Les DRG, Diagnosis Related Groups, sont une classification permettant de regrouper les hospitalisations de soins aigus en fonction des affections traitées et des coûts engendrés. Ils ont été développes dans les années 70 à l'université de Yale aux Etats-Unis par le professeur Robert Fetter dans le but de standardiser les clientèles hospitalières dans un objectif de contrôle qualité. En Suisse, le financement en APDRG a commencé comme un projet pilote à Lausanne en 2002. Ce dernier a été repris du système déjà introduit aux Etat-Unis. Le 18 janvier 2008, la Suisse a décidé de passer à un financement en DRG au niveau national. La société anonyme SwissDRG a ainsi vu le jour en se basant sur Les G-DRG (=DRG allemand). Le coeur de ce mémoire consiste à déterminer si l'introduction des DRG ne causera pas une perte de qualité dans le système de soins. Afin de répondre à cette question, nous nous sommes basés sur l'expérience des autres pays notamment celle des Etats-Unis. L'étude réalisée par Kahn va être souvent prise en exemple. Les points forts qui en ressortent sont une augmentation de la sévérité des admissions, une baisse de la durée moyenne de séjour, une mortalité inchangée, ainsi que l'augmentation de l'état de santé instable à la sortie de l'hôpital. Ce qui ressort également dans l'ensemble des études documentées dans ce travail, c'est l'augmentation de l'efficience des hôpitaux et le risque qu'il y ait une sélection inverse des patients. While collective identity markers are constantly evolving in today's city, the hyper-consumption society has allowed retail, even though greatly transformed, to remain a community hub for socialization. Society's commodification takes shape precisely within the public realm and the urban scene: sprawling analogous drivable retail-only suburbs and turning city centers into bland, soulless and undefined places displaying homogeneous frontages characterized by the same retail storefronts worldwide. More than ever, urban policies on retail oriented servicescapes seem crucial to urban and regional planning from a sustainable perspective. Thorough the study case of Lausanne, our research focuses on how the urban planning model and its policies could set into place spatial strategies to enhance urban quality and to reorganize retail centralities without succumbing to the prevailing commodification of the public realm. -- Les indicateurs d'identité collective se reformulent dans la ville actuelle, mais la société de l'hyperconsommation a permis au commerce, aussi transformé soit-il, de rester un lieu d'échange et de brassage social. La marchandisation de la société prend des formes précises dans l'espace et a un impact certain sur le paysage urbain : multipliant les espaces marchands périphériques monofonctionnels ou encore transformant nos centres-villes en paysages anodins, présentant des vitrines semblables quel que soit le pays où elles se situent. Plus que jamais, une régulation des espaces de consommation semble incontournable pour l'aménagement du territoire et l'urbanisme dans une perspective de durabilité. À travers le cas lausannois, cette thèse s'interroge sur la façon dont les modèles urbanistiques de régulation des jeux d'acteurs privés peuvent participer à la mise en place de stratégies de qualification urbaine et d'organisation des centralités commerciales sans céder à la tentation de la marchandisation des espaces publics.

Dispersal, i.e. individual movement between breeding sites, is a key process for metapopulation dynamics and gene flow. Its success can be modulated by phenotypic differences between dispersing and philopatric individuals, or dispersal syndromes. However, the environmental (external) and physiological (internal) constraints underlying such syndromes remain poorly known. This project aimed at clarifying the impact of environmental variation and oxidative constraints, linked to the reactive oxygen species produced during respiration, on phenotypes associated to dispersal in a passerine bird, the collared flycatcher Ficedula albicollis. Energetic demand was experimentally (i) increased through a wing load manipulation or (ii) relieved through food supplementation. The oxidative balance of breeding flycatchers was influenced by complex interactions of dispersal status and extrinsic factors (breeding density, year, experimental treatments). Interestingly, antioxidant capacity was influenced both by permanent individual differences and by food availability, whereas measures of pro-oxidants were highly variables within individuals. Environmental variation and energetic constraints also modulated the differences in reproduction associated with dispersal: dispersing and philopatric birds differ in their management of the oxidative balance when it is competing with reproductive investment. This thesis highlights that reaction norms, rather than fixed differences, often shape traits associated to dispersal. ----- Le déplacement d'un individu entre sites de reproduction, ou dispersion, est un processus clé pour la dynamique des métapopulations et les flux de gènes. Son succès peut être modulé par des différences de phénotype, ou syndromes de dispersion. Cependant, les contraintes environnementales et physiologiques qui sous-tendent ces syndromes restent mal connues. Ce projet vise à clarifier l'impact des variations environnementales et des contraintes oxydatives (liées aux espèces réactives de l'oxygène produites durant la respiration) sur les phénotypes associés à la dispersion chez un passereau, le gobemouche à collier Ficedula albicollis. La demande énergétique a été expérimentalement (i) augmentée en manipulant la surface alaire ou (ii) diminuée par une supplémentation en nourriture. L'équilibre oxydo-réducteur des gobemouches en reproduction est influencé par des interactions complexes entre statut de dispersion et facteurs extrinsèques (densité de couples reproducteurs, année, traitement expérimental). La capacité antioxydante dépend principalement de différences permanentes entre individus, alors que les pro-oxydants présentent de grandes variations intra-individu. Environnement et contraintes énergétiques modulent aussi les différences de reproduction liées à la dispersion : les oiseaux dispersants et philopatriques diffèrent dans leur gestion de l'équilibre oxydo-réducteur lorsqu'il est en compétition avec l'investissement reproducteur. Ce travail souligne que les traits associés à la dispersion sont souvent déterminés par des normes de réaction à l'environnement et non des différences fixées entre individus.

The importance of the regional level in research has risen in the last few decades and a vast literature in the fields of, for instance, evolutionary and institutional economics, network theories, innovations and learning systems, as well as sociology, has focused on regional level questions. Recently the policy makers and regional actors have also began to pay increasing attention to the knowledge economy and its needs, in general, and the connectivity and support structures of regional clusters in particular. Nowadays knowledge is generally considered as the most important source of competitive advantage, but even the most specialised forms of knowledge are becoming a short-lived resource for example due to the accelerating pace of technological change. This emphasizes the need of foresight activities in national, regional and organizational levels and the integration of foresight and innovation activities. In regional setting this development sets great challenges especially in those regions having no university and thus usually very limited resources for research activities. Also the research problem of this dissertation is related to the need to better incorporate the information produced by foresight process to facilitate and to be used in regional practice-based innovation processes. This dissertation is a constructive case study the case being Lahti region and a network facilitating innovation policy adopted in that region. Dissertation consists of a summary and five articles and during the research process a construct or a conceptual model for solving this real life problem has been developed. It is also being implemented as part of the network facilitating innovation policy in the Lahti region.

Tämän diplomityön tavoitteena on tutkia kattilalaitosten järjestelmällistä suunnittelua, huomioiden laitoksen elinkaariprosessit ja modulaarinen tuoterakenne. Kattilalaitokset investointituotteina toteutetaan tyypillisesti projektitoimintamallilla, jolloin jokainen toimitus suunnitellaan alusta kyseisen kohteen vaatimusten perusteella. Päätöksenteon peruste on laitosasiakkaiden osalta tyypillisesti hinta, jonkavuoksi laitoksen perusratkaisut tulee olla ennalta määriteltyjä ja toimitusprosessi huolellisesti suunniteltu, jotta kustannukset ja aikataulu ovat ennustettavissa. Projektitoimituksissa projektipäälliköllä on merkittävä vastuu myös suunnittelun onnistumisesta ja käytössä olevat projektinhallintamallit ovat hyvin tuotekeskeisiä, jolloin vaatimuksia ei riittävästi huomioida ja siten toteutettu laitosratkaisu ei täytä usein sidosryhmien asettamia vaatimuksia. Monimutkaisten järjestelmien kehittämiseen ja suunnitteluun on olemassa Systems Engineering - suunnittelumalli, jonka hyödyntäminen energiatekniikan alalla on ollut vielä vähäistä. Malli tarjoaa yhdessä ISO/ICE 15288 standardin kanssa valmiit prosessit tuotteen järjestelmälliselle suunnittelulle. Modulaariset tuoterakenteet ovat olleet perusvaatimus komponenttien toimitusten osalta, mutta laitosjärjestelmien tuotteistaminen on koettu vaikeaksi runsaan räätälöintitarpeen takia. Prosessimaisella toimintamallilla voidaan tuottaa modulaarisia tuoterakenteita ja tehostaa kattilalaitosten muutosten hallintaa. Tutkimuksessa tunnistettiin tyypilliset kattilalaitoksen suunnittelun prosessit kattilalaitosten kokonaistoimituksiin erikoistuneelle KPA Unicon Oy:lle.

Tämä tutkimus käsittelee strategiamuutoksen toimeenpanoa projektiliiketoimintaympäristössä toimivan liiketoimintayksikön tuotekehitys-, suunnittelu- ja tuotetoimitusprosesseissa. Muutoksen tavoitteena on tuoterakenteiden ja toimitusprosessien hallinta pelkän resurssien hallinnan sijaan. Työn tavoitteena on kuvaus siitä, mitä muutoksia strategisten tavoitteiden saavuttaminen edellyttää liiketoimintayksikön tuoteprosessien, tuotearkkitehtuurin ja tietojärjestelmien osalta. Tutkimuksen teoriaosuus käsittelee teknologiastrategiaa, tuotetiedon hallintaa, tuotearkkitehtuureja sekä tuotekehitystoiminnan prosesseja ja menestystekijöitä. Tutkimuksen empiriaosuudessa peilataan liiketoimintayksikön nykytilaa ja strategisia tavoitteita esitettyyn teoreettiseen viitekehykseen, sekä suoritetaan analyysi tavoitellun muutoksen edellytyksistä. Työn tulos sisältää sarjan toimenpiteitä ja näistä muodostetun käsitekartan, joka kuvaa muutosprosessia kokonaisuutena: tuoterakenteiden, tuotetiedon hallinnan, prosessien ja organisaatioiden osalta.

The objective of the thesis is to enhance the understanding about the management of the front end phases of the innovation process in a networked environment. The thesis approaches the front end of innovation from three perspectives, including the strategy, processes and systems of innovation. The purpose of the use of different perspectives in the thesis is that of providing an extensive systemic view of the front end, and uncovering the complex nature of innovation management. The context of the research is the networked operating environment of firms. The unit of analysis is the firm itself or its innovation processes, which means that this research approaches the innovation networks from the point of view of a firm. The strategy perspective of the thesis emphasises the importance of purposeful innovation management, the innovation strategy of firms. The role of innovation processes is critical in carrying out innovation strategies in practice, supporting the development of organizational routines for innovation, and driving the strategic renewal of companies. The primary focus of the thesis from systems perspective is on idea management systems, which are defined as a part of innovation management systems, and defined for this thesis as any working combination of methodology and tools (manual or IT-supported) that enhance the management of innovations within their early phases. The main contribution of the thesis are the managerial frameworks developed for managing the front end of innovation, which purposefully “wire” the front end of innovation into the strategy and business processes of a firm. The thesis contributes to modern innovation management by connecting the internal and external collaboration networks as foundational elements for successful management of the early phases of innovation processes in a dynamic environment. The innovation capability of a firm is largely defined by its ability to rely on and make use of internal and external collaboration already during the front end activities, which by definition include opportunity identification and analysis, idea generation, profileration and selection, and concept definition. More specifically, coordination of the interfaces between these activities, and between the internal and external innovation environments of a firm is emphasised. The role of information systems, in particular idea management systems, is to support and delineate the innovation-oriented behaviour and interaction of individuals and organizations during front end activities. The findings and frameworks developed in the thesis can be used by companies for purposeful promotion of their front end processes. The thesis provides a systemic strategy framework for managing the front end of innovation – not as a separate process, but as an elemental bundle ofactivities that is closely linked to the overall innovation process and strategy of a firm in a distributed environment. The theoretical contribution of the thesis relies on the advancement of the open innovation paradigm in the strategic context of a firm within its internal and external innovation environments. This thesis applies the constructive research approach and case study methodology to provide theoretically significant results, which are also practically beneficial.

Sales configurators are essential tools for companies that offer complicated case specifically crafted products for customers. Most sophisticated of them are able to design an entire end product on the fly according to given constraints, calculate price for the offer and move the order into production. This thesis covers a sales configurator acquisition project in a large industrial company that offers cranes for its customers. The study spans the preliminary stages of a large-scale software purchase project starting from the specification of problem domain and ending up presenting the most viable software solution that fulfils the requirements for the new system. The project consists of mapping usage environment, use cases, and collecting requirements that are expected from the new system. The collected requirements involve fitting the new sales system into enterprise application infrastructure, mitigating the risks involved in the project and specifying new features to the application whilst preserving all of the admired features of the old sales system currently used in the company. The collected requirements were presented to a number of different sales software vendors who were asked to provide solution suggestions that would fulfil all the demands. All of the received solution proposals were exposed to an evaluation to determine the most feasible solutions, and the construction of evaluation criteria itself was a part of the study. The final outcome of this study is a short-list of the most feasible sales configurator solutions together with a description of how software purchase process in large enterprises work, and which aspects should be paid attention in large projects of similar kind.

The purpose of this Thesis was to study what is the present situation of Business Intelligence of the company unit. This means how efficiently unit uses possibilities of modern information management systems. The aim was to resolve how operative informa-tion management of unit’s tender process could be improved by modern information technology applications. This makes it possible that tender processes could be faster and more efficiency. At the beginning it was essential to acquaint oneself with written literature of Business Intelligence. Based on Business Intelligence theory is was relatively easy but challenging to search and discern how tender business could be improved by methods of Busi-ness Intelligence. The empirical phase of this study was executed as qualitative research method. This phase includes theme and natural interviews on the company. Problems and challenges of tender process were clarified in a part an empirical phase. Group of challenges were founded when studying information management of company unit. Based on theory and interviews, group of improvements were listed which company could possible do in the future when developing its operative processes.

The aim of this Thesis is to study how to manage the front-end of the offering planning process. This includes actual process development and methods to gather and analyze information to achieve the best outcome in customer oriented product offering. Study is carried out in two parts: theoretical part and company related part. Theoretical framework is created introducing different types of approaches to manage product planning processes. Products are seen as platforms and they are broken down to subsystems to show different parts of the development. With the help of the matrix-based approaches product platform related information is gathered and analyzed. In this kind of analysis business/market drivers and cus-tomer/competitor information are connected with product subsystems. This gives possibilities to study product gaps/needs and possible future ideas/scenarios in different customer segments. Company related part consists of offering planning process development in real company environment. Process formation includes documents and tools that guide planning from the information gathering to the prioritization and decision making.

Keeping track of software assets and managing software installations in IT environments can be a hard endeavor, especially when the size and diversity of the environment grows. How to install and uninstall software efficiently and cost effectively? Are there too few or too many software licenses purchased? If installed, is the software actually in use? Software Asset Management (SAM) is a process that involves managing and optimizing the purchase, deployment, maintenance, utilization, and disposal of software applications within an organization. This master’s thesis describes a special Software Lifecycle Management Framework to provide solutions to the multitude of challenges within SAM. The main objectives when designing the framework was to provide a set of tools to control the software assets during their entire lifecycle while trying to minimize the costs related to owning and managing them.

The goal of the thesis is to make a supplier evaluation using analytical hierarchy process. Before the supplier evaluation is performed there will be introduced the principles of purchasing which gives a viewpoint to the supplier evaluation and management. The thesis will also give an overview on quality, performance and forecasts which are very important to the supplier evaluation and future improvements. The chapter which describes analytical hierarchy process will show the reader what exactly is analytical hierarchy process and how can it be utilized in supplier evaluation. In the later stages, thesis will provide information about the case company EADS Secure Networks Oy, the processes applied there towards purchasing and how the analytical hierarchy process is applied in practise. In the end of the thesis there will be an overview about each supplier’s strong and weak points as well as some comments and ideas about developing also EADS Secure Networks procedures to a direction which would benefit the whole customer–supplier–chain.

This study sought to assess the impact of health care professional (HCP) communication on breast cancer patients across the acute care process as perceived by patients. Methodological approach was based on eight focus groups conducted with a sample of patients (n ¼ 37) drawn from 15 Spanish Regions; thematic analysis was undertaken using the National Cancer Institute (NCI) framework of HCP communication as the theoretical basis. Relevant results of this study were the identification of four main communication components: (1) reassurance in coping with uncertainty after symptom detection and prompt access until confirmed diagnosis; (2) fostering involvement before delivering treatments, by anticipating information on practical and emotional illness-related issues; (3) guidance on the different therapeutic options, through use of clinical scenarios; and, (4) eliciting the feeling of emotional exhaustion after ending treatments and addressing the management of potential treatment-related effects. These communication-related components highlighted the need for a comprehensive approach in this area of cancer care

The bachelor’s thesis concentrates on the innovativeness in the construction industry. The purpose of the thesis is to define the innovation as a concept reflected on a context of the construction industry. The second objective is to examine how the construction companies could foster and increase the innovativeness. The third objective was to find out tools, methods and phases of the front-end of the innovation process. The construction industry is often considered as a traditional and an old-fashioned manufacturing industry. The innovation or the innovativeness rarely linked to the construction industry. Productivity is a common problem in the construction industry. The construction industry needs to increase the productivity to compete in a globalized world. The productivity can be increased by the innovation. The thesis based on a literature review. The findings from the literature include a description of the innovation as a concept, the innovative culture and the innovation process as a context of the construction industry. The phases of the front-end of the innovation process were explained. Customers centered approach was taken into account in the innovation process. The required tools and methods for managing the front-end of the innovation process were illustrated. The thesis ensures the importance of the innovation facing challenges of the construction industry. Managing the front-end of the innovation is the most important aspect to stand out from the less innovative companies. To take a full advantage of the innovation companies cannot fear of changes. The innovation process requires a full support of the top management of the company. Taking into consideration a theoretical aspect of the thesis a further research is required to respond practical needs of the company. Tools and methods should be considered according the company’s needs and activities. Company’s existing state and culture should be examined before implementing the front-end of the innovation process to ensure the functionality.

Fast development in the operating environment and fierce competition have driven companies to pursue efficiency and success through lean and global supply chains. At the same time overall uncertainty has increased in the business environment and supply chains have become a priority in risk management since their vulnerability may endanger business continuity. Although risk management should start at procurement strategy development phase, proactive contingency planning is also essential because it enables correct reaction and fast changes in process execution in the case of risk realization. This thesis is a case study conducted in the pharmaceutical industry where purchasing and materials management organizations face a number of challenges and limitations that have to be considered in supply risk management. The goal of the study was to discuss the operating environment, and identify and analyze supply risks and potential risk management practices. The study was concluded with suggestions for purchasing strategy development that take risk management considerations into account. This copy is the public version of the thesis.
