Objective. Clinical interest in C-reactive protein (CRP) - a component of the innate immune system - has focused mainly on its worth as an indicator of disease activity. There has been a resurgence of interest in CRP in the Crohn's disease ( CD) literature because several trials of new treatments for active CD have been characterized by both a large proportion of patients with low CRP ( < 10 mg/l) at entry to the trials and by a negative therapeutic outcome. It is therefore of interest to study the clinical characteristics of patients who are thought to have at the same time both active CD and a low CRP. Material and methods. Patients were prospectively recruited as part of the Brisbane IBD clinical and research programme. Subjects were included in the low CRP group only if there were complete datasets for CRP on all occasions of active CD, and CRP was < 10 mg/l. Active disease was defined as CD activity index (CDAI) > 200. The low CRP group was compared with patients in the raised CRP group for a range of clinical variables as well as the major NOD2 variants. Results. There were data sets for 223 CD patients, with a mean disease duration of 12 years. Of these, 22 patients fulfilled the criteria for low CRP. The low CRP group ( group 1) showed significant differences for disease site (p < 0.01) and for BMI (p = 0.006) compared to the raised CRP group ( group 2). Specifically, group 1 had a predominance of pure ileal disease (95% versus 53%) and lack of pure colonic disease (0% versus 24%) compared to group 2, and their BMI was significantly lower (20.3 kg/m(2) versus 25.0 kg/m(2)). Groups 1 and 2 did not differ with respect to Vienna behaviour at diagnosis, smoking, appendicectomy, extra-intestinal manifestations of CD, or NOD2 SNP variants. There was a trend for low CRP patients with previous ileal resection to evolve to a stricturing phenotype. Fat wrapping was noted in 11/13 (85%) of low CRP patients undergoing ileal resections. Conclusions. Patients with CD and a persistently low CRP in the face of active disease were characterized by an almost exclusive ileal disease distribution and a low BMI, compared to those with a raised CRP. These patients had a similar frequency and distribution of NOD2/CARD15 variants. Stricturing ( v inflammatory or penetrating) behaviour may explain some low CRP. Despite the abnormally low BMI, fat wrapping was noted in the majority of low CRP patients undergoing ileal resection.

Coral bleaching (the loss of symbiotic dinoflagellates from reef-building corals) is most frequently caused by high-light and temperature conditions. We exposed the explants of the hermatypic coral Stylophora pistillata to four combinations of light and temperature in late spring and also in late summer. During mid-summer, two NOAA bleaching warnings were issued for Heron Island reef (Southern Great Barrier Reef, Australia) when sea temperature exceeded the NOAA bleaching threshold, and a 'mild' (in terms of the whole coral community) bleaching event occurred, resulting in widespread S. pistillata bleaching and mortality. Symbiotic dinoflagellate biomass decreased by more than half from late spring to late summer (from 2.5x10(6) to 0.8x10(6) dinoflagellates cm(2) coral tissue), and those dinoflagellates that remained after summer became photoinhibited more readily (dark-adapted F (V) : F (M) decreased to (0.3 compared with 0.4 in spring), and died in greater numbers (up to 17% dinoflagellate mortality compared with 5% in the spring) when exposed to artificially elevated light and temperature. Adding exogenous antioxidants (D-mannitol and L-ascorbic acid) to the water surrounding the coral had no clear effect on either photoinhibition or symbiont mortality. These data show that light and temperature stress cause mortality of the dinoflagellate symbionts within the coral, and that susceptibility to light and temperature stress is strongly related to coral condition. Photoinhibitory mechanisms are clearly involved, and will increase through a positive feedback mechanism: symbiont loss promotes further symbiont loss as the light microenvironment becomes progressively harsher.

Adiponectin is a secreted, multimeric protein with insulin-sensitizing, antiatherogenic, and antiinflammatory properties. Serum adiponectin consists of trimer, hexamer, and larger high-molecular-weight (HMW) multimers, and these HMW multimers appear to be the more bioactive forms. Multimer composition of adiponectin appears to be regulated; however, the molecular mechanisms involved are unknown. We hypothesize that regulation of adiponectin multimerization and secretion occurs via changes in posttranslational modifications (PTMs). Although a structural role for intertrimer disulfide bonds in the formation of hexamers and HMW multimers is established, the role of other PTMs is unknown. PTMs identified in murine and bovine adiponectin include hydroxylation of multiple conserved proline and lysine residues and glycosylation of hydroxylysines. By mass spectrometry, we confirmed the presence of these PTMs in human adiponectin and identified three additional hydroxylations on Pro71, Pro76, and Pro95. We also investigated the role of the five modified lysines in multimer formation and secretion of recombinant human adiponectin expressed in mammalian cell lines. Mutation of modified lysines in the collagenous domain prevented formation of HMW multimers, whereas a pharmacological inhibitor of prolyl- and lysyl-hydroxylases, 2,2'-dipyridyl, inhibited formation of hexamers and HMW multimers. Bacterially expressed human adiponectin displayed a complete lack of differentially modified isoforms and failed to form bona fide trimers and larger multimers. Finally, glucose-induced increases in HMW multimer production from human adipose explants correlated with changes in the two-dimensional electrophoresis profile of adiponectin isoforms. Collectively, these data suggest that adiponectin multimer composition is affected by changes in PTM in response to physiological factors.

The degree to which palaeoclimatic changes in the Southern Hemisphere co-varied with events in the high latitude Northern Hemisphere during the Last Termination is a contentious issue, with conflicting evidence for the degree of 'teleconnection' between different regions of the Southern Hemisphere. The available hypotheses are difficult to test robustly, however, because there are few detailed palaeoclimatic records in the Southern Hemisphere. Here we present climatic reconstructions from the southwestern Pacific, a key region in the Southern Hemisphere because of the potentially important role it plays in global climate change. The reconstructions for the period 20-10 kyr BP were obtained from five sites along a transect from southern New Zealand, through Australia to Indonesia, supported by 125 calibrated C-14 ages. Two periods of significant climatic change can be identified across the region at around 17 and 14.2 cal kyr BP, most probably associated with the onset of warming in the West Pacific Warm Pool and the collapse of Antarctic ice during Meltwater Pulse-1A, respectively. The severe geochronological constraints that inherently afflict age models based on radiocarbon dating and the lack of quantified climatic parameters make more detailed interpretations problematic, however. There is an urgent need to address the geochronological limitations, and to develop more precise and quantified estimates of the pronounced climate variations that clearly affected this region during the Last Termination. Background. Postoperative cognitive dysfunction (POCD) after coronary artery bypass graft surgery is a common complication for which, despite many clinical investigations, no definitive etiology has been found. The current use of both high and low-dose fentanyl as anesthetic techniques allowed us to investigate the effect of fentanyl on the incidence of POCD. Methods. Three hundred fifty patients scheduled to undergo elective coronary artery bypass graft surgery were randomized to receive either high-dose fentanyl (50 mu g/kg) or low-dose fentanyl (10 mu g/kg) as the basis of the anesthetic. All patients underwent neuropsychological testing before surgery and at 1 week, 3 months, and 12 months after surgery. Results. One hundred sixty-eight patients in the low-dose group and 158 patients in the high-dose group were included in the final analysis. Neuropsychological testing was performed on 88%, 93%, and 92% of patients at 1 week, 3 months, and 12 months, respectively. There was no difference between group mean scores at any of the three testing times. Analysis of individual patients by the 20% rule did not detect any differences between groups. The one SD rule, which has fewer false-positive results, detected significantly more patients with POCD in the low-dose group than in the high-dose group at 1 week (23.6% vs. 13.7%; P = 0.03) but not at the other testing times. Patients with POCD spent an average of 1.2 days longer in the hospital than those without POCD (P = 0.021). Conclusions: High-dose fentanyl is not associated with a difference in the incidence of POCD at 3 or 12 months after surgery. Low-dose fentanyl leads to shorter postoperative ventilation times and may be associated with a greater incidence of POCD 1 week after surgery. Early POCD is associated with an increased duration of stay in the hospital.

Os produtos de luxo são capazes de transmitir significados sociais e individuais, bem como sua carga simbólica e emocional. Por meio desses produtos, é possível conhecer um pouco dos valores, crenças e atitudes das pessoas que os consomem. O número de pessoas com deficiência no Brasil chega a 45 milhões da população brasileira, e o mercado brasileiro voltado para pessoas com deficiência ainda é pouco explorado pelas empresas. Esse trabalho objetiva analisar os efeitos dos fatores antecedentes da atitude face aos bens de luxo entre pessoas portadoras de deficiência física e pessoas sem deficiência. A pesquisa fundamenta-se em uma abordagem de investigação quantitativa, onde os dados foram coletados por meio de um instrumento de pesquisa com a obtenção de 239 questionários respondidos, possibilitando a análise dos relacionamentos entre os construtos Status, Influência Social, Autoestima, Não funcionalidade sobre a Atitude Face aos Bens de Luxo. Para realizar os testes das hipóteses do estudo, foi realizada a análise de equações estruturais, com base no PLS-PM (Partial Least Squares Path Modeling), onde foram apresentadas medidas satisfatórias para os construtos investigados e o modelo proposto, sendo significantes algumas relações entre os construtos. O estudo concluiu que a percepção de status influencia positivamente a Atitude face ao luxo, pois os indivíduos buscam aumentar seu status através do consumo de bens de luxo. Concluiu-se também que a influência social e autoestima impactam positivamente a Atitude face aos bens de luxo. Não foram encontradas influências significantes da não funcionalidade com a atitude face ao luxo e, além disso, não foi confirmado que a autoestima influencia a percepção de status e de influência social. Os resultados evidenciam também que pessoas sem deficiência possuem maior percepção de Status se comparados com as pessoas com deficiência física. O grupo de pessoas com deficiência física tem maior percepção de Funcionalidade atribuída aos produtos e maior Autoestima se comparado com o grupo de pessoas sem deficiência. De acordo com os resultados, o impacto da Autoestima sobre a Atitude Face ao Luxo é moderado pelo porte de deficiência.

Desde tempos remotos é notória a busca da humanidade para entender e conquistar a felicidade, qualidade de vida, bem-estar e saúde na sua plenitude bio-psico-social. Assim, o objetivo geral deste estudo foi analisar as relações entre percepções de suporte (social, social no trabalho e organizacional) e bem-estar no trabalho (satisfação no trabalho, envolvimento com o trabalho e comprometimento organizacional afetivo) em trabalhadores com deficiência, pois são poucas as pesquisas sobre pessoas com deficiência (PCD). O propósito em abordar o trabalho é por ser um importante elemento de integração social e por constituir um símbolo de reconhecimento social, valorizando a capacidade de estreitar contatos e de estabelecer relações sociais. Deste estudo, participaram 44 trabalhadores com algum tipo de deficiência que atuam em cargos operacionais, técnicos e administrativos. Todos foram escolhidos por conveniência, sendo 24 (54,5%) do sexo masculino e 20 (45,5%) do sexo feminino, com idade entre 18 e 65 anos. Foi possível classificar as deficiências dos participantes em quatro categorias: deficiência nos membros superiores: 9 (20,5%) trabalhadores; deficiência nos membros inferiores: 11 (25%) trabalhadores; deficiência auditiva: 21 (47,7%) trabalhadores; deficiência visual: 3 (6,8%) trabalhadores. Para a coleta de dados foi utilizado questionário de auto-preenchimento, composto de seis escalas que avaliam satisfação no trabalho, envolvimento com o trabalho e comprometimento organizacional, além de suporte social, suportes social no trabalho e organizacional. Foram realizadas análises estatísticas descritivas, testadas diferenças entre médias, bem como calculados coeficientes de correlação entre variáveis. Os resultados apontam que em termos de satisfação no trabalho, não revelam discrepâncias entre estudos realizados com trabalhadores sem deficiências (considerados normais ). Também foi possível observar que as PCD declaram ter orgulho da empresa em que trabalham, além de estarem contentes, entusiasmadas, interessadas e animadas com a organização empregadora. O estudo revelou que as PCD obtêm de sua rede social, ajuda emocional que lhes proporciona sentimento de apoio frente às dificuldades ou carências afetivas, pois provavelmente entendam que podem contar com essa rede para comemorar realizações e sucessos, da mesma forma que receber carinho e consolo quando se frustram ou passam por algum momento triste. É possível afirmar que as PCD percebem que essa mesma rede seria capaz de lhes prover algum apoio prático, como receber informações acerca de sua saúde, talvez reabilitação, também informações para atualização profissional ou até acompanhamento do seu desenvolvimento, inclusive busca de novas oportunidades e desafios para crescimento pessoal e profissional. Os resultados desta pesquisa indicam que as PCD tendem a manter uma forte convicção de que a empresa em que trabalham preocupa-se com seu bem-estar e está disposta a oferecer ajuda diante uma necessidade. Demais resultados sinalizam que as PCD tendem a aumentar o seu vínculo com o trabalho vivenciando mais satisfação na medida em que também aumentam os suportes ofertados pela organização, pela rede social no contexto do trabalho e fora dele. A análise de todo o conteúdo confeccionado é a grande contribuição deste estudo, por ser considerado pioneiro nesta discussão, mas futuros estudos podem vir a confirmar tais resultados e corroborar com mais informações.(AU)

In Alzheimer's disease (AD), neurofibrillary tangles (NFT) occur within neurons in both the upper and lower cortical laminae. Using a statistical method that estimates the size and spacing of NFT clusters along the cortex parallel to the pia mater, two hypotheses were tested: 1) that the cluster size and distribution of the NFT in gyri of the temporal lobe reflect degeneration of the feedforward (FF) and feedback (FB) cortico-cortical pathways, and 2) that there is a spatial relationship between the clusters of NFT in the upper and lower laminae. In 16 temporal lobe gyri from 10 cases of sporadic AD, NFT were present in both the upper and lower laminae in 11/16 (69%) gyri and in either the upper or lower laminae in 5/16 (31%) gyri. Clustering of the NFT was observed in all gyri. A significant peak-to-peak distance was observed in the upper laminae in 13/15 (87%) gyri and in the lower laminae in 8/ 12 (67%) gyri, suggesting a regularly repeating pattern of NFT clusters along the cortex. The regularly distributed clusters of NFT were between 500 and 800 μm in size, the estimated size of the cells of origin of the FF and FB cortico-cortical projections, in the upper laminae of 6/13 (46%) gyri and in the lower laminae of 2/8 (25%) gyri. Clusters of NFT in the upper laminae were spatially correlated (in phase) with those in the lower laminae in 5/16 (31%) gyri. The clustering patterns of the NFT are consistent with their formation in relation to the FF and FB cortico-cortical pathways. In most gyri, NFT clusters appeared to develop independently in the upper and lower laminae.
