). However, there is no research investigating the relationship between pupils’ attitudes and teacher confidence to talk about such issues in the classroom, which is explored in this paper.

In the context of Northern Ireland, a divided society emerging from sectarian conflict, social identity, outgroup attitudes and outgroup behaviours have been key concepts addressed by peace educators for many years. Building on this work, this paper provides a detailed picture of young people’s strength of group identification and their willingness to explore ingroup perspectives, sectarian attitudes and their reported willingness to challenge sectarian bullying. Using data from a baseline survey, which forms part of a randomized control trial investigating the effect of an educational intervention aimed to promote reconciliation, the sample involved young people of different denominational backgrounds, attending separate school sectors. The baseline data will be used to compare with post intervention data. Therefore, the data and its findings would be of particular interest to educators and policy makers in other European countries who are working to develop peace education interventions in societies emerging from conflict. Additionally, this paper considers the results from a baseline teacher survey, collected before training and teaching of the intervention began. The teacher survey focused on confidence in tackling sectarian issues in the classroom, previous experience of teaching such issues and their hopes and concerns for the reconciliation intervention.

This paper therefore set out to investigate the relationship between dimensional concept of social identity, sectarian attitudes and pupils’ reported willingness to challenge sectarian bullying and to compare this with their teachers’ attitudes to teaching about sectarian issues in the classroom.



Method

The pupil sample included 35 primary and post-primary schools and about 800 pupils from 8-11 years old who completed an online questionnaire in December 2011 and January 2012. Main instruments for young people’s survey included an adapted version of the Multi-ethnic identity measure (Phinney, 1992) incorporating the dimensions identity affirmation and exploration, outgroup attitude scales, including an adaptation of the social distance measure (Bogardus, 1947), as well as a measure adapted from Palmer and Cameron (2011) involving scenarios to capture pupils’ intentions in bystander situations relating to intergroup discrimination. Results are analysed using regression analysis and take account of potential gender and religious differences. The teacher questionnaire was completed by the 35 primary and post-primary teachers who will deliver the intervention. Results are analysed in terms of how teachers’ responses compare with their pupil attitudes by considering their confidence in tackling sectarian issues in the classroom and how their previous experience and training relate to their hopes and concerns for the intervention.


Expected Outcomes

Results from the young people’s survey are discussed in the light of the role of social identity dimensions and their relationship to sectarian attitudes and reported bystander behaviour in sectarian school incidents. Furthermore, results related to pupils’ sectarian attitudes will be compared with teachers’ reported confidence in tackling sectarianism in the classroom. The teacher questionnaire also presents interesting findings in relation to teachers’ previous training and experience and how this may influence different perspectives on peace and reconciliation interventions and their expectations of what these could achieve. The paper concludes with potential implications for peace education initiatives and related teacher training in Northern Ireland and beyond. The implications will be of particular interest to policy makers, educators and those working in the area of peace education to design and implement interventions.

BACKGROUND: Palliative care is expected to incorporate comprehensive support for family caregivers given that many caregivers suffer psychological morbidity. However, systematically implemented evidence-based psychological support initiatives are lacking.
AIM: The objective of this study was to prepare caregivers for the role of supporting a patient with advanced cancer receiving home-based palliative care by offering a one-to-one psycho-educational intervention. We hypothesised that primary family caregivers who participated in the intervention would report decreased psychological distress (primary outcome), fewer unmet needs and increased levels of perceived preparedness, competence and positive emotions.
METHODS: A three-arm randomised controlled trial was conducted comparing two versions of the intervention (one face-to-face visit versus two visits) plus standard care to a control group (standard care) across four sites in Australia.
RESULTS: A total of 298 participants were recruited; 148 were in the Control condition, 57 in Intervention 1 (one visit) and 93 in Intervention 2 (two visits). Relative to participants in the control group; the psychological well-being of participants in the intervention condition was improved by a small amount but non-significantly. No significant reduction in unmet needs or improvements in positive aspects of caregiving amongst the intervention group were identified. However, the intervention demonstrated significant improvements in participants' levels of preparedness and competence for Intervention 2.
CONCLUSION/IMPLICATIONS: This research adds to accumulating body of evidence demonstrating that relatively short psycho-educational interventions can enable family caregivers to feel more prepared and competent in the role of supporting a dying relative. Further investigation is required to determine the longer term outcomes of such interventions.

This paper considers the potential for improving the reputation of the Irish accountancy profession by exploring undergraduate accounting students’ intolerance of academic cheating as a predictor of future attitudes to unethical workplace practices. The study reports that females are significantly more intolerant of cheating than males. Further, with regard to ethical ideology, idealism was found to have a significant positive association with intolerance of cheating while relativism reported no association. It is anticipated that the growing admission of women to professional accountancy membership together with educational intervention to increase idealism may improve ethical attitudes and help restore the profession's reputation.

O estudo Educação de Infância como tempo fundador: Repensar a Formação de Educadores para uma acção educativa integrada inscreve-se no processo de reflexão acerca das finalidades da educação básica e da qualidade da formação e da intervenção dos profissionais de educação de infância. Considerando as condições de instabilidade, crescente insegurança e grande imprevisibilidade que caracterizam as sociedades contemporâneas e, considerando também, os avanços científicos que vêm ocorrendo nas últimas décadas, fundamentando e alertando para a importância decisiva das aprendizagens realizadas na Infância nos processos de desenvolvimento subsequentes, releva-se neste estudo a necessidade de dar continuidade e aprofundar essa reflexão procurando responder, também pela investigação, aos desafios que as mudanças sociais suscitam. Nesta linha e tendo como referente o significado que, em termos de estruturação identitária pessoal e colectiva, é reconhecido à educação de infância, enquanto contexto primeiro de educação básica e complementar da acção educativa da família, o principal objectivo do estudo consiste em aprofundar o conhecimento acerca da natureza e qualidade dos saberes básicos a promover na educação pré-escolar e das competências reconfiguradoras do perfil de desempenho profissional dos educadores de infância para que, em articulação com as famílias das crianças, se tornem facilitadores do seu desenvolvimento, no quadro de uma ampla perspectiva de cidadania e de sucesso para todos. Os eixos investigativos que se cruzam no estudo pressupõem uma dimensão de pesquisa (teórica, documental e empírica) de natureza complexa, na qual, se procura tornar compreensíveis as interacções entre os participantes anteriormente referidos, no sentido de uma possível coerência conceptual e funcional, que regule e sustente a qualidade dos processos de desenvolvimento. Do ponto de vista metodológico, a investigação inscreve-se numa abordagem de natureza qualitativa, de matriz complexa e com características de estudo de caso, centrado nos processos de formação e de intervenção dos educadores de infância em exercício de funções, no distrito de Bragança. No sentido de construir uma visão integrada do objecto de estudo foi desenvolvida uma revisão temática de literatura e de análise documental e, na dimensão empírica do estudo, foram promovidos processos mistos de recolha de dados, com recurso à inquirição por questionário e por entrevista (semi-estruturada). A inquirição por questionário foi feita a 229 educadores de infância e a 1340 pais (ou seus representantes), das crianças que frequentavam a educação pré-escolar e a entrevista a 6 educadoras, que integravam os conselhos executivos dos Agrupamentos de Escolas e, cujas funções de gestão e administração, lhes permitiam ter uma perspectiva mais global das problemáticas em estudo. Os instrumentos de recolha e de análise da informação foram validados de modo a garantir-lhes fiabilidade e credibilidade. Os resultados do estudo podem ser lidos em dois níveis, considerando a sua abrangência e especificidade. Num primeiro plano, numa leitura mais global e transversal às questões em estudo e, num segundo plano, como enfoque mais específico em função de quatro dimensões temáticas decorrentes do quadro de fundamentação teórica e organizadoras do processo de reflexão e de pesquisa. Assim, globalmente, os resultados confirmam a importância que todos os inquiridos reconhecem, quer às aprendizagens ocorridas na Infância, como factor importante no desenvolvimento pessoal e social das crianças ao longo da vida, quer ao papel que, nele, os educadores e respectiva formação (inicial e contínua) devem desempenhar. Com algumas variações, as representações dos educadores de infância e dos pais inquiridos neste estudo, embora diferentes em algumas das questões específicas, apresentam-se maioritariamente coerentes e próximas das perspectivas teóricas mais actuais, que consideram a natureza processual das aprendizagens e a importância que a qualidade dos contextos e das transições, que neles ocorrem, assumem nos processos de desenvolvimento. Ou seja, inscrevem-se na linha das teorias socioconstrutivista e ecológica também subjacentes às orientações curriculares, ao nível nacional e aos quadros teóricos de referência, ao nível internacional. Identificam a aprendizagem da cidadania (ou do aprender a ser em sociedade) como o saber mais estruturante a ser desenvolvido no conjunto da acção educativa e perspectivam-na como processo de responsabilidade partilhada e cooperado. Tratando-se de uma amostra extensa e de um distrito geograficamente marcado pela interioridade, e sem esquecer que os dados se referem a representações expressas ao nível dos discursos, é importante reconhecer os sinais de actualidade das perspectivas e das sugestões apontadas para dar continuidade aos processos de desenvolvimento integrado de todos os participantes no processo educativo. Ainda numa leitura global, as principais diferenças, genericamente observadas entre educadores e pais, evidenciam, da parte destes, uma perspectiva de cidadania mais restrita e, da parte dos educadores, uma visão mais alargada do conceito. Com efeito, são os pais com mais elevada qualificação académica que partilham com os educadores esta perspectiva ampliada e transformadora de cidadania. Numa leitura mais enfocada e mais detalhada, e tal como referido anteriormente, os resultados podem ser lidos no cruzamento de quatro dimensões que interligam as questões de pesquisa: os saberes básicos, as estratégias de intervenção para o seu desenvolvimento; a formação e intervenção dos educadores de infância e a identificação de competências que possam vir a aprofundarem a sua formação. No que se refere aos saberes básicos, e não obstante a ocorrência de variações, quer quanto aos próprios saberes, quer quanto à terminologia usada, são considerados como fundamentais: 1. O aprender a ser na perspectiva do desenvolvimento da identidade; 2. O aprender a exercer a cidadania na linha da aprendizagem e da vivência democrática na relação com o mundo e com o outro; 3. O aprender a aprender como ferramenta indispensável à aprendizagem ao longo da vida; 4. O aprender a desenvolver o pensamento crítico, enquanto possibilidade de criteriosa escolha pessoal entre alternativas possíveis e 5. O aprender a comunicar como condição relacional inalienável nos processos de interacção com os contextos e com as pessoas. A segunda dimensão tem a ver com as estratégias consideradas facilitadoras do desenvolvimento destes saberes e são considerados três níveis da intervenção educativa: a acção dos educadores propriamente dita, a cooperação dos pais no processo de aprendizagem das crianças e a interacção da instituição pré-escolar com os pais/família. A acção dos educadores surge, tendencialmente perspectivada como facilitadora do desenvolvimento dos saberes básicos, embora em relação a algumas práticas essa perspectiva surgisse pouco evidente e distingue-se quanto ao desenvolvimento da acção e relação educativa, manifestando os educadores mais experientes uma opinião mais favorável. No que se refere à cooperação dos pais no processo de aprendizagem das crianças, os resultados indicam que a maioria dos pais manifesta uma opinião favorável a práticas configuradoras de um clima facilitador do desenvolvimento dos saberes básicos enunciados, mas variando os seus pontos de vista. São os pais de habilitações académicas mais elevadas, de idade intermédia e situados em contexto urbano os que apresentam opiniões mais favoráveis. Por fim, e no que diz respeito à interacção com as famílias, os resultados evidenciam uma opinião positiva com os meios de interacção utilizados, mas deixando perceber a necessidade de melhorar o processo de cooperação, manifestando os pais uma opinião menos positiva do que os educadores sobre esse processo. A terceira dimensão diz respeito às representações sobre a formação e intervenção profissional dos educadores, evidenciando os resultados que a maioria dos educadores atribuiu muita relevância aos contributos do curso de formação inicial para o desenvolvimento da maioria das competências necessárias para o seu desempenho profissional. Permitem ainda verificar que os educadores de formação mais recente manifestaram uma opinião mais favorável desses contributos, quanto ao desenvolvimento de conhecimentos em áreas, tais como a matemática, conhecimento do mundo e expressão musical, o que significa um avanço relativamente ao reconhecimento da necessidade de fazer investimento nessas áreas sugeridas em alguns estudos e projectos. Quanto ao desenvolvimento da actividade profissional, os resultados relevam que a maior preocupação dos educadores se centra em torno do seu desempenho profissional e das condições de exercício da actividade profissional. No que se refere ao início de carreira, esta última dimensão assume maior evidência, sendo ainda possível perceber que a entrada na vida profissional tem vindo a ocorrer, nos últimos anos, através da rede privada. A quarta dimensão tem a ver com as competências profissionais a desenvolver pelos futuros educadores, e não obstante algumas diferenças nas opiniões manifestadas pelos três grupos de participantes, surgem relevadas competências que a literatura e os perfis de desempenho profissional docente apontam como devendo ser promovidas e incluídas nos programas de formação de educadores de infância/professores. Estas podem ser vistas, quer numa dimensão geral relativa ao grupo docente, quer numa dimensão mais específica da intervenção em educação de infância, tal como é especificado ao longo do trabalho.
